México ha registrado ajustes en su estructura agrícola inducidos por cambios de política económica; el más reciente y relevante se llevó a cabo durante el periodo 1980-2019. La transición de una economía cerrada a una economía abierta produjo un cambio sin precedentes en la estructura productiva que persiste en la actualidad. Este trabajo tuvo como objetivo medir y comparar la variación del ingreso bruto real y el riesgo de los principales productos agrícolas de exportación, a través de los siguientes instrumentos financieros: tasas de crecimiento continuas, distribución normal, valores críticos y portafolios de inversión. Dicho análisis se realizó en dos periodos denominados como economía cerrada (1980-1999) y economía abierta (2000-2019). Se analizaron productos agrícolas seleccionados en lo individual y agrupados: berries, hortalizas, frutales y verduras. Los resultados evidencian que los riesgos fueron menores en el periodo de economía abierta para todos los productos estudiados y en la mayoría de ellos las tasas de crecimiento fueron positivas lo cual indica que sus ingresos reales registraron una tendencia creciente. El análisis por grupo de productos en el periodo de economía abierta evidenció que las berries y las verduras, son los que mejores tasas de crecimiento mostraron con menor nivel de riesgo en comparación con el periodo de economía cerrada. Las probabilidades de que los ingresos reales decrezcan en economía abierta disminuyeron, por lo que es más recomendable invertir en ambos grupos de productos agrícolas en este ambiente económico en comparación con el periodo de economía cerrada.

The diagnosis of allergic reactions to penicillins (AR-PEN) is very complex as there is a loss of sensitization over time, which leads to negative skin tests (STs) and specific IgE in serum, and even to tolerance to the drug involved. However, STs may become positive after subsequent exposure to the culprit drug (resensitization), with the risk of inducing potentially severe reactions. The exact rate of resensitization to penicillins is unknown, ranging from 0% to 27.9% in published studies.To analyze the rate of resensitization in patients with suggestive AR-PEN by repeating STs (retest) after an initial evaluation (IE).Patients with suspected AR-PEN were prospectively evaluated between 2017 and 2020. They underwent STs, and a randomized group also underwent a drug provocation test (DPT) with the culprit. Only patients with negative STs and/or DPT were included. All included cases were retested by STs at 2-8 weeks.A total of 545 patients were included: 296 reporting immediate reactions (IRs) and 249 non-immediate reactions (NIRs). Eighty (14.7%) cases had positive results in retest (RT+): 63 (21.3%) IRs and 17 (6.8%) NIRs (p 0.0001). The rate of RT+ was higher in anaphylaxis compared with all other reactions (45.8% vs 9.1%, p 0.0001). The risk of RT+ was higher from the fifth week after IE (OR: 4.64, CI: 2.1-11.6; p 0.001) and increased with the patient's age (OR: 1.02; CI: 1.01-1.04; p = 0.009).Due to the high rate of resensitization, retest should be included in the diagnostic algorithm of IRs to penicillins after an initial negative study, especially in anaphylaxis, to avoid potentially severe reactions after subsequent prescriptions of these drugs.

Introducción: El problema de investigación consistió en la identificación de medidas de capacidad, involucradas en los procesos de la práctica artesanal de la elaboración del bollo de mazorca en el barrio Pinar del Río en la ciudad de Barranquilla, en el departamento del Atlántico. Objetivo: Conocer las medidas de capacidad no convencionales presentes en la elaboración del bollo mazorca en el municipio de Barranquilla Atlántico. Materiales y métodos: La metodología empleada es de tipo cualitativa con diseño etnográfico con base en la observación participante, utilizando entrevistas semiestructuradas, con el empleo dispositivos para el registro audiovisual, lo cual se llevó a cabo en dos visitas a la artesana. Los referentes teóricos están enmarcados en el Programa Etnomatemáticas. Resultados: se evidenció en la práctica artesanal observada. El empleo de medidas de capacidad no convencionales tales como el balde, el bulto, el cucharón, la taza, entre otras; también, en esta actividad se ven inmersas otras prácticas. Conclusión: se considera la pertinencia de problematizar los resultados obtenidos en el aula de clase, como alternativa para disminuir el desinterés que en muchas ocasiones muestra el estudiante durante el desarrollo de la clase de matemáticas, en este caso el sistema de medidas de capacidad en séptimo grado. Es allí cuando la enseñanza paralela y comparativa que nos propone el enfoque didáctico del Programa Etnomatemáticas juega un papel importante, pues, llevar este tipo de prácticas con las cuales el estudiante se sienta identificado cambiaría el modo en que este desarrollará su aprendizaje.

The Social Network Analysis (SNA) is a tool for studying the structure and importance of livestock markets and their elements. The aim of the investigation was to analyze the network structure for the mobility of bovines produced in the state of Chihuahua, Mexico, between the 2010–2019 period. The Social Network Analysis methodology was used, with centrality measures at origin and destination centers taken into account. In the 2010–2019 period, the structure of bovine livestock mobility was determined by 67 centers of origin (producers) and 32 destination centers (consumers). Producers mobilized 10 012 227 heads with the intentions of exporting calves for fattening (39.2 %), pasturing (26.6 %), fattening (13.9 %), slaughtering (13.5 %) and auctioning, breeding stock, and events (6.7 %). These seven products, which comprised three destination markets: export, national, and local, determined the structure and mobility of bovine livestock. The main center of origin was the municipal area of Chihuahua, whereas the main destinations were the USA and the municipal area of Chihuahua, which are therefore considered the main elements of the network and key nodes of the dynamism of bovine livestock in the state. It was also possible to confirm that the export of calves for fattening was the main purpose of cattle mobility for the state of Chihuahua.

Se presenta caso clínico de luxofractura expuesta de rodilla y pierna derecha, asociado a fractura multifragmentaria de tibia y fíbula ipsilateral, secundario a accidente automovilístico, en paciente sin antecedentes mórbidos. La luxofractura expuesta de rodilla es una lesión infrecuente, corresponde al 0,02% de todos los traumatismos musculoesqueléticos, más frecuente en hombres y asociadas a traumas de alta energía. Si bien la literatura es escasa dado lo inusual de esta lesión, su resolución debe ser individualizada a cada paciente. En relación al paciente, se define manejo con fijación definitiva mediante tutor circular tipo Ilizarov híbrido de fémur y tibia. A los 30 meses, se realiza retiro de tutor externo, y osteosíntesis con placa de tibia proximal asociado a injerto autólogo de cresta iliaca. En controles radiológicos posteriores se observa adecuada osteointegración de injerto y consolidación satisfactoria. Tambien flexion de rodilla de 50 grados y extensión de 90 grados. El método de Ilizarov emplea técnicas biológicas mediante el aparato circular de distracción–osteogénesis y no solo corrige el defecto óseo, también permite realizar simultáneamente alargamiento, transporte óseo, compresión y corrección de deformidades angulares. Además, permite mantener la movilidad articular y deambulación precoz para estimular la formación ósea.

El estudio que de seguida se presenta tiene como objetivo fundamental realizar una relectura de la más reciente jurisprudencia comunitaria a fin de conocer cuál ha sido y es el grado de afectación en las novedades legislativas habidas en el ordenamiento jurídico español respecto a la configuración de la protección social por desempleo de quienes prestan servicios en el hogar familiar. Para alcanzar tal finalidad se ha procedido a la revisión normativa, jurisprudencial y de doctrina judicial existente sobre esta materia a nivel comunitario y nacional. The study that is presented below has as its fundamental objective to carry out a rereading of the most recent community jurisprudence to know what has been and is the degree of affectation in the legislative novelties in the Spanish legal system regarding the configuration of protection. social unemployment of those who provide services in the family home. To achieve this goal, a regulatory, jurisprudential and existing judicial doctrine review on this matter has been carried out at the community and national levels. La ley 27.348 invitó a las provincias y a la Ciudad Autónoma de Buenos Aires a adherir a las disposiciones contenida en el Título I de la ley (art. 4) . Esa adhesión importó la obligatoriedad de la intervención de las comisiones médicas jurisdiccionales con carácter previo y excluyente. Por ello los trabajadores deben necesariamente solicitar la definición de esos organismos del carácter profesional de su enfermedad o contingencia, la determinación de su incapacidad y las correspondientes prestaciones dinerarias previstas en la Ley de Riesgos del Trabajo. Algunas provincias al dictar las leyes de adhesión establecieron un plazo de caducidad para acceder a la instancia judicial. El análisis de esa caducidad es lo que se propone realizar en el presente trabajo con el objeto de verificar la compatibilidad del instituto con la Constitución Nacional y tratados internacionales que integran el Bloque de Constitucionalidad Federal (art. 75, inc. 22 de la CN). Amoxicillin (AX) combined or not with clavulanic acid (CLV) is frequently involved in IgE mediated reactions. Drug provocation test (DPT) is considered as the gold standard for diagnosis, although contraindicated in high-risk patients. Basophil activation test (BAT) can help diagnose immediate reactions to betalactams, although, controversy exists regarding the best activation marker. We have performed a real-life study in a prospective cohort to analyse the real value of BAT as diagnostic tool and the best activation marker, CD63 and CD203c, for the evaluation of immediate reactions to these drugs.We prospectively evaluated patients with a clinical suspicion of immediate reactions after AX or AX-CLV administration during a 6-years period. The allergological workup was done following the EAACI recommendations. BAT was performed in all patients using CD63 and CD203c as activation markers.In AX allergic patients, both activation markers, CD63 and CD203c, showed similar SE values (48.6% and 46.7% respectively), however specificity was of 81.1% and 94.6% respectively, with CD203c showing with good positive predictive value and like-hood ratio. In CLV allergic patients, CD203c showed higher SE (50%) than CD63 (42.9%), maintaining the same value of SP (80%). Combining results of both markers can slightly increase the sensitivity (51.4% for AX and 54.8% for CLV), although decreasing the specificity (79.7% and 73% respectively). Interestingly, all patients with an anaphylactic shock showed a positive BAT to CLV using CD203c.BAT using CD203c showed a good confirmatory power, especially for AX allergy. Placing BAT as a first step in the diagnostic procedure can help reduce the need of performing a complete allergological-work up in a 46.6% of patients, diminishing the risk of re-inducing allergic reactions.

Evidence indicates that the microbiome plays a significant role in HIV immunopathogenesis as well as in HIV-associated complications. In Argentina, men who have sex with men (MSM) and transgender women (TGW) are at the highest risk for infection with HIV and AIDS-related viruses. This study aimed to characterize the oral and anal microbiome of MSM and TGW with and without HIV. One hundred and thirty oral and anal DNA-derived samples were obtained from 78 participants and subjected to shotgun metagenomics sequencing for further microbiome analysis. Results: A significantly lower alpha diversity and higher beta diversity were found in the anal samples compared with oral samples. Significant differences in the microbiome composition were found among subjects associated with HIV infection, gender, sex behavior, CD4+ T cell counts, antiretroviral therapy (ART) and the presence of HPV-associated precancerous anal lesions. For viruses, results confirm the occurrence of oncogenic viromes in this high HIV-risk population. The HIV-associated oral microbiome was characterized by the enrichment of several bacteria related to periodontal disease pathogenesis. Conversely, predominantly anal bacteria showed a significant decrease in HIV-infected subjects (Coprococcus comes, Finegoldia magna, Blautia obeum, Catenibacterium mitsuokai). TGW showed enrichment in species related to sexual transmission, which concurs that most recruited TGW are or have been sex workers. Prevotella bivia and Fusobacterium gonidiaformans were positively associated with the presence of anal precancerous lesions among HIV-infected subjects. The enrichment of Holdemanella biformis and C. comes were associated with detectable viral load and ART untreated patients. Metabolic pathways were distinctively affected according to whether the predominant factors were associated with sexual behavior or HIV pathogenesis. Gene family analysis identified bacterial gene signatures that may potentially be prognostic and/or predictive biomarkers of HIV/AIDS-associated malignancies. Conclusions: We have identified distinctive microbial features at two easily accessible sites related to HIV immunopathogenesis. These could also be implemented as potential biomarkers to predict the risk of precancerous anal lesions or as therapeutic targets.

El objetivo del estudio es determinar la importancia de las brigadas comunitarias en los procesos de conservación y gestión de los recursos forestales en San Miguel Topilejo, Ciudad de México (CDMX). Se aplicó una encuesta a miembros de las brigadas, que fue complementada con observación directa. Los resultados indican que hay factores como la urbanización, la tala ilegal, plagas e incendios forestales que han afectado el estado del bosque. Las labores de conservación y protección de los recursos forestales están limitadas por condiciones sociales, económicas y técnicas bajo las que operan las brigadas. El valor de la investigación es que recopila la visión de los brigadistas que son la vanguardia social para la gestión de los bosques en la CDMX. En conclusión, los brigadistas consideran que han mejorado las condiciones de los bosques debido a los trabajos de reforestación, mantenimiento y vigilancia que realizan. Sin embargo, la conservación de los bosques no depende sólo de la operación eficiente de las brigadas, sino también de factores como: condiciones laborales adecuadas, capacidad de organización de la comunidad, planeación para realizar actividades de conservación, apoyo financiero y técnico, redes de colaboración, así como de factores internos y externos en torno a la gobernanza del territorio.

En las últimas décadas la Seguridad Alimentaria (SA) se ha convertido en un tema de interés mundial, siendo la escasez de alimentos y las dietas deficitarias en los hogares, un grave problema confrontado por la humanidad. Se tuvo como objetivo, analizar el papel de la ganadería en la SA de México, periodo 1994-2012, con el fin de evaluar y contrastar, la producción pecuaria y la disponibilidad de alimentos para la población nacional. Se efectuó un análisis descriptivo y comparativo de tipo multidimensional con estadísticas de fuentes oficiales, encontrándose un aumento significativo en el volumen de producción pecuaria, un alza significativa de precios y una balanza comercial sostenidamente deficitaria. Por tanto, la ganadería muestra un aporte significativo en la SA mexicana, suministrando con producción nacional los requerimientos nutricionales establecidos por el Consejo Nacional de Evaluación de la Política de Desarrollo Social (CONEVAL) para una dieta balanceada y sana.

El objetivo del trabajo fue construir a través de un análisis multivariado, grupos homogéneos de estados a partir de variables estructurales determinantes de la pobreza con el fin de identificar los tipos de pobreza pre­valentes en México que sirvan como referente para el diseño de políticas públicas diferenciadas. Con informa­ción de la Encuesta Nacional de Ocupación y Empleo (ENOE), anexos estadísticos del Consejo Nacional de la Evaluación de la Política de Desarrollo Social (CONEVAL) e indicadores estatales de México, ¿cómo vamos? se identificaron cuatro grupos estadísticamente diferenciados los cuales fueron ubicados geográficamente. Se concluye que estos grupos cuentan con estructuras y condiciones socioeconómicas distintas, generando, por tanto, un tipo de pobreza estructural distinta que requieren acciones diferenciadas con el fin de optimizar el uso de los recursos para lograr reducciones significativas en los niveles de pobreza en el mediano y largo plazo.

Se analizó la producción individual y colectiva del sistema vaca-becerro en agostaderos de uso común, con el objetivo de identificar factores que influyen en su sostenibilidad. Mediante entrevistas semiestructuradas se obtuvo información de 24 ejidos, con la técnica de paneles de ejidatarios ganaderos se recabó información económica y productiva de dos Unidades Representativas de Producción —tanto individual como colectiva—. Background: Rationale: Internal communication within an organization ensures that workers carry out their activities effectively; which, in the health context, translates into prioritizing the patients´ well-being and quality of life.Objective: to determine the relationship between internal communication and job performance in Peruvian health professionals during the COVID-19 pandemic.Methods: descriptive correlational study, carried out with the participation of 100 professionals from the maternal-infant department of a public hospital in Chimbote, Peru. Two data collection instruments were applied using online Google questionnaires, corresponding to the variables internal communication and job performance. The information was recorded and processed through descriptive statistics, through frequencies and percentages. The Statistical Package for the Social Sciences program version 26 was used, and a reliability of 95% was considered for Pearson's bivariate correlation.Results: the work performance of the maternal and child department professionals showed a high level (51%), and that of internal communication was medium (57%). In the analysis by dimensions, a direct and significant relationship was found between descending and job performance, as well as between responsibility and internal communication.Conclusion: the level of internal communication was medium, adequate with respect to the COVID-19 pandemic situation, and was directly and significantly related to job performance in the Peruvian health professionals who participated in the study.

Background: It is very important that health teams are prepared to provide care according to the required needs, and with this, meet the expectations of users. This is the basis of the interest in user care quality studies in the health context.Objective: to determine the factors associated with the quality of the service in Peruvian public hospitals.Methods: descriptive study, which included 210 patients treated in two hospitals in the province of Santa, Peru. A 22-item instrument based on the SERVQUAL model was used. Inferential analysis was performed based on the normality of the data, to establish a significant difference between the dimensions of the quality of service for the hospitals under study. Likewise, to evaluate the association between the factors and the quality of the service, the Pearson bivariate correlation test was used.Results: the level of quality was high (62%) and medium (78%) for the La Caleta and Eleazar Guzmán Barrón hospitals, respectively. There was a difference in the quality of the service, represented by the dimensions of reliability, responsiveness, security, empathy and tangible elements. In the correlation analysis, only in the Eleazar Guzmán Barrón Hospital was a significant and positive association found between the type of user and the quality of service.Conclusion: the quality of the service provided in outpatient clinics behaves in a variable way among public hospitals, being only dependent on the type of user attended.

Las empresas agrícolas familiares tienen un importante papel sobre el mantenimiento a largo plazo de la economía agrícola en las zonas rurales, por su conocimiento de la producción local, capacidad de adaptación, y transmisión de conocimientos de una generación a otra. Estos aspectos implican una estrecha relación entre capacidades, potencialidades y objetivos relativos al desarrollo desde el punto de vista socioeconómico. El estudio analiza dichas cuestiones desde ese marco teórico, tomando como referencia el caso específico el desarrollo de la agricultura en Nayarit. Se considera la edad del agricultor, ingresos de la empresa agrícola, trabajadores remunerados, tasa de empleo y multiculturalismo como indicadores del logro de objetivos socioeconómicos. El trabajo se desarrolló de septiembre 2015 a enero 2016; se aplicó encuesta a 91 productores. Los datos fueron analizados mediante modelos de regresión lineal múltiple y regresión Poisson. Los resultados muestran que la edad de los agricultores, ingreso, empleo generado y la multiculturalidad que implica la actividad económica agrícola, son influenciados por: i) la escala de la explotación y existencia de industrias auxiliares; ii) la proactividad a innovar, el nivel de educación y la consideración de heredar la empresa; y iii) la participación de mujeres en las decisiones de la empresa.

Background: The evaluation policies that currently govern the Peruvian educational system may be contributing to an increase in the levels of pressure and work stress of teachers, which could also negatively affect their professional performance and the organizational climate.Objective: to determine the relationship between work stress and the organizational climate in Peruvian teachers.Methods: descriptive and correlation study, carried out with the participation of 140 teachers. Two research instruments were used: the Maslash Burnout Inventory and the Organizational Climate Scale; the first, taking into account the dimensions: emotional exhaustion, depersonalization and personal fulfillment; and the second, in correspondence with eight other dimensions. The analysis was carried out using Spearman's correlation, considering a 5% margin of error.Results: a predominance of the medium level (80.7%) of work stress was found; and of the high level (78.6%) for the organizational climate. The correlation analysis showed a negative, highly significant association between the study variables.Conclusion: The inversely proportional relationship between the study variables allowed us to infer that an adequate organizational climate should be promoted in educational institutions; that it be inclusive and present sustainability over time; that allows reducing the manifestations of stress that arise daily in the working day.

Con el objetivo de identificar factores de éxito y debilidades en la cobertura de seguros catastróficos y las características de su distribución regional en México, se trabajaron datos de unidades animal protegidas, primas aportadas por los productores, gobierno federal y gobiernos estatales y suma asegurada. Se encontró que con recursos aportados en un 83% de subsidio y 17% de los productores, cinco instituciones de seguros y un fondo de aseguramiento protegieron hasta 83 millones de Unidades Animal de la totalidad de los productores inscritos en el Padrón Ganadero Nacional. Con análisis de conglomerados se obtuvieron tres grupos de entidades federativas y con el análisis de varianza se encontraron diferencias estadísticas significativas (p < 0.05) en monto de primas, valor protegido y número de declaratorias de emergencia y desastre. Se identificaron como factores de éxito la flexibilidad del programa para incorporar esquemas de seguros y el subsidio de la prima con recursos públicos.

Abstract: Drug hypersensitivity is increasing worldwide as the consumption of drug is increasing. Many clinical presentations of drug hypersensitivity are complex and take place in the setting of illness and/or polypharmacotherapy. To review the most recent findings in the diagnosis and management of immediate drug hypersensitivity reactions. Studies were selected based on their relevance, originality and date of publication. The understanding of endotypes, biomarkers and phenotypes has improved the categorization of immediate hypersensitivity reactions. In this review, we discussed the short- and long-term management of anaphylaxis with a special focus on in vivo and in vitro diagnostic methods. Moreover, the clinical management of drug-induced anaphylaxis, the role of hidden allergens and the importance of delabeling are discussed. Endophenotyping is crucial to correctly diagnose and treat patients with immediate drug hypersensitivity reactions, preventing future episodes through drug desensitization. A partir de la emergencia sanitaria dictada en el marco de la pandemia producida por COVID-19, observamos que el art. 223 bis de la LCT fue adquiriendo una nueva configuración que le ha dado identidad propia. Con el análisis de los antecedentes y las nuevas disposiciones aplicables al instituto se busca facilitar su aplicación sin perjuicio de la problemática práctica que su ejecución plantea y que determina la necesidad de continuar con el debate constructivo respecto de una regulación que responda en mejores términos a la realidad laboral existente. Objective The primary objective was to evaluate the prevalence of Salmonella spp. and Escherichia coli O157 in feces of Cervus elaphus scoticus during summer and winter of 2015 to 2017. Additionally, the seasonality and environmental conditions were evaluated to investigate association with pathogen shedding. Materials and Methods Preliminary evaluation of Salmonella spp. detection was achieved by PCR-amplification of the invA, spvR, and sopB genes, whereas for E. coli O157, the eae and rfbE genes were amplified. Candidate genes with low limit detection were not used to evaluate the prevalence of target pathogens. Results and Discussion Red deer fecal samples (n = 89) were evaluated to detect invA and rfbE genes. The results showed that E. coli O157 prevalence was 0%, due to rfbE being absent; nevertheless, Salmonella spp. prevalence identified through the invA gene amplification was 56.2%, with the least prevalence during winter 2017. A positive correlation was observed between bacterial prevalence, atmospheric humidity, and temperature. Implications and Applications The results showed that E. coli O157 was not detected in the stool of captive red deer; however, Salmonella spp. were present. Therefore, good practices in food handling and hygiene are indispensable to avoid the emergence of foodborne diseases.
