Abstract

Charles III arrived in Spain from Naples accompanied by a multidisciplinary team of military advisers, including Count Gazzola from Piacenza, who had already drawn up his plans for the urgent military reforms required for the artillery. These included the institutionalisation of that branch of the army and the foundation of the college for artillery officers and industrial engineers to run the military factories being taken over by the State. The most eye-catching element of this undertaking is the formation of its magnificent Enlightenment scientific library. A study of this library reveals an exceptional collection, the origins of the first books brought to the institution, the way it was organised and the military and scientific topics addressed. But its fundamental contribution can be measured in terms of results: indeed, textbooks were created for the cadets, as well as research works on military, artillery and scientific topics written by teachers from the college, and translations of important European scientific books. Finally, as a contribution to demonstrating the cost-effectiveness of the Crown’s investment in education for war, we highlight the work of three captains, showing the versatility they achieved thanks to their education at the college and their reputation outside it, in the service of society, as guiding lights of the Sociedad Económica de Amigos del País (SEAP – Economic Society of Friends of the Country) in Segovia. Without this magnificent library, the research, monographs and translations – ultimately the college’s own published works which brought it great academic credit in Enlightenment Europe – would not have been possible.

Abstract

El presente artículo aúna dos objetivos. Por un lado, aproximarse brevemente a la nueva historiografía militar de la Edad Moderna y su vuelta a la reconsideración de fuentes primarias. Por otro lado, estudiar y analizar la documentación existente en el Archivo General Militar de Segovia, decano de los archivos históricos militares en España. El desarrollo de este segundo apartado incluye, primero, la historia, etapas de formación y organización de los fondos documentales del archivo. A continuación se considera el uso instrumental para la investigación histórica que dichos fondos permiten. Un uso múltiple dada la amplitud temática de las informaciones contenidas, que van desde lo personal (expedientes personales y matrimoniales, hojas de servicios, pasaportes, pleitos, testamentarías, etc.) a lo institucional (Montepío Militar, Justicia Militar).

6. Public–private partnership as an innovative approach for sustainable tourism in Guanacaste, Costa Rica

Abstract

Winner as Highly Commended Paper in the 2020 Emerald Literati Award, Purpose: The current tourism model based on luxury hotel resorts in the Gulf of Papagayo (Guanacaste, Costa Rica) is largely affecting the living condition of its nearby communities. This paper aims to discuss the importance of promoting public–private partnerships (PPPs) as innovative forms of governance to increase the sustainability of this tourism model. Design/methodology/approach: Based on the review of institutional documents and the PPP literature, this article critically maps each stage of the process to design PPPs for sustainable tourism, taking into account the case of Guanacaste. In this way, it offers a practical guideline to plan partnerships involving academia, public institutions and private partners in particular tourism sites. Findings: The paper shows that the feasibility of a PPP in Guanacaste strongly depends on the alignment of partners’ local interests, on the adequacy of the partnership to the social and economic conditions of the context wherein it has to be carried out, and on the appropriation of results from partners. Abstract

One of the effects of globalisation has been population mobility as a result of famine, climate warming and war conflicts, among other things. This flow of refugees, however, is often seen as a menace to the rule of law and human rights concomitant with the Western lifestyle. Refugees are no longer regarded as human beings and victims, but rather as danger, even as potential terrorists, which has led many governments, including the Australian, to detain them indefinitely in detention centres where they are confined in inhuman conditions. The main aim of this paper will be to describe Australian immigration policies and how contemporary Australian narratives on and by refugees are reflecting this situation, mainly by analysing a selection of texts from three recently published collections, namely, A Country Too Far (2013), They Cannot Take the Sky (2017) and Seabirds Crying in the Harbour Dark (2017), and Behrouz Boochani’s No Friend but the Mountains (2018). Uno de los efectos de la globalización ha sido la movilidad de la población como resultado de la hambruna, el calentamiento climático y los conflictos bélicos, entre otras cosas. Sin embargo, este flujo de refugiados a menudo se considera una amenaza para el estado de derecho y los derechos humanos concomitantes con el estilo de vida occidental. Los refugiados ya no son considerados seres humanos y víctimas, sino más bien un peligro, incluso como potenciales terroristas, lo que ha llevado a muchos gobiernos, incluido el australiano, a detenerlos indefinidamente en centros de detención donde se encuentran confinados en condiciones inhumanas. El síndrome de Lady Windermere fue definido hace más de 20 años por Reich y Johnson, quienes utilizan este término para describir la infección pulmonar por Mycobacterium avium complex (MAC) limitada al lóbulo medio y/o a la língula. Presentamos un caso de una mujer de 95 años, sin antecedentes de enfermedad pulmonar conocida, que presenta síndrome constitucional y tos crónica, con diagnostico de infección por MAC en estas áreas.

Abstract

Purpose This paper aims to find answers to two research questions: What is the perception of the Moravia community on tourism in their neighbourhood? What are the conditions under which they can accept slum tourism? Design/methodology/approach The foundation of this paper was laid during a field trip to Medellin, Colombia, in March 2018. More than three weeks were spent in the neighbourhood of Moravia, one of Medellin’s “barrios” or slums, where a qualitative study was led. Semi-structured interviews with the local community were done around the neighbourhood. Findings The main finding of this research is the positive perception of slum tourism within the Moravia community. The researchers found that the locals are proud when outsiders visit to see their neighbourhood. The locals felt that, it breaks the prejudice surrounding their homes, and tourists are seen as spokespeople for the barrio. There are visible improvements made to the barrio owing to tourism. This research drew attention to the conditions under which this type of tourism can be accepted in the neighbourhood: community participation, interaction between hosts and tourists, education and respect of tourists and the visible improvements to the neighbourhood. Originality/value Slum tourism is an understudied topic in Colombia, especially in Medellin. By researching on the host perception on slum tourism, this paper contributes to literature on slum tourism from a new angle.

Abstract

REIPI-SEIMC COVID-19 group and COVID@HULP group., [Background] The clinical presentation of COVID-19 in patients admitted to hospital is heterogeneous. We aimed to determine whether clinical phenotypes of patients with COVID-19 can be derived from clinical data, to assess the reproducibility of these phenotypes and correlation with prognosis, and to derive and validate a simplified probabilistic model for phenotype assignment. Phenotype identification was not primarily intended as a predictive tool for mortality., [Methods] In this study, we used data from two cohorts: the COVID-19@Spain cohort, a retrospective cohort including 4035 consecutive adult patients admitted to 127 hospitals in Spain with COVID-19 between Feb 2 and March 17, 2020, and the COVID-19@HULP cohort, including 2226 consecutive adult patients admitted to a teaching hospital in Madrid between Feb 25 and April 19, 2020. The COVID-19@Spain cohort was divided into a derivation cohort, comprising 2667 randomly selected patients, and an internal validation cohort, comprising the remaining 1368 patients. The COVID-19@HULP cohort was used as an external validation cohort. A probabilistic model for phenotype assignment was derived in the derivation cohort using multinomial logistic regression and validated in the internal validation cohort. The model was also applied to the external validation cohort. 30-day mortality and other prognostic variables were assessed in the derived phenotypes and in the phenotypes assigned by the probabilistic model., [Findings] Three distinct phenotypes were derived in the derivation cohort (n=2667)—phenotype A (516 [19%] patients), phenotype B (1955 [73%]) and phenotype C (196 [7%])—and reproduced in the internal validation cohort (n=1368)—phenotype A (233 [17%] patients), phenotype B (1019 [74%]), and phenotype C (116 [8%]). Patients with phenotype A were younger, were less frequently male, had mild viral symptoms, and had normal inflammatory parameters. Patients with phenotype B included more patients with obesity, lymphocytopenia, and moderately elevated inflammatory parameters. Patients with phenotype C included older patients with more comorbidities and even higher inflammatory parameters than phenotype B. We developed a simplified probabilistic model (validated in the internal validation cohort) for phenotype assignment, including 16 variables. In the derivation cohort, 30-day mortality rates were 2·5% (95% CI 1·4–4·3) for patients with phenotype A, 30·5% (28·5–32·6) for patients with phenotype B, and 60·7% (53·7–67·2) for patients with phenotype C (log-rank test p, [Interpretation] Patients admitted to hospital with COVID-19 can be classified into three phenotypes that correlate with mortality. Este trabajo se centra en uno de los exitos del reformismo militar, el nuevo modelo de ensenanza militar y cientifico-tecnica de los Cuerpos Facultativos. La formacion del oficial del XVIII sera el punto de partida de un intento de aproximacion a la rentabilidad de la nueva politica educativa y de los colegios militares de ingenieros y artilleria, este ultimo paradigma de la innovacion docente militar. Y desde el universo academico de esta institucion, rastreamos la rentabilidad de la educacion de Marte y entre sus reditos mas notables, se analiza la versatilidad profesional de oficiales que se desenvolvieron en territorios como el espionaje industrial por Europa, trabajando conjuntamente artilleros y el ingeniero Betancourt; o que transitaron de la ciencia a la nueva economia politica con naturalidad, como el capitan Vicente Alcala Galiano.

Abstract

Purpose The current tourism model based on luxury hotel resorts in the Gulf of Papagayo (Guanacaste, Costa Rica) is largely affecting the living condition of its nearby communities. This paper aims to discuss the importance of promoting public–private partnerships (PPPs) as innovative forms of governance to increase the sustainability of this tourism model. Design/methodology/approach Based on the review of institutional documents and the PPP literature, this article critically maps each stage of the process to design PPPs for sustainable tourism, taking into account the case of Guanacaste. In this way, it offers a practical guideline to plan partnerships involving academia, public institutions and private partners in particular tourism sites. Findings The paper shows that the feasibility of a PPP in Guanacaste strongly depends on the alignment of partners’ local interests, on the adequacy of the partnership to the social and economic conditions of the context wherein it has to be carried out, and on the appropriation of results from partners. Originality/value This work combines a theoretical and practical perspective to understand the interactive process to be carried out to design PPPs in developing tourism destinations.

Abstract

The scope of this paper was to study the relationship between pain intensity, health-related quality of life, disability, sleep quality and demographic data in elderly people with total knee arthroplasty (TKA). 24 subjects who had been subjected to TKA the previous month (4 females; 66 ± 9years) and 21 comparable controls (8 male; 70 ± 9years) participated in the study. Intensity of pain, and highest and lowest pain intensity experienced in the preceding week were collected. The Western Ontario and McMaster Universities index function, quality of life (Medical Outcomes Study Short Form 36), and Pittsburgh Sleep Quality Index were assessed. Age, gender, weight, height, body mass index were also collected. Individuals with TKA presented worse physical function (P < 0.01), social role (P = 0.01), physical performance (P < 0.01), pain (P = 0.04), disability (P = 0.04) and sleep quality (P = 0.03) than the controls. Higher intensity of pain was associated with lower physical function, social role, mental health, vitality and general health, and with higher disability and sleep quality. Disability and sleep quality were negatively associated with several quality of life domains. The associations between the intensity of pain, disability, quality of life and sleep reveal the multidimensional experience of TKA. Objetivo: Identificar as relações entre intensidade da dor, qualidade de vida relacionada à saúde, incapacidade, qualidade do sono e dados demográficos em idosos com artroplastia total de joelho (ATJ). Métodos: Participaram vinte e quatro pacientes que receberam ATJ no mês anterior (4 mulheres; 66 ± 9 anos) e 21 controles comparáveis (8 homens; 70 ± 9 anos). Foram coletadas a intensidade da dor no momento da avaliação, a maior e a menor intensidade de dor sentida na semana anterior. A função (Western Ontario and McMaster Universities index), a qualidade de vida (Medical Outcomes Study Short Form 36), e a qualidade do sono (Pittsburgh Sleep Quality Index) foram avaliadas. Idade, sexo, peso, altura, índice de massa corporal também foram coletados. Resultados: Indivíduos com ATJ mostraram pior função física (P < 0,01), papel social (P = 0,01), desempenho físico (P < 0,01), dor (P = 0,04), incapacidade (P = 0,04) e qualidade do sono (P = 0,03) do que os controles. Maior intensidade da dor foi associada com menor função física, papel social, saúde mental, vitalidade e saúde geral, e com maior incapacidade e qualidade do sono. Incapacidade e qualidade do sono estiveram associa das negativamente com vários domínios de qualidade de vida. As associações entre a intensidade da dor, incapacidade, qualidade de vida e do sono mostram a experiência multidimensional da ATJ.

Abstract

Mitochondrial DNA mutations have been associated with different illnesses in humans, such as Kearns-Sayre syndrome (KSS), which is related to deletions of different sizes and positions among patients. Here, we report a Mexican patient with typical features of KSS containing a novel deletion of 7629 bp in size with 85% heteroplasmy, which has not been previously reported. Sequence analysis revealed 3-bp perfect short direct repeats flanking the deletion region, in addition to 7-bp imperfect direct repeats within 9-10 bp. Furthermore, sequencing, alignment and phylogenetic analysis of the hypervariable region revealed that the patient may belong to a founder Native American haplogroup C4c.

18. From Pillar of Salt to Child of Nature and Viceversa: Postcolonial Criticism in David Malouf's An Imaginary Life

Abstract

This Master Thesis analyzes An Imaginary Life from a postcolonial perspective in order to work on some issues which are crucial in Australian postcolonial literature. Firstly, this analysis focuses on the importance that the landscape and the sense of unbelonging to it have in postcolonial Australia, and how An Imaginary Life presents a cosmovision which echoes the relationship of the Indigenous Australians with the land. Secondly, a close analysis of this novel reveals that it questions western ideas about cultural hegemony and alludes to the need to open our minds up and consider the fact that cultures are just human constructions that can be equally deconstructed and modified. This Master Thesis takes into account the revision that Stuart Hall makes of Michel Foucault’s theories of power/knowledge and power’s circulation, in order to show how the text subverts the conventional colonial power relations to make Ovid play the role of the oppressed. Therefore, this work also studies the relationship between the representatives of the European Empires and the natives of these new settlements. This work also analyzes the fundamental role played by language in An Imaginary Life. On the one hand, if you lack the words you cannot communicate with anybody and, consequently, you are isolated. On the other hand, Ovid has to get rid of human language and the world of culture and embrace the language of nature if he wants to transcend the human world of culture and be one with the universe. Another essential aspect of An Imaginary Life is the development of Ovid’s identity process in a hybrid space in which cultural negotiations with the “others” are always in constant modification. Accordingly, this Master Thesis also explores the initial feelings of Ovid and the British convicts sent to Australia when they arrived in a different and hostile territory.

Abstract

In this article, conceived from the perspective of the researcher, we start from a short introduction to the state of the art of Early Modern Military Historiography and the return to the primary sources. From this point we focus on the Archivo General Militar, Segovia, the oldest document repository of the Spanish Armies. As the oldest military archive, we make an approach to the documentation generated by the Army over the centuries and preserved in the Alcázar, to its specific document types, and to their use as instrument for the historical research. The analysis of these documentary resources is a valuable tool for research in Early Modern Spanish Military History. El presente artículo aúna dos objetivos. Por un lado, aproximarse brevemente a la nueva historiografía militar de la Edad Moderna y su vuelta a la reconsideración de fuentes primarias. Por otro lado, estudiar y analizar la documentación existente en el Archivo General Militar de Segovia, decano de los archivos históricos militares en España. El desarrollo de este segundo apartado incluye, primero, la historia, etapas de formación y organización de los fondos documentales del archivo. A continuación se considera el uso instrumental para la investigación histórica que dichos fondos permiten. Un uso múltiple dada la amplitud temática de las informaciones contenidas, que van desde lo personal (expedientes personales y matrimoniales, hojas de servicios, pasaportes, pleitos, testamentarías, etc.) a lo institucional (Montepío Militar, Justicia Militar).

Abstract

Mitochondrial diseases, or diseases of the oxidative phosphorylation system, consist of a group of disorders originated by a deficient synthesis of ATP. This system is composed of proteins codified in the two genetic systems of the cell, the nuclear and the mitochondrial genomes, and, therefore, the mode of inheritance could be either mendelian or maternal. The diseases can also appear sporadically. Due to the central role that mitochondria play in cellular physiology, these diseases are a social and health problem of great importance. They are considered rare diseases; however, together they constitute a large variety of genetic disorders. It is also believed that mitochondria are involved, directly or indirectly, in many other human diseases, mainly in age-related diseases. This review will focus mainly on describing the special characteristics of the mitochondrial genetic system and the diseases caused by mitochondrial DNA mutations. We will also note the difficulties in studying these pathologies, and the possible involvement of the genetic variability of the mitochondrial genome in the development of these diseases.

Abstract

The incidence or severity of certain vaccine-preventable diseases is higher in HIV-infected individuals. However, immune responses to vaccination may be diminished, particularly in those with severe immunosuppression. Higher doses of vaccine, more frequent boosters, or revaccination after antiretroviral therapy-induced immune reconstitution are strategies to be considered for patients in certain circumstances. In addition, some vaccines may be harmful when given to severely immunocompromised patients. The challenge for healthcare providers is assessing the safety and effectiveness of vaccines for HIV-infected patients, especially when information on vaccines has not been fully characterized in the HIV-setting. This review presents state-of-the-art knowledge about immunizations for HIV-adults. The efficacy and safety of current vaccines, their current indications in HIV-infected adults, and the strategies aimed to enhance their results are discussed.

Frente a las afirmaciones de Antonio Miguel Bernal sobre la única y exclusiva función del riesgo como instrumento para la financiación de la Carrera de Indias, la legislación y los tratadistas del siglo XVIII, la documentación comercial gaditana y los autores españoles y extranjeros que han tratado la cuestión, tanto para Cádiz como para otros ámbitos, permiten concluir en la coexistencia de ambas funciones crediticias y aseguradoras en el riesgo marítimo.The ‘préstamo a la gruesa’ or ‘riesgo marítimo’ (maritime risk) always had a double function: credit and insurance. Against Antonio Miguel Bernal’s statements about the function of the ‘riesgo marítimo’ exclusively as an instrument to finance the Carrera de Indias (Spanish Colonial Trade), legislation and writers of the Eighteenth Century, Cadiz mencantile documentation of the same epoch and the authors who at present deal with this questions, both in Spain and out of Spain, both for Cadiz and for other regions, allow us to come to the conclusion that there was a permanent coexistence of the two functions (credit and insurance) in the ‘riesgo marítimo’.

Abstract

Abstract The aim of this clinical trial (ID Number NCT01803347) was to determine the safety and efficacy of autologous adipose‐derived stem cells (ASCs) for treatment of cryptoglandular fistula. This research was conducted following an analysis of the mistakes of a same previous phase III clinical trial. We designed a multicenter, randomized, single‐blind clinical trial, recruiting 57 patients. Forty‐four patients were categorized as belonging to the intent‐to‐treat group. Of these, 23 patients received 100 million ASCs plus intralesional fibrin glue (group A) and 21 received intralesional fibrin glue (group B), both after a deeper curettage of tracks and closure of internal openings. Fistula healing was defined as complete re‐epithelialization of external openings. Those patients in whom the fistula had not healed after 16 weeks were eligible for retreatment. Patients were evaluated at 1, 4, 16, 36, and 52 weeks and 2 years after treatment. Results were assessed by an evaluator blinded to the type of treatment. After 16 weeks, the healing rate was 30.4% in group A and 42.8% in group B, rising to 55.0% and 63.1%, respectively, at 52 weeks. At the end of the study (2 years after treatment), the healing rate remained at 50.0% in group A and had reduced to 26.3% in group B. The safety of the cellular treatment was confirmed and no impact on fecal continence was detected. The main conclusion was that autologous ASCs for the treatment of cryptoglandular perianal fistula is safe and can favor long‐term and sustained fistula healing.

Abstract

Merlinda Bobis was born in the Philippines but now lives in Australia, which turns her into a ‘translated’ woman, that is, a woman who has been carried across different cultures and who consequently cannot be defined by making exclusive reference to any of them. Her condition of in-betweenness provides her with a privileged perspective that allows her to talk from different angles, and thus bridge the gap, or else bring to the fore, the discontinuities that separate one world from another. The aim of this paper will be to show how “White Turtle”, the short story that gives the title to the collection of stories that Bobis published in 1999, makes use of magic realism and relies on Bakhtin’s well-known notions of heteroglossia, hybridization and polyphony to encapsulate the most pressing concerns of postcoloniality and, by extension, of the multicultural diasporic subject. 28. Clinical Differences and Viral Diversity between Newly HIV Type 1-Diagnosed African and Non-African Patients in Spain (2005–2007).

Abstract

AbstractThe diagnosis of HIV-1 is increasing in African-born persons residing in Europe. They present a high prevalence of HIV-1 non-B variant infections and of parasitic infections, both of which are infrequent in Western countries. Immigration favors their presence in nonendemic countries. In this study, all newly HIV-diagnosed individuals at an HIV/AIDS and Tropical Medicine reference center in Madrid from 2005 through 2007 were retrospectively studied. HIV-1 subtyping was performed in gag, pol,and gp41coding regions by phylogenetic analyses. The presence of other pathogens was also evaluated. Furthermore, all HIV-1-infected Africans were screened for parasitic infections. Newly diagnosed HIV-1 subjects included 90 sub-Saharan Africans and 188 non-Africans (116 Spaniards, 13 other Europeans, and 59 Latin Americans). Significantly higher numbers of HIV-1-infected Africans than non-Africans were females, acquired HIV-1 by heterosexual contact, and presented a more advanced clinical CDC stage and criteria for starting antiretroviral therapy in the first clinical visit. They predominantly carried non-B subtype infections, mainly intersubtype recombinants. Half of HIV-1-infected Africans had parasitic infections. CD4+T cell counts were lower among Africans than Europeans at the time of HIV-1 diagnosis. At 12 months of follow-up after starting antiretroviral treatment, a significantly lower proportion of Africans than non-Africans achieved undetectable viremia due to their higher loss to follow-up. However, CD4+T cell recovery and virological failure rates were similar. Therefore, the profile of African HIV-1-infected immigrants varies widely with respect to Spanish HIV-infected individuals. More advanced immunodeficiency and the coexistence of parasitic diseases and infections with a large diversity of HIV-1 non-B and recombinant variants are expected. [ABSTRACT FROM AUTHOR]

El presente artículo trata de los sistemas de captación social y de educación militar en el xviii, que partió del acceso discriminatorio y se articuló sobre dos procedimientos que coexistieron para educar a los cadetes. El modelo fue desigual promoviendo oficiales con instrucción diferente, destacando los de la academia de ingenieros de Barcelona; de artillería en Barcelona y Cádiz, y de Segovia. En el universo de la educación para la guerra, se integraron efímeros centros con innovadores planteamientos docentes; alguno focalizado premonitoriamente para todos los oficiales. La documentación oficial es definitiva para el análisis en clave de balance, desde la perspectiva del mito de las reformas militares borbónicas. Como aportación de interés y complementaria acreditamos que la educación militar entró en el debate social, dando voz a un contemporáneo, Cadalso, militar y escritor que en El buen militar a la violeta… aborda la formación del oficial y su representación social, con lo que cerramos el círculo del conocimiento sobre el modelo de educación para la guerra de los ejércitos borbónicos.
