3. Genetic variation in PPARG, TCF7L2, and FTO are associated with plasma calcium, fasting glucose and obesity in an island population of the eastern Adriatic

We performed an association analysis of 97 Single Nucleotide Polymorphisms (SNPs) in eight candidate genes (FTO, INSIG2, PPARG, MC4R, SLC30A8, CDKN2A, TCF7L2 and HHEX) with 17 metabolic traits in a sample of >950 unrelated adult individuals from the island of Hvar in the eastern Adriatic coast of Croatia. This population of Slavic origins, living in relative isolation for over 400 years, has a high prevalence (~26%) of metabolic syndrome despite maintaining traditional life style patterns and adherence to a Mediterranean diet. The traits include anthropometric, blood pressure measurements, fasting plasma glucose, insulin, total cholesterol, HDL-C, LDL-C, triglyceride, uric acid, calcium, creatinine, fibrinogen and HbA1c levels. SNPs were either selected from published GWAS (SNPs with genome-wide significance in FTO, INSIG2, MC4R, SLC30A8, CDKN2A, TCF7L2, and HHEX) or tagSNPs (FTO, INSIG2, and PPARG). Three SNPs maintain significance after permutation tests and Bonferroni adjustments (p values reported here are permutation p-values multiplied by the number of traits). Two SNPs in TCF7L2, rs7903146 and rs1225536, are significantly associated with fasting plasma glucose (p=.0042 and p=.0069, respectively) and HbA1c (p=.0031 and p=.0046, respectively). These results are consistent with previous findings that variants in TCF7L2 influence Type 2 diabetes related phenotypes. Our most important finding is an association between plasma calcium concentration and rs1152005, a tagging SNP downstream of PPARG (p=.011). Calcium dysregulation in platelets, reported in patients with diabetes, causes hyperactivity and increased aggregation of platelets. While variants in PPARG have been implicated in the development of obesity and diabetes, to our knowledge, this is the first reported association between a variant in or near PPARG and plasma calcium concentration. Eight SNPs, including five previously reported with genome-wide significance, within a 45Kb region in FTO show nominal significance with BMI and waist circumference. Although their significance do not hold after multiple test adjustments, the FTO region needs further investigation given the gene being strongly implicated in obesity related traits.

We present an updated measurement of time-dependent CP-violating asymmetries in neutral B decays with the BABAR detector at the PEP-II asymmetric B Factory at SLAC. This result uses an additional sample of Upsilon(4S) decays collected in 2001, bringing the data available to 32 million B-anti-B pairs. We select events in which one neutral B meson is fully reconstructed in a final state containing charmonium and the flavor of the other neutral B meson is determined from its decay products. The amplitude of the CP-violating asymmetry, which in the Standard Model is proportional to sin2beta, is derived from the decay time distributions in such events. The result sin2beta = 0.59 +/- 0.14 (stat) +/- 0.05 (syst) establishes CP violation in the B^0 meson system. We also determine |lambda| = 0.93 +/- 0.09 {stat} +/- 0.03 {syst}, consistent with no direct CP violation., 8 pages, 2 figures, submitted to Physical Review Letters

- Abstract
We search for direct CP violation in charmless hadronic B decays observed in a sample of about 22.7 million $B\bar{B}$ pairs collected with the BABAR detector at the PEP-II asymmetric-energy $e^+e^-$ collider. We measure the following charge asymmetries: ${\cal A}_{CP} (B^{\pm}\to\eta^\prime K^{\pm}) = -0.11\pm 0.11\pm 0.02$, ${\cal A}_{CP} (B^{\pm}\to\omega \pi^{\pm}) = -0.01^{+0.29}_{-0.31}\pm 0.03$, ${\cal A}_{CP} (B^{\pm}\to\phi K^{\pm}) = -0.05\pm 0.20\pm 0.03$, ${\cal A}_{CP} (B^{\pm}\to\phi K^{*\pm}) = -0.43^{+0.36}_{-0.30}\pm 0.06$, and ${\cal A}_{CP} (B^{0}\to\phi K^{*0}) = 0.00\pm 0.27\pm 0.03$.
7. Giant Broad Ligament Leiomyoma: A Diagnostic and Surgical Challenge.

- Abstract
Broad ligament leiomyomas are rare tumors present outside the uterine body. The incidence of giant broad ligament leiomyomas (weighing over 3 kg) remains relatively rare. In this report, we present the case of a peri-menopausal woman who came in with a large abdominal mass. She was provisionally diagnosed with a right ovarian mass and planned for an exploratory laparotomy. During the procedure, a large tumor originating from the right broad ligament was seen. The tumor was occupying almost the whole of the abdominal cavity, and we faced lots of challenges to resect it without damaging the ureter and the bowel loops. This case report highlights the difficulty of diagnosing a giant broad ligament fibroid pre-operatively. 8. Acute-on-Chronic Subdural Hemorrhage Due to Late Vitamin K Deficiency.

- Abstract
Vitamin K deficiency bleeding (VKDB) is closely associated with the hemorrhagic disease of the newborn (HDN) and can have a late onset, after one week of birth up to six months of age. It is a major concern in developing countries where vitamin K prophylaxis is not often given to newborns and can lead to significant mortality and morbidity. We report a case of a three-month-old child who was exclusively breastfed. He presented with repeated vomiting and was eventually diagnosed as a case of acute-on-chronic subdural hemorrhage. 9. Synergistic Effects of Sanglifehrin-Based Cyclophilin Inhibitor NV651 with Cisplatin in Hepatocellular Carcinoma.

- Abstract
Hepatocellular carcinoma (HCC), commonly diagnosed at an advanced stage, is the most common primary liver cancer. Owing to a lack of effective HCC treatments and the commonly acquired chemoresistance, novel therapies need to be investigated. Cyclophilins-intracellular proteins with peptidyl-prolyl isomerase activity-have been shown to play a key role in therapy resistance and cell proliferation. Here, we aimed to evaluate changes in the gene expression of HCC cells caused by cyclophilin inhibition in order to explore suitable combination treatment approaches, including the use of chemoagents, such as cisplatin. Our results show that the novel cyclophilin inhibitor NV651 decreases the expression of genes involved in several pathways related to the cancer cell cycle and DNA repair. We evaluated the potential synergistic effect of NV651 in combination with other treatments used against HCC in cisplatin-sensitive cells. NV651 showed a synergistic effect in inhibiting cell proliferation, with a significant increase in intrinsic apoptosis in combination with the DNA crosslinking agent cisplatin. This combination also affected cell cycle progression and reduced the capacity of the cell to repair DNA in comparison with a single treatment with cisplatin. 10. Multiple Intracranial Meningiomas in Absence of Neurofibromatosis Type 2: A Case Report and Literature Review.

- Abstract
Meningiomas are one of the most common primary intracranial tumors known to exist since pre-historic times. Most of these tumors are benign, sporadic, and solitary. Multiple meningiomas are rare and have mostly been described in patients with neurofibromatosis type 2 (NF2). The presence of multiple lesions poses a unique challenge in strategizing the treatment. We present a rare case of multiple intracranial meningiomas in the absence of NF2, which we treated at Tata Main Hospital, Jamshedpur. - Abstract
The materials used in electrical and electronic applications have great importance and broader applications, but they have severe electromagnetic interference (EMI). These materials have extensive applications in broadcasting, medical industries, research, defence sectors, communication and similar fields. The EMI can be addressed by using effective EMI shielding materials. This review presents a detailed, comprehensive description for making electromagnetic interference shielding material by recycling various wastes. It starts with highlighting the overview of electromagnetic interference shielding (EMI) and its theoretical aspects. It provides a comprehensive and detailed understanding of recent trends in the novel approaches towards fabricating EMI shielding from industrial waste, agricultural waste and other miscellaneous wastes. This paper critically reviews the works related to the recycling of wastes like red mud (waste from the aluminium refining industry), ground tyre rubber, tea waste (biowaste) from tea industries, bagasse (waste from sugar cane industry), peanut and hazelnut shells (agricultural waste), waste tissue paper and polyethylene and other miscellaneous wastes like hydrocarbon carbon black and ash for the fabrication of highly effective electromagnetic (EM) interference shielding materials. Highly effective results have been reported using red mud showing maximum efficiency of 51.4 dB in X-band range, various agricultural waste displaying reflection loss of up to - 87.117 dB (in the range 0.01 to 20 GHz) and miscellaneous waste giving EMI SE of 80 dB in X-band frequency. Spinal epidural abscess (SEA) is a severe pyogenic infection of the epidural space. Bacteria gain access to the epidural space either by hematogenous spread, contiguous spread from a neighboring infected structure, or through iatrogenic inoculation. The diagnosis of SEA is a challenge, largely because of its rarity. 13. Intracranial en-plaque tuberculoma impersonating en-plaque meningioma: Case report and brief review of literature.

Background: Tuberculosis of the central nervous system continues to be a major health hazard in developing countries like India. There are various manifestations of central nervous system tuberculosis including meningitis and space occupying lesions. We present a case of tuberculoma en-plaque which is often initially confused with a meningioma on presentation. We also review the literature relevant to this unusual entity., Methods: A search of PubMed, PubMed Central, the Cochrane Library, and MEDLINE were performed to identify all English language reports of intracranial en-plaque tuberculomas. The combinations of text strings "tuberculoma," "en*," and "plaque" were used to perform the query on PubMed. Only the studies reporting intracranial en-plaque tuberculomas were included., Results: Literature review revealed six reports on en-plaque tuberculomas including seven patients. The mean age at presentation was 40.5 years, and no sex predilection was seen. Five of the six reported studies used anti-tuberculous therapy with or without surgical excision of the mass. Most of the patients in reported cases recovered fully., Conclusion: It is imperative to consider tuberculoma as an important differential when encountering intracranial en-plaque masses as en-plaque tuberculomas can be effectively treated with a combination of anti-tuberculosis therapy and surgical resection, yet consequences of a missed or late diagnosis can prove fatal., Competing Interests: There are no conflicts of interest.

- Abstract
Background: We have initially published our experience with the robotic transthoracic esophagectomy in 32 patients from a single institute. The present paper is the extension of our experience with robotic system and to best of our knowledge this represents the largest series of robotic transthoracic esophagectomy worldwide. The objective of this study was to investigate the feasibility of the robotic transthoracic esophagectomy for esophageal cancer in a series of patients from a single institute., Methods: A retrospective review of medical records was conducted for 83 esophageal cancer patients who underwent robotic esophagectomy at our institute from December 2009 to December 2012. All patients underwent a thorough clinical examination and pre-operative investigations. All patients underwent robotic esophageal mobilization. En-bloc dissection with lymphadenectomy was performed in all cases with preservation of Azygous vein. Relevant data were gathered from medical records., Results: The study population comprised of 50 men and 33 women with mean age of 59.18 years. The mean operative time was 204.94 mins (range 180 to 300). The mean blood loss was 86.75 ml (range 50 to 200). The mean number of lymph node yield was 18. 36 (range 13 to 24). None of the patient required conversion. The mean ICU stay and hospital stay was 1 day (range 1 to 3) and 10.37 days (range 10 to 13), respectively. A total of 16 (19.28%) complication were reported in these patents. Commonly reported complication included dysphagia, pleural effusion and anastomotic leak. No treatment related mortality was observed. After a median follow-up period of 10 months, 66 patients (79.52%) survived with disease free stage., Conclusions: We found robot-assisted thoracoscopic esophagectomy feasible in cases of esophageal cancer. The procedure allowed precise en-bloc dissection with lymphadenectomy in mediastinum with reduced operative time, blood loss and complications.

15. Quality of rusks prepared by incorporation of concentrated whey.

- Abstract
Concentrated paneer whey was utilized in place of water in the production of rusks. The studies indicated that paneer whey concentrated to 30% total solids can be used without adversely affecting the sensory attributes. The production time cycle can be reduced by incorporating ammonium phosphate at 2500 ppm which served as a nutritional supplement for the growth of yeasts and thus catalyzing fermentation process. The modified process required additional sugar incorporation to maintain the sweetness to that of market samples. The rusks prepared by the modified process had 2.5% moisture, 12.1% fat, 9.4% total protein, 1.9% ash and 74.1% total carbohydrate. Rusks with and without whey concentrate stored at 37°C for 9 days remained in good condition without significant variation in sensory attributes.

- Abstract
Background and Purpose: Although several studies have reported an association between obstructive sleep apnea (OSA) and the chromosomal region containing the Apolipoprotein E (ApoE) gene, findings about the exact location in the ApoE gene have been inconsistent. The objective of our study was thus to determine the allele, genotype, and haplotype frequencies at several single nucleotide polymorphisms (SNPs) in the region of ApoE and test their association with OSA status in children., Patients and Methods: Caucasian children, ranging in age from 2 to 21 years, with polysomnographic evidence of OSA (>1 obstructive apnea or obstructive hypopnea episodes per hour of sleep) were recruited in the case group. Our race- and gender-matched control group was recruited from a population-based cohort of children enrolled in the Princeton School District Study., Results: Comparison of allele and genotype frequencies between cases (n=92) and controls (n=92) revealed significant differences for SNPs rs405509 and rs7412. Multivariate logistic regression analysis with age and body mass index (BMI) as covariates revealed a significant association between OSA status and SNPs rs157580, rs405509, rs769455 and rs7412. The sliding window haplotype trend regression test revealed that SNP rs405509 was included in all haplotypes that are significantly associated with OSA status., Conclusions: We conclude that polymorphisms involving more than one locus in the ApoE gene and its regulatory region are associated with OSA in children. Further studies replicating these findings in different populations are needed as are studies involving fine mapping of this region.
