In the surgical treatment of the most common fracture of the mandible, which is a fracture of the condylar base, a great choice of different plate shapes is observed. The aim of this study was to determine which shape gives the greatest fixation stiffness. To ensure homogeneity in comparison, tests were performed on polyurethane models divided at the level of the condylar base fracture and each were fixed with 51 plates. The plates were cut from a 1 mm thick grade 23 titanium sheet. The models were then loaded and the force required for 1 mm of fracture displacement was recorded. It was noted that in addition to osteosynthesis from two simple plates, there were also two dedicated single plates with similar rigidity. Among the large number of described designs of plates, there is considerable variation in terms of the stability of the fixation performed with them. The proposed Mechanical Excellence Factor allows a pre-evaluation of the expected rigidity of fixation with a given plate shape without the need for a loading experiment. The authors expect this to be helpful for surgeons in the application of relevant plates, as well for inventors of new plates for the osteosynthesis of basal fractures in mandibular condyle.

Introduction. Lymphoedema of the skull's facial part is one of the main complications observed in a patient after surgical treatment of disorders of this area. Another noticeable and frequently reported complication by the patient is irregularities in facial expression muscles' work. A relationship between them and reducing edema performed in patients in the first days after surgery will reduce the noted disturbances in motor branches' function. Aim. To investigate the impact of early swelling reduction on the extent of nervous system disorders and its reinnervation rate. Materials and methods. Patients of the Maxillofacial Surgery Clinic of the University Clinical Hospital of the Military Medical Academy in Łódź were enrolled in the study. The study was conducted on 60 people who underwent maxillofacial surgery procedures from February to December 2019. Lymphatic drainage and dynamic taping were used to reduce patients' edema. Forty-five people were divided into three equal groups. Fifteen patients constituted the comparative group. The research tools were a survey created following the author's idea, linear measurements for measuring edema, and the Pietruski scale for assessing the state of reinnervation imaged by mimic muscle motility. On the first day after surgery, patients were examined four times, then on the third and seventh day after surgery, and finally four months after surgery. Results. The use of anti-edema therapy noticeably (from 2-5 points on the Pietruski scale) reduces the disturbances in facial expression muscles' function. The difference has been visible already on the third day after surgery. Regarding the patient's pain, the lack of manual lymphatic system development resulted in pain sensations remaining in each of the three tests performed. The occurrence of edema, pain level, and level of facial expression muscle disorders were similar between groups and made the following research objective in terms of observed relationships. Conclusions. The level of disturbances in the area of the mobility of expressive facial muscles (based on Pietruski scale) indicates that the implementation of anti edematous therapy, and thus the reduction of excess lymph, already in the first days after surgery provides a reduction of disorders of conduction of efferent impulses of motor fibers, appearing already in the first days after surgery. Increased drainage rate also accelerates the reinnervation process, shortens it by several weeks, and reduces pain symptoms.

Background: The mandible is the most injured part of the facial skeleton, and 25–40% of mandibular fractures involve the condyle process. The aim of this study is to answer the question of the relationship between screw pullout and/or plate fracture during osteosynthesis. Methods: We tested polyurethane models of mandibles whose condylar process was cut (simulating a fracture) and fused using plates and screws. Results: A total of 672 plates were tested. A total of 25.6% of them were fractured during the test, with most being fractures of the base of the condyle. More screws (81.97%) are pulled out from the ramus than from the condyle—69.15%. Conclusions: The gold standard in the osteosynthesis of condylar fractures is two straight plates. Other than these, there is no one-size-fits-all plate for every type of fracture. Plates fixed with fewer screws (smaller plates used in higher-lying fractures) are more likely to result in screw pullout. On the other hand, in plates fixed with more screws, plate fracture is more common.

Chemical composition and physical parameters of the implant surface, such as roughness, regulate the cellular response leading to implant bone osseointegration. Possible implant surface modifications include anodization or the plasma electrolytic oxidation (PEO) treatment process that produces a thick and dense oxide coating superior to normal anodic oxidation. Experimental modifications with Plasma Electrolytic Oxidation (PEO) titanium and titanium alloy Ti6Al4V plates and PEO additionally treated with low-pressure oxygen plasma (PEO-S) were used in this study to evaluate their physical and chemical properties. Cytotoxicity of experimental titanium samples as well as cell adhesion to their surface were assessed using normal human dermal fibroblasts (NHDF) or L929 cell line. Moreover, the surface roughness, fractal dimension analysis, and texture analysis were calculated. Samples after surface treatment have substantially improved properties compared to the reference SLA (sandblasted and acid-etched) surface. The surface roughness (Sa) was 0.59–2.38 µm, and none of the tested surfaces had cytotoxic effect on NHDF and L929 cell lines. A greater cell growth of NHDF was observed on the tested PEO and PEO-S samples compared to reference SLA sample titanium.

There are significant discrepancies in the reported prevalence of condylar process fractures among all mandibular fractures (16.5–56%) in the available literature. In addition, it seems that the actual number of difficult-to-treat fractures of the mandibular head is unknown. The purpose of this study is to present the current incidence of the different types of mandibular process fractures with a special focus on mandibular head fractures. The medical records of 386 patients with single or multiple mandibular fractures were reviewed. Of the fractures found, 58% were body fractures, 32% were angle fractures, 7% were ramus fractures, 2% were coronoid process fractures, and 45% were condylar process fractures. The most common fracture of the condylar process was a basal fracture (54% of condylar fractures), and the second most common fracture was a fracture of the mandibular head (34% of condylar process fractures). Further, 16% of patients had low-neck fractures, and 16% had high-neck fractures. Of the patients with head fractures, 8% had a type A fracture, 34% had a type B fracture, and 73% had a type C fracture. A total of 89.6% of the patients were surgically treated with ORIF. Mandibular head fractures are not as rare as previously thought. Head fractures occur twice as often in the pediatric population than in adults. A mandibular fracture is most likely related to a mandible head fracture. Such evidence can guide the diagnostic procedure in the future.

Several measures describing the transformation of trabecular bone to cortical bone on the basis of analysis of intraoral radiographs are known (including bone index or corticalization index, CI). At the same time, it has been noted that after functional loading of dental implants such transformations occur in the bone directly adjacent to the fixture. Intuitively, it seems that this is a process conducive to the long-term maintenance of dental implants and certainly necessary when immediate loading is applied. The authors examined the relationship of implant design features to marginal bone loss (MBL) and the intensity of corticalization over a 10-year period of functional loading. This study is a general description of the phenomenon of peri-implant bone corticalization and an attempt to interpret this phenomenon to achieve success of implant treatment in the long term. Corticalization significantly increased over the first 5-year functional loading (CI from 200 ± 146 initially to 282 ± 182, p < 0.001) and maintained a high level (CI = 261 ± 168) in the 10-year study relative to the reference bone (149 ± 178). MBL significantly increased throughout the follow-up period—5 years: 0.83 ± 1.26 mm (p < 0.001), 10 years: 1.48 ± 2.01 mm (p < 0.001). MBL and radiographic bone structure (CI) were evaluated in relation to intraosseous implant design features and prosthetic work performed. In the scope of the study, it can be concluded that the phenomenon of peri-implant jawbone corticalization seems an unfavorable condition for the future fate of bone-anchored implants, but it requires further research to fully explain the significance of this phenomenon.

After the insertion of dental implants into living bone, the condition of the peri-implant bone changes with time. Implant-loading phenomena can induce bone remodeling in the form of the corticalization of the trabecular bone. The aim of this study was to see how bone index (BI) values behave in areas of bone loss (radiographically translucent non-trabecular areas) and to propose other indices specifically dedicated to detecting corticalization in living bone. Eight measures of corticalization in clinical standardized intraoral radiographs were studied: mean optical density, entropy, differential entropy, long-run emphasis moment, BI, corticalization index ver. 1 and ver. 2 (CI v.1, CI v.2) and corticalization factor (CF). The analysis was conducted on 40 cortical bone image samples, 40 cancellous bone samples and 40 soft tissue samples. It was found that each measure distinguishes corticalization significantly (p < 0.001), but only CI v.1 and CI v.2 do so selectively. CF or the inverse of BI can serve as a measure of peri-implant bone corticalization. However, better measures are CIs as they are dedicated to detecting this phenomenon and allowing clear clinical deduction.

Orthodontic removable appliances made of transparent thermoplastic materials—aligners—are becoming increasingly popular in contemporary orthodontic practice. It is important for the clinician to fully understand the mechanical properties and behavior of the appliance used. Because of that, the aim of our study was to investigate the changes in aligner surface after immersion in Coca-Cola and orange juice. For surface evaluation, fractal analysis, texture analysis, and wetting angle measurement were performed. Statistically significant changes were found between some of the groups in the fractal dimension analysis. In texture analysis, all but one intergroup comparison showed statistically significant differences. For wetting angle assessment, statistically significant differences were found. These were, however, more numerous when assessing glycol droplets, rather than water droplets. Fractal dimension analysis confirmed a correlation between the intensity of changes in the aligner surface with immersion time in the liquids assessed. Texture analysis showed a high sensitivity to the changes in aligner surface. It failed, however, to reveal changes relative to immersion time. Wetting angle analysis revealed aligner surface degradation for Coca-Cola. It did not, however, prove the dependence of the intensity of this degradation as a function of time. Both Coca-Cola and orange juice can cause aligner surface degradation.

The phenomenon of peri-implant bone corticalization after functional loading does not yet have a definite clinical significance and impact on prognosis. An attempt was made to assess the clinical significance of this phenomenon. This prospective study included 554 patients. Standardized intraoral radiographs documenting the jawbone environment of 1556 implants were collected. The follow-up period was 10 years of functional loading. Marginal alveolar bone loss (MBL) and radiographic bone structure (bone index, BI) were evaluated in relation to intraosseous implant design features and prosthetic work performed. After five years, bone structure abnormalities expressed by a reduction of BI to 0.47 ± 0.21 and MBL = 0.88 ± 1.27 mm were observed. Both values had an inverse relationship with each other (p < 0.0001). Reference cancellous bone showed BI = 0.85 ± 0.18. The same relationship was observed after ten years of functional loading: BI = 0.48 ± 0.21, MBL = 1.49 ± 1.94 mm, and again an inverse relationship (p < 0.0001). Increasing corticalization (lower BI) is strongly associated with increasing marginal bone loss and increasing corticalization precedes future marginal bone loss. Marginal bone loss will increase as corticalization progresses.

The differential diagnosis of benign nevi (BN), dysplastic nevi (DN), and melanomas (MM) represents a considerable clinical problem. These lesions are similar in clinical examination but have different prognoses and therapeutic management techniques. A texture analysis (TA) is a mathematical and statistical analysis of pixel patterns of a digital image. This study aims to demonstrate the relationship between the TA of digital images of pigmented lesions under polarized and non-polarized light and their histopathological diagnosis. Ninety pigmented lesions of 76 patients were included in this study. We obtained 166 regions of interest (ROI) images for MM, 166 for DN, and 166 for BN. The pictures were taken under polarized and non-polarized light. Selected image texture features (entropy and difference entropy and long-run emphasis) of ROIs were calculated. Those three equations were used to construct the texture index (TI) and bone index (BI). All of the presented features distinguish melanomas, benign and dysplastic lesions under polarized light very well. In non-polarized images, only the long-run emphasis moment and both indices effectively differentiated nevi from melanomas. TA is an objective method of assessing pigmented lesions and can be used in automatic diagnostic systems.

Three-dimensional printing is finding increasing applications in today’s world. Due to the accuracy and the possibility of rapid production, the CAD/CAM (computer-aided design/computer-aided manufacturing) technology has become the most desired approach in the preparation of elements, especially in medicine and dentistry. This study aimed to compare the biomechanical properties, fractal dimension (FD), and texture of three selected materials used for 3D printing in dentistry. Three biomaterials used in 3D printing were evaluated. The materials were subjected to the compression test. Then, their mechanical features, FD, and texture were analyzed. All the tested materials showed different values for the studied properties. The only statistically insignificant difference was observed for the force used in the compression test. All three materials showed differences in width and height measurements. The difference in the decrease between the compression plates was also significant. For Dental LT Clear, the mean value was 0.098 mm (SD = 0.010), while for BioMed Amber it was 0.059 mm (SD = 0.019), and for IBT it was 0.356 mm (SD = 0.015). The nominal strain also differed between the materials. IBT had the highest mean value (7.98), while BioMed Amber had the smallest (1.31). FD analysis revealed that Dental LT Clear did not show differences in the structure of the material. The other two materials showed significant changes after the compression test. Texture analysis (TA) revealed similar results: BioMed Amber resin showed significantly less pronounced texture changes compared to the other two materials. BioMed Amber also showed the most stable mechanical properties, whereas those of IBT changed the most. Fractal analysis revealed that IBT showed significant differences from the other two materials, whereas TA showed that only Dental LT Clear did not show changes in its texture after the compression test. Before the compression, however, BioMed Amber differed the most when bone index was taken into account.

The surface topography of orthodontic brackets can have a significant impact on both the effectiveness of the therapy and the behavior of these elements in the oral cavity environment. In this situation, striving to obtain the most uniform, smooth surface in a repeatable manner for each manufactured element should be a sine qua non condition for each supplier of orthodontic brackets. Therefore, it is necessary to analyze the surfaces of orthodontic brackets using different methods. One of them—that is relatively simple and repeatable—is the analysis of the fractal dimension and the analysis of the textures of the optical images on the surface. In the presented study, fractal dimension analysis and texture analysis were performed by selecting four brackets from three different manufacturers (Mini Sprint, Sprint, Nu-Edge, Orthos SS). The area of each bracket slot was analyzed at six predefined points. The smoothest and most uniform and reproducible surface structure was shown by the Mini Sprint bracket. On the other hand, Sprint brackets showed the least homogeneous and least repeatable surface structure.

Currently, an increasing number of medical centers are treating mandibular head fractures surgically. Dedicated screws for compression osteosynthesis have been developed. However, due to the very limited size of the fractured bones and the considerable technical difficulties accompanying the execution of the fixation, there is little room for correction of the positioning and reinsertion of the screws. Therefore, knowing the optimal position of the fixation material is crucial for therapeutic success. The aim of this study is the evaluation of fixation screw position on the mandibular ramus height obtained in the treatment of the condylar head fracture. A total of 57 patients were included in this study. The loss of mandibular ramus height on computed tomography twelve months after mandibular head osteosynthesis was evaluated in relation to the initial distance of the screws from the fracture line, the angle of insertion of the screw into the bone, and the size of the protrusion to the inner side of the condyle. The relationship of the proximity of the screw to the fracture line, angulation, and the size of the protrusion with the loss of ramus height was confirmed. Conclusions: the optimal location for the superior screw is approx. 4 mm below the fracture line (with any angulation), inferior screw is approx. 8 mm (with any angulation), and anterior screw position is approx. 4–5 mm distant from fracture line with the best angulation of 130 degrees to the lateral mandible ramus surface in the coronal plane.

Treatment of temporomandibular ankylosis is challenging and frequently leads to re-ankylosis, relapse, dangerous complications and, in turn, the need for multiple operations. In this article, we present a protocol for the treatment of ankylosis of the temporomandibular joints that assumes earlier intervention with the assistance of 3D virtual surgical planning (3DVSP) and custom biomaterials for better and safer surgical outcomes. Thirty-three patients were treated due to either uni- or bilateral temporomandibular ankylosis. Twenty individuals received temporomandibular prosthesis, whereas seventeen required simultaneous 3D virtual surgical/planned orthognathic surgery as the final correction of the malocclusion. All patients exhibited statistically significant improvements in mouth opening (from 1.21 ± 0.74 cm to 3.77 ± 0.46 cm) and increased physiological functioning of the mandible. Gap arthroplasty and aggressive rehabilitation prior to temporomandibular prosthesis (TMJP) placement were preferred over costochondral autografts. The use of 3DVSP and custom biomaterials enables more precise, efficient and safe procedures to be performed in the paediatric and adolescent population requiring treatment for temporomandibular ankylosis.

The normative functioning of the stomatognathic system and masticatory apparatus requires specific coordination between several structures such as teeth with good occlusion, tongue without ankyloglossia or thrusting, and well-balanced facial muscles. This study aimed to evaluate the influence of non-normative primary functions such as breathing, swallowing, biting and chewing on the consonant pronunciation outcome in adults affected with facial skeletal deformities. Moreover, the authors would like to promote a new kind of speech therapy-orthognathic speech therapy. A total of 181 adults affected by skeletal class II and III malocclusion were included, along with the relationship between the malocclusion, speech deficiency (20 phonemes tested) and primary function disorders, in the subjects before and after surgical correction. The impact of surgery on pronunciation and primary function improvement and types of Polish phonemes most often misarticulated by Polish adults were also examined. Patients underwent combined treatment and received a full speech pathology examination. The treatment improved speech (p < 0.05), and primary functions (p < 0.05). Palatal, alveolar (p < 0.05), fricatives (p < 0.05), and labiodental consonant pronunciation (p < 0.05) improved. The surgical correction of malocclusion leads to better oral motor control and articulation of Polish consonants in adults.

Titanium alloys are used in skeletal surgery. However, once bone union is complete, such fixation material becomes unnecessary or even harmful. Resorbable magnesium materials have been available for several years (WE43 alloy). The aim of this study was to clinically compare magnesium versus titanium open reduction and rigid fixations in mandible condylar heads. Ten patients were treated for fractures of the mandibular head with magnesium headless compression screws (2.3 mm in diameter), and 11 patients were included as a reference group with titanium screws (1.8 mm in diameter) with similar construction. The fixation characteristics (delay, time, and number of screws), distant anatomical results (mandibular ramus height loss, monthly loss rate, and relative loss of reconstructed ramus height), basic functional data (mandibular movements, facial nerve function, and cutaneous perception) and the influence of the effects of the injury (fracture type, fragmentation, occlusion, additional fractures, and associated diseases) on the outcome were evaluated. The long-term results of treatment were evaluated after 18 months. Treatment results similar to those of traditional titanium fixation were found with magnesium screws. Conclusions: Resorbable metal screws can be a favored option for osteosynthesis because surgical reentry can be avoided. These materials provide proper and stable treatment results.

Post- resection defects in the area of the head and neck frequently result in functional impairment of the masticatory system and unaesthetic outcome in the facial appearance. In pediatric population they exert a devastating effect on speech development, breathing and facial growth leading to secondary deformations which are extremely challenging in reconstruction by means of conventional surgery protocols.21 patients (14 males/ 7 females) aged 4-17 years old, treated between 2015 and 2019 due to malignant/benign tumors or congenital deformities requiring mandible resection were enrolled in the study. All patients underwent Virtual Surgical Planning (VSP), mandible tumor resection with use surgery guides and defect reconstruction with microvascular bone graft supported with custom implants. Postoperatively, 3D models used for the planning and postoperative 3D model of the reconstructed mandible were compared using authors method based on easily measurable morphometric measurements (3 angles, 3 linear dimensions), allowing a three-dimensional quantitative assessment of postoperative outcomes of the surgery. Linear regression analysis and one-way analysis of variance were used to evaluate the clinical material. The difference was considered significant if p 0.05.Free fibula flap (FFF) was most commonly used type of the reconstruction of the pediatric mandible. The average maximum deviation for the analyzed cases was 7.7 mm, and the average minimum deviation was -6.09 mm, however without significant differences and the postoperative position of the mandible condyle was comparable to the position designed in the 3DVSP preoperatively. Position of the whole reconstructed mandible by means of individually planned grafts and plates provided comparable position of the ramus and mandible body and no chin deviation as the differences between the pre-operative axial and sagittal angles and their post-operative equivalents were insignificant (p 0,05).This study confirms applicability of the 3DVSP in pediatric craniomaxillofacial surgery. Preoperative planning of the osteotomies, grafting technique and production of the individualized guides and implants provides precise tumor resection and immediate reconstruction adjusted to the specific anatomy of the pediatric patients.

Purpose: Many printers are tempting at low prices, but later their accuracy turns out to be insufficient. The study has included checking the accuracy of printing and reproducing details of 3D printers used in dental technology and dentistry such as MultiJet Printing (ProJet MP3000, 3D Systems) and Fused Deposition Modelling (Inspire S2000, Tiertime). Design/methodology/approach: The 3D prints were created from scans of the maxillary gypsum model with the loss of left premolar. In the test, objects were set to the X and Y-axis. In order to check the dimensional differences after printing, scans of the printed models were superimposed on scans of the plaster model in the GOM Inspect V8 SR1 (Braunschweig, Germany). The focus was on the distance of scans from each other and a deviation map was created for each object. Findings: The average absolute value of deviations for each of models were equalled: FDMfor X-axis 0.06 ± 0.04 mm, for Y-axis 0.07 ± 0.04 mm; MJP- for X-axis- 0.04 ± 0.02 mm, for Y-axis- 0.06 ± 0.02 mm. A chart of arithmetic averages calculated for each tooth for the best printouts in each series show that higher deviation values exist in case of FDM printout. The models printed in the X-axis have smaller values of deviations from those printed in the Y-axis. Practical implications: MultiJet Printing technology can be used to create more precise models than the FDM, but these printouts meet the requirements of dimensional accuracy too. Originality/value: CAD / CAM technology in the future will exist in every dental technology laboratory so it is important to be aware of the way the 3D printers works. By paying attention to the quality of detail reproduction, a Dental Technician is able to choose the best 3D printer for them.

Thanks to the continuous development of light-curing resin composites it is now possible to print permanent single-tooth restorations. The purpose of this study was to compare resin composites for milling -Gandio Blocks (GR), Brilliant Crios (CR) and Enamic (EN) with resin composite for 3D printing—Varseo Smile Crown plus (VSC). Three-point bending was used to measure flexural strength (σf) and flexural modulus (Ef). The microhardness was measured using a Vickers method, while fractographic, microstructural, texture and fractal dimension (FD) analyses were performed using SEM, optical microscope and picture analysis methods. The values of σf ranged from 118.96 (±2.81) MPa for EN to 186.02 (±10.49) MPa for GR, and the values of Ef ranged from 4.37 (±0.8) GPa for VSC to 28.55 (±0.34) GPa for EN. HV01 ranged from 25.8 (±0.7) for VSC to 273.42 (±27.11) for EN. The filler content ranged from 19–24 vol. % for VSC to 70–80 vol. % for GR and EN. The observed fractures are typical for brittle materials. The correlation between FD of materials microstructure and Ef was observed. σf of the printed resin depends on layers orientation and is significantly lower than σf of GR and CR. Ef of the printed material is significantly lower than Ef of blocks for milling.

Venous lake (VL) is a common lesion resulting from the enlargement of thin-walled veins surrounded by a thick wall of fibrous tissue. Photocoagulation is becoming one of the basic methods for treating small vascular lesions. The aim of this study was an application of texture analysis (TA) and fractal dimension analysis (FDA) to evaluate VL treatment. Twenty-three VLs were included in the study. We used a 980 nm diode laser, 6 W, 100 ms pulse mode with a 50% duty cycle. The total dose of energy was in a range from 80 J to 600 J. We used the difference of intensity algorithm for FDA and microcontrast and a co-occurrence matrix for TA. A complete therapeutic effect was achieved in 83%, and in 9%, scar formation was observed after three months. No healing was observed in 4%, and there was partial healing in 4%. No differences in FD were observed between the lesions after three months and the healthy mucosa. The fractal dimension and microcontrast of a vascular lesion are mutually coupled. FDA and TA is a useful and objective method of assessing treatment effects for venous lakes. The non-contact mode of the 980 nm diode laser is an effective and safe method for treating a VL.

Mechanical properties of orthodontic wires can have a very significant impact both on the resistance of the entire appliance to the oral cavity conditions and directly on the effectiveness of the therapy. Striving to achieve repeatability of mechanical characteristics of orthodontic wires of a given type should be an obligatory condition in their production. To achieve it, these components should be thoroughly analyzed using various mechanical tests. Twenty-four steel and nickel-titanium orthodontic wires from four different manufacturers were examined. Each wire was subjected to fractal dimension analysis and texture analysis. The two sides of each wire were compared against each other, as well as in terms of variation in the surface area for each wire type made by different manufacturers. Most wires showed significant variation in fractal dimension and texture, both when comparing two sides of the same wire and between individual wires of a given type made by a single manufacturer. When conducting research and clinically using orthodontic wires made of Ni-Ti alloys and stainless steel, it should be assumed that the surface of orthodontic wires shows a significant degree of variation, and wires of the same type from the same manufacturer may differ significantly in this respect.

Orthodontists correct dental malocclusion, but major facial skeleton deformations (skeletal malocclusion) are often subject to surgical correction. Several speech pathologies are associated with both of the occlusal anomalies mentioned above. The majority of articulation disorders and primary functions cannot be improved without skeletal correction. This study aimed to investigate the outcome of the multimodal and logopaedics treatment of Polish adults affected by skeletal malocclusion and speech-language pathology. A total of 37 adults affected by skeletal Class II and III malocclusion were included, along with the relationship between the malocclusion and speech deficiency (20 phonemes tested) in the subjects before and after surgical correction. The impact of surgery on pronunciation improvement and types of Polish phonemes most often misarticulated by Polish adults were also examined. Patients underwent combined treatment and received a full speech pathology examination. The treatment improved speech (p &lt, 0.05), but the study did not prove that a specific surgery type was associated with pronunciation improvement. Some patients were provided with speech therapy during childhood, yet most had some minor difficulties with lip and tongue movements. Palatal, alveolar (p &lt, 0.05), fricatives (p &lt, 0.05), and labiodental consonant pronunciation (p &lt, 0.05) improved. The surgical correction of malocclusion leads to better articulation of Polish consonants in adults and improves some primary functions.

Treating oncologic patients remains a challenge for surgeons aiming to provide patients with safe margins of resection while maintaining the highest possible quality of life. The latter, in the case of malignancies, requires using sophisticated methods of reconstruction. Thus, we present a case of a 75-year-old patient treated in our department with a rare neoplasm in the region of the temporomandibular joint—a myxofibrosarcoma that was mimicking an inflammatory lesion. The patient underwent two surgeries—firstly alloplasty of the TMJ due to the suspicion of an inflammatory lesion, lately extended to the resection of glenoid fossa and subtemporal fossa contents when the mandible was reconstructed using UHMW-PE (ultra-high molecular weight polyethylene). The patient was also referred for adjuvant radiotherapy and has remained disease-free for over 96 months with very good aesthetics and function of the mandible. The presented case highlights not only the need for increased oncologic awareness but also the possible use of UHMW-PE as a reconstruction material in the broad resection of the maxillofacial region.

Modern treatment in the field of head and neck surgery aims for the least invasive therapy and places great emphasis on restorative treatment, especially in the case of injury and deformation corrective surgery. More and more often, surgeons use CAD/CAM (Computer-Aided Design and Computer-Aided Manufacturing) tools in their daily practice in the form of models, templates, and computer simulations of planning. These tools are based on DICOM (Digital Imaging and Communications in Medicine) files derived from computed tomography. They can be obtained from both fan-beam (FBCT) and cone-beam tomography (CBCT) acquisitions, which are subsequently segmented in order to transform them into a 1-bit 3D model, which is the basis for further CAD processes. Aim: Evaluation of differences in the dimensions of mandible condyle models in fan- versus cone-beam computer tomography for surgical treatment purposes. Methods: 499 healthy condyles were examined in CT-based 3D models of Caucasians aged 8–88 years old. Datasets were obtained from 66 CBCT and 184 FBCT axial image series (in each case, imaging both mandible condyles resulted in the acquisition of 132 condyles from CBCT and 368 condyles from FBCT) and were transformed into three-dimensional models by digital segmentation. Eleven different measurements were performed to obtain information whether there were any differences between FBCT and CBCT models of the same anatomical region. Results: 7 of 11 dimensions were significantly higher in FBCT versus lower in CBCT (p &lt, 0.05).

Reconstruction of the facial skeleton is challenging for surgeons because of difficulties in proper shape restoration and maintenance of the proper long-term effect. ZrO2 implant application can be a solution with many advantages (e.g., osseointegration, stability, and radio-opaqueness) and lacks the disadvantages of other biomaterials (e.g., metalosis, radiotransparency, and no osseointegration) or autologous bone (e.g., morbidity, resorption, and low accuracy). We aimed to evaluate the possibility of using ZrO2 implants as a new application of this material for craniofacial bone defect reconstruction. First, osteoblast (skeleton-related cell) cytotoxicity and genotoxicity were determined in vitro by comparing ZrO2 implants and alumina particle air-abraded ZrO2 implants to the following: 1. a titanium alloy (standard material), 2. ultrahigh-molecular-weight polyethylene (a modern material used in orbital surgery), 3. a negative control (minimally cytotoxic or genotoxic agent action), 4. a positive control (maximally cytotoxic or genotoxic agent action). Next, 14 custom in vivo clinical ZrO2 implants were manufactured for post-traumatologic periorbital region reconstruction. The soft tissue position improvement in photogrammetry was recorded, and clinical follow-up was conducted at least 6 years postoperatively. All the investigated materials revealed no cytotoxicity. Alumina particle air-abraded ZrO2 implants showed genotoxicity compared to those without subjection to air abrasion ZrO2, which were not genotoxic. The 6-month and 6- to 8-year clinical results were aesthetic and stable. Skeleton reconstructions using osseointegrated, radio-opaque, personalized implants comprising ZrO2 material are the next option for craniofacial surgery.

Background: Magnesium has been used as degradable fixation material for osteosynthesis, but it seems that mechanical strength is still a current issue in these fixations. The aim of this study was to evaluate the axial pull-out force of compression headless screws made of magnesium alloy during their resorption. Methods: The tests included screws made for osteosynthesis of the mandible head: 2.2 mm diameter magnesium alloy MgYREZr (42 screws) and 2.5 mm diameter polylactic-co-glycolic acid (PLGA) (42 pieces, control). The screws were resorbed in S&oslash, rensen&rsquo, s buffer for 2, 4, 8, 12, and 16 weeks, and force was measured as the screw was pulled out from the polyurethane block. Results: The force needed to pull the screw out was significantly higher for MgYREZr screws than for PLGA ones (p &lt, 0.01). Within eight weeks, the pull-out force for MgYREZr significantly decreased to one third of its initial value (p &lt, 0.01). The dynamics of this decrease were greater than those of the pull-out force for PLGA screws (p &lt, 0.05). After these eight weeks, the values for metal and polymer screws equalized. It seems that the described reduction of force requires taking into account when using magnesium screws. This will provide more stable resorbable metallic osteosynthesis.

The development of oral surgery and implantology has led to the need for better and more predictable materials. Various substitute materials are now used for bone regeneration. The replacement of scaffolding material by new bone tissue is the most important condition. This study aimed to evaluate the effects of the resorbability of bone substitute materials during regeneration to the jawbone. The study included 88 patients during the 12-month follow-up. All the patients had undergone oral surgical procedures using two different substitute materials&mdash, Cerasorb (high-rate resorbable (&beta, tricalcium phosphate)) and Endobone (low-rate resorbable (hydroxyapatite)). Texture analysis was performed in intraoral radiographs, in which regions of interest were established for the bone substitute materials and reference bone. Five texture features were calculated, namely the sum average (SumAverg), entropy (Entropy), and three Harr discrete wavelet transform coefficients. This study revealed that all 5 features described the healing process well. Entropy was decreased in both cases with time, however, in Cerasorb cases, the texture feature values were very close to those of the reference bone after 12 months of healing (p &lt, 0.05). The wavelet transform coefficient at scale 6 also showed that longitudinal objects appeared in implantation sites, similar to trabecular bone (p &lt, 0.05) after 12 months of healing. The slow-resorbing material restored the structure of the alveolar crest better in terms of producing large objects similar to the components of a barrel bone image (wavelet coefficients), but required a longer time for reconstruction. The fast-resorbing material showed a texture image with a similar scattering of structures to that of the reference bone (entropy) after 12 months.

Background: Oral leukoplakia (OL) is a potential neoplasmic lesion. The aim of this study was to apply texture analysis (TA) and fractal dimension analysis (FDA) to estimate the effectiveness of OL treatment using an Er:YAG laser. Methods: Eighteen patients with 32 lesions were treated. Laser procedures were conducted using the LiteTouch&trade, Er:YAG Dental Laser. The diameter of the operational tip was 1.3 mm, the power was 50 mJ, the frequency was 50 Hz, and the wavelength was 2940 nm. TA was based on long and short-run emphasis inverse moments, difference entropy, inverse difference moment, and wavelet decomposition for two-dimensional photography. FDA was measured using the box-counting method. Results: Total response was achieved in 50% of lesions, partial response was observed in 47%, and 3% of lesions did not respond to treatment. Recurrence occurred in 34% of lesions. TA features indicated pathological images depicting leukoplakia and complete reconstruction of the correct mucosal image after laser ablation. The discrete wavelet transformation feature detects much larger structures than the properties derived from the run-length matrix and co-occurrence matrix. Conclusions: The Er:YAG laser is an effective treatment method in cases of oral leukoplakia. Leukoplakia treatment by Er:YAG laser is an effective modality, as revealed by the oral mucosa microstructure. TA and FDA are promising methods to estimate the effectiveness of OL treatment.

BACKGROUND A noninvasive, accurate and quick diagnosis is very important to general practitioners and specialists who care for the health of patients' oral cavity mucosa. The main enemies are precancerous lesions: leukoplakia and lichen planus (LP). OBJECTIVES The aim of this study was to attempt to formulate a differential diagnosis for leukoplakia vs LP in the oral mucosa based on digital texture analysis in intraoral macrophotography. MATERIAL AND METHODS The study was comprised of 21 patients affected by leukoplakia, 21 affected by LP and 21 healthy volunteers. Intraoral photography of all participants was taken perpendicularly to the buccal mucosa. To achieve the maximum possible contrast, a high-pass filter was applied and level tools were then used to equalize the histograms of the images. After that, the images were converted into 8-bit grayscale. Two features of run length matrix and 2 of co-occurrence matrix were used for texture analysis. Analysis of variance (ANOVA) was used to check for differences. Factor analysis (FA) and classification with artificial neural network (ANN) were performed. RESULTS The results revealed a simple possible differentiation of both types of precancerous lesions from normal mucosa (p < 0.05). Factor analysis and ANN can help in differentiating the 3 study groups from one another. CONCLUSIONS Differential diagnosis of leukoplakia and LP in the oral mucosa based on digital texture analysis in intraoral macrophotography is possible. It can be used to develop smartphone applications and can be also a helpful tool for general dentists to define the clinical problem before a consultation with a specialist.

This article presents a comparison of bone replacement materials in terms of their ability to produce living bone image at the place of their implantation. Five bone replacement materials are compared (Osteovit&mdash, porous collagen, Cerasorb Foam&mdash, collagen scaffolding of synthetic &beta, tricalcium phosphate, Osbone&mdash, synthetic hydroxyapatite, Endobone&mdash, deproteinized bovine-derived cancellous bone hydroxyapatite, and Cerasorb&mdash, synthetic &beta, tricalcium phosphate). Intraoral radiographs are taken immediately after implantation and 12 months later. The texture analysis was performed to assess (texture index, TI) the level of structure chaos (entropy) in relation to the presence of longitudinal elements visible in radiographs (run length emphasis moment). The reference ratio of the chaotic trabecular pattern (Entropy) to the number of longitudinal structures, i.e., trabeculae (LngREmph), is 176:100 (i.e., 1.76 &plusmn, 0.28). Radiological homogeneity immediately after the implantation procedure is a result of the similar shape of its particles (Osbone, Endobone and Cerasorb) or radiolucency (Osteovit, Cerasorb Foam). The particles visible in radiographs were similar in the LngREmph parameters applied to the reference bone, but not in the co-occurrence matrix features. The TI for Osteovit during a 12-month follow-up period changed from 1.55 &plusmn, 0.26 to 1.48 &plusmn, 0.26 (p &gt, 0.05), for Cerasorb Foam from 1.82 &plusmn, 0.27 to 1.63 &plusmn, 0.24 (p &lt, 0.05), for Osbone from 1.97 &plusmn, 0.31 to 1.74 &plusmn, 0.30 (p &lt, 0.01), and for Endobone from 1.86 &plusmn, 0.25 to 1.84 &plusmn, 0.25 (p &gt, 0.05), The observed structure in the radiological image of bone substitute materials containing calcium phosphates obtains the characteristics of a living bone image after twelve months.

The purpose of this study is to compare bone regeneration properties of recently available collagen-calcium phosphate (C-CP) blend as bone substitute (BS) material in oral surgery with calcium phosphate (CP) as well as collagen material (Collagen). Is C-CP better than the classic loose CP, or is it at least equally effective in the jawbone regeneration with the superiority of a coherent consistency?This study included 102 patients during 12-month follow-up. All patients underwent the following surgical procedures: sinus lift (52patients) and tooth extraction (50patients). Patients were divided into 3 groups which are as follows: experimental group with C-CP, CP and pure Collagen as control groups. Texture analysis was performed in intra-oral radiographs. Analyses were performed in the MaZda 4.6 software. Average 2444-pixel ROIs were established in the image of BS materials and normal trabecular bone for texture comparison to evaluate the jawbone regeneration process. Four features were calculated and investigated.Texture analyses revealed that all 4 features described the healing process well. Reference textural value of feature SumOfSqrs102.37 was soft tissue, DifEntr1.1 was not the bone, Entr2.62 was not a bone and LngREmph1.8 was soft tissue. For 12 months, bone regeneration was proved by 3 of 4 investigated features. Only Entr indicated to CP remnants in the ROI (p0.05).This study proves that blended collagen-calcium phosphate as a BS material can bring satisfactory and predictable outcomes in jawbone regeneration.Clinicians can choose a satisfactory and predictable material for bone regeneration treatment.

Idiopathic solitary cysts have a predilection for long bones and the mandible. Although the origin of these cysts is unclear, the prognosis seems to be good, given proper diagnosis and surgical treatment. A 14-year-old female patient with a bone lesion in the right mandibular ramus was referred to the Department of Pediatric Dentistry at the Medical University of Gdansk, Poland. Due to severe headache lasting two days, the patient had previously undergone magnetic resonance imaging (MRI) examination of the head. A unilocular bone cyst in the mandible was discovered incidentally. Comparison with a previous orthopantomogram (OPG) taken two years earlier for routine dental treatment revealed a clearly visible but smaller bone lesion at the same location. Surgery was carried out in order to achieve bone regeneration. This report documents the importance of careful evaluation and assessment of all radiographs taken, as the lesion could have been diagnosed and treated much earlier, thus lowering the risk of complications.

Laser-induced periodic surface structures (LIPSS) are the sub-wavelength periodic nanostructures generated by the femtosecond laser. Implant topography and its nanostructural changes can be important for biomedical applications. In order to compare the surface topography of different implants, appropriate mathematical and physical descriptive methods should be provided. The aim of the study was to evaluate the experimental LIPSS-based—Low Spatial Frequency LIPSS (LSFL) dental implant surfaces. Novel methods of surface analysis, such as Fractal Dimension Analysis and Texture Analysis, were compared to the standard surface roughness evaluation. Secondary, cell viability, and attachment tests were applied in order to evaluate the biological properties of the new titanium surface and to compare their correlation with the physical properties of the new surfaces. A Normal Human Dermal Fibroblast (NHDF) cytotoxicity test did not show an impact on the vitality of the cells. Our study has shown that the laser LIPSS implant surface modifications significantly improved the cell adhesion to the tested surfaces. We observed a strong correlation of adhesion and the growth of cells on the tested surface, with an increase in implant surface roughness with the best results for the moderately rough (2 μm) surfaces. Texture and fractal dimension analyses are promising methods to evaluate dental implants with complex geometry.

For some years now, fixation devices created with resorbable magnesium alloys for the mandibular head have been clinically available and are beginning to be used. It is thus valuable to evaluate the quality of unions in these cases. The aim of this study was radiological comparison of magnesium versus titanium open reduction and rigid fixations in the mandible condylar head. Thirty-one patients were treated for fractures of the mandibular head with magnesium WE43 alloy headless compression screws (diameter 2.3 mm) and, as a reference group, 29 patients were included with similar construction titanium screws (diameter 1.8 mm). The 12-month results of the treatment were evaluated by the texture analysis of CT. Near similar treatment results were found with magnesium screws in traditional titanium fixation. Magnesium screws result in a higher density of the bone structure in the mandibular head. Conclusions: The quantitative evaluation of bone union after surgical treatment of mandibular head fracture with magnesium compression headless screws indicates that stable consolidation was achieved. Undoubtedly, the resorption process of the screws was found to be incomplete after 12 months, evidenced by a marked densification of the bone structure at the fracture site.
