- Abstract
Primary immunodeficiencies (PIDs) are heterogeneous disorders, characterized by variable clinical and immunological features. National PID registries offer useful insights on the epidemiology, diagnosis, and natural history of these disorders. In 1999, the Italian network for primary immunodeficiencies (IPINet) was established. We report on data collected from the IPINet registry after 20 years of activity. A total of 3352 pediatric and adult patients affected with PIDs are registered in the database. In Italy, a regional distribution trend of PID diagnosis was observed. Based on the updated IUIS classification of 2019, PID distribution in Italy showed that predominantly antibody deficiencies account for the majority of cases (63%), followed by combined immunodeficiencies with associated or syndromic features (22.5%). The overall age at diagnosis was younger for male patients. The minimal prevalence of PIDs in Italy resulted in 5.1 per 100.000 habitants. Mortality was similar to other European registries (4.2%). Immunoglobulin replacement treatment was prescribed to less than one third of the patient cohort. Collectively, this is the first comprehensive description of the PID epidemiology in Italy.

- Abstract
Background: Rituximab (RTX; anti-CD20 mAb) is a treatment option in children with refractory immune thrombocytopenia, autoimmune hemolytic anemia (AHA), and Evans syndrome (ES). Prevalence and clinical course of RTX-induced hypogammaglobulinemia in these patients are poorly known. Objective: To evaluate the prevalence and risk factors for persistent hypogammaglobulinemia (PH) after RTX use. Methods: Clinical and immunologic data from children treated with RTX for immune thrombocytopenia, AHA, and ES were collected from 16 Italian centers and 1 UK center at pre-RTX time point (0), +6 months, and yearly, up to 4 years post-RTX. Patients with previously diagnosed malignancy or primary immune deficiency (PID) were excluded. Results: We analyzed 53 children treated with RTX for immune thrombocytopenia (n = 36), AHA (n = 13), and ES (n = 4). Median follow-up was 30 months (range, 12-48). Thirty-two percent of patients (17 of 53) experienced PH, defined as IgG levels less than 2 SD for age at last follow-up (>12 months after RTX). Significantly delayed B-cell recovery was observed in children experiencing PH (hazard ratio, 0.55; P <.05), and 6 of 17 (35%) patients had unresolved B-cell lymphopenia at last follow-up. PH was associated with IgA and IgM deficiency, younger age at RTX use (51 vs 116 months; P <.01), a diagnosis of AHA/ES, and better response to RTX. Nine patients with PH (9 of 17 [53%]) were eventually diagnosed with a PID. Conclusions: Post-RTX PH is a frequent condition in children with autoimmune cytopenia; a sizable proportion of patients with post-RTX PH were eventually diagnosed with a PID. In-depth investigation for PID is therefore recommended in these patients.

FPGA System-on-Chips (SoCs) are heterogeneous platforms that combine general-purpose processors with a field-programmable gate array (FPGA) fabric. The FPGA fabric is composed of a programmable logic in which hardware accelerators can be deployed to accelerate the execution of specific functionality. The main source of unpredictability when bounding the execution times of hardware accelerators pertains the access to the shared memories via the on-chip bus. This work is focused on bounding the worst-case bus contention experienced by the hardware accelerators deployed in the FPGA fabric. To this end, this work considers the AMBA AXI bus, which is the de-facto standard communication interface used in most the commercial off-the-shelf (COTS) FPGA SoCs, and presents an analysis technique to bound the response times of hardware accelerators implemented on such platforms. A fine-grained modeling of the AXI bus and AXI interconnects is first provided. Then, contention delays are studied under hierarchical bus infrastructures with arbitrary depths. Experimental results are finally presented to validate the proposed model with execution traces on two modern FPGA-based SoC produced by Xilinx (Zynq-7000 and Zynq-Ultrascale+ families) and to assess the performance of the proposed analysis.

The continuous emergence of environmental crisis is analysed by several studies confirming the undeniable human’s responsibility in the planetary changes. Humans seems to strongly require education on their relationship with the natural environment. Currently, immigrants are 8.2% of the entire Italian population: exploring their perspective on what they deem necessary to improve the relationship with the natural world, through an unprecedented quali-quanty methods research, led to some interesting results. Highlighting the fundamental importance ascribed to the relationship of closeness with nature for people’s physical, mental and spiritual well-being, the majority of respondents confirms the essential function education plays to know, love and respect nature. The role of school is considered as elemental for education and awareness towards nature and animals. Schools in accounted as an ideal place for proposing experiences of direct relationship with them to the children, who have to be accompanied from an early age to get close to nature, especially those living in highly urbanized environments. Teaching eco-sustainable behaviours is also required, such as recycling and preserving. Families well play a basic role for giving good example on respect towards nature and animals. Education for adults also results as extremely necessary

- Abstract
The diagnosis of cow’s milk allergy (CMA) is particularly challenging in infants, especially with non-Immunoglobulin E (IgE)-mediated manifestations, and inaccurate diagnosis may lead to unnecessary dietary restrictions. The aim of this study was to assess the accuracy of the cow’s milk-related symptom score (CoMiSSTM) in response to a cow’s milk-free diet (CMFD). We prospectively recruited 47 infants (median age three months) who had been placed on a CMFD due to persisting unexplained gastrointestinal symptoms. We compared data with 94 healthy controls (median age three months). The CoMiSSTM score was completed at recruitment and while on the exclusion diet. In 19/47 (40%) cases a response to the diet occurred. At recruitment CoMiSSTM was significantly higher in cases compared to controls (median score 8 vs. 3; p-value: <0.05), 9 cases had a score ≥12 and 8/9 normalized on CMFD. An oral milk challenge was performed in all 19 responders and six of these had a positive reaction to cow’s milk (CM). In eight infants IgE allergy tests were positive. The receiver operation characteristic (ROC) curve identified a CoMISSTM score of 9 to be the best cut-off value (84% sensitivity, 85% specificity, 80% positive (PPV) and 88% negative predictive value (NPV)) for the response to CMFD. We found CoMiSSTM to be a useful tool to help identify infants with persisting gastrointestinal symptoms and suspected CMA that would benefit from CMFD.

- Abstract
Chronic inflammatory demyelinating polyneuropathy (CIDP) is a chronically progressive or relapsing sensorimotor disorder presumably due to antibody-mediated reactions. It is a rare condition in children, with estimated prevalence as 0.48 per 100,000 among patients younger than 20 years of age. Recommended treatments include immune modulators, intravenous immunoglobulins (IVIgs), steroids, and plasmapheresis. Management of pediatric CIDP is challenging because of the lack of evidence-based efficacy of the current therapies in children. Because of the rarity of this condition, there are no double-blind randomized studies to support the therapeutic choice as well as to identify the optimal first-line therapeutic regimen. IVIgs are widely used but the intravenous administration is usually uncomfortable, especially for children. Subcutaneous immunoglobulins (SCIgs) have proven to be effective in adults with CIDP and in children affected by antibody deficiencies and other different immune and inflammatory disorders. Herein, we described the case of a 7-year-old boy, affected by CIDP who clinically responded to IVIg but was dependent on this therapy. In order to improve his quality of life, we switched to SCIg with excellent result.

- Abstract
The continuous emergence of environmental crisis is analysed by several studies confirming the undeniable human’s responsibility in the planetary changes. Humans seems to strongly require education on their relationship with the natural environment. Currently, immigrants are 8.2% of the entire Italian population: exploring their perspective on what they deem necessary to improve the relationship with the natural world, through an unprecedented quali-quanty methods research, led to some interesting results. Highlighting the fundamental importance ascribed to the relationship of closeness with nature for people’s physical, mental and spiritual well-being, the majority of respondents confirms the essential function education plays to know, love and respect nature. The role of school is considered as elemental for education and awareness towards nature and animals. Schools in accounted as an ideal place for proposing experiences of direct relationship with them to the children, who have to be accompanied from an early age to get close to nature, especially those living in highly urbanized environments. Teaching eco-sustainable behaviours is also required, such as recycling and preserving. Families well play a basic role for giving good example on respect towards nature and animals. Education for adults also results as extremely necessary

A 5-years multicenter prospective study on 201 patients with common variable immunodeficiencies and 101 patients with X-linked agammaglobulinemia over a cumulative follow-up period of 1,365 patient-years was conducted to identify prognostic markers and risk factors for associated clinical co-morbidities, the effects of long-term immunoglobulin treatment and the IgG trough level to be maintained over time required to minimise infection risk. Overall, 21% of the patients with common variable immunodeficiencies and 24% of patients with X-linked agammaglobulinemia remained infection free during the study. A reduction of pneumonia episodes has been observed after initiation of Ig replacement. During the observation time, pneumonia incidence remained low and constant over time. Patients with pneumonia did not have significant lower IgG trough levels than patients without pneumonia, with the exception of patients whose IgG trough levels were persistently < 400 mg/dL. In X-linked agammaglobulinemia, the only co-morbidity risk factor identified for pneumonia by the final multivariable model was the presence of bronchiectasis. In common variable immunodeficiencies, our data allowed us to identify a clinical phenotype characterised by a high pneumonia risk: patients with low IgG and IgA levels at diagnosis; patients who had IgA level < 7 mg/dL and who had bronchiectasis. The effect of therapy with immunoglobulins at replacement dosage for non-infectious co-morbidities (autoimmunity, lymphocytic hyperplasia and enteropathy) remains to be established. A unique general protective trough IgG level in antibody deficiency patients will remain undefined because of the major role played by the progression of lung disease in X-linked agammaglobulinemia and in a subset of patients with common variable immunodeficiencies.

- Abstract
Prendendo le mosse dall’evoluzione semantica del termine «letteratura», questo articolo sostiene che l’idea di letteratura modernamente intesa, ben distinta dalle humanae litterae e non documentata prima del XVIII secolo – cioè la letteratura concepita come insieme di testi nati con specifiche finalità estetiche – nasce all’interno di una dimensione squisitamente riflessiva. Se la letteratura di secondo grado è sempre esistita, e se è verosimile che non esistano forme di intertestualità (o transtestualità) ignote alla cultura classica, è però vero che nel Settecento l’attività dello scrittore è investita come mai prima della necessità di «situarsi». Ora la creazione letteraria deve definire la propria posizione rispetto alla tradizione, alla cultura popolare, alle altre forme espressive. In questa luce, la nascita della «letteratura» è inscindibile dallo sviluppo del discorso meta-letterario: i frammenti di Friedrich Schlegel sulla poesia moderna lo testimoniano con un acume che non ha perso nulla della sua esemplarità.

- Abstract
La riflessione pedagogica, nell’indagare le possibilità educative lungo tutto l’arco della vita, richiama alla necessità di analizzare in modo multidisciplinare le questioni attuali per addivenire a progettualità formative di sviluppo efficaci e orientate alla “ecologia integrale”. La rilevanza etico-educativa della nozione di sostenibilità si pone come luogo ermeneutico per provocare la consapevolezza critica della pedagogia. La progettazione pedagogica si confronta con diversi saperi in uno scenario, mutevole come quello attuale, in cui le città rappresentano una prospettiva emblematica di formazione e di confronto nel rispetto dei diritti umani e della custodia del creato. Tra i modelli talora contrapposti di riforma dell’istruzione e divergenti politiche per l’ambiente e per il lavoro, la riflessione sull’educazione è impegnata a prefigurare la trasformazione della società nel tempo futuro e a favorire nelle coscienze individuali quella sensibilità ecologica capace di muovere comportamenti sostenibili e responsabili. Aziende, imprese sociali e nuove professionalità rappresentano ambiti emblematici per dare vita a Human Grids alleanze sul piano della progettazione educativa e di sviluppo. Promuovere un’economia civile implica educare in modo intenzionale a comportamenti non-strumentali ma espressivi di virtù civili, per un futuro contrassegnato da una “crescita umana” durevole, connotata da “cura, attenzione, riguardo e rispetto per l’ambiente.

Objective To investigate the clinical manifestations at diagnosis and during follow-up in patients with 22q11.2 deletion syndrome to better define the natural history of the disease. Study design A retrospective and prospective multicenter study was conducted with 228 patients in the context of the Italian Network for Primary Immunodeficiencies. Clinical diagnosis was confirmed by cytogenetic or molecular analysis. Results The cohort consisted of 112 males and 116 females; median age at diagnosis was 4 months (range 0 to 36 years 10 months). The diagnosis was made before 2 years of age in 71% of patients, predominantly related to the presence of heart anomalies and neonatal hypocalcemia. In patients diagnosed after 2 years of age, clinical features such as speech and language impairment, developmental delay, minor cardiac defects, recurrent infections, and facial features were the main elements leading to diagnosis. During follow-up (available for 172 patients), the frequency of autoimmune manifestations ( P = .015) and speech disorders ( P = .002) increased. After a median follow-up of 43 months, the survival probability was 0.92 at 15 years from diagnosis. Conclusions Our data show a delay in the diagnosis of 22q11.2 deletion syndrome with noncardiac symptoms. This study provides guidelines for pediatricians and specialists for early identification of cases that can be confirmed by genetic testing, which would permit the provision of appropriate clinical management.

Obesity is now considered to be a global epidemic, impacting a great number of women and leading to a higher risk of obstetrical and gestational complications. One of such possible adverse outcomes in gestating female is placental hypoxia, which has been related to vascular remodeling and hypertension, as well as adaptive phenomena to reduce levels of oxidative stress and damage. A pool of female Sprague Dawley rats (n=63) was first assigned into two dietary groups (Control and High Sugar). Following mating, the pregnant rats (n=39) were again distributed into two oxygen treatment groups (Normoxia and Hypoxia) for 3 weeks, and tissue sampling and biochemical analyses were carried out. The main results of this study are the following: 1) Hypoxia during gestation may lead to a reduction in the average number of pups per mother, 2) Hypoxia during gestation treatment may lead to a decrease in maternal serum TG levels, and consequentially 3) Hypoxia during gestation may lead to a reduction in TyG Index levels. These results suggest that hypoxia could generate a beneficial response in pregnant Sprague Dawley rats to salvage both maternal and fetal viability. Thus, reproducing mild hypoxic conditions could result being a viable therapeutic option in preventing gestational adversities. In conclusion, progress was made in recognizing the possible role of a mild hypoxic environment in stimulating a maternal protective response.

The lack of prospective, randomized, controlled trial data to guide optimal antibiotic use in bacteraemic critically ill patients has led to a wide variety of strategies and major issues with drug resistance. We therefore prospectively investigated the epidemiology of bacteraemia and fungaemia in intensive care units (ICUs); and the impact of timing, type and appropriateness of antibiotic intervention.We conducted a multinational, multicentre, prospective observational study in 132 ICUs from 26 countries with no interventions.1702 patients [European (69.6%), Australasian (12.2%), South American (8.3%) and Asian (9.9%)] developed 1942 bacteraemic episodes over the study period. Mortality rates were similar for those receiving empirical (40.5%), semi-targeted (37.6%) or fully targeted (33.3%) antibiotic therapy (P=0.40), and in those initially receiving broad- (39.3%) or restricted-spectrum (39.1%) therapy (P=0.94). First-line therapy was effective in terms of the antibiogram (where available) in 70.4% of cases. This in vitro susceptibility ranged from 76.3% for broad-spectrum antibiotics to 46.3% for restricted-spectrum antibiotics (P0.0001). However, no antibiotic policy-associated variable, including in vitro susceptibility (odds ratio 0.89, 95% confidence interval 0.61-1.30), was a statistically significant predictor of mortality.We could not show an impact of antibiotics on mortality in critically ill patients, despite in vitro activity and early commencement. Randomized, multicentre trials are urgently needed to establish the appropriate duration, timing and combinations of antibiotics that will both optimally treat infection and minimize development of resistance and other complications.

- Abstract
The continuous emergence of environmental crisis is analyzed by severeal studies confirming the undeniable human's responsibility in the planetary changes. Humans seem to strongly require education on their relationship with the natural environment. Currently, immigrants are 8,2% of the entire Italian population: exploring their perspective on what they deem necessary to improve the relationship with the natural world, through an unprecedented qualitative methods research, led to some interesting results. Highlighting the fundamental importance ascribed to the relationship of closeness with the Nature for people's physical, mental and spiritual well-being, the majority of respondents confirms the essential function that education plays to know, love and respect Nature.The role of school is considered elemental for education and awareness towards Nature and animals. School is accounted as an ideal place for proposing experiences of direct relationship with them to the children, who have to be accompained from an early age to get close to Nature, especially those living in highly urbanized environments. Teaching of eco-sustainable behaviours is also required, such as recycling and preserving. Families as well play a basic role for giving good example on respect towards Nature and animals. Education for adults also results as extremely necessary

- Abstract
Le condizioni in cui versa il nostro pianeta sono sempre più gravi: sembra emergere in modo piuttosto evidente la responsabilità umana in relazione ai cambiamenti climatici e ambientali a livello planetario. Tale responsabilità assume almeno due aspetti principali: da un lato, rispetto a quelle visioni del mondo, della natura e dell’ambiente che hanno orientato le scelte, le decisioni e i comportamenti umani nei confronti dell’ambiente naturale; dall’altro, rispetto alla responsabilità, alla quale il genere umano è chiamato, di operare in maniera tale da interrompere il ciclo di sfruttamento e distruzione, compiendo una vera e propria ‘inversione di rotta’. La progressiva comparsa di nuove e sempre più forti crisi ambientali richiama alla necessità di focalizzarsi sulla relazione tra esseri umani e natura e sui valori che sono alla base di tale relazione. Dalla fine degli anni ’60 del secolo scorso quando iniziò a manifestarsi la crisi ambientale, si può notare come la filosofia abbia rivolto la sua attenzione, in maniera sempre maggiore, alla relazione tra esseri umani e natura, cercando di individuare le cause dei problemi ecologici. La riflessione filosofica su tale relazione si colloca all’interno della più ampia riflessione dell’etica ambientale, che nel corso della storia del pensiero umano ha esteso sempre di più il proprio campo d’interesse, attraverso la metafora del “cerchio morale di espansione”, ampliandolo sia dal punto di vista temporale, includendo le generazioni future, sia dal punto di vista spaziale, includendo dapprima il mondo degli animali non-umani e, in seguito, anche l’ambiente e i sistemi viventi. Diversi studi internazionali in questo campo sono stati condotti, con differenti gruppi etnici, a livello nazionale e cross-national. Non sembrano, tuttavia, essere presenti degli studi che abbiano esplorato i punti di vista sulla natura e l’ambiente di soggetti provenienti da diversi gruppi etnici immigrati in un singolo stato, utilizza, The Planet Earth has entered into the Anthropocene Era, scientist J.P.Crutzen said. It’s the Era of human beings, distinguished from the previous ones by the impact that humans are having on the climate and planetary environment. It’s rather clear that human responsibility is strongly implicated in the planetary changes. This responsibility takes at least two main aspects. On the one hand, with respect to those visions of the world, the nature and the environment that have guided the choices, the decisions and the human behavior towards nature and the environment. On the other, regarding the responsibility to which the mankind is called, in working to stop the cycle of exploitation and destruction, and make a real 'change of direction'. The emergence of new and increasingly strong environmental crisis emphasizes the need to focus on the relationships between humans and nature and the values underpinning them. Since the '70s, with the emergence of the environmental crisis, philosophy has increasingly turned its attention towards the relationship ‘humans-nature’. Philosophy reflection is inscribed within the Environmental Ethics perspective that has gradually extended its field of interest, through the metaphor of the "moral circle expansion", widening temporally, including future generations, and spatially, including the world's non-human animals and the environment, understood as ecosphere and biosphere. Several international studies in this field were conducted with different ethnic groups at a national and at cross-national level. No studies were found aiming to investigate immigrants’ world views on nature and environment conducted through a qualitative and/or mixed method approach with key respondents belonging to different ethnic groups, immigrated into a single state, without a comparison among cultures. Introduction: The health-related quality of life in X-linked agammaglobulinemia was investigated in 25 children and adolescents patients through the Italian version of Pediatric Quality of Life Inventory 4.0 Generic Core Scale for patients aged less then 18 years, comparing child perception to that of the parents and the physician's evaluation. The data were compared with the ones of 80 healthy controls and the literature data of a group of patients with rheumatic diseases. Discussion: The agammaglobulinemia subjects perceived a lower global quality of life than the healthy subjects, but significantly higher than the rheumatic diseases controls. The clinical relevance of health-related quality of life assessment in X-linked agammaglobulinemia pediatric patients is discussed. © 2008 Springer Science+Business Media, LLC.

Introduction The human responsibility in the planetary changes is undeniable. The emergence of new and increasingly strong environmental crisis emphasizes the need to focus on the relationships between humans and nature and the values underpinning them. Many international studies in this field were made with different ethnic groups, but there are no studies investigating whether and if the environmental values of the original cultures change once people live in a new cultural context. Objectives Inside this perspective, grounded on the relevance of humans’ values and world vision guiding their behaviours, this research aims to investigate people’s values and some aspects of humans’ cultures related to environmental and sustainability issues. Methods The attention is focused on people from different ethnic groups immigrated to Italy. With the above aims in mind, the sample was selected, gathering people from Immigrants Associations in two areas: Milano, Parma and their provinces, where these associations are widespread and usually promote integration through their activities and initiatives. Foreign adults, from different ethnic groups, immigrated to Italy as first generation will be selected, do not forgetting that the study aim is not to obtain quantitative results on a large scale but it is an in depth analysis of individual thinking. In consideration of this framework, a mixed approach, composed by a quantitative tool and a structured interview with multiple choice answers on Lickert scale, along with a qualitative narrative interview, promoting the narration of experiences referred to the respondents’ original culture, was elaborated. The tool is based on previous quantitative studies, mainly conducted among psychological, sociological, economic disciplines, based on scales aiming to investigate values, awareness, knowledge and declared pro-environmental behaviours.
