Abstract

Energetic plasma is everywhere in the Universe. The terrestrial Magnetospheric System is a key case where direct measures of plasma energization and energy transport can be made in situ at high resolution. Despite the large amount of available observations, we still do not fully understand how plasma energization and energy transport work. Key physical processes driving much plasma energization and energy transport occur where plasma on fluid scales couple to the smaller ion kinetic scales. These scales (1 RE) are strongly related to the larger mesoscales (several RE) at which large-scale plasma energization and energy transport structures form. All these scales and processes need to be resolved experimentally, however existing multi-point in situ observations do not have a sufficient number of measurement points. New multiscale observations simultaneously covering scales from mesoscales to ion kinetic scales are needed. The implementation of these observations requires a strong international collaboration in the coming years between the major space agencies. The Plasma Observatory is a mission concept tailored to resolve scale coupling in plasma energization and energy transport at fluid and ion scales. It targets the two ESA-led Medium Mission themes Magnetospheric Systems and Plasma Cross-scale Coupling of the ESA Voyage 2050 report and is currently under evaluation as a candidate for the ESA M7 mission. MagCon (Magnetospheric Constellation) is a mission concept being studied by NASA aiming at studying the flow of mass, momentum, and energy through the Earth magnetosphere at mesoscales. Abstract

An analysis of noise attenuation during eighty solar flares between 2013 and 2017 was carried out at frequencies 8-20 MHz using thirty-four SuperDARN radars and the EKB ISTP SB RAS radar. The attenuation was determined on the basis of noise measurements performed by the radars during the intervals between transmitting periods. The location of the primary contributing ground sources of noise was found by consideration of the propagation paths of radar backscatter from the ground. The elevation angle for the ground echoes was determined through a new empirical model. It was used to determine the paths of the noise and the location of its source. The method was particularly well suited for daytime situations which had to be limited for the most part to only two crossings through the D region. Knowing the radio path was used to determine an equivalent vertical propagation attenuation factor. The change in the noise during solar flares was correlated with solar radiation lines measured by GOES/XRS, GOES/EUVS, SDO/AIA, SDO/EVE, SOHO/SEM and PROBA2/LYRA instruments. Radiation in the 1 to 8$\mathring{A}$ and and near 100$\mathring{A}$ are shown to be primarily responsible for the increase in the radionoise absorption, and by inference, for an increase in the D and E region density. The data are also shown to be consistent with a radar frequency dependence having a power law with an exponent of -1.6. Abstract

The complex interaction between shocks and plasma turbulence is extremely important to address crucial features of energy conversion in a broad range of astrophysical systems. We study the interaction between a supercritical, perpendicular shock and pre-existing, fully developed plasma turbulence, employing a novel combination of magnetohydrodynamic and small-scale, hybrid-kinetic simulations where a shock is propagating through a turbulent medium. The variability of the shock front in the unperturbed case and for two levels of upstream fluctuations is addressed. We find that the behaviour of shock ripples, i.e. shock surface fluctuations with short (a few ion skin depths, di) wavelengths, is modified by the presence of pre-existing turbulence, which also induces strong corrugations of the shock front at larger scales. We link this complex behaviour of the shock front and the shock downstream structuring with the proton temperature anisotropies produced in the shock-turbulence system. Finally, we put our modelling effort in the context of spacecraft observations, elucidating the role of novel cross-scale, multispacecraft measurements in resolving shock front irregularities at different scales. Abstract

BACKGROUND: Among patients having noncardiac surgery, perioperative hemodynamic abnormalities are associated with vascular complications. Uncertainty remains about what intraoperative blood pressure to target and how to manage long-term antihypertensive medications perioperatively. OBJECTIVE: To compare the effects of a hypotension-avoidance and a hypertension-avoidance strategy on major vascular complications after noncardiac surgery. DESIGN: Partial factorial randomized trial of 2 perioperative blood pressure management strategies (reported here) and tranexamic acid versus placebo. (ClinicalTrials.gov: NCT03505723). SETTING: 110 hospitals in 22 countries. PATIENTS: 7490 patients having noncardiac surgery who were at risk for vascular complications and were receiving 1 or more long-term antihypertensive medications. INTERVENTION: In the hypotension-avoidance strategy group, the intraoperative mean arterial pressure target was 80 mm Hg or greater; before and for 2 days after surgery, renin-angiotensin-aldosterone system inhibitors were withheld and the other long-term antihypertensive medications were administered only for systolic blood pressures 130 mm Hg or greater, following an algorithm. In the hypertension-avoidance strategy group, the intraoperative mean arterial pressure target was 60 mm Hg or greater; all antihypertensive medications were continued before and after surgery. MEASUREMENTS: The primary outcome was a composite of vascular death and nonfatal myocardial injury after noncardiac surgery, stroke, and cardiac arrest at 30 days. Outcome adjudicators were masked to treatment assignment. RESULTS: The primary outcome occurred in 520 of 3742 patients (13.9%) in the hypotension-avoidance group and in 524 of 3748 patients (14.0%) in the hypertension-avoidance group (hazard ratio, 0.99 [95% CI, 0.88 to 1.12]; P = 0.92). The multifacets and uncertainty surrounding the development of inhibitors further complicate the process of estimating inhibitor rate from the limited data. Bayesian statistical modeling provides a useful tool in generating, enhancing, and exploring the evidence through incorporating all the available information.Methods: We built our Bayesian analysis using three study cases to estimate the inhibitor rates of patients with hemophilia A in three different scenarios: Case 1, a single cohort of previously treated patients (PTPs) or previously untreated patients; Case 2, a meta-analysis of PTP cohorts; and Case 3, a previously unexplored patient population – patients with baseline low-titer inhibitor or history of inhibitor development. The data used in this study were extracted from three published ADVATE (antihemophilic factor [recombinant] is a product of Baxter for treating hemophilia A) post-authorization surveillance studies. Noninformative and informative priors were applied to Bayesian standard (Case 1) or random-effects (Case 2 and Case 3) logistic models. Bayesian probabilities of satisfying three meaningful thresholds of the risk of developing a clinical significant inhibitor (10/100, 5/100 [high rates], and 1/86 [the Food and Drug Administration mandated cutoff rate in PTPs]) were calculated. The effect of discounting prior information or scaling up the study data was evaluated.Results: Results based on noninformative priors were similar to the classical approach. Using priors from PTPs lowered the point estimate and narrowed the 95% credible intervals (Case 1: from 1.3 [0.5, 2.7] to 0.8 [0.5, 1.1]; Case 2: from 1.9 [0.6, 6.0] to 0.8 [0.5, 1.1]; Case 3: 2.3 [0.5, 6.8] to 0.7 [0.5, 1.1]). All probabilities of satisfying a threshold of 1/86 were above 0.65. Increasing the number of patients by two and ten times substantially narrowed the credible intervals for the single cohort study (1.4 [0.7, 2.3] and 1.4 [1.1, 1.8], respectively). Abstract

BACKGROUND: Perioperative bleeding is common in patients undergoing noncardiac surgery. Tranexamic acid is an antifibrinolytic drug that may safely decrease such bleeding. METHODS: We conducted a trial involving patients undergoing noncardiac surgery. Patients were randomly assigned to receive tranexamic acid (1-g intravenous bolus) or placebo at the start and end of surgery (reported here) and, with the use of a partial factorial design, a hypotension-avoidance or hypertension-avoidance strategy (not reported here). The primary efficacy outcome was life-threatening bleeding, major bleeding, or bleeding into a critical organ (composite bleeding outcome) at 30 days. The primary safety outcome was myocardial injury after noncardiac surgery, nonhemorrhagic stroke, peripheral arterial thrombosis, or symptomatic proximal venous thromboembolism (composite cardiovascular outcome) at 30 days. To establish the noninferiority of tranexamic acid to placebo for the composite cardiovascular outcome, the upper boundary of the one-sided 97.5% confidence interval for the hazard ratio had to be below 1.125, and the one-sided P value had to be less than 0.025. RESULTS: A total of 9535 patients underwent randomization. A composite bleeding outcome event occurred in 433 of 4757 patients (9.1%) in the tranexamic acid group and in 561 of 4778 patients (11.7%) in the placebo group (hazard ratio, 0.76; 95% confidence interval [CI], 0.67 to 0.87; absolute difference, -2.6 percentage points; 95% CI, -3.8 to -1.4; two-sided P<0.001 for superiority). A composite cardiovascular outcome event occurred in 649 of 4581 patients (14.2%) in the tranexamic acid group and in 639 of 4601 patients (13.9%) in the placebo group (hazard ratio, 1.02; 95% CI, 0.92 to 1.14; upper boundary of the one-sided 97.5% CI, 1.14; absolute difference, 0.3 percentage points; 95% CI, -1.1 to 1.7; one-sided P = 0.04 for noninferiority). Abstract

BACKGROUND: For patients undergoing noncardiac surgery, bleeding and hypotension are frequent and associated with increased mortality and cardiovascular complications. Tranexamic acid (TXA) is an antifibrinolytic agent with the potential to reduce surgical bleeding; however, there is uncertainty about its efficacy and safety in noncardiac surgery. Although usual perioperative care is commonly consistent with a hypertension-avoidance strategy (i.e., most patients continue their antihypertensive medications throughout the perioperative period and intraoperative mean arterial pressures of 60 mmHg are commonly accepted), a hypotension-avoidance strategy may improve perioperative outcomes. METHODS: The PeriOperative Ischemic Evaluation (POISE)-3 Trial is a large international randomized controlled trial designed to determine if TXA is superior to placebo for the composite outcome of life-threatening, major, and critical organ bleeding, and non-inferior to placebo for the occurrence of major arterial and venous thrombotic events, at 30 days after randomization. Using a partial factorial design, POISE-3 will additionally determine the effect of a hypotension-avoidance strategy versus a hypertension-avoidance strategy on the risk of major cardiovascular events, at 30 days after randomization. The target sample size is 10,000 participants. Patients ≥45 years of age undergoing noncardiac surgery, with or at risk of cardiovascular and bleeding complications, are randomized to receive a TXA 1 g intravenous bolus or matching placebo at the start and at the end of surgery. Patients, health care providers, data collectors, outcome adjudicators, and investigators are blinded to the treatment allocation. Abstract

BACKGROUND: Most patients who take antihypertensive medications continue taking them on the morning of surgery and during the perioperative period. However, growing evidence suggests this practice may contribute to perioperative hypotension and a higher risk of complications. This protocol describes an acute kidney injury substudy of the Perioperative Ischemic Evaluation-3 (POISE-3) trial, which is testing the effect of a perioperative hypotension-avoidance strategy versus a hypertension-avoidance strategy in patients undergoing noncardiac surgery. OBJECTIVE: To conduct a substudy of POISE-3 to determine whether a perioperative hypotension-avoidance strategy reduces the risk of acute kidney injury compared with a hypertension-avoidance strategy. DESIGN: Randomized clinical trial with 1:1 randomization to the intervention (a perioperative hypotension-avoidance strategy) or control (a hypertension-avoidance strategy). INTERVENTION: If the presurgery systolic blood pressure (SBP) is <130 mmHg, all antihypertensive medications are withheld on the morning of surgery. If the SBP is ≥130 mmHg, some medications (but not angiotensin receptor blockers [ACEIs], angiotensin receptor blockers [ARBs], or renin inhibitors) may be continued in a stepwise manner. During surgery, the patients' mean arterial pressure (MAP) is maintained at ≥80 mmHg. During the first 48 hours after surgery, some antihypertensive medications (but not ACEIs, ARBs, or renin inhibitors) may be restarted in a stepwise manner if the SBP is ≥130 mmHg. CONTROL: Patients receive their usual antihypertensive medications before and after surgery. The patients' MAP is maintained at ≥60 mmHg from anesthetic induction until the end of surgery. SETTING: Recruitment from 108 centers in 22 countries from 2018 to 2021. Abstract

The risk of recurrence after discontinuation of anticoagulation for a combined oral contraceptive (COC)-associated venous thromboembolism (VTE) is unclear. Therefore, we conducted a systematic review and meta-analysis to estimate the incidence of recurrent VTE among women with COC-associated VTE, unprovoked VTE and to compare the incidence of recurrent VTE between the two groups. The Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, Embase Classic +Embase and Medline ALL to July 2020 and citations from included studies were searched. Randomized controlled trials, prospective cohort studies and meta-analyses of these study types were selected. The analysis was conducted by random-effects model. Nineteen studies were identified including 1537 women [5828 person-years (PY)] with COC-associated VTE and 1974 women (7798 PY) with unprovoked VTE. Studies were at low risk of bias. The incidence rate of VTE recurrence was 1.22/100 PY [95% confidence interval (CI) 0.92-1.62, I-2 = 6%] in women with COC-associated VTE, 3.89/100 PY (95% CI 2.93-5.17, I-2 = 74%) in women with unprovoked VTE and the unadjusted incidence rate ratio was 0.34 (95% CI 0.26-0.46, I-2 = 3%). The recurrence risk in women after COC-associated VTE is low and lower than after an unprovoked VTE.

Abstract

Background: In the last decade, the family system has changed significantly. Although in the past, older people used to live with their children, nowadays, they cannot always depend on assistance of their relatives. Many older people wish to remain as independent as possible while remaining in their homes, even when living alone. To do so, there are many tasks that they must perform to maintain their independence in everyday life, and above all, their well-being. Information and communications technology (ICT), particularly robotics and domotics, could play a pivotal role in aging, especially in contemporary society, where relatives are not always able to accurately and constantly assist the older person. Objective: The aim of this study was to understand the needs, preferences, and views on ICT of some prefrail older people who live alone. In particular, we wanted to explore their attitude toward a hypothetical caregiver robot and the functions they would ask for. Methods: We designed a qualitative study based on an interpretative phenomenological approach. A total of 50 potential participants were purposively recruited in a big town in Northern Italy and were administered the Fried scale (to assess the participants' frailty) and the Mini-Mental State Examination (to evaluate the older person's capacity to comprehend the interview questions). In total, 25 prefrail older people who lived alone participated in an individual semistructured interview, lasting approximately 45 min each. Overall, 3 researchers independently analyzed the interviews transcripts, identifying meaning units, which were later grouped in clustering of themes, and finally in emergent themes. Constant triangulation among researchers and their reflective attitude assured trustiness. Abstract

Context. Turbulence dominated by large-amplitude, nonlinear Alfven-like fluctuations mainly propagating away from the Sun is ubiquitous in high-speed solar wind streams. Recent studies have demontrated that slow wind streams may also show strong Alfvenic signatures, especially in the inner heliosphere. Aims. The present study focuses on the characterisation of an Alfvenic slow solar wind interval observed by Solar Orbiter between 14 and 18 July 2020 at a heliocentric distance of 0.64 AU. Methods. Our analysis is based on plasma moments and magnetic field measurements from the Solar Wind Analyser (SWA) and Magnetometer (MAG) instruments, respectively. We compared the behaviour of different parameters to characterise the stream in terms of the Alfvenic content and magnetic properties. We also performed a spectral analysis to highlight spectral features and waves signature using power spectral density and magnetic helicity spectrograms, respectively. Moreover, we reconstruct the Solar Orbiter magnetic connectivity to the solar sources both via a ballistic and a potential field source surface (PFSS) model. Results. The Alfvenic slow wind stream described in this paper resembles, in many respects, a fast wind stream. Indeed, at large scales, the time series of the speed profile shows a compression region, a main portion of the stream, and a rarefaction region, characterised by different features. Moreover, before the rarefaction region, we pinpoint several structures at different scales recalling the spaghetti-like flux-tube texture of the interplanetary magnetic field. Finally, we identify the connections between Solar Orbiter in situ measurements, tracing them down to coronal streamer and pseudostreamer configurations. Conclusions. The characterisation of the Alfvenic slow wind stream observed by Solar Orbiter and the identification of its solar source are extremely important aspects for improving the understanding of future observations of the same solar wind regime

Abstract

This White Paper outlines the importance of addressing the fundamental science theme “How are charged particles energized in space plasmas” through a future ESA mission. The White Paper presents five compelling science questions related to particle energization by shocks, reconnection, waves and turbulence, jets and their combinations. Answering these questions requires resolving scale coupling, nonlinearity, and nonstationarity, which cannot be done with existing multi-point observations. In situ measurements from a multi-point, multi-scale L-class Plasma Observatory consisting of at least seven spacecraft covering fluid, ion, and electron scales are needed. The Plasma Observatory will enable a paradigm shift in our comprehension of particle energization and space plasma physics in general, with a very important impact on solar and astrophysical plasmas. It will be the next logical step following Cluster, THEMIS, and MMS for the very large and active European space plasmas community. Being one of the cornerstone missions of the future ESA Voyage 2050 science programme, it would further strengthen the European scientific and technical leadership in this important field., QC 20220517

Abstract

Since inter-particle collisions are to a large extent negligible, the solar wind plasma is typically observed in a state far from thermodynamic equilibrium, meaning that the velocity distributions of particles of different species display significant deviations from a single Maxwellian. These deviations retain the whole history of the interaction of particles with the turbulent electromagnetic fields. Simultaneous measurements of fields and particle velocity distributions at kinetic scales made at different radial distances can help in tracking this history backwards allowing one to identify the physical mechanisms at play to heat and accelerate particles in the interplanetary medium close to the Sun and all along solar wind expansion. In this scenario, data from the Solar Wind Analyzer instrument onboard the ESA Solar Orbiter mission are very promising in addressing the kinetic physics of protons, ions and electrons in the solar wind close to the Sun. The Italian SWA-Solar Orbiter Working Group entitled "Kinetic Processes" has been formed in May 2020 and gathers scientists from several Italian and international institutions and with different expertises (theoretical modeling, observational data analysis and numerical simulations), who convene periodically to discuss these scientific subjects. The main scientific goals of this working group are (i) to maximize the scientific return of Solar Orbiter data within the Heliospheric communities and (ii) to identify open scientific questions and relevant physical problems for which Solar Orbiter data could play a key role and help scientists to find answers and explanations. Exploitation of combined measurements from the in-situ instruments (SWA, MAG, EPD and RPW) onboard Solar Orbiter will provide the unique opportunity of attacking fundamental plasma physics processes responsible for energy exchanges between fluctuations and particles. In this Abstract, we will present some of the activities carried on within the Italian SWA-Solar Orbiter WG "Kinetic Processes" in the last few months, with the aim of engaging the broader scientific community into the discussion and in preparation of the release of the first Orbiter data.

Abstract

Despite more than a half-century of study, the basic physical processes responsible for heating and accelerating the solar wind are still not fully understood. These phenomena are at the center of a hot debate that is of great interest for the Solar Orbiter mission (as discussed in details in the Science Activity Plan, SAP) and are strictly linked to the turbulent nature of solar wind fluctuations which cover an extended range of spatial and temporal scales. So the identification of these physical processes is of primary importance for understanding the origins and evolution of the solar wind and its impact on the different bodies of the solar system. Moreover, in a broader context, it would allow also to achieve significant progress in our understanding of stellar astrophysics. Within this context, the Italian Solar Orbiter-SWA Working Group (WGs) on `Multiscale Physics' was created in response to the interest manifested by scientists from several Italian and international institutions on some important topics such as radial evolution of turbulence and Alfvénicity and link between fluid and kinetic scales; solar wind origin; reconnection, intermittency and particle acceleration in the turbulent solar wind (just to cite some of them), with a particular attention to the synergies with other in-situ and remote sensing instruments on board Solar Orbiter and also with other ESA and NASA missions (e.g. L1 observatories and Parker Solar Probe). The `Multiscale Physics' WG involves scientists with an extensive experience in solar wind turbulence and reconnection processes including expertise in data analysis, simulations and modeling. In this contribution, we present the activity developed so far with a particular focus on the scientific cases identified.

Abstract

International audience; The exponential growth of data volume experienced by astronomy and astrophysics causes new disciplines like machine learning (ML) and data mining (DM) to gain more and more ground in these fields. Applications like clustering, feature selection, automatic classification of events are proving to be a valuable aid in exploiting space data in the era of the synergy between "pure" science and "data-driven" science. The Italian Solar Orbiter-SWA Working Group on Machine Learning and Artificial Intelligence1 together with the European Commission Horizon 2020 project AIDA2, has the scope of applying ML and DM analysis techniques to the Solar Orbiter data. The implementations are numerous. First of all these new techniques can be used to discover unexpected relations between data, can automate tasks so that they can be carried out without human intervention, and can help to forecast physical properties and events. A non-exhaustive list of these activities includes automatic detection of coronal holes in images, automatic recognition of plasma regions, prediction of solar wind properties at 1 AU, classification of solar wind type based on new indicators, analysis of particle velocity distribution functions. In addition, this Working Group will integrate the existing software developed in the context of the various heliospheric missions with the parts regarding Solar Orbiter. These packages are able to handle complex data set with ease and provide statistical analysis and visualization tools. Catalogs of scientific data are also produced, which report, among others, magnetic reconnection and particle acceleration events, detected by routines trained to browse data and select physical processes and features of interest. Abstract

Background: Physicians commonly judge whether a myocardial infarction (MI) is type 1 (thrombotic) vs type 2 (supply/demand mismatch) based on clinical information. Little is known about the accuracy of physicians’ clinical judgement in this regard. We aimed to determine the accuracy of physicians’ judgement in the classification of type 1 vs type 2 MI in perioperative and nonoperative settings. Methods: We performed an online survey using cases from the Optical Coherence Tomographic Imaging of Thrombus (OPTIMUS) Study, which investigated the prevalence of a culprit lesion thrombus based on intracoronary optical coherence tomography (OCT) in patients experiencing MI. Four MI cases, 2 perioperative and 2 nonoperative, were selected randomly, stratified by etiology. Physicians were provided with the patient's medical history, laboratory parameters, and electrocardiograms. Physicians did not have access to intracoronary OCT results. The primary outcome was the accuracy of physicians' judgement of MI etiology, measured as raw agreement between physicians and intracoronary OCT findings. Fleiss’ kappa and Gwet's AC1 were calculated to correct for chance. Results: The response rate was 57% (308 of 536). Respondents were 62% male; median age was 45 years (standard deviation ± 11); 45% had been in practice for > 15 years. Respondents’ overall accuracy for MI etiology was 60% (95% confidence interval [CI] 57%-63%), including 63% (95% CI 60%-68%) for nonoperative cases, and 56% (95% CI 52%-60%) for perioperative cases. Overall chance-corrected agreement was poor (kappa = 0.05), consistent across specialties and clinical scenarios. Conclusions: Physician accuracy in determining MI etiology based on clinical information is poor. Physicians should consider results from other testing, such as invasive coronary angiography, when determining MI etiology.
