Abstract

Background Findings from the T.O.S.CA. Registry recently reported that patients with concomitant chronic heart failure (CHF) and impairment of insulin axis (either insulin resistance—IR or diabetes mellitus—T2D) display increased morbidity and mortality. However, little information is available on the relative impact of IR and T2D on cardiac structure and function, cardiopulmonary performance, and their longitudinal changes in CHF. Methods Patients enrolled in the T.O.S.CA. Registry performed echocardiography and cardiopulmonary exercise test at baseline and at a patient-average follow-up of 36 months. Patients were divided into three groups based on the degree of insulin impairment: euglycemic without IR (EU), euglycemic with IR (IR), and T2D. Results Compared with EU and IR, T2D was associated with increased filling pressures (E/e′ratio: 15.9 ± 8.9, 12.0 ± 6.5, and 14.5 ± 8.1 respectively, p 2) in TD2 vs EU and IR patients was recorded (respectively, 15.8 ± 3.8 ml/Kg/min, 18.4 ± 4.3 ml/Kg/min and 16.5 ± 4.3 ml/Kg/min, p 2 in the T2D group (+ 13% increase in RV dimension, − 21% decline in TAPSE/PAPS ratio and − 20% decrease in peak VO2). Conclusion The higher risk of death and CV hospitalizations exhibited by HF-T2D patients in the T.O.S.CA. Registry is associated with progressive RV ventricular dysfunction and exercise impairment when compared to euglycemic CHF patients, supporting the pivotal importance of hyperglycaemia and right chambers in HF prognosis. Abstract

Aims Recent evidence supports the occurrence of multiple hormonal and metabolic deficiency syndrome (MHDS) in chronic heart failure (CHF). However, no large observational study has unequivocally demonstrated its impact on CHF progression and outcome. The T.O.S.CA. (Trattamento Ormonale nello Scompenso CArdiaco; Hormone Treatment in Heart Failure) Registry has been specifically designed to test the hypothesis that MHDS affects morbidity and mortality in CHF patients. Methods and Results The T.O.S.CA. Registry is a prospective, multicentre, observational study involving 19 Italian centres. Thyroid hormones, insulin-like growth factor-1, total testosterone, dehydropianoandrosterone sulfate, insulin resistance, and the presence of diabetes were evaluated. A MHDS was defined as the presence of ≥2 hormone deficiencies (HDs). Primary endpoint was a composite of all-cause mortality and cardiovascular hospitalizations. Four hundred and eighty heart failure patients with ejection fraction ≤45% were enrolled. MHDS or diabetes was diagnosed in 372 patients (77.5%). A total of 271 events (97 deaths and 174 cardiovascular hospitalizations) were recorded, 41% in NO-MHDS and 62% in MHDS (P Conclusion MHDS is common in CHF and independently associated with increased all-cause mortality and cardiovascular hospitalization, representing a promising therapeutic target. 9. Enhancement of crystallization kinetics of poly(l-lactic acid) by copolymerization with optically pure branches

Abstract

This work details the first example of enhancement of crystallization rate of poly (L-lactic acid) (PLLA) by introducing optically pure short chain branches. A biobased PLLA copolymer was prepared by initial radical functionalization of commercial PLLA with itaconic anhydride (IAH) in a Brabender mixer, then by reaction with a tailor-made hydroxyl-terminated, optically pure PLLA with molar mass Mn = 4 kDa, the latter prepared via ring opening polymerization. Gel permeation chromatography and infrared spectroscopy proved the efficiency of grafting reaction, with the amount of grafted IAH quantified via UV-Vis. The synthesized graft copolymer displays faster crystallization rate with respect not only to the commercial grade, but also to a binary blend with the same nominal composition. The role played by the short branches in favoring both crystal nucleation and growth was discussed in terms of molecular nucleation. The results detailed in this manuscript demonstrate that synthesis of a graft copolymer with optically pure short branches is an efficient way to improve the poor crystallization kinetics of PLLA, which is one of the major drawbacks of this polymer.

12. Nanoparticles decorated with folate based on a site-selective αCD-rotaxanated PEG-b-PCL copolymer for targeted cancer therapy

Abstract

Nanoparticles (NPs) made up of biodegradable block copolymers offer significant potential in the field of innovative anticancer therapies. However, the accumulation of NPs in tumour cells remains challenging. Surface decoration with targeting ligands recognizing specific receptors that are overexpressed on the cancer cell membrane represents the most common approach, but the effective exposure of the targeting molecule on the NP surface is often limited. An original method to favor the exposure of folate (Fol) as a targeting ligand on the NP surface is reported here. We focused on the introduction in NPs based on a PEG-b-PCL diblock copolymer of Fol-PEG-b-PCL in which the PEG chain is threaded with α-cyclodextrin (αCD). This selectively rotaxanated copolymer (Fol-PEG(αCD)-b-PCL) was prepared from an α,ω-azido-tosyl-PEG(αCD) pseudorotaxane, taking advantage of the different terminal groups of PEG. Fol and the PCL block, which act as caps to prevent αCD dethreading, were covalently linked to PEG with a "one-step"procedure, giving a rotaxanated copolymer with a high yield and a low host coverage (∼9%). The formation of the inclusion complex was confirmed by 2D-DOSY spectroscopy. Rotaxanated copolymers were fully characterized by elemental analysis, FTIR and 1H NMR spectroscopy, thermal and thermogravimetric analysis, wide angle X-ray diffraction and polarised optical microscopy. Abstract

Recent evidence supports the concept that progression of chronic heart failure (CHF) depends upon an imbalance of catabolic forces over the anabolic drive. In this regard, multiple hormonal deficiency syndrome (MHDS) significantly has impacts upon CHF progression, and is associated with a worse clinical status and increased mortality. The T.O.S.CA. (Trattamento Ormonale nello Scompenso CArdiaco; Hormone Therapy in Heart Failure) Registry (clinicaltrial.gov = NCT02335801) tests the hypothesis that anabolic deficiencies reduce survival in a large population of mild-to-moderate CHF patients. The T.O.S.CA. Registry is a prospective multicenter observational study coordinated by “Federico II” University of Naples, and involves 19 centers situated throughout Italy. Thyroid hormones, insulin-like growth factor-1, total testosterone, dehydroepiandrosterone , and insulin are measured at baseline and every year for a patient-average follow-up of 3years. Subjects with CHF are divided into two groups: patients with one or no anabolic deficiency, and patients with two or more anabolic deficiencies at baseline. The primary endpoint is the composite of all-cause mortality and cardiovascular hospitalization. Secondary endpoints include the composite of all-cause mortality and hospitalization, the composite of cardiovascular mortality and cardiovascular hospitalization, and change of VO 2 peak. Patient enrollment started in April 2013, and was completed in July 2017. Demographics and main clinical characteristics of enrolled patients are provided in this article. Detailed cross-sectional results will be available in late 2018. The T.O.S.CA. Registry represents the most robust prospective observational trial on MHDS in the field of CHF. The study findings will advance our knowledge with regard to the intimate mechanisms of CHF progression and hopefully pave the way for future randomized clinical trials of single or multiple hormonal replacement therapies in CHF.

A new polymeric blend, polymethyl methacrylate-poly(ehtylene-co-vinylacetate) (PMMA-EVA) has been produced recently in our laboratories. The optothermal characterization of PMMA-EVA was carried out by measuring the ultraviolet photoacoustic spectra of PMMA, EVA, and PMMA-EVA (25 degrees C, 220-350 nm), of a PMMA-EVA blend (250-750 nm) at two temperatures (19 degrees C and 65 degrees C), and by measuring the scattering coefficient (at 514.5 nm) of PMMA-EVA versus the temperature in the range 38-86 degrees C. Our measurements show that, in the explored thermal range, the blend undergoes a phase transition when its optical properties change drastically. The peculiar optical and thermal properties observed appear attractive for possible applications as temperature-controlled optical devices.

Abstract

The present paper reports on pectin based films modified with carboxymethyl cellulose intended for food packaging. The films are prepared by solvent-casting method with different carboxymethyl cellulose content and cross-linker concentration (Ca2+ ions) in the presence of glycerol as a plasticizer. FT-IR spectra of the prepared films propose that carboxyl group from pectin are mainly involved in interactions with CMC, whereas −OH groups are mainly involved in self-associated hydrogen bonding of neat polymers. Further, an addition of carboxymethyl cellulose improved mechanical properties compared to pure pectin films, while TGA analysis confirmed satisfying thermal stability regarding their potential application as packaging material. Finally, water vapor permeability values are in the range of 1.32 × 10−7 up to 2.03 × 10−7 g/m h Pa.

Abstract

Recent evidence supports the concept that progression of chronic heart failure (CHF) depends upon an imbalance of catabolic forces over the anabolic drive. In this regard, multiple hormonal deficiency syndrome (MHDS) significantly has impacts upon CHF progression, and is associated with a worse clinical status and increased mortality. The T.O.S.CA. (Trattamento Ormonale nello Scompenso CArdiaco; Hormone Therapy in Heart Failure) Registry (clinicaltrial.gov = NCT02335801) tests the hypothesis that anabolic deficiencies reduce survival in a large population of mild-to-moderate CHF patients. The T.O.S.CA. Registry is a prospective multicenter observational study coordinated by “Federico II” University of Naples, and involves 19 centers situated throughout Italy. Thyroid hormones, insulin-like growth factor-1, total testosterone, dehydroepiandrosterone , and insulin are measured at baseline and every year for a patient-average follow-up of 3 years. Subjects with CHF are divided into two groups: patients with one or no anabolic deficiency, and patients with two or more anabolic deficiencies at baseline. The primary endpoint is the composite of all-cause mortality and cardiovascular hospitalization. Secondary endpoints include the composite of all-cause mortality and hospitalization, the composite of cardiovascular mortality and cardiovascular hospitalization, and change of VO2 peak. Patient enrollment started in April 2013, and was completed in July 2017. Demographics and main clinical characteristics of enrolled patients are provided in this article. Detailed cross-sectional results will be available in late 2018. The T.O.S.CA. Registry represents the most robust prospective observational trial on MHDS in the field of CHF. The study findings will advance our knowledge with regard to the intimate mechanisms of CHF progression and hopefully pave the way for future randomized clinical trials of single or multiple hormonal replacement therapies in CHF.
