Abstract

Abstract Background Triglyceride-glucose index (TyG), a surrogate marker of insulin resistance (IR), could be a potential prognostic marker in patients with acute coronary syndromes (ACS). We evaluated the effect of the TyG index on major adverse cardiac and cerebrovascular events (MACCE) in patients with ACS undergoing percutaneous coronary intervention (PCI). Methods This registry-based cohort study was conducted at Tehran Heart Center from 2015 to 2021 and the median follow-up duration was 378 days. The primary outcome was MACCE and the secondary outcomes were MACCE components: all-cause mortality, myocardial infarction, stroke, target vessel revascularization, target lesion revascularization, and coronary artery bypass grafting. For comparison among TyG quartiles (Q), the log-rank test was used. Unadjusted and adjusted hazard ratios (HRs) and their 95% confidence intervals (CIs) were used to describe the association between TyG quartiles and MACCE. A subgroup of euglycemic patients was also evaluated. Results A total of 13,542 patients were included. Patients in the fourth TyG quartile (Q4) were younger, had higher mean BMI, and higher prevalence of hypertension, diabetes, and dyslipidemia. Abstract

Purpose: Retinopathy of prematurity (ROP) stage is defined by the visual appearance of the vascular-avascular border, which reflects a spectrum of pathologic neurovascular tissue (NVT). Previous work demonstrated that the thickness of the ridge lesion, measured using OCT, corresponds to higher clinical diagnosis of stage. This study evaluates whether the volume of anomalous NVT (ANVTV), defined as abnormal tissue protruding from the regular contour of the retina, can be measured automatically using deep learning to develop quantitative OCT-based biomarkers in ROP. Design: Single-center retrospective case series. Participants: Thirty-three infants with ROP in the Oregon Health & Science University neonatal intensive care unit. Methods: OCT B-scans were collected using an investigational ultrawidefield OCT. The ANVTV was manually segmented. A set of 3347 B-scans and corresponding manual segmentations from 12 volumes from 6 patients were used to train an automated segmentation tool using a U-Net. An additional held-out test data set of 60 B-scans from 6 infants was used to evaluate model performance. The Dice–Sorensen coefficient (DSC) comparing manual and automated segmentation of ANVTV was calculated. Scans from 21 additional infants were used for clinical evaluation of ANVTV using the visit in which they had developed their peak stage of ROP. Each infant had every B-scan in a volume automatically segmented for ANVTV (total number of segmented voxels within the 60° temporal to the optic disc). The ANVTV was compared between infants with stage 1 to 3 ROP using a Kruskal–Wallis test and tracked over time in all infants with stage 3 ROP. Main Outcome Measurements: Cross sectional and longitudinal association between ANVTV and stages 1 to 3 ROP. Results: Comparing automated and manual segmentation of ANVTV achieved a DSC of 0.61 ± 0.13. Using the U-Net, ANVTV was associated with higher disease stage both cross sectionally and longitudinally. Median ANVTV significantly increased as ROP stage worsened from 1 (0, [interquartile range: 0–0] kilovoxels) to 2 (170.1 [interquartile range: 104.2–183.6] kilovoxels) to 3 (421.4 [interquartile range: 312.3–1110.8] kilovoxels; P < 0.001). Conclusions: Automated OCT-based measurement of ANVTV was associated with clinical disease stage in ROP, both cross sectionally and longitudinally. Ultrawidefield-OCT may facilitate more objective screening, diagnosis, and monitoring in the future. 3. Association between sleep duration and hypertension incidence: Systematic review and meta-analysis of cohort studies.

Abstract

AimSleep duration has been suggested to be associated with hypertension (HTN). However, evidence of the nature of the relationship and its direction has been inconsistent. Therefore, we performed a meta-analysis to assess the relationship between sleep duration and risk of HTN incidence, and to distinguish more susceptible populations.MethodsPubMed, Embase, Scopus, Web of Science, and ProQuest were searched from January 2000 to May 2023 for cohort studies comparing short and long sleep durations with 7-8 hours of sleep for the risk of HTN incidence. Random-effect model (the DerSimonian-Laird method) was applied to pool risk ratios (RR) and 95% confidence interval (CI).ResultsWe included sixteen studies ranging from 2.4 to 18 years of follow-up duration evaluating HTN incidence in 1,044,035 people. Short sleep duration was significantly associated with a higher risk of developing HTN (HR: 1.07, 95% CI: 1.06-1.09). The association was stronger when the sleep duration was less than 5 hours (HR: 1.11, 95% CI: 1.08-1.14). In contrast to males, females (HR: 1.07, 95% CI: 1.04-1.09) were more vulnerable to developing HTN due to short sleep duration. No significant difference between different follow-up durations and age subgroups was observed. Long sleep duration was not associated with an increased incidence of HTN.ConclusionShort sleep duration was associated with higher risk of HTN incidence, however, there was no association between long sleep duration and incidence of HTN. These findings highlight the importance of implementing target-specific preventive and interventional strategies for vulnerable populations with short sleep duration to reduce the risk of HTN.

Abstract

Moisture management is a critical feature of any sportswear, as it improves the fabric's comfort level. Comfort differs from person to person depending upon the perception of the wearer. Comfort is the relation between a psychological and physiological behavior of the wearer. The moisture transmission of the fabric is the key determinant of thermophysiological comfort. The moisture management features of trilayer knitted fabrics constructed of a range of yarns on the inner and outer sides, including micro denier filament polyester, Monofilament, wool, bamboo, and cotton, are examined in this article. The sample B/MDPF/C, which included an inner layer of bamboo, a middle layer of Micro denier Filament polyester, and an outer layer of cotton shows appreciable moisture management property, providing an excellent comfort nature due to its appreciable property required for a moisture management fabric.

Abstract

Objective: Management of follow-up protocols after endovascular aortic repair (EVAR) varies significantly between centers and is not standardized according to sac regression. By designing an international expert-based Delphi consensus, the study aimed to create recommendations on follow-up after EVAR according to sac evolution. Methods: Eight facilitators created appropriate statements regarding the study topic that were voted, using a 4-point Likert scale, by a selected panel of international experts using a three-round modified Delphi consensus process. Based on the experts' responses, only those statements reaching a grade A (full agreement ≥75%) or B (overall agreement ≥80% and full disagreement <5%) were included in the final document. Results: One-hundred and seventy-four participants were included in the final analysis, and each voted the initial 29 statements related to the definition of sac regression (Q1-Q9), EVAR follow-up (Q10-Q14), and the assessment and role of sac regression during follow-up (Q15-Q29). At the end of the process, 2 statements (6.9%) were rejected, 9 statements (31%) received a grade B consensus strength, and 18 (62.1%) reached a grade A consensus strength. Of 27 final statements, 15 (55.6%) were classified as grade I, whereas 12 (44.4%) were classified as grade II. Experts agreed that sac regression should be considered an important indicator of EVAR success and always be assessed during follow-up after EVAR. Conclusions: Based on the elevated strength and high consistency of this international expert-based Delphi consensus, most of the statements might guide the current clinical management of follow-up after EVAR according to the sac regression. Future studies are needed to clarify debated issues.

Abstract

Background: It remains controversial whether adding ezetimibe to low/moderate‐intensity statins has a more beneficial impact on the treatment efficacy and safety of patients with existing atherosclerotic cardiovascular disease (ASCVD) compared to high‐intensity statin regimens. Hypothesis: A combination of low/moderate‐intensity statins plus ezetimibe might be more effective and safer than high‐intensity statin monotherapy. Methods: We searched databases for randomized controlled trials comparing lipid profile alterations, drug‐related adverse events, and MACE components between high‐intensity statin monotherapy and low/moderate‐intensity statin plus ezetimibe combination therapy. Pooled risk ratios (RR), mean differences (MD), and 95% confidence intervals (95% CI) were estimated using a random‐effects model. Results: Our comprehensive search resulted in 32 studies comprising 6162 patients treated with monotherapy against 5880 patients on combination therapy. Combination therapy was more effective in reducing low‐density lipoprotein cholesterol (LDL‐C) levels compared to monotherapy (MD = −6.6, 95% CI: −10.6 to −2.5); however, no significant differences were observed in other lipid parameters. Furthermore, the combination therapy group experienced a lower risk of myalgia (RR = 0.27, 95% CI: 0.13–0.57) and discontinuation due to adverse events (RR = 0.61, 95% CI: 0.51–0.74). The occurrence of MACE was similar between the two treatment groups. Conclusions: Adding ezetimibe to low/moderate‐intensity statins resulted in a greater reduction in LDL‐C levels, a lower rate of myalgia, and less drug discontinuation compared to high‐intensity statin monotherapy in patients with existing cardiovascular disease. However, according to our meta‐analysis, the observed reduction in LDL‐C levels in the combination group did not correlate with a reduction in MACE compared to the high‐intensity statin group. Summary: Combination therapy of low/moderate‐intensity statin with ezetimibe led to a greater reduction in LDL‐C levels, a lower rate of myalgia, and less drug discontinuation compared to high‐intensity statin monotherapy, but no significant effect on MACE components. Abstract

Cardiovascular diseases, particularly coronary artery disease, pose big challenges to human life. Deployment of the stent is a preferable treatment for the above-mentioned disease. However, stents are usually made up of shape memory alloy called Nitinol. The poorer surface finish on the machined nitinol stents accelerates the migration of Nickel ions from the implanted nitinol stent, which is considered toxic and can lead to stenosis. The current study deals with controlling surface quality by minimising surface roughness and improving corrosion resistance. Femtosecond laser (fs-laser 10-15 s) micromachining was employed to machine the Nitinol surface to achieve sub-micron surface roughness. The Grey relational analysis (GRA)-coupled design of the experimental technique was implemented to determine optimal levels of four micromachining parameters (laser power, pulse frequency, scanning speed, and scanning pattern) varied at three levels to achieve minimum surface roughness and to maximise the volume ablation. The results show that to yield minimum surface roughness and maximum volume ablation, laser power and scanning speed are in a higher range. In contrast, the pulse frequency is lower, and the scanning pattern is in a zig-zag manner. ANOVA results manifest that scanning speed is the predominant factor in minimising surface roughness, followed by pulse frequency. Furthermore, the corrosion behaviour of the machined nitinol specimens was evaluated, and the results show that specimens with lower surface roughness had lower corrosion rates.

26. Association between sleep duration and hypertension incidence: Systematic review and meta-analysis of cohort studies.

Abstract

Aim: Sleep duration has been suggested to be associated with hypertension (HTN). However, evidence of the nature of the relationship and its direction has been inconsistent. Therefore, we performed a meta-analysis to assess the relationship between sleep duration and risk of HTN incidence, and to distinguish more susceptible populations. Methods: PubMed, Embase, Scopus, Web of Science, and ProQuest were searched from January 2000 to May 2023 for cohort studies comparing short and long sleep durations with 7–8 hours of sleep for the risk of HTN incidence. Random-effect model (the DerSimonian-Laird method) was applied to pool risk ratios (RR) and 95% confidence interval (CI). Results: We included sixteen studies ranging from 2.4 to 18 years of follow-up duration evaluating HTN incidence in 1,044,035 people. Short sleep duration was significantly associated with a higher risk of developing HTN (HR: 1.07, 95% CI: 1.06–1.09). The association was stronger when the sleep duration was less than 5 hours (HR: 1.11, 95% CI: 1.08–1.14). In contrast to males, females (HR: 1.07, 95% CI: 1.04–1.09) were more vulnerable to developing HTN due to short sleep duration. No significant difference between different follow-up durations and age subgroups was observed. Long sleep duration was not associated with an increased incidence of HTN. Conclusion: Short sleep duration was associated with higher risk of HTN incidence, however, there was no association between long sleep duration and incidence of HTN. These findings highlight the importance of implementing target-specific preventive and interventional strategies for vulnerable populations with short sleep duration to reduce the risk of HTN. 40. Antimicrobial, UV-repellent and comfort properties of various herbal treated fabrics.

Abstract

Aloe vera, Adhathoda vasica, Agele marmelos, Calendula officinalis, and Solanum trilobatum herbal extracts have been used to treat cotton fabric using the pad-dry cure technique. Three distinct herbal treatments have been used on the fabrics, each of which has its unique therapeutic properties which is more suited to older individuals. The study focuses on developing herbal finished fabrics for old age people to prevent them from microbes and UV rays, and to increase the comfort property. Air permeability, wickability, absorbency, thermal conductivity, tensile strength anti-microbial and UV repellence properties have been studied. It is found that the finished fabrics show moderately good UV repellent protection and has antimicrobial creativity. [ABSTRACT FROM AUTHOR]
