Abstract

Purpose: Analysis of [18F]-Fluorodeoxyglucose (FDG) kinetics in cancer has been most often limited to the evaluation of the average uptake over relatively large volumes. Nevertheless, tumor lesion almost contains inflammatory infiltrates whose cells are characterized by a significant radioactivity washout due to the hydrolysis of FDG-6P catalyzed by glucose-6P phosphatase. The present study aimed to verify whether voxel-wise compartmental analysis of dynamic imaging can identify tumor regions characterized by tracer washout. Materials & Methods: The study included 11 patients with lung cancer submitted to PET/CT imaging for staging purposes. Tumor was defined by drawing a volume of interest loosely surrounding the lesion and considering all inside voxels with standardized uptake value (SUV) >40% of the maximum. After 20 minutes dynamic imaging centered on the heart, eight whole body scans were repeated. Six parametric maps were progressively generated by computing six regression lines that considered all eight frames, the last seven ones, and so on, up to the last three. Results: Progressively delaying the starting point of regression line computation identified a progressive increase in the prevalence of voxels with a negative slope. Conclusions: The voxel-wise parametric maps provided by compartmental analysis permits to identify a measurable volume characterized by radioactivity washout. The spatial localization of this pattern is compatible with the recognized preferential site of inflammatory infiltrates populating the tumor stroma and might improve the power of FDG imaging in monitoring the effectiveness of treatments aimed to empower the host immune response against the cancer.

Abstract

The radioactive glucose analogue 2-deoxy-2-[18F]fluoro-D-glucose (FDG) is widely used to reconstruct glucose metabolism and other biological functions in cells and tissues. The analysis of data on the time course of FDG tracer distribution is performed by the use of appropriate compartmental models. Motivated by recent results in cell biochemistry, we describe a new compartmental model aiming at the reconstruction of tracer kinetics in cells and tissues, which emphasizes the different roles of the cytosol and of the endoplasmic reticulum. Two applications of the new model are examined, that are concerned with real data from cancer cell cultures in vitro, and cancer tissues in vivo. The results are compared with those obtained through application of more standard compartmental models against the same datasets and appear to be in a better agreement with respect to recent biochemical experimental evidence. In particular, it is shown that tracer tends to accumulate in the endoplasmic reticulum, rather than cytosol, and that the rate of phosphorylation is higher than predicted by current models.

Abstract

OBJECTIVES Increased detection of prostate cancer (PCa) recurrences using [$^{68}$Ga]Ga-PSMA-11 PET/CT has been reported by adding forced diuresis or late-phase imaging to the standard protocol. However, the combination of these procedures in the clinical setting is still not standardized. METHODS One hundred prospectively recruited biochemical recurrent PCa patients were restaged with dual-phase [$^{68}$Ga]Ga-PSMA-11 PET/CT from September 2020 to October 2021. All patients received a standard scan (60 min), followed by diuretics (140 min) and a late-phase abdominopelvic scan (180 min). PET readers with low (n = 2), intermediate (n = 2), or high (n = 2) experience rated (i) standard and (ii) standard + forced diuresis late-phase images in a stepwise fashion according to E-PSMA guidelines, scoring their level of confidence. Study endpoints were (i) accuracy against a composite reference standard, (ii) reader's confidence level, and (iii) interobserver agreement. RESULTS Forced diuresis late-phase imaging increased the reader's confidence category for local and nodal restaging (both p < 0.0001), and the interobserver agreement in identifying nodal recurrences (from moderate to substantial, p < 0.01). However, it significantly increased diagnostic accuracy exclusively for local uptakes rated by low-experienced readers (from 76.5 to 84%, p = 0.05) and for nodal uptakes rated as uncertain at standard imaging (from 68.1 to 78.5%, p < 0.05). In this framework, SUVmax kinetics resulted in an independent predictor of PCa recurrence compared to standard metrics, potentially guiding the dual-phase PET/CT interpretation. CONCLUSIONS The present results do not support the systematic combination of forced diuresis and late-phase imaging in the clinical setting, but allow the identification of patients-, lesions-, and reader-based scenarios that might benefit from it. Abstract

Aims: Embedded system plays a vital role in today’s life. Hence, our interest is in areadelay- energy efficient embedded system design in post-CMOS technology, i.e., QCA. Objectives: The research is focused on efficient area-delay-energy Configurable Logic Block (CLB) design for Field-Programmable Gate Array architecture (FPGA) with successful simulation-based on next-generation technology, Quantum-dot cellular automata. Methods: Each proposed circuit is designed on post CMOS 4 dot 2 electron technology, i.e. QCA (Quantum dot Cellular Automata), is adopted in circuit implementation due to low power dissipation, high clock frequency and high package density. QCADesigner is used to verify the functionality of every circuit. QCAPro tool is used for determining power dissipation. Results: In contrast, a new approach of using de-multiplexer replacing the decoder has been introduced that results in the reduction of the average energy dissipation by almost 57%. A NOR based D flip-flop memory architecture and multiplexer are also used in the lookup table for the configurable logic block. The proposed architecture thus reduces the overall latency. The proposed CLB consists of 6356 QCA cells covering 7.44 um2 area. Write and read latency of proposed CLB are 12 and 7.25 QCA clock, respectively. Conclusion: The present paper concludes that read and write latency reduction occurs; average energy dissipation, leakage, and switching energy dissipation are reduced in a large amount resulting in an advantage of the overall minimization of the latency for the proposed CLB in the process.

Abstract

Objectives: Increased detection of prostate cancer (PCa) recurrences using [68Ga]Ga-PSMA-11 PET/CT has been reported by adding forced diuresis or late-phase imaging to the standard protocol. However, the combination of these procedures in the clinical setting is still not standardized. Methods: One hundred prospectively recruited biochemical recurrent PCa patients were restaged with dual-phase [68Ga]Ga-PSMA-11 PET/CT from September 2020 to October 2021. All patients received a standard scan (60 min), followed by diuretics (140 min) and a late-phase abdominopelvic scan (180 min). PET readers with low (n = 2), intermediate (n = 2), or high (n = 2) experience rated (i) standard and (ii) standard + forced diuresis late-phase images in a stepwise fashion according to E-PSMA guidelines, scoring their level of confidence. Study endpoints were (i) accuracy against a composite reference standard, (ii) reader's confidence level, and (iii) interobserver agreement. Results: Forced diuresis late-phase imaging increased the reader's confidence category for local and nodal restaging (both p < 0.0001), and the interobserver agreement in identifying nodal recurrences (from moderate to substantial, p < 0.01). However, it significantly increased diagnostic accuracy exclusively for local uptakes rated by low-experienced readers (from 76.5 to 84%, p = 0.05) and for nodal uptakes rated as uncertain at standard imaging (from 68.1 to 78.5%, p < 0.05). In this framework, SUVmax kinetics resulted in an independent predictor of PCa recurrence compared to standard metrics, potentially guiding the dual-phase PET/CT interpretation. Conclusions: The present results do not support the systematic combination of forced diuresis and late-phase imaging in the clinical setting, but allow the identification of patients-, lesions-, and reader-based scenarios that might benefit from it. Key Points: • Increased detection of prostate cancer recurrences has been reported by adding diuretics administration or an additional late abdominopelvic scan to the standard [68Ga]Ga-PSMA-11 PET/CT procedure. • We verified the added value of combined forced diuresis and delayed imaging, showing that this protocol only slightly increases the diagnostic accuracy of [68Ga]Ga-PSMA-11 PET/CT, thus not justifying its systematic use in clinics. • However, it can be helpful in specific clinical scenarios, e.g., when PET/CT is reported by low-experienced readers.
