This consensus paper provides an overview of the state of the art in research on the aetiology and treatment of nightmare disorder and outlines further perspectives on these issues. It presents a definition of nightmares and nightmare disorder followed by epidemiological findings, and then explains existing models of nightmare aetiology in traumatized and non‐traumatized individuals. Chronic nightmares develop through the interaction of elevated hyperarousal and impaired fear extinction. This interplay is assumed to be facilitated by trait affect distress elicited by traumatic experiences, early childhood adversity and trait susceptibility, as well as by elevated thought suppression and potentially sleep‐disordered breathing. Accordingly, different treatment options for nightmares focus on their meaning, on the chronic repetition of the nightmare or on maladaptive beliefs. Clinically, knowledge of healthcare providers about nightmare disorder and the delivery of evidence‐based interventions in the healthcare system is discussed. Based on these findings, we highlight some future perspectives and potential further developments of nightmare treatments and research into nightmare aetiology.

Hintergrund Alpträume kommen auch im Jugend- und Erwachsenenalter vor und können zum Teil massive Auswirkungen auf das Tagesbefinden der Betroffenen haben. Die wenigen Fragebögen zur Erfassung der Auswirkungen von Alpträumen erheben oft nur einen Teilbereich der möglichen Beeinträchtigungen und berücksichtigen nur das Erwachsenenalter. Fragestellung Der Nightmare Effects Questionnaire (NEQ) wurde entwickelt, um die Lücke an fehlenden umfassenden Messinstrumenten zur Erfassung von Alptraumauswirkungen auf das Tagesbefinden von Jugendlichen und Erwachsenen zu schließen. Material und Methoden Der NEQ ist in zwei Abschnitte unterteilt. Der erste Abschnitt erfasst anhand von sechs Fragen die Häufigkeit verschiedener Traumarten. Im zweiten Abschnitt wird durch 35 Items die Tagesbeeinträchtigung durch Alpträume erfasst. 382 Teilnehmer (14–24 Jahre) füllten den NEQ vollständig aus, und mit den Daten von 73 Probanden mit regelmäßigen Alpträumen wurden die psychometrischen Daten des NEQ geprüft. Ergebnisse Mithilfe einer Hauptkomponentenanalyse wurde die Faktorenstruktur des NEQ geprüft. Es ergaben sich folgende sechs Faktoren: 1) Emotionsregulation, 2) Stress und Aggressivität, 3) Depression, 4) Aufmerksamkeit/Konzentration, 5) Angst, 6) Hyperaktivität. Die Reliabilität der Gesamtskala lag im sehr guten Bereich mit α = 0,929. Diskussion Der NEQ weist eine hohe Reliabilität auf und kann damit als ein gut einsetzbares Instrument zu Erfassung der Tagesbeeinträchtigungen durch Albträume bei Jugendlichen und Erwachsenen angesehen werden. Er ist somit gut in Forschung und Praxis einsetzbar., Background Nightmares are common in adolescents and adults and can sometimes have severe consequences for everyday life. The rare questionnaires for assessment of daytime impairment caused by nightmares only examine some of the possible impairments. In addition, these questionnaires address only adults. Objectives The Nightmare Effects Questionnaire (NEQ) was developed to close the existing gap in measurement possibilities for daytime impairment caused by nightmares in adolescents and adults. Materials and methods The NEQ consists of two sections. Section one contains six questions concerning the frequency of different dreams. The second section consists of 35 daytime impairment items caused by nightmares. The NEQ was completed by 382 participants (14–24 years). The data from 73 nightmare sufferers with at least weekly nightmares were included in the examination of the psychometric properties of the NEQ. Results Using a principal component analysis the factor structure of the NEQ was examined. The following six factors were extracted: 1) emotion regulation, 2) stress and aggression, 3) depression, 4) attention/concentration, 5) anxiety, and 6) hyperactivity. The overall reliability of the NEQ scale was high with α = 0.929. Conclusion The NEQ shows a high reliability and therefore can be considered a useful questionnaire to assess the amount of daytime impairments caused by nightmares in adolescents and adults. Consequently, it is useful for research and practice.
