Background Sex is a major source of diversity among patients and a sex-informed approach is becoming a new paradigm in precision medicine. We aimed to describe sex diversity in myelodysplastic syndromes in terms of disease genotype, phenotype, and clinical outcome. Moreover, we sought to incorporate sex information into the clinical decision-making process as a fundamental component of patient individuality. Methods In this multicentre, observational cohort study, we retrospectively analysed 13 284 patients aged 18 years or older with a diagnosis of myelodysplastic syndrome according to 2016 WHO criteria included in the EuroMDS network (n=2025), International Working Group for Prognosis in MDS (IWG-PM; n=2387), the Spanish Group of Myelodysplastic Syndromes registry (GESMD; n=7687), or the Dusseldorf MDS registry (n=1185). Recruitment periods for these cohorts were between 1990 and 2016. The correlation between sex and genomic features was analysed in the EuroMDS cohort and validated in the IWG-PM cohort. The effect of sex on clinical outcome, with overall survival as the main endpoint, was analysed in the EuroMDS population and validated in the other three cohorts. Finally, novel prognostic models incorporating sex and genomic information were built and validated, and compared to the widely used revised International Prognostic Scoring System (IPSS-R). This study is registered with ClinicalTrials.gov, NCT04889729. Hematological toxicity represents the most common grade =3 toxicity after chimeric antigen receptor (CAR) T-cell therapy. However, its underlying pathophysiology is incompletely understood and its grading and management remains ill-defined. To inform the forthcoming European Hematology Association/European Society for Blood and Marrow Transplantation (EHA/EBMT) guidelines on the management of immune effector cell-associated hematotoxicity (ICAHT), we undertook a survey of experienced clinicians using an online survey focusing on (1) grading, (2) risk-stratification and diagnostic work-up, (3) short-term, and (4) long-term management of ICAHT. There were 81 survey respondents across 18 countries. A high degree of variability was noted for cytopenia grading in regards to depth, duration, and time from CAR-T infusion. The majority of experts favored pre-CAR-T bone marrow studies, especially in case of a high-risk profile. Most respondents felt that the work-up for patients with severe hematotoxicity should rule-out viral infections (96%), substrate deficiency (80%), or coincident sHLH/MAS (serum ferritin, 92%), and should include bone marrow aspiration (86%) and/or biopsy (61%). Clinicians were divided as to whether the occurrence of coincident immunotoxicity should influence the decision to apply G-CSF, and when to initiate G-CSF support. In case of prolonged thrombocytopenia, most survey participants favored thrombopoietin agonists (86%). Conversely, autologous hematopoietic cell boosts represented the preferred choice for neutropenia (63%), although they were frequently not available and no consensus was reached regarding the optimal trigger point. These findings underline the current heterogeneity of practice patterns regarding ICAHT and invite the development of consensus guidelines, which may harmonize grading, establish standard operating procedures for diagnosis, and set management guidelines.

Chimeric antigen receptor (CAR) T-cell systemic immunotherapy has revolutionized how clinicians treat several refractory and relapsed hematologic malignancies. Due to its peculiar mechanism of action, CAR T-cell-based therapy has enlarged the spectrum of neurological toxicities. CAR T-cell-associated neurotoxicity—initially defined as CAR T-cell-related encephalopathy syndrome (CRES) and currently coined within the acronym ICANS (immune effector cell-associated neurotoxicity syndrome)—is perhaps the most concerning toxicity of CAR T-cell therapy. Importantly, hematologic malignancies (especially lymphoid malignancies) may originate in or spread to the central nervous system (CNS) in the form of parenchymal and/or meningeal disease. Due to the emergence of deadly and neurological adverse events, such as fatal brain edema in some patients included in early CAR T-cell trials, safety concerns for those with CNS primary or secondary infiltration arose and contributed to the routine exclusion of individuals with pre-existing or active CNS involvement from pivotal trials. However, based primarily on the lack of evidence, it remains unknown whether CNS involvement increases the risk and/or severity of CAR T-cell-related neurotoxicity. Given the limited treatment options available for patients once they relapse with CNS involvement, it is of high interest to explore the role of novel clinical strategies including CAR T cells to treat leukemias/lymphomas and myeloma with CNS involvement. The purpose of this review was to summarize currently available neurological safety data of CAR T-cell-based immunotherapy from the clinical trials and real-world experiences in adult patients with CNS disease due to lymphoma, leukemia, or myeloma. Increasing evidence supports that CNS involvement in hematologic disease should no longer be considered per se as an absolute contraindication to CAR T-cell-based therapy. While the incidence may be high, severity does not appear to be impacted significantly by pre-existing CNS status. Close monitoring by trained neurologists is recommended.

CD6 is a lymphocyte-specific scavenger receptor expressed on adaptive (T) and innate (B1a, NK) immune cells, which is involved in both fine-tuning of lymphocyte activation/differentiation and recognition of bacterial-associated molecular patterns (i.e., lipopolysaccharide). However, evidence on CD6's role in the physiological response to bacterial infection was missing. Our results show that induction of monobacterial and polymicrobial sepsis in Cd6 -/- mice results in lower survival rates and increased bacterial loads and pro-inflammatory cytokine levels. Steady state analyses of Cd6 -/- mice show decreased levels of natural polyreactive antibodies, concomitant with decreased cell counts of spleen B1a and marginal zone B cells. Adoptive transfer of wild-type B cells and mouse serum, as well as a polyreactive monoclonal antibody improve Cd6 -/- mouse survival rates post-sepsis. These findings support a nonredundant role for CD6 in the early response against bacterial infection, through homeostatic expansion and functionality of innate-related immune cells.

Introduction The prevalence of catheter-associated bloodstream infections (CLABSI) is high and is a severe health problem associated with an increase in mortality and elevated economic costs. There are discrepancies related to the risk factors of CLABSI since the results published are very heterogeneous and there is no synthesis in the description of all the predisposing factors. Objective We aimed to perform a systematic review and meta-analysis to synthesize and establish the risk factors predisposing to CLABSI reported in the literature. Method This is a systematic review of observational studies following the PRISMA recommendations. MEDLINE and CINAHL databases were searched for primary studies from 2007 to 2021. The protocol was registered in PROSPERO CRD42018083564. Results A total of 654 studies were identified, 23 of which were included in this systematic review. The meta-analysis included 17 studies and 9 risk factors were analyzed (total parenteral nutrition (TPN), chemotherapy, monolumen and bilumen catheters, days of catheterization, immunosuppression, kidney disease and diabetes mellitus) due to the homogeneity of their definitions and measurements. The risk factors found to increase the probability of developing CLABSI were TPN, multilumen devices, chemotherapy treatment, immunosuppression and the number of days of catheterization. On the other hand, monolumen devices presented a lower likelihood of triggering this infection.

Red blood cells play an important role in the rheological properties of the blood and are the main responsible for the shear thinning behaviour of blood. Some hematological diseases can change the geometrical shape of red blood cells and thus their viscosity. In this work we have computed the viscosity of different blood samples that were obtained with a microfluidic device and normalized the viscosities for hematocrit using statistical analysis tools. We have also used different machine learning methods as Logistic Regressions or Artificial Neural Networks (ANN) to predict if a sample of blood corresponds to healthy blood or to a blood with an hematological disease. We have obtained different performance for the different methods, some of them with very good results and an accuracy of 94% of correct prediction has been achieved with an ANN model

We aimed to assess the effects of the antioxidant-rich Mediterranean diet (MedDiet) on white blood cell count. Our study population included participants in the PREvención con DIeta MEDiterránea study (average age 67 years old, 58% women, high cardiovascular risk). We assessed whether a MedDiet intervention enriched in extra-virgin olive oil or nuts, versus a low-fat control diet, modified the incidence of leukocytosis (&gt, 11 × 109 leukocytes/L), mild leukopenia (&lt, 4.5 × 109 leukocytes/L), or severe leukopenia (&lt, 3.5 × 109 leukocytes/L) in individuals without the condition at baseline (n = 3190, n = 2925, and n = 3190, respectively). We also examined whether MedDiet modified the association between leukocyte count alterations and all-cause mortality. Both MedDiet interventions were associated with a lower risk of developing leukopenia (incidence rates: 5.06% in control diet, 3.29% in MedDiet groups combined, hazard ratio [95% confidence interval]: 0.54 [0.36–0.80]) and severe leukopenia (incidence rates: 1.26% in control diet, 0.46% in MedDiet groups combined, hazard ratio: 0.25 [0.10–0.60]). High cumulative adherence to a MedDiet was linked to lower risk of leukocytosis (incidence rates: 2.08% in quartile 1, 0.65% in quartile 4, HRQ4-Q1: 0.29 [0.085–0.99]) and attenuated the association between leukopenia and all-cause mortality (P-interaction = 0.032). In brief, MedDiet decreased the incidence of white blood cell count-related alterations in high cardiovascular risk individuals.

[eng] Clinical pathologists identify visually many morphological features to characterize the different normal cells, as well as the abnormal cell types whose presence in peripheral blood is the evidence of serious diseases. Disadvantages of visual morphological analysis are that it is time consuming, needs expertise to perform an objective review of the smears and is prone to inter-observer variability. Also, most of the morphological descriptions are given in qualitative terms and there is a lack of quantitative measures. The general objective of this thesis is the automatic recognition of normal and dysplastic cells circulating in blood in myelodysplastic syndromes using convolutional neural networks and digital image processing techniques. In order to accomplish this objective, this work starts with the design and development of a Mysql database to store information and images from patients and the development of a first classifier of four groups of cells, using convolutional neural networks as feature extractors. Then, a high- quality dataset of around 17,000 images of normal blood cells is compiled and used for the development of a recognition system of eight groups of blood cells. In this work, we compare two transfer learning approaches to find the best to classify the different cell types. In the second part of the thesis, a new convolutional neural network model for the diagnosis of myelodysplastic syndromes is developed. This model was validated by means of a proof of concept. It is considered among the first models that have been built for diagnosis support. The final work of the thesis is the integration of two convolutional networks in a modular system for the automatic classification of normal and abnormal cells. Algunas de las desventajas del análisis morfológico visual incluyen que toma mucho tiempo, necesita experiencia para realizar una revisión objetiva de los frotis y es propenso a la variabilidad entre observadores. Además, la mayoría de las descripciones morfológicas se proporcionan en términos cualitativos. Debido a lo expuesto anteriormente, es necesario establecer medidas cuantitativas. El objetivo general de esta tesis es el reconocimiento automático de células normales y células displásicas circulantes en sangre en síndromes mielodisplásicos mediante redes neuronales convolucionales y técnicas de procesamiento digital de imágenes. Para lograr este objetivo, este trabajo comenzó con el diseño y desarrollo de una base de datos Mysql para almacenar información e imágenes de pacientes y el desarrollo de un primer clasificador de cuatro grupos de células, utilizando redes neuronales convolucionales como extractores de características. Luego, se compila un conjunto de datos de alta calidad de alrededor de 17.000 imágenes de células sanguíneas normales y se utiliza para el desarrollo de un sistema de reconocimiento de ocho grupos de células sanguíneas. En este trabajo, comparamos dos enfoques de aprendizaje por transferencia para encontrar el mejor para clasificar los diferentes tipos de células. En la segunda parte de la tesis se desarrolla un nuevo modelo de red neuronal convolucional para el diagnóstico de síndromes mielodisplásicos. Este modelo fue validado mediante prueba de concepto. Se considera uno de los primeros modelos que se han construido para apoyar el diagnóstico. El trabajo final de la tesis es la integración de dos redes convolucionales en un sistema modular para la clasificación automática de células normales y anormales. La metodología y los modelos desarrollados constituyen un paso adelante hacia la implementación de un sistema modular para reconocer automáticamente todos los tipos de células en una configuración real en el laboratorio.

Background Clinical outcomes of novel coronavirus 2019 disease (COVID‐19) in onco‐hematological patients are unknown. When compared to non‐immunocompromised patients, onco‐hematological patients seem to have higher mortality rates. Aims We describe the characteristics and outcomes of a consecutive cohort of 24 onco‐hematological patients with COVID‐19 during the first month of the pandemic. We also describe variations in healthcare resource utilization within our hematology department. Methods and Results Data from patients between the first month of the pandemic were retrospectively collected. Clinical and logistic data were also collected and compared with the average values from the prior 3 months of activity. Prevalence of COVID‐19 in our hematological population was 0.4%. Baseline characteristics were as follows: male sex: 83%, lymphoid diseases: 46%, median age: 69 (22‐82) years. Median follow‐up in survivors was 14 (9‐28) days and inpatient mortality rate was 46%. Average time to moderate/severe respiratory insufficiency and death were 3 (1‐10) and 10 (3‐18) days, respectively. Only 1 out of every 12 patients who developed moderate to severe respiratory insufficiency recovered. Upon univariate analysis, the following factors were associated with higher mortality: age ≥ 70 years (P = .01) and D‐dimer ≥900 mcg/L (P = .04). With respect to indirect effects during the COVID‐19 pandemic, and when compared with the prior 3 months of activity, inpatient mortality (excluding patients with COVID‐19 included in the study) increased by 56%. This was associated with a more frequent use of vasoactive drugs (+300%) and advanced respiratory support (+133%) in the hematology ward. In the outpatient setting, there was a reduction in initial visits (−55%) and chemotherapy sessions (−19%). A significant increase in phone visits was reported (+581%). Conclusion COVID‐19 pandemic is associated with elevated mortality in hematological patients. Negative indirect effects are also evident within this setting.

Hematological malignancies are broadly divided into myeloid and lymphoid neoplasms, reflecting the two major cellular lineages of the hematopoietic system. It is generally rare for hematological malignancies to spontaneously progress with a switch from myeloid to lymphoid lineage. We describe the exceptional case of a patient who sequentially developed myelodysplastic syndrome (MDS), chronic myelomonocytic leukemia (CMML), and B-cell acute lymphoblastic leukemia (B-ALL), as well as our investigation into the underlying pathogenesis. Using whole-exome sequencing (WES) performed on sorted CMML and B-ALL cell fractions, we identified both common and unique potential driver mutations, suggesting a branching clonal evolution giving rise to both diseases. Interestingly, we also identified a germline variant in the cancer susceptibility gene CHEK2. We validated that this variant (c.475T > C; p.Y159H), located in the forkhead-associated (FHA) domain, impairs its capacity to bind BRCA1 in cellulo. This unique case provides novel insight into the genetics of complex hematological diseases and highlights the possibility that such patients may carry inherited predispositions.

Severe infections and their attributable mortality are major complications in recipients of allogeneic hematopoietic stem cell transplantation (alloSCT). We herein report 236 adult patients who received haploSCT with PTCy. The median follow-up for survivors was 37 months. The overall incidence of bloodstream infections by gram-positive and gram-negative bacteria at 37 months was 51% and 46%, respectively. The incidence of cytomegalovirus infection was 69%, while Epstein Barr virus infections occurred in 10% of patients and hemorrhagic cystitis in 35% of cases. Invasive fungal infections occurred in 11% at 17 months. The 3-year incidence of infection-related mortality was 19%. The median interval from transplant to IRM was 3 months (range 1-30), 53% of IRM occurred >100 days post-haploSCT. Risk factors for IRM included age >50 years, lymphoid malignancy, and developing grade III-IV acute GvHD. Bacterial infections were the most common causes of IRM (51%), mainly due to gram-negative bacilli BSI. In conclusion, severe infections are the most common causes of NRM after haploSCT with PTCy, with a reemergence of gram-negative bacilli as the most lethal pathogens. More studies focusing on the severe infections after haploSCT with PTCy and differences with other types of alloSCT in adults are clearly warranted.

Background Febrile neutropaenia (FN) is a very common complication in patients with haematological malignancies and is associated with considerable morbidity and mortality. Broad-spectrum antipseudomonal β-lactam antibiotics (BLA) are routinely used for the treatment of cancer patients with FN. However, the clinical efficacy of BLA may be diminished in these patients because they present with pathophysiological variations that compromise the pharmacokinetic (PK) parameters of these antibiotics. Optimised administration of BLA in prolonged infusions has demonstrated better clinical outcomes in critically ill patients. However, there is a paucity of data on the usefulness of this strategy in patients with FN. The aim of this study is to test the hypothesis that the administration of BLA would be clinically more effective by extended infusion (EI) than by intermittent infusion (II) in haematological patients with FN. Methods A randomised, multicentre, open-label, superiority clinical trial will be performed. Patients with haematological malignancies undergoing chemotherapy or haematopoietic stem-cell transplant and who have FN and receive empirical antibiotic therapy with cefepime, piperacillin-tazobactam or meropenem will be randomised (1:1) to receive the antibiotic by EI (during half the time of the dosing interval) in the study group, or by II (30 min) in the control group. The primary endpoint will be clinical efficacy, defined as defervescence without modifying the antibiotic treatment administered within the first 5 days of therapy. The primary endpoint will be analysed in the intention-to-treat population. The secondary endpoints will be pharmacokinetic/pharmacodynamic (PK/PD) target achievement, bacteraemia clearance, decrease in C-reactive protein, overall (30-day) case-fatality rate, adverse events and development of a population PK model of the BLA studied. Discussion Data on the usefulness of BLA administration in patients with FN are scant. Only three clinical studies addressing this issue have been published thus far, with contradictory results. Moreover, these studies had some methodological flaws that limit the interpretation of their findings. If this randomised, multicentre, phase IV, open-label, superiority clinical trial validates the hypothesis that the administration of BLA is clinically more effective by EI than by II in haematological patients with FN, then the daily routine management of these high-risk patients could be changed to improve their outcomes. Background Prognostic factors of poor outcome in patients with hematological malignancies and COVID-19 are poorly defined. Patients and methods This was a Spanish transplant group and cell therapy (GETH) multicenter retrospective observational study, which included a large cohort of blood cancer patients with laboratory-confirmed SARS-CoV-2 infection through PCR assays from March 1st 2020 to May 15th 2020. Results We included 367 pediatric and adult patients with hematological malignancies, including recipients of autologous (ASCT) (n = 58) or allogeneic stem cell transplantation (allo-SCT) (n = 65) from 41 hospitals in Spain. Median age of patients was 64 years (range 1–93.8). Recipients of ASCT and allo-SCT showed lower mortality rates (17% and 18%, respectively) compared to non-SCT patients (31%) (p = 0.02). Prognostic factors identified for day 45 overall mortality (OM) by logistic regression multivariate analysis included age > 70 years [odds ratio (OR) 2.1, 95% confidence interval (CI) 1.2–3.8, p = 0.011]; uncontrolled hematological malignancy (OR 2.9, 95% CI 1.6–5.2, p 20 mg/dL (OR 3.3, 95% CI 1.7–6.4, p

Purpose Polycythemia vera (PV) is a myeloproliferative neoplasm (MPN) associated with disabling symptoms and a heightened risk of life-threatening complications. Recent studies have demonstrated the effectiveness of JAK inhibitor therapy in patients with PV patients who have a history of prior hydroxyurea (HU) use (including resistance or intolerance), phlebotomy requirements, and palpable splenomegaly. We aimed to determine how these features contribute alone and in aggregate to the PV symptom burden. Patients and Methods Through prospective evaluation of 1,334 patients with PV who had characterized symptom burden, we assessed patient demographics, laboratory data, and the presence of splenomegaly by disease feature (ie, known HU use, known phlebotomy requirements, splenomegaly). Results The presence of each feature in itself is associated with a moderately high symptom burden (MPN symptom assessment form [SAF] total symptom score [TSS] range, 27.7 to 29.2) that persists independent of PV risk category. In addition, symptoms incrementally increase in severity with the addition of other features. Patients with PV who had all three features (PV-HUPS) faced the highest total score (MPN-SAF TSS, 32.5) but had similar individual symptom scores to patients with known HU use (PV-HU), known phlebotomy (PV-P), and splenomegaly (PV-S). Conclusion The results of this study suggest that patients with PV who have any one of the features in question (known HU use, known phlebotomy, or splenomegaly) have significant PV-associated symptoms. Furthermore, it demonstrates that many PV symptoms remain severe independent of the number of features present.

Clinical Trial, Phase II; Journal Article; Multicenter Study; BACKGROUND AND OBJECTIVES The elective treatment of patients with post-transplant lymphoproliferative disorders is controversial. The purpose of this trial was to evaluate the efficacy of treatment with extended doses of rituximab adapted to the response in patients with post-transplant lymphoproliferative disorders after solid organ transplantation. DESIGN AND METHODS This was a prospective, multicenter, phase II trial. Patients were treated with reduction of immunosuppression and four weekly infusions of rituximab. Those patients who did not achieve complete remission (CR) received a second course of four rituximab infusions. The primary end-point of the study was the CR rate. RESULTS Thirty-eight patients were assesable. One episode of grade 4 neutropenia was the only severe adverse event observed. After the first course of rituximab, 13 (34.2%) patients achieved CR, 8 patients did not respond, and 17 patients achieved partial remission. Among those 17 patients, 12 could be treated with a second course of rituximab, and 10 (83.3%) achieved CR, yielding an intention-to-treat CR rate of 60.5%. Eight patients excluded from the trial because of absence of CR were treated with rituximab combined with chemotherapy, and six (75%) achieved CR. Event-free survival was 42% and overall survival was 47% at 27.5 months. Fourteen patients died, ten of progression of their post-transplant lymphoproliferative disorder. INTERPRETATION AND CONCLUSIONS These results confirm that extended treatment with rituximab can obtain a high rate of CR in patients with post-transplant lymphoproliferative disorders after solid organ transplantation without increasing toxicity, and should be recommended as initial therapy for these patients. [cat] En les darreres dècades s’han produït molts avenços en el maneig dels pacients amb càncer però en molts casos, el sistema immune es veu severament afectat, i els pacients estan predisposats a patir infeccions. De fet, la infecció és la complicació més important del tractament dels pacients amb càncer i entre les infeccions, la bacterièmia és la infecció més representativa de la infecció bacteriana aguda i és d’especial importància per la seva elevada freqüència i pel seu impacte en el pronòstic del pacient, doncs sovint implica la reducció en les dosis dels tractaments quimioteràpics i allarga l’estància hospitalària. Fins ara, tota la informació sobre bacterièmies en pacients amb tumors sòlids ha estat extrapolada d'estudis en pacients amb neoplàsies hematològiques o estudis que incloïen pacients amb càncer en un sol grup. Hem dut a terme tres estudis prospectius de pacients amb tumors sòlids i neoplàsies hematològiques amb bacterièmia per conèixer les característiques, etiologia, resistència antibiòtica i factors pronòstics de la bacterièmia en el pacient amb càncer. En el primer estudi, “Estudi de la bacterièmia en el pacient amb tumor sòlid: epidemiologia, tractament antibiòtic i evolució de 528 casos en un centre oncològic especialitzat”, vam trobar que la bacterièmia en els tumors sòlids es va donar principalment en els pacients amb tumors hepatobiliars i pulmonars, essent la colangitis el focus d’infecció més freqüent. Els bacils gramnegatius van ser els microorganismes més freqüentment aïllats, principalment Escherichia coli, Pseudomonas. aeruginosa i Klebsiella pneumoniae. En el nostre estudi, la incidència de bacterièmia per microorganismes multiresistents va ser del 13%, essent el mecanisme de resistència més freqüent la producció de beta-lactamases d’espectre estès. Els pacients amb bacterièmia causada per bacils gramnegatius multiresistents van rebre tractament antibiòtic empíric inadequat amb més freqüència, i van presentar un pitjor pronòstic. La mortalitat en els pacients amb tumors sòlids i bacterièmia va ser alta (32%), especialment entre els pacients amb neoplàsies avançades cròniques, aquells que van rebre corticosteroides i els que es van presentar en forma de xoc. En el segon estudi, “Estudi de la bacterièmia en el pacient neutropènic amb càncer: identificació de diferències entre el pacient amb neoplàsia hematològica i el pacient amb tumor sòlid”, vam trobar que la incidència de bacterièmia en pacients amb neutropènia va ser vuit vegades més gran en pacients amb neoplàsies hematològiques. Els focus de bacterièmia van ser diferents en els dos grups; la pneumònia i el tracte urinari van ser els focus d’infecció més rellevants en el grup de tumors sòlids, mentre que l’origen endogen i la bacterièmia de catèter van ser més freqüents en pacients hematològics. La bacterièmia gramnegativa va ser més freqüent en els dos grups de pacients, essent Pseudomonas aeruginosa l’agent causal més freqüent en els pacients amb tumors sòlids. Els bacils gramnegatius multiresistents es van aïllar amb més freqüència en els pacients hematològics, principalment Enterobacteris productors de beta-lactamases d’espectre estès. Aquest grup de pacients va tenir un pitjor pronòstic. La mortalitat precoç i global van ser significativament superiors en els pacients amb tumors sòlids. Finalment en el tercer estudi, “Factors pronòstics de mortalitat en pacients neutropènics amb bacterièmia i malalties hematològiques o tumors sòlids”, vam concloure que la taxa de mortalitat global va ser del 12% en pacients amb neoplàsies hematològiques i del 36% en els pacients amb tumors sòlids. Els factors de risc de mortalitat comuns a ambdós grups de pacients van ser la presència d'una neoplàsia avançada i el tractament amb corticosteroides. En els pacients hematològics, l’ ingrés a la UCI, la bacterièmia causada per bacils gramnegatius multiresistents i la puntuació en l’escala de MASCC inferior a 21 punts, van ser identificats com a factors de risc independents de mortalitat, mentre que la bacterièmia produïda per estafilococs coagulasa negatius i el tractament antibiòtic empíric combinat, van resultar factors protectors de mortalitat. En el grup de pacients amb tumors sòlids, a banda de la neoplàsia avançada i el tractament amb corticoides, el xoc sèptic també es va identificar com a factor associat a mortalitat., [spa] En las últimas décadas se han producido muchos avances en el manejo de los pacientes con cáncer pero en muchos casos, el sistema inmune se ve severamente afectado, y los pacientes están predispuestos a padecer infecciones. De hecho, la infección es la complicación más importante del tratamiento de los pacientes con cáncer y entre las infecciones, la bacteriemia es la infección más representativa de la infección bacteriana aguda y es de especial importancia por su elevada frecuencia y por su impacto en el pronóstico del paciente, pues a menudo implica la reducción en las dosis de los tratamientos quimioterápicos y alarga la estancia hospitalaria. Hasta ahora, toda la información sobre bacteriemias en pacientes con tumores sólidos ha sido extrapolada de estudios en pacientes con neoplasias hematológicas o estudios que incluían pacientes con cáncer como un solo grupo. Hemos llevado a cabo tres estudios prospectivos de pacientes con tumores sólidos y neoplasias hematológicas con bacteriemia para conocer las características, etiología, resistencia antibiótica y factores pronósticos de la bacteriemia en el paciente con cáncer. En el primer estudio, "Estudio de la bacteriemia en el paciente con tumor sólido: epidemiología, tratamiento antibiótico y evolución de 528 casos en un centro oncológico especializado", encontramos que la bacteriemia en los tumores sólidos se dio principalmente en los pacientes con tumores hepatobiliares y pulmonares, siendo la colangitis el foco de infección más frecuente. Los bacilos gramnegativos fueron los microorganismos más frecuentemente aislados, principalmente Escherichia coli, Pseudomonas. aeruginosa y Klebsiella pneumoniae. En nuestro estudio, la incidencia de bacteriemia por microorganismos multirresistentes fue del 13%, siendo el mecanismo de resistencia más frecuente la producción de beta-lactamasas de espectro extendido. Los pacientes con bacteriemia causada por bacilos gramnegativos multirresistentes recibieron tratamiento antibiótico empírico inadecuado con más frecuencia, y presentaron un peor pronóstico. La mortalidad en los pacientes con tumores sólidos y bacteriemia fue alta (32%), especialmente entre los pacientes con neoplasias avanzadas crónicas, aquellos que recibieron corticosteroides y los que se presentaron en forma de shock. En el segundo estudio, "Estudio de la bacteriemia en el paciente neutropénico con cáncer: identificación de diferencias entre el paciente con neoplasia hematológica y el paciente con tumor sólido", encontramos que la incidencia de bacteriemia en pacientes con neutropenia fue ocho veces mayor en pacientes con neoplasias hematológicas. Los focos de bacteriemia fueron diferentes en los dos grupos; la neumonía y el tracto urinario fueron los focos de infección más relevantes en el grupo de tumores sólidos, mientras que el origen endógeno y la bacteriemia de catéter fueron más frecuentes en pacientes hematológicos. La bacteriemia gramnegativa fue más frecuente en los dos grupos de pacientes, siendo Pseudomonas aeruginosa el agente causal más frecuente en los pacientes con tumores sólidos. Los bacilos gramnegativos multirresistentes se aislaron con más frecuencia en los pacientes hematológicos, principalmente Enterobacterias productoras de beta-lactamasas de espectro extendido. Este grupo de pacientes tuvo un peor pronóstico. La mortalidad precoz y global fueron significativamente superiores en los pacientes con tumores sólidos. Finalmente en el tercer estudio, "Factores pronósticos de mortalidad en pacientes neutropénicos con bacteriemia y enfermedades hematológicas o tumores sólidos", concluimos que la tasa de mortalidad global fue del 12% en pacientes con neoplasias hematológicas y del 36% en los pacientes con tumores sólidos. En los pacientes hematológicos, el ingreso en la UCI, la bacteriemia causada por bacilos gramnegativos multirresistentes y la puntuación en la escala de MASCC inferior a 21 puntos, fueron identificados como factores de riesgo independientes de mortalidad, mientras que la bacteriemia producida por estafilococos coagulasa negativos y el tratamiento antibiótico empírico combinado, resultaron factores protectores de mortalidad. En el grupo de pacientes con tumores sólidos, además de la neoplasia avanzada y el tratamiento con corticoides, el shock séptico también se identificó como factor asociado a mortalidad.

This is the first prospective study of deferasirox in adult allogeneic hematopoietic stem cell transplant recipients with transfusional iron overload in hematologic malignancies. Patients at least six months post transplant were treated with deferasirox at a starting dose of 10 mg/kg/day for 52 weeks or until serum ferritin was less than 400 ng/mL on two consecutive occasions. Thirty patients were enrolled and 22 completed the study. A significant reduction from baseline in median serum ferritin and in liver iron concentration at 52 weeks was observed in the overall population: from 1440 to 755.5 ng/mL (P=0.002) and from 14.5 to 4.6 mg Fe/g dw (P=0.0007), respectively. Reduction in serum ferritin in patients who did not discontinue deferasirox therapy was significantly greater than that found in those who prematurely discontinued the treatment (from 1541 to 581 ng/mL vs. from 1416 to 1486 ng/mL; P=0.008). Drug-related adverse events, reported in 17 patients (56.7%), were mostly mild to moderate in severity. There were no drug-related serious adverse events. Twelve patients (40.0%) showed an increase of over 33% in serum creatinine compared to baseline and greater than the upper limit of normal on two consecutive visits. Two patients (6.7%) with active graft-versus-host disease showed an increase in alanine aminotransferase exceeding 10 times upper limit of normal; both resolved. In this prospective study, deferasirox provided a significant reduction in serum ferritin and liver iron concentration over one year of treatment in allogeneic hematopoietic stem cell transplant recipients with iron overload. In addition, the majority of adverse events related to deferasirox were mild or moderate in severity. (clinicaltrials.gov identifier:01335035).

Two randomized controlled clinical trials have evaluated the utility of posaconazole in antifungal prophylaxis in high-risk hematology patients.1,2 Posaconazole use was associated with a significant reduction in fungal infections and death due to invasive fungal disease in patients undergoing induction chemotherapy for acute myelogenous leukemia or myelodysplasia (AML/MDS)1 and those with graft-versus-host disease (GvHD) following allogeneic hematopoietic stem cell transplantation.2 The strength of these trials and subsequent real life studies3,4 led to FDA and EMA approval of posaconazole in 2007 and widespread recommendations for posaconazole use by international guidelines.1,2,5–8 The success of posaconazole prophylaxis in the registration trials appear, however, to be at odds with the finding that serum levels of posaconazole were surprisingly low in both studies (Cavg of 583 ng/mL in AML/MDS and 1103 ng/mL in GVHD).9,10 Furthermore, serum posaconazole levels were paradoxically lowest in the AML/MDS trial in which the most dramatic clinical effect was observed. Since the minimum inhibitory concentration (MIC) of posaconazole for most strains of Aspergillus fumigatus is 500 ng/mL,11 it would appear that in the AML/MDS trial essentially half the patients in this trial should have had insufficient drug exposure to offer protection against infection. This discrepancy between serum drug levels and observed efficacy has raised new questions about our understanding of the pharmacokinetics and pharmacodynamics of posaconazole in relation to its use for primary antifungal prophylaxis.

Background: Patients undergoing conventiona l hemodialysis (C-HD) present a greater immuno-inflam- matory state probably related to uremia, sympathetic nervous system (SNS) activation and /or membrane bioincompat ibility, which could improve with a technique-switch ing to online hemodiafiltration (OL-HD). The antigen-indep endent pathway activation of this modified immunologic state turns dendritic cells (DC) into an accurate cell model to study these patients. The aim of this study is to further evaluate the immune-inflammat ory state of patients in C-HD assessed by DC maturation. Methods: 31 patients were submitted to C-HD and after 4 months switched to the OL-HD technique. Monocytes-derive d DCs from HD patients were cultured in the presence of IL-4/GM-CSF. DC-maturation was evaluated by assessing the maturation phenotype by flow cytometry (FACs). DCs-functiona l capacity to elicit T-cell alloresponse was studied by mixed leuco- cyte reaction. Cytokine release was assessed by FACs and SNS was evaluated measuring renalase levels by ELISA. Results: An up-regulation of maturation markers was observed in C-HD DCs which induced two fold more T cells proliferation than OL-HD DCs. Also, C-HD-mDCs presented with over-produc- tion of pro-inflammatory cytokines (IL-6, IL-1 β , IL-8, IL-10 and TNF- α ) compared with OL- HD-mDC (P < 0·05). Results were correlated with clinical data. When SNS was evaluated, hypotension events and blood pressure were significantly lower and renalase levels were significantly higher after conversion to OL-HD. Diabetes mellitus type 2 patients also found beneficial reduction of mDC when converted to OL-HD compared to non-diabetics. Conclusions: OL-HD could interfere with immuno-inflammatory state in HD patients with an improvement of renalase levels as potential key mediators in the mechanistic pathway of down-regulation of DC maturation

El linfoma de Hodgkin es una neoplasia de células B caracterizada por la escasa presencia de células tumorales, células de Hodgkin/Reed-Sternberg, localizadas en un microambiente no tumoral. El comportamiento del LH se determina por las características intrínsecas de las células HRS y por las características de las células inflamatorias no neoplásicas del microambiente. Por eso, el análisis del papel pronóstico que juegan los miRNAs en el ganglio completo y no únicamente en las células tumorales adquiere un gran significado. La vía JAK/STAT es una vía de señalización utilizada por muchas citoquinas. La activación constitutiva de esta vía resulta de gran importancia en la patogénesis del LH. Se han descrito mutaciones puntuales en el gen JAK2 en síndromes mieloproliferativos; sin embargo, estas mutaciones son escasas en el LH, donde el aumento de la proteína JAK2 se asocia a amplificaciones del gen JAK2, mutaciones en genes reguladores como SOCS-1 o a una activación constitutiva de las proteínas STAT. Otro posible mecanismo que podría intervenir en la sobreexpresión de las proteínas JAK2 es la desregulación de miRNAs cuya función sea controlar la traducción de JAK2. La terapia estándar en LH es a base de antraciclina, con doxorubicina, bleomicina, vinblastina y dacarbacina (ABVD). A pesar de que el LH está considerado uno de los cánceres humanos más curable, con tasas de curación del 80-90%, el tratamiento de pacientes recaídos o con enfermedad refractaria, especialmente aquellos que recaen después de un autotrasplante de células madre, sigue siendo un reto. La FDA (US Food and Drug Administration) no ha aprobado nuevos tratamientos para el LH en los últimos 30 años. Por consiguiente, se necesitan nuevos fármacos, así como nuevas estrategias de tratamiento basadas en el conocimiento de la biología del LH y las vías de señalización implicadas, para mejorar el pronóstico de los pacientes. Lestaurtinib (CEP-701) es un inhibidor de tirosina quinasas con múltiples dianas. En estudios pre-clínicos, se ha demostrado que Lestaurtinib inhibe potentemente a FLT3 en concentraciones nanomolares. Por otra parte, estudios recientes han demostrado que la actividad inhibitoria de Lestaurtinib no se limita a FLT3 y puede suprimir la señalización de JAK2/STAT5 mediante la inhibición específica de JAK2 en SMP; sin embargo la eficiencia de Lestaurtinib en el LH permanece desconocida, aunque podría ser un fármaco de interés debido a las vías moleculares que inhibe, como la vía de JAK2/SAT5 que tan importante es en el LH. Basándonos en estas hipótesis, nos planteamos los siguientes objetivos: 1. Analizar si el patrón de expresión de microRNAs en ganglios linfáticos de pacientes afectos de LH tiene implicaciones pronósticas. 2. Determinar las vías a través de las cuales los miRNAs que actúan como marcadores pronósticos en LH están teniendo un papel clave. De forma específica: a. Determinar si JAK2 es un gen diana de miR-135a. b. Analizar si miR-135a está jugando un papel como gen supresor de tumor en el LH. 3. Analizar posibles acciones terapéuticas basadas en inhibidores de la vía de JAK2 en el LH. Específicamente: a. Evaluar la eficacia in vitro de Lestaurtinib (CEP701), un inhibidor de JAK2, en el LH. b. Evaluar la eficacia de Lestaurtinib en células primarias de ganglios de pacientes afectos de LH.

Les síndromes mielodisplàsiques (SMD) també anomenades preleucemia són un conjunt heterogeni de malalties del sistema hematopoiètic caracteritzades per un grau variable de displasia a la medul·la òssia i en sang perifèrica, un nombre de citopenies en sang perifèrica i un risc de transformació a leucemia mieloide aguda (LMA). La majoria de casos de SMD primaris es presenten sense cap relació amb circumstàncies etiològiques conegudes i tenen una major incidènica en persones d'edat avançada (>75 anys) tot i que també poden afectar a joves. El primer signe que es pot manifestar en un pacient amb aquesta malaltia i més comunment és l'anèmia, tot i que també és possible trobar un baix número de leucòcits en sang perifèrica i un número de plaquetes per sota del que és habitual.El Limfoma de Hodgkin (LH) és un càncer que s'origina al teixit limfàtic, generalment als ganglis linfàtics de la part superior del cos, i es pot propagar a través dels vasos limfàtics a d'altres ganglis, així com també es pot arribar a propagar al fetge i als pulmons. El LH suposa un 1% dels càncers aproximadament i un 10% de tots els limfomes. L'incidència és de 3 casos nous a l'any per cada 100.000 habitants, al contrari que els limfomes no Hodgkin la seva incidència es manté estable al llarg del temps.Els microRNAs són molècules de 22 nucleòtids de cadena senzilla, que no codifiquen per proteïna; aquestes molècules poden regular l'expressió dels seus gens diana unint-se de formacomplementària a l'RNA missatger. Des de que es van descubrir, han anat apareixent diversos estudis que avalen el seu paper d'aquests en processos tumorals. Se sap que els microRNAs es troben de forma freqüent en regions genòmiques "fràgils", associades al càncer. De fet, s'ha establert que els microRNAs tant poden actuar com a oncogens com a gens supressors de tumors.Des de fa pocs anys s'han establert patrons d'expressió de microRNAs per relacionar-los d'una forma directa amb el diagnòstic, l'estadi, la progressió de la malaltia, el pronòstic o la resposta al tractament. Aquests patrons s'han utilitzat per identificar aquells que són diana d'activació de vies oncogèniques o aquells que estan involucrats amb gens que codifiquen per proteïnes relacionades amb el càncer.En les SMD no es coneix amb precisió el mecanisme molecular que dóna lloc al curs evolutiu de la malaltia, tot i que se sap que en les fases inicials de la malaltia predomina el procés d'apoptosis cel·lular, mentre que en la transformació leucèmica predomina una autorenovació ilimitada. Tampoc no se sap si aquesta transformació es deu a canvis a la cèl·lula mare o a canvis en l'expressió gènica de les cèl·lules madures que adquireixen característiques de cèl·lula mare.Pel que fa al Limfoma de Hodgkin és una malaltia en que les cèl·lules neoplàsiques, les cèl·lules de Hodgkin y les de Reed-Sternberg, conformen menys de l'1 % de la cel·lularitat total. Ambdues cèl·lules deriven de cèl·lules B madures que en els centres germinals haurien d'haver entrat en apoptosi després d'adquirir mutacions poc beneficioses en els IgG, però que enlloc d'això aconsgueixen escapar de l'apoptosi malignitzant-se.Recentment, els microRNAs han adquirit una gran importància en la biologia tumoral ja que s'ha observat que poden jugar un paper clau en l'iniciació i progressió tumoral, tant si actuen com a oncogèns o com a gens supressors de tumors. En malalties hematooncològiques s'ha observat la importància dels microRNAs en la caracterització de leucèmies i linfomes, on la desregulació dels quals dóna lloc a una desregulació de la supervivència i la diferenciació de les cèl·lules progenitores hematopoiètiques, mitjançant la regulació de proteïnes claus en el control del cicle celul·lular o de l'apoptosis. A més, s'ha donat molta importància als microRNAs com a eina de gran ajut a l'hora de fer el diagnòstic, sobretot en aquelles patologies on el diagnòstic no és tant evident, ja que mitjançant l'anàlisi del patró d'expressió de microRNAs es pot realitzar la classificació tumoral de forma molt més acurada que mitjançant l'anàlisi de l'expressió de RNAs missatgers.Segons aquests fonaments, considerem que els microRNAs juguen un paper essencial en l'evolució de les SMD, tant en les seves fases més inicials com durant el procés de transformació a leucèmia mieloide aguda (LMA). Per tant, l'anàlisi del patró d'expressió dels microRNAs ens permetrà entendre millor la malaltia, així com detectar aquells pacients que arriben a fer una transformació leucèmica.Per altra banda, en el Limfoma de Hodgkin l'anàlisi dels miRNAs en ganglis limfàtics dels pacients ens permetran caracteritzar millor la malaltia, tant a nivell de célula tumoral com a nivell de microambient tumoral. D'aquesta manera, l'anàlisi dels microRNAs ens permetrà entendre millor els mecanismes pel quals les cèl·lules B de les que deriva la cèl·lula de H/RS escapen de l'apoptosis malignitzant-se.Basant-nos en tot allò exposat anteriorment, a l'hora de preparar la següent Tesi Doctoral ens hem marcat els següents objectius:A) OBJECTIUS GENERALS:Analitzar en pacients afectes d'una Síndrome Mielodisplàsica i en pacients amb un limfoma de Hodgkin l'expressió de microRNAs per tal de aprofundir en el coneixement de les dues malalties.B) OBJECTIUS ESPECÍFICS:Per les SMD:1. Analitzar en pacients afectes d'una Síndrome Mielodisplàsica l'expressió de microRNAs relacionats amb la hematopoiesis en comparació amb controls sans.2. Estudiar la relació entre l'expressió de microRNAs en mèdula òsea (MO) i sang perifèrica (SP) en pacientes afectes d'una SMD.3. Relacionar l'expressió dels microRNAs amb les característiques clíniques i el seu paper com a marcadors pronòstic.4. Investigar les cèl·ules diana on aquests microRNAs estan sobreexpresats mitjançant hibridació in-situ.Pel Limfoma de Hodgkin:5. Analitzar el patró d'expressió de microRNAs en ganglis linfàtics de pacients afectes d'un LH, respecte a ganglis linfàtics normals.6. Analitzar les possibles diferències entre l'expressió de microRNAs en els diferents subtipus (esclerosi nodular, cel·lularitat mixta).7. Identificar aquells microRNAs expressats en les cèl·lules tumorals mitjançant hibridació "in situ".8. Estudiar el paper que juga el virus de l'Epstein Barr en el desenvolupament del LH mitjançant els microRNAs.

INTRODUCCIÓN Y OBJETIVOS: La causa fundamental de la trombosis en la policitemia vera y la trombocitemia esencial es el aumento de la viscosidad sanguínea secundaria a la eritrocitosis así como el incremento del número de plaquetas y las alteraciones funcionales de las mismas. Sin embargo, estudios recientes sugieren un posible papel al respecto de los granulocitos. Se desconoce si, al igual que en la PV y la TE, los pacientes con mielofibrosis primaria tienen un riesgo aumentado de trombosis. El objetivo de la presente tesis consiste en analizar la activación de los granulocitos, las plaquetas y la interacción leucocito-plaqueta en pacientes con SMPC con y sin trombosis a fin de contribuir al conocimiento de los mecanismos que intervienen en la patogenia de la trombosis en dichos enfermos.En este contexto, nos planteamos realizar tres trabajos de investigación con los siguientes objetivos:1. Analizar la activación de los granulocitos y la interacción leucocito-plaqueta en pacientes con PV, con y sin trombosis del eje esplenoportal o síndrome de Budd-Chiari, a fin de contribuir al conocimiento de los mecanismos que intervienen en la patogenia de la trombosis en dichos enfermos. 2. Determinar la incidencia de trombosis en los pacientes con MFP, comparándola con la de la población general, y analizar los factores de riesgo para su aparición.3. Investigar si los pacientes con MFP presentan una activación leucocitaria, plaquetaria, endotelial y de la coagulación similar a la existente en la PV y la TE. 4. Determinar si en la MFP la posible sobreexpresión de dichos parámetros de activación se correlaciona con la presencia de la mutación del gen JAK2.RESUMEN TRABAJO 1:La policitemia vera (PV) se caracteriza por una intensa proliferación de la serie eritroide y, a veces, de la megacariocítica y granulocítica, siendo las complicaciones trombóticas la causa fundamental de morbilidad y mortalidad en estos pacientes. Dentro de las trombosis de la PV, las que afectan a las venas suprahepáticas (síndrome de Budd-Chiari, SBC) o a las del eje esplenoportal (TP) se encuentran entre las complicaciones más graves de la enfermedad, siendo, de hecho, la PV la principal causa de este tipo de trombosis.La causa fundamental de la trombosis en la PV es el aumento de la viscosidad sanguínea, debido al incremento en el número de hematíes y plaquetas, junto a las alteraciones funcionales de éstas. Sin embargo, estudios recientes sugieren un posible papel de los granulocitos. Así, se ha descrito un aumento de la activación leucocitaria en los pacientes con PV, demostrado por un incremento en la expresión del antígeno CD11b en la membrana de los neutrófilos y de la fosfatasa alcalina granulocitaria, así como del contenido de elastasa celular y plasmática. En estos enfermos se observó asimismo un aumento de ciertos marcadores plasmáticos de hipercoagulabilidad, en concreto, el complejo trombina-antitrombina, el fragmento 1+2 de la protrombina (F1+2) y el dímero-D. Con la finalidad de dilucidar los mecanismos involucrados en la génesis del SBC y la TP de los pacientes con PV, en el presente trabajo de investigación se estudió la expresión de CD11b granulocitario, el metabolismo oxidativo de los neutrófilos y el porcentaje de complejos leucocito-plaqueta circulantes en 17 pacientes afectos de PV que habían presentado SBC o TP. Como grupos control se estudiaron 16 pacientes con PV sin ningún antecedente de trombosis, 20 pacientes con SBC/TP en los que se había descartado la existencia de un síndrome mieloproliferativo crónico concomitante y 20 individuos sanos. Los resultados obtenidos mostraron que la expresión del antígeno CD11b granulocitario, el metabolismo oxidativo de los neutrófilos y el porcentaje de complejos leucocito-plaqueta circulantes eran significativamente más altos en los pacientes con PV que en los pacientes sin síndrome mieloproliferativo o los controles sanos. Además, la expresión de CD11b granulocitario, tanto basal como tras estímulo con F-MLP, fue significativamente más elevada en los pacientes con PV y trombosis que en los pacientes con PV sin trombosis. Estos resultados sugieren que la activación leucocitaria, y en concreto la sobreexpresión del antígeno CD11b, pueden desempeñar un papel en la patogenia del SBC y la TP en la PV. RESUMEN TRABAJO 2:La policitemia vera (PV), la trombocitemia esencial (TE) y la mielofibrosis primaria (MFP) son síndromes mieloproliferativos crónicos cromosoma Filadelfia negativos que comparten ciertas características clínicas y biológicas. En este sentido, el reciente descubrimiento de la mutación V617F del gen JAK2 en el 95% de los pacientes con PV y en la mitad de los enfermos con TE y MFP ha venido a apoyar la agrupación de estas tres entidades en una posición nosológica común.Desde el punto de vista clínico, dos de estos SMPC, la PV y la TE, se asocian a un riesgo aumentado de trombosis, siendo las complicaciones trombóticas la principal causa de morbilidad y mortalidad en estos pacientes. Así, en torno al 40% de los pacientes con PV y al 30% de los enfermos con TE presentan trombosis, bien en el momento del diagnóstico o a lo largo de la evolución de la enfermedad.Con respecto a la MFP, no existen trabajos realizados en series amplias en los que se haya estudiado de forma específica la incidencia real de trombosis y los factores de riesgo asociados a la aparición de dicha complicación. A partir de estudios realizados en series de escaso tamaño y de la observación de que los pacientes con MFP podían presentar trombosis tras la esplenectomía, algunos autores han sugerido que los pacientes con MFP podrían tener un mayor riesgo de trombosis. Sin embargo, el aumento en la incidencia de trombosis podría estar en relación con la edad avanzada de la mayoría de los pacientes con MFP, por lo que podría no existir una diferencia real con respecto a la población general. Por otro lado, se ha referido en la MFP que la presencia de la mutación V617F de JAK2 se asocia a un mayor riesgo trombótico, lo cual sugiere que, al igual que en la PV y la TE, los mecanismos involucrados en la patogénesis de la trombosis descritos en estas enfermedades también podrían intervenir en la MFP.El objetivo del presente estudio fue determinar la frecuencia y el tipo de trombosis en la MFP, así como los factores asociados a su aparición. Para ello se revisaron las historias clínicas de 155 pacientes diagnosticados de forma consecutiva en el servicio de Hematología del Hospital Clínic. Con un seguimiento mediano de 4,2 años, se observaron un total de 31 complicaciones trombóticas (19 arteriales y 12 venosas) en 18 pacientes. En 6 enfermos la trombosis fue simultánea al diagnóstico de MFP o la precedió en pocos meses, mientras que en 14 la trombosis constituyó una complicación que apareció en el curso evolutivo de la enfermedad. Con respecto a la población general, los pacientes con MFP presentaron un riesgo significativamente incrementado de presentar trombosis venosa profunda y accidentes vasculares cerebrales. La trombocitosis, la presencia de factores de riesgo cardiovascular y la fase celular de la mielofibrosis fueron las variables que se asociaron de forma significativa a un riesgo mayor de trombosis en el análisis multivariado. La probablidad actuarial de trombosis a los 5 años fue del 9,6% para la serie global, 19,4% para los pacientes con plaquetas > 450x109/L, 17,4% para los pacientes con algún factor de riesgo vascular y 14,9% para los pacientes con mielofibrosis en fase celular. Estos resultados indican que los pacientes con MFP presentan un riesgo incrementado de desarrollar complicaciones trombóticas, siendo el riesgo más alto en las formas hiperproliferativas de la enfermedad y en los pacientes con factores de riesgo cardiovascular. RESUMEN TRABAJO 3:En la PV y la TE se ha demostrado la existencia de un aumento en los marcadores de activación leucocitaria y plaquetaria, correlacionándose el grado de activación de algunos de estos marcadores con la presencia de la mutación V617F de JAK2 y el antecedente de trombosis. El hecho de que las tres enfermedades compartan una etiopatogenia común, a partir de la adquisición de la mutación de JAK2, y que recientemente hayamos descrito que en la MFP existe también un aumento de complicaciones trombóticas sugieren que en esta enfermedad podría existir asimismo una sobreexpresión de dichos marcadores de activación. Con la finalidad de determinar si en la MFP existe un aumento en la activación leucocitaria, plaquetaria, endotelial y de la coagulación, se estudió una cohorte constituida por 26 pacientes afectos de MFP y un grupo control formado por 22 sujetos sanos. Las parámetros evaluados incluyeron la determinación de la P-selectina (basal y tras estímulo con agonistas plaquetarios), la expresión del antígeno CD11b granulocitario, los complejos leucocito-plaqueta circulantes y la concentración plasmática de P-selectina, CD40L, factor tisular, trombomodulina, fragmento 1+2 de la protrombina (F1+2) y dímero-D. Los pacientes con MFP mostraron una expresión de P-selectina basal más elevada y un porcentaje mayor de complejos monocito-plaqueta que los controles sanos, siendo las diferencias estadísticamente significativas. La expresión de CD11b en neutrófilos y monocitos fue significativamente más alta en los pacientes con MFP JAK2 positiva que en los pacientes con MFP JAK2 negativa o en los controles. Además, la concentración plasmática de trombomodulina y de F1+2 fue significativamente más alta en la MFP que en los individuos sanos, mostrando, a su vez, los pacientes con la mutación un valor de F1+2 significativamente mayor que los pacientes sin la mutación. De los anteriores resultados se puede concluir que los pacientes con MFP presentan parámetros de activación leucocitaria, plaquetaria, endotelial y de la coagulación similares a los observados en la PV y la TE y que algunos de estos parámetros se correlacionan con la presencia de la mutación V617F del gen JAK2. CONCLUSIONES:1. La expresión del antígeno CD11b granulocitario está notablemente incrementada en los pacientes con PV que presentan síndrome de Budd-Chiari o trombosis del eje esplenoportal. Dicha alteración podría intervenir en la patogenia de la trombosis de la PV al facilitar la adhesión de los granulocitos al endotelio y las plaquetas. 2. En comparación con la población general, los pacientes con MFP presentan un riesgo aumentado de presentar trombosis venosas profundas e infarto cerebral. 3. En los sujetos con MFP el riesgo de trombosis es mayor en aquellos con formas hiperproliferativas de la enfermedad y en los que presentan factores de riesgo cardiovascular concomitantes.4. Los pacientes con MFP presentan datos de activación leucocitaria, plaquetaria, endotelial y de la coagulación similares a los previamente descritos en los enfermos con PV y TE. 5. En la MFP la presencia de la mutación V617F del gen JAK2 se asocia a una sobreexpresión del antígeno CD11b y a una concentración plasmática elevada del fragmento 1+2 de la protrombina.

Study design: The study was carried out on 97 patients admitted to the Hematology Service of the Hospital Duran y Reynals in Barcelona during 2002-2003. These patients received treatment with chemotherapy or conditioning prior to bone marrow transplant. A descriptive study was made, analyzing oral hygiene, one dental index, and two gingivales indices, and evaluating their relationship with the appearance of mucositis. Results: The patients with high plaque (PI) and gingival (GI) indices during chemotherapy presented a higher percentage of mucositis (77.4% and 65.7% respectively) against those who had little or no visible plaque. In the case of the PI, the differences were statistically significant (p=0.015). Likewise, patients who brushed their teeth 3 times/day presented mucositis in only 26.7% of cases, against those who did not brush, or brushed only once a day (65.9% and 68.4%), these differences also being statistically significant (p=0.013). The CAO showed similar results in patients with or without mucositis (7.59 and 7.03 respectively). Conclusions: In our study, a good gingival status as well as good oral hygiene during chemoradiotherapy is associated with a lower incidence and severity of mucositis.

Podeu consultar la versió en anglès a http://hdl.handle.net/2445/52423, El objetivo del presente trabajo fue valorar si un buen estado buco-dental (valorado mediante índices dento-gingivales), se asociaba a una menor incidencia y gravedad de mucositis oral en pacientes con enfermedades hematológicas que iban a recibir tratamiento con quimioterapia o un trasplante de médula ósea. Diseño del estudio: El estudio se llevó a cabo en 97 pacientes ingresados en el Servicio de Hematología del Hospital Duran y Reynals en Barcelona en los años 2002-2003. Estos pacientes recibían tratamiento con quimioterapia o bien el acondicionamiento previo a un trasplante de médula ósea. Se realizó un estudio descriptivo analizando un índice dental, dos índices gingivales y la higiene oral y su relación con la aparición de mucositis. Resultados: Los pacientes que durante la quimioterapia mostraron valores elevados del índice de placa (IP) y gingival (IG) presentaron un mayor porcentaje de mucositis (77.4% y 65.7% respectivamente) frente a los que tenían poca placa o ésta no era visible. En el caso del IP las diferencias fueron estadísticamente significativas (p=0.015). Asimismo los pacientes que cepillaban los dientes 3veces/día sólo presentaron mucositis en un 26.7% de los casos, frente a los que no cepillaban o lo hacían una vez al día (65.9% y 68.4%), siendo estas diferencias también estadísticamente significativas (p=0.013). El ICAO mostró resultados similares en los pacientes con o sin mucositis (7.59 y 7.03 respectivamente). Conclusiones: En nuestro trabajo, un buen estado gingival así como una buena higiene oral durante la quimiorradioterapia, se asocian a una menor incidencia y gravedad de mucositis.

La trombofilia és la tendència genètica al tromboembolisme venós. Amb la finalitat d'aprofundir en les bases genètiques de la trombofília hereditària, l'objectiu principal d'aquesta tesi és la identificació dels factors genètics que determinen la variabilitat dels nivells de FVII a la població espanyola. En primer lloc, es va calcular l'heretabilitat dels nivells de FVII en un 53%, i mitjançant una anàlisi global del genoma es va localitzar el gen responsable de la variabilitat dels nivells de FVII en la zona 13q, just on es localitza el gen estructural del FVII (F7). Aquest resultat indica que els factors genètics que determinen la variabilitat de FVII en plasma es troben en el gen estructural. A continuació es va realitzar una anàlisi genètica exhaustiva i es van obtenir 49 posicions polimòrfiques al llarg del gen (F7), la majoria canvis d'una sola base (SNPs). A més, mitjançant una anàlisi de l'organització haplotípica de la variació en la seqüència del (F7), es va observar que existeixen tres clars llinatges d'haplotips que s'han mantingut paral·lelament en la població espanyola per selecció estabilitzadora, molt probablement per un avantatge dels individus heterozigots. L'anàlisi d'associació entre aquests llinatges i els nivells de FVII indica que un d'ells, caracteritzat pels al·lels [-670C, -630G, -402A] està associat a nivells elevats en la població, i un altre, caracteritzat pels al·lels [-2989A, -401T, -323ins10, -122C] està associat a nivells reduïts.Les anàlisis posteriors de transfecció in vitro amb els diferents al·lels utilitzant un sistema reporter GFP (Green Fluorescent Protein) van permetre determinar que les variants -323ins10 i -122C tenen un efecte reductor sobre els nivells de proteïna, i que la variant -402A provoca un augment significatiu dels nivells de FVII. Finalment, la nova variant -2989A incrementa també de forma molt significativa els nivells d'expressió. Aquests resultats es confirmen a més en un estudi cas-control amb pacients amb trombosi venosa profunda, així com en dues famílies amb deficiència de FVII. El disseny, l'estratègia i la metodologia emprades són un bon exemple de com localitzar i identificar loci implicats en caràcters complexes., There is a definite genetic predisposition to venous thromboembolism. The levels of FVII in plasma has been clearly implicated in thrombophilia. The aim of this thesis was to identify and characterize the genetic factors that determine the variability of FVII plasma levels in the Spanish population. First, the heritability of FVII levels was calculated as 53%indicating a substantial genetic component. A Genome Wide Scan allowed the localization of the responsible gene on the long arm of chromosome 13, exactly where the FVII "(F7)" structural gene has been located. This indicates that the genetic factors that determine FVII variability are localized in the structural gene.Second, from an exhaustive genetic analysis, we obtained 49 polymorphisms (variants) in the "F7" gene. Further, by an analysis of sequence variations and haplotype organization, three main haplotype lineages were detected that have been maintained presumably by balanced selection, probably due to an advantage of heterozygous individuals. The association analysis among these lineages and the FVII levels indicates that one of these lineages, containing allelic variants [-670C, -630G, -402A] was associated with elevated FVII levels. Also, another lineage, containing allelic variants [-2989A, -401T, -323ins10, -122C] was associated with reduced levels of FVII. Posterior functional analyses of "in vitro" transfection of the different variants in a "reporter" system with GFP "(Green Fluorescent Protein)" showed that allelic variants -323ins10 i -122C significantly reduce expression levels while allelic variant -402A significantly increases them. The new allelic variant -2989A also increases expression levels significantly. Finally, these results have been confirmed in a case-control study with patients with deep venous thrombosis and in a study of two families with a FVII deficiency. The strategy and methodology of the present work are good examples of how to localize and identify genes implicated in complex human diseases.

Objetivo: Conocer la incidencia real de las lesiones en la cavidad bucal de los enfermos con hemopatías malignas, las debidas a la enfermedad y las ocasionadas por los tratamientos recibidos, con el fin de prevenir, diagnosticar y tratar las manifestaciones orales de la hemopatía así como las mucositis originadas por los diferentes tratamientos de quimioterapia o radioterapia. Diseño: se revisaron a pacientes con hemopatías malignas en fase de tratamiento o a los que se les iba a realizar un transplante hematopoyético. Se les realizó un estudio clínico, y en casos necesarios un cultivo microbiológico o un estudio anatomopatológico. Resultados: la realización de un protocolo en estos pacientes para el control de sus lesiones orales así como la realización de una pauta profiláctica y de tratamiento para sus lesiones. Conclusión: mejor conocimiento y manejo de los pacientes con hemopatías malignas.

DE LA TESIS:Mi interés por el tema nació hace ya algunos años, durante mi permanencia en la sala de Medicina Interna, cuya dirección está a cargo del Dr. A. URBANO MARQUEZ, a quien quiero dar testimonio de mi agradecimiento y afecto. En ella tuve ocasión de vivir la problemática clínica, fundamentalmente infecciosa, del enfermo hematológico ingresado en una sala convencional. La creación de una unidad de aislamiento libre de gérmenes y la terapéutica de soporte con transfusiones de granulocitos (GN) ha supuesto un gran avance en el tratamiento de los enfermos con granulocitopenia. En nuestro Servicio la aplicación clínica de las transfusiones de granulocitos se efectúa desde hace aproximadamente 5 ó 6 años. Una vez establecida esta primera etapa asistencial, sin duda la más importante, he creído de interés profundizar en el estudio de los GN obtenidos mediante el procedimiento de leucaferesis, con el ánimo de que nuestros hallazgos podrían de alguna forma contribuir a mejorar la asistencia clínica de los enfermos con granulocitopenia. La posibilidad que tenía de combinar los estudios funcionales con la observación morfológica ultraestructural de las células, influyó notablemente en la elección de la presente tesis doctoral. En el proceso de leucaféresis intervienen diversos factores, como el tiempo requerido para obtener un número suficiente de GN, la premedicación que reciben los donante, el traumatismo mecánico que supone el propio proceso de obtención, etc., que pueden alterar en grado variable a los GN. Estas alteraciones pueden repercutir de forma indirecta sobre la respuesta clínica del receptor con granulocitopenia. El objetivo de la presente tesis doctoral es identificar las posibles alteraciones morfológicas y funcionales en los GN obtenidos con los procedimientos de centrifugación a flujo continuo y de filtración, e intentar averiguar cuáles pueden ser las causas responsables de las mismas.

Sobre un total de 133 recién nacidos, distribuidos en tres grupos según su patología hematológica (recién nacidos sanos, recién nacidos anémicos y recién nacidos poliglobúlicos), se realiza un estudio del valor de la viscosidad sanguínea relativa en cada grupo citado. La determinación se ha hecho mediante el viscosímetro de Hess. Se realiza también un estudio de las poliglobulias neonatales bajo el punto de vista clínico, analítico y terapéutico. Se han valorado también las correlaciones entre hematocrito central y periférico, viscosidad sanguínea relativa y hematocrito periférico. Se propone un protocolo terapéutico para las poligobulias neonatales y se juzga la eficacia que se ha obtenido con dicho protocolo. La viscosidad sanguínea relativa del recién nacido normal es del 4,99 y el hematocrito periférico es un 8 por ciento superior al central.

Introduction Diarrhea is a frequent complication in hematologic patients, being an infectious cause frequently suspected. Rapid and accurate detection of gastrointestinal pathogens is vital in immunocompromised hosts. The aim of this study was to compare routine diagnostic methods versus a multiplex polymerase chain reaction (PCR) assay for the diagnosis of infectious diarrhea in immunocompromised hematologic patients. Material and methods We conducted a prospective observational study from March 2015 to January 2016 to compare conventional methods for the diagnosis of infectious diarrhea with FIlmArray GI Panel (BioFire-bioMerieux, France). Samples from adult immunocompromised hematologic patients with acute diarrhea were collected. In cases with discordant results, a second multiplex assay was performed (Allplex, Seegene, Korea). The result was considered positive or negative when the same result was obtained by at least two of the methods. Results A total of 95 samples were obtained from 95 patients (median age of 52 years (46–64)). Sixty-one (64%) episodes were hospital-acquired and 34 (36%) were community-acquired diarrhea. Twenty-five (26%) patients had a positive microbiological result, being Clostridium difficile the most frequent pathogen, followed by Campylobacter spp and norovirus. The concordance between FilmArray methods was good (k = 0.79). The FilmArray GI panel showed a sensitivity of 95%, a specificity of 100% for positive results. The time required to obtain results was markedly reduced with the use of multiplex PCR methods. Conclusions Multiplex molecular panels provide a rapid and sensitive tool for the diagnosis of infectious diarrhea, thereby allowing more timely clinical decisions in immunocompromised hematologic patients.

Prior to Janus kinase inhibitors, available therapies for myelofibrosis were generally supportive and did not improve survival. This analysis compares efficacy outcomes of patients with myelofibrosis in the control arms (placebo [n=154] and best available therapy [n=73]) from the two phase 3 COntrolled MyeloFibrosis study with ORal JAK inhibitor Treatment (COMFORT) studies. Spleen volume was assessed by magnetic resonance imaging/computed tomography at baseline and every 12 weeks through week 72; spleen length was assessed by palpation at each study visit. Health-related quality of life and symptoms were assessed using the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30 Items at baseline and in weeks 4, 8, 12, 16 and 24 in COMFORT-I and in weeks 8, 16, 24 and 48 in COMFORT-II. The demographic and baseline characteristics were similar between the control arms of the two studies. One patient who received placebo and no patients who received best available therapy had a ≥35% reduction in spleen volume from baseline at week 24. At 24 weeks, neither placebo nor best available therapy had produced clinically meaningful changes in global quality of life or symptom scales. Non-hematologic adverse events were mostly grade 1/2; the most frequently reported adverse events in each group were abdominal pain, fatigue, peripheral edema and diarrhea. These data suggest that non-Janus kinase inhibitor therapies provide little improvement in splenomegaly, symptoms or quality of life as compared with placebo. Both COMFORT-I (NCT00952289) and COMFORT-II (NCT00934544) studies have been appropriately registered with clinicaltrials.gov.

The role of antiplatelet therapy as primary prophylaxis of thrombosis in low-risk essential thrombocythemia has not been studied in randomized clinical trials. We assessed the benefit/risk of low-dose aspirin in 433 patients with low-risk essential thrombocythemia (271 with a CALR mutation, 162 with a JAK2(V617F) mutation) who were on antiplatelet therapy or observation only. After a follow up of 2215 person-years free from cytoreduction, 25 thrombotic and 17 bleeding episodes were recorded. In CALR-mutated patients, antiplatelet therapy did not affect the risk of thrombosis but was associated with a higher incidence of bleeding (12.9 versus 1.8 episodes per 1000 patient-years, P=0.03). In JAK2(V617F)-mutated patients, low-dose aspirin was associated with a reduced incidence of venous thrombosis with no effect on the risk of bleeding. Coexistence of JAK2(V617F)-mutation and cardiovascular risk factors increased the risk of thrombosis, even after adjusting for treatment with low-dose aspirin (incidence rate ratio: 9.8; 95% confidence interval: 2.3-42.3; P=0.02). Time free from cytoreduction was significantly shorter in CALR-mutated patients with essential thrombocythemia than in JAK2(V617F)-mutated ones (median time 5 years and 9.8 years, respectively; P=0.0002) and cytoreduction was usually necessary to control extreme thrombocytosis. In conclusion, in patients with low-risk, CALR-mutated essential thrombocythemia, low-dose aspirin does not reduce the risk of thrombosis and may increase the risk of bleeding. La finalidad fue evaluar la efectividad de la implantación de evidencias mediante una investigación-acción-participante (IAP). Diseño cuasi-experimental prospectivo con dos grupos no equivalentes ni concurrentes (2006 a 2008) en una unidad de enfermería de trasplante de medula ósea de un hospital español de tercer nivel. La intervención estudiada fue la implantación de evidencias mediante una IAP. Las variables dependientes: desempeño profesional y resultados de salud (área psico-emocional y efectos adversos). Se reclutaron 125 pacientes (1er grupo=56; 2º grupo=69). Se aprecia una mejoría significativa en el segundo grupo en la calidad de los registros de valoración de signos y síntomas. En el área psico-emocional, el ajuste psico-social mejora significativamente; no hay cambios en la sobrecarga ni la satisfacción. En efectos adversos, la obstrucción e infección relacionada con catéter mejoran significativamente; no hay diferencias en dolor y mucositis. La IAP ha servido para implantar evidencias y mejorar los resultados de salud.
