3. Functional outcome of floating knee injuries after fixation: A follow up study

Abstract

Floating knee is a major trauma following high velocity road traffic accidents. We assessed prospectively 25 cases of floating knee injuries that came to Govt. T D MCH casualty over a period of 1-year 6months. Majority had significant associated injuries requiring emergency interventions, multidisciplinary approach and prolonged hospital stay. Majority of cases were male patients around the age of 20-29 year, sustaining high velocity road traffic accidents. Head trauma and Upper Limb fractures were the most common associated injury. FRASER type 1(femur shaft+tibial shaft fracture) was the most common fracture encountered. Zero hour/early fixation seemed to significantly improve post-operative morbidity and less complications. Closed nailing of both tibia and femur were found to be the most successful surgery as evaluated by karlstorm and olerud criteria. On assessment by karlstorm and olerud criteria around 44% had excellent outcome, 32% good, 16% acceptable and 8% poor.

Abstract

Chronic osteomyletis is one of the most dreaded complication of 0pen fractures and a surgeons nightmare. Eradication of COM requires aggressive surgical excision combined with effective antibiotic treatment and reconstructive procedures. The goal of therapy in such patients is to restore the functional anatomy of the limb. Depending on our experience and the type of infected nonunion we individualize a modality of management for the patient and the type of nonunion. The various modalities used were Corticotomy and lengthening using Ilizarov or LRS, Customized antibiotic rod implantation, Membrane osteosynthesis, Resection and endoprosthesis and Papineau technique. We followed up these patients till the radiological and clinical union occured and eradication of infection. The outcomes were based on bony union, functional outcome based on range of motion and occupational rehabilitation with eradication of infection at the last follow up. We publish our series of such cases and review of literature with representative cases.

Abstract

The objective of this study was to classify the different clinical scenarios in ankle fractures, looking at their mechanism of injury, relevant to the mode of treatment and to determine the functional outcome and role of fixation of the fibula. Out of forty patients treated, overall good to excellent results were obtained in 36 patients. The results were fair and poor in two patients each. There were no intraoperative complications. The only postoperative complication seen was superficial skin infection in eight patients.

Abstract

© 2020 Elsevier Ltd Background: Observational studies have suggested that accelerated surgery is associated with improved outcomes in patients with a hip fracture. The HIP ATTACK trial assessed whether accelerated surgery could reduce mortality and major complications. Methods: HIP ATTACK was an international, randomised, controlled trial done at 69 hospitals in 17 countries. Patients with a hip fracture that required surgery and were aged 45 years or older were eligible. Research personnel randomly assigned patients (1:1) through a central computerised randomisation system using randomly varying block sizes to either accelerated surgery (goal of surgery within 6 h of diagnosis) or standard care. The coprimary outcomes were mortality and a composite of major complications (ie, mortality and non-fatal myocardial infarction, stroke, venous thromboembolism, sepsis, pneumonia, life-threatening bleeding, and major bleeding) at 90 days after randomisation. Patients, health-care providers, and study staff were aware of treatment assignment, but outcome adjudicators were masked to treatment allocation. Patients were analysed according to the intention-to-treat principle. This study is registered at ClinicalTrials.gov (NCT02027896). Findings: Between March 14, 2014, and May 24, 2019, 27 701 patients were screened, of whom 7780 were eligible. 2970 of these were enrolled and randomly assigned to receive accelerated surgery (n=1487) or standard care (n=1483). Abstract

Callus progression is a great concern during limb lengthening. In this study, we investigated the difference in callus progression between tibial lengthenings with and without intramedullary nail.Seventy tibiae in 38 patients with an average age of 24 years were lengthened with Ilizarov external fixator and nail; 56 tibiae in 40 patients with an average age of 28.6 years were lengthened with the same Ilizarov external fixator but without nail. The callus progression was compared with reference to pixel value ratio (PVR) and Ru Li's classification. Statistical analysis was performed to compare the variation trend of PVR and incidence of various callus pathways (particular patterns of callus progression as outlined in Ru Li's classification) and shapes of each aspect of callus between the two groups.The trend of PVR was not statistically different in posterior, lateral and medial aspects of the callus between the two groups, but averagely lower in the anterior aspect in the group without nail than that with nail. The group without nail presented less incidence of homogeneous pathway, greater incidence of heterogeneous pathway; also greater incidence of fusiform callus, less incidence of cylindrical callus.It was concluded that with nail, the callus underwent a more favorable progression and even longer lengthening could be allowed.
