Vertebral detection and segmentation are critical steps for treatment planning in spine surgery and radiation therapy. Accurate identification and segmentation are complicated in imaging that does not include the full spine, in cases with variations in anatomy (T13 and/or L6 vertebrae), and in the presence of fracture or hardware. This paper proposes VertDetect, a fully automated end-to-end 3D vertebral instance segmentation Convolutional Neural Network (CNN) model to predict vertebral level labels and segmentations for all vertebrae present in a CT scan. The utilization of a shared CNN backbone provides the detection and segmentation branches of the network with feature maps containing both spinal and vertebral level information. A Graph Convolutional Network (GCN) layer is used to improve vertebral labelling by using the known structure of the spine. This model achieved a Dice Similarity Coefficient (DSC) of 0.883 (95% CI, 0.843-0.906) and 0.882 (95% CI, 0.835-0.909) in the VerSe 2019 and 0.868 (95\% CI, 0.834-0.890) and 0.869 (95\% CI, 0.832-0.891) in the VerSe 2020 public and hidden test sets, respectively. Machine learning models experience deteriorated performance when trained in the presence of noisy labels. This is particularly problematic for medical tasks, such as survival prediction, which typically face high label noise complexity with few clear-cut solutions. Inspired by the large fluctuations across folds in the cross-validation performance of survival analyses, we design Monte-Carlo experiments to show that such fluctuation could be caused by label noise. We propose two novel and straightforward label noise detection algorithms that effectively identify noisy examples by pinpointing the samples that more frequently contribute to inferior cross-validation results. We first introduce Repeated Cross-Validation (ReCoV), a parameter-free label noise detection algorithm that is robust to model choice. We further develop fastReCoV, a less robust but more tractable and efficient variant of ReCoV suitable for deep learning applications. Through extensive experiments, we show that ReCoV and fastReCoV achieve state-of-the-art label noise detection performance in a wide range of modalities, models and tasks, including survival analysis, which has yet to be addressed in the literature. Validation metrics are key for the reliable tracking of scientific progress and for bridging the current chasm between artificial intelligence (AI) research and its translation into practice. However, increasing evidence shows that particularly in image analysis, metrics are often chosen inadequately in relation to the underlying research problem. This could be attributed to a lack of accessibility of metric-related knowledge: While taking into account the individual strengths, weaknesses, and limitations of validation metrics is a critical prerequisite to making educated choices, the relevant knowledge is currently scattered and poorly accessible to individual researchers. Based on a multi-stage Delphi process conducted by a multidisciplinary expert consortium as well as extensive community feedback, the present work provides the first reliable and comprehensive common point of access to information on pitfalls related to validation metrics in image analysis. Focusing on biomedical image analysis but with the potential of transfer to other fields, the addressed pitfalls generalize across application domains and are categorized according to a newly created, domain-agnostic taxonomy. To facilitate comprehension, illustrations and specific examples accompany each pitfall. Increasing evidence shows that flaws in machine learning (ML) algorithm validation are an underestimated global problem. Particularly in automatic biomedical image analysis, chosen performance metrics often do not reflect the domain interest, thus failing to adequately measure scientific progress and hindering translation of ML techniques into practice. To overcome this, our large international expert consortium created Metrics Reloaded, a comprehensive framework guiding researchers in the problem-aware selection of metrics. Following the convergence of ML methodology across application domains, Metrics Reloaded fosters the convergence of validation methodology. The framework was developed in a multi-stage Delphi process and is based on the novel concept of a problem fingerprint - a structured representation of the given problem that captures all aspects that are relevant for metric selection, from the domain interest to the properties of the target structure(s), data set and algorithm output. Based on the problem fingerprint, users are guided through the process of choosing and applying appropriate validation metrics while being made aware of potential pitfalls. Metrics Reloaded targets image analysis problems that can be interpreted as a classification task at image, object or pixel level, namely image-level classification, object detection, semantic segmentation, and instance segmentation tasks. To improve the user experience, we implemented the framework in the Metrics Reloaded online tool, which also provides a point of access to explore weaknesses, strengths and specific recommendations for the most common validation metrics. Deep artificial neural networks (DNNs) have moved to the forefront of medical image analysis due to their success in classification, segmentation, and detection challenges. A principal challenge in large-scale deployment of DNNs in neuroimage analysis is the potential for shifts in signal-to-noise ratio, contrast, resolution, and presence of artifacts from site to site due to variances in scanners and acquisition protocols. DNNs are famously susceptible to these distribution shifts in computer vision. Currently, there are no benchmarking platforms or frameworks to assess the robustness of new and existing models to specific distribution shifts in MRI, and accessible multi-site benchmarking datasets are still scarce or task-specific. To address these limitations, we propose ROOD-MRI: a platform for benchmarking the Robustness of DNNs to Out-Of-Distribution (OOD) data, corruptions, and artifacts in MRI. The platform provides modules for generating benchmarking datasets using transforms that model distribution shifts in MRI, implementations of newly derived benchmarking metrics for image segmentation, and examples for using the methodology with new models and tasks. We apply our methodology to hippocampus, ventricle, and white matter hyperintensity segmentation in several large studies, providing the hippocampus dataset as a publicly available benchmark. By evaluating modern DNNs on these datasets, we demonstrate that they are highly susceptible to distribution shifts and corruptions in MRI. We show that while data augmentation strategies can substantially improve robustness to OOD data for anatomical segmentation tasks, modern DNNs using augmentation still lack robustness in more challenging lesion-based segmentation tasks. Cancer stage is a large determinant of patient prognosis and management in many cancer types, and is often assessed using medical imaging modalities, such as CT and MRI. These medical images contain rich information that can be explored to stratify patients within each stage group to further improve prognostic algorithms. Although the majority of cancer deaths result from metastatic and multifocal disease, building imaging biomarkers for patients with multiple tumors has been a challenging task due to the lack of annotated datasets and standard study framework. In this paper, we process two public datasets to set up a benchmark cohort of 341 patient in total for studying outcome prediction of multifocal metastatic cancer. We identify the lack of expressiveness in common multiple instance classification networks and propose two injective multiple instance pooling functions that are better suited to outcome prediction. Our results show that multiple instance learning with injective pooling functions can achieve state-of-the-art performance in the non-small-cell lung cancer CT and head and neck CT outcome prediction benchmarking tasks. We will release the processed multifocal datasets, our code and the intermediate files i.e. extracted radiomic features to support further transparent and reproducible research.

Radiology reports are one of the main forms of communication between radiologists and other clinicians and contain important information for patient care. In order to use this information for research and automated patient care programs, it is necessary to convert the raw text into structured data suitable for analysis. State-of-the-art natural language processing (NLP) domain-specific contextual word embeddings have been shown to achieve impressive accuracy for these tasks in medicine, but have yet to be utilized for section structure segmentation. In this work, we pre-trained a contextual embedding BERT model using breast radiology reports and developed a classifier that incorporated the embedding with auxiliary global textual features in order to perform section segmentation. This model achieved a 98% accuracy at segregating free text reports sentence by sentence into sections of information outlined in the Breast Imaging Reporting and Data System (BI-RADS) lexicon, a significant improvement over the Classic BERT model without auxiliary information. We then evaluated whether using section segmentation improved the downstream extraction of clinically relevant information such as modality/procedure, previous cancer, menopausal status, the purpose of the exam, breast density, and breast MRI background parenchymal enhancement. Using the BERT model pre-trained on breast radiology reports combined with section segmentation resulted in an overall accuracy of 95.9% in the field extraction tasks. This is a 17% improvement compared to an overall accuracy of 78.9% for field extraction with models using Classic BERT embeddings and not using section segmentation. Our work shows the strength of using BERT in radiology report analysis and the advantages of section segmentation in identifying key features of patient factors recorded in breast radiology reports.

We investigate the utility of pretraining by contrastive self supervised learning on both natural-scene and medical imaging datasets when the unlabeled dataset size is small, or when the diversity within the unlabeled set does not lead to better representations. We use a two step approach which is analogous to supervised training with ImageNet initialization, where we pretrain networks that are already pretrained on ImageNet dataset to improve downstream task performance on the domain of interest. To improve the speed of convergence and the overall performance, we propose weight scaling and filter selection methods prior to second step of pretraining. We demonstrate the utility of this approach on three popular contrastive techniques, namely SimCLR, SWaV and BYOL. Benefits of double pretraining include better performance, faster convergence, ability to train with smaller batch sizes and smaller image dimensions with negligible differences in performance. We hope our work helps democratize self-supervision by enabling researchers to fine-tune models without requiring large clusters or long training times.

Self-supervised learning (SSL) has recently shown tremendous potential to learn generic visual representations useful for many image analysis tasks. Despite their notable success, the existing SSL methods fail to generalize to downstream tasks when the number of labeled training instances is small or if the domain shift between the transfer domains is significant. In this paper, we attempt to improve self-supervised pretrained representations through the lens of curriculum learning by proposing a hardness-aware dynamic curriculum learning (HaDCL) approach. To improve the robustness and generalizability of SSL, we dynamically leverage progressive harder examples via easy-to-hard and hard-to-very-hard samples during mini-batch downstream fine-tuning. We discover that by progressive stage-wise curriculum learning, the pretrained representations are significantly enhanced and adaptable to both in-domain and out-of-domain distribution data. We performed extensive validation on three histology benchmark datasets on both patch-wise and slide-level classification problems. Our curriculum based fine-tuning yields a significant improvement over standard fine-tuning, with a minimum improvement in area-under-the-curve (AUC) score of 1.7% and 2.2% on in-domain and out-of-domain distribution data, respectively. Further, we empirically show that our approach is more generic and adaptable to any SSL methods and does not impose any additional overhead complexity. Besides, we also outline the role of patch-based versus slide-based curriculum learning in histopathology to provide practical insights into the success of curriculum based fine-tuning of SSL methods. While the importance of automatic image analysis is continuously increasing, recent meta-research revealed major flaws with respect to algorithm validation. Performance metrics are particularly key for meaningful, objective, and transparent performance assessment and validation of the used automatic algorithms, but relatively little attention has been given to the practical pitfalls when using specific metrics for a given image analysis task. These are typically related to (1) the disregard of inherent metric properties, such as the behaviour in the presence of class imbalance or small target structures, (2) the disregard of inherent data set properties, such as the non-independence of the test cases, and (3) the disregard of the actual biomedical domain interest that the metrics should reflect. This living dynamically document has the purpose to illustrate important limitations of performance metrics commonly applied in the field of image analysis. In this context, it focuses on biomedical image analysis problems that can be phrased as image-level classification, semantic segmentation, instance segmentation, or object detection task. This article introduces a design of a Low Noise Amplifier (LNA), for the field of diamond particle detectors. This amplifier is described from simulation to measurements, which include pulses from {\alpha} particles detection. In hadron therapy, with high-frequency pulsed particle beams, the diamond detector is a promising candidate for beam monitoring and time-stamping, with prerequisite of fast electronics. The LNA is designed with surface mounted components and RF layout techniques to control costs and to allow timing performance suitable for sub-nanosecond edges of pulses. Also this amplifier offers the possibility of high voltage biasing, a characteristic essential for driving diamond detectors. Finally the greatest asset of this study is certainly the minimization of the power consumption, which allows us to consider designs with multiple amplifiers, in limited space, for striped diamond detectors., Comment: 8 pages, 13 figures, 1 tables. Training a neural network with a large labeled dataset is still a dominant paradigm in computational histopathology. However, obtaining such exhaustive manual annotations is often expensive, laborious, and prone to inter and Intra-observer variability. While recent self-supervised and semi-supervised methods can alleviate this need by learn-ing unsupervised feature representations, they still struggle to generalize well to downstream tasks when the number of labeled instances is small. In this work, we overcome this challenge by leveraging both task-agnostic and task-specific unlabeled data based on two novel strategies: i) a self-supervised pretext task that harnesses the underlying multi-resolution contextual cues in histology whole-slide images to learn a powerful supervisory signal for unsupervised representation learning; ii) a new teacher-student semi-supervised consistency paradigm that learns to effectively transfer the pretrained representations to downstream tasks based on prediction consistency with the task-specific un-labeled data. We carry out extensive validation experiments on three histopathology benchmark datasets across two classification and one regression-based tasks, i.e., tumor metastasis detection, tissue type classification, and tumor cellularity quantification. Under limited-label data, the proposed method yields tangible improvements, which is close or even outperforming other state-of-the-art self-supervised and supervised baselines. Furthermore, we empirically show that the idea of bootstrapping the self-supervised pretrained features is an effective way to improve the task-specific semi-supervised learning on standard benchmarks. A beam tagging hodoscope prototype made of squared 1 mm$^2$ fibers arranged in two perpendicular planes and coupled to multi-anode photomultipliers has been studied using 65 MeV proton as well as 95 MeV/u $^{12}$C beams at various intensities. This hodoscope successfully provided 2D images of proton beams with a detection efficiency larger than 98 % with logical OR condition between the two fiber planes. The detection efficiency with a coincidence between the two planes is close to 75 % for beam intensities up to $\sim1$ MHz. Moreover, the timing resolution is around 1.8 ns FWHM. Colorectal cancer is one of the most common and lethal cancers and colorectal cancer liver metastases (CRLM) is the major cause of death in patients with colorectal cancer. Multifocality occurs frequently in CRLM, but is relatively unexplored in CRLM outcome prediction. Most existing clinical and imaging biomarkers do not take the imaging features of all multifocal lesions into account. In this paper, we present an end-to-end autoencoder-based multiple instance neural network (AMINN) for the prediction of survival outcomes in multifocal CRLM patients using radiomic features extracted from contrast-enhanced MRIs. Specifically, we jointly train an autoencoder to reconstruct input features and a multiple instance network to make predictions by aggregating information from all tumour lesions of a patient. Also, we incorporate a two-step normalization technique to improve the training of deep neural networks, built on the observation that the distributions of radiomic features are almost always severely skewed. Experimental results empirically validated our hypothesis that incorporating imaging features of all lesions improves outcome prediction for multifocal cancer. The proposed AMINN framework achieved an area under the ROC curve (AUC) of 0.70, which is 11.4% higher than the best baseline method. A risk score based on the outputs of AMINN achieved superior prediction in our multifocal CRLM cohort. The effectiveness of incorporating all lesions and applying two-step normalization is demonstrated by a series of ablation studies. Whole slide images (WSIs) pose unique challenges when training deep learning models. They are very large which makes it necessary to break each image down into smaller patches for analysis, image features have to be extracted at multiple scales in order to capture both detail and context, and extreme class imbalances may exist. Significant progress has been made in the analysis of these images, thanks largely due to the availability of public annotated datasets. We postulate, however, that even if a method scores well on a challenge task, this success may not translate to good performance in a more clinically relevant workflow. Many datasets consist of image patches which may suffer from data curation bias; other datasets are only labelled at the whole slide level and the lack of annotations across an image may mask erroneous local predictions so long as the final decision is correct. In this paper, we outline the differences between patch or slide-level classification versus methods that need to localize or segment cancer accurately across the whole slide, and we experimentally verify that best practices differ in both cases. We apply a binary cancer detection network on post neoadjuvant therapy breast cancer WSIs to find the tumor bed outlining the extent of cancer, a task which requires sensitivity and precision across the whole slide. We extensively study multiple design choices and their effects on the outcome, including architectures and augmentations. Furthermore, we propose a negative data sampling strategy, which drastically reduces the false positive rate (7% on slide level) and improves each metric pertinent to our problem, with a 15% reduction in the error of tumor extent.

Unsupervised learning has been a long-standing goal of machine learning and is especially important for medical image analysis, where the learning can compensate for the scarcity of labeled datasets. A promising subclass of unsupervised learning is self-supervised learning, which aims to learn salient features using the raw input as the learning signal. In this paper, we use a contrastive self-supervised learning method called SimCLR that achieved state-of-the-art results on natural-scene images and apply this method to digital histopathology by collecting and pretraining on 57 histopathology datasets without any labels. We find that combining multiple multi-organ datasets with different types of staining and resolution properties improves the quality of the learned features. Furthermore, we find using more images for pretraining leads to a better performance in multiple downstream tasks. Linear classifiers trained on top of the learned features show that networks pretrained on digital histopathology datasets perform better than ImageNet pretrained networks, boosting task performances by more than 28% in F1 scores on average. These findings may also be useful when applying newer contrastive techniques to histopathology data. In order to fully exploit the ballistic potential of particle therapy, we propose an online range monitoring concept based on high-resolution Time-Of-Flight (TOF)-resolved Prompt Gamma (PG) detection in a single proton counting regime. In a proof of principle experiment, different types of monolithic scintillating gamma detectors are read in time coincidence with a diamond-based beam hodoscope, in order to build TOF spectra of PG generated in a heterogeneous target presenting an air cavity of variable thickness. Since the measurement was carried out at low beam currents ($<$ 1 proton/bunch) it was possible to reach excellent coincidence time resolutions, of the order of 100 ps ($\sigma$). Our goal is to detect possible deviations of the proton range with respect to treatment planning within a few intense irradiation spots at the beginning of the session and then carry on the treatment at standard beam currents. The measurements were limited to 10 mm proton range shift. A Monte Carlo simulation study reproducing the experiment has shown that a 3 mm shift can be detected at 2$\sigma$ by a single detector of $\sim 1.4 \times 10^{-3}$ absolute detection efficiency within a single irradiation spot ($\sim$10$^{8}$ protons) and an optimised experimental set-up., Comment: 17 pages, 18 figures

Two of the most common tasks in medical imaging are classification and segmentation. Either task requires labeled data annotated by experts, which is scarce and expensive to collect. Annotating data for segmentation is generally considered to be more laborious as the annotator has to draw around the boundaries of regions of interest, as opposed to assigning image patches a class label. Furthermore, in tasks such as breast cancer histopathology, any realistic clinical application often includes working with whole slide images, whereas most publicly available training data are in the form of image patches, which are given a class label. We propose an architecture that can alleviate the requirements for segmentation-level ground truth by making use of image-level labels to reduce the amount of time spent on data curation. In addition, this architecture can help unlock the potential of previously acquired image-level datasets on segmentation tasks by annotating a small number of regions of interest. Histopathological images contain rich phenotypic information that can be used to monitor underlying mechanisms contributing to diseases progression and patient survival outcomes. Recently, deep learning has become the mainstream methodological choice for analyzing and interpreting cancer histology images. In this paper, we present a comprehensive review of state-of-the-art deep learning approaches that have been used in the context of histopathological image analysis. From the survey of over 130 papers, we review the fields progress based on the methodological aspect of different machine learning strategies such as supervised, weakly supervised, unsupervised, transfer learning and various other sub-variants of these methods. We also provide an overview of deep learning based survival models that are applicable for disease-specific prognosis tasks. Diamond is a material in use at many nuclear and high energy facilities due to its inherent radiation tolerance and ease of use. We have characterized detectors based on chemical vapor deposition (CVD) diamond before and after proton irradiation. We present preliminary results of the spatial resolution of unirradiated and irradiated CVD diamond strip sensors. The number of biomedical image analysis challenges organized per year is steadily increasing. These international competitions have the purpose of benchmarking algorithms on common data sets, typically to identify the best method for a given problem. Recent research, however, revealed that common practice related to challenge reporting does not allow for adequate interpretation and reproducibility of results. To address the discrepancy between the impact of challenges and the quality (control), the Biomedical I mage Analysis ChallengeS (BIAS) initiative developed a set of recommendations for the reporting of challenges. The BIAS statement aims to improve the transparency of the reporting of a biomedical image analysis challenge regardless of field of application, image modality or task category assessed. This article describes how the BIAS statement was developed and presents a checklist which authors of biomedical image analysis challenges are encouraged to include in their submission when giving a paper on a challenge into review. Quantitative medical image computing (radiomics) has been widely applied to build prediction models from medical images. However, overfitting is a significant issue in conventional radiomics, where a large number of radiomic features are directly used to train and test models that predict genotypes or clinical outcomes. In order to tackle this problem, we propose an unsupervised learning pipeline composed of an autoencoder for representation learning of radiomic features and a Gaussian mixture model based on minimum message length criterion for clustering. By incorporating probabilistic modeling, disease heterogeneity has been taken into account. The performance of the proposed pipeline was evaluated on an institutional MRI cohort of 108 patients with colorectal cancer liver metastases. Our approach is capable of automatically selecting the optimal number of clusters and assigns patients into clusters (imaging subtypes) with significantly different survival rates. The segmentation of the breast from the chest wall is an important first step in the analysis of breast magnetic resonance images. 3D U-nets have been shown to obtain high segmentation accuracy and appear to generalize well when trained on one scanner type and tested on another scanner, provided that a very similar T1-weighted MR protocol is used. There has, however, been little work addressing the problem of domain adaptation when image intensities or patient orientation differ markedly between the training set and an unseen test set. To overcome the domain shift we propose to apply extensive intensity augmentation in addition to geometric augmentation during training. We explored both style transfer and a novel intensity remapping approach as intensity augmentation strategies. For our experiments, we trained a 3D U-net on T1-weighted scans and tested on T2-weighted scans. By applying intensity augmentation we increased segmentation performance from a DSC of 0.71 to 0.90. This performance is very close to the baseline performance of training and testing on T2-weighted scans (0.92). Furthermore, we applied our network to an independent test set made up of publicly available scans acquired using a T1-weighted TWIST sequence and a different coil configuration. On this dataset we obtained a performance of 0.89, close to the inter-observer variability of the ground truth segmentations (0.92). To alleviate the burden of gathering detailed expert annotations when training deep neural networks, we propose a weakly supervised learning approach to recognize metastases in microscopic images of breast lymph nodes. We describe an alternative training loss which clusters weakly labeled bags in latent space to inform relevance of patch-instances during training of a convolutional neural network. We evaluate our method on the Camelyon dataset which contains high-resolution digital slides of breast lymph nodes, where labels are provided at the image-level and only subsets of patches are made available during training., Comment: Machine Learning for Health (ML4H) Workshop at NeurIPS 2018
