A class of planar antenna which offers better performance over broad band of frequencies that fits itself in wide range of applications are in need of research. The license free band from 3.1 GHz to 10.6 GHz that do not cause interference to narrowband signals is chosen as operating range of frequencies for the chosen Vivaldi antenna. The novelty of the proposed work consists in the remodeling of a mathematical equations for elliptic antenna structure and the study of the Vivaldi antenna operating over the frequency range between 2.63 GHz to 14.58 GHz that lies in Super High Frequency (SHF) bands – S, C and X bands. The designed antenna is fabricated and its radiation characteristics are studied and analyzed. Purpose. Analysis of the designed antenna on the basis of an elliptical curve mathematical model will allow the antenna to operate over wide band of frequencies thus extending its applications over the areas of application in the areas of satellite communication, military, weather radar, surface ship radar. Methods. The antenna is being designed using HFSS software and fabricated antenna is being tested at anechoic chamber. Results. A mathematical model of a structure of Vivaldi antenna was developed and studied. The performance measures of designed antenna such as return loss, gain, VSWR and radiation pattern are simulated and analyzed. The antenna is being fabricated and tested using vector network analyzer and anechoic chamber. Practical value. The analysis of designed antenna over wide band of frequencies is demonstrated. The further research directions of the designed antenna can be extended for the subsequent implementation of the results in suitable applications.

For comprehensive management of paediatric bipolar disorder (PBD), it is imperative to combine psychopharmacotherapy with specific psychotherapy. This article proposes a model that incorporates (1) an overview of psychopathology, (2) a review of outcomes in psychopharmacotherapy trials, and (3) a summary of evidence-based forms of psychotherapy to complement pharmacotherapy. Results: The psychopathology of PBD is unique compared to that of adult bipolar disorder with prominent irritability, rapid cycling, high rates of co-morbid attention deficit hyperactivity disorder, mixed episodes and chronicity. Combination therapy with a second generation antipsychotic and a mood stabilizer is proving to be more effective than monotherapy with a mood stabilizer. Empirical findings for the support of family-focused, cognitive behavioral therapies with individual family or multifamily psychoeducation groups suggest that these psychosocial treatments are valuable complementary tools for clinicians who treat youths diagnosed with PBD. Conclusion: As pharmacotherapy and psychotherapy are most beneficial when applied together, the clinician’s understanding of the science behind these forms of treatment is likely to be of great value in effectively providing services to youths diagnosed with PBD.

Aim: To compare the efficacy of fine-needle non-aspiration cytology (FNNAC) with that of fine-needle aspiration cytology (FNAC) of thyroid lesions. Materials and Methods: FNAC and FNNAC techniques were studied in 50 cases of thyroid lesions. All the needle-sampling procedures were done by a single operator. The samples were assessed cytologically and evaluated using five parameters, that is, background blood or clot, amount of cellular material, degree of cellular degeneration, and degree of cellular trauma and retention of appropriate architecture. Statistical Analysis: Wilcoxon signed rank test was performed using SPSS14 software. Differences between all the individual parameters as observed in FNAC and FNNAC smears were insignificant. Results and Conclusion: After evaluation of FNAC and FNNAC on the basis of these scores, greater numbers of diagnostically superior samples were obtained by FNNAC; however, by FNAC more number of diagnostically adequate smears were observed. The numbers of unsuitable smears were also more by FNNAC technique.

It has been studied as a monotherapy treatment in early onset schizophrenia and as both monotherapy and combination therapy for pediatric bipolar disorder. Studies to date indicate that risperidone is an effective treatment for positive and negative symptoms of schizophrenia and mania symptoms of bipolar disorder. In young patient populations, side effects such as weight gain, extrapyramidal side effects, and prolactin elevation require consideration when evaluating the risk benefit ratio for individual patients. Here we review published studies of risperidone for the treatment of bipolar disorder and schizophrenia in children and adolescents to provide practitioners with an overview of published data on the efficacy and safety of risperidone in these patient populations.Keywords: risperidone, bipolar disorder, schizophrenia, children, adolescents

Background and Objective: Argyrophilic nucleolar organizer regions (AgNORs) have found widespread application in the past, especially in tumor histopathology. This study was undertaken to evaluate the significance of various AgNOR parameters and to assess their role in differentiating hyperplastic, premalignant, and malignant lesions. Materials and Methods: The study sample consisted of archival biopsy specimens of ten squamous cell carcinomas, ten premalignant lesions, and five inflammatory lesions. Two biopsies from normal mucosa acted as control. AgNORs were assessed both quantitatively and qualitatively. The data were analyzed using Student′s independent t-test, one-way analysis of variance (ANOVA), and multiple range test (Tukey-HSD). Results: Quantitatively significant difference existed in the number of AgNORs between the normal mucosa, inflammatory lesions, and carcinomas, but the premalignant lesions failed to differ significantly from the normal mucosa. The number of AgNORs was found to be related to epithelial proliferation. Qualitatively, in terms of size, shape, and pattern of distribution, the normal mucosa and inflammatory lesion were alike, but the premalignant and malignant lesions differed significantly from the normal, with a marked degree of AgNOR pleomorphism being observed in carcinomas. Conclusions: AgNOR quantity is strictly proportional to the proliferative activity of the cell and does not necessarily indicate malignancy. It is the qualitative characteristics of AgNOR that help to differentiate hyperplastic, premalignant, and malignant lesions.

Objectives: Identifying early markers of brain function among those at high risk for pediatric bipolar disorder (PBD) could serve as a screening measure when children and adolescents present with sub-syndromal clinical symptoms prior to the conversion to bipolar disorder. Studies on the offspring of patients with bipolar disorder who are genetically at high risk (HR) have each been limited in establishing a biomarker, while an analytic review in summarizing the findings offers an improvised opportunity towards that goal. Methods: An activation likelihood estimation meta-analysis of mixed cognitive and emotional activities using the GingerALE software from the BrainMap Project was completed. The meta-analysis of all fMRI studies contained a total of 29 reports and included PBD, HR and typically developing (TD) groups.Results: The HR group showed significantly greater activation relative to the TD group in the right DLPFC-insular-parietal-cerebellar regions. Similarly, the HR group exhibited greater activity in the right DLPFC and insula as well as the left cerebellum compared to patients with PBD. Patients with PBD, relative to TD, showed greater activation in regions of the right amygdala, parahippocampal gyrus, medial PFC, left ventral striatum, and cerebellum and lower activation in the right VLPFC and the DLPFC.Conclusions: The HR population showed increased activity, presumably indicating greater compensatory deployment, in relation to both the TD and the PBD, in the key cognition and emotion processing regions, such as the DLPFC, insula and parietal cortex. In contrast, patients with PBD, relative to HR and TD, showed decreased activity, which could indicate a decreased effort in multiple PFC regions in addition to widespread subcortical abnormalities, which are suggestive of a more entrenched disease process.

Pulmonary carcinosarcoma is an aggressive rare malignant tumor comprising a mixture of carcinoma and sarcoma components containing differentiated mesenchymal elements, such as malignant cartilage, bone, and skeletal muscle. We report a case which presented with unusual clinical features and proved cryptic until death. At autopsy, it was a stage IV lung malignancy and histopathology revealed a carcinosarcoma comprising an adenocarcinoma and an osteosarcoma with metastasis to the heart, lymph nodes, and both adrenals. To our knowledge, this is the first case of this subtype with metastasis to the heart. The present case had an unusual clinical presentation and its elusive nature towards diagnosis despite dissemination is noteworthy and unique in the literature.

This retrospective analysis examines the local control and toxicity of 5 fraction fiducial-free CyberKnife stereotactic body radiation therapy (SBRT) for single vertebral body metastases. All patients had favorable performance status (ECOG 0-1), oligometastatic disease and no prior spine irradiation. A prescribed dose of 30 to 35 Gy was delivered in 5 fractions to the planning treatment volume (PTV) using the CyberKnife with X-sight spine tracking. Suggested maximum spinal cord and esophagus point doses were 30 Gy and 40 Gy, respectively. A median 30 Gy (range, 30 - 35 Gy) dose was delivered to a median prescription isodose line of 70% (range, 60 - 80%) to 20 patients. At 34 months median follow-up (range, 12-62 months) for surviving patients, the 1- and 2-year Kaplan-Meier local control estimates were 80% and 73%, respectively. Two of the 5 local failures were infield in patients who had received partial vertebral body irradiation and 3 were paravertebral failures just outside the PTV in patients with prior corpectomy. No local failures occurred in patients who completed total vertebral body radiation alone. The 1- and 2-year Kaplan-Meier overall survival estimates were 80% and 57%, respectively. Most deaths were attributed to metastatic disease; one death was attributed to local recurrence. The mean maximum point doses were 26.4 Gy (range, 16.3 - 31.8 Gy) to the spinal cord and 30.7 Gy (15.4- 40.0 Gy) to the esophagus. Patients receiving maximum esophagus point doses greater than 35 Gy experienced acute dysphagia (Grade I/II). No spinal cord toxicity was documented. Five fraction fiducial-free CyberKnife SBRT is an acceptable treatment option for newly diagnosed vertebral body metastases with promising local control rates and minimal toxicity despite the close proximity of such tumors to the spinal cord and esophagus. A prospective study aimed at further enhancing local control by targeting the entire intact vertebral body and escalating the total dose is planned

Introduction: The 2016 World Health Organisation (WHO) classification of tumours of the Central Nervous System (WHO CNS 2016 classification) is used to classify diffuse gliomas as astrocytoma, Oligodendroglioma (ODG), glioblastoma which are three prognostically distinct groups based on Isocitrate Dehydrogenase (IDH1), alpha thalassaemia/mental retardation, x-linked (ATRX) mutations, p53 and 1p/19q co-deletion status. Although WHO CNS 2022 classification has been brought in use, it is based on molecular studies. In a resource limited setting like in many Indian diagnostic centres it's difficult to apply the WHO CNS 2022 classification. It is felt that WHO CNS 2016 classification has not lost its utility. Aim: To investigate the Immunohistochemistry (IHC) status of IDH1, ATRX, p53 in diagnosis of diffuse glial tumours and to classify them according to WHO 2016. Materials and Methods: The present cross-sectional study was conducted at Department of Pathology, Bharati Vidyapeeth Medical College, Hospital and Research Centre, Pune, Maharashtra, India for two years and six months (July 2020 to December 2022). Thirty-two diffuse glioma cases and IHC markers IDH1, ATRX, p53 were evaluated. Ki-67 index was additionally done. Results: Total 32 cases were studied, 19 cases were male. Mean age of the patients was 40.13 years. Fourteen patients belonged to WHO Grade-II, six to Grade-III, and 12 to Grade-IV. As per the IHC findings and histopathological features, there were 16 (50%) patients with diffuse astrocytoma, while 12 (37.5%) and 4 (12.5%) patients were diagnosed as glioblastoma and ODG, respectively. Reclassification of these cases was done depending on IHC results where IDH1 was positive in 71.9% cases, ATRX was positive in 40.6% cases and p53 was positive in 15.6% cases. This result includes all the cases where these IHC markers showed reactivity. The diagnosis of four patients was modified based on findings of IHC markers. Conclusion: The study demonstrates subgrouping of gliomas based on IDH1, ATRX, p53. There was no significant association between grade of tumour and Ki-67 expression. [ABSTRACT FROM AUTHOR]
