Abstract Background Cancer is a multifactorial disease. The large impact of occupational exposure on the burden of cancer continues to be a paramount public health concern that deserves more attention. The study aims to evaluate cancer-specific mortality risk in relation to sectors of employment. Methods We used a cohort from the Rome Longitudinal Study (ROL) and linked it with the National Social Insurance Agency (INPS) database to obtain working histories. We considered the longest duration of employment in a sector as a proxy of exposure, and insurance activities as the reference category. A Cox regression adjusted model was used to examine the associations between cancer-specific causes of death and the sector of employment in men and women. A similar analysis was performed considering the length of employment (≤ 10 years versus ≥ 10 years). Results The study population comprised 910,559 (52% of the total population of the cohort after linkage with INPS) 30 + yr employees (53% men and 47% women) followed for a total period of approximately 7 million years. The outcomes confirmed some well-known associations (e.g. lung and pleura in construction, pleura in paper and printing, and lung in wood and leather) and suggested possible high-risk sectors that have not yet been thoroughly investigated. In men we observed an increased mortality risk for stomach cancer in the printing and paper industry (HR = 1.69, 95% CI:1.11–2.57) as well as for stomach and lung cancer in cleaning activities (HR = 1.98, 95% CI:1.13–3.49 and HR = 1.55, 95% CI:1.22–1.98, respectively). Among women, there was an elevated mortality risk in the cleaning industry for all malignant cancers (HR = 1.15, 95% CI:1.03–1.29), liver cancer (HR = 1.94, 95% CI:1.08–3.48) and cancer of the lympho-hematopoietic tissue (HR = 1.65, 95% CI:1.09–2.50). Conclusions The results showed an increased risk of cancer death in some traditional industrial sectors compared to the reference category of insurance activities such as construction and wood and leather products and limited evidence in sectors like cleaning, accommodation and food services and hairdressing. The adopted method proved to be effective in monitoring occupational risks and activating proper prevention initiatives and further insights.

Neurodegenerative diseases are illnesses that affect the central nervous system (CNS) characterized by a series of symptoms such as dementia, motor disturbances, behavioural and psychological disorders, and cognitive impairments. The most common neurodegenerative disorders are Alzheimer's disease and Parkinson's disease, for which radiopharmaceuticals have been developed and approved for the purpose of PET investigation. Biomarkers are molecules that can be studied and used for diagnostic purposes, to monitor diseases, and to identify potential risk factors early.

ObjectiveThe diagnosis of malignant mesothelioma (MM) can be devastating for both patients and caregivers, who may experience intense suffering from a physical, psychological, and interpersonal perspective. Despite the extensive literature on caregiver distress, there is a lack of validated measures to provide evidence of psychological distress of caregivers of MM patients. Therefore, in a previous study we developed the Mesothelioma Psychological Distress Tool–Caregivers (MPDT-C) to evaluate the specific profile of psychological distress in this population. This paper describes the item selection, factor analysis, and psychometric evaluation of the revised MPDT-C.MethodsThe analyses of this work first aimed to confirm the previous factorial structure. In the case of nonfit, it aimed to find an alternative structure and causes of nonfit in the model. Examination of the fit of the factorial model was conducted using a Bayesian approach.ResultsThe final version of the MPDT-C is a 7-item self-report questionnaire consisting of one factor (Burden for the caregiver).ConclusionHaving a short self-report questionnaire to assess the psychological distress experienced by caregivers of MM patients has several advantages. First, it is suited to epidemiological studies where multiple variables and measures are involved. Second, it is easy to administer and does not take much time to complete. Therefore, the MPDT-C can also be administered in clinical contexts, perhaps when MM patients attend follow-up medical evaluation. Lastly, short measures are less affected by cognitive fatigue, which is very common in a long battery of tests and could affect response reliability.

BackgroundThe widespread use of radiofrequency (RF) sources, ranging from household appliances to telecommunications devices and military equipment, raises concerns among people and regulatory agencies about the potential health risks of RF exposure. Consequently, several in vitro and in vivo studies have been done to investigate the biological effects, in particular non-thermal, of this non-ionizing radiation. To date, this issue is still being debated due to the controversial results that have been reported. Furthermore, the impact of different RF signal modulations on biological systems remains poorly investigated. The present in vitro study aims to evaluate the cytotoxicity and genotoxicity of continuous or pulsed 1.6 GHz RF in human dermal fibroblasts (HDF).MethodsHDF cultures were exposed to continuous and pulsed 1.6 GHz RF, for 2 h, with Specific Absorption Rate (SAR) of 0.4 W/kg. The potential biological effects of 1.6 GHz RF on HDF were assessed with a multi-methodological approach, analyzing the effects on cell cycle, ultrastructure, protein expression, mitotic spindle, CREST stained micronuclei, chromosome segregation and γ-H2AX/53BP1 foci.Results1.6 GHz RF exposure modified proteins expression and morphology of HDF. Specifically, the expression of different heat-shock proteins (HSP) (i.e., HSP-90, HSP-60, and HSP-25) and phospho-AKT were affected. In addition, both continuous and pulsed RF modified the cytoskeletal organization in HDF and increased the number of lysosomes, while the formation of autophagosomes was observed only after pulsed RF exposure. Mitotic spindle anomalies were also found after exposure. However, no significant effect was observed on cell cycle, chromosome segregation, CREST-stained micronuclei and γ-H2AX/53BP1 foci.ConclusionThe results of the present study show the absence of genotoxic damage in 1.6 GHz RF exposed HDF and, although mitotic spindle alterations were observed, they did not have an aneugenic effect. On the other hand, changes in some proteins expression and cell ultrastructure in exposed HDF suggest that RF can potentially induce cell alterations at the morphological and molecular levels.

In this work, a completely green method was applied for the extraction of lycopene from tomato skins waste. The combination of the ultrasound-assisted technique (UAE) and the use of volatile natural deep eutectic solvent (VNADES) menthol: thymol 1:1 allowed to obtain a green extraction process. Lycopene content was assessed by HPLC-DAD and compared with the lycopene extraction yields obtained using α-pinene and n-hexane as extraction solvents. The evaporation of the DES menthol:thymol was conducted using a Vacuum Vortex Evaporator. The highest concentration of extracted lycopene was obtained with n-hexane (735.9 mg/g of dried extract (DE)) compared to terpene-based solvents; 468.1 mg/g and 358.7-484.2 mg/g DE were recovered from α-pinene and DES menthol/thymol as azeotropic mixture and as such, respectively.

Background: Starting from March 2020 until December 2021, different phases of Covid-19 pandemic have been identified in Italy, with several containing/lifting measures progressively enforced by the National government. In the present study, we investigate the change in occupational risk during the subsequent pandemic phases and we propose an estimate of the incidence of the cases by economic sector, based on the analysis of insurance claims for compensation for Covid-19. Methods: Covid-19 epidemiological data available for the general population and injury claims of workers covered by the Italian public insurance system in 2020–2021 were analyzed. Monthly Incidence Rate of Covid-19 compensation claims per 100,000 workers (MIRw) was calculated by the economic sector and compared with the same indicator for general population in different pandemic periods. Results: The distribution of Covid-19 MIRw by sector significantly changed during the pandemic related to both the strength of different waves and the mitigation/lifting strategies enforced. The level of occupational fraction was very high at the beginning phase of the pandemic, decreasing to 5% at the end of 2021. Healthcare and related services were continuously hit but the incidence was significantly decreasing in 2021 in all sectors, except for postal and courier activities in transportation and storage enterprises. Conclusion: The analysis of compensation claim data allowed to identify time trends for infection risk in different working sectors. The claim rates were highest for human health and social work activities but the distribution of risk among sectors was clearly influenced by the different stages of the pandemic.

Introduction Countries have made policy commitments through the sustainable development goals (SDGs) to ensure health and promote decent work for all. (1) SDG targets 3.9 and 8.8 are to 'substantially [...], Through sustainable development goals 3 and 8 and other policies, countries have committed to protect and promote workers' health by reducing the work-related burden of disease. To monitor progress on these commitments, indicators that capture the work-related burden of disease should be available for monitoring workers' health and sustainable development. The World Health Organization and the International Labour Organization estimate that only 363 283 (19%) of 1 879 890 work-related deaths globally in 2016 were due to injuries, whereas 1 516 607 (81%) deaths were due to diseases. Most monitoring systems focusing on workers' health or sustainable development, such as the global indicator framework for the sustainable development goals, include an indicator on the burden of occupational injuries. Few such systems, however, have an indicator on the burden of work-related diseases. To address this gap, we present a new global indicator: mortality rate from diseases attributable to selected occupational risk factors, by disease, risk factor, sex and age group. We outline the policy rationale of the indicator, describe its data sources and methods of calculation, and report and analyse the official indicator for 183 countries. We also provide examples of the use of the indicator in national workers' health monitoring systems and highlight the indicator's strengths and limitations. We conclude that integrating the new indicator into monitoring systems will provide more comprehensive and accurate surveillance of workers' health, and allow harmonization across global, regional and national monitoring systems. Inequalities in workers' health can be analysed and the evidence base can be improved towards more effective policy and systems on workers' health. Par le biais des objectifs de developpement durable 3 et 8 ainsi que d'autres mesures, plusieurs pays se sont engages a proteger et promouvoir la sante des travailleurs en reduisant l'impact des maladies liees au travail. Mais pour evaluer leurs progres en la matiere, il convient de mettre en place des indicateurs estimant l'impact des maladies liees au travail afin de placer le developpement durable et la sante des travailleurs sous surveillance. D'apres l'Organisation mondiale de la Sante et l'Organisation internationale du Travail, seulement 363 283 (19%) des 1 879 890 deces lies au travail dans le monde en 2016 decoulaient de blessures, tandis que 1 516 607 (81%) d'entre eux etaient causes par des maladies. La plupart des systemes de surveillance qui s'interessent a la sante des travailleurs ou au developpement durable, comme le cadre mondial d'indicateurs pour les objectifs de developpement durable, comportent un indicateur relatif a l'impact des accidents de travail. Cependant, rares sont ceux qui possedent un indicateur concernant l'impact des maladies professionnelles. Pour combler cette lacune, nous devoilons un nouvel indicateur mondial: le taux de mortalite du aux maladies attribuables a certains facteurs de risque professionnels classe par maladie, facteur de risque, sexe et categorie d'age. Nous exposons le motif politique de l'indicateur, decrivons l'origine des donnees et les methodes de calcul, et communiquons et analysons l'indicateur officiel pour 183 pays. Nous fournissons egalement des exemples de la facon dont l'indicateur peut etre utilise dans des systemes nationaux de surveillance de la sante des travailleurs et soulignons ses forces et faiblesses. Nous concluons en affirmant que l'integration de ce nouvel indicateur dans les systemes de surveillance offrira un suivi plus complet et precis de la sante des travailleurs et ouvrira la voie a une harmonisation des systemes mondiaux, nationaux et regionaux. Il est possible d'analyser les inegalites en matiere de sante des travailleurs et d'en ameliorer les bases factuelles afin d'etablir des politiques et systemes plus efficaces dans ce domaine. Para supervisar los avances en el cumplimiento de estos compromisos, deberia disponerse de indicadores que reflejen la carga de morbilidad relacionada con el trabajo, a fin de controlar la salud de los trabajadores y el desarrollo sostenible. La Organizacion Mundial de la Salud y la Organizacion Internacional del Trabajo estiman que solo 363 283 (19%) de las 1 879 890 muertes relacionadas con el trabajo a nivel mundial en 2016 se debieron a lesiones, mientras que 1 516 607 (81%) muertes se debieron a enfermedades. La mayoria de los sistemas de vigilancia centrados en la salud de los trabajadores o el desarrollo sostenible, como el marco de indicadores mundiales para los objetivos de desarrollo sostenible, incluyen un indicador sobre la carga de las lesiones laborales. No obstante, pocos de estos sistemas cuentan con un indicador sobre la carga de las enfermedades relacionadas con el trabajo. Para subsanar esta carencia, presentamos un nuevo indicador mundial: la tasa de mortalidad por enfermedades atribuibles a factores de riesgo laborales seleccionados, por enfermedad, factor de riesgo, sexo y grupo de edad. Describimos la justificacion politica del indicador, describimos sus fuentes de datos y metodos de calculo, e informamos y analizamos el indicador oficial para 183 paises. Tambien proporcionamos ejemplos del uso del indicador en los sistemas nacionales de vigilancia de la salud de los trabajadores y destacamos las ventajas y las limitaciones del indicador. Concluimos que la integracion del nuevo indicador en los sistemas de vigilancia proporcionara una vigilancia mas exhaustiva y precisa de la salud de los trabajadores, y permitira la armonizacion entre los sistemas de vigilancia mundiales, regionales y nacionales. Se podran analizar las desigualdades en la salud de los trabajadores y se podra mejorar la base de evidencias para lograr politicas y sistemas mas eficaces en materia de salud de los trabajadores.

Telomere length can be maintained either by the telomerase enzyme or by alternative lengthening of telomeres (ALT), which is based on telomeric recombination. However, both mechanisms are inactive in most human somatic cells. ATRX has been previously identified as an ALT repressor gene. Nonetheless, TP53 is also deficient in most ALT cell lines, and previous works showed that it is an inhibitor of homologous recombination (HR). Despite this, the role of p53 as an ALT repressor has not been previously examined. Therefore, we investigated the effects of p53 and ATRX inhibition on normal human fibroblasts (devoid of any mutation), in the presence or absence of X‐ray‐induced telomeric damage. Performing immunofluorescence with antibodies for RAD51, H2AX, and TRF1 (for studying HR‐mediated DNA damage repair) and CO‐FISH (for telomeric sister chromatid exchanges), we observed that HR is a normal mechanism for the repair of telomeric damage, present also in noncancer cells. Moreover, we discovered that telomeric HR, as for HR in general, is significantly inhibited by p53. Indeed, we observed that inhibition of p53 drastically increases telomeric sister chromatid exchanges. We also confirmed that ATRX inhibition increases telomeric recombination. In particular, we observed an increase in crossover products, but a much higher increase in noncrossover products.

In order to avoid replicative senescence, tumor cells must acquire a telomere maintenance mechanism. Beside telomerase activation, a minority of tumors employs a recombinational mechanism called Alternative Lengthening of Telomeres (ALT). Several studies have investigated the potential ALT stimulation by inactivation of ATRX in tumor cells, obtaining contrasting results. Differently, since ALT can be viewed as a mechanism to overcome telomere shortening-mediated replicative senescence, we have investigated the effects of the inhibition of ATRX and p53 in aging primary fibroblasts. We observed that senescence leads to a phenotype that seems permissive for ALT activity, i.e. high levels of ALT-associated PML bodies (APB), telomeric damage and telomeric cohesion. On the other hand, RAD51 is highly repressed and thus telomeric recombination, upon which the ALT machinery relies, is almost absent. Silencing of ATRX greatly increases telomeric recombination in young cells, but is not able to overcome senescence-induced repression of homologous recombination. Conversely, inhibition of both p53 and ATRX leads to a phenotype reminiscent of some aspects of ALT activity, with a further increase of APB, a decrease of telomere shortening (and increased proliferation) and, above all, an increase of telomeric recombination.

One of the most intriguing and still pending questions in radiobiology is to understand whether and how natural environmental background radiation has shaped Life over millions of years of evolution on Earth. Deep Underground Laboratories (DULs) represent the ideal below-background exposure facilities where to address such a question. Among the few worldwide DULs, INFN-Laboratorio Nazionale del Gran Sasso (LNGS) is one of the largest in terms of size and infrastructure. Designed and built to host neutrino and dark matter experiments, since the 1990 s the LNGS has been one of the first DULs to systematically host radiobiology experiments. Here we present the DISCOVER22 (DNA Damage and Immune System Cooperation in VEry low Radiation environment 2022) experiment recently started at LNGS. DISCOVER22 aims at investigating how the low radiation background modulates the Immune System (IS) response in in vitro and in vivo models. Underground radiobiology experiments are particularly complex and tricky to design and perform. In these studies, the accurate characterization of exposure scenarios is mandatory, but a challenging aspect is to understand how the very few ionizing tracks in the ultra-Low Radiation Environment (LRE) interact with the living matter in space and time in order to trigger different biological responses. In this Perspective, we describe these challenges and how we address them through a microdosimetric and a radiobiological approaches. We aim at linking physical microdosimetric measurements and the corresponding biological radiation responses by using radiation biophysical models that could shed light on many as yet unresolved questions.

Objective: Colorectal cancer (CRC) during pregnancy is a rare occurrence, with a reported incidence of 0.8 cases per 100,000 pregnancies. Managing CRC during pregnancy poses substantial challenges for clinicians: the diagnosis is often complicated and delayed due to symptom overlap with pregnancy-related manifestations, and medical imaging is constrained by safety concerns for the foetus. Methods: This article presents two cases of advanced CRC diagnosed and managed during pregnancy. Additionally, we conducted a systematic review of the literature to assess diagnostic and prognostic factors involved in CRC in pregnant individuals. The systematic review, with pre-registration and approval through Prospero, involved an extensive search of medical databases (Pubmed, Web of Science, Scopus and Scholar) and statistical analysis using t-test for continuous variables and chi square for dichotomous variables. Results: A total of 1058 studies were identified. After applying exclusion criteria, sixty-six studies were included. Women whose initial symptoms were severe abdominal pain not responsive to common medical treatments and constipation (acute abdomen) had a mean gestational age at delivery lower than those who presented with paucisymptomatic onset. In our study groups, women who underwent chemotherapy during pregnancy had a higher mean gestational age at delivery and did not experience worse neonatal outcomes compared to those who did not undergo chemotherapy. Conclusions: CRC during pregnancy poses unique diagnostic and therapeutic challenges. Collaborative efforts among various medical disciplines are essential to manage CRC during pregnancy.

IntroductionWhile there is consistent evidence on the effects of heat on workers' health and safety, the evidence on the resulting social and economic impacts is still limited. A scoping literature review was carried out to update the knowledge about social and economic impacts related to workplace heat exposure.MethodsThe literature search was conducted in two bibliographic databases (Web of Science and PubMed), to select publications from 2010 to April 2022.ResultsA total of 89 studies were included in the qualitative synthesis (32 field studies, 8 studies estimating healthcare-related costs, and 49 economic studies). Overall, consistent evidence of the socioeconomic impacts of heat exposure in the workplace emerges. Actual productivity losses at the global level are nearly 10% and are expected to increase up to 30–40% under the worst climate change scenario by the end of the century. Vulnerable regions are mainly low-latitude and low- and middle-income countries with a greater proportion of outdoor workers but include also areas from developed countries such as southern Europe. The most affected sectors are agriculture and construction. There is limited evidence regarding the role of cooling measures and changes in the work/rest schedule in mitigating heat-related productivity loss.ConclusionThe available evidence highlights the need for strengthening prevention efforts to enhance workers' awareness and resilience toward occupational heat exposure, particularly in low- and middle-income countries but also in some areas of developed countries where an increase in frequency and intensity of heat waves is expected under future climate change scenarios.

Objectives Workers in the construction industry have been exposed to asbestos in various occupations. In Italy, a National Mesothelioma Registry has been implemented more than 20 years ago. Using cases selected from this registry and exploiting existing control data sets, we estimated relative risks for pleural mesothelioma (PM) among construction workers.Design Case–control study.Setting Cases from the National Mesothelioma Registry (2000–2018), controls from three previous case–control studies.Methods We selected male PM incident cases diagnosed in 2000–2018. Population controls were taken from three studies performed in six Italian regions within two periods (2002–2004 and 2012–2016). Age-adjusted and period-adjusted unconditional logistic regression models were fitted to estimate odds ratios (OR) for occupations in the construction industry. We followed two approaches, one (primary) excluding and the other (secondary) including subjects employed in other non-construction blue collar occupations for >5 years. For both approaches, we performed an overall analysis including all cases and, given the incomplete temporal and geographic overlap of cases and controls, three time or/and space restricted sensitivity analyses.Results The whole data set included 15 592 cases and 2210 controls. With the primary approach (4797 cases and 1085 controls), OR was 3.64 (2181 cases) for subjects ever employed in construction. We found elevated risks for blue-collar occupations (1993 cases, OR 4.52), including bricklayers (988 cases, OR 7.05), general construction workers (320 cases, OR 4.66), plumbers and pipe fitters (305 cases, OR 9.13), painters (104 cases, OR 2.17) and several others. Sensitivity analyses yielded very similar findings. Using the secondary approach, we observed similar patterns, but ORs were remarkably lower.Conclusions We found markedly increased PM risks for most occupations in the construction industry. These findings are relevant for compensation of subjects affected with mesothelioma in the construction industry.

Abstract Background The Italian mesothelioma registry (ReNaM) estimates mesothelioma incidence and addresses its etiology by assessing cases’ exposures but cannot provide relative risk estimates. Objectives i) To estimate pleural mesothelioma relative risk by industry and occupation and by ReNaM categories of asbestos exposure; and ii) to provide quantitative estimates of the exposure–response relationship. Methods A population-based mesothelioma case–control study was conducted in 2012–2014 in five Italian regions. Cases and age and gender frequency-matched controls were interviewed using a standard ReNaM questionnaire. Experts coded work histories according to international standard classifications of industries/occupations and assigned asbestos exposure according to ReNaM categories. Job codes were further linked to SYN-JEM, a quantitative job-exposure matrix. Cumulative exposure (CE, f/mL-years) was computed by summing individual exposures over lifetime work history. Unconditional logistic regression analyses adjusted by gender, centre and age were fitted to calculate odds ratios (OR) and 95% confidence intervals (CI). Results Among men we observed increased risks of mesothelioma in many industries and associated occupations, including: asbestos-cement (OR = 3.43), manufacture of railroad equipment (OR = 8.07), shipbuilding and repairing (OR = 2.34), iron and steel mills (OR = 2.15), and construction (OR = 1.94). ORs by ReNaM exposure categories were as follows: definite/probable occupational exposure (OR = 15.8, men; OR = 8.80, women), possible occupational (OR = 2.82, men; OR = 3.70, women), sharing home with an exposed worker (OR = 2.55, men; OR = 10.3, women), residential (OR = 2.14, men; OR = 3.24, women). Based on SYN-JEM, mesothelioma risk increased by almost 30% per f/mL-year (OR = 1.28, CI 1.16–1.42). Conclusions Out study involved five regions with historically different types and levels of industrial development, encompassing one third of the Italian population and half of Italian mesothelioma cases. As expected, we found increased pleural mesothelioma risk in the asbestos industry and in trades with large consumption of asbestos materials. Clear associations were found using both qualitative (ReNaM classifications) and quantitative estimates (using SYN-JEM) of past asbestos exposure, with clear evidence of an exposure–response relationship.

Some tumors employ a mechanism called alternative lengthening of telomeres (ALT) to counteract telomere shortening‐induced replicative senescence. Several hallmarks are used to identify cell lines and tumors as ALT‐positive. Here, we analyzed a panel of ALT‐positive and ‐negative cancer cell lines to investigate the specificity and sensibility of ALT‐associated markers. We found that all the markers showed high sensitivity, indicating that cells not showing ALT markers are not ALT cells. Conversely, specificity varied significantly, i.e., many markers yield false positives. Detection of false positives may have influenced previous estimations of ALT incidence among tumors. Moreover, claims on the ‘coexistence’ of ALT and telomerase perhaps should be reconsidered. ABSTRACT Given Taiwan's limited formal diplomatic recognition, this paper examines interviews with diplomats/officials from one of Taiwan's Pacific allies, Tuvalu, as well as interviews with other Pacific diplomats in Taiwan and Taiwanese diplomats/officials, to outline diverging conceptions of diplomacy in Taiwan's official alliances. The paper first analyses interviews with Pacific diplomats in Taiwan, discussing how diplomats link meanings of diplomacy to socio‐cultural protocols in their home countries. Three themes that emerge in explanations of diplomacy are bartering/exchange, building/maintaining friendships and properly building/reinforcing inter‐island relationships. The paper next considers how interviews with Tuvaluan diplomats/officials focused on themes similar to those outlined by other Pacific diplomats but also revealed specific cultural mechanisms of diplomacy including the alofa, or ‘giving of gifts’. Subsequently, the article examines how Taiwanese diplomats/officials contemplate diplomacy, specifically how Taiwan's competition with China/the PRC has created negative conceptions of formal diplomacy. Taiwanese ideas of diplomacy often clash with Tuvaluan/Pacific conceptions because Taiwan sees diplomatic allies as always on the verge of severing ties and inherently disloyal. The conclusion discusses how cultural values and their influence on conceptions of diplomacy can engender tension in diplomatic relationships while also underscoring the importance of seeing diplomacy as both multifaceted and multiply understood. Newspapers and archives from Taiwan indicate a long history of Taiwan's commercial fishing in the Pacific that has largely been overlooked. Using these resources, this article analyses the relationship between the Taiwan government and Taiwan fishermen in the Pacific from the 1940s to the 1980s, demonstrating how non-state actors such as fishermen dictated Taiwan's official engagements in the region. I first outline the history of legal and illegal fishing by commercial Taiwan vessels in the Pacific from the 1940s to the 1980s. I then draw on two case studies: illegal fishing incidents in Tuvalu and Australia involving commercial Taiwan fishermen, and legal fishing operations in American Samoa. These case studies demonstrate how commercial fishermen impacted Taiwan-government engagements in the Pacific and were viewed by the government as a liability. The article concludes by connecting Taiwan's commercial fishing history in the Pacific with the larger history of Taiwan in the region. [ABSTRACT FROM AUTHOR]

Simple Summary: Mesothelioma of the tunica vaginalis testis (MTVT) is an exceedingly rare and aggressive cancer, mostly related to previous exposure to asbestos. Diagnosis is complex, and there is currently no standard treatment protocol. Radical surgery following accurate radiological staging is the mainstay of treatment. The effect of adjuvant therapies (including radiotherapy, chemotherapy, or the combination of the two) on disease progression is not clear yet. Given the rarity of MTVT, the purpose of this narrative review is to collect and analyze the reported cases evaluating diagnostic and therapeutic management. The aim of this work was to improve the knowledge of this disease and to suggest proper diagnostic and therapeutic strategies. Background: Mesothelioma of the tunica vaginalis testis (MTVT) is an extremely rare and aggressive cancer. The diagnosis and management of MTVT is complex, and no standard treatment protocol is available. Methods: We conducted a systematic literature review from 1 January 1982 to 14 March 2024 using PubMed to collect all the available case reports and case series. A descriptive analysis of patient characteristics with clinical presentation, diagnostic work-up, therapeutic management, and past asbestos exposure was performed. Survival times of patients treated with different therapeutic approaches were evaluated. Results: Overall, 289 patients with MTVT were included in our analysis. The most common clinical presentations were scrotal/testicular swelling or mass (187 patients, 65%) and the presence of hydrocele (159, 55%). Imaging evaluation, mostly with ultrasonography or CT scan, was reported in two-thirds of cases. Radical surgery (216 patients, 75%) with orchiectomy and, in select cases, hemiscrotectomy and inguinal lymphadenectomy was the most frequent therapeutic approach. A minority of patients (49, 17%) received adjuvant therapy after surgery (radiotherapy, chemotherapy, or a combination of the two), with no evidence of survival improvement. Conclusions: No standard guidelines for MTVT are available so far. Radical surgery following accurate radiological staging should be the mainstay of treatment. The role of adjuvant treatments remains undefined. Due to its rarity, MTVT should be treated in referral centers, and patients' data should be collected in a dedicated register in order to improve the knowledge of this exceedingly rare disease and establish optimal diagnostic and therapeutic management.
