13. Polytraumaversorgung auf der Intensivstation

Abstract

Zusammenfassung: Die intensivmedizinische Behandlung polytraumatisierter Patienten gehört zu den größten Herausforderungen der Medizin. Bereits im frühen Verlauf muss engmaschig eine suffiziente Reevaluation der klinischen und apparativen Befunde in einem geschulten interdisziplinären Team erfolgen („tertiary trauma survey“), um die teils deletären Folgen von in der Erstdiagnostik im Schockraum übersehenen Verletzungen („missed injuries“) zu verhindern. Während des im Mittel 11-tägigen Aufenthalts der Patienten auf der Intensivstation muss dann eine Therapiestrategie gewählt werden, die neben einer qualitativ hochwertigen allgemeinen Intensivmedizin insbesondere die Spezifika des individuell vorliegenden Verletzungsmusters berücksichtigt. Da sich bei schwer verletzten Patienten zunehmend das Konzept der „damage control surgery“ durchsetzt, ist die Intensivstation häufig Dreh- und Angelpunkt in der kritischen Zeit zwischen der operativen Erstversorgung und frühelektiven Eingriffen. Daher ist eine enge Abstimmung zwischen dem Intensivmediziner und den beteiligten operativen Disziplinen zum Erreichen des gemeinsamen Ziels aller therapeutischen Bemühungen unabdingbar: Der polytraumatisierte Patient soll nach Überleben der Akutphase rasch rehabilitiert und wieder ins Berufs- sowie Sozialleben eingegliedert werden können.

16. ETS transcription factor ELF3 (ESE-1) is a cell cycle regulator in benign and malignant prostate

Abstract

This study aimed to elucidate the role of ELF3, an ETS family member in normal prostate growth and prostate cancer. Silencing ELF3 in both benign prostate (BPH-1) and prostate cancer (PC3) cell lines resulted in decreased colony forming ability, inhibition of cell migration and reduced cell viability due to cell cycle arrest, establishing ELF3 as a cell cycle regulator. Increased ELF3 expression in more advanced prostate tumours was shown by immunostaining of tissue microarrays and from analysis of gene expression and genetic alteration studies. This study indicates that ELF3 functions as part of normal prostate epithelial growth but also as a potential oncogene in advanced prostate cancers.

Abstract

Background: In recent years, the rapidly advancing field of low-temperature atmospheric pressure plasmas has shown considerable promise for future translational biomedical applications, including cancer therapy, through the generation of reactive oxygen and nitrogen species. Method: The cytopathic effect of low-temperature plasma was first verified in two commonly used prostate cell lines: BPH-1 and PC-3 cells. The study was then extended to analyse the effects in paired normal and tumour (Gleason grade 7) prostate epithelial cells cultured directly from patient tissue. Hydrogen peroxide (H2O2) and staurosporine were used as controls throughout. Results: Low-temperature plasma (LTP) exposure resulted in high levels of DNA damage, a reduction in cell viability, and colony-forming ability. H2O2 formed in the culture medium was a likely facilitator of these effects. Necrosis and autophagy were recorded in primary cells, whereas cell lines exhibited apoptosis and necrosis. Conclusions: This study demonstrates that LTP treatment causes cytotoxic insult in primary prostate cells, leading to rapid necrotic cell death. It also highlights the need to study primary cultures in order to gain more realistic insight into patient response.
