The fulfillment of rights to maternal healthcare is a key factor for the wellbeing of women. However, there is a lack of an instrument to ascertain the experience of women during maternity to enable adequate monitoring. The aim of this study was to validate a new instrument to measure women’s perception of the fulfillment of rights during healthcare in pregnancy and childbirth and immediately postpartum. The initial version of the instrument consists of 50 items and was validated using exploratory factor analysis. Additionally, the final version of the instrument consists of 29 items and was validated by confirmatory factor analysis and known-group validity. The instrument was applied to 185 Spanish women. The global Aiken’s V of the initial instrument proposal was 0.89. The process resulted in an instrument with five factors (information, privacy, consent, support, and participation) that explained the 60% of the total variance. The score of the instrument was correlated with resilience, maternity beliefs, and positive and negative affect. External validation showed relations with age, gravida, and the number of times a woman has been in labor. Additionally, the Cronbach’s α reliability was 0.93 [0.91; 0.94]. In conclusion, the instrument developed is consistent and has appropriate psychometric properties for assessing the fulfillment rights of maternity healthcare.

Maternity rights are perceived and fulfilled differently according to women’s psychosocial characteristics, leading to varying maternal experiences and outcomes. It is necessary to know the impact of cultural context, emotional well-being, and resource availability on the maternal woman’s clinical care experience. The aim is to identify if these factors contribute to disparities in the perception of maternity rights fulfillment in Spain and Colombia. This retrospective observational study focused on women who received maternity-related healthcare in Spain or Colombia. A total of 185 women were included (Spanish = 53; Colombian = 132). Data collected included social and obstetric history, as well as psychological variables such as resilience, positive and negative affect, derailment, and maternity beliefs. The study also assessed women’s knowledge of healthcare rights (MatCODE), perceptions of resource scarcity (MatER), and the fulfillment of maternity rights (FMR). C-section was more prevalent in Colombia, where women also scored higher on maternity beliefs as a sense of life and as a social duty compared to Spanish women. Conversely, FMR was higher in the Spanish context. Colombian women reported lower levels of social support and less involvement in medical decision-making. The FMR was positively correlated with positive affect, MatCODE, and MatER. Predictive modeling identified negative factors for FMR, including giving birth in Colombia (β = −0.30 [−0.58; −0.03]), previous miscarriage (β = −0.32 [−0.54; −0.09]), C-section in the most recent labor (β = −0.46 [−0.54; −0.0]), and higher MatER scores. Positive predictors included gestational age, maternal age, and previous C-section (β = 0.39 [0.11; 0.66]). The perception of the fulfillment of maternity rights depends on socio-healthcare contexts, women’s age, obstetric history, and resources. It is suggested to apply culturally sensitive strategies focused on women’s needs in terms of information, emotional and social support, privacy, and autonomy to manage a positive experience.

Abstract Objectives To develop gestational age-based reference ranges for cervical length in triplet pregnancies. The secondary objective was to assess the performance of cervical length measured between 18 and 20 + 6 days for the prediction of preterm delivery before 28 and 32 weeks, respectively. Methods Observational retrospective study of triplet pregnancies in three Spanish tertiary-care hospitals between 2001 and 2019. Cervical length measurements were consecutively obtained between 15 and 34 weeks of gestation. Pregnancies undergoing multifetal reduction or fetal surgery were excluded. Results Two hundred and six triplet pregnancies were included in the final analysis. There was a quadratic decrease in cervical length with gestational age. The median and fifth centiles for cervical length at 20 weeks were 35 and 13 mm. In the prediction of preterm birth

IntroductionPrognostic markers for fetal transmission of Cytomegalovirus (CMV) infection during pregnancy are poorly understood. Maternal CMV-specific T-cell responses may help prevent fetal transmission and thus, we set out to assess whether this may be the case in pregnant women who develop a primary CMV infection.MethodsA multicenter prospective study was carried out at 8 hospitals in Spain, from January 2017 to April 2020. Blood samples were collected from pregnant women at the time the primary CMV infection was diagnosed to assess the T-cell response. Quantitative analysis of interferon producing specific CMV-CD8+/CD4+ cells was performed by intracellular cytokine flow cytometry.ResultsIn this study, 135 pregnant women with a suspected CMV infection were evaluated, 60 of whom had a primary CMV infection and samples available. Of these, 24 mothers transmitted the infection to the fetus and 36 did not. No association was found between the presence of specific CD4 or CD8 responses against CMV at the time maternal infection was diagnosed and the risk of fetal transmission. There was no transmission among women with an undetectable CMV viral load in blood at diagnosis.ConclusionsIn this cohort of pregnant women with a primary CMV infection, no association was found between the presence of a CMV T-cell response at the time of maternal infection and the risk of intrauterine transmission. A detectable CMV viral load in the maternal blood at diagnosis of the primary maternal infection may represent a relevant biomarker associated with fetal transmission.

Twin pregnancies are high-risk gestations that increase the odds of obstetrical complications. They can also present specific and rare complications such as single intrauterine fetal death (IUFD). This complication has been extensively studied in monochorionic but not in bichorionic gestations. Today, the repercussions of IUFD may have on the surviving fetus, mother and bichorionic pregnancy are not known. Our objective was to study materno-obstetrical, fetal, and immediate delivery neonatal complications in bichorionic twin gestations with single IUFD compared to those with both fetuses alive. A retrospective and observational case-control study was performed in bichorionic biamniotic twin pregnancies, 22 complicated with single IUFD after 14 weeks (cases; IUFD group) and 51 with both fetuses alive (controls; non-IUFD group, from Obstetrics Service of La Paz Hospital (Madrid, Spain). The data were collected from obstetrical records. No significant differences were found in the rates of gestational diabetes, gestational hypertension, preeclampsia, neonatal complications, and prematurity between IUFD and non-IUFD groups. Statistical differences were found for the incidence of intrauterine growth restriction in the surviving fetus compared to first fetus of pregnancy with both fetuses alive (22.7% versus 2.0%, respectively; p-value = 0.012). There were no differences compared to second fetus (11.8%; p-value = 0.23). There was a high C-section rate in both groups (IUFD = 63.6%, non-IUFD = 64.7%; p-value = 0.19). In conclusion, single IUFD in bichorionic biamniotic twin gestations is a rare complication that should be closely monitored. It is essential that these gestations be attended by a clinical multidisciplinary team.

Background: Zika virus (ZIKV) infection has been associated with congenital microcephaly and other neurodevelopmental abnormalities. There is little published research on the effect of maternal ZIKV infection in a non-endemic European region. We aimed to describe the outcomes of pregnant travelers diagnosed as ZIKV-infected in Spain, and their exposed children. Methods: This prospective observational cohort study of nine referral hospitals enrolled pregnant women (PW) who travelled to endemic areas during their pregnancy or the two previous months, or those whose sexual partners visited endemic areas in the previous 6 months. Infants of ZIKV-infected mothers were followed for about two years. Results: ZIKV infection was diagnosed in 163 PW; 112 (70%) were asymptomatic and 24 (14.7%) were confirmed cases. Among 143 infants, 14 (9.8%) had adverse outcomes during follow-up; three had a congenital Zika syndrome (CZS), and 11 other potential Zika-related outcomes. The overall incidence of CZS was 2.1% (95%CI: 0.4–6.0%), but among infants born to ZIKV-confirmed mothers, this increased to 15.8% (95%CI: 3.4–39.6%). Conclusions: A nearly 10% overall risk of neurologic and hearing adverse outcomes was found in ZIKV-exposed children born to a ZIKV-infected traveler PW. Longer-term follow-up of these children is needed to assess whether there are any later-onset manifestations.

Neonatal herpes simplex virus infections are rare, but are associated with significant morbidity and mortality. Most newborns acquire herpes simplex virus infection in the peripartum period. For peripartum transmission to occur, women must be shedding the virus in their genital tracts symptomatically or asymptomatically around the time of delivery. There are evidence-based interventions in pregnancy to prevent the transmission to the newborn. Caesarean section should be performed in the presence of herpetic lesions, and antiviral prophylaxis in the last weeks of pregnancy is recommended to suppress genital tract herpes simplex virus at the time of delivery. The diagnosis and early treatment of neonatal herpes simplex virus infections require a high index of suspicion, especially in the absence of skin lesions. It is recommended to rule out herpes simplex virus infections in those newborns with mucocutaneous lesions, central nervous system involvement, or septic appearance. The prognosis of newborns with skin, eye, and/or mouth disease in the high-dose acyclovir era is very good. Antiviral treatment not only improves mortality rates in disseminated and central nervous system disease, but also improves the rates of long-term neurodevelopmental impairment in the cases of disseminated disease. Interestingly, a 6-month suppressive course of oral acyclovir following the acute infection has improved the neurodevelopmental prognosis in patients with CNS involvement. Twin pregnancies are increasing due to the rise in mothers’ childbearing age and have a higher risk of fetal growth restriction (FGR) and prematurity. Therefore, early prediction of these events is important. Our aim was to analyze in the first trimester of pregnancy a possible association between antioxidants, including melatonin, in maternal plasma and the development of fetal complications in twin pregnancies. A single-center, prospective, and observational study was performed in 104 twin-pregnant women. A blood sample was extracted between the 9th and the 11th week of gestation, and plasma was obtained. Antioxidants (thiols, reduced glutathione, phenolic compounds, catalase, superoxide dismutase) and oxidative damage biomarkers (carbonyl groups and malondialdehyde) were assessed by spectrophotometry, and global scores were calculated from these parameters (Antiox-S, Prooxy-S). Melatonin and cortisol were evaluated by a competitive immunoassay. In the first trimester of pregnancy, Antiox-S was significantly lower in women who developed FGR compared to those with normal fetal growth; plasma melatonin was significantly lower in women with preterm compared to those with full-term births and exhibited a positive correlation with birth weight. Maternal cortisol showed a negative correlation with birth weight. We conclude that, for twin gestations, maternal plasma antioxidant status and melatonin could be potential biomarkers to be included in algorithms to predict FGR and preterm labor.

Antecedentes: El embarazo de gemelos monocoriales tienen un mayor riesgo de mortalidad y morbilidad perinatal, que la gestación única y el embarazo gemelar bicorial. Objetivos: Estudiar la incidencia y el manejo de las complicaciones fetales propias de las gestaciones gemelares monocoriales biamnióticas. Métodos: Se realizó un análisis retrospectivo de 94 embarazos gemelares monocoriales biamnióticos, seguidos en las Consultas de Tocología de Alto Riesgo del Hospital Universitario La Paz de Madrid, entre 2008 y 2010. Resultados: Se recogieron 94 gestaciones monocoriales biamnióticas. Aparecieron complicaciones fetales en el 23,4% de las mismas: síndrome de transfusión feto-fetal en el 9,57% de los casos, retraso del crecimiento intrauterino selectivo en el 11,7 %, muerte fetal intrauterina en el 2,1%, y malformaciones fetales en el 7,4%. Conclusiones: Las gestaciones monocoriales biamnióticas son un tipo de embarazo gemelar de alto riesgo, asociado con más complicaciones fetales y muerte perinatal. Se recomienda un estrecho seguimiento ecográfico cada 2 semanas, desde la semana 16, para diagnosticar y tratar precozmente las complicaciones fetales.Background: The monochorionic twin pregnancy has a higher risk of perinatal mortality and morbidity, than the single gestation and the dichorionic twin gestation. Objectives: To detail the incidence and management of specific fetal complications of monochorionic diamniotic twin pregnancies. Methods: We performed a retrospective analysis of monochorionic diamniotic pregnancies followed in the High Risk Obstetrics Unit of the University Hospital La Paz, Madrid, between 2008-2010. Results: We collected 94 monochorionic diamniotic pregnancies. Fetal complications occurred in 23.4% of them: feto-fetal transfusion syndrome in 9.57% of cases, selective intrauterine growth retardation in 11.7%, stillbirth in 2.1%, and fetal malformations in 7.4%. Conclusions: Monochorionic diamniotic twin pregnancies are a high-risk type of twin pregnancy, associated with an increased risk of death and perinatal complications. Ultrasound monitoring is recommended for uncomplicated monochorionic pregnancies every 2 weeks from week 16, to detect and treat fetal complications.

Objetivos: Analizar el diagnóstico ecográfico, la evolución y el tratamiento de los tumores cardiacos fetales diagnosticados en el Hospital La Paz de Madrid, entre los años 1995 y 2010. Método: Se realizó un estudio retrospectivo descriptivo recogiendo los principales datos ecográficos, de la historia clínica materna, neonatal y pediátrica. Resultados: En el periodo de estudio se diagnosticaron ecográficamente 28 tumores cardiacos fetales, 21 casos fueron catalogados como rabdomiomas. Once de ellos tuvieron una regresión prácticamente completa, con ocho casos diagnosticados hasta el momento de esclerosis tuberosa. Hubo 2 muertes intraútero, 1 interrupción legal del embarazo y 4 diagnósticos ecográficos de rabdomiomas, no se confirmaron al nacimiento. Se diagnosticaron prenatalmente dos fibromas; de ellos una gestante optó por la interrupción legal del embarazo y en el otro caso se produjo la muerte neonatal. Dos neonatos fueron sometidos a cirugía con una resección completa del tumor, con resultado anatomopatológico de heman-gioma capilar en uno y teratoma en el otro. En un caso se realizó una biopsia que confirmó la presencia de un hemangiopericitoma auricular que se redujo posteriormente con quimioterapia. Conclusiones: Los tumores cardiacos son una patología poco frecuente. La mayor parte de ellos son rabdomiomas, cuya sin-tomatología y evolución depende de su localización. Estos suelen regresar espontáneamente, pero pueden asociarse al diagnóstico de esclerosis tuberosa, lo que empeora su pronóstico.Aims: To analize the diagnosis, clinical course and management of fetal cardiac tumors diagnosed at La Paz Hospital (Madrid) between 1995 and 2010. Methods: We performed a retrospective descriptive study collecting the main ultrasound dates of the maternal, newborn and pediatric history. Results: During the study period, 28 fetal cardiac tumors were dignosed. Rhabdomyomas were diagnosed in 21 fetuses; 11 rhabdom-yomas returned almost completely. Eight of them were diagnosed of tuberous sclerosis up to the moment. Other two cases died in utero. One pregnant decided to be practised a miscarriage and four rhabdomyomas which were diagnosed by ultrasound, were not found in the newborns. Two fibroms were diagnosed by ultrasound; one of the pregnant woman decided to be practised a miscarriage and the other fetus died when he was born. Two newborns were operated, with the pathological anatomy result of a hemangioma and a teratoma. A biopsy was made that confirmed the presence of an atrial hemangiopericitoma which was treated by quimiotherapy. Conclusion: Fetal cardiac tumors are a rare disease. Most of them are rhabdomyomas which syntomatology and clinical course depend on its location. They usually regret spontaneously, but they can be associated with tuberous sclerosis, and this aggravates their prognosis.

La enfermedad de Steinert es una enfermedad genética que se hereda con un patrón autosómico dominante, resultado de la expansión de la repetición de trinucleótidos CTG gen en el cromosoma 19, que codifica una proteína quinasa. Es una forma grave de distrofia muscular caracterizada por debilidad generalizada y degeneración muscular. El debut puede ocurrir en cualquier momento desde el nacimiento hasta la edad madura. Las complicaciones durante el embarazo son el aborto espontáneo, parto prematuro, el hidram-nios, atonía uterina posparto, distocias intraparto y accidentes anestésicos. Presentamos el caso de una gestante de 37 años, asintomática con una anamnesis familiar sin patología de interés. Fue diagnosticada de diabetes gestacional y polihidramnios inespecífico en la semana 30 de embarazo. Acudió a nuestro centro en semana 35 de gestación por dinámica uterina y metrorragia. Se realizó una cesárea extrayéndose un feto masculino, con graves dificultades respiratorias y miotonía generalizada. Posteriormente, el neonato fue diagnosticado de enfermedad congénita de Steinert. El análisis genético de la madre reveló que ella padecía la misma enfermedad. El diagnóstico de enfermedad congénita de Steinert es muy difícil, sobretodo cuando los padres no son conscientes de la enfermedad. Nuestro objetivo es enfatizar en la importancia de una buena anamnesis y los marcadores que se pueden encontrar por ultrasonidos, como hidramnios, reducción del tono fetal y los movimientos activos, artrogriposis, micrognatia, que nos puede proporcionar al menos una sospecha prenatal.Steinert's disease is a genetic condition, which is inherited in an autosomal dominant pattern, result from expansion of CTG trinucleotide repeat gene on chromosome 19, enconding a putative protein kinase. It is a severe form of muscular dystrophy marked by generalised weakness and muscular wasting. The onset can be any time from birth to middle age. The complications during pregnancy are miscarriage, premature labour, hydramnios, atonic postpartum hemorrhage, difficulties during delivery, anesthetic accidents. We report the case of a healthy 37 years old pregnant, with an ordinary family anamnesis. She was diagnosed of gestational diabetes and inespecific polyhydramnios during her 30 week of pregnancy. Due to labour contractions and metrorrhagia in the 35 week she came to our emergency department. She underwent a cesarean section delivery of a male baby, who suffered severe breathing difficulties and generalized myotonia. Afterwards, the baby was diagnosticated with Steinert's congenital disease. Following genetic analysis of the mother revealed that she also suffers Steinert's disease. The diagnosis of the congenital Steinert's disease is really difficult, when the parents are unaware of the disease. Our objective is to emphasize in the importance of a good anamnesis and the characters that can be found out by ultrasound like hydramnios, reduction of fetal tone and active movements, micrognathia, arthrogryposis, that can bring us at least to a prenatal suspicion.

Antecedentes: Se están usando diferentes tipos de suplementación nutricional durante la gestación, sin embargo, existen escasez de estudios que se centren en los resultados metabólicos de los mismos. Objetivos: Comprobar si mediante la suplementación se logra el control del impacto de los múltiples cambios metabólicos que conlleva una gestación múltiple, usando análisis comunes. Método: Comparamos tres diferentes grupos de pacientes entre la semana gestacional 25 y 27. Un grupo de 46 gestaciones únicas, un grupo de 38 gestaciones gemelares espontáneas y otro grupo de 32 gestaciones gemelares tras técnicas de reproducción asistida. Comparamos niveles de colesterol LDL, HDL, triglicéridos, folato sérico, vitamina B12, homocisteína, hemoglobina, creatinina y plaquetas. La acromegalia es el exceso de secreción de hormona de crecimiento (HC). Esta alteración está relacionada con esterilidad y con complicaciones maternas durante el embarazo por aumento de la secreción de la HC. Presentamos el caso de una gestante diagnosticada de acromegalia persistente a pesar de haber sido operada vía transesfenoidal y tratada con cabergolina. Durante la gestación, la paciente no presentó complicaciones metabólicas ni tensionales, observándose un progresivo descenso de los niveles del factor de crecimiento insulínico-1 (IGF-I). El feto tuvo un crecimiento acorde con la amenorrea hasta la semana 31 en la que hizo una restricción del crecimiento con oligoamnios y alteración en el doppler en la semana 37. Se finalizó la gestación realizando una cesárea naciendo un recién nacido de 2400 gramos sin complicaciones neonatales. Aunque el curso del embarazo de las gestantes con acromegalia transcurre generalmente sin complicaciones, presentamos el primer caso descrito de un crecimiento intrauterino retardado en una paciente con acromegalia.Pregnancy in acromegalic patients is an infrequent event, due to perturbed gonadotroph function. On the other hand, pregnancy may cause an enlargement of the adenoma or an increase of growth hormone (GH) secretion. We report the case of a pregnant acromegalic woman who had been previously operated by transphenoidal approach and treated with cabergolin. A progressive decrease of insuline-like growth factor-1 (IGF1) level during pregnancy without tumoral syndrome or visual troubles was shown during pregnancy. Neither metabolic complication nor hypertension were detected. Ultrasound performed at 31th week showed a decrease of the fetal growth. At 37th week, an intrauterine growth restriction and oligohydramnios was diagnosed. It was performed a cesarean section and it was delivered a healthy 2700 grams male newborn. Our report suggests that pregnancy could not influence negatively in acromegalic patient but may be associated with intrauterine growth restriction.

Introducción: la traquelectomía radical (TR) es una técnica quirúrgica utilizada en estadios precoces del cáncer de cérvix en aquellas mujeres que desean conservar la capacidad reproductiva. Los embarazos posteriores a esta técnica quirúrgica pueden desencadenar complicaciones obstétricas. El presente artículo tiene como objetivo hacer una revisión de la literatura en relación al pronóstico obstétrico de estas pacientes. Metodología: en este estudio se presenta un caso clínico. Posteriormente, se realiza una búsqueda en las bases de datos MEDLINE, vía PubMed, y Cochrane con las palabras clave "cáncer de cérvix", "traquelectomía radical", "embarazo" y "complicaciones obstétricas". Resultados: se evaluó un total de 17 artículos que comprendieron revisiones, artículos de opinión y casos clínicos. Conclusión: la preservación de la fertilidad en estadios precoces del cáncer de cérvix mediante la traquelectomía radical está adquiriendo cada vez mayor aceptación a medida que se publican más casos en la literatura. Los resultados obstétricos parecen ser cada vez más favorables aunque son más numerosas las complicaciones en comparación con la población general.Introduction: radical trachelectomy (TR) is a surgical technique which is used in the early stages of cervical cancer in females wishing to conserve their reproductive ability. Pregnancies following this surgical technique having been applied could trigger obstetric complications. This article was thus aimed at reviewing the literature related to these patients’ obstetric prognosis. Methodology: a clinical case is presented. A literature search was made in MEDLInE databases, via PubMed and Cochrane, using the following key words: "cervical cancer", "radical trachelectomy", "pregnancy", "obstetric complications". Results: 17 articles were evaluated, covering review articles, articles expressing opinion and clinical cases. Conclusion: preserving fertility in early stages of cervical cancer by radical trachelectomy is increasingly acquiring more acceptance as more cases are published in the literature. Obstetric results seem to be becoming more favorable even though complications are becoming more numerous compared to incidence in the general population.

La principal causa de anemia fetal es la isoinmunización Rh por el desarrollo de anticuerpos frente al antí-geno D. La aloinmunización antiKell es una patología poco frecuente aunque puede producir un cuadro de anemia fetal muy grave. Su incidencia relativa ha aumentado en los últimos años debido al mayor número de transfusiones sanguíneas por disminución de la isoinmunización anti-D. Presentamos 26 casos de isoin-munización antiKell controlados en el Hospital La Paz de Madrid, durante los años 2003-2009 y una revisión de la literatura.The main cause of fetal anemia is red-cell alloimmunization. Kell alloinmunization is a rare disease, although it can produce severe fetal damages. The relative incidence of antiKell isoinmunization has increased last years due to the blood transfusions has grown also, and anti-D aloinmunization has decreased. We report twenty six cases of pregnant women with isoinmunization antikell controlled in La Paz Hospital, Madrid, between 2003-2009 and a review of the literature.

Objetivo: Comprobar si el sobrepeso y la obesidad materna son factores de riesgo de complicaciones pe-rinatales. Método: Estudio transversal de 1.223 gestantes nulíparas atendidas en el Hospital La Paz entre agosto 2007 y abril 2008. Las gestantes se dividieron en tres grupos según el índice de masa corporal (IMC) al inicio del embarazo: 482 con peso normal (IMC: 18,5-24,9 Kg/m²); 419 con sobrepeso (IMC: 25-29,9Kg/m²) y 322 con obesidad (IMC: >30Kg/m²). Se estudiaron las complicaciones obstétricas, semanas de gestación en el momento del parto, tipo de parto y peso fetal. Resultados: El riesgo de cesárea en las gestantes con sobrepeso fue casi el doble que el de las gestantes de peso normal (OR: 1,9; IC95% 1,4-2,5) Las gestantes obesas sufrieron el triple de cesáreas que las de peso normal (OR: 3,1; IC95% 2,8-4,3). El número de inducciones del parto fue mayor en las gestantes con sobrepeso (OR: 1,7; IC95% 1,4-1,9) y el doble en las obesas (OR: 2,0; IC95% 1,8-2,9) con respecto a las de peso normal. El riesgo de macrosomía fetal (>4.000 gramos) fue mayor en las gestantes con sobrepeso (OR: 1,5; IC95% 1,4-2,2) y en las obesas (OR: 1,9; IC95% 1,3-2,8) que en las de peso normal. En el análisis de regresión logística multivariante (ajusfando por edad, diabetes e hipertensión gestacionales) persistió la asociación entre sobrepeso y cesárea (OR: 2,1; IC95% 1,7-2,7) y entre obesidad y cesárea (OR: 3,0; IC95% 2,6-3,8). Conclusiones: El sobrepeso y la obesidad materna al inicio del embarazo están asociados con un aumento del número de cesáreas y de inducciones del parto. Este riesgo es mayor a medida que aumenta el IMC materno.Objective: The objective of this study is to determine if the maternal prepregnancy overweight and obesity are related to risk of cesarean delivery and other obstetrical complications. Methods: Transversal study of 1223 nulliparous women that delivered in our maternity between August 2007 and April 2008. Women were categorized by the body mass index (BMI): 482 normal weight (BMI: 18.5-24.9 Kg/m²), 419 overweight (BMI: 25-29.9 Kg/m²) and 322 obesity (BMI: >30 Kg/m²). Obstetrical complications, labor and its complications, and neonatal weight were studied. Results: Women who were overweight was nearly twice as likely to deliver by cesarean section as women with normal weight (OR: 1.8; 95%CI 1.4-2.9). Obese women experienced a three-fold increase in risk of cesarean section in comparison to normal weight women (OR: 3.02; 95%CI 2.6-4.5). Number of labor inductions was higher in overweight and obese group (OR: 1.6; 95%CI 1.3-1.9) and (OR: 2.1; 95%CI 1.7-2.9) respectively in comparison to control group. The highest risk of fetal macrosomia (weight >4000 g) was in the overweight (OR: 1.5; 95%CI 1.4-2.2) and obese women (OR: 1.9; 95%CI 1.3-2.8) compared with the normal weight women. In multivariate logistic regression analysis, the association persisted between obesity and cesarean delivery (OR 3.0; 95%CI 2.6-3.8) and among overweight and cesarean section (OR 2.1; 95%CI 1.7-2.7) after adjusting for variables such as age, diabetes and hypertension. Conclusions: Overweight or obesity prior to pregnancy are associated with an increased risk of cesarean delivery and labor inductions. This risk increases as the BMI increases.

La hemocromatosis neonatal es una enfermedad hepática muy severa del recién nacido y se asocia a una alta mortalidad. Se cree que su etiología es de tipo aloinmune, debido a la presencia de un anticuerpo materno hasta ahora desconocido que interfiere con el metabolismo férrico del feto, llegando a producir gran morb i mortal ¡dad. Basándonos en esta teoría, el tratamiento materno con inmunoglobulinas intravenosas en gestaciones sucesivas podría prevenir el desarrollo de un nuevo cuadro de hemocromatosis neonatal. Se describe el caso de una gestante con un hijo anterior diagnosticado y fallecido neonatalmente por hemocromatosis, a la que en el embarazo actual se le trató con inmunoglobulinas intravenosas consiguiendo un hijo sano y vivo. Es el primer caso descrito en España y demuestra el éxito de esta terapia, tal como describe la literatura.Neonatal hemochromatosis is a severe neonatal liver disease with a high mortality and recurrence rate. It is supposed to be a gestational alloimmune disease because of the existence of maternal antibodies against fetal hepatic metabolism. On the basis of this hypothesis, the administration of intravenous immunoglobulin has been reported as a successful treatment during the following pregnancy. We describe the first case of this treatment in Spain which confirms the data available in the literature.

During pregnancy, physiological changes in the immune response make pregnant women more susceptible to serious infection, increasing the risk for the mother as well as the foetus, newborn and infant. All women should be correctly and fully vaccinated as they enter their reproductive years, especially against diseases such as tetanus, hepatitis B, measles, rubella and varicella. In addition to the recommended vaccines, in risk situations, inactivated vaccines could be administered to women who were not correctly vaccinated before, while attenuated vaccines are contraindicated.Despite the fact that vaccination during pregnancy is a very important preventive measure and the existing recommendations from public health authorities, scientific societies and health professionals, the vaccination coverage could clearly be improved, especially against influenza and SARS-CoV-2, so any health professional involved in the care of pregnant women should proactively recommend these vaccines.The Spanish Association of Pediatrics (AEP), through its Advisory Committee on Vaccines, and the Spanish Society of Gynaecology and Obstetrics (SEGO) recommend vaccination against the following diseases during pregnancy: against influenza and COVID-19, in any trimester of pregnancy and during the postpartum period (up to 6 months post birth) in women not vaccinated during pregnancy; against pertussis, with the Tdap vaccine, between weeks 27 and 36 of gestation (in the CAV-AEP recommendations, preferably between weeks 27 and 28); and against RSV, with RSVPreF, between weeks 24 and 36 of gestation, preferably between weeks 32 and 36. Resumen: Durante el embarazo, los cambios fisiológicos en la respuesta inmunitaria favorecen que las gestantes sean más susceptibles a infecciones graves, tanto para ellas como para el feto, recién nacido y lactante. Todas las mujeres deberían entrar en el período reproductivo con su calendario vacunal correctamente cumplimentado, sobre todo en lo que respecta a enfermedades como tétanos, hepatitis B, sarampión, rubeola y varicela. Además de las vacunas recomendadas, en situaciones de riesgo las vacunas inactivadas podrían ser administradas en aquellas mujeres que no estuvieran correctamente inmunizadas con anterioridad, mientras que las atenuadas están contraindicadas.A pesar de que la vacunación durante el embarazo es una medida preventiva muy importante, y de las recomendaciones de autoridades sanitarias, sociedades científicas y profesionales sanitarios, las coberturas vacunales son claramente mejorables, especialmente en lo que respecta a gripe y COVID-19, por lo que todo profesional sanitario que atienda a la embarazada debe ser proactivo en aconsejarlas.La Asociación Española de Pediatría, a través de su Comité Asesor de Vacunas, y la Sociedad Española de Ginecología y Obstetricia recomiendan las siguientes vacunaciones durante la gestación: frente a gripe y COVID-19, en cualquier trimestre del embarazo, y durante el puerperio (hasta los 6 meses) en aquellas que no hubieran sido vacunadas durante la gestación; frente a tosferina con Tdpa, entre las 27 y 36 semanas de gestación (el CAV-AEP da preferencia entre las 27 y 28 semanas); y frente al VRS con RSVPreF, entre las 24 y 36 semanas de gestación, de preferencia entre las 32 y 36 semanas.
