Abstract: Live-cell imaging generally requires pretreatment with fluorophores to either monitor cellular functions or the dynamics of intracellular processes and structures. We have recently introduced full-field optical coherence tomography for the label-free live-cell imaging of fungi with potential clinical applications for the diagnosis of invasive fungal mold infections. While both the spatial resolution and technical set up of this technology are more likely designed for the histopathological analysis of tissue biopsies, there is to our knowledge no previous work reporting the use of a light interference-based optical technique for direct mycological examination and monitoring of intracellular processes. We describe the first application of dynamic full-field optical transmission tomography (D-FF-OTT) to achieve both high-resolution and live-cell imaging of fungi. First, D-FF-OTT allowed for the precise examination and identification of several elementary structures within a selection of fungal species commonly known to be responsible for invasive fungal infections such as Candida albicans, Aspergillus fumigatus, or Rhizopus arrhizus. Furthermore, D-FF-OTT revealed the intracellular trafficking of organelles and vesicles related to metabolic processes of living fungi, thus opening new perspectives in fast fungal infection diagnostics.

Abstract: Abstract Biofilm (BF) growth is believed to play a major role in the development of ventilator-associated pneumonia (VAP) in the intensive care unit. Despite concerted efforts to understand the potential implication of endotracheal tube (ETT)-BF dispersal, clinically relevant data are lacking to better characterize the impact of its mesostructure and microbiological singularity on the occurrence of VAP. We conducted a multicenter, retrospective observational study during the third wave of the COVID-19 pandemic, between March and May 2021. In total, 64 ETTs collected from 61 patients were included in the present BIOPAVIR study. Confocal microscopy acquisitions revealed two main morphological aspects of ETT-deposited BF: (1) a thin, continuous ribbon-shaped aspect, less likely monobacterial and predominantly associated with Enterobacter spp., Streptococcus pneumoniae or Viridans streptococci, and (2) a thicker, discontinuous, mushroom-shaped appearance, more likely characterized by the association of bacterial and fungal species in respiratory samples. The microbiological characterization of ETT-deposited BF found higher acquired resistance in more than 80% of analyzed BF phenotypes, compared to other colonization sites from the patient’s environment. These findings reveal BF as a singular microbiological compartment, and are of added clinical value, with a view to future ETT-deposited BF-based antimicrobial stewardship in critically ill patients. Abstract: IntroductionThe diagnosis of cutaneous manifestations of deep mycoses relies on both histopathological and direct examinations. Yet, the current diagnostic criteria cannot prevent missed cases, including invasive aspergillosis, which requires the development of a novel diagnostic approach and imaging tools. We recently introduced the use of dynamic full-field optical coherence tomography (D-FF-OCT) in fungal diagnostics with a definition approaching that of conventional microscopy and the ability to return metabolic information regarding different fungal species. The present work focuses on subcellular dynamics and live-cell imaging of Aspergillus fumigatus with D-FF-OCT to follow the fungal growth stages.MethodsThe A. fumigatus ATCC 204305 quality-control strain was used for all imaging experiments, following incubation times varying between 24 and 72 h at 30°C in a humidified chamber on Sabouraud dextrose agar. Fungal growth was subsequently monitored with D-FF-OCT for up to 5 h at room temperature and following the pharmacological stress of either voriconazole, amphotericin B, or caspofungin gradient concentration.ResultsD-FF-OCT images allow not only the visualization of intracellular trafficking of vacuoles but also an evolving dynamic segmentation of conidiophores depending on the chronological development and aging of the hyphae or the effect of antifungal treatment. The same applies to conidial heads, with the most intense D-FF-OCT signal coming from vesicles, revealing a changing dynamic within a few hours only, as well as complete extinction following subsequent drying of the Sabouraud dextrose agar.DiscussionThese results provide additional data on the ability of D-FF-OCT to monitor some of the main life cycle processes, dynamics, and intracellular trafficking of vacuoles in A. fumigatus, with or without the effect of pharmacological stress. Such complementary metabolic information could help both clinicians and microbiologists in either mechanistic studies toward experimental mycology or the development of a potential D-FF-OCT-guided diagnosis of superficial fungal infections.

Abstract: Abstract Candida albicans is the most common human fungal pathogen. This yeast is described as a commensal of human and animal mucosa, and very few studies have focused on its isolation in natural environments. We investigated the presence of C. albicans in a large panel of French soils. Because a culture‐based method didn't allow isolation of the yeast in a panel of 70 soils, we adapted a nested‐PCR for detecting C. albicans DNA in a panel of 460 soils. Only 7 of the 460 soil samples (1.5%) were PCR‐positive for Candida albicans. To understand which parameters influence the survival of the yeast, we studied the decline of a population of C. albicans over a period of one month in a collection of 20 soils collected throughout France. C. albicans was able to survive up to 30 days in 80% of the soils tested. Using a Spearman correlation test, we showed that the short‐term survival of C. albicans in soils was correlated with some soils chemical factors such as pH and presence of some minerals (Al, Mn, and Na). Concerning survival after 30 days, Cation Exchange Capacity (CEC) and clay content were identified as beneficial determinants of the long‐term survival of C. albicans in soils.

Abstract: Abstract Background External ophtalmomyiasis (EOM) is a zoonosis related to the presence of Oestrus ovis larvae at the ocular level in small ruminants (i.e. ovine, caprine). In humans, EOM is a rare cosmopolitan disorder, mostly described in warm and dry rural areas in patients living close to livestock areas. In metropolitan France (excluding Corsica), EOM is an exceptional disease with less than 25 cases recorded since 1917. Case presentation We report a case of EOM in a 19-years old man in the last week of September 2016 in Burgundy. Conclusion The diagnosis of an EOM in Burgundy, a French region described as cold and humid, is surprising and could be due to a more marked climatic warming during the vegetative season in Burgundy resulting in the implantation of Diptera of the genus Oestrus sp. in this region.

Abstract: Fusarium spp. are widespread environmental fungi as well as pathogens that can affect plants, animals and humans. Yet the epidemiology of human fusariosis is still cloudy due to the rapidly evolving taxonomy. The Mass Spectrometry Identification database (MSI) has been developed since 2017 in order to allow a fast, accurate and free-access identification of fungi by matrix-assisted laser desorption ionization—time of flight (MALDI-TOF) mass spectrometry. Taking advantage of the MSI database user network, we aim to study the species distribution of Fusarium spp. isolates in an international multicenter prospective study. This study also allowed the assessment of the abilities of miscellaneous techniques to identify Fusarium isolates at the species level. The identification was performed by PCR-sequencing and phylogenic-tree approach. Both methods are used as gold standard for the evaluation of mass spectrometry. Identification at the species complex was satisfactory for all the tested methods. However, identification at the species level was more challenging and only 32% of the isolates were correctly identified with the National Center for Biotechnology Information (NCBI) DNA database, 20% with the Bruker MS database and 43% with the two MSI databases. Improvement of the mass spectrometry database is still needed to enable precise identification at the species level of any Fusarium isolates encountered either in human pathology or in the environment.

Abstract: Human cryptosporidiosis remains underdiagnosed, and rapid/accurate diagnosis is of clinical importance. Diagnosis of the Cryptosporidium oocyst in stool samples by conventional microscopy is labor-intensive, time-consuming, and requires skillful experience. Thus, we aimed to evaluate the usefulness of a coproantigen enzyme-linked immunosorbent assay (ELISA) test in detecting Cryptosporidium spp. from fecal specimens. For this aim, we evaluated the performances of a commercial ELISA (CoproELISA Cryptosporidium kit, Savyon Diagnostics, Israel) for the detection of Cryptosporidium spp. in random clinical stool samples through a multicenter study. The sensitivity and specificity for coproantigen ELISA were 98.86% and 94.32%, respectively. The coproantigen ELISA results indicate that the simple, rapid, reliable, and standardized immunoassay test is sensitive and specific for routine diagnosis, and may be useful for large-scale epidemiological studies of cryptosporidiosis.

Abstract: Cryptosporidiosis is currently recognized worldwide as a leading cause of moderate to severe diarrhea. In Europe, large water- and foodborne outbreaks have been reported, highlighting the widespread distribution of the parasite and its important health impact. Surveillance networks have been progressively set up and the aim of this study was to present recent epidemiological data obtained in France from 2017 to 2019 by the National Reference Center—Expert Laboratory of cryptosporidiosis (Centre National de Référence–Laboratoire Expert cryptosporidioses CNR-LE). Data were obtained from online reports of volunteer network participants and stools were sent to the CNR-LE for species identification and GP60 genotyping. During this period, data from 750 online reports were available. Cryptosporidiosis occurred predominantly in young children (Cryptosporidium parvum was largely predominant (72% of cases) over C. hominis (24%) and some other uncommon species. C. parvum GP60 subtypes IIa and IId were the most represented, which suggests frequent zoonotic transmission. For C. hominis, subtypes IbA10G2 and IaA22R2 were predominant.

Abstract: Cell wall proteins are central to the virulence of Candida albicans. Hwp1, Hwp2 and Rbt1 form a family of hypha-associated cell surface proteins. Hwp1 and Hwp2 have been involved in adhesion and other virulence traits but Rbt1 is still poorly characterized. To assess the role of Rbt1 in the interaction of C. albicans with biotic and abiotic surfaces independently of its morphological state, heterologous expression and promoter swap strategies were applied. The N-terminal domain with features typical of the Flo11 superfamily was found to be essential for adhesiveness to polystyrene through an increase in cell surface hydrophobicity. A 42 amino acid-long domain localized in the central part of the protein was shown to enhance the aggregation function. We demonstrated that a VTTGVVVVT motif within the 42 amino acid domain displayed a high β-aggregation potential and was responsible for cell-to-cell interactions by promoting the aggregation of hyphae. Finally, we showed through constitutive expression that while Rbt1 was directly accessible to antibodies in hyphae, it was not so in yeast. Similar results were obtained for another cell wall protein, namely Iff8, and suggested that modification of the cell wall structure between yeast and hyphae can regulate the extracellular accessibility of cell wall proteins independently of gene regulation.

Abstract: Histopathology and microscopic examination of infected tissue are the gold standards to prove the diagnosis of invasive fungal infection (IFI). Yet, they suffer from essential limitations that hamper rapid diagnosis and require the future development of new imaging tools dedicated to fungal diagnostics. To this end, the present work introduces the first use of dynamic full-field optical coherence tomography (D-FF-OCT) for the visualization of microscopic filamentous fungi. Data collected from the observation of three different fungal species (Nannizzia gypsea, Aspergillus fumigatus and Rhizopus arrhizus) confirm the ability of D-FF-OCT to visualize not only the main structures of all selected fungal species (hyphae, spores, conidia, sporulating structures), but also the metabolic activity of the organisms, which could provide additional help in the future to better characterize the signature of each fungal structure. These results demonstrate how D-FF-OCT could serve as potential complementary tool for rapid diagnosis of IFI in both intensive and non-intensive care units.

Abstract: We determined the susceptibility of 182 Fusarium species isolates to five antifungal drugs (amphotericin B, voriconazole, posaconazole, isavuconazole, and terbinafine) by the EUCAST method. Based on the latest taxonomic insights, isolates collected from 20 European centers were distributed into seven complexes and 27 species. The susceptibility was variable, depending on the species. Comparison with the gradient concentration strip method, which was used for 77 isolates, showed essential agreement values for voriconazole, posaconazole, isavuconazole, and amphotericin B of 17%, 91%, 83%, and 70%, respectively.

Abstract: Fungemia is a severe invasive fungal infection that combines rapid progression and a high mortality rate. This type of infection is a vital emergency, and early diagnosis is crucial. Currently, only the BD-BACTEC® Automated Blood Culture System (Becton Dickinson, New Jersey, USA) has a medium specifically dedicated to the detection of fungal agents: the BD-BACTEC®MycosisIC/F bottle.Thus, it is important to assess the performance of the different bottles offered by the BD-BACTEC® Automated Blood Culture System for the diagnosis of fungemia.The aim of this study was to evaluate the performance of the BD-BACTEC® MycosisIC/F culture medium in comparison to bacteriologic culture bottle media for the detection of fungemia in different clinical situations.This retrospective study was conducted over a period of 4 years at the Dijon University Hospital. Three hundred and thirty-one pairs of blood cultures (i.e. a BD-BACTEC® MycosisIC/F culture bottle associated with at least one bacteriologic culture media bottle) were included in this study.We showed that the BD-BACTEC® MycosisIC/F culture bottles performed significantly better (i.e. diagnostic advantage either because it was the only positive bottle of the pair or time to positive result was shorter) than the bacteriological culture bottles in 57.7% (191/331) of cases (p0.01). Multivariate analysis revealed that BD-BACTEC®MycosisIC/F bottles had better diagnostic performance than BD-BACTEC®Bacteriologic bottles in the context of: (i) the initial versus follow-up diagnostic subgroup, (ii) venipuncture or arterial sampling versus other sampling methods, and (iii) detection of filamentous versus yeast fungi.We concluded that the use of BD-BACTEC® MycosisIC/F culture bottles is a relevant addition to media optimized for routine bacterial detection.

Abstract: Fusarium spp. are widespread environmental fungi as well as pathogens that can affect plants, animals and humans. Yet the epidemiology of human fusariosis is still cloudy due to the rapidly evolving taxonomy. The Mass Spectrometry Identification database (MSI) has been developed since 2017 in order to allow a fast, accurate and free-access identification of fungi by matrix-assisted laser desorption ionization—time of flight (MALDI-TOF) mass spectrometry. Taking advantage of the MSI database user network, we aim to study the species distribution of Fusarium spp. isolates in an international multicenter prospective study. This study also allowed the assessment of the abilities of miscellaneous techniques to identify Fusarium isolates at the species level. The identification was performed by PCR-sequencing and phylogenic-tree approach. Both methods are used as gold standard for the evaluation of mass spectrometry. Identification at the species complex was satisfactory for all the tested methods. However, identification at the species level was more challenging and only 32% of the isolates were correctly identified with the National Center for Biotechnology Information (NCBI) DNA database, 20% with the Bruker MS database and 43% with the two MSI databases. Improvement of the mass spectrometry database is still needed to enable precise identification at the species level of any Fusarium isolates encountered either in human pathology or in the environment.

Abstract: International audience; A survey of mycology laboratories for antifungal susceptibility testing (AFST) was undertaken in France in 2018, to better understand the difference in practices between the participating centers and to identify the difficulties they may encounter as well as eventual gaps with published standards and guidelines. The survey captured information from 45 mycology laboratories in France on how they perform AFST (number of strains tested, preferred method, technical and quality aspects, interpretation of the MIC values, reading and interpretation difficulties). Results indicated that 86% of respondents used Etest as AFST method, with a combination of one to seven antifungal agents tested. Most of the participating laboratories used similar technical parameters to perform their AFST method and a large majority used, as recommended, internal and external quality assessments. Almost all the participating mycology laboratories (98%) reported difficulties to interpret the MIC values, especially when no clinical breakpoints are available. The survey highlighted that the current AFST practices in France need homogenization, particularly for MIC reading and interpretation.

Abstract: International audience; Cryptosporidiosis is a common disease in children and immunodeficient individuals. In 2006, a national network was set up on the surveillance of human cryptosporidiosis in France. Since January 2015, the 41 tertiary care hospitals and the 3 private laboratories of the French National Network on the surveillance of human cryptosporidiosis have been able to declare confirmed cases of cryptosporidiosis online. Between 2015 and 2017, 210 cases of cryptosporidiosis were declared in immunodeficient patients in France; Cryptosporidium parvum and Cryptosporidium hominis represented 66% and 22% of cases, respectively. A peak was observed in autumn. Cryptosporidiosis occurred mainly in a context of solid organ transplantation (SOT) (49%) and of HIV infection (30%). In SOT recipients, cryptosporidiosis appeared more frequently in the first 6 months post transplantation. Regarding cases declared in SOT recipients, mycophenolate mofetil was used in 68%. A mortality rate of 6% was observed. Present results underline the importance of screening for cryptosporidiosis in immunocompromised patients suffering from diarrhea, especially in the course of major cell mediated immunodeficiency or even systematic screening before SOT. Exclusive Cryptosporidium free water feeding could be suggested during major cell mediated immunodeficiency.

Abstract: IF 1.606 (2017); International audience; Trichoderma species are saprophytic filamentous fungi that can be found all over the word. These fungi show increasing medical importance as opportunistic human pathogens, particularly in immunocompromised patients. Invasive infections due to Trichoderma are rare and definitive diagnosis is complex to achieve because of the lack of specific diagnosis tools. We report in this work the first proven case of invasive pulmonary infection due to T. longibrachiatum in a 69-year-old white male with hematologic malignancy. The patient was successfully treated initially with voriconazole alone followed by a combination of voriconazole and caspofungine.

Abstract: Background External ophtalmomyiasis (EOM) is a zoonosis related to the presence of Oestrus ovis larvae at the ocular level in small ruminants (i.e. ovine, caprine). In humans, EOM is a rare cosmopolitan disorder, mostly described in warm and dry rural areas in patients living close to livestock areas. In metropolitan France (excluding Corsica), EOM is an exceptional disease with less than 25 cases recorded since 1917. Case presentation We report a case of EOM in a 19-years old man in the last week of September 2016 in Burgundy. Conclusion The diagnosis of an EOM in Burgundy, a French region described as cold and humid, is surprising and could be due to a more marked climatic warming during the vegetative season in Burgundy resulting in the implantation of Diptera of the genus Oestrus sp. in this region. Abstract: Candida albicans is the most frequent yeast responsible for systemic infections in humans. These infections mainly originate from the gastrointestinal tract where C. albicans can invade the gut epithelial barrier to gain access to the bloodstream. Along the gut, pathogens can use Microfold (M) cells as a portal of entry to cross the epithelial barrier. M cells are specialized cells mainly located in the follicule-associated epithelium of Peyer patches. In this study, we used scanning electron and fluorescence microscopy, adhesion and invasion assays and fungal mutants to investigate the interactions of C. albicans with M cells obtained in an established in vitro model whereby enterocyte-like Caco-2 cells co-cultured with the Raji B cell line undergo a phenotypic switch to morphologically and functionally resembling M cells. Our data demonstrate that C. albicans co-localizes with and invades preferentially M cells, providing evidence that the fungus can use M cells as a portal of entry into the intestinal barrier. In addition to active penetration, F-actin dependent endocytosis contributes to internalization of the fungus into M cells through a mechanism involving hypha-associated invasins including Ssa1 and Als3.

Abstract: Pneumocystis jirovecii (Pj) est un champignon pathogene opportuniste strictement humain. Il colonise transitoirement et sans symptome l’arbre respiratoire des patients immunocompetents, et peut etre responsable d’infections pulmonaires severes chez les patients immunodeprimes. Le diagnostic biologique de pneumocystose repose sur la mise en evidence du pathogene a l’examen direct d’un prelevement pulmonaire profond. Si l’examen direct manque souvent de sensibilite, de nombreuses techniques de PCR se sont developpees pour ameliorer la sensibilite de detection du champignon. Les techniques de PCR quantitatives (qPCR) permettent de quantifier la charge fongique dans le prelevement, et potentiellement de differencier la colonisation de l’arbre respiratoire par Pj d’une veritable infection generalement associee a une charge fongique plus elevee. L’objectif de notre travail etait d’evaluer les performances du kit MycoGENIE ® P. jirovecii (Ademtech) dans le diagnostic de la pneumocystose en comparaison avec la technique de qPCR utilisee en routine au laboratoire de parasitologie-mycologie du CHU de Dijon. Soixante-dix-huit echantillons respiratoires (72 LBA, 3 ATP, 3 crachats) ont ete testes en parallele : 46 echantillons retrospectifs (preleves entre janvier 2016 et juin 2016 et conserves a −80 °C) et 32 echantillons prospectifs preleves entre juillet 2016 et septembre 2016. Les 78 echantillons ont ete extraits par l’automate Magtration System 12GC ® (Bionobis). Les deux techniques de PCR utilisees dans notre etude ciblaient la meme sequence d’ADN de 79 pb du gene mitochondrial codant la grosse sous-unite de l’ARN ribosomal (mtLSU rRNA). Les PCR ont ete realisees sur l’automate LC480 II (Roche Diagnostics). Le test MycoGENIE ® Pj contenait une gamme etalon permettant de quantifier la charge fongique du prelevement en nombre de copies/mL ainsi qu’un controle interne d’inhibition. Le kit est marque CE-IVD. Sur les 46 echantillons testes retrospectivement, 36 etaient positifs et 9 etaient negatifs par les deux techniques. Un seul resultat etait discordant (negatif avec la technique de notre laboratoire et positif avec le kit MycoGENIE ® Pj ). Les resultats des 32 echantillons testes prospectivement etaient concordants (12 positifs et 20 negatifs). La concordance entre les 2 tests est donc de 98,7 %. Aucun inhibiteur n’a ete detecte avec le kit MycoGENIE ® Pj . La detection de l’ADN a ete plus precoce pour l’ensemble des echantillons positifs avec le kit MycoGENIE ® Pj , avec un seuil de detection plus precoce en moyenne de 1,2 cycle. Le resultat discordant concernait un patient atteint d’une leucemie aigue myeloblastique en aplasie post-inductive intensive pour lequel la radiologie et la clinique orientaient le diagnostic vers une pneumopathie interstitielle hypoxemiante et bilaterale. La quantification de l’ADN n’a pas permis de definir de seuil pour differencier pneumocystose maladie et colonisation : des taux tres faibles ont ete retrouves chez des patients dont le diagnostic retenu etait une pneumocystose ; a l’inverse, des taux eleves d’ADN de Pj etaient observes chez des patients consideres comme colonises. D’autres elements comme le terrain, l’aspect radiologique, la clinique et d’autres marqueurs biologiques ( ex. β -glucanes) devraient etre pris en compte pour preciser le diagnostic de pneumocystose ou de colonisation a Pj.

Abstract: Recent literature has shown the growing importance of opportunistic fungal infections due to Fusarium spp. However, disseminated fusariosis remains rare in patients without neutropenia. We report a case of fungaemia in a 78-year-old French woman without definite immunodeficiency. Fusarium proliferatum grew from both central and peripheral blood cultures. Fever was the only clinical sign of the infection. An appropriate antifungal therapy with voriconazole led to the recovery of the patient. An environmental investigation was undertaken but failed to find a reservoir of Fusarium spores. A contaminated central venous catheter might have been the source of fungaemia.

Abstract: Background Cryptosporidium spp. is a ubiquitous parasite affecting humans as well as domestic and wild vertebrates, causing diarrhea in both immunocompetent and immunocompromised hosts worldwide. Its transmission occurs primarily by the fecal-oral route. In humans, C. parvum and C. hominis are the most prevalent species, whereas immunocompetent and immunocompromised individuals can also be infected by other zoonotic species. Renal transplant patients are prone to develop cryptosporidiosis, which can induce severe and life-threatening diarrhea. Case presentation We report here a series of nearly concomitant cases of acute symptomatic cryptosporidiosis in three renal transplant patients attending the Strasbourg University Hospital Nephrology Unit. The clinical presentation was persistent diarrhea and acute renal failure. The diagnosis was confirmed by microscopic stool examination using a modified Ziehl-Neelsen staining method and species identification by molecular tools. All patients were treated with nitazoxanide and recovered from diarrhea after 14 days of therapy. Conclusion Genotypic species identification was not consistent with an epidemic context, thus underlining the need for genotyping to monitor at risk patients.

Abstract: In 23 leukemia patients with proven (n = 17) or possible (n = 6) pulmonary mucormycosis (PM), the presence of reversed halo sign on computed tomography was strongly associated with the positivity of quantitative polymerase chain reaction assays targeting Mucorales in the serum, confirming the value of these two tools for the diagnosis of PM in this setting.

Abstract: Fusarium oxysporum is typically a soilborne fungus but can also be found in aquatic environments. In hospitals, water distribution systems may be reservoirs for the fungi responsible for nosocomial infections. F. oxysporum was previously detected in the water distribution systems of five French hospitals. Sixty-eight isolates from water representative of all hospital units that were previously sampled and characterized by translation elongation factor 1α sequence typing were subjected to microsatellite analysis and full-length ribosomal intergenic spacer (IGS) sequence typing. All but three isolates shared common microsatellite loci and a common two-locus sequence type (ST). This ST has an international geographical distribution in both the water networks of hospitals and among clinical isolates. The ST dominant in water was not detected among 300 isolates of F. oxysporum that originated from surrounding soils. Further characterization of 15 isolates by vegetative compatibility testing allowed us to conclude that a clonal lineage of F. oxysporum circulates in the tap water of the different hospitals. IMPORTANCE We demonstrated that a clonal lineage of Fusarium oxysporum inhabits the water distribution systems of several French hospitals. Abstract: In vitrosusceptibility of 933Candidaisolates, from 16 French hospitals, to micafungin was determined using the Etest in each center. All isolates were then sent to a single center for determination of MICs by the EUCAST reference method. Overall essential agreement between the two tests was 98.5% at ±2 log2dilutions and 90.2% at ±1 log2dilutions. Categorical agreement was 98.2%. The Etest is a valuable alternative to EUCAST for the routine determination of micafungin MICs in medical mycology laboratories.

Abstract: International audience; Objective of the study. - Study of the spatio-temporal fungal colonization in a new medical mycology laboratory. Methods. - A 17-month survey of airborne fungal contamination was conducted in a new medical mycology laboratory at a tertiary care university hospital. This survey was implemented at three different periods: before the new premises were occupied (period A), during the move into the new laboratory (period B) and after resumption of the mycological activities in these new premises (period C). Results. - During period A, the airborne fungal load ranged from 2.3 to 6 cfu/m(3). The most frequently recovered airborne fungi were Penicillium spp. (75 to 100%). During period B, a dramatic increase in Penicillium chrysogenum conidia was observed in the air of the new laboratory (40 to 160 cfu/m(3)). During period C, the fungal load ranged from 4.5 to 8.4 cfu/m(3). Penicillium was the most common genus identified in rooms of the laboratory where no filamentous fungi were handled, while Aspergillus was clearly the predominant genus (78%) in the room dedicated to the culture of filamentous fungi. Conclusions. - We suggest that the specific fungal ecology in air of the room dedicated to the culture of filamentous fungi is due to the handling of a large number of medical strains of A. fumigatus.; Objectif de l’étude : Étude spatio-temporelle de la colonisation fongique dans un nouveau laboratoire de mycologie médicale. Méthodes : Durant 17 mois, l’aérocontamination fongique a été évaluée dans trois pièces d’un nouveau laboratoire de mycologie médicale d’un centre hospitalo-universitaire. Cette surveillance a été programmée à trois périodes différentes : avant l’occupation des nouveaux locaux (période A), durant l’emménagement dans le nouveau laboratoire (période B) et après la reprise des activités de mycologie médicale dans ces nouveaux locaux (période C). Résultats : Durant la période A, la charge fongique dans l’air était comprise entre 2,3 et 6 ufc/m3. Le micromycète le plus fréquemment isolé a été Penicillium spp. (75 à 100 %). Durant la période B, une très forte augmentation de la charge en conidies de Penicillium chrysogenum a été observée dans l’air du nouveau laboratoire (40 à 160 ufc/m3). Durant la période C, la charge fongique a été comprise entre 4,5 et 8,4 ufc/m3. Penicillium a été le genre le plus fréquemment identifié dans les deux pièces du laboratoire où aucun champignon filamenteux n’est manipulé, alors qu’Aspergillus a clairement été le genre prédominant (78 %) dans la pièce consacrée à la culture des champignons filamenteux. Conclusions : Nous suggérons que la manipulation d’un grand nombre de souches médicales d’Aspergillus est à l’origine de l’écologie spécifique qui existe dans la pièce du laboratoire destinée à la manipulation des champignons filamenteux.

Abstract: International audience; IntroductionL’objectif de ce travail a été d’étudier le profil épidémiologique et mycologique des dermatophytoses diagnostiquées au laboratoire de parasitologie-mycologie du CHU de Dijon.Matériel et méthodesIl s’agit d’une étude rétrospective qui couvre une période de 10 ans (2007–2016). Seuls les examens mycologiques positifs pour les dermatophytes ont été pris en compte pour cette étude (genres Epidermophyton, Microsporum et Trichophyton).De janvier 2007 à août 2014, l’identification de ces micromycètes a été réalisée selon les caractères macro- et microscopiques des colonies fongiques. À partir de septembre 2014, leur identification a été réalisée par microscopie et spectrométrie de masse MALDI-TOF.RésultatsParmi les 788 dermatophytes identifiés sur la période 2007-2016, les espèces les plus fréquemment isolées ont été Trichophyton rubrum (44 %), T. mentagrophytes (19 %), T. tonsurans (12 %), T. violaceum (12 %), T. soudanense (5 %), Microsporum canis (3 %) et M. audouinii var. langeroni (3 %).Quelques dermatophytes zoophiles atypiques ont parfois été isolés à partir de lésions cutanées : T. equinum (cheval), T. erinacei (hérisson), T. mentagrophytes var. porcellae (cochon d’inde), et T. verrucosum (bovidés).Au niveau clinique, les lésions ont été observées au niveau de l’ongle dans 35 % des cas, au niveau du cuir chevelu (34 % des cas) et au niveau cutané (31 % des cas).Chez l’enfant et l’adolescent (patients de moins de 15 ans), l’atteinte dermatophytique a principalement touché le cuir chevelu (91 % des cas), et les 3 principales espèces incriminées ont été T. tonsurans (33 %), T. violaceum (30 %) et T. soudanense (11 %). Pour les patients de plus de 15 ans, les lésions ont prédominé au niveau des ongles (54 %) et de la peau (44 %). La principale espèce responsable d’onyxis a été T. rubrum (82 %), et en seconde intention T. mentagrophytes (16 %) ; ces 2 espèces sont également incriminées majoritairement dans les atteintes cutanées : T. rubrum (45 %) et T. mentagrophytes (42 %).

Abstract: Background: The Dijon University Hospital in Dijon, France is involved in a large construction program with heavy truck traffic and a very dusty environment. This study aimed to assess the impact of outdoor hospital construction work on Aspergillus air contamination in the immediate environment of patients at high risk for aspergillosis in the presence of protective measures. Methods: Prospective air and surface sampling (n 5 1301) was performed in 3 hospital units over a 30-month period. Generalized estimating equations were used to test the relationship between Aspergillus air contamination and the different variables (construction period, air treatment system, and surface contamination). Results: Positivity rates of Aspergillus spp varied from 21.1% before construction work to 16.9% during work for air samples (P 5 .07), and the associated mean fungal load varied from 1.21 colony-forming units (CFU)/m 3 to 0.64 CFU/m 3 (P 5 .04). In multivariate analysis, only the use of an air treatment system was associated with decreased airborne Aspergillus contamination (P , .0001). No significant difference was observed between the presence or absence of construction work and the proportion of airborne Aspergillus contamination (P 5 .91) or the Aspergillus fungal load (P 5 .10). Conclusions: No influence of hospital construction work on airborne Aspergillus contamination was demonstrated when protective measures were taken, including reinforcement of the importance of environmental cleaning.

Air was sampled with a portable Air System Impactor and surfaces with contact Sabouraud plates. During this survey the mean concentration of viable fungi in air was 4.2 cfu/m(3) and that for surfaces was 1.7 cfu/plate. At the beginning of construction work, there were increases in airborne fungal spores (from 3.0 to 9.8 cfu/m(3)) in the unit, but concentrations did not exceed 10 cfu/m(3) during the 18-month period. The most frequently recovered airborne fungi were Penicillium spp. (27-38%), Aspergillus spp. (25%) and Bjerkandera adusta, a basidiomycete identified with molecular tools (7-12%). Blastomycetes accounted for more than 50% of the fungal flora on surfaces. Investigating the impact of a new air-treatment system (mobile Plasmair units), there were significant reductions in fungal contamination for the Plasmer -treated rooms, and in these rooms we observed the same level of fungal load whether construction work was in progress or not.

Abstract: This review will summarize some of the important reports on the chemistry and the biological activities of Dioscorea steroid saponins from the literature data of recent years (2000–2006) and from the authors’ studies. These discoveries became possible as a result of the scientific development of isolation, structure elucidation, and the development of in vitro bioassays. Over 50 steroid saponins of furostane-, spirostane-, and pregnane-type skeleton have been discovered and characterized from 13 Dioscorea species, namely, D. bulbifera L., D. cayenensis Lam.-Holl, D. colletii Hook. F. var. hypoglauca (Palib.) Pei et Ting, D. deltoidea Wall var. orbiculata, D. futschauensis R. Kunth, D. nipponica Mak., D. panthaica Prain et Burkill, D. parviflora Ting, D. polygonoides Humb. et Bonpl., D. pseudojaponica Yamamoto, D. spongiosa Xi, Mizuno et Zhao, D. villosa L., D. zingiberensis Wright. The main biological and pharmacological properties of Dioscorea saponins concern cytotoxic and antifungal activity, which are highlighted.

Abstract: Introduction : Trichoderma longibrachiatum est un champignon filamenteux appartenant à la famille des hyalohyphomycètes, rarement isolé lors d’infections fongiques invasives chez l’Homme mais quelques cas ont déjà été décrits chez des sujets immunodéprimés. Observation : Un homme de 69 ans a développé un syndrome fébrile une semaine après la fin d’une chimiothérapie pour le traitement d’une leucémie aiguë myéloïde alors qu’il était en aplasie profonde (GB < 100 G/L). Devant la positivité de l’antigénémie aspergillaire à 1.4 (Platelia™ Aspergillus Galactomannan test), un scanner thoracique est réalisé. L’aspect scannographique (signe du halo) était évocateur d’une aspergillose pulmonaire invasive (API), motivant la réalisation d’un lavage broncho-alvéolaire (LBA). Le LBA a mis en évidence des filaments mycéliens régulièrement septés. La culture a permis l’isolement et l’identification de Trichoderma sp. Le séquençage par biologie moléculaire de la région polymorphe ITS3-4 de l’ADN fongique extrait à partir de la culture fongique a permis d’identifier l’espèce T. longibrachiatum. L’examen anatomopathologique d’un fragment de biopsie pulmonaire a par la suite confirmé le diagnostic de mycose pulmonaire invasive d’aspect compatible avec une aspergillose. Devant la proximité hilaire droite du foyer infectieux, faisant craindre un risque hémorragique, une lobectomie inférieure droite a été réalisée. Des filaments mycéliens ont été mis en évidence à l’examen direct de la pièce opératoire, et la présence de Trichoderma longibrachiatum a été confirmée par la culture puis la biologie moléculaire (séquençage de l’ADN fongique extrait à partir de la pièce opératoire) alors que la PCR Aspergillus était négative. La détermination des CMI par la méthode E-test® (Biomérieux) a révélé une bonne sensibilité pour le voriconazole (0,38μg/ml), l’amphotéricine B (3μg/ml) et la caspofungine (0,094 μg/ml). Le traitement a reposé sur une association Voriconazole + Caspofungine (10 jours) relayé par du voriconazole per os et le patient est toujours en rémission hématologique en mars 2015. Conclusion : ce patient qui avait tous les caractères cliniques, biologiques et scannographiques d’une API était finalement porteur d’une infection à fongique invasive à Trichoderma longibrachiatum.

Their structures were determined by spectroscopic methods including 1D- and 2D-NMR experiments. Compounds 1 and 2 exhibited antifungal activity against the human pathogenic yeasts Candida albicans, C. glabrata and C. tropicalis (MICs between 6.25 and 50 microg/mL) whereas 3 was inactive.

Abstract: Aeromonas hydrophila is sometimes considered as a controversial human pathogen and reported to be susceptible to food processing procedures and environmental stresses. In this study, we have shown that early stationary phase cells of A. hydrophila were readily killed during up shifts in temperature (in the range 50-70 degrees C), the course of drying (at relative humidity, temperature and brightness of the laboratory) and after 5 min exposure to 20%, 30% and 40% v/v ethanol. However, this bacterium was found moderately susceptible to down shift to 4 degrees C in nutrient poor water, sodium chloride stresses (1.5 and 2 M) and to 12% and 15% v/v ethanol stresses. Tolerance against 1 M NaCl and 10% v/v ethanol was observed. At ambient temperature (24.5 degrees C), this microorganism exhibited a starvation survival state, which was largely independent of the initial cell concentrations (8.82, 7.71 and 6.76 log units). The cross-protection experiments showed that cells starved for short (1 day) or prolonged (50 days) periods developed increased resistance to down shift at 4 degrees C and ethanol stress. This may be of concern to the food-processing industry from the public health perspective.

Abstract: SPE IPM MERS EA; International audience; Members of the Fusarium group were recently detected in water distribution systems of several hospitals in the world. An epidemiological investigation was conducted over 2 years in hospital buildings in Dijon and Nancy (France) and in non-hospital buildings in Dijon. The fungi were detected only within the water distribution systems of the hospital buildings and also, but at very low concentrations, in the urban water network of Nancy. All fungi were identified as Fusarium oxysporum species complex (FOSC) and Fusarium dimerum species complex (FDSC) by sequencing part of the translation elongation factor 1- alpha (TEF-1a) gene. Very low diversity was found in each complex, suggesting the existence of a clonal population for each. Density and heterogeneous distributions according to buildings and variability over time were explained by episodic detachments of parts of the colony from biofilms in the pipes. Isolates of these waterborne populations as well as soilborne isolates were tested for their ability to grow in liquid medium in the presence of increasing concentrations of sodium hypochlorite, copper sulfate, anti-corrosion pipe coating, at various temperatures (4 e42 C) and on agar medium with amphotericin B and voriconazole. The waterborne isolates tolerated higher sodium hypochlorite and copper sulfate concentrations and temperatures than did soilborne isolates but did not show any specific resistance to fungicides. In addition, unlike waterborne isolates, soilborne isolates did not survive in water even supplemented with glucose, while the former developed in the soil as well as soilborne isolates. We concluded the existence of homogeneous populations of FOSC and FDSC common to all contaminated hospital sites. These populations are present at very low densities in natural waters, making them difficult to detect, but they are adapted to the specific conditions offered by the complex water systems of public hospitals in Dijon and Nancy and probably other localities in the world.

Abstract: Candida albicans is an opportunistic yeast. Its pathogenicity is linked to the susceptibility of the host surface as well as to particular factors of the strain: adhesion, filamentous growth and secretion of proteolytic enzymes. Chlorhexidine digluconate is an antiseptic with fungicidal properties. The action of the antiseptic on the growth of the yeast shows a minimal inhibitory concentration (MIC) at 50 μg·mL−1 and a minimal fungicidal concentration (MFC) at 100 μg·mL−1. The consequences of antiseptic treatment are studied using two indicators of pathogenicity: filamentation and the secretion of acid proteinase. Concerning the morphological indicator, a complete inhibition of filamentation for antiseptic concentration far below the MIC is observed; concerning the biochemical indicator, the results show a maintenance of proteolytic activity for concentration equal to the MIC and a disappearance of this activity between the MIC and the MFC. Concerning the proteolytic activity, Skim Milk (Difco) was substituted for bovine serum albumin with success. This mixture of casein allows to use a cheaper and more easily available substrate.

43. Les cellules M : une porte d’entrée pour le franchissement de la barrière intestinale par Candida albicans

Abstract: Candida albicans est un pathogene opportuniste pouvant provoquer des infections systemiques chez des patients immunodeprimes. L’origine de ces infections est principalement d’origine endogene, notamment a partir du tractus gastro-intestinal, ou le champignon peut penetrer a travers la barriere epitheliale intestinale pour gagner ensuite la circulation sanguine. Au niveau de la muqueuse intestinale, les enterocytes forment une monocouche de cellules assurant l’integrite et l’impermeabilite du tissu digestif. Au niveau des plaques de Peyer notamment, les cellules Microfold (ou cellules M) sont associees aux enterocytes et jouent un role important dans l’homeostasie digestive. En effet, ces cellules epitheliales sont specialisees dans l’endocytose d’antigenes, de molecules ou encore de microorganismes presents dans la lumiere intestinale. Elles jouent le role de cellules presentatrices d’antigene puisqu’elles presentent au systeme immunitaire sous-jacent les antigenes endocytes. Ces cellules peuvent egalement etre utilisees par des pathogenes comme Yersinia enterocolitica ou Salmonella typhi comme porte d’entree pour traverser la barriere epitheliale intestinale. L’interaction de certaines bacteries pathogenes avec les cellules M constitue donc une etape importante dans la physiopathologie des infections causees par ces bacteries. Cependant, aucune etude n’a a ce jour etudie l’interaction de C. albicans avec les cellules M. Afin de mieux comprendre les premieres etapes des candidoses invasives a C. albicans , nous avons developpe un modele in vitro decrit par des Rieux et al., 2007, permettant d’obtenir des cellules M a partir de la differenciation d’enterocytes de la lignee Caco-2 mis en contact avec des lymphocytes B Raji. Nous avons ensuite etudie l’interaction specifique de C. albicans avec des cultures mixtes enterocytes/cellules M. A l’aide de techniques de microscopie electronique a balayage et a fluorescence, et de protocoles permettant d’evaluer l’adherence et l’invasion de souches sauvages et de mutants de C. albicans , nous avons pu montrer que C. albicans adherait preferentiellement aux cellules M dans des monocouches mixtes enterocytes/cellules M. De meme, nous avons observe que C. albicans pouvait utiliser les cellules M comme porte d’entree au franchissement de la barriere intestinale. Par ailleurs, alors que C. albicans est capable d’envahir la muqueuse intestinale par penetration active des hyphes dans les cellules epitheliales intestinales, nous avons montre que l’internalisation des levures etait egalement possible au niveau des cellules M par penetration active des hyphes mais egalement par endocytose F-actine dependante : alors que la penetration active dans les monocouches mixtes enterocytes/cellules M est facilitee en partie par certaines invasines associees aux hyphes de C. albicans (dont Als3 et Ssa1), la nature des invasines de C. albicans capables d’induire son endocytose par les cellules M reste a determiner.

44. Contamination des circuits d’alimentation en eau par Fusarium

Abstract: La decouverte de micromycetes du genre Fusarium dans le reseau hydrique du CHU de Dijon nous a conduit a mettre en place un protocole hospitalier de recherche clinique (PHRC) dont l’objectif a ete de decrire en prospectif, dans le temps et dans l’espace, la contamination par Fusarium dans l’eau de 2 CHU francais (Dijon et Nancy), a des periodes d’activites de restructuration et de construction. Cette etude a ainsi permis de mettre en evidence la presence chronique de Fusarium spp. dans les circuits de distribution de l’eau de certains bâtiments hospitaliers. Nous avons en effet observe que ces micromycetes etaient « installes » dans le reseau, que leur densite de population variait au cours du temps malgre : – le traitement chlore de l’eau ; – l’existence de portions de la tuyauterie en cuivre (liberant des ions Cu susceptibles d’inhiber le developpement des champignons) ; – l’absence a priori de sources de C et N utilisables comme substrat de croissance par les champignons. L’identification des souches par biologie moleculaire (amplification du gene du facteur d’elongation TEF-1-alpha ; par les amorces EF1 et EF2) a permis de montrer la presence de 2 especes de Fusarium dans le reseau hydrique du CHU : Fusarium oxysporum (majoritaire) et F. dimerum . A notre surprise, ces 2 memes especes ont egalement ete retrouvees dans les prelevements d’eau de Nancy. En utilisant d’autres outils de biologie moleculaire, mais aussi une etude de compatibilite vegetative, il a ete possible de montrer l’existence de populations clonales de Fusarium (especes oxysporum et dimerum ), presentes a la fois dans le reseau d’eau de Dijon (Bocage et hopital general), celui du CHU de Nancy, mais egalement dans les circuits d’alimentation en eau d’hopitaux d’autres pays du monde. A ce stade, nous avons donc emis l’hypothese que certaines souches de Fusarium etaient donc plus adaptees a une vie en milieu aquatique que d’autres souches, et qu’il serait interessant de comparer le comportement phenotypique de ces souches hydriques avec des souches de Fusarium d’origine tellurique (le sol etant le reservoir naturel du champignon). Divers tests en microplaques ont donc ete mis au point pour confirmer cette hypothese. Nous avons ainsi compare la tolerance de souches hydriques ou telluriques vis-a-vis de divers agents chimiques comme le chlore ou le cuivre (element fongistatique), et nous avons par exemple pu constater une meilleure tolerance au cuivre de la part de tous les isolats d’origine hydrique. Enfin, des etudes de dynamique de population ont egalement ete envisagees pour mieux comprendre le comportement atypique de ces souches isolees de l’eau. Les resultats obtenus nous a permis de conclure que nous etions en presence de souches fongiques de Fusarium particulierement adaptees a une vie en milieu aquatique.

Abstract: Cell wall proteins are central to the virulence of Candida albicans. Hwp1, Hwp2 and Rbt1 form a family of hypha-associated cell surface proteins. Hwp1 and Hwp2 have been involved in adhesion and other virulence traits but Rbt1 is still poorly characterized. To assess the role of Rbt1 in the interaction of C. albicans with biotic and abiotic surfaces independently of its morphological state, heterologous expression and promoter swap strategies were applied. The N-terminal domain with features typical of the Flo11 superfamily was found to be essential for adhesiveness to polystyrene through an increase in cell surface hydrophobicity. A 42 amino acid-long domain localized in the central part of the protein was shown to enhance the aggregation function. We demonstrated that a VTTGVVVVT motif within the 42 amino acid domain displayed a high beta-aggregation potential and was responsible for cell-to-cell interactions by promoting the aggregation of hyphae. Finally, we showed through constitutive expression that while Rbt1 was directly accessible to antibodies in hyphae, it was not so in yeast. Similar results were obtained for another cell wall protein, namely Iff8, and suggested that modification of the cell wall structure between yeast and hyphae can regulate the extracellular accessibility of cell wall proteins independently of gene regulation.

Abstract: Phytochemical investigation of the rhizome of Dioscorea opposita has led to the isolation of a new phenanthrene glycoside, 3,4,6-trihydroxyphenanthrene-3-O-beta-D-glucopyranoside (1), and five known compounds, soyacerebroside I (2), adenosine (3), beta-sitosterol (4), palmitic acid (5) and palmitoyloleoylphosphatidylcholine (6). Their structures were determined by spectroscopic methods, including 1D- and 2D-NMR. Compounds 1-6 exhibited no antifungal activity against the human pathogenic yeasts Candida albicans, C. glabrata and C. tropicalis.

Abstract: The new 26-O-beta-D-glucopyranosyl-3beta,26-dihydroxy-20,22-seco-25(R)-furost-5-en-20,22-dione-3-O-alpha-L-rhamnopyranosyl-(1-->4)-alpha-L-rhamnopyranosyl-(1-->4)-[alpha-L-rhamnopyranosyl-(1-->2)]-beta-D-glucopyranoside (1), along with the known methyl protodioscin (2), asperoside (3) and prosapogenin A of dioscin (4) were isolated from the rhizomes of Dioscorea cayenensis LAM.-HOLL (Dioscoreaceae). Their structures were established mainly on the basis of 600 MHz 2D-NMR spectral data. 4 exhibited antifungal activity against the human pathogenic yeasts Candida albicans, C. glabrata and C. tropicalis (MICs of 20.8, 6.25, 25 microg/ml, respectively), whereas saponins 1-3 were inactive.

Abstract: La presence du signe du halo inverse (SHI) au scanner thoracique est etroitement associee a la presence d’ADN circulant de mucorales dans le serum de patients presentant des signes d’infections fongiques invasives. Contexte Chez les patients atteints de leucemies aigues, l’incidence des mucormycoses pulmonaires invasives (MPI) est en forte augmentation. Compte tenu (i) du fort taux de mortalite associe aux MPI, (ii) des difficultes du diagnostic biologique (ED, culture, diagnostic indirect), clinique et radiologique differentiel avec l’aspergillose pulmonaire invasive (API) et (iii) de la resistance des mucorales au Voriconazole utilise en premiere ligne du traitement des API, il etait urgent de disposer de nouveaux outils pour le diagnostic precoce de MPI. Le signe du halo inverse (SHI) et la PCR en temps reel (qPCR) ciblant l’ADN des principales especes de mucorales dans le serum sont 2 marqueurs ayant recemment montre leur interet dans ce diagnostic [1] , [2] . Objectifs L’efficacite de ces deux marqueurs pour le diagnostic precoce de MPI a ete evaluee retrospectivement chez 23 patients traites pour leucemie aigue et presentant une MPI prouvee (groupe 1, n = 16) ou une infection fongique invasive possible (IFIp, groupe 2, n = 7) selon les criteres de l’EORTC/MSG. Methodes Les serums encadrant (J-9 a J + 9) le jour (J0) de mise en evidence du SHI ont ete testes par qPCR mucorales. 3 a 10 serums par patients ont pu etre testes. Resultats Tous les patients sauf 2 (un dans chaque groupe) ont presente au moins une recherche de mucorales par qPCR positive avant J0, soit une sensibilite de la qPCR de 94 % et de 86 % pour les groupes 1 et 2 respectivement. La PCR etait positive en moyenne 4 jours avant J0, sans difference significative entre les 2 groupes. Rhyzomucor etait l’agent le plus frequemment identifie dans notre serie de patients (15/22). Conclusion Cette etude confirme (i) la specificite du SHI dans le diagnostic de MPI chez les patients atteints de leucemie aigue et (ii) l’interet de la qPCR mucorale dans le diagnostic precoce de MPI et le monitoring des patients leucemiques a risque de MPI. L’integration des ces 2 marqueurs dans les criteres EORTC/MSG sera interessante pour le diagnostic precoce de MPI chez les patients a risque d’IFI qui pourront beneficier d’emblee d’un traitement antifongique adapte.

Abstract: International audience; Introduction: The opportunistic yeast Candida albicans gains access to the bloodstream mainly through translocation across the intestinal barrier. Using an in vitro model of interaction with the enterocytic-like cell line Caco-2, it has been shown that C. albicans is capable of adhering to, invading and damaging intestinal cells, probably through a combination of phenotypic properties including adherence, yeast to hyphae transition and secretion of lytic enzymes (Dalle et al., Cell. Microbiol. 12:248, 2010). However the sequence of events together with the genes involved in these processes remain to be clarified. The aim of our study was to screen a C. albicans isogenic collection of homozygous knock-out strains for genes involved in the infectivity of enterocytes using a Caco-2 in vitro model. Methods: A high throughput screening tool adapted from our Caco- 2 model was used for testing the infectivity of 674 C. albicans homozygous deletion strains (equivalent to ~ 11% of the genome, Noble et al., Nat. Genet. 42:590, 2010). Infectivity was measured as the cytotoxic effect of individual mutants upon Caco-2 cell monolayers. For each of the mutants tested, cytotoxicity was expressed as the percentage of variation as compared to the C. albicans SC5314 reference strain. Results: We identified 66 mutants out of 674 with marked increase or decrease in Caco-2 cell cytotoxicity. These mutants were selected as the 10% fraction of the population lying in the tails of the Gaussian distribution, with 5% on each side of the distribution. A gene ontology term analysis of the 66 ORFs showed functional enrichment of genes involved in filamentous growth (p-value of 0.07, false discovery rate of 0%) among the 674 ORFs tested. An additional screen of independent mutants for these 66 genes is currently under investigation to confirm these observations. Conclusions: Our study demonstrated the potential of our screen to identify genes involved in C. albicans ability to cause enterocyte cell damage. Known genes and yet uncharacterized ORFs have been identified for which deletion increases or decreases C. albicans virulence in vitro. Further studies will be conducted to confirm the involvement of these genes or ORFs in in vivo models of disseminated candidiasis originating from the gastrointestinal tract.
