With the increasing rate of infections caused by multidrug‐resistant organisms (MDRO), selecting appropriate empiric antibiotics has become challenging. We aimed to develop and externally validate a model for predicting the risk of MDRO infections in patients with cirrhosis. Methods: We included patients with cirrhosis and bacterial infections from two prospective studies: a transcontinental study was used for model development and internal validation (n = 1302), and a study from Argentina and Uruguay was used for external validation (n = 472). All predictors were measured at the time of infection. Both culture‐positive and culture‐negative infections were included. The model was developed using logistic regression with backward stepwise predictor selection. We externally validated the optimism‐adjusted model using calibration and discrimination statistics and evaluated its clinical utility. Results: The prevalence of MDRO infections was 19% and 22% in the development and external validation datasets, respectively. The model's predictors were sex, prior antibiotic use, type and site of infection, MELD‐Na, use of vasopressors, acute‐on‐chronic liver failure, and interaction terms. Upon external validation, the calibration slope was 77 (95% CI.48–1.05), and the area under the ROC curve was.68 (95% CI.61–.73). The application of the model significantly changed the post‐test probability of having an MDRO infection, identifying patients with nosocomial infection at very low risk (8%) and patients with community‐acquired infections at significant risk (36%). Conclusion: This model achieved adequate performance and could be used to improve the selection of empiric antibiotics, aligning with other antibiotic stewardship program strategies. El hígado graso no alcohólico (HGNA) es la enfermedad hepática crónica más frecuente en todo el mundo, con una prevalencia aproximada de 25% a nivel global. Su prevalencia es mucho mayor en pacientes con sobrepeso, obesidad y diabetes tipo 2 y es considerada como la manifestación hepática del síndrome metabólico. El espectro de la enfermedad hepática es muy amplio, desde la esteatosis simple a la esteatohepatitis, fibrosis, cirrosis y sus complicaciones, como el hepatocarcinoma. La mayoría de los pacientes afectados no progresará a la fibrosis avanzada/cirrosis. A pesar de esto, se ha descripto que la hepatopatía es la tercera causa de muerte entre los pacientes con HGNA, luego de las enfermedades cardiovasculares y las malignas. Entre la enorme cantidad de afectados, lo más importante es identificar a los que están en riesgo de evolución a la cirrosis o sus complicaciones y conocer las opciones de diagnóstico y tratamiento. En esta Guía organizada por la Asociación Argentina para el Estudio de las Enfermedades del Hígado se revisan las definiciones, los aspectos epidemiológicos, la historia natural y un enfoque práctico sobre algoritmos posibles para estimar la gravedad de la hepatopatía en cada caso, además de analizar los avances en el tratamiento y recomendaciones para el seguimiento. Es importante señalar que no se han publicado datos sobre incidencia o prevalencia de la enfermedad en población general de Argentina, y se alienta a la realización de los mismos. . Nonalcoholic fatty liver disease (NAFLD) is the most frequent chronic liver disease worldwide, with an estimated global prevalence of approximately 25%, that is much higher in patients with overweight, obesity and type 2 diabetes. NAFLD is considered as the hepatic manifestation of metabolic syndrome. It has a wide spectrum, from simple steatosis to steatohepatitis, fibrosis, cirrhosis and its complications, such as hepatocellular carcinoma. Most of the affected patients will not evolve to advanced fibrosis or cirrhosis. Despite this, it has been described that the hepatic disease is the third cause of death among patients with nonalcoholic fatty liver, after cardiovascular and malignant diseases. Among the huge number of patients affected, the main challenge is to identify those who are at risk of developing cirrhosis or its complications and to recognize the diagnostic and treatment options. In this Guideline, endorsed by the Argentine Association for the Study of Liver Diseases, the definitions, epidemiological aspects, natural history and a practical approach to possible algorithms to estimate the severity of liver disease in the individual patient are reviewed; in addition to analyzing advances in treatment and proposing recommendations for follow-up. - Abstract
En base a observaciones meteorológicas de superficie y altura se ha encarado en esta Tesis el estudio de la variabilidad de la precipitación en Argentina en diferentes escalas temporales, relacionada con actividad convectiva observada. Uno de los objetivos de esta Tesis fue documentar la metodología de control de consistencia de los datos meteorológicos. Para ello, se hizo una discusión de los datos disponibles, de la problemática de los errores encontrados y la forma en que se trató de minimizar su incidencia. La metodología empleada permitió mejorar la consistencia de los datos, temporal y espacialmente, el correcto cálculo de los parámetros y la verificación de los valores extremos. Se ha ensayado una climatología mensual y anual de situaciones sinópticas en la Argentina para el período 1998-2005. La coincidencia de resultados de esta climatología sinóptica con otros autores, en trabajos anteriores, permite consistir la base de datos de situaciones, a pesar de que la designación de situaciones sinópticas y tiempos significativos no se hizo con un método objetivo. Se analizó la variabilidad interanual de las tormentas desde los datos observados, en puntos distribuidos en la parte continental de Argentina. En todas las estaciones analizadas puntualmente, la variabilidad interanual del número de tormentas mostró una tendencia positiva, en el período 1968-2006, sin embargo durante los años 2004 a 2006 se observó una disminución de tormentas en todas ellas. Se analizaron los periodogramas correspondientes al análisis armónico del número de tormentas anuales. La poca varianza explicada permite decir que no hay procesos fundamentales que definan la variabilidad interanual del número de tormentas, sino que esta es la resultante de un conjunto de procesos físicos que le dan un carácter de aleatoridad a este comportamiento. Se estudiaron los índices de inestabilidad asociados a eventos extremos de precipitación para aquellos puntos con mediciones de radiosondeos. Existen pocos casos con umbrales extremos de los índices en situación de tormentas severas en el horario del radiosondeo, 12 UTC. No obstante cuando se dan estos valores hay una alta probabilidad de que se produzcan tormentas, conformando un indicio importante en el momento del pronóstico. Se realizó un estudio de la variabilidad interanual de las precipitaciones, en los parámetros cantidad, intensidad y número de días con precipitación, en diferentes zonas de la Argentina. Se estudió, además, la tendencia interanual de 10 índices de precipitación en la región centro y noreste de la Argentina, con énfasis en los eventos extremos, en el período 1959-2002. Los estadísticos estudiados en el área al este de 65° W y norte de 37° S de nuestro país mostraron cambios que son consistentes con clima más húmedo. Se estudió la intensidad de precipitación en 60 minutos para Buenos Aires. Estos estudios llevan a pensar que la atmósfera, en la última década, fue capaz de producir eventos aislados de una intensidad no observada con anterioridad. También se hizo un estudio de las preferencias horarias de la precipitación, para diferentes umbrales de cantidad diaria, en la ciudad de Buenos Aires. A medida que se aumentó el umbral la diferencia de frecuencia horaria se hizo más marcada y las mayores frecuencias se produjeron hacia la madrugada. Se observó que en invierno existen dos máximos que se hacen más notorios cuanto mayor es el umbral de precipitación. Análisis similares se hicieron para otras estaciones. En base a las salidas de un modelo regional ETA, de pronóstico de malla fina y de las observaciones de precipitación medida, se examinó la verificación del desempeño del modelo para la representación de la precipitación. Se estudió la verificación de la precipitación en pronósticos a 24, 48 y 72 horas, en 9 puntos de observación distribuidos en el territorio argentino. Se estudiaron las diferencias entre el valor dado por el modelo y la observación, así como el porcentaje de aciertos de casos con y sin precipitación. El modelo pronosticó mejor los días sin precipitación que con precipitación. Los errores reiterados, es decir que se mantuvieron durante 3 días, oscilaron entre el 8% y 24% de los casos analizados. Cuando se produjeron errores reiterados de sorpresas el rango de la precipitación se encontró en su mayoría entre 4 y 12 mm. Los aciertos del modelo, para las estaciones estudiadas, estuvieron por encima del 76% para 24 horas de pronóstico, por encima de 71% para 48 horas y mayores a 65% a 72 horas. This Thesis studies the precipitation variability in Argentina, in different time scales, related with convective observed activity, based on surface and height meteorological observations. Among the objectives of this Thesis it has figured to document the consistence control methodology of the meteorological data. For this purpose, it was made a discussion of available data, the problem of errors and the way to minimize their incidence. The methodology used allowed to improve the data consistence, spatial and temporary, the correct calculation of the parameters and the verification of the extreme values. A monthly and annual Argentinean climatology of synoptic situations has been rehearsed, for period 1998-2005. The coincidence of results between this synoptic climatology and other authors, in previous works, allows consist the meteorological situations database, although the appointment of synoptic situations and significant weather were not made with an objective method. The inter-annual storm variability from observed data, distributed in continental Argentina points, was analyzed. All the stations showed a positive tendency in inter-annual variability of the storms number, in period 1968-2006, however during the years 2004 at 2006 a decrease of storms is observed in all of them. The periodograms corresponding to harmonic analysis of annual number of precipitations days were analyzed. The little explained variance allows say that there are not fundamental processes that define the inter-annual variability of storms number, but rather this is the resultant of a group of physical processes that give a random character to this behavior. The instability indexes associated to extreme precipitation events were studied for those points with radiosondes measures. Few cases exist for extreme thresholds indexes, corresponding to severe storms situations, at 12UTC, in time of radiosonde, nevertheless when they are given there is a high probability that storms take place, conforming an important indication at forecast moment. It was carried out a study of inter-annual precipitation parameters variability, quantity, intensity and number of days with precipitation, for different Argentinean zones. It was also studied the inter-annual tendency of 10 precipitation indexes in center and northeast region of Argentina, with emphasis in extreme events, in 1959-2002 period. The statistical studied in the area at east of 65° W and north of 37° S, show changes in precipitation indexes that are consistent with more humid climate. The precipitation intensity for 60 minutes in Buenos Aires was studied. These studies carry to think that the atmosphere, in last decade, is able to produce isolated events of an intensity not observed previously. A study of preferences of precipitation horary, for Buenos Aires city, for different precipitation thresholds of daily quantity, it was made too. As is increased the threshold, the difference of horary frequency becomes more notable and the biggest frequencies take place toward the dawn. In winter, are observed two maximum, they become more notorious when the precipitation threshold is greater. Similar analysis was done for other stations. On the other hand, based on a fine mesh regional forecast ETA model outputs, and measured precipitation, the verification of model acting was examined for precipitation representation. The precipitation verification was studied for 24, 48 and 72 hours of forecast, in 9 observation points, distributed in Argentinean territory. It was studied the difference between the model and the observations, as well as the percentage of successes for cases with and without precipitation. The forecast model was better the days without precipitation than those with precipitation. The days with reiterated errors, that is the error stayed during 3 days, oscillated between 8% and 24% of analyzed cases. When reiterated errors of surprises took place, the precipitation range was majority between 4 and 12 mm. The model successes, for the studied stations, were above 76% for 24 hours of forecast, above 71% for 48 hours and bigger to 65% for 72 hours. Fil: Marino, Mónica Beatriz.
