Abstract

Nucleotide excision repair is a constitutive stress response that eliminates DNA lesions induced by multiple genotoxic agents. Unlike the immune system, which generates billions of immunoglobulins and T cell receptors for antigen recognition, the nucleotide excision repair complex uses only a few generic factors to detect an astounding diversity of DNA modifications. New data favor an unexpected strategy whereby damage recognition is initiated by the detection of abnormal oscillations in the undamaged strand opposite to DNA lesions. Another core subunit recognizes the increased susceptibility of DNA to be kinked at injured sites. We suggest that early nucleotide excision repair factors gain substrate versatility by avoiding direct contacts with modified residues and exploiting instead the altered dynamics of damaged DNA duplexes.

Abstract

The monitoring of losses is crucial in any accelerator. In the new high intensity hadron facilities even low energy beam can damage or activate the materials so the detection of small losses in this region is very important. A new type of neutron beam loss monitor has been developed specifically targeting this region, where only neutrons and photons can be produced and where typical BLM, based on charged particle detection, could not be appropriate because of the photon background due to the RF cavities. The BLM proposed is based on gaseous Micromegas detectors, designed to be sensitive to fast neutrons and with little sensitivity to photons. Development of the detectors presented here has been done to fulfil the requirements of ESS and they will be part of the ESS-BI systems. The detector has been presented in previous editions of the conference. Here we focus on the neutron/gamma rejection with the final FEE and in the first operation of one of the modules in a beam during the commissioning of LINAC4 (CERN) with the detection of provoked losses and their clear separation from RF gammas. Abstract

Beam loss monitors are of high importance in high-intensity hadron facilities where any energy loss can produce damage or/and activation of materials. A new type of neutron BLM have been developed for hadron accelerators aiming to cover the low energy part. In this region typical BLMs based on charged particle detection are not appropriate because the expected particle fields will be dominated by neutrons and photons. Moreover, the photon background due to the RF cavities can produce false beam loss signals. The BLM proposed is based on gaseous Micromegas detectors, designed to be sensitive to fast neutrons and insensitive to photons (X and gamma). In addition, the detectors will be insensitive to thermal neutrons, since part of them will not be directly correlated to beam loss location. The appropriate configuration of the Micromegas operating conditions will allow excellent timing, intrinsic photon background suppression and individual neutron counting, extending thus the dynamic range to very low particle fluxes. The concept of the detectors and the first results from tests in several facilities will be presented. Abstract

A new type of Beam Loss Monitor (BLM) system is being developed for use in the European Spallation Source (ESS) linac, primarily aiming to cover the low energy part (proton energies between 3-100 MeV). In this region of the linac, typical BLM detectors based on charged particle detection (i.e. Ionization Cham-bers) are not appropriate because the expected particle fields will be dominated by neutrons and photons. Another issue is the photon background due to the RF cavities, which is mainly due to field emission from the electrons from the cavity walls, resulting in brems-strahlung photons. The idea for the ESS neutron sensi-tive BLM system (ESS nBLM) is to use Micromegas detectors specially designed to be sensitive to fast neutrons and insensitive to low energy photons (X and gammas). In addition, the detectors must be insensitive to thermal neutrons, because those neutrons may not be directly correlated to beam losses. 34. Trends in initiation of direct oral anticoagulant therapies for atrial fibrillation in a national populationbased cross-sectional study in the French health insurance databases.

Abstract

Objective Unlike several other national health agencies, French health authorities recommended that the newer direct oral anticoagulant (DOAC) agents only be prescribed as second choice for the treatment of newly diagnosed nonvalvular atrial fibrillation (NVAF), with vitamin K antagonists (VKA) remaining the first choice. We investigated the patterns of use of DOACs versus VKA in the treatment of NVAF in France over the first 5 years of DOAC availability. We also identified the changes in patient characteristics of those who initiated DOAC treatment over this time period. Methods Based on the French National Health Administrative Database, we constituted a populationbased cohort of all patients who were newly treated for NVAF between January 2011 and December 2015. Trends in drug use were described as the percentage of patients initiating each drug at the time of treatment initiation. A multivariate analysis using logistic regression model was performed to identify independent sociodemographic and clinical predictors of initial anticoagulant choice. Results The cohort comprised 814 446 patients who had received a new anticoagulant treatment for NVAF. The proportion of patients using DOACs as initial anticoagulant therapy reached 54% 3 months after the Health Ministry approved the reimbursement of dabigatran for NVAF, and 61% by the end of 2015, versus VKA use. In the multivariate analysis, we found that DOAC initiators were younger and healthier overall than VKA initiators, and this tendency was reinforced over the 2011-2014 period. DOACs were more frequently prescribed by cardiologists in 2012 and after (adjusted OR in 2012: 2.47; 95% CI 2.40 to 2.54). Conclusion Despite recommendations from health authorities, DOACs have been rapidly and massively adopted as initial therapy for NVAF in France. Observational studies should account for the fact that patients selected to initiate DOAC treatment are healthier overall, as failure to do so may bias the risk-benefit assessment of DOACs. 35. La toxicovigilance à travers la base de données des centres antipoison et celle du dispositif de toxicovigilance océan Indien

Abstract

Le dispositif de toxicovigilance océan Indien (DTV-OI) a été établi en 2006 pour surveiller les effets des insecticides et répulsifs pendant l’épidémie de Chikungunya à La Réunion. Depuis la loi de modernisation de santé publique en 2016, le DTV-OI, sous la responsabilité de l’Anses, assure la veille et l’alerte sanitaire pour La Réunion et Mayotte dans le cadre de la toxicovigilance. Le DTV-OI collecte des données des intoxications via le programme de médicalisation des systèmes d’information. Le centre antipoison de Marseille (CAPM) assure la réponse téléphonique à l’urgence (RTU) pour la zone La Réunion et Mayotte et collecte enregistre les cas dans le système national des centres antipoison (SICAP). Certains cas peuvent être communs aux deux bases de données. Le DTV-OI n’assure pas de mission de RTU. Cette étude compare les bases de données de toxicovigilance du logiciel commun des centres antipoison, le SICAP et du DTV-OI pour les cas d’intoxication survenus à La Réunion entre le 1erjanvier 2020 et le 31 décembre 2022. Les cas symptomatiques inclus provenaient des appels pris en charge par le CAPM et des données hospitalières collectées par le DTV-OI, en excluant les cas de pharmacovigilance. Entre 2020 et 2022, le DTV-OI et le SICAP ont respectivement enregistré 785 et 844 cas symptomatiques, dont 141 cas communs aux deux bases. Les données montrent une distribution constante des cas communs entre les deux bases tout au long de l’année. La répartition mensuelle des cas montrait une fréquence plus élevée en janvier, février et mars. Une prédominance masculine est observée, avec une majorité des cas dans la tranche d’âge des 25–64 ans. Les systèmes de collecte d’information du CAP et du DTV-OI sont complémentaires. Le CAP fournit des avis médicaux et recommandations en temps réel, tandis que le DTV-OI collecte des données hospitalières de toxicovigilance détaillées a posteriori. Une intégration des données du DTV-OI dans le SICAP améliorerait la qualité et l’exhaustivité des données de toxicovigilance, renforçant ainsi les actions d’alerte et de prévention.

Abstract

Entry into S phase is carefully regulated and, in most organisms, under the control of a G1-S checkpoint. We have previously described a G1-S checkpoint in fission yeast that delays formation of the prereplicative complex at chromosomal replication origins after exposure to UV light (UVC). This checkpoint absolutely depends on the Gcn2 kinase. Here, we explore the signal for activation of the Gcn2-dependent G1-S checkpoint in fission yeast. If some form of DNA damage can activate the checkpoint, deficient DNA repair should affect the length of the checkpoint-induced delay. We find that the cell-cycle delay differs in repair-deficient mutants from that in wild-type cells. However, the duration of the delay depends not only on the repair capacity of the cells, but also on the nature of the repair deficiency. First, the delay is abolished in cells that are deficient in the early steps of repair. Second, the delay is prolonged in repair mutants that fail to complete repair after the incision stage. We conclude that the G1-S delay depends on damage to the DNA and that the activating signal derives not from the initial DNA damage, but from a repair intermediate(s). Surprisingly, we find that activation of Gcn2 does not depend on the processing of DNA damage and that activated Gcn2 alone is not sufficient to delay entry into S phase in UVC-irradiated cells.
