1. SUHMO: an adaptive mesh refinement SUbglacial Hydrology MOdel v1.0

Abstract

Water flowing under ice sheets and glaciers can have a strong influence on ice dynamics, particularly through pressure changes, suggesting that a comprehensive ice sheet model should include the effect of basal hydrology. Modeling subglacial hydrology remains a challenge, however, mainly due to the range of spatial and temporal scales involved - from subglacial channels to vast subglacial lakes. Additionally, networks of subglacial drainage channels dynamically evolve over time. To address some of these challenges, we have developed an adaptive mesh refinement (AMR) model based on the Chombo software framework. We extend the model proposed by with a small but significant change to accommodate the transition from unresolved to resolved flow features. We handle the strong nonlinearities present in the equations by resorting to an efficient nonlinear full approximation scheme multigrid (FAS-MG) algorithm. We outline the details of the algorithm and present convergence analysis results demonstrating its good performance. Additionally, we present results validating our approach, using test cases from the Subglacial Hydrology Model Intercomparison Project (SHMIP) . We finish by presenting a more complex, 100 km-by-100 km synthetic test case with peaks and valleys that we use to investigate the effective pressure distribution as the number of AMR levels increases. These preliminary results suggest that a minimum spatial resolution is needed to properly capture channel features, but additional work is required to precisely quantify this and its impact on accurately modeling the coupled ice sheet-hydrology system. The efficiency of our approach, relying on localized refinement, is also demonstrated. Future work will include coupling the SUbglacial Hydrology MOdel (SUHMO) with the BISICLES AMR ice sheet model , both built on the same numerical framework.

2. Simulating ice-shelf extent using damage mechanics

Abstract

Inaccurate representations of iceberg calving from ice shelves are a large source of uncertainty in mass-loss projections from the Antarctic ice sheet. Here, we address this limitation by implementing and testing a continuum damage-mechanics model in a continental scale ice-sheet model. The damage-mechanics formulation, based on a linear stability analysis and subsequent long-wavelength approximation of crevasses that evolve in a viscous medium, links damage evolution to climate forcing and the large-scale stresses within an ice shelf. We incorporate this model into the BISICLES ice-sheet model and test it by applying it to idealized (1) ice tongues, for which we present analytical solutions and (2) buttressed ice-shelf geometries. Our simulations show that the model reproduces the large disparity in lengths of ice shelves with geometries and melt rates broadly similar to those of four Antarctic ice shelves: Erebus Glacier Tongue (length ∼ 13 km), the unembayed portion of Drygalski Ice Tongue (∼ 65 km), the Amery Ice Shelf (∼ 350 km) and the Ross Ice Shelf (∼ 500 km). These results demonstrate that our simple continuum model holds promise for constraining realistic ice-shelf extents in large-scale ice-sheet models in a computationally tractable manner.

Abstract

PurposeTo determine if treatment with a photobiomodulation (PBM) device results in greater improvement in central subfield thickness (CST) than placebo in eyes with center-involved diabetic macular edema (CI-DME) and good vision.DesignPhase 2 randomized clinical trial.ParticipantsParticipants had CI-DME and visual acuity (VA) 20/25 or better in the study eye and were recruited from 23 clinical sites in the United States.MethodsOne eye of each participant was randomly assigned 1:1 to a 670-nm light-emitting PBM eye patch or an identical device emitting broad-spectrum white light at low power. Treatment was applied for 90 seconds twice daily for 4 months.Main outcome measuresChange in CST on spectral-domain OCT at 4 months.ResultsFrom April 2019 to February 2020, 135 adults were randomly assigned to either PBM (n = 69) or placebo (n = 66); median age was 62 years, 37% were women, and 82% were White. The median device compliance was 92% with PBM and 95% with placebo. OCT CST increased from baseline to 4 months by a mean (SD) of 13 (53) μm in PBM eyes and 15 (57) μm in placebo eyes, with the mean difference (95% confidence interval [CI]) being -2 (-20 to 16) μm (P = 0.84). CI-DME, based on DRCR Retina Network sex- and machine-based thresholds, was present in 61 (90%) PBM eyes and 57 (86%) placebo eyes at 4 months (adjusted odds ratio [95% CI] = 1.30 (0.44-3.83); P = 0.63). VA decreased by a mean (SD) of -0.2 (5.5) letters and -0.6 (4.6) letters in the PBM and placebo groups, respectively (difference [95% CI] = 0.4 (-1.3 to 2.0) letters; P = 0.64). There were 8 adverse events possibly related to the PBM device and 2 adverse events possibly related to the placebo device. None were serious.ConclusionsPBM as given in this study, although safe and well-tolerated, was not found to be effective for the treatment of CI-DME in eyes with good vision.

7. A survey of software implementations used by application codes in the Exascale Computing Project

Abstract

The US Department of Energy Office of Science and the National Nuclear Security Administration initiated the Exascale Computing Project (ECP) in 2016 to prepare mission-relevant applications and scientific software for the delivery of the exascale computers starting in 2023. The ECP currently supports 24 efforts directed at specific applications and six supporting co-design projects. These 24 application projects contain 62 application codes that are implemented in three high-level languages—C, C++, and Fortran—and use 22 combinations of graphical processing unit programming models. The most common implementation language is C++, which is used in 53 different application codes. The most common programming models across ECP applications are CUDA and Kokkos, which are employed in 15 and 14 applications, respectively. This article provides a survey of the programming languages and models used in the ECP applications codebase that will be used to achieve performance on the future exascale hardware platforms.

Abstract

The land ice contribution to global mean sea level rise has not yet been predicted1 using ice sheet and glacier models for the latest set of socio-economic scenarios, nor using coordinated exploration of uncertainties arising from the various computer models involved. Two recent international projects generated a large suite of projections using multiple models2-8, but primarily used previous-generation scenarios9 and climate models10, and could not fully explore known uncertainties. Here we estimate probability distributions for these projections under the new scenarios11,12 using statistical emulation of the ice sheet and glacier models. We find that limiting global warming to 1.5 degrees Celsius would halve the land ice contribution to twenty-first-century sea level rise, relative to current emissions pledges. The median decreases from 25 to 13 centimetres sea level equivalent (SLE) by 2100, with glaciers responsible for half the sea level contribution. The projected Antarctic contribution does not show a clear response to the emissions scenario, owing to uncertainties in the competing processes of increasing ice loss and snowfall accumulation in a warming climate. However, under risk-averse (pessimistic) assumptions, Antarctic ice loss could be five times higher, increasing the median land ice contribution to 42 centimetres SLE under current policies and pledges, with the 95th percentile projection exceeding half a metre even under 1.5 degrees Celsius warming. This would severely limit the possibility of mitigating future coastal flooding. Given this large range (between 13 centimetres SLE using the main projections under 1.5 degrees Celsius warming and 42 centimetres SLE using risk-averse projections under current pledges), adaptation planning for twenty-first-century sea level rise must account for a factor-of-three uncertainty in the land ice contribution until climate policies and the Antarctic response are further constrained.

10. Modelling binary alloy solidification with adaptive mesh refinement

Abstract

The solidification of a binary alloy results in the formation of a porous mushy layer, within which spontaneous localisation of fluid flow can lead to the emergence of features over a range of spatial scales. We describe a finite volume method for simulating binary alloy solidification in two dimensions with local mesh refinement in space and time. The coupled heat, solute, and mass transport is described using an enthalpy method with flow described by a Darcy-Brinkman equation for flow across porous and liquid regions. The resulting equations are solved on a hierarchy of block-structured adaptive grids. A projection method is used to compute the fluid velocity, whilst the viscous and nonlinear diffusive terms are calculated using a semi-implicit scheme. A series of synchronization steps ensure that the scheme is flux-conservative and correct for errors that arise at the boundaries between different levels of refinement. We also develop a corresponding method using Darcy's law for flow in a porous medium/narrow Hele-Shaw cell. We demonstrate the accuracy and efficiency of our method using established benchmarks for solidification without flow and convection in a fixed porous medium, along with convergence tests for the fully coupled code. Finally, we demonstrate the ability of our method to simulate transient mushy layer growth with narrow liquid channels which evolve over time.

Abstract

Structured-grid adaptive mesh refinement (SAMR) is an approach to mesh generation that supports structured access to data and adaptive mesh refinement for discretized partial differential equations (PDEs). Solution algorithms often require that an inverse of an operator be applied, a system of algebraic equations must be solved, and this process is often the primary computational cost in an application. SAMR is well suited to geometric multigrid solvers, which can be effective, but often do not adapt well to complex geometry including material coefficients. Algebraic multigrid (AMG) is more robust in the face of complex geometry, in both boundary conditions and internal material interfaces. AMG requires a stored matrix linearization of the operator. We discuss an approach, and an implementation in the Chombo block-structured AMR framework, for constructing composite grid matrices from a SAMR hierarchy of grids for use in linear solvers in the PETSc numerical library. We consider a case study with the Chombo-based BISICLES ice sheet modeling application.

12. Millennial‐Scale Vulnerability of the Antarctic Ice Sheet to Regional Ice Shelf Collapse

The Antarctic Ice Sheet (AIS) remains the largest uncertainty in projections of future sea level rise. A likely climate-driven vulnerability of the AIS is thinning of floating ice shelves resulting from surface-melt-driven hydrofracture or incursion of relatively warm water into subshelf ocean cavities. The resulting melting, weakening, and potential ice shelf collapse reduces shelf buttressing effects. Upstream ice flow accelerates, causing thinning, grounding-line retreat, and potential ice sheet collapse. While high-resolution projections have been performed for localized Antarctic regions, full-continent simulations have typically been limited to low-resolution models. Here we quantify the vulnerability of the entire present-day AIS to regional ice shelf collapse on millennial timescales treating relevant ice flow dynamics at the necessary ∼1-km resolution. Collapse of any of the ice shelves dynamically connected to the West Antarctic Ice Sheet (WAIS) is sufficient to trigger ice sheet collapse in marine-grounded portions of the WAIS. Vulnerability elsewhere appears limited to localized responses.

16. Advances in Cross-Cutting Ideas for Computational Climate Science

Abstract

This report presents results from the DOE-sponsored workshop titled, Advancing X-Cutting Ideas for Computational Climate Science Workshop,'' known as AXICCS, held on September 12--13, 2016 in Rockville, MD. The workshop brought together experts in climate science, computational climate science, computer science, and mathematics to discuss interesting but unsolved science questions regarding climate modeling and simulation, promoted collaboration among the diverse scientists in attendance, and brainstormed about possible tools and capabilities that could be developed to help address them. Emerged from discussions at the workshop were several research opportunities that the group felt could advance climate science significantly. These include (1) process-resolving models to provide insight into important processes and features of interest and inform the development of advanced physical parameterizations, (2) a community effort to develop and provide integrated model credibility, (3) including, organizing, and managing increasingly connected model components that increase model fidelity yet complexity, and (4) treating Earth system models as one interconnected organism without numerical or data based boundaries that limit interactions. The group also identified several cross-cutting advances in mathematics, computer science, and computational science that would be needed to enable one or more of these big ideas. It is critical to address the need for organized, verified, and optimized software, which enables the models to grow and continue to provide solutions in which the community can have confidence. Effectively utilizing the newest computer hardware enables simulation efficiency and the ability to handle output from increasingly complex and detailed models. This will be accomplished through hierarchical multiscale algorithms in tandem with new strategies for data handling, analysis, and storage. These big ideas and cross-cutting technologies for enabling breakthrough climate simulation advancements also need the "glue" of outreach and learning across the scientific domains to be successful. The workshop identified several strategies to allow productive, continuous engagement across those who have a broad knowledge of the various angles of the problem. Specific ideas to foster education and tools to make material progress were discussed. Examples include follow-on cross-cutting meetings that enable unstructured discussions of the types this workshop fostered. A concerted effort to recruit undergraduate and graduate students from all relevant domains and provide them experience, training, and networking across their immediate expertise is needed. This will broaden and expand their exposure to the future needs and solutions, and provide a pipeline of scientists with a diversity of knowledge and know-how. Providing real-world experience with subject matter experts from multiple angles may also motivate the students to attack these problems and even come up with the missing solutions.

Abstract

PurposeTo describe fluocinolone acetonide implant dissociations in the Multicenter Uveitis Steroid Treatment (MUST) Trial.DesignRandomized clinical trial with extended follow-up.MethodsReview of data collected on the first implant in the eye(s) of participants. Dissociation was defined as the drug pellet no longer being affixed to the strut and categorized as spontaneous or surgically related.ResultsA total of 250 eyes (146 patients) had at least 1 implant placed. Median follow-up time after implant placement was 6 years (range 0.5-9.2 years). Thirty-four dissociations were reported in 30 participants. There were 22 spontaneous events in 22 participants; 6-year cumulative risk of a spontaneous dissociation was 4.8% (95% confidence interval [CI]: 2.4%-9.1%). The earliest event occurred 4.8 years after placement. Nine of 22 eyes with data had a decline in visual acuity ≥5 letters temporally related to the dissociation. Thirty-nine implant removal surgeries were performed, 33 with replacement. Twelve dissociations were noted during implant removal surgeries in 10 participants (26%, 95% CI 15%-48%); 5 of these eyes had a decline in visual acuity ≥5 letters after surgery. The time from implant placement to removal surgery was longer for the surgeries at which dissociated implants were identified than for those without one (5.7 vs 3.7 years, P < .001). Overall, visual acuity declined 15 or more letters from pre-implant values in 22% of affected eyes; declines were frequently associated with complications of uveitis or its treatment.ConclusionThere is an increasing risk of dissociation of Retisert implants during follow-up; the risk is greater with removal/exchange surgeries, but the risk of both spontaneous and surgically related events increases with longevity of the implants. In 22% of affected eyes visual acuity declined by 15 letters. In the context of eyes with moderate to severe uveitis for years, this rate is not unexpected.

Abstract

Coupled ice sheet-ocean models capable of simulating moving grounding lines are just becoming available. Such models have a broad range of potential applications in studying the dynamics of marine ice sheets and tidewater glaciers, from process studies to future projections of ice mass loss and sea level rise. The Marine Ice Sheet-Ocean Model Intercomparison Project (MISOMIP) is a community effort aimed at designing and coordinating a series of model intercomparison projects (MIPs) for model evaluation in idealized setups, model verification based on observations, and future projections for key regions of the West Antarctic Ice Sheet (WAIS). Here we describe computational experiments constituting three interrelated MIPs for marine ice sheet models and regional ocean circulation models incorporating ice shelf cavities. These consist of ice sheet experiments under the Marine Ice Sheet MIP third phase (MISMIP+), ocean experiments under the Ice Shelf-Ocean MIP second phase (ISOMIP+) and coupled ice sheet-ocean experiments under the MISOMIP first phase (MISOMIP1). All three MIPs use a shared domain with idealized bedrock topography and forcing, allowing the coupled simulations (MISOMIP1) to be compared directly to the individual component simulations (MISMIP+ and ISOMIP+). The experiments, which have qualitative similarities to Pine Island Glacier Ice Shelf and the adjacent region of the Amundsen Sea, are designed to explore the effects of changes in ocean conditions, specifically the temperature at depth, on basal melting and ice dynamics. In future work, differences between model results will form the basis for the evaluation of the participating models.

Abstract

PurposeTo identify factors associated with best-corrected visual acuity (BCVA) presentation and 2-year outcome in 479 intermediate, posterior, and panuveitic eyes.DesignCohort study using randomized controlled trial data.MethodsMulticenter Uveitis Steroid Treatment (MUST) Trial masked BCVA measurements at baseline and at 2 years follow-up used gold-standard methods. Twenty-three clinical centers documented characteristics per protocol, which were evaluated as potential predictive factors for baseline BCVA and 2-year change in BCVA.ResultsBaseline factors significantly associated with reduced BCVA included age ≥50 vs 10 vs grade 0; cataract; macular thickening; and exudative retinal detachment. Over 2 years, eyes better than 20/50 and 20/50 or worse at baseline improved, on average, by 1 letter (P = .52) and 10 letters (P < .001), respectively. Both treatment groups and all sites of uveitis improved similarly. Factors associated with improved BCVA included resolution of active AC cells, resolution of macular thickening, and cataract surgery in an initially cataractous eye. Abstract

ObjectiveTo describe the incidence and outcomes of endophthalmitis after intravitreal injections of anti-vascular endothelial growth factor agents in the Comparison of Age-Related Macular Degeneration Treatments Trials (CATT) and to assess the effect of prophylactic topical antimicrobials on incidence.DesignCohort study within a randomized clinical trial.ParticipantsPatients enrolled in CATT.MethodsPatients with neovascular age-related macular degeneration received intravitreal injections of ranibizumab or bevacizumab under 1 of 3 dosing regimens. The study protocol specified preinjection preparation to include use of a sterile lid speculum and povidone iodine (5%). Use of preinjection and postinjection antibiotics was at the discretion of the treating ophthalmologist. Patients were followed up monthly for 2 years.Main outcome measuresDevelopment of endophthalmitis and visual acuity.ResultsEndophthalmitis developed after 11 of 18 509 injections (1 per 1700 [0.06%]; 95% confidence interval, 0.03%-0.11%), and in 11 of 1185 patients (0.93%; 95% confidence interval, 0.52-1.66). Incidence of endophthalmitis was 0.15% among injections with no antibiotic use, 0.08% among injections with preinjection antibiotics only, 0.06% among injections with postinjection antibiotics only, and 0.04% among injections with preinjection and postinjection antibiotics (P = 0.20). All eyes were treated with intravitreal antibiotics and 4 underwent vitrectomy. Among the 11 affected eyes, the final study visual acuity was 20/40 or better in 4 eyes (36%), 20/50 to 20/80 in 2 eyes (18%), 20/100 to 20/160 in 3 eyes (27%), and worse than 20/800 in 2 eyes (18%). The final visual acuity was within 2 lines of the visual acuity before endophthalmitis in 5 eyes (45%).ConclusionsRates of endophthalmitis were low and similar to those in other large-scale studies. Use of topical antibiotics either before or after injection does not seem to reduce the risk for endophthalmitis.

Abstract

Performing a stable, long duration simulation of driven MHD turbulence with a high thermal Mach number and a strong initial magnetic field is a challenge to high-order Godunov ideal MHD schemes because of the difficulty in guaranteeing positivity of the density and pressure. We have implemented a robust combination of reconstruction schemes, Riemann solvers, limiters, and Constrained Transport EMF averaging schemes that can meet this challenge, and using this strategy, we have developed a new Adaptive Mesh Refinement (AMR) MHD module of the ORION2 code. We investigate the effects of AMR on several statistical properties of a turbulent ideal MHD system with a thermal Mach number of 10 and a plasma $\beta_0$ of 0.1 as initial conditions; our code is shown to be stable for simulations with higher Mach numbers ($M_rms = 17.3$) and smaller plasma beta ($\beta_0 = 0.0067$) as well. Our results show that the quality of the turbulence simulation is generally related to the volume-averaged refinement. Abstract

We present the implementation of a three-dimensional, second order accurate Godunov-type algorithm for magneto-hydrodynamic (MHD), in the adaptive-mesh-refinement (AMR) cosmological code {\tt CHARM}. The algorithm is based on the full 12-solve spatially unsplit Corner-Transport-Upwind (CTU) scheme. The fluid quantities are cell-centered and are updated using the Piecewise-Parabolic-Method (PPM), while the magnetic field variables are face-centered and are evolved through application of the Stokes theorem on cell edges via a Constrained-Transport (CT) method. The multidimensional MHD source terms required in the predictor step for high-order accuracy are applied in a simplified form which reduces their complexity in three dimensions without loss of accuracy or robustness. The algorithm is implemented on an AMR framework which requires specific synchronization steps across refinement levels. These include face-centered restriction and prolongation operations and a {\it reflux-curl} operation, which maintains a solenoidal magnetic field across refinement boundaries. The code is tested against a large suite of test problems, including convergence tests in smooth flows, shock-tube tests, classical two- and three-dimensional MHD tests, a three-dimensional shock-cloud interaction problem and the formation of a cluster of galaxies in a fully cosmological context. 27. Numerical Implementation of Streaming Down the Gradient: Application to Fluid Modeling of Cosmic Rays and Saturated Conduction

Abstract

The equation governing the streaming of a quantity down its gradient superficially looks similar to the simple constant velocity advection equation. In fact, it is the same as an advection equation if there are no local extrema in the computational domain or at the boundary. However, in general when there are local extrema in the computational domain it is a non-trivial nonlinear equation. The standard upwind time evolution with a CFL-limited time step results in spurious oscillations at the grid scale. These oscillations, which originate at the extrema, propagate throughout the computational domain and are undamped even at late times. These oscillations arise because of unphysically large fluxes leaving (entering) the maxima (minima) with the standard CFL-limited explicit methods. Regularization of the equation shows that it is diffusive at the extrema; because of this, an explicit method for the regularized equation with $\Delta t \propto \Delta x^2$ behaves fine. We show that the implicit methods show stable and converging results with $\Delta t \propto \Delta x$; however, surprisingly, even implicit methods are not stable with large enough timesteps. In addition to these subtleties in the numerical implementation, the solutions to the streaming equation are quite novel: non-differentiable solutions emerge from initially smooth profiles; the solutions show transport over large length scales, e.g., in form of tails. The fluid model for cosmic rays interacting with a thermal plasma (valid at space scales much larger than the cosmic ray Larmor radius) is similar to the equation for streaming of a quantity down its gradient, so our method will find applications in fluid modeling of cosmic rays., Comment: accepted in SIAM J. of Scient. We present simulations following the ISMIP6 framework with the BISICLES ice sheet model, alongside new experiments extending the ISMIP6 protocol to more comprehensively explore uncertain ice sheet processes. These results contributed to Antarctic projections of Edwards et al. (2021), which formed the basis of sea level projections for the Sixth Assessment Report of the Intergovernmental Panel on Climate Change (AR6). The BISICLES experiments presented here show the important interplay between surface mass balance forcing and ocean driven melt, with high warming, high accumulation forcing conditions leading to mass gain (negative sea level contribution) under low sensitivity to ocean driven melt. Conversely, we show that when sensitivity to ocean warming is high, ocean melting drives increased mass loss despite high accumulation. Finally, we show that collapse of ice shelves due to surface warming increases sea level contribution by 25 mm for both moderate and high sensitivity of ice shelf melting to ocean forcing tested. We present a variation of an adaptive projection method for computing solutions to the incompressible Navier-Stokes equations with suspended particles. To compute the divergence-free component of the momentum forcing due to the particle drag, we employ an approach which exploits the locality and smoothness of the Laplacian of the projection operator applied to the discretized particle drag force. We present convergence and performance results to demonstrate the effectiveness of this approach.

38. The Antarctic Ice Sheet's response to idealised geoengineering scenarios

Abstract

Sea level will continue to rise beyond the 22nd century due to inertia in the climate system, even if temperatures were immediately stabilised, leading to the proposal of more dramatic measures, such as geoengineering. To provide initial sea level contribution (SLC) estimates from the AIS and address long-term commitment and reversibility questions, a set of idealized geoengineering scenarios were performed with the BISICLES ice sheet model. Climate forcings were extended beyond 2100 to 2200 by either fixing the climate at 2100 (2050) levels at the end (middle) of the century (commitment scenarios), or immediately returning to 2015 levels (e.g. via an instantaneous implementation of geoengineering). Results show that for both high (RCP8.5) and low (RCP2.6) forcing scenarios, reverting back to 2015 climate does not prevent significant loss from the AIS. However, if geoengineering methods are adopted in 2050, SLC is lower than in the commitment scenarios. If geoengineering is implemented in 2100, results are more uncertain but indicate a higher SLC over the 22nd century compared with doing nothing, for RCP8.5 scenarios. This is because increased surface mass balance in the RCP8.5 commitment scenarios offsets more of the dynamic losses than in the geoengineered scenarios. Abstract

Key Points: Question: In patients with nonproliferative diabetic retinopathy (NPDR) and good vision but without center-involved diabetic macular edema (CI-DME), does early aflibercept reduce disease progression and improve long-term visual acuity compared with initial observation and treatment only if disease worsens? Findings: This study presents 4-year primary outcomes of a randomized clinical trial that included 328 patients (399 eyes), randomized to 2.0 mg aflibercept injections or sham injections. Among those receiving aflibercept, proliferative diabetic retinopathy or CI-DME developed in 33.9% vs 56.9% among those who received sham—a difference that was statistically significant. Change in visual acuity was −2.7 vs −2.4 letters, a difference that was not statistically significant. Meaning: At 4 years, treatment of NPDR with aflibercept vs sham treatment resulted in statistically significant anatomic improvement, but no improvement in visual acuity. Importance: Anti–vascular endothelial growth factor (VEGF) injections in eyes with nonproliferative diabetic retinopathy (NPDR) without center-involved diabetic macular edema (CI-DME) reduce development of vision-threatening complications from diabetes over at least 2 years, but whether this treatment has a longer-term benefit on visual acuity is unknown. Objective: To compare the primary 4-year outcomes of visual acuity and rates of vision-threatening complications in eyes with moderate to severe NPDR treated with intravitreal aflibercept compared with sham. The primary 2-year analysis of this study has been reported. Design, Setting, and Participants: Randomized clinical trial conducted at 64 clinical sites in the US and Canada from January 2016 to March 2018, enrolling 328 adults (399 eyes) with moderate to severe NPDR (Early Treatment Diabetic Retinopathy Study [ETDRS] severity level 43-53; range, 0 [worst] to 100 [best]) without CI-DME. Interventions: Eyes were randomly assigned to 2.0 mg aflibercept (n = 200) or sham (n = 199). Eight injections were administered at defined intervals through 2 years, continuing quarterly through 4 years unless the eye improved to mild NPDR or better. Aflibercept was given in both groups to treat development of high-risk proliferative diabetic retinopathy (PDR) or CI-DME with vision loss. Main Outcomes and Measures: Development of PDR or CI-DME with vision loss (≥10 letters at 1 visit or ≥5 letters at 2 consecutive visits) and change in visual acuity (best corrected ETDRS letter score) from baseline to 4 years. Results: Among participants (mean age 56 years; 42.4% female; 5% Asian, 15% Black, 32% Hispanic, 45% White), the 4-year cumulative probability of developing PDR or CI-DME with vision loss was 33.9% with aflibercept vs 56.9% with sham (adjusted hazard ratio, 0.40 [97.5% CI, 0.28 to 0.57]; P <.001). The mean (SD) change in visual acuity from baseline to 4 years was −2.7 (6.5) letters with aflibercept and −2.4 (5.8) letters with sham (adjusted mean difference, −0.5 letters [97.5% CI, −2.3 to 1.3]; P =.52). Antiplatelet Trialists' Collaboration cardiovascular/cerebrovascular event rates were 9.9% (7 of 71) in bilateral participants, 10.9% (14 of 129) in unilateral aflibercept participants, and 7.8% (10 of 128) in unilateral sham participants. Conclusions and Relevance: Among patients with NPDR but without CI-DME at 4 years treatment with aflibercept vs sham, initiating aflibercept treatment only if vision-threatening complications developed, resulted in statistically significant anatomic improvement but no improvement in visual acuity. Aflibercept as a preventive strategy, as used in this trial, may not be generally warranted for patients with NPDR without CI-DME.
