Abstract

Aim: Low anterior resection syndrome (LARS) is pragmatically defined as disordered bowel function after rectal resection leading to a detriment in quality of life. This broad characterization does not allow for precise estimates of prevalence. The LARS score was designed as a simple tool for clinical evaluation of LARS. Although the LARS score has good clinical utility, it may not capture all important aspects that patients may experience. The aim of this collaboration was to develop an international consensus definition of LARS that encompasses all aspects of the condition and is informed by all stakeholders. Method: This international patient–provider initiative used an online Delphi survey, regional patient consultation meetings, and an international consensus meeting. Three expert groups participated: patients, surgeons and other health professionals from five regions (Australasia, Denmark, Spain, Great Britain and Ireland, and North America) and in three languages (English, Spanish, and Danish). The primary outcome measured was the priorities for the definition of LARS. Results: Three hundred twenty-five participants (156 patients) registered. The response rates for successive rounds of the Delphi survey were 86%, 96% and 99%. Eighteen priorities emerged from the Delphi survey. Patient consultation and consensus meetings refined these priorities to eight symptoms and eight consequences that capture essential aspects of the syndrome. Sampling bias may have been present, in particular, in the patient panel because social media was used extensively in recruitment. There was also dominance of the surgical panel at the final consensus meeting despite attempts to mitigate this. Conclusion: This is the first definition of LARS developed with direct input from a large international patient panel. The involvement of patients in all phases has ensured that the definition presented encompasses the vital aspects of the patient experience of LARS. The novel separation of symptoms and consequences may enable greater sensitivity to detect changes in LARS over time and with intervention.

7. New [sup 40] Ar/[sup 39] Ar ages from the central Paterson Orogen, Western Australia.

Abstract

[sup 40] Ar/[sup 39] Ar muscovite laser step-heating plateaus from the Rudall Complex and unconformably overlying Coolbro Sandstone of the Throssell Group yield plateau ages between 671 and 646 Ma, and one [sup 40] Ar/[sup 39] Ar K-feldspar sample from the Rudall Complex gave a complex age spectrum that defines a plateau age of 554 Ma. The muscovite data suggest that the Broadhurst map sheet area, comprising the Rudall Complex and unconformably overlying Throssell Group, experienced a thermal event (∼365°C) at ca 655 Ma. These muscovite ages pre-date published ages of felsic plutonism in the Telfer area (ca 630–610 Ma), pre-date estimates of the Paterson Orogeny (ca 550 Ma), and are similar to an unpublished age of mafic plutonism in the immediate area (ca 692 Ma). The new ages are attributed to thermal resetting of Ar isotope systematics during the Miles Orogeny (ca 680–630 Ma). These data suggest that the Miles Orogeny thermal event might have an effect on the interpretation of geochemical signatures attributed to economically important mineralisation such as the Telfer gold and Kintyre uranium deposits. Abstract

The deposit, hosted by Palaeoproterozoic metamorphic rocks, has high-grade ore confined to a reverse fault/shear zone that crosscuts a series of interbedded schists and amphibolites. Petrographic studies, fluid-inclusion and isotope analysis and Ar-Ar dating have identified up to eight significant fluid events. Uranium mineralisation is associated with an inner and outer alteration halo that extends as far as 1 km from the fault. Abstract

Pb isotope ratios and associated U, Th and Pb concentrations extracted by partial leaching of Proterozoic sandstones and basement rocks reveal much about fluid evolution in basins hosting unconformity-related uranium deposits and have great potential for exploration in basins of any age. Radiogenic Pb isotope ratios are found at key geological localities distal to known high-grade U mineralisation and particularly in altered zones proximal to mineralisation. Sandstones completely cemented by quartz overgrowths, on the other hand, typically have non-radiogenic Pb isotope ratios. Mafic volcanic units with relatively reducing lithologies that have removed U from basinal brines have U-supported radiogenic Pb isotope ratios.
