International audience; PurposeIn vivo dosimetry transit using portal imaging is a promising approach for quality assurance in radiotherapy. A comparative evaluation was conducted between a commercial solution, EPIgray® and an in vivo dosimetry control reference using semiconductors diodes.Material and methodsThe performance of the two in vivo dosimetry methods was assessed. The primary endpoint was the dose deviation between the reconstructed dose at the prescription point and the measured dose using the ionization chamber in phantoms or the calculated predictive dose by the treatment planning system with patients. The deviation threshold was set to ±5%. In total, 107 patients were prospectively included and treated with 3D-conformal radiotherapy (3D-CRT) or intensity-modulated radiotherapy (IMRT) techniques for tumours of the brain, chest and head and neck.ResultsThe dosimetric accuracy of EPIgray® in phantom were comparable to diodes in terms of repeatability (0.11%), reproducibility (0.29–0.51%) with a mean dose deviation of 0.17% (SD: 1.11). The rates of radiotherapy sessions out of the tolerance for the brain (3D-CRT and IMRT), thorax (3D-CRT) and the head and neck (IMRT) were respectively 0%, 9.6% and 5.3% with a mean dose deviation ranging between 0.49% and 1.53%. The mean of dose deviation between three consecutive sessions with EPIgray® validates 99.1% of treatments.ConclusionThe performance of EPIgray® in in vivo dosimetry is consistent with the recommendations of the European Society for Radiotherapy and Oncology (ESTRO) and equivalent to semiconductor diodes for 3D-CRT. It also allows adequate control for IMRT, which is technically difficult to perform with the diodes.; Objectifs de l’étudeLa dosimétrie in vivo de transit utilisant l’imageur portal est une approche prometteuse pour l’assurance qualité en radiothérapie. Une évaluation comparative a été menée entre une solution commerciale, EPIgray® et un contrôle de dosimétrie in vivo de référence utilisant des diodes à semi-conducteurs.Matériel et méthodesLes performances des deux méthodes de dosimétrie in vivo ont été évaluées. Le critère principal était l’écart de dose au point de prescription par rapport aux mesures de la chambre d’ionisation sur fantôme ou par rapport à la dose prévisionnelle calculée par le système de planification de traitement avec les patients. Le seuil de tolérance de l’écart de dose était défini à ±5 %. Au total, 107 patients traités par irradiation conformationnelle tridimensionnelle avec ou sans modulation d’intensité pour des tumeurs de l’encéphale, du thorax, de la tête et du cou ont été inclus prospectivement.RésultatsLa précision dosimétrique d’EPIgray® sur fantôme était comparable à celle des mesures par diodes en termes de répétabilité (0,11 %) et de reproductibilité (0,29–0,51 %), avec un écart de dose moyen de 0,17 % (écart-type : 1,11). Les taux de séances hors tolérances pour l’encéphale (irradiation conformationnelle tridimensionnelle avec ou sans modulation d’intensité), le thorax (irradiation conformationnelle tridimensionnelle) et la tête et le cou (irradiation conformationnelle avec modulation d’intensité) étaient respectivement de 0 % ; 9,6 % et 5,3 % avec un écart de dose moyen entre 0,49 % et 1,53 %. La moyenne de l’écart de dose sur trois séances consécutives avec EPIgray® permettait de valider 99,1 % des traitements.ConclusionLes performances d’EPIgray® en dosimétrie in vivo sont compatibles avec les recommandations de l’European Society for Radiotherapy and Oncology (ESTRO) et équivalentes à celles des diodes à semi-conducteur pour l’irradiation conformationnelle tridimensionnelle. Elle permet un contrôle satisfaisant des irradiations conformationnelles avec modulation d’intensité, techniquement difficiles à réaliser avec les diodes.

International audience; Purpose: Between 1987 and 2006, the Radiotherapy department of the Jean Monnet Hospital in inal (France) had a series of mistakes leading to overexposure of many patients. We report the second accident identified in March 2007, and describe its consequences, on the 409 affected patients. Materials: In 2000, the radiation therapy team implemented three dimensional conformal radiotherapy (3D-CRT) to treat patients with prostate cancer. The quality control of patient repositioning was assessed with daily portal imaging (PI). The "Institut de Radioprotection et de Sretclre" (IRSN) reconstructed doses delivered by those PI after the discovery of the accident and found that each week, 4 sessions were conducted after 2 PI delivering 0.17 Gy, and one session was conducted after 6 IP delivering 0.36 Gy. These additional doses were not substracted from the prescribed fraction dose (2 Gy). Between October 2000 and October 2006, 433 patients were treated with 3D-CRT for prostate cancer. Twenty-four of these patients were victims of the first accident, not described here, due to incorrect use of a treatment planning system, and are reported in another report. Results: For prescribed doses of 70 Gy to 78 Gy, total doses received by 409 patients ranged from 77 Gy to 81 Gy in 35 to 39 fractions, corresponding to 8% to 10% overexposure. All these patients were evaluated starting March 2007, and were informed of their irradiation conditions. Of 409 patients, 405 were evaluable for toxicity. A total of 155 patients (38%) developed Grade 2 late gastrointestinal toxicities with rectal bleeding, and 66 patients (16%) developed Grade 3 proctitis that was treated with argon plasma coagulation, or required blood transfusions for hematologic support. Two patients developed Grade 4 rectal fistula and died. Of the 409 patients, 72 were irradiated for rising PSA after prostatectomy, 69 of them were evaluable for urinary toxicity. Thirty-six patients (54%) had Grade 2 permanent urinary incontinence requiring protective padding, and 9 patients (13%) developed Grade 2 hematuria. Of 330 evaluable patients treated with definitive radiotherapy, 56 (17%) experienced Grade 2 chronic frequency and urgency, and 35 (11%) had Grade 2 permanent urinary incontinence. Conclusions: This accident highlights the importance of the dose delivered with the PI controls during 3D-CRT. Doses given by PI should be integratedin the dose calculation of each session. The complications observed in this cohort emphasize the importance of the dose per fraction (here 2.17 to 2.36 Gy) in the genesis of late complications after radiotherapy.

Introduction : Le test AQA (Automatic Quality Assurance) est un contrôle de qualité mécanique quotidien proposé par ACCURAY pour vérifier la précision submillimétrique du système robotisé Cyberknife., Il est basé sur le test de Winston-Lutz conçu pour vérifier la précision des machines iso-centriques. L'étude a porté sur les mesures quotidiennes réalisées entre 2007 et 2009. La représentation graphique de la boite à moustaches est utilisée pour mieux appréhender ce test essentiel pour garantir le bon fonctionnement du système. Matériel et méthode : Le test est réalisé en utilisant un fantôme dédié. Celui-ci est composé d'un cube en plastique, avec un trou central contenant une boule métallique radio-opaque, qui doit être positionnée à l'isocentre du système d'imagerie. Ce cube peut recevoir deux films radiochromiques, Gafchromique EBT, placés perpendiculairement. L'objet tests est irradié par deux faisceaux orthogonaux qui produisent une couronne de densité élevée en périphérie de la zone sous-exposée dans l'ombre de la bille en acier. Un scanner Epson Perfection V700/V750 est utilisé pour numériser ces deux films. Un outil d'analyse commercialisé par ACCURAY permet de traiter les deux films, d'extraire les décalages dans les trois directions et de calculer l'erreur radiale à partir de l'écart de position du centre de chacun de cercles. Résultats : L'étude a porté sur 500 mesures. L'ensemble des résultats est présenté graphiquement à l'aide de boîte à moustache qui permet de mettre en évidence les paramètres de tendance centrale et les paramètres de dispersion. L'analyse des tests AQA montre des limites comprises entre -0.5 et 0.75 pour la direction Supérieure-Inférieure, entre -1 et 1 mm pour la direction Gauche-Droite, entre -0.9 et 1.2 pour la direction Antérieure-Postérieure et des limites entre 0.2 et 1.3 mm pour l'erreur radiale. En dehors de ces intervalles les résultats doivent être considérés atypiques et peuvent nécessiter une intervention corrective sur la position du robot. Conclusion : L'analyse des résultats du contrôle de qualité quotidien, AQA, du Cyberknife par la boîte à moustaches permet de visualiser la répartition et la dispersion des valeurs mesurées, tout en permettant de définir les limites acceptées pour ces valeurs. Cette représentation graphique est très utile pour mieux analyser résultats de ce test.

International audience; Purpose: For several months in 1999, the radiotherapy department of the Jean Monnet Hospital in inal (France) could not use electron beams during the dismantling of its second linear accelerator, and the installation of a new CLINAC 2100. During this time, the radiation therapy team decided to treat the internal mammary lymph node chain (IMC) of patients with breast cancer with a direct 6 MV photon beam energy, to deliver a dose of 50 Gy, instead of using mixed beams of photons and electrons. Materials: From February to June 1999, 37 patients had received an irradiation of IMC by a direct 6 MV photon beam during adjuvant radiotherapy for breast cancer. Seventeen of these patients received adjuvant chemotherapy with epirubicine, endoxan and 5FU. Following the report of a coronary disease event, this cohort was identified. The surviving patients were examined by a radiation oncologist and a cardiologist. They were offered a cardiac evaluation including an electrocardiogram, echocardiography, supplemented by a stress test or thallium scintigraphy if clinically required. Cardiac toxicity was evaluated according to the CTCAE v3 classification. Results: Among these 37 patients, 4 were lost to follow-up. After a median follow-up of 9 years, 24 patients had no cardiac complications, and 9 patients had serious cardiac events. Three patients experienced Grade 4 ischemic heart disease, one patient had a Grade 4 constrictive pericarditis, 3 patients developed Grade 4 valvular dysfunction, and 2 patients developed Grade 4 congestive heart failure. Seven patients died, but not from cardiac causes. In this cohort, the 9-year probability of developing Grade 4 cardiac event was 27%. Multivariate analysis showed that neither age (p = 0.20), nor chemotherapy (p = 0.33) nor cardiovascular risk factors were associated with the occurrence of cardiac events. Conclusions: Exclusive use of a direct 6 MV photon beam energy to irradiate the IMC during adjuvant radiotherapy for breast cancer exposes patients to an unacceptable increased risk of Grade 4 cardiac complications.
