The Coronavirus Disease 2019 (COVID-19) can affect mental health in different ways. There is little research about psychiatric complications in hospitalized patients with COVID-19. The aim of the study was to describe the psychiatric clinical profile and pharmacological interactions in COVID-19 inpatients referred to a Consultation-Liaison Psychiatry (CLP) unit. This is a cross-sectional study, carried out at a tertiary hospital in Spain, in inpatients admitted because of COVID-19 and referred to our CLP Unit from March 17,2020 to April 28,2020. Clinical data were extracted from electronic medical records. The patients were divided in three groups depending on psychiatric diagnosis: delirium, severe mental illness (SMI) and non-severe mental illness (NSMI). Of 71 patients included (median [ICR] age 64 [54–73] years; 70.4% male), 35.2% had a delirium, 18.3% had a SMI, and 46.5% had a NSMI. Compared to patients with delirium and NSMI, patients with SMI were younger, more likely to be institutionalized and were administered less anti-COVID19 drugs. Mortality was higher among patients with delirium (21.7%) than those with SMI (0%) or NSMI (9.45%). The rate of side effects due to interactions between anti-COVID19 and psychiatric drugs was low, mainly drowsiness (4.3%) and borderline QTc prolongation (1.5%). Patients affected by SMI were more often undertreated for COVID-19. However, the rate of interactions was very low, and avoidable with a proper evaluation and drug-dose adjustment. Half of the patients with SMI were institutionalized, suggesting that living conditions in residential facilities could make them more vulnerable to infection.

Resumen Introduccion Las diferencias de genero en la asistencia a consultas por trastorno delirante han sido escasamente estudiadas. Ademas, el trastorno delirante es tradicionalmente considerado como un trastorno resistente al tratamiento. Material y metodos Se llevo a cabo un estudio retrospectivo longitudinal que incluyo 78 pacientes con trastorno delirante (DSM- IV -TR) que ingresaron consecutivamente en nuestro centro en un periodo de 10 anos. Se realizo un seguimiento de los pacientes durante un ano con el objetivo de describir variables demograficas y clinicas y comparar la efectividad terapeutica. En este estudio, debido a su naturaleza no intervencionista, se realizo un analisis de las medidas de efectividad indirecta. La muestra se dividio en 3 grupos segun el tratamiento antipsicotico recibido en el primer ingreso. Resultados Veintitres pacientes recibieron risperidona inyectable de accion prolongada (RILD), 30 risperidona oral y 25 otros antipsicoticos atipicos orales. Las mujeres con trastorno delirante presentaron un inicio mas tardio del trastorno y requirieron un mayor tiempo de hospitalizacion. No se encontraron diferencias estadisticamente significativas entre los 3 grupos de tratamiento con respecto a los datos demograficos, al funcionamiento social y personal, asi como a la psicopatologia en el ingreso. En el seguimiento durante un ano el grupo RILD mostro un mayor porcentaje de permanencia al final del periodo evaluado. Ademas, el grupo RILD requirio con menor frecuencia un tratamiento con antidepresivos y benzodiazepinas. Conclusiones Concluimos que el tratamiento con RILD podria disminuir las perdidas en el seguimiento del trastorno delirante en comparacion con otros antipsicoticos atipicos.

Introduction: For many decades, clinical and epidemiological associations between gynaecological illness and psychia- tric disorders have been reported. Thus, in recent years, special attention has been paid to the gynaecological health of women with chronic psychosis.We aimed to determine the rates of oestrogen and non-oestrogen dependent gynae- cological disorders in women diagnosed with delusional disorder (DwDD), and its relationship with clinical outcomes. Methods: We conducted a longitudinal and observational study in 90 women DwDD who attended our Department of Psychiatry, from 2000 until 2013. Subjects were selected by consecutive sampling and followed-up for one year. The presence or absence of inflammatory and non inflammatory-gynaecological disorders was recorded according to ICD-10 criteria, and attendance rates at appointments (psychiatric and nursing) were collected. For comparisons, T and Chi-square tests were performed. Multinomial logistic regression was applied to investigate multivariate differences. Results: Thirty-six (40%) women DwDD had a history of gynaecological disease (oestrogen dependent disorders: 36.7%). Women diagnosed with a gynaecological disease had higher unemployment rates than those without gynae- cological conditions ( p = .03). Patients without gynaecological disorders attended later our psychiatric service and were more frequently admitted ( p = .048). Within the gynaecological group, leiomyoma of uterus (n = 9; 10%) was the most common condition followed by misscarriage (n = 6; 6.7%). Hysterectomy (n = 7; 7.8%) was the most common surgical treatment. After one year, women diagnosed with gynaecological illness showed a tendency to lower rates of affective comorbidity and less antidepressant prescription. After controlling for age at first appointment and admissions, differences in affective comorbidity remained significant ( p = .020); however, differences in antidepressant prescription and employment status were no longer significant. Conclusions: Women DwDD might be at particular risk of non-inflammatory oestrogen dependent gynaecological disorders. The role of mental health nurses should be addressed to improve attendance rates at gynaecological appoint- ments and prevention programs.

Introduction Gender differences in attendance rates for appointments in delusional disorder have been poorly studied. Furthermore, delusional disorder is traditionally considered a treatment-resistant disorder. Material and methods We conducted a longitudinal retrospective study with a one-year follow-up, including 78 delusional disorder patients (DSM- IV -TR), consecutively admitted over a period of 10 years. We performed a follow-up for one year in order to describe demographic and clinical variables, and to compare the therapeutic effectiveness. In this study, due to the non-interventional nature of this study, indirect measures were used to measure treatment effectiveness. The sample was divided into three groups according to the antipsychotic received at the first admission. Results Twenty-three patients received Risperidone Long-Acting Injection (RILD), 30 oral risperidone, and 25 patients received other oral atypical antipsychotics. Delusional disorder women had a later age at onset and needed a longer duration of hospitalisation. No statistically significant differences were found between the three treatment groups as regards demographic data, social and personal functioning, and psychopathology at admission. The RILD group showed higher maintenance rates at the end of the follow-up period. Furthermore, the RILD group required treatment with antidepressants and benzodiazepines less often. Conclusions We concluded that treatment with RLAI may increase maintenance rates at follow-up in DD patients when compared with other oral atypical antipsychotics.
