Abstract

Restrictions imposed during the COVID-19 pandemic might have changed recreational habits. In this study, the results of toxicological tests for alcohol and drugs in blood were compared among drivers stopped at roadside checks in the periods before (1 January 2018 to 8 March 2020) and after the lockdown measures (9 March 2020 to 31 December 2021). A total of 123 (20.7%) subjects had a blood alcohol level above the legal limit for driving of 0.5 g/L, 21 (3.9%) subjects tested positive for cocaine, and 29 (5.4%) subjects positive for cannabis. In the COVID-19 period, the mean blood alcohol level was significantly higher than in the previous period. Cannabis use, which was more frequent among younger subjects, was statistically associated with cocaine use. There has also been a quantitative increase in alcohol levels in the population with values above the legal limits, indicative of greater use of alcohol in the population predisposed to its intake.

Abstract

Investigating a body found hanged, without eyewitnesses, to determine the cause and manner of death can be difficult. Video surveillance footage can help document the events and provide useful data for the proper assessment of the case. The authors report the case of an 86-year-old man found suspended on the outer wall of his home and who died following the hanging carried out as a suicide. The images taken by the surveillance camera are useful for reconstructing the incident and to confirm that the cause of death is attributable to the hanging, without the interference of extrinsic factors. The sequence of events relating to hanging and subsequent convulsive movements typical of the last moments of life, as seen from the video surveillance recordings, does not seem to differ much from those described in the experiments reported in the literature. The reporting of this suicidal event has yet to be considered a significant isolated case study for the study of the human hanging process. [ABSTRACT FROM AUTHOR]

Abstract

The sodium rhodizonate test is a simple colorimetric technique used to identify traces of lead and barium. A positive test provides rapid and photographically documentable results for shots fired at distances not exceeding 50 cm. Usually, this method is not used for identification of the firing hand, which is detected by electron microscopy with energy dispersive analysis (SEM-EDX), requiring the removal of any particles on the surface to be analyzed by STUB. The use of Sodium Rhodizonate, which is undoubtedly less selective, has the disadvantage of target element solubilization (lead and barium) and thus the loss of the preliminary morphological investigation of the particles. The purpose of this paper is to demonstrate that the two indicated methods can be considered complementary in the following order: 1. buffer sampling for electron microscopy investigation (STUB); 2. and subsequent performance of the Sodium Rhodizonate Test. In some special cases, when it is necessary to obtain the result more quickly, with regards to respecting the chronology of the aforementioned tests, it may be useful to carry out the Sodium Rhodizonate Test first, especially when particular and perhaps antiquated weapons were used for suicidal purposes causing considerable gas dispersion and shot residues from the mechanisms. This research illustrates how significant results were obtained substantiating the suicide hypothesis, following the use of an old revolver (year of manufacture around 1915-1920). Abstract

One of the cornerstones for enhancing the patient safety culture is the incident reporting system (IRS). It is a process for detecting, reporting, collecting, and summarizing adverse events (AEs) and near-misses in healthcare, and so it represents a vital tool for clinical risk management. We analyzed the 5-year experience of a third-level hospital's IRSs, showing its trends and highlighting its main strengths and weaknesses. Patients’ falls and physical or verbal aggression toward the providers or between patients are the most reported events. Underreporting is the main limitation of the system, especially among nurses. Visible actions, forceful analysis of the reports, operators’ education, no-blame culture promotion, and organizational adjustments may improve operators’ adherence to IRS. Providers do not willingly inform patients’ relatives about fatal incidents. Despite that, the IRS is far from its potential, and the number of data collected has increased.

Abstract

In recent years, violence against minors has been the subject of in-depth study in all its historical, social and cultural dimensions, being able to be identified and treated according to rigorous scientific methodologies. The A.A. report the case of an 8-year-old girl, who disappeared in a rural area, found dead a few hours later, on the recommendation of a young man who, despite having admitted to having gone away with the child, had not provided further details. The forensic inspection revealed that the discovery area included an area with burnt undergrowth about 4 meters in diameter. Examination of the external genitalia showed that the hymen was hemorrhagic in the right hemicycle, at 11 o'clock, with laceration of the most superficial layers of the anterior face. In the present case, therefore, despite having limited historical and circumstantial data, it was possible to reconstruct the following damaging dynamic: the little girl, after having gone away with her aggressor, was first the victim of an attempt of sexual violence by the latter and later immobilized by placing a foot on the slender abdomen. She had been covered with brushwood which the attacker had set fire to, thus killing her. This reconstruction of the criminal events, proposed to the investigating magistrate, was confirmed in each passage by the confessions made by the murderer, during the various degrees of trial, so much so that the final sentence reported in full the specific steps of the medico-legal relationship. Abstract

The figure of the junior doctor is defined in different ways depending on age, educational path, and contract terms related to the conditions of the work contract. The issues related to this particular professional figure should be considered one of the greatest tests to which Health Systems have to respond. In this paper, these aspects are discussed together with practical advice that could be tailored to individual working situations in order to adequately address these issues. Cornerstones and common practices of clinical risk management which may be successfully applied in these contexts are also discussed. The application of Risk Management tools can both provide proactive solutions to reduce the risk of litigation, and create an environment based on trust and one in which junior doctors feel encouraged to signal adverse events and to learn from them. Abstract

A mass disaster is a situation that involves criticality between the number of victims and resources, in terms of both men and means, present on the site of an event that is mostly unexpected and sudden. In the multidisciplinary teams that intervene, the role of forensic pathologists, who are responsible for the direction and coordination of post-mortem operations, is central, and must remain so. The authors report the case of an explosion of a pyrotechnic artifice factory, as a result of which numerous victims and injuries are recorded. So, the team completed the autopsies and created a protocol to obtain biological samples (bones, blood, teeth, muscles), while the forensic pathologists contacted the families of the alleged victims and each provided a blood sample that was collected for the DNA. The geneticist, using the method of gene extraction and amplification, obtained the DNA from each bone, tooth, and muscle of blood taken from the victims and then compared it with that extracted from the blood samples of the relatives; the electropherograms showed at least one allele for each genetic marker of the "Combined DNA Index System" in common between the victims and the families, thus allowing to establish the identity of all the subjects involved in the event. Having established the identity of all workers, it was possible to determine their whereabouts in the environment at the time of the location of fires and explosions. The results of the various forensic analyzes (autopsies, genetic investigations and even traumatological investigations) have allowed us to validate a scientific method useful in all mass disasters even when any type of anthropological or forensic dental research is difficult.
