Calls from North Pacific right whales (Eubalaena japonica) were recorded in the eastern Bering Sea during a visual and acoustic survey aboard the US Coast Guard buoy tender Sweetbrier, in July 1999. Calls were commonly detected to 20km, and in one case approximately 30km, via deployment of arrays of directional sonobuoys. Acoustic detections (clusters of right whale calls separated by time and location) numbered 26, but only five right whales were seen. Only one right whale produced calls while under visual observation. The types of calls recorded from North Pacific right whales were similar in duration and frequency to calls recorded from right whales in the South Atlantic. The predominant call type (85%; 436 of 511 calls) was the ‘up’ call, a signal sweeping from about 90Hz to 150Hz in 0.7s. Two call types are described as ‘down’ and ‘constant’ calls, based upon nomenclature established for southern right whales (Eubalaena australis). One call type, the ‘down-up’ was unique to the North Pacific repertoire. Right whales commonly produced calls in series lasting several minutes and then became silent for an hour or more, with some animals not calling for periods of at least four hours. Other cetaceans detected acoustically by ‘random’ sonobuoy deployments during the cruise included fin whales (19 times), killer whales (3 times) and sperm whales (once).

We summarize recent progress in the development of top-down control over bottom-up colloidal processes using a taxonomy based on how active control is used to guide equilibrium, near-equilibrium, and far-from-equilibrium assembly.

Postoperative mortality for oropharynx squamous cell carcinoma (OPSCC) with transoral robotic surgery (TORS) varies from 0.2% to 6.5% on trials; the real-world rate is unknown.NCDB study from 2010 to 2017 for patients with cT1-2N0-2M0 OPSCC with Charleson-Deyo score 0-1. Ninety-day mortality assessed from start and end of treatment at Commission on Cancer-accredited facilities.3639 patients were treated with TORS and 1937 with radiotherapy. TORS cohort had more women and higher income, was younger, more often treated at academic centers, and more likely to have private insurance (all p 0.05). Ninety-day mortality was 1.3% with TORS and 0.7% or 1.4% from start or end of radiotherapy, respectively. From end of therapy, there was no significant difference on MVA between treatment modality.There is minimal difference between 90-day mortality in patients treated with TORS or radiotherapy for early-stage OPSCC. While overall rates are low, for patients with expectation of cure, work is needed to identify optimal treatment.

Purpose For patients with high-risk bladder cancer (pT3+ or N+), local regional failure remains a challenge after chemotherapy and cystectomy. An ongoing prospective phase 2 trial (NCT01954173) is examining the role of postoperative photon radiation therapy for high-risk patients using volumetric modulated arc therapy. Proton beam therapy (PBT) may be beneficial in this setting to reduce hematologic toxicity. We evaluated for dosimetric relationships with pelvic bone marrow (PBM) and changes in hematologic counts before and after pelvic radiation therapy and explored the potential of PBT treatment plans to achieve reductions in PBM dose. Materials and Methods All enrolled patients were retrospectively analyzed after pelvic radiation per protocol with 50.4 to 55.8 Gy in 28 to 31 fractions. Comparative PBT plans were generated using pencil-beam scanning and a 3-beam multifield optimization technique. Changes in hematologic nadirs were assessed using paired t test. Correlation of mean nadirs and relative PBM dose levels were assessed using the Pearson correlation coefficient (CC). Results Eighteen patients with a median age of 70 were analyzed. Mean cell count values after radiation therapy decreased compared with preradiation therapy values for white blood cells (WBCs), absolute neutrophil count (ANC), absolute lymphocyte count (all P Conclusion Increased PBM mean dose and V20 to V40 were associated with lower hematologic nadirs. PBT plans reduced PBM dose and may be a valuable strategy to reduce the risk of hematologic toxicity in these patients.

Metallic implants have been correlated to local control failure for spinal sarcoma and chordoma patients due to the uncertainty of implant delineation from computed tomography (CT). Such uncertainty can compromise the proton Monte Carlo dose calculation (MCDC) accuracy. A component method is proposed to determine the dimension and volume of the implants from CT images.The proposed component method leverages the knowledge of surgical implants from medical supply vendors to predefine accurate contours for each implant component, including tulips, screw bodies, lockers, and rods. A retrospective patient study was conducted to demonstrate the feasibility of the method. The reference implant materials and samples were collected from patient medical records and vendors, Medtronic and NuVasive. Additional CT images with extensive features, such as extended Hounsfield units and various reconstruction diameters, were used to quantify the uncertainty of implant contours.For in vivo patient implant estimation, the reference and the component method differences were 0.35, 0.17, and 0.04 cmA component method was developed and demonstrated using phantom and patient studies with implants. The proposed method provides more accurate implant characterization for proton MCDC and can potentially enhance the treatment quality for proton therapy. The current proof-of-concept study is limited to the implant characterization for lumbar spine. Future investigations could be extended to cervical spine and dental implants for head-and-neck patients where tight margins are required to spare organs at risk.

Purpose To evaluate the clinical outcomes and treatment related toxicities of charged particle-based re-irradiation (reRT; protons and carbon ions) for the definitive management of recurrent or second primary skull base and head and neck tumors. Materials and Methods The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were applied for the conduct of this systematic review. Published work in English language evaluating the role of definitive charged particle therapies in the clinical setting of reRT for recurrent or second primary skull base and head and neck tumors were eligible for this analysis. Results A total of 26 original studies (15 protons, 10 carbon ions, and 1 helium/neon studies) involving a total of 1,118 patients (437 with protons, 670 with carbon ions, and 11 with helium/neon) treated with curative-intent charged particle reRT were included in this systematic review. All studies were retrospective in nature, and the majority of them (n=23, 88 %) were reported as single institution experiences (87% for protons, and 90% for carbon ion-based studies). The median proton therapy reRT dose was 64.5 Gy (RBE 1.1) (range, 50.0 – 75.6 Gy ), while the median carbon ion reRT dose was 53.8 Gy (RBE 2.5 – 3.0) (range, 44.8 – 60 Gy ). Induction and/or concurrent chemotherapy was administered to 232 (53%) of the patients that received a course of proton reRT, and 122 (18%) for carbon ion reRT patients. ReRT with protons achieved 2-year local control rates ranging from 50% to 86%, and 41% to 92% for carbon ion reRT. The 2-year overall survival rates for proton and carbon ion reRT ranged from 33% to 80%, and 50% to 86% respectively. Late ≥ G3 toxicities ranged from 0% to 37%, with brain necrosis, ototoxicity, visual deficits, and bleeding as the most common complications. Grade 5 toxicities for all treated patients occurred in 1.4% (n= 16/1118) with fatal bleeding as the leading cause. Conclusions Based on current data, curative intent skull base and head and neck reRT with charged particle radiotherapy is feasible and safe in well-selected cases, associated with comparable or potentially improved local control and toxicity rates compared to historical reRT studies using photon radiotherapy. Prospective multi-institutional studies reporting oncologic outcomes, toxicity, and dosimetric treatment planning data are warranted to further validate these findings and to improve the understanding of the clinical benefits of charged particle radiotherapy in the reRT setting.

Purpose Dual-energy computed tomography (DECT) has been used to derive relative stopping power (RSP) maps by obtaining the energy dependence of photon interactions. The DECT-derived RSP maps could potentially be compromised by image noise levels and the severity of artifacts when using physics-based mapping techniques. This work presents a noise-robust learning-based method to predict RSP maps from DECT for proton radiation therapy. Materials and Methods The proposed method uses a residual attention cycle-consistent generative adversarial network to bring DECT-to-RSP mapping close to a 1-to-1 mapping by introducing an inverse RSP-to-DECT mapping. To evaluate the proposed method, we retrospectively investigated 20 head-and-neck cancer patients with DECT scans in proton radiation therapy simulation. Ground truth RSP values were assigned by calculation based on chemical compositions and acted as learning targets in the training process for DECT datasets; they were evaluated against results from the proposed method using a leave-one-out cross-validation strategy. Results The predicted RSP maps showed an average normalized mean square error of 2.83% across the whole body volume and an average mean error less than 3% in all volumes of interest. With additional simulated noise added in DECT datasets, the proposed method still maintained a comparable performance, while the physics-based stoichiometric method suffered degraded inaccuracy from increased noise level. The average differences from ground truth in dose volume histogram metrics for clinical target volumes were less than 0.2 Gy for D95% and Dmax with no statistical significance. Maximum difference in dose volume histogram metrics of organs at risk was around 1 Gy on average. Conclusion These results strongly indicate the high accuracy of RSP maps predicted by our machine-learning–based method and show its potential feasibility for proton treatment planning and dose calculation.

Background: Feelings of ownership play an important role in experiential education and are associated with several positive outcomes. However, researchers have yet to explore the dynamic nature of psychological ownership and its antecedents in experiential courses from the perspectives of the students. Purpose: The authors used the lens of psychological ownership theory to explore the experiences of students in an experiential learning setting. Methodology/Approach: In this study, the authors used repeated waves of semi-structured interviews in a two-semester classroom-as-organization (CAO). They then used a five-step process to identify themes describing the students’ experience. Findings/Conclusions: The authors found that different antecedents to psychological ownership worked together at different points in the development of the CAO, that students did not want full control of the CAO (despite their feelings of ownership toward it), and that students anticipated feelings of ownership that persisted after the class had ended. Implications: Educators may struggle to identify the level of control students’ need in a CAO to reap the benefits of the experience. Persistent feelings of ownership after experiential learning ends can contribute to student development.

PURPOSE This work aims to reduce dose delivery time of pencil beam scanning (PBS) proton plans, which is the dominant factor of total plan delivery time. A proton PBS system, such as Varian ProBeam proton therapy system, can be equipped with the proton dose rate that is linearly proportional to the minimum monitor unit (MU) (i.e., number of protons) of PBS spots before saturation. Thus dose delivery time can be potentially reduced by increasing the MU threshold. However, commercially available treatment planning systems and current methods only allow for a single MU threshold globally for all PBS spots (i.e., all energy layers), and consequently the room to increase this minimum-MU for reducing dose delivery time is very limited since higher minimum-MU can greatly degrade treatment plan quality. METHODS Two major innovations of this work are the proposal of using variable MU thresholds locally adaptive to each energy layer, that is, minimum-MU-per-energy-layer (MMPEL), for reducing dose delivery time, and the joint optimization of plan delivery time and plan quality. Minimum-MU-per-energy-layer is formulated as a constrained optimization problem with objectives of dose-volume-histogram based planning constraints and plan delivery time, and minimum-MU constraints per energy layer for deliverable PBS spots. Minimum-MU-per-energy-layer is solved by iterative convex relaxations via alternating direction method of multipliers. RESULTS Representative prostate, lung, brain, head-and-neck, breast, liver and pancreas cases were used to validate MMPEL. Minimum-MU-per-energy-layer reduced dose delivery time to 53%, 67%, 67%, 53%, 54%, 32%, and 14% respectively while maintaining a similar plan quality. Accepting a slightly degraded plan quality that still met all physician planning constraints, the treatment time could be further reduced to 26%, 35%, 41%, 34%, 32%, 16%, and 11% respectively, or in another word MMPEL accelerated the PBS plan delivery by 2-10 fold. CONCLUSIONS A new proton PBS treatment planning method MMPEL with variable energy-adaptive MU thresholds is developed to optimize dose delivery time jointly with plan quality. The preliminary results suggest that MMPEL could substantially reduce dose delivery time.

Purpose In intensity-modulated proton therapy (IMPT), protons are used to deliver highly conformal dose distributions, targeting tumors, and sparing organs-at-risk. However, due to uncertainties in both patient setup and relative stopping power (RSP) calculation, margins are added to the treatment volume during treatment planning, leading to higher doses to normal tissues. Cone-beam computed tomography (CBCT) images are taken daily before treatment; however, the poor image quality of CBCT limits the use of these images for online dose calculation. In this work, we use a deep-learning-based method to predict RSP maps from daily CBCT images, allowing for online dose calculation in a step toward adaptive radiation therapy. Methods Twenty-three head-and-neck cancer patients were simulated using a Siemens TwinBeam dual-energy CT (DECT) scanner. Mixed-energy scans (equivalent to a 120 kVp single-energy CT scan) were converted to RSP maps for treatment planning. Cone-beam computed tomography images were taken on the first day of treatment, and the planning RSP maps were registered to these images. A deep learning network based on a cycle-GAN architecture, relying on a compound loss function designed for structural and contrast preservation, was then trained to create an RSP map from a CBCT image. Leave-one-out and holdout cross validations were used for evaluation, and mean absolute error (MAE), mean error (ME), peak signal-to-noise ratio (PSNR), and structural similarity (SSIM) were used to quantify the differences between the CT-based and CBCT-based RSP maps. The proposed method was compared to a deformable image registration-based method which was taken as the ground truth and two other deep learning methods. For one patient who underwent resimulation, the new planning RSP maps and CBCT images were used for further evaluation and validation. Results The CBCT-based RSP generation method was evaluated on 23 head-and-neck cancer patients. From leave-one-out testing, the MAE between CT-based and CBCT-based RSP was 0.06 ± 0.01 and the ME was -0.01 ± 0.01. The proposed method statistically outperformed the comparison DL methods in terms of MAE and ME when compared to the planning CT. In terms of dose comparison, the mean gamma passing rate at 3%/3 mm was 94% when three-dimensional (3D) gamma index was calculated per plan and 96% when gamma index was calculated per field. Conclusions The proposed method provides sufficiently accurate RSP map generation from CBCT images, allowing for evaluation of daily dose based on CBCT and possibly allowing for CBCT-guided adaptive treatment planning for IMPT.

PURPOSE: Human papillomavirus (HPV)–related squamous cell carcinomas of the head and neck (SCCHNs) tend to have a distinct prognosis. Socioeconomic and demographic factors associated with metastatic disease at presentation and diagnosis in patients with HPV-related SCCHN tumors were examined. METHODS: The National Cancer Database (NCDB) was queried to assess patients with HPV-related oropharyngeal carcinomas (HPVOPCAs) and HPV-related nonoropharyngeal carcinomas (HPVNOPCAs) diagnosed between 2010 and 2014. Rate of metastases at presentation was analyzed using clinical M stage. Multivariable analysis was performed evaluating race, ethnicity, sex, age, facility location, facility type, insurance status, income, education, and tumor and nodal stage using logistic regression. RESULTS: A total of 12,857 patients with HPVOPCA and 952 patients with HPVNOPCA were included. Private insurance was carried by 64% and 47% of patients with HPVOPCA and HPVNOPCA, respectively. HPVOPCA was located in the tonsil in 56% of patients. For both HPVOPCA and HPVNOPCA, there was no meaningful difference in distant metastasis at presentation based on facility type or location, sex, race, Hispanic ethnicity, or urban or rural location. For HPVOPCA, there were significantly lower odds of metastasis in privately insured patients compared with uninsured patients (odds ratio [OR], 0.37; 95% CI, 0.21 to 0.64; P < .001) and higher odds of metastasis for patients living in census tracts with the lowest rates of high school graduates compared with the highest rates (OR, 1.81; 95% CI, 1.02 to 3.19; P = .041) and for patients with higher tumor stage (OR, 3.67, 95% CI, 2.25 to 5.99; P < .001) and nodal stage (OR, 3.34; 95% CI, 2.11 to 5.29; P < .001). For HPVNOPCA, neither higher T or N stage nor any demographic features were found to be associated with metastasis at presentation. CONCLUSION: This large retrospective analysis identifies likely modifiable risk factors for metastatic presentation in HPVOPCA. Educational interventions may result in modifications of these patterns.

Quality assurance computed tomography (QACT) is the current clinical practice in proton therapy to evaluate the needs for replan. QACT could falsely indicate replan because of setup issues that would be solved on the treatment machine. Deforming the treatment planning CT (TPCT) to the pretreatment CBCT may eliminate this issue. We investigated the performance of replan evaluation based on deformed TPCT (TPCTdir) for proton head and neck (HN) therapy.Twenty-eight HN datasets along with pretreatment CBCT and QACT were used to validate the method. The changes in body volume were analyzed between the no-replan and replan groups. The dose on the TPCTdir, the deformed QACT (QACTdir), and the QACT were calculated by applying the clinical plans to these image sets. Dosimetric parameters' changes, including ΔD95, ΔDmean, and ΔD1 for the clinical target volumes (CTVs) were calculated. Receiver operating characteristic curves for replan evaluation based on ΔD95 on QACT and TPCTdir were calculated, using ΔD95 on QACTdir as the reference. A threshold for replan based on ΔD95 on TPCTdir is proposed. The specificities for the proposed method were calculated.The changes in the body contour were 95.8 ± 83.8 cc versus 305.0 ± 235.0 cc (p 0.01) for the no-replan and replan groups, respectively. The ΔD95, ΔDmean, and ΔD1 are all comparable for all the evaluations. The differences between TPCTdir and QACTdir evaluations were 0.30% ± 0.86%, 0.00 ± 0.22 Gy, and -0.17 ± 0.61 Gy for CTV ΔD95, ΔDmean, and ΔD1, respectively. The corresponding differences between the QACT and QACTdir were 0.12% ± 1.1%, 0.02 ± 0.32 Gy, and -0.01 ± 0.71 Gy. CTV ΔD95 2.6% in TPCTdir was chosen as the threshold to trigger QACT/replan. The corresponding specificity was 94% and 98% for the clinical practice and the proposed method, respectively.The replan evaluation based on TPCTdir provides better specificity than that based on the QACT.

Purpose While intensity modulated proton therapy can deliver simultaneous integrated boost (SIB) to the dominant intraprostatic lesion (DIL) with high precision, it is sensitive to anatomic changes. We investigated the dosimetric effects from these changes based on pretreatment cone-beam computed tomographic (CBCT) images and identified the most important factors using a multilayer perceptron neural network (MLPNN). Methods and Materials DILs were contoured based on coregistered multiparametric magnetic resonance images for 25 previously treated prostate cancer patients. SIB plans were created with (1) prostate clinical target volume − V70 Gy = 98%; (2) DIL − V98 Gy > 95%; and (3) all organs at risk (OARs)"?> within clinical constraints. SIB plans were applied to daily CBCT-based deformed planning computed tomography (CT)"?>. DIL − V98 Gy, bladder/rectum maximum dose (Dmax) and volume changes, femur shifts, and the distance from DIL to organs at riskOARs"?> in both planning computed tomogramsCT"?> and CBCT were calculated. Wilcoxon signed-ranks tests were used to compare the changes. MLPNNs were used to model the change in ΔDIL − V98 Gy > 10% and bladder/rectum Dmax > 80 Gy, and the relative importance factors for the model were provided. The performances of the models were evaluated with receiver operating characteristic curves. Results Comparing initial plan to the average from evaluation plans, respectively, DIL − V98 Gy was 89.3% ± 19.9% versus 86.2% ± 21.3% (P = .151); bladder Dmax 71.9 ± 0.6 Gy versus 74.5 ± 2.9 Gy (P < .001); and rectum Dmax 70.1 ± 2.4 Gy versus 74.9 ± 9.1Gy (P = .007). Bladder and rectal volumes were 99.6% ± 39.5% and 112.8% ± 27.2%, respectively, of their initial volume. The femur shift was 3.16 ± 2.52 mm. In the modeling of ΔDIL V98 Gy > 10%, DIL to rectum distance changes, DIL to bladder distance changes, and rectum volume changes ratio are the 3 most important factors. The areas under the receiver operating characteristic curves were 0.89, 1.00, and 0.99 for the modeling of ΔDIL − V98 Gy > 10%, and bladder and rectum Dmax > 80 Gy, respectively. Conclusions Dosimetric changes in DIL SIB with intensity modulated proton therapy can be modeled and classified based on anatomic changes on pretreatment images by an MLPNN.

Biological systems rely on chemical gradients to direct motion through both chemotaxis and signaling, but synthetic approaches for doing the same are still relatively naïve. Consequently, we present a novel method for using chemical gradients to manipulate the position and velocity of colloidal particles in a microfluidic device. Specifically, we show that a set of spatially localized chemical reactions that are sufficiently controllable can be used to steer colloidal particles via diffusiophoresis along an arbitrary trajectory. To accomplish this, we develop a control method for steering colloidal particles with chemical gradients using nonlinear model predictive control with a model based on the unsteady Green’s function solution of the diffusion equation. We illustrate the effectiveness of our approach using Brownian dynamics simulations that steer single particles along paths, such as circle, square, and figure-eight. We subsequently compare our results with published techniques for steering colloids using electric fields, and we provide an analysis of the physical parameter space where our approach is useful. Based on these findings, we conclude that it is theoretically possible to explicitly steer particles via chemical gradients in a microfluidics paradigm.

Background We assessed locoregional control with omission of intentional primary site radiation after transoral robotic surgery (TORS) and quantified nontargeted primary site dose. Methods Following Institutional Review Board (IRB) approval, patients treated with primary TORS resection for squamous cell carcinomas of the oropharynx were reviewed. Patients with cT1-2 tumors, >2 mm margins, in whom the surgeon resected the primary without revising specimen-driven margins, qualified for omission of primary site radiation. Results From 2014 to 2019, 112 patients met criteria. Fifty-nine (52%) patients did not receive radiation targeting the primary site; of whom, 22 received no radiation. In this group, there were no local failures; mean age was 58 years and median follow-up was 25 months. Thirty-seven patients received adjuvant radiation targeting the neck, mean bystander dose to the primary site was 28.8 Gy (range, 13.3-50.6 Gy). Conclusion In a 59 patient population, omission of radiation to the primary site after TORS resulted in no locoregional failures.

Agricultural soils account for less than 10% of the total greenhouse gas (GHG) emissions in the United States but about 75% of nitrous oxide (N2O) emissions. Soil conservation practices, such as no-tillage, have the potential to mitigate GHG emissions. We examined the short-term consequences of no-tillage with a winter wheat cover crop (NTW) and no-tillage winter fallow (NT) on N2O emissions, N2O reducing bacterial populations, and overall soil bacterial abundance during the summer growing season in the southern Great Plains, United States. Conservation practices were coupled with nitrogen (N) fertilizer application timing (100% pre-plant, 100% mid-season, 40% pre-plant 60% mid-season, 100% pre-plant with N stabilizer). In addition, N2O emissions were measured to determine any functional effects of altering N fertilizer timing and changing bacterial populations. The combination of N treatment and conservation practice affected nosZ clade II abundance in the second year of the study. Diversity of nosZ clade II was evaluated to determine effects on non-typical N2O reducers which were highly abundant in this study. No nosZ clade II diversity effects were determined, although some clustering of conservation system and N treatments was observed in the second year. Nitrogen treatment affected N2O-N emissions during the summer of both years, likely related to overall increased microbial activity and N fertilizer application. Negative fluxes (consumption) of N2O-N were observed in every treatment and tillage combination and were most pronounced in the control (0 kg N ha−1). Negative fluxes are likely due to a combination of low inorganic-N concentrations at various points during the year and a robust clade II population driving N2O consumption. Altering conservation system and the timing of N fertilizer application affects the microbial community and will likely continue to select for unique communities as the system matures. This will also likely further impact N2O emissions from the system and may increase the rate and frequency of N2O consumption.

Purpose Proton beam therapy can significantly reduce cardiopulmonary radiation exposure compared with photon-based techniques in the postmastectomy setting for locally advanced breast cancer. For patients with metallic port tissue expanders, which are commonly placed in patients undergoing a staged breast reconstruction, dose uncertainties introduced by the high-density material pose challenges for proton therapy. In this report, we describe an intensity modulated proton therapy planning technique for port avoidance through a hybrid single-field optimization/multifield optimization approach. Methods and Materials In this planning technique, 3 beams are utilized. For each beam, no proton spot is placed within or distal to the metal port plus a 5 mm margin. Therefore, precise modeling of the metal port is not required, and various tissue expander manufacturers/models are eligible. The blocked area of 1 beam is dosimetrically covered by 1 or 2 of the remaining beams. Multifield optimization is used in the chest wall target region with blockage of any beam, while single-field optimization is used for remainder of chest wall superior/inferior to the port. Results Using this technique, clinical plans were created for 6 patients. Satisfactory plans were achieved in the 5 patients with port-to-posterior chest wall separations of 1.5 cm or greater, but not in the sixth patient with a 0.7 cm separation. Conclusions We described a planning technique and the results suggest that the metallic port-to-chest wall distance may be a key parameter for optimal plan design.

Background: The Classroom-as-Organization (CAO) is an experiential learning course in which students create and manage an organization as part of class activities. Student interaction with peers is an important feature of the CAO. Educators suggested that student interactions in CAOs follow certain patterns, but these observations have not been tested in empirical research. Purpose: The research focused on exploring patterns of interaction in a CAO. Methodology/Approach: Network data were collected from students and instructors at four different times during a two-semester CAO course. The authors then used social status theory, friendship formation literature, and Simulation Investigation for Empirical Network Analysis (SIENA) to test hypotheses about interactions in the CAO. Findings/Conclusions: The authors found evidence that (a) localized informal leaders emerge in the class, (b) reciprocation ties between individuals happens within, but not necessarily between departments, and (c) there is a close connection between class-related interactions and social interactions outside of the class, impacting leadership in the CAO. Implications: Instructors need to pay close attention to understanding and managing interdepartmental relationships in CAOs. Furthermore, educators may only have a limited understanding of CAO group dynamics, given that they may not have a high awareness of student social interactions outside of class.

PURPOSE Transitioning from two-dimensional to three-dimensional treatment planning requires developing contouring skills. Contouring atlases are excellent resources, but they do not provide users active feedback. Developing countries may not have many radiation oncologists experienced in three-dimensional planning to provide training. We sought to develop a standardized self-guided educational module with integrated feedback to teach contouring skills. METHODS AND MATERIALS All 18 oncology residents at Black Lion Hospital/Addis Ababa University in Ethiopia were trained to contour the level II lymph node station. Residents took a baseline pretest quiz, survey, and contouring evaluation. Residents then watched an instructional contouring lecture and performed three additional cases with integrated feedback by comparing their contours to gold-standard contours. Residents then took a post-training quiz, survey, and contouring evaluation. Paired t tests and analysis of variance were used for analysis. RESULTS Before training, the average number of total cases ever contoured was 2.4 and the average number of head and neck cases contoured was 0.5. Comfort with contouring improved from being “not at all comfortable” to “quite comfortable” after the 3-hour training ( P < .001). The standard deviation between the resident contours and gold standard improved from 72.6 cm3 (pretest) to 7.4 cm3 (post-test). The average percentage overlap with the gold-standard contours and Dice similarity coefficient improved with each case performed, from 27.7% and 0.26 (pretest) to 80.1% and 0.77 (post-test), respectively ( P < .001). After training, 16 of 18 (88.9%) residents produced a Dice similarity coefficient greater than 0.7, the threshold generally accepted for excellent agreement. CONCLUSION This self-guided teaching module was an effective tool for developing level II lymph node contouring skills by providing active feedback and resulted in improved user confidence and accuracy compared with a gold standard. This module can be expanded to other disease sites and countries to further facilitate transitioning to three-dimensional treatment planning in developing countries.

Headaches in children and adolescents remain a very common problem with migraine being the most common headache disorder to present to medical attention. The approach to the treatment of migraine in children has consisted of treatment with acute and preventive medications, combined with lifestyle modification and behavioral interventions, such as cognitive behavioral therapy. With increasing frequency, complementary and alternative medicine (CAM) approaches, including acupuncture, are often recommended in the pediatric population to address significant disability with limited evidence-based treatment options. In this article, the authors conduct a review of acupuncture in pediatric headache, including neurobiological mechanisms, adult headache studies, pediatric headache studies, safety, and use of acupuncture in other conditions in children. This article aims to summarize the currently available evidence with which to recommend acupuncture in children for the adjunctive treatment of headache. Acupuncture appears to be safe and effective for the treatment of migraine in children.

Background Small cell carcinomas of the head and neck (SmCCHNs) are rare neoplasms with an unfavorable prognosis. Population-based data describing survival and prognostic factors for SmCCHN are limited. Methods Data were obtained from the U.S. National Cancer Institute's Surveillance, Epidemiology, and End Results (SEER) database for 1973–2013. Patient and tumor-related characteristics for SmCCHN were compared with those for squamous cell carcinoma of the head and neck (SCCHN). Survival was compared by constructing Kaplan-Meier curves and Cox proportional hazard models with and without propensity score matching. Results The data set included 609 SmCCHN and 227,943 SCCHN cases. Both histological subtypes were more common in men than women and more common in white patients. SmCCHN was most likely to originate in the larynx, glottis and hypopharynx, or salivary glands and to present with more advanced stage and grade. SCCHN was most likely to originate in the oral cavity and was found infrequently in the salivary glands. Overall 5- and 10-year survival estimates were 27% and 18% for SmCCHN and 46% and 31% for SCCHN, respectively. In multivariable survival analyses adjusting for age, sex, race, marital status, year of diagnosis, stage, grade, and receipt of radiation, the hazard ratio (HR) comparing SmCCHN with SCCHN was 1.53 with a 95% confidence interval (CI) from 1.39 to 1.68. Average 5-year survival varied widely between the histologic types when comparing tumor sites: 14.5% for SmCCHN versus 48.9% for SCCHN in the oropharynx. In propensity score matched analyses, the corresponding HR was 1.27 (95% CI, 1.15–1.40). Conclusion Compared with SCCHN, SmCCHN carries a worse survival and is more likely to present with more advanced stage. Implications for Practice Small cell carcinoma of the head and neck (SmCCHN) is a rare subtype of head and neck cancer. In this Surveillance, Epidemiology, and End Results (SEER) data analysis, the characteristics and survival of SmCCHN are compared with those of the common squamous cell carcinoma of the head and neck. Results show that SmCCHN carries a worse prognosis and tends to present at a more advanced stage; SmCCHN also is ten times more likely to originate from the salivary glands. These findings may have implications for clinical practice, as location of the tumor may strongly associate with the pathologic diagnosis. If a SmCCHN is diagnosed, a disseminated disease is likely; hence vigilance in staging procedures is indicated.

PURPOSE Auto-segmentation algorithms offer a potential solution to eliminate the labor-intensive, time-consuming, and observer-dependent manual delineation of organs-at-risk (OARs) in radiotherapy treatment planning. This study aimed to develop a deep learning-based automated OAR delineation method to tackle the current challenges remaining in achieving reliable expert performance with the state-of-the-art auto-delineation algorithms. METHODS The accuracy of OAR delineation is expected to be improved by utilizing the complementary contrasts provided by computed tomography (CT) (bony-structure contrast) and magnetic resonance imaging (MRI) (soft-tissue contrast). Given CT images, synthetic MR images were firstly generated by a pre-trained cycle-consistent generative adversarial network. The features of CT and synthetic MRI were then extracted and combined for the final delineation of organs using mask scoring regional convolutional neural network. Both in-house and public datasets containing CT scans from head-and-neck (HN) cancer patients were adopted to quantitatively evaluate the performance of the proposed method against current state-of-the-art algorithms in metrics including Dice similarity coefficient (DSC), 95th percentile Hausdorff distance (HD95), mean surface distance (MSD), and residual mean square distance (RMS). RESULTS Across all of 18 OARs in our in-house dataset, the proposed method achieved an average DSC, HD95, MSD, and RMS of 0.77 (0.58-0.90), 2.90 mm (1.32-7.63 mm), 0.89 mm (0.42-1.85 mm), and 1.44 mm (0.71-3.15 mm), respectively, outperforming the current state-of-the-art algorithms by 6%, 16%, 25%, and 36%, respectively. On public datasets, for all nine OARs, an average DSC of 0.86 (0.73-0.97) were achieved, 6% better than the competing methods. CONCLUSION We demonstrated the feasibility of a synthetic MRI-aided deep learning framework for automated delineation of OARs in HN radiotherapy treatment planning. The proposed method could be adopted into routine HN cancer radiotherapy treatment planning to rapidly contour OARs with high accuracy.

Background Tobacco and alcohol use are risk factors for Squamous Cell Carcinoma of the Head and Neck (SCCHN); however, there is growing recognition of HPV as a risk factor for SCCHN. HPV‐related SCCHN is thought to affect mostly middle‐aged individuals but as the US population ages, it is important to evaluate the change in incidence of HPV‐ and non‐HPV‐related SCCHN in individuals who are ≥65 years old. Methods This was a retrospective study using data from a population‐based cancer registry (SEER) to identify individuals ≥65 years old diagnosed with SCCHN between 2000 and 2016 also stratified by sex, race, and birth cohort. The subgroups of HPV‐associated and non‐HPV associated sites were analyzed independently. The incidence per year was calculated and joinpoint detection was used to identity significant changes in incidence trends and annual percent change (APC). Results For HPV‐associated sites from 2000 to 2016, there was an average annual rate of 10.8 per 100,000 individuals with an APC of 2.92% (p =, This study reports results that are consistent with other studies demonstrating that the prevalence HPV associated of head and neck cancer overall is increasing in the elderly population age ≥65 years. This increase is occurring when stratified by race, sex, and birth cohort however, the White male population is experiencing the most significant annual percent increase in the incidence of HPV‐associated head and neck cancers. HPV‐associated head and neck cancers are no longer a disease associated with younger patients and the increasing prevalence of this disease in the elderly population is important when considering the unique challenges of treatment for this age group.
