Para el desarrollo sostenible es necesario impulsar acciones inherentes a la economía circular (EC), como el manejo y aprovechamiento de los residuos sólidos urbanos (RSU), incluyendo su fracción orgánica (FORSU) para la obtención de productos de interés comercial. Sin embargo, los criterios y métricas para evaluar la circularidad de los procesos presentan una variedad de formas y referencias y en algunos casos carecen de un marco metodológico claro que permita aplicarlos de forma concreta en los procesos de la bioeconomía circular (BEC), es decir, aquellos que se centran en la gestión de materiales de origen biológico a través de bioprocesos. En este trabajo se propuso revisar críticamente los indicadores publicados para evaluar los procesos de EC, con la finalidad de proponer los más adecuados para su aplicación en sistemas de bioeconomía circular, particularmente en aquellos que involucran el tratamiento de la FORSU. Estos indicadores pueden ser cuantitativos o cualitativos y permiten hacerse de elementos para tomar decisiones económicas, ambientales, sociales y de eficiencia productiva. El conjunto de indicadores propuesto para la evaluación de la circularidad se basó en una revisión crítica para la selección final de 17 métricas e indicadores por la pertinencia de las variables que involucran y su potencial de aplicación a los bioprocesos de tratamiento de la FORSU. Este estudio es relevante porque proporciona información útil para la conformación de un marco conceptual que favorezca el progreso de la bioeconomía circular y su aplicación en la gestión de la FORSU.

Lignocellulose hydrolysates are rich in fermentable sugars such as xylose, cellobiose and glucose, with high potential in the biotechnology industry to obtain bioproducts of higher economic value. Thus, it is important to search for and study new yeast strains that co-consume these sugars to achieve better yields and productivity in the processes. Summary Background Bleeding is the most serious complication of the use of oral anticoagulation in the prevention and treatment of thromoboembolic complications. We studied the frequency of bleeding complications in outpatients treated routinely in anticoagulation clinics. Methods In a prospective cohort from thirty-four Italian anticoagulation clinics, 2745 consecutive patients were studied from the start of their oral anticoagulation (warfarin in 64%, acenocourmarol in the rest). The target anticoagulation-intensity was low (international normalised ratio [INR] ≤2·8) in 71% of the patients and high (> 2·8) in the remainder. We recorded demographic details and the main indication for treatment and, every 3·4 months, INR and outcome events. Such events included all complications (bleeding, thrombosis, other), although only bleeding events are reported here, and deaths. We divided bleeding into major and minor categories. Findings 43% of the patients were women. Nearly three-fifths of the patients were aged 60·79; 8% were over 80. The main indication for treatment was venous thrombolism (33%), followed by non-ischaemic heart disease (17%). Mean follow-up was 267 days. Over 2011 patient-years of follow-up, 153 bleeding complications occurred (7·6 per 100 patient-years). 5 were fatal (all cerebral haemorrhages, 0·25 per 100 patient-years), 23 were major (1·1), and 125 were minor (6·2). The rate of events was similar between sexes, coumarin type, size of enrolling centre, and target INR. Por su contenido de proteínas y minerales, el pulido de arroz (PA) podría ser empleado en alimentación humana o animal si son eliminados el aceite y las fracciones con alto contenido de fibra. Para ello se realizó la extracción sólido-líquido al PA, con una eficiencia en la extracción del extracto etéreo del 95,6%, a fin de obtener el pulido de arroz desengrasado (PAD). Se efectuó la clasificación granulométrica del PA y PAD, y de cada fracción se determinó su composición química proximal: proteína, extracto etéreo, cenizas, fibra neutro detergente (FND) y extracto libre de nitrógeno (ELN). El PA tiene un alto contenido de fracciones gruesas (>70% corresponden a partículas con tamaño de malla >120), mientras que el PAD posee una distribución de partícula más uniforme y un contenido de grasas 70% of the mass corresponds to mesh sizes >120), whereas DRB exhibits a more uniform granulometric profile. High levels of factor VIII are a common risk factor for venous thromboembolism (VTE). The aim of this study was to evaluate the possible interaction between the presence of high factor VIII levels and oral contraceptive (OC) use.Factor VIII levels were measured by a chromogenic assay in 174 women who suffered from VTE in reproductive age and in 484 healthy women. Subjects carrying thrombophilic alterations were excluded.Factor VIII levels were stratified into quartiles. The adjusted odds ratio (OR) of VTE in subjects with factor VIII levels in the upper quartile (1.95 IU/mL) was 7.45 (95%CI: 3.80-14.6). Among the 174 patients, 85 had experienced VTE during OC use. The 179 healthy women who had used OC for at least 6 months in the two years before presentation but had stopped OC at least 3 months before the blood sampling were considered as OC users. The risk of VTE among OC users with factor VIII levels in the upper quartile was increased about 13-fold (OR: 13.0, 95% CI: 4.92-34.3).Our results showed that there is an increased risk of VTE due to oral contraceptive use in women with elevated factor VIII and, as has been previously described for factor V Leiden and G20210A prothrombin mutations, the raised level of the coagulation factor and oral contraceptive use seem to have a synergistic effect.

The changes in blood rheologic properties induced by exposure to high altitude plus heavy and prolonged physical strain were investigated in 14 climbers of an Italian expedition to Himalaya (maximum altitude reached m 7350). In blood samples collected immediately after return, alterations of some haemorheologic parameters were observed in comparison with baseline values, and namely an increase in relative blood viscosity, fibrinogen and erythrocyte filtration time along with a fall in platelet count. Treatment with a drug acting on the red cell deformability (Pentoxifylline) appeared to prevent the changes in blood viscosity and erythrocyte filterability. In the 9 climbers whom we were able to re-examine after three months, all altered parameters were found to be restored to baseline values. These results contribute to the knowledge of the pathogenesis of high altitude disease and stress the significance of altered haemorheology as one of the mechanisms possibly involved in the vascular events observed in this condition.

This study was carried out in order to evaluate changes in pulmonary vascular reactivity and in hemorheology induced by pentoxifylline infusion (100 mg) at rest and during standardized exercise in patients with chronic cor pulmonale secondary to chronic obstructive pulmonary disease. The administration of pentoxifylline at rest was associated with reduction in mean pulmonary artery pressure (p less than 0.01), pulmonary vascular resistance (p less than 0.01) and right ventricular stroke work index (p less than 0.02). Standard exercise performed after pentoxifylline infusion was also associated with significant reduction in mean pulmonary artery pressure and pulmonary vascular resistance. Rheologic tests showed less evident changes. Our data suggest that pentoxifylline significantly improves pulmonary hemodynamics at rest and partly reverses its changes, after exercise, in patients with chronic obstructive pulmonary disease.

SummaryThe aims of the present study were: l) interlaboratory normalization of prothrombin time (PT) testing for anticoagulant therapy control through calibration of customary thromboplastins against international reference materials, and 2) “on field” validation of the advantages offered by expression of results as International Normalized Ratio (INR) as opposedto percentage activity. PT tests were carried out over 8 days on the same normal subjects (16) and patients on oral an ticoagulants (48) in the 9 laboratories of the Bologna area. The use of customary thromboplastins and coagulometers was maintained in all labs throughout the study. The main results were: 1) the interlaboratory CV of the prothrombin ratios obtained for each sample with all customary thromboplastins (5 different brands) was 15%, but was reduced to levels of 5.8 to 8.9 when using constant thromboplastin brands and batches; 2) the International Sensitivity Index (ISI) values obtained in the different labs were only slightly influenced by the use of different coagulometers; 3) comparable ISI values were obtained through direct calibration with the international reference material and through intermediate calibration with a locally selected standard; 4) use of INR values instead of percentage activity greatly reduced interlaboratory variability and significantly improved uniformity of anticoagulation level measurements. thus reducing the possibility of erroneous prescriptions. The Bologna exercise is therefore of educational value for laboratory and community doctors of the area in understanding and accepting the INR system.

The aim of this study was to investigate the blood coagulation changes in three patients with homocystinuria, in baseline condition and during therapy. At baseline, antithrombin III activity and factor VII levels were reduced in all three patients; antithrombin III protein and protein C antigen were also slightly lowered in one patient, and factor X in another. beta-Thromboglobulin, a measure of platelet activation, was increased in one case. During pyridoxine treatment, antithrombin III activity was rapidly restored to normal; factor VII increased and beta-thromboglobulin decreased. These data suggest that, in addition to platelet activation, abnormalities of blood clotting, and particularly reduction of antithrombin III, may play a role in the thrombotic tendency associated with homocystinuria. The nature of these clotting alterations is still uncertain, but their improvement during active metabolic treatment suggests that the defect in amino acid transsulfuration of homocystinuria may directly affect synthesis or activity of some liver-dependent clotting factors.

Hemorheologic parameters were measured in cerebrovascular patients with a history of stroke or recurrent ischemic attacks in a quiescent phase of the disease. All patients were submitted to aortic arch angiography and then classified according to the presence or absence of detectable arterial lesions. In comparison with a group of control subjects, cerebrovascular patients had significantly elevated levels of blood and plasma viscosity, fibrinogen, factors VIII:C and VIIIR:Ag and lowered values of erythrocyte filtration. Blood viscosity was increased and erythrocyte filtration reduced only in male patients. Elderly patients had higher blood viscosity and fibrinogen levels, and those with a history of stroke also showed the highest hematocrit and plasma viscosity values. Patients with a stroke history compared with those affected by remittent ischemic attacks, and patients with positive in comparison with those with negative angiograms were characterized only by higher fibrinogen (and sometimes factor VIII) levels, but not by other differences in hemorheologic values. It is concluded that changes in hemorheologic values consistent with hyperviscosity are a feature of cerebrovascular patients also in the quiescent phase of the disease and not only as a result of acute ischemic attacks. These changes are however most frequent in males and in elderly subjects, while they seem to be almost evenly distributed in patients with or without evidence of arterial disease.

SummaryThe paper reports on rate and type of thrombotic events occurring during the observational, prospective, inception-cohort, multicenter ISCOAT study. 2,745 unselected, daily practice patients, consecutively referring to 34 Italian anticoagulation clinics to monitor the oral anticoagulant treatment, were included in the study from beginning of their first anticoagulant course. During a total follow-up of 2,011 patient-years of treatment 70 thrombotic events (3.5 per 100 patient years) were recorded in 67 patients: 20 fatal (1%), 39 major (1.9%) and 11 minor (0.6%). 34/70 events occurred within the first 90 days of treatment (relative risk - at multivariate analysis - of 90 = 20.6, C.I. 12.7-33.5; p 70 y (1.62, C.I. 1.0-2.61; p 2 = 1.88, C.1.1.16-3.07; p The recorded rate of thrombotic events was lower than that reported in the few available studies. A greater risk should be expected during the first 90 days of treatment, when anticoagulation levels are 70 years and in those with cerebrovascular/peripheral arterial disease.
