Kamdar [2]; Sharad Narayan Shenoy [2] INTRODUCTION The prevalence of residual or missed common bile duct (CBD) stones post-treatment is a [...], Introduction: Endoscopic retrograde cholangiopancreatography (ERCP) facilitates the removal of common bile duct (CBD) calculi by endoscopy. When ERCP fails, exploration of CBD is required for the clearance of CBD calculi. The optimum way for the exploration of CBD is by choledochoscopy. Dedicated flexible or rigid choledochoscopes are expensive and available only in few places in India. Since 1991, we subjected patients with suspected CBD calculi to ERCP, followed by laparoscopic cholecystectomy (LC). Patients in whom ERCP failed to clear CBD were subjected to open exploration of CBD using any easily available, suitable, straight rigid scope for choledochoscopy. Patients and Methods: Since March 1991, out of 8866 patients with cholelithiasis, 862 underwent ERCP. Ninety-six patients in whom ERCP failed to clear CBD underwent open exploration of CBD. In each case of exploration of CBD, choledochoscopy was performed using a straight rigid scope, either a cystoscope, paediatric cystoscope, hysteroscope or 5-mm laparoscopy telescope with a 5-mm cannula. Results: The CBD clearance was complete in 95 patients, and one patient had an impacted calculus at the ampulla. CBD explorations were followed by choledochoduodenostomy, T-tube placement or suturing of choledochotomy. No residual calculi were observed after such exploration. Conclusion: From our results, we advocate the following algorithm for CBD calculi in resource-limited settings. Subject patients with CBD calculi to ERCP followed by LC. In case of failed ERCP, open exploration of CBD with choledochoscopy using any suitable rigid scope. Dedicated flexible or rigid choledochoscope is not essential. The proposed methodology leverages transfer learning with convolutional neural networks (CNNs) for automatic DR detection using a single fundus photograph, demonstrating high effectiveness with a quadratic weighted kappa score of 0.92546 in the APTOS 2019 Blindness Detection Competition. The paper reviews existing literature on DR detection, spanning classical computer vision methods to deep learning approaches, particularly focusing on CNNs. Abstract

Antimicrobial Stewardship Programme (ASP) is an organizational or system-wide health-care strategy that promotes optimal antibiotic use through the implementation of evidence-based interventions. The Indian Council of Medical Research (ICMR) issued the ASP guidelines in 2018, that are applicable for reducing antimicrobial resistance (AMR) effects. In comparison with older children, neonates receive broad spectrum antimicrobials more frequently. As neonates are having high risk of morbidity and mortality, so they are more susceptible to early upgradation of antibiotic. Antimicrobial use in terms of daily dose, cost, and treatment days, antimicrobial adverse events, resistance tendencies, intervention, and monitoring can all be utilized to assess an ASP's progress. An effective antimicrobial stewardship programme requires the presence of at least one infectious diseases-trained physician who devotes a portion of their time to the program's design, implementation, and operation. A pharmacist's most critical duties and responsibilities include developing, reviewing, and implementing antimicrobial stewardship (AMS) guidelines and policies, providing clinical guidance to optimise antimicrobial prescribing and usage, monitoring, auditing, and providing feedback. Clinical microbiology services are an essential component of successful AMS systems. Challenges in implementing Stewardship program are competition among doctors, absence of champion, utility value of an antibiogram, poor enforcement of regulations, time constraints, lack of inter-departmental co-ordination, minimal supporting facilities. A combination of particular antibodies, organism-specific bacteriophages, and small molecules might be used in future therapies that inhibit specific virulence factors, and antibiotic resistance-fighting drugs.

Abstract

The global emergence of many severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) variants jeopardizes the protective antiviral immunity induced after infection or vaccination. To address the public health threat caused by the increasing SARS-CoV-2 genomic diversity, the National Institute of Allergy and Infectious Diseases within the National Institutes of Health established the SARS-CoV-2 Assessment of Viral Evolution (SAVE) programme. This effort was designed to provide a real-time risk assessment of SARS-CoV-2 variants that could potentially affect the transmission, virulence, and resistance to infection- and vaccine-induced immunity. The SAVE programme is a critical data-generating component of the US Government SARS-CoV-2 Interagency Group to assess implications of SARS-CoV-2 variants on diagnostics, vaccines and therapeutics, and for communicating public health risk. Here we describe the coordinated approach used to identify and curate data about emerging variants, their impact on immunity and effects on vaccine protection using animal models. We report the development of reagents, methodologies, models and notable findings facilitated by this collaborative approach and identify future challenges. This programme is a template for the response to rapidly evolving pathogens with pandemic potential by monitoring viral evolution in the human population to identify variants that could reduce the effectiveness of countermeasures.

Abstract

Abstract Prophylactic vaccines for SARS-CoV-2 have lowered the incidence of severe COVID-19, but emergence of viral variants that are antigenically distinct from the vaccine strains are of concern and additional, broadly acting preventive approaches are desirable. Here, we report on a glycolipid termed 7DW8-5 that exploits the host innate immune system to enable rapid control of viral infections in vivo. This glycolipid binds to CD1d on antigen-presenting cells and thereby stimulates NKT cells to release a cascade of cytokines and chemokines. The intranasal administration of 7DW8-5 prior to virus exposure significantly blocked infection by three different authentic variants of SARS-CoV-2, as well as by respiratory syncytial virus and influenza virus, in mice or hamsters. We also found that this protective antiviral effect is both host-directed and mechanism-specific, requiring both the CD1d molecule and interferon- $$\gamma$$ γ . A chemical compound like 7DW8-5 that is easy to administer and cheap to manufacture may be useful not only in slowing the spread of COVID-19 but also in responding to future pandemics long before vaccines or drugs are developed.

Abstract

Recent advancements in artificial intelligence (AI) have significantly impacted the diagnosis and treatment of kidney diseases, offering novel approaches for precise quantitative assessments of nephropathology. The collaboration between computer engineers, renal specialists, and nephropathologists has led to the development of AI- assisted technology, presenting promising avenues for renal pathology diagnoses, disease prediction, treatment effectiveness assessment, and outcome prediction. This review provides a comprehensive overview of AI applications in renal pathology, focusing on computer vision algorithms for kidney structure segmentation, specific pathological changes, diagnosis, treatment, and prognosis prediction based on images along with the role of machine learning (ML) and deep learning (DL) in addressing global public health issues related to various nephrological conditions. Despite the transformative potential, the review acknowledges challenges such as data privacy, interpretability of AI models, the imperative need for trust in AI-driven recommendations for broad applicability, external validation, and improved clinical decision-making. Overall, the ongoing integration of AI technologies in nephrology paves the newer way for more precise diagnostics, personalized treatments, and improved patient care outcome.

Abstract

Diabetic nephropathy (DN) has become an utmost reason for long-standing renal dysfunction and end-stage renal disease globally. Oxidative stress induced by persistent hyperglycemia is considered a fundamental element in the evolution of DN. The goal of this research was to discover the outcome of appendages of natural antioxidants such as lycopene and co-enzyme Q10 (CoQ10) in DN rats and to observe the preventive effects in DN. A diabetes model was developed in a Wistar strain of male rats (200–250 g) by subcutaneous injection of streptozotocin (55 mg/kg). Development of nephropathy was assessed by renal function tests including blood glucose, creatinine, albumin, total protein, total bilirubin, uric acid, total cholesterol, triglycerides, and CRP level. Oxidative stress markers such as LPO and GSH content and activity of membrane-bound Na+/K+ ATPases were measured in kidney homogenate. Renal damage was assessed by performing a histopathological analysis. DN rats showed a significant elevation in creatinine, albumin, total protein, total bilirubin, uric acid, total cholesterol, triglycerides, CRP, and LPO levels whereas a significant reduction in creatinine clearance and GSH level. Treatment with antioxidants such as lycopene (5 mg/kg/p.o.) and CoQ10 (10 mg/kg/p.o.) along with their combination for 4 weeks notably altered the level of renal function biomarkers and oxidative stress markers. These antioxidants and their combination also protected the kidney from abnormal morphological changes. The present findings suggest that the combined administration of lycopene and CoQ10, which are antioxidants, exhibits synergistic effects in mitigating renal injury by reducing hyperglycemia, oxidative stress markers, and histopathological alterations.

Abstract

The repeated emergence of highly pathogenic human coronaviruses as well as their evolving variants highlight the need to develop potent and broad-spectrum antiviral therapeutics and vaccines. By screening monoclonal antibodies (mAbs) isolated from COVID-19-convalescent patients, we found one mAb, 2-36, with cross-neutralizing activity against SARS-CoV. We solved the cryo-EM structure of 2–36 in complex with SARS-CoV-2 or SARS-CoV spike, revealing a highly conserved epitope in the receptor-binding domain (RBD). Antibody 2–36 neutralized not only all current circulating SARS-CoV-2 variants and SARS-COV, but also a panel of bat and pangolin sarbecoviruses that can use human angiotensin-converting enzyme 2 (ACE2) as a receptor. We selected 2-36-escape viruses in vitro and confirmed that K378 T in SARS-CoV-2 RBD led to viral resistance. Taken together, 2–36 represents a strategic reserve drug candidate for the prevention and treatment of possible diseases caused by pre-emergent SARS-related coronaviruses. Its epitope defines a promising target for the development of a pan-sarbecovirus vaccine.

49. Design and Implementation of a SMART Learning Environment for the Upskilling of Cybersecurity Professionals in Mauritius

Abstract

Teaching and Learning confined to within the four walls of a classroom or even online Learning through Massive Online Courses (MOOCs) and other Learning Content Management Systems (LCMS) are no longer seen as the optimal approach for competency and skills development, especially for working professionals. Each of these busy learners have their own training needs and prior knowledge. Adopting the one-size-fits-all teaching approach is definitely not effective, motivating and encouraging. For some learners, the content might be too easy and for others, it might be too difficult. This is why this research presents the use of SMART Learning Environment that makes use of Intelligent Techniques to personalise the learning materials for each learner. This mismatch in skills is becoming a pressing issue and is having a direct impact on the ICT Sector, which is one of the pillars of the Mauritian Economy. This research, therefore, besides proposing a novel approach to learning, also attempts to address an issue of national importance. Data was collected during three phases, namely an Expert Reference Group Discussion, a pre-test questionnaire and a survey questionnaire. The Expert Reference Group Discussion was carried out to further understand the training needs and expectations of Cybersecurity professionals in Mauritius. A SMART Learning Environment making use of Artificial Neural Networks and Backpropagation Algorithm to personalise learning materials was eventually designed and implemented. The major findings of this research were that personalisation of learning materials through the use of a SMART Learning Environment can be used to address the training needs of Cybersecurity professionals in Mauritius.
