Antimalarials have been administered widely to prevent clinical malaria and researchers have explored how end-users' perspectives influence uptake and adherence. Drawing on a systematic search, this review aims to synthesise qualitative research on end-user perceptions of antimalarials for disease prevention. Searches were undertaken in PubMed and ISI Web of Knowledge. After applying exclusion criteria, identified sources underwent thematic analysis. Identified sources were published between 2000 and 2020 and drew on studies undertaken across Africa, Asia, Europe, Oceania and America. The sources revealed end-user concerns about the potential benefits and harms of preventive treatment that are entwined with broader understandings of the disease, the intervention, its implementation, accompanying information, and how it is embedded in wider healthcare and social relationships. The implications for antimalarials as preventive therapy encompass the need to build trust, including interpersonal trust, engage diverse stakeholders and to address broader health and wellbeing concerns during implementation. Using data on 825 under-5 children from the Ouagadougou Health and Demographic Surveillance System collected in 2010, this article examines the effects of aspects of the immediate environment on childhood fever. Logit regression models were estimated to assess the effects of the quality of the local environment on the probability that a child is reported to have had a fever in the two weeks preceding the survey, after controlling for various demographic and socioeconomic variables. While the estimated impact of some environmental factors persisted in the full models, the effects of variables such as access to water and type of household waste management decreased in the presence of demographic, socioeconomic and neighbourhood factors. The management of waste water was found to significantly affect the occurrence of childhood fever. Overall, the results of the study call for more efforts to promote access to tap water to households at prices that are affordable for the local population, where the threats to child health appears to be greatest. In African growing cities, vector-borne diseases (such as malaria and dengue) contribute to a large burden of childhood morbidity and mortality. During the peak of transmission, environmental factors can have an influence on those fevers, apart from the individual and household characteristics. A household survey conducted in 2008 in Dakar was completed by a community questionnaire on environmental threats that could be factored into multilevel analyses. Using a randomized sample of 7,300 children from 3,000 households dispatched within 50 neighborhoods, a three-level modeling process is presented. Rates of recent fever varied substantially from one neighborhood to another, ranging between 10 and 37 %. Findings indicate that the onset of fever is influenced by factors from all three hierarchical levels, with neighborhood factors playing a relatively lower role than the other two.
