Vogt Koyanagi Harada disease affects several parts of the body, such as eyes, meninges, ears, and skin. The progressive course of the disease can lead to blindness and deafness. The case is presented of a Hispanic woman (mixed-race) with visual alterations, headache, tinnitus, hearing loss, and posterior uveitis with serous detachments of the retina in both eyes, as well as lymphocytic meningitis. The aim of the present study is to review the literature, the diagnostic strategies, and the appropriate treatment, as well as to update the immunogenetic pathogenesis of the disease. RESUMEN La enfermedad de Vogt Koyanagi Harada compromete múltiples órganos tales como ojos, meninges, oídos y piel. El curso progresivo de la enfermedad puede llevar a ceguera y cofosis. Se describe un caso de esta enfermedad en mujer hispana (mestiza) con alteraciones visuales, cefalalgia, tinnitus e hipoacusia a quien se le encuentra uveítis posterior con desprendimientos serosos de retina en ambos ojos y meningitis linfocitaria. El objetivo del presente estudio es, mediante una revisión de la literatura, actualizar la patogénesis inmunogenética, conocer las estrategias diagnósticas y el tratamiento apropiado.

Objectives: To assess the impact of educational level on rheumatoid arthritis through a systematic review of the literature and assessing the age at onset, the educational level as risk factor, and to identify outcomes related to educational level and rheumatoid arthritis. Methods: A systematic review was performed using PubMed, SciELO and LILACS as data bases in order to identify original articles written in English, Portuguese or Spanish that used accepted classification criteria for rheumatoid arthritis and a comparison was made between educational level and outcomes of rheumatoid arthritis. Final articles were identified by two independent reviewers and three blinded reviewers created a new list and extracted data from selected studies. Each record was classified based on the quality score of the studies. Results: Through the systematic review of the literature, the factors and outcomes related to educational level and rheumatoid arthritis identified were: the risk of developing the disease, radiographic progression, depression and anxiety, work disability, functional disability, quality of life, and mortality. Conclusion: The information available in the literature about the impact of the educational level in several outcomes related to rheumatoid arthritisis variable. Only work disability is an outcome related to a low education level in all the articles reviewed. Objetivos: Evaluar el impacto del nivel educativo sobre la artritis reumatoide a través de una revisión sistemática de la literatura analizando la edad de inicio, el nivel educativo como factor de riesgo e identificando los desenlaces relacionados con el nivel educativo y la artritis reumatoide. Métodos: Se realizó una revisión sistemática en PubMed, SciELO y LILACS como bases de datos con el fin de identificar artículos originales en inglés, portugués o español, que utilizaban criterios de clasificación aceptados para artritis reumatoide y comparaban el nivel educativo con diferentes desenlaces de la enfermedad. Los artículos finales fueron identificados por 2 revisores independientes. Tres revisores ciegos crearon una lista y extrajeron los datos de los estudios seleccionados. Cada registro fue clasificado en función de la calidad de los estudios. Resultados: Los factores y desenlaces identificados relacionados con el nivel educativo y artritis reumatoide fueron el riesgo de desarrollar la enfermedad, la progresión radiográfica, depresión y ansiedad, incapacidad laboral, incapacidad funcional, calidad de vida y mortalidad. Conclusión: La información disponible en la literatura sobre el impacto del nivel educativo en la artritis reumatoide en los diferentes desenlaces encontrados es variable. Solo la discapacidad funcional es un desenlace relacionado con bajo nivel educativo en todos los artículos encontrados.

Similar pathophysiological mechanisms within autoimmune diseases have stimulated searches for common genetic roots. Polyautoimmunity is defined as the presence of more than one autoimmune disease in a single patient. When three or more autoimmune diseases coexist, this condition is called multiple autoimmune syndrome (MAS). We analyzed the presence of polyautoimmunity in 1,083 patients belonging to four autoimmune disease cohorts. Polyautoimmunity was observed in 373 patients (34.4%). Autoimmune thyroid disease (AITD) and Sjögren's syndrome (SS) were the most frequent diseases encountered. Factors significantly associated with polyautoimmunity were female gender and familial autoimmunity. Through a systematic literature review, an updated search was done for all MAS cases (January 2006–September 2011). There were 142 articles retrieved corresponding to 226 cases. Next, we performed a clustering analysis in which AITD followed by systemic lupus erythematosus and SS were the most hierarchical diseases encountered. Our results indicate that coexistence of autoimmune diseases is not uncommon and follows a grouping pattern. Polyautoimmunity is the term proposed for this association of disorders, which encompasses the concept of a common origin for these diseases.

Para aplicar esquemas terapéuticos como la terapia de control estrecho en pacientes con Artritis Reumatoide (AR), Es necesario contar con mediciones objetivas de la actividad de la enfermedad. Para esto se han creado las herramientas de clinimetría. Las hay desarrollados con mediciones hechas por el médico, como elDAS28, el SDAI y el CDAI ó realizadas mediante cuestionario auto administrados, como los RAPID, producto del cuestionario R808-NP2-Spanish, ó el esquema SAI para auto conteo articular. Existen dudas respecto a la validez del cuestionario R808-NP2-Spanish, en pacientes de origen hispánico.Objetivo: Estipular el grado de asociación existente, entre las medidas de actividad de la Artritis reumatoide (AR), producidas mediante las herramientas de clinimetria auto-administradas, (cuestionario R808 - Np2- Spanish y esquema SAI), con las mediciones de clinimetria producidas mediante las mediciones realizadas por el personal médico entrenado y marcadores inflamatorios(DAS28 PCR, CDAI, SDAI), cuando dichas herramientas se aplican de forma colectiva a un grupo de pacientes latinoamericanos con AR.Métodos y Resultados: Este fue un estudio de corte trasversal en el que se analizaron 130 pacientes con AR, mediante los RAPID, el DAS28PCR el CDAI y el SDAI.
