Abstract

Cardiac arrhythmia, or irregular heartbeat, is a serious, yet fairly common ailment in both normal subjects and patients with cardiovascular disease, although it is more prevalent in the latter group. Cardiac arrhythmia has been reported with increasing frequency in children, owing in part to greater awareness and detection and in part to increased incidence in children with successfully repaired congenital heart abnormalities. After one fairly common type of pediatric cardiovascular operation, for example, it has been reported that 20 percent of the surviving patients have atrial flutter (a type of arrhythmia caused by rapid contraction of the atrial chamber of the heart); 10 percent of this group of patients suffer sudden cardiac death. Anti-arrhythmic medication is often given to treat atrial flutter. To evaluate the efficacy of propafenone, an orally administered anti-arrhythmic agent, a study was conducted in which this drug was administered to 57 children suffering from cardiac arrhythmia. The average age of the children was 4.8 years, but 23 were less than one year and 10 were less than one month. Arrhythmia was present in children with unrepaired congenital heart defects, in children who underwent surgical repair of congenital abnormalities, and in children with otherwise normal hearts. Normalization of the arrhythmia was seen in about 50 percent of the cases. The drug was less effective in patients with heart disease (30 percent success) than in patients having no symptoms other than the arrhythmia (63 percent success). Two children (infants) suffered transient side effects that resolved upon discontinuation of the drug. Abstract

Although 2-dimensional, Doppler, color flow echocardiography is accepted as a safe diagnostic method to guide the surgical treatment of certain heart defects, cardiac catheterization remains mandatory for patients with tetralogy of Fallot. Based on the excellent diagnostic correlation between echocardiography and angiocardiography, a policy of echo-guided primary repair of uncomplicated, selected cases of tetralogy of Fallot was introduced at Ospedale Bambino Gesu'. In the last 5 years, of 139 patients who had surgery for tetralogy of Fallot, 105 underwent primary repair. In 56,patient (53.3%), surgery was guided only by echocardiography (group I). In the remaining 49 patients (46.7%) (group II), unclear imaging of cardiovascular anatomy (n = 23), or echo-suspected malformations (n = 26) promoted cardiac catheterization. The 2 groups did not differ in age and weight at surgery. Intraoperative examination did not show diagnostic errors in patients of group I; cardiac catheterization and surgery confirmed the suspected associated anomalies in 16 of 26 patients of group II (38.4% false-positive). Echocardiography showed an overall sensitivity of 100% and specificity of 85% for detection of associated malformations. In conclusion, echocardiography proved to be adequate for selection of patients with uncomplicated forms of tetralogy of Fallot for primary repair.

Of 353 children who underwent surgical repair of a congenital heart defect, including closure of a ventricular septal defect (VSD), 12 patients (four with tetralogy of Fallot, five with a VSD, and three with a double-outlet right ventricle) develop subaortic stenosis, which was diagnosed one to six years after the surgical procedure. Five patients required surgical treatment of the subaortic stenosis, and one required percutaneous balloon angioplasty. Postsurgical subaortic stenosis appears to be an uncommon progressive acquired disease. Abstract

Children with Down's syndrome, a congenital disorder which results in mental retardation, frequently have heart defects. Atrioventricular canal (AVC) is one of the most common cardiac complications among those with Down's syndrome. This heart defect results from an embryonic malformation in which the common atrioventricular canal fails to divide; this causes septal defect and valve incompetence. Both Down's syndrome and AVC have been studied, but AVC as it typically occurs in Down's syndrome patients has not been fully explored. To further investigate this, the medical records of 220 patients with AVC were reviewed. Approximately half (105 children) also had Down's syndrome, and the other half (115 children) did not. Records of echocardiograms and angiocardiograms, diagnostic imaging techniques, were a part of the clinical review which assessed the anatomic types that were associated with each group. The children who also had Down's syndrome more frequently had a complete form of AVC, which was often associated with Fallot's tetralogy (another congenital condition, which is characterized by a defective interventricular septum, narrowing of the pulmonary artery, misplaced aorta, and an enlarged right ventricle). Patients without Down's syndrome more frequently had left-sided abnormalities and a partial atrioventricular canal. These results support other reports which indicate that AVC that is associated with Down's syndrome usually has predictable and distinct characteristics. Abstract

Acute myocarditis (inflammation of heart muscle) is usually diagnosed by analyzing a sample of heart tissue. The biopsy process may cause major complications, especially in children, and is usually done at specialized hospital centers. Magnetic resonance imaging (MRI) has been useful in characterizing changes in tissue that is diseased in a fashion similar to myocarditis. The effectiveness of MRI in the diagnosis of 11 children suspected of having acute myocarditis children was evaluated. Myocarditis was diagnosed in six children on the basis of biopsy results which showed edema (fluid accumulation) and infiltration by immune cells. Images obtained by MRI showed distinct differences, and calculations of signal intensities, including ratios of heart muscle to skeletal muscle signal intensities, were significantly increased in children with myocarditis. Further research is needed to confirm these results, which suggest that MRI may be a useful noninvasive alternative to biopsy in diagnosis of acute myocarditis, especially in children.
