Patients with head and neck cancer are predominantnly at the risk for malnutrition. Monitoring of nutritional status and pre-treatment variables can favorably observe many prognostic indicators for overall survival in head and neck cancer before and during oncological treatment up to three years in head and neck cancer pacients.In 726 patients diagnosed head and neck cancer patients with curative intent, were collected data according to the monitoring scheme of observation. As a clinical important of uninteded weight loss was defined weight loss 10% in input (T0). Differences in groups were analysed by Coxs regression with adjustments for important demografic and tumor-related data.The acceptance of the percutaneous endoscopic gastrostomy was a key factor for less complications and to improve tolerance of anticancer treatment. Men-smokers have the highest risk of cardiovascular mortality. Men-stop-smokers have the most significant life extension of 11 to 22 months; p0.007. Men-non-smokers have a life extension of 15 to 23 months (p0.005) and having its lowest cardiovascular mortality (11%). Women tolerate cancer better, have minor subjective complaints and a lower number of complications but significantly higher consumption of antidepressants (p0.003). While men have higher analgesic consumption (p0.001). Weight loss10% significantly correlates with the clinical manifestations of malnutrition and is associated with an increase in complications and mortality dependent cancer (p0.008).Consistent assessment of nutritional status with early intervention is considered as an essential part of comprehensive anticancer treatment. An independent risk factor for cancer-dependent mortality is considered weight loss of5% with a BMI21 at time T0 or weight loss10% weight T0 in BMI 21- 29 and continuing weight loss to T12 and also BMI 30. The highest-risk profile has a male sex-smoker, age63, hypopharyngeal carcinoma, stage III- IV, weight loss of10% and non-acceptance of percutaneous endoscopic gastrostomy. Another long-term observation of monitored nutritional status with intervention, clinical status and quality of life are needed.

Summary: Background: Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide. Methods: A multimethods analysis was performed as part of the GlobalSurg 3 study—a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital. Findings: Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. Prior research demonstrated that environmental enrichment creates individual differences in behavior leading to a protective addiction phenotype in rats. Understanding the mechanisms underlying this phenotype will guide selection of targets for much-needed novel pharmacotherapeutics. The current study investigates differences in proteome expression in the nucleus accumbens of enriched and isolated rats and the proteomic response to cocaine self-administration using a liquid chromatography mass spectrometry (LCMS) technique to quantify 1917 proteins. Results of complementary Ingenuity Pathways Analyses (IPA) and gene set enrichment analyses (GSEA), both performed using protein quantitative data, demonstrate that cocaine increases vesicular transporters for dopamine and glutamate as well as increasing proteins in the RhoA pathway. Further, cocaine regulates proteins related to ERK, CREB and AKT signaling. Environmental enrichment altered expression of a large number of proteins implicated in a diverse number of neuronal functions (e.g., energy production, mRNA splicing and ubiquitination), molecular cascades (e.g., protein kinases), psychiatric disorders (e.g., mood disorders), and neurodegenerative diseases (e.g., Huntington's and Alzheimer’s diseases). Upregulation of energy metabolism components in EC rats was verified using RNA sequencing. Most of the biological functions and pathways listed above were also identified in the Cocaine X Enrichment interaction analysis, providing clear evidence that enriched and isolated rats respond quite differently to cocaine exposure. The overall impression of the current results is that enriched saline-administering rats have a unique proteomic complement compared to enriched cocaine-administering rats as well as saline and cocaine-taking isolated rats. These results identify possible mechanisms of the protective phenotype and provide fertile soil for developing novel pharmacotherapeutics.

Background: Fitness trackers could represent an easy-to-use and cheap tool for continuous tracking of physical activity of stroke survivors during the period of their recovery at home. The aim of the study was to examine the accuracy of the Fitbit activity tracker in locomotor activity monitoring of stroke survivors with respect to gait disorders, walking speed, walking aid, and placement of the tracker on body. Methods: Twenty-four ambulatory stroke survivors (15 men and 9 women) with locomotion/gait disorder were involved in the study. Patients underwent two walking tests with the Fitbit Alta HR trackers attached on 5 different places on body. The accuracy of the trackers has been analyzed on 3 groups of patients-those walking without any walking aid, those using a single-point stick and those using a rolling walker. Results: For no-aid patients, the most accurate place was the waist. Patients with a single-point stick revealed the smallest deviations for a tracker attached to a healthy lower limb, and patients with a rolling walker revealed the smallest deviations for a tracker attached on the paretic lower limb. Conclusions: An accuracy comparable with the healthy population can be reached for all of the three groups of patients, while fulfilling the conditions for minimum speed of 2 km/h and optimal placement of the trackers with respect to a walking aid and aspect to impairment.

- Abstract
Purpose: Omalizumab has demonstrated clinical efficacy in severe allergic asthma by reducing exacerbation rates and increasing quality of life. However, data concerning its sustained effect after treatment discontinuation are still needed., Methods: This analysis was an observational pilot study (simple within-subjects design) of 12 patients experiencing severe asthma, treated with omalizumab, for 1 year after treatment discontinuation. We prospectively analyzed clinical measurements (pulmonary functions, inhaled corticosteroid [ICS] doses, Asthma Control Test [ACT] scores, skin prick test [SPT] positivity, fraction of exhaled nitric oxide, and exacerbation rates) and laboratory test results (eosinophils and total immunoglobulin E levels) at the time of discontinuation and 6 and 12 months thereafter. Baseline data (before the treatment period; range, 11-61 months) were collected retrospectively. The treatment effect until discontinuation was calculated. To determine its persistence, repeated measures were compared with baseline levels and analyzed by using a general linear model for repeated measures or the Friedman ANOVA, and χ
2 tests in case of normality assumption violation or frequencies. Post hoc analysis was applied by using a simple or repeated contrasts analysis or Wilcoxon signed rank test with Bonferroni correction., Findings: We proved a significant reduction in ICS doses and SPT reactivity and an increase in ACT score during the retrospective treatment phase. Moreover, persistence of these statistically significant effects was recorded 6 months after treatment discontinuation. - Abstract
Objective: To determine the incidence of infection with ganciclovir-resistant cytomegalovirus (CMV) in adult allogeneic hematopoietic stem cell transplant (HSCT) recipients. Clinical resistance or treatment failure was defined as persistent DNAemia or increasing viral load in peripheral blood after 2 weeks of virostatic treatment. The association between the treatment failure and viral resistance was analysed. The presence of ganciclovir-resistant CMV strains was confirmed by genotypic testing able to detect mutations conferring resistance., Methods: In 2012 and 2014, 40 patients who underwent allogeneic HSCT for hematologic malignancies and were treated for human CMV reactivation/disease were followed up prospectively. In patients with treatment failure, CMV DNA was isolated and analysed by nucleotide sequence analysis of the UL 97 and UL 54 genes conferring resistance to the virostatic agent., Results: The treatment failure occurred in seven patients, but ganciclovir resistance conferring mutations were only detected in two of them (mutations L595F and M460I in the UL 97 gene). Another mutation in the UL 97 gene (N510S) was found in a patient with recurrent CMV replication who needed to be retreated but did not meet the criteria for treatment failure., Conclusion: The low incidence of genetically confirmed ganciclovir-resistant CMV isolates in HSCT recipients with relatively common clinical treatment failure suggests that the mechanism underlying slower viral clearance is often other than mutations conferring ganciclovir resistance to the virus.

Introduction: Systemic lupus erythematosus (SLE) is a chronic autoimmune multisystem disease. The aim of our study was to clarify the frequency of decreased serum immunoglobulin levels in SLE patients. There were evaluated 799 results of serum immunoglobulin levels gained from 157 patients fulfilling revised ACR criteria in the retrospective study., Results: The immunoglobulin levels under the normal range were found in 29/157 (18.5 %) patients. The most frequent was isolated reduction of IgG 12/157 (7.6 %), two persons fulfilled criteria for selective IgA deficiency, and one case possible diagnosis of common variable immunodeficiency (CVID). Additionally we report two cases of SLE patients complicated by severe hypogammaglobulinaemia and infectious complications with necessity of long-term immunoglobulin substitution therapy. The diagnosis of CVID is highly probable in the first case. The second case presents sever drug-induced hypogammaglobulinaemia. This female with lymphoma history and multiorgan impairment due to acute SLE was treated with rituximab after convention therapy failure., Conclusion: Humoral immunodeficiency may occur in SLE patients. The monitoring of serum immunoglobulin levels could be a routine in these patients. The CVID diagnosis is possible in patients suffering from recurrent sinopulmonary infections, especially in combination with absence of lupus activity. Rituximab therapy could cause long-term suppression of B lymphocytes with secondary humoral deficiency requiring immunoglobulin substitution therapy.

- Abstract
Background: Malnutrition in head and neck cancer (HNC) patients decreases survival, quality of life (QOL) and oncological outcomes. The aim of the prospective three-year study was to compare QOL, clinical symptoms and variables (complications, survival and mortality rates in HNC patients)., Patients and Methods: A total of 726 patients aged 55 to 72 years with treatable HNC were included from January 2004 to December 2009; these patients were randomized to either group with PEG and enteral nutrition and nonPEG group with nutritional counselling according to nutritional care. We used EORTC questionnaires QOL C-30 and Head and neck module (HN-35) for measuring of QOL. The following variables due to expectable influence on QOL (demographic data, oncological data, nutritional screening, Clinical symptom score, Karnofsky performance status score, Charlson comorbidity index) were included. Monitoring was done five times in three years., Results: In the first six months, we found decrease of weight and body mass index (BMI). After this critical time point and finish of oncological treatment, a marked difference in the development of patients treated with PEG. Negative factors influencing patients survival, QOL, clinical status were males aged > 63 years, hypopharyngeal cancer (stage III- IV), smoking, weight loss > 10%, BMI < 21 and disallowance of PEG., Conclusions: QOL is an essential factor for cancer patients. Our study showed that nutritional intervention with early enteral nutrition may improve QOL and survival in HNC patients. The PEG group better tolerated oncological treatment, had lower incidence of complications, shorter time to re-entry of permanent increase in weight, lower rate of rehospitalization and its shorter length. We found QOL questionnaires to be very important for better understanding and communication and a key instrument for improving solution of patients difficulties during their therapy in multidisciplinary approach.

- Abstract
Cell death is still a matter of debate and scientific opinions have been challenged and are not uniform due to complexity of this issue. Recent research has brought some new evidence about the very subtle border between programmed cell death and necrosis. The concept of their mutual independence, broadly accepted for decades, is now significantly challenged. Lack of unified terminology led to the establishment of the Nomenclature Committee on Cell Death (NCCD) which provides recommendations for clear definition of distinct cell death programs. It also appeals for consistent application of this nomenclature in scientific literature. In this work, some keystone knowledge addressing three specific programmed cell death types - apoptosis, autophagic cell death, and pyroptosis which is recognized as a controversial cell death scenario on the border between programmed cell death and necrosis, is reviewed. These cell death scenarios are discussed in the context of pathogenesis of infectious diseases.

- Abstract
Intracellular parasitism is a phenomenon present in nature for more than one billion years. Its keystone is the intriguing ability of viruses and some bacteria to survive and multiply inside eukaryotic host cells and to parasitize on their metabolic machinery. According to the classical definition, germs are classified as intracellular parasites only if they are able to survive inside macrophages. However, the ability of germs to survive inside eukaryotic cells is much more common than it was expected earlier. Reaction of macrophages to invading microbes is the key point in the complex immunological resistance of the host. The outcome of the host is substantially linked to macrophage reactivity. For example, if an evading microbe with a replication time of 20 minutes survived inside a host for 24 hours without reaction of innate immunity, there would be more than 2 x 1021 microbes at the end of this period. It would be fatal for the host, indeed. The key activities of macrophages in the sense of protection against intracellular parasites are reviewed. Some mechanisms of microbial defence and some new approaches to clinical diagnosis of the functional status of cells of innate immunity are also discussed.

Fertilization treatment is established therapy of infertility in the cases, where other approaches have failed or were impossible. In eating disorders, majority of fertility problems has been secondary and so fully reversible if appropriately treated. Fertilization treatment should not be used in patients refusing the psychological treatment of the illness. In patients in active phase of the illness fertility treatment at fertility are contra-indicated. Two cases of negative impact of fertility treatment in patients with anorexia nervosa are presented.

The author analyzes comprehensive therapy of aggressive behaviour-preventive, family, behavioural and psychopharmacological treatment (neuroleptics, incl. atypical ones-doses used in children and adolescents, antidepressants, incl. AD of the third generation, anxiolytics, incl. atypical ones, antiepileptics and lithium). Psychopharmaceutical preparations in self-injuring behaviour are reviewed and problems of external stimulation are analyzed.

- Abstract
The etiological aspects of enuresis are diverse. Biological factors such as infection, developmental delay and genitourinary tract malformation must be excluded before the child psychiatrist ever undertakes to treat enuresis. In enuresis patients treated by a psychiatrist, an important role is played by the patients' social setting. These children came more often than controls from broken families, unhappy and quarreling parents who tends to give less attention to their children and encourage early separation. Enuresis is also more frequently associated with psychosocial changes (e.g., hospitalization, birth of a sibling, moving to a new home, etc.). The paper examines developmental stages of the child from the point of view of toilet training, circadian rhythms, and sleep stages from the point of view of enuresis incidence. What has a child psychiatrist to offer? Treatment of enuresis should involve a detailed analysis of the lifestyle, psychotherapy and drug therapy, rehabilitation and re-educational training. The most effective drugs are reviewed; these include tricyclic antidepressives (with a number of side effects) and Adiuretin as the drug of first choice, therapeutically hard-to-control enuresis, enuresis in adolescence, and the Peter Pan syndrome.

A psychiatric disorder well known for more than fifty years in children and adolescents has changed several times its name. Mild child encephalopathy, minimal (mild) cerebral dysfunction, impaired attention, hyperkinetic disorder. The cause is not known, there is, however, evidence of familial disposition. Symptoms of this disorder were fully manifested in patients with generalized resistance to thyroid hormone. The disease is due to a genetic mutation of the beta-thyroid receptor and characterized by a reduced response of the pituitary and peripheral tissue to thyroid hormone. The authors refer to the work of authors, who discovered the association between the hyperkinetic disorder and generalized resistance to thyroid hormone.

- Abstract
A total of 120 children and adolescents with the diagnosis of schizophrenia were hospitalized at the psychiatric clinic in Prague in 1946-1975. Of the 120 patients (58% males and 42% females) 12.5%, i.e. 15 died by 1988. The results are consistent with data in the literature which pertain to pregnancy, family background, relations with people already before the disease developed, social maladaptation, more frequent loss of a parent, depressive symptoms at the onset of the disease, changes on the EEG and neurological examination. The results differ as regards--1. A much greater hereditary load in grade II relatives 45% (19% psychoses, 14% suicides, 12% alcoholism). 2. A greater hereditary load in grade I relatives-parents and siblings (16% psychoses and suicides, 5% alcoholism, 12% personality disorders). 3. The authors detected more psychoses among grade I relatives 14% (11% schizophrenia, 3% manic-depressive psychosis). The hereditary load in the narrower sense of the word in grade I relatives is significantly elevated 16% (14.5% psychoses, 1.5% suicides). Based on the results pertaining to the 15-42-year catamnestic investigation of education, occupation, attempts to work, a certain socialization, attacks and remissions, the authors elaborated the prognosis. The author's prognosis of early onset schizophrenia: 10% complete cure, 17% remission alternating with relapses, 73% chronic patients (20% are capable of a certain socialization; in 53% permanent care by another person is necessary-this applies to chronic patients with a defect-deterioratio and postpsychotic psychopathization.

- Abstract
In the introduction the authors mention the best known authors who investigated the dynamics of interpersonal relations in families with a schizophrenic patient. The illustrative statement of an intelligent loving sister is adopted from article by Brodoff and according to the authors it has transcultural validity. In our country the solution is only modified by socioeconomic dependence of the adolescent and young adult on the nuclear family. Therefore the separation of the healthy sibling takes frequently an inopportune course and should be adjusted by psychological psychiatric intervention within primary prevention. In the conclusion the authors inform on the psychosomatic reaction of mothers after the establishment of the diagnosis of schizophrenia. A total of seven mothers, sick adolescent (four times a son, three times a daughter). In all instances severe prolonged metrorrhagia was involved. According to catamnestic data four times hysterectomy was performed (always without a pathological finding).

Aggressive behaviour of adolescents may be based either on an inborn personality structure, or cultivated by training, i.e. the character of punishment. It is encountered in children with organic disorders of the CNS or psychotic disease. A specific case is aggressive behaviour in communities, being found in particular in communities of boys. The authors describe a typical case of an aggressor and the typical profile of the victim. Psychiatric intervention involves psychotherapeutic treatment and influencing of the social and family which should be focused on possible aggressive behaviour.

- Abstract
In child psychiatry the developmental aspect is emphasized. Recognition of normal sequence of development in so-called "normal children" makes it possible to reveal pathological conditions (either retardation or acceleration of development, distortion or atypical development). Change-motion-development are philosophical categories considered a manifestation or life--of psychobiological maturation and growth. When investigating the moral development three theories were applied: the developmental theory (Piaget)--learning (Kohlberg), the dynamic theory (Sandler). Among factors which influence in a certain way the moral behaviour the following were selected: age, sex, temperament, family environment. The author describes three stages of moral development in children: preventional stage (moral heteronomy) up to the age of 8 years, conventional and pst conventional (moral autonomy) in adolescence.
