Abstract

Abstract Backgound In the workup of follicular lymphoma (FL), bone marrow biopsy (BMB) assessment is a key component of FLIPI and FLIPI2, the most widely used outcome scores. During the previous decade, several studies explored the role of FDG‐PET/CT for detecting nodal and extranodal disease, with only one large study comparing both techniques. Methods The aim of our study was to evaluate the diagnostic accuracy and the prognostic impact of both procedures in a retrospective cohort of 299 FL patients with both tests performed at diagnosis. In order to avoid a collinearity bias, FLIPI2 was deconstructed in its founding parameters, and the bone marrow involvement (BMI) parameter separately included as: a positive BMB, a positive PET/CT, the combined “PET/CT and BMB positive” or “PET/CT or BMB positive”. These variables were also confronted independently with the POD24 in 233 patients treated with intensive regimens. Results In the total cohort, bone marrow was involved in 124 and 60 patients by BMB and PET/CT, respectively. In terms of overall survival, age > 60 y.o. and the combined “PET/CT or BMB positive” achieved statistical independence as a prognostic factor. In patients treated with an intensive regimen, only the combined “PET/CT or BMB positive” added prognostic value for a shorter overall survival, when confronted with the POD24. Conclusion Our results show that in FL both BMB and PET/CT should be considered at diagnosis, as their combined assessment provides independent prognostic value in the context of the most widely use clinical scores.

2. Exploring the Deuterium Excess of Cretaceous Arctic Paleoprecipitation Using Stable Isotope Composition of Clay Minerals from the Prince Creek Formation (Maastrichtian) in Northern Alaska

Abstract

We report estimated stable isotope compositions of Artic paleoprecipitation using phyllosilicates sampled from three paleosols and two bentonites in the Prince Creek Formation (Maastrichtian) in northern Alaska. Previous studies reported a deuterium excess in estimates of Arctic paleoprecipitation from the Late Cretaceous by combining hydrogen and oxygen proxy sources, including pedogenic minerals, dinosaurian tooth enamel phosphates, pedogenic siderites, and n-alkane biomarkers. The new dataset produced in this study removes uncertainty on possible explanations (photosynthetic and transpiration) of the deuterium excess by producing stable hydrogen and oxygen isotopic signatures from the same source material. The δD of the phyllosilicates range from −171‰ to −72‰ VSMOW and δ18O ranges from 5.0 to 11.8‰ VSMOW. By assuming a MAT of 6.3 °C and calculating uniquely derived fractionation equations for each phyllosilicate, we report estimated isotopic composition of Late Cretaceous paleoprecipitation with an average δD value of −133‰ VSMOW, corresponding to an average δ18O value of −20.3‰ VSMOW. The estimates of Late Cretaceous paleoprecipitation do not intersect the Global Meteoric Water Line and reveal a reported deuterium excess ranging from 7 to 46 per mil. These results confirm the presence of a deuterium excess in Late Cretaceous Arctic paleoprecipitation and provide new insight to assessing possible explanations for this phenomenon.

En los albores de la democracia en Buenos Aires, un grupo de jovencísimos estudiantes se encontraron en una búsqueda común de técnicas actorales diversas. Con edades y trayectorias diferentes se nuclearon en torno a una profesora de clown que traía nuevas enseñanzas, ideas y experiencias. Los lazos afectivos entre los jóvenes fueron afianzándose y, en las clases, comenzaron a gestarse situaciones, personajes, escenas y obras. La emergencia de un “nosotros” por fuera de las singularidades funcionó como un resguardo emocional que favoreció la creatividad en la intimidad de los encuentros y también en las calles donde presentaban sus números. Así ocuparon colectivamente un lugar vedado durante los años de dictadura militar: el espacio público. Este artículo explora, desde una metodología cualitativa y relacional, la historia de El Clú del Claun. Partiendo del giro afectivo, indaga en los vínculos entre los integrantes, las búsquedas que los unieron y las dinámicas afectivas que estuvieron en la génesis de la singularidad del grupo., At the dawn of democracy in Buenos Aires, a group of very young students met in a common search for different acting techniques. With different ages and backgrounds, they gathered around a clown teacher who brought new teachings, ideas and experiences. The bonds of affection among the young people became stronger and, in the classes, situations, characters, scenes and plays began to develop. The emergence of a “we” outside the singularities worked as an emotional shelter that favored creativity in the intimacy of the meetings, and in the streets where they presented their acts. Thus, they collectively occupied a place forbidden during the years of military dictatorship: the public space. This article explores, from a qualitative and relational methodology, the history of El Clú del Claun. Starting from the affective turn, it explores the links between the members, the searches that united them and the affective dynamics that were at the genesis of the singularity of the group., Na aurora da democracia em Buenos Aires, um grupo de estudantes muito jovens se encontrou numa busca comum por diferentes técnicas de atuação. Com diferentes idades e origens, eles se reuniram em torno de um professor palhaço que trouxe novos ensinamentos, idéias e experiências. Os laços de afeto entre os jovens se fortaleceram e, nas aulas, situações, personagens, cenas e peças teatrais começaram a se desenvolver. O surgimento de um "nós" fora das singularidades funcionou como um abrigo emocional que favoreceu a criatividade na intimidade dos encontros e também nas ruas onde eles apresentavam seus atos. Desta forma, eles ocupavam coletivamente um lugar que era proibido durante os anos da ditadura militar: o espaço público. Este artigo explora, a partir de uma metodologia qualitativa e relacional, a história de El Clú del Claun. A partir da virada afetiva, explora os vínculos entre os membros, as buscas que os reuniam e as dinâmicas afetivas que estavam na gênese da singularidade do grupo.

Abstract

Ga-68-DOTATOC PET/CT is a recently introduced imaging technique for the diagnosis and follow-up of neuroendocrine tumors. A prospective observational study was conducted in seven patients who underwent a Ga-68-DOTATOC PET/CT study. They were suspected of active neuroendocrine tumor lesions, either on initial diagnosis or as a possible recurrence and/or progression of already known tumors. The results of prior imaging studies (MRI, thoracoabdominal CT scan, octreotide...), had been negative or inconclusive. All positive Ga-68-DOTATOC PETs were true positives, confirmed by pathological examination. There were no false positive results. Only one false negative result was found. Ga-68-DOTATOC PET/CC is more sensitive and specific for the detection of primary neuroendocrine tumor lesions, allows for a more complete extension study, and detects recurrences in earlier stages, conditioning changes in staging and surgical treatment. It provides additional information on somatostatin receptor overexpression, which is essential for the indication of PRRT (peptide receptor radionuclide therapy) with Lu177 dotatate.

Abstract

Resumen El PET/TC Ga68-DOTATOC es una tecnica de diagnostico por la imagen, de reciente introduccion en nuestro medio, para el diagnostico y seguimiento de tumores neuroendocrinos. Estudio observacional prospectivo de 7 pacientes a quienes se les realizo un estudio PET/TC Ga68-DOTATOC. Los pacientes tenian sospecha de lesiones tumorales neuroendocrinas activas, en diagnostico inicial o con posible recidiva y/o progresion de tumores ya conocidos. Los resultados de los estudios de imagen realizados previamente (RM, TC toracoabdominal, octreotida…) habian sido negativos o no concluyentes. Todos los PET Ga68-DOTATOC positivos fueron verdaderos positivos, confirmandose por anatomia patologica. No se obtuvieron resultados falsos positivos. Solo se obtuvo un falso negativo. PET/TC Ga68-DOTATOC es mas sensible y especifico en la deteccion de lesiones tumorales primarias neuroendocrinas, permite un estudio de extension mas completo y detecta recidivas en estadios mas precoces, condicionando cambios en la estadificacion y el tratamiento quirurgico. Aporta informacion adicional sobre la sobreexpresion de receptores de somatostatina, esencial para la indicacion de PRRT (terapia con peptidos marcados con radionuclidos) con Lu177-Dotatate.

Abstract

Objectives: This study aimed to assess the real use of cefazolin for methicillin-susceptible Staphylococcus aureus (MSSA) infective endocarditis (IE) in the Spanish National Endocarditis Database (GAMES) and to compare it with antistaphylococcal penicillin (ASP). Methods: Prospective cohort study with retrospective analysis of a cohort of MSSA IE treated with cloxacillin and/or cefazolin. Outcomes assessed were relapse; intra-hospital, overall, and endocarditis-related mortality; and adverse events. Risk of renal toxicity with each treatment was evaluated separately. Results: We included 631 IE episodes caused by MSSA treated with cloxacillin and/or cefazolin. Antibiotic treatment was cloxacillin, cefazolin, or both in 537 (85%), 57 (9%), and 37 (6%) episodes, respectively. Abstract

Ga-68-DOTATOC PET/CT is a recently introduced imaging technique for the diagnosis and follow-up of neuroendocrine tumors. A prospective observational study was conducted in seven patients who underwent a Ga-68-DOTATOC PET/CT study. They were suspected of active neuroendocrine tumor lesions, either on initial diagnosis or as a possible recurrence and/or progression of already known tumors. The results of prior imaging studies (MRI, thoracoabdominal CT scan, octreotide...), had been negative or inconclusive. All positive Ga-68-DOTATOC PETs were true positives, confirmed by pathological examination. There were no false positive results. Only one false negative result was found. Ga-68-DOTATOC PET/CC is more sensitive and specific for the detection of primary neuroendocrine tumor lesions, allows for a more complete extension study, and detects recurrences in earlier stages, conditioning changes in staging and surgical treatment. 18. The Lower Cretaceous in east-central Utah—The Cedar Mountain Formation and its bounding strata

Abstract

Although only recognized as a discrete stratigraphic unit since 1944, the Cedar Mountain Formation represents tens of millions of years of geological and biological history on the central Colorado Plateau. This field guide represents an attempt to pull together the results of recent research on the lithostratigraphy, chronostratigraphy, sequence stratigraphy, chemostratigraphy, and biostratigraphy of these medial Mesozoic strata that document the dynamic and complex geological history of this region. Additionally, these data provide a framework by which to examine the history of terrestrial faunas during the final breakup of Pangaea. In fact, the medial Mesozoic faunal record of eastern Utah should be considered a keystone in understanding the history of life across the northern hemisphere. Following a period of erosion and sediment bypass spanning the Jurassic–Cretaceous boundary, sedimentation across the quiescent Colorado Plateau began during the Early Cretaceous. Thickening of these basal Cretaceous strata across the northern Paradox Basin indicate that salt tectonics may have been the predominant control on deposition in this region leading to the local preservation of fossiliferous strata, while sediment bypass continued elsewhere. Thickening of overlying Aptian strata west across the San Rafael Swell provides direct evidence of the earliest development of a foreland basin with Sevier thrusting that postdates geochemical evidence for the initial development of a rain shadow.

Abstract

Acquired von Willebrand syndrome (AVWS) is a relatively infrequent but often overlooked finding when testing for von Willebrand factor (VWF) levels in a patient with mucocutaneous bleeding. Known causes include cardiovascular disorders, hematologic and solid tumors, autoimmune disorders, hypothyroidism, and drugs. An incoercible oral bleeding after a two-teeth removal in a patient with a mechanical aortic valve eventually raised suspicion about an AVWS that was confirmed through laboratory testing. Substitution therapy with an exogenous VWF factor was required to control the bleeding. Known causes of AVWS, including Heyde’s syndrome, were ruled out. VWF levels finally normalized two days after suspension of allopurinol, which the patient received for symptomatic hyperuricemia, leading to his complete recovery and early discharge without any other complications. AVWS is an underdiagnosed entity due to a lack of testing. Allopurinol has never been postured before as a possible etiology and should be evaluated when reaching a diagnosis.

Abstract

Reported values of D(2) receptor occupancy (RO) achieved by antipsychotic drugs tend to be lower when measured with (123)I-IBZM SPECT than with (11)C-Raclopride PET. Image degrading factors such as attenuation, distance-dependent collimator response and scatter could account for this difference. While attenuation correction is routinely applied to SPECT images, the other degradations are not usually accounted for. The aim of this work was to assess the impact of scatter correction on D(2) RO quantification with (123)I-IBZM SPECT, and to compare the results of both corrected and un-corrected SPECT values with (11)C-Raclopride PET measurements. Phantom experiments as well as within-subject human data from a previous study were used for this purpose. SPECT images were reconstructed using filtered back-projection including attenuation correction (FBP(A)), ordered subsets expectation maximization including attenuation and point spread function corrections (OSEM(A+PSF)) and ordered subsets expectation maximization including attenuation, point spread function and scatter corrections (OSEM(A+PSF+SCT)). PET images were reconstructed using the FBP algorithm and corrected for attenuation, scatter, random coincidences and dead time. Quantification of receptor availability was performed using the tissue ratio at pseudoequilibrium for SPECT, and the simplified reference tissue model (SRTM) for PET. Analysis was performed using both occipital cortex (occ) and cerebellum (cer) as reference regions for both modalities. When images were reconstructed using FBP(A), SPECT D(2) RO values were significantly lower as compared with PET leading to a D(2) RO difference of -20% (CI(95%): -13, -27%) (occ) and -23% (CI(95%): -14, -31%) (cer). When images were reconstructed using OSEM(A+PSF), SPECT D(2) RO values were also lower as compared with PET leading to a D(2) RO difference of -21% (CI(95%): -14, -27%) (occ) and -24% (CI(95%): -18, -30%) (cer). When images were reconstructed using OSEM(A+PSF+SCT), the D(2) RO bias was reduced to -6% (CI(95%): 0, -13%) (occ) and -11% (CI(95%): -4, -18%) (cer). These data suggest that the scatter correction plays a major role in explaining the differences between D(2) RO measurements using (123)I-IBZM SPECT and (11)C-Raclopride PET.

Abstract

Antipsychotic-induced D2 receptor occupancy values tend to be lower when measured with [123I]IBZM SPECT than with [11C]Raclopride PET. To clarify this issue, D2 receptor occupancy was measured in the same subjects using both techniques. Twenty patients with schizophrenia on monotherapy with risperidone (n = 7; 3–9 mg/d), olanzapine (n = 5; 5–20 mg/d) or clozapine (n = 8; 150–450 mg/d) at stable doses, and ten healthy volunteers (HV) underwent both a [123I]IBZM SPECT and a [11C]Raclopride PET examinations in random order on different days within a week. Patients with schizophrenia were scanned at a fixed interval after last dose administration. Quantification of receptor availability was performed using the most conventional methods from the literature: the tissue ratio derived specific uptake ratios (SUR) were used for SPECT, and simplified reference tissue model (SRTM) derived binding potentials (BPND) for PET. Analysis was performed using both occipital cortex and cerebellum as reference regions for both modalities. Striatal D2 receptor occupancy was measured as the percentage reduction of [123I]IBZM SUR or [11C]Raclopride BPND compared to the population average measured in HV using the same modality. Occupancy values measured by SPECT were lower than those measured with PET, by 12.4% and 13.8% when occipital cortex and cerebellum were used as reference regions. This difference should be taken in consideration when interpreting reported antipsychotic striatal D2 receptor occupancy values from the literature.

Abstract

To assess the paroxetine-induced serotonin transporter (SERT) occupancy (SERTocc) using in vivo (123)I-ADAM SPECT.(123)I-ADAM SPECT was used to investigate the SERTocc induced by paroxetine in major depression disorder (MDD) patients, to compare the SERT availability in drug-free MDD patients and healthy volunteers, and to study the relationship between paroxetine plasma concentrations (Cp) and SERTocc.Measures of SERT availability by means of (123)I-ADAM SPECT were obtained in ten MDD patients before and after 4- to 6-week treatment with paroxetine 20 mg/day. (123)I-ADAM SPECT measures of SERT availability from a group of ten previously studied age-matched healthy volunteers were used for comparison. The relationship between percentages of SERTocc and paroxetine Cp was studied using an E (max) model.Mean SERTocc values were 66.4 +/- 9.5% in midbrain, 63.0 +/- 9.6% in thalamus, and 61.3 +/- 10.9% in striatum. No significant differences in SERTocc were found among these three regions. No significant differences in mean SERT availability were found in any region between drug-free MDD patients (midbrain = 1.14 +/- 0.15; thalamus = 0.85 +/- 0.13; striatum = 0.70 +/- 0.07) and healthy volunteers (midbrain = 1.19 +/- 0.22; thalamus = 0.96 +/- 0.14; striatum = 0.67 +/- 0.15). The E (max) model returned a SERTocc(max) = 70.5% and a Cp(50) = 2.7 ng/ml.Using (123)I-ADAM SPECT, treatment with paroxetine 20 mg/day leads to more than 60% SERTocc on average in cerebral regions with known high SERT density. Data from this study do not support the existence of SERT availability differences between drug-free MDD patients and healthy volunteers. Finally, the E (max) model is suitable for the study of paroxetine Cp relationship to (123)I-ADAM SPECT-measured SERTocc. This approach may be useful for pharmacokinetic-pharmacodynamic relationships in drug development.

Abstract

El diseño gráfico resulta tan cotidiano como inadvertido. De esta paradoja surge la necesidad de su actualización. Remitiéndose a su función más primigenia, la de comunicar, este artículo propone replantear el concepto desde la base con la intención de generar nuevas estrategias que despierten el interés en los públicos y multipliquen las posibilidades de la exposición en cuanto a experiencia dialéctica. Para este propósito las artes visuales sirven de inspiración y las nuevas tecnologías nos permiten experimentar con el diseño y creación de espacios de exposición.

Abstract

Towards the end of the dictatorship and the beginning of democracy, Buenos Aires witnessed the formation of an artistic movement cataloged by the public and the press as "underground". Their intense and ephemeral actions were developed mainly in precariously sustained production spaces, often transitory and also on public roads ". However, they also soon joined official institutions linked to "high culture" or public dependency. The city that during the years of military dictatorship (1976-1983) had remained strictly, controlled, watched, limited, became, with the return of democracy, an extension for experimentation, configuring new ways of inhabiting it. It was so that the artists did not delay in reproducing it. The objective of the present work is to reconstruct a cartography of counterculture spaces (in opposition to those of the hegemonic culture) postulating that artists traveled, not only by improvised and precarious sites, but also by official and public institutions.
