Objectives: We aimed to assess the predictive value of the total metabolic tumor burden prior to treatment in patients with advanced non-small-cell lung cancer (NSCLC) receiving immune checkpoint inhibitors (ICIs). Methods: Pre-treatment 2-deoxy-2-[18F]fluoro-D-glucose positron emission tomography/computed tomography (PET/CT) scans performed in two consecutive years for staging in adult patients with confirmed NSCLC were considered. Volume, maximum/mean standardized uptake value (SUVmax/SUVmean), metabolic tumor volume (MTV) and total lesion glycolysis (TLG) were assessed per delineated malignant lesion (including primary tumor, regional lymph nodes and distant metastases) in addition to the morphology of the primary tumor and clinical data. Total metabolic tumor burden was captured by totalMTV and totalTLG. Overall survival (OS), progression-free survival (PFS) and clinical benefit (CB) were used as endpoints for response to treatment. Results: A total of 125 NSCLC patients were included. Osseous metastases were the most frequent distant metastases (n = 17), followed by thoracal distant metastases (pulmonal = 14 and pleural = 13). Total metabolic tumor burden prior to treatment was significantly higher in patients treated with ICIs (mean totalMTV ± standard deviation (SD) 72.2 ± 78.7; mean totalTLG ± SD 462.2 ± 538.9) compared to those without ICI treatment (mean totalMTV ± SD 58.1 ± 233.8; mean totalTLG ± SD 290.0 ± 784.2). Among the patients who received ICIs, a solid morphology of the primary tumor on imaging prior to treatment was the strongest outcome predictor for OS (Hazard ratio HR 28.04, p < 0.01), PFS (HR 30.89, p < 0.01) and CB (parameter estimation PE 3.46, p < 0.01), followed by the metabolic features of the primary tumor. Interestingly, total metabolic tumor burden prior to immunotherapy showed a negligible impact on OS (p = 0.04) and PFS (p = 0.01) after treatment given the hazard ratios of 1.00, but also on CB (p = 0.01) given the PE < 0.01. Overall, biomarkers on pre-treatment PET/CT scans showed greater predictive power in patients receiving ICIs, compared to patients without ICI treatment. Conclusions: Morphological and metabolic properties of the primary tumors prior to treatment in advanced NSCLC patients treated with ICI showed great outcome prediction performances, as opposed to the pre-treatment total metabolic tumor burdens, captured by totalMTV and totalTLG, both with negligible impact on OS, PFS and CB. However, the outcome prediction performance of the total metabolic tumor burden might be influenced by the value itself (e.g., poorer prediction performance at very high or very low values of total metabolic tumor burden). Further studies including subgroup analysis with regards to different values of total metabolic tumor burden and their respective outcome prediction performances might be needed.

We aimed to assess the frequency of additional primary malignancies detected incidentally on [18F]fluoro-D-glucose positron emission tomography/computed tomography (FDG-PET/CT) at staging in NSCLC patients. Moreover, their impact on patient management and survival was assessed. Consecutive NSCLC patients with available staging FDG-PET/CT between 2020 and 2021 were retrospectively enrolled. We reported whether further investigations of suspicious findings presumably not related to NSCLC were recommended and performed after FDG-PET/CT. Any additional imaging, surgery or multimodal management was considered as an impact on patient management. Patient survival was defined using overall survival OS and progression-free survival PFS. A total of 125 NSCLC patients were included, while 26 findings in 26 different patients were suspicious for an additional malignancy on FDG-PET/CT at staging. The most frequent anatomical site was the colon. A total of 54.2% of all additional suspicious lesions turned out to be malignant. Almost every malignant finding had an impact on patient management. No significant differences were found between NSCLC patients with suspicious findings versus no suspicious findings with regards to their survival. FDG-PET/CT performed for staging might be a valuable tool to identify additional primary tumors in NSCLC patients. Identification of additional primary tumors might have substantial implications for patient management. An early detection together with interdisciplinary patient management could prevent a worsening of survival compared to patients with NSCLC only.

Objectives: We aimed to investigate the predictive value of baseline 2-deoxy-2-[18F]fluoro-D-glucose positron emission tomography/computed tomography (FDG-PET/CT) for durable responses to immune checkpoint inhibitors (ICIs) by linking the morphological and metabolic features of primary tumors (PTs) in nonsmall cell lung cancer (NSCLC) patients. Methods: For the purpose of this single-center study, the imaging data of the patients with a first diagnosis of NSCLC and an available baseline FDG-PET/CT between 2020 and 2021 were retrospectively assessed. The baseline characteristics were collected based on clinical reports and interdisciplinary tumor board documentation. The metabolic (such as standardized uptake value SUV maximum and mean (SUVmax, SUV mean), metabolic tumor volume (MTV), total lesion glycolysis (TLG)) and morphological (such as volume, morphology, margin, and presence of lymphangiosis through imaging) features of all the PTs were retrospectively assessed using FDG-PET/CT. Overall survival (OS), progression-free survival (PFS), clinical benefit (CB) and mortality rate were used as endpoints to define the long-term response to therapy. A backward, stepwise logistic regression analysis was performed in order to define the best model for predicting lasting responses to treatment. Statistical significance was assumed at p < 0.05. Results: A total of 125 patients (median age ± standard deviation (SD) 72.0 ± 9.5 years) were enrolled: 64 men (51.2%) and 61 women (48.8%). Adenocarcinoma was by far the most common histological subtype of NSCLC (47.2%). At the initial diagnosis, the vast majority of all the included patients showed either locally advanced disease (34.4%) or metastatic disease (36.8%). Fifty patients were treated with ICIs either as a first-line (20%) or second-line (20%) therapy, while 75 patients did not receive ICIs. The median values ± SD of PT SUVmax, mean, MTV, and TLG were respectively 10.1 ± 6.0, 6.1 ± 3.5, 13.5 ± 30.7, and 71.4 ± 247.7. The median volume of PT ± SD was 13.7 ± 30.7 cm3. The PTs were most frequently solid (86.4%) with irregular margins (76.8%). Furthermore, in one out of five cases, the morphological evidence of lymphangiosis was seen through imaging (n = 25). The median follow-up ± SD was 18.93 ± 6.98 months. The median values ± SD of OS and PFS were, respectively, 14.80 ± 8.68 months and 14.03 ± 9.02 months. Age, PT volume, SUVmax, TLG, the presence of lymphangiosis features through imaging, and clinical stage IV were very strong long-term outcome predictors of patients treated with ICIs, while no significant outcome predictors could be found for the cohort with no ICI treatment. The optimal cut-off values were determined for PT volume (26.94 cm3) and SUVmax (15.05). Finally, 58% of NSCLC patients treated with ICIs had a CB vs. 78.7% of patients in the cohort with no ICI treatment. However, almost all patients treated with ICIs and with disease progression over time died (mortality in the case of disease progression 95% vs. 62.5% in the cohort without ICIs). Conclusion: Baseline FDG-PET/CT could be used to predict a durable response to ICIs in NSCLC patients. Age, clinical stage IV, lymphangiosis features through imaging, PT volume (thus PT MTV due to a previously demonstrated linear correlation), PT SUVmax, and TLG were very strong long-term outcome predictors. Our results highlight the importance of linking clinical data, as much as morphological features, to the metabolic parameters of primary tumors in a multivariate outcome-predicting model using baseline FDG-PET/CT.

Objective Duplex ultrasound (DUS) and computed tomography angiography (CTA) are both used as first-line noninvasive methods to investigate patients for internal carotid artery (ICA) disease. Although manual assessment of CTA is well established, semiautomated vessel analysis programs have yet to prove their clinical benefit. We compared one such vessel analysis program (TeraRecon, Foster City, Calif) with DUS. Methods A total of 85 arteries in 50 patients (35 men, 15 women; mean age, 73 ± 10 years) were eligible for comparison with the North American Symptomatic Carotid Endarterectomy Trial method. Duplex scanning comprised stenosis estimation based on (1) the intrastenotic and distal ICA diameter measurements on color-coded imaging (CCI), (2) the application of German Society for Ultrasound in Medicine (DEGUM) criteria (intrastenotic peak systolic velocity [PSV] ≥200 cm/s indicates 50% stenosis; intrastenotic PSV ≥300 cm/s together with a PSV of ≥50 cm/s in the distal ICA indicates 70% stenosis), and (3) the application of the University of Washington stenosis criteria (≥50% stenosis is indicated by PSV >125 cm/s and end-diastolic velocity 125 cm/s and end-diastolic velocity ≥140 cm/s). The minimum and maximum diameters at the stenosis site and at the distal reference ICA were automatically measured with the CTA vessel analysis tool. In addition, automated tracking generated corresponding cross-sectional areas at these two sites. Angiographic stenosis was then calculated using the minimum diameter (CTAmin), the average of the minimum and maximum diameters (CTAavg), and the areas (CTAarea) at both sites. Results Compared with duplex CCI, the three CTA modalities exhibited only a moderate agreement in terms of regression analysis (R 2 = 0.41-0.54) and Bland-Altman analysis (the standard deviation of the stenosis differences was >20%). In terms of sensitivity, specificity, positive predictive value, negative predictive value, and accuracy, DEGUM stenosis graduation was best balanced by duplex CCI (50% stenosis: 100%, 93%, 85%, 100%, 95%; 70% stenosis: 71%, 100%, 100%, 97%, 98%) followed by CTAarea (50% stenosis: 80%, 73%, 54%, 90%, 75%; 70% stenosis: 66%, 94%, 55%, 96%, 96%). University of Washington stenosis was best balanced by duplex CCI followed by CTAarea. Conclusions CTA analysis with a semiautomated vessel analysis tool provides variable results. Large discrepancies between methods in the degree of reported stenosis must be taken into consideration when CTA and DUS are used for clinical purposes. The semiautomated software tools need further improvements.

Objective This study evaluated the changes of the aortic diameter at the suprarenal and infrarenal segment after open repair (OR) and endovascular repair (EVAR) of abdominal aortic aneurysms (AAAs). Methods This was a retrospective analysis of all patients undergoing AAA repair between 1997 and 2008. Inclusion criteria were at least 3 months of follow-up at our institution, elective aneurysm repair, and absence of false, mycotic, or inflammatory aneurysms. For EVAR, standard computed tomography (CT) scans from follow-up were used; in the OR group, CT scans performed for unrelated nonvascular indications were used. Diameters of the aorta were measured at the first slice below the lowest renal artery and at the first slice above the highest renal artery. A 2-mm change was defined as measurable aortic neck dilatation. Results Inclusion criteria were met by 46 patients in the OR group and 103 in the EVAR group. After a follow-up of 34.1 months (range, 5.5-131.7 months) in the OR group and 39.4 months (range, 3-108.9 months) in the EVAR group, the mean changes were 1.75 ± 3.50 mm (OR) and 0.9 ± 2.3 mm (EVAR; P = .305) in the suprarenal diameters and 0.8 ± 2.9 mm (OR) and 1.2 ± 2.5 mm (EVAR; P = .311) in the infrarenal diameters. The absolute suprarenal vs infrarenal sizes were 29.7 ± 7.1 and 28.7 ± 6.8 mm in the OR group and 28.7 ± 3.2 and 28.5 ± 3.6 mm, respectively, in the EVAR group (suprarenal, P = .749; infrarenal, P = .273). Increase of the aortic diameter >2 mm, defined as aortic neck dilatation, was found in 23 of 103 EVAR patients (22.3% ± 0.862%), and in nine of 46 OR patients (19.57% ± 0.484%; P = .870). Increase in the suprarenal change >2 mm occurred in 21 of 103 EVAR patients (20.39% ± 1.04%) and in 14 of 46 OR patients (30.4% ± 0.446%; P = .260). Reintervention rate of patients with an increase >2 mm was 31% (seven of 23) in EVAR and 11.1% (one of nine) in the OR group (P = .386). Conclusions The AAA groups treated with EVAR or OR demonstrated similar increases of aneurysmal neck diameters. This suggests that aortic neck dilatation may be caused by a natural progression of the disease rather than by deviating therapeutic strategies.

Purpose The purpose of this study was to compare planimetric aortic valve area (AVA) measurements from 256-slice CT to those derived from transesophageal echocardiography (TEE) and cardiac catheterization in high-risk subjects with known high-grade calcified aortic stenosis. Methods and materials The study included 26 subjects (10 males, mean age: 79 ± 6; range, 61–88 years). All subjects were clinically referred for aortic valve imaging prior to percutaneous aortic valve replacement from April 2008 to March 2009. Two radiologists, blinded to the results of TEE and cardiac catheterization, independently selected the systolic cardiac phase of maximum aortic valve area and independently performed manual CT AVA planimetry for all subjects. Repeated AVA measurements were made to establish CT intra- and interobserver repeatability. In addition, the image quality of the aortic valve was rated by both observers. Aortic valve calcification was also quantified. Results All 26 subjects had a high-grade aortic valve stenosis (systolic opening area 2 ) via CT-based planimetry, with a mean AVA of 0.62 ± 0.18. In four subjects, TEE planimetry was precluded due to severe aortic valve calcification, but CT-planimetry was successfully performed with a mean AVA of 0.46 ± 0.23 cm 2 . Mean aortic valve calcium mass score was 563.8 ± 526.2 mg. Aortic valve area by CT was not correlated with aortic valve calcium mass score. A bias and limits of agreement among CT and TEE, CT and cardiac catheterization, and TEE and cardiac catheterization were −0.07 [–0.37 to 0.24], 0.03 [−0.49 to 0.55], 0.12 [−0.39 to 0.63] cm 2 , respectively. Differences in AVA among CT and TEE or cardiac catheterization did not differ systematically over the range of measurements and were not correlated with aortic valve calcium mass score. Conclusion Planimetric aortic valve area measurements from 256-slice CT agree well with those derived from TEE and cardiac catheterization in high-risk subjects with known high-grade calcified aortic stenosis.

Rationale and Objectives The aim of this study was to investigate whether a respiratory biofeedback system could increase navigator efficiency and maintain image quality compared to conventional respiratory-gated magnetic resonance coronary angiography (MRCA). Materials and Methods Eighteen healthy volunteers underwent MRCA using three different respiratory-gating protocols. A conventional expiratory free-breathing (FB) sequence was compared to two approaches using navigator echo biofeedback (NEB), a midinspiratory approach (NEBin) and an expiratory approach (NEBex). Navigator data reflecting the position of the diaphragm relative to a 3-mm gating window were made available to the subject using a video projector in combination with a Plexiglas screen and mirror goggles. Image quality was graded by two radiologists in consensus using a visual score ranging from 1 (not visible) to 4 (excellent vessel depiction). Results The NEB approaches improved navigator efficiency (71.1% with NEBex and 68.0% with NEBin vs 42.2% with FB), thus reducing total imaging time. This difference was statistically significant (PNEBin = .007; PNEBex = .001). Image quality in the NEBex group was comparable to that in the FB group (median score, 2.44 vs 2.52), but it proved to be significantly lower (median score, 1.94 vs 2.52) for the right coronary artery and the left anterior descending coronary artery in the NEBin group. Conclusion NEB maintains image quality and significantly increases navigator efficiency, thereby decreasing total imaging time by about 40% compared to a conventional FB acquisition strategy.

The purpose of this study was to investigate whether spectral computed tomography (CT) has the potential to improve luminal depiction by differentiating among intravascular gadolinium-based contrast agent, calcified plaque, and stent material by using the characteristic k edge of gadolinium. A preclinical spectral CT scanner with a photon-counting detector and six energy threshold levels was used to scan a phantom vessel. A partially occluded stent was simulated by using a calcified plaque isoattenuated to a surrounding gadolinium chelate solution. The reconstructed images showed an effective isolation of the gadolinium with subsequent clear depiction of the perfused vessel lumen. The calcified plaque and the stent material are suppressed.

Objective: To analyze changes in attenuation of non-contrast-enhanced and contrast-enhanced myocardium throughout the cardiac cycle and to determine the effect of myocardial location, cardiac phase, and dominant coronary perfusion type on myocardial attenuation to evaluate the impact of contraction-dependent factors on multidetector computed tomography (MDCT) perfusion studies. Material and Methods: Cardiac computed tomography imaging was performed on a 40-row MDCT scanner. Myocardial attenuation was assessed in 50 men during 20 cardiac phases. Ninety myocardial regions of interest were placed in the distribution of the right, left anterior descending, and left circumflex coronary arteries on contrast-enhanced MDCT. In 26 patients, myocardial attenuation was also assessed on non-contrast-enhanced MDCT. Myocardial attenuation was analyzed on isopycnic graphs. Attenuation changes due to myocardial contraction were evaluated on non-contrast-enhanced MDCT. The impact of cyclic perfusion on myocardial enhancement was determined on contrast-enhanced MDCT. Student t test and Pearson correlation were used. The influence of dominant coronary perfusion type, supplying coronary artery, and cardiac phase were evaluated using univariate general linear model analysis. Results: Isopycnic graphs revealed that myocardial contraction during systole led to significant increase in myocardial attenuation on non-contrast-enhanced MDCT [HU systole = 33.2 vs HU diastole = 29.1, P < 0.001]. Reduction in myocardial perfusion during systole led to decrease in myocardial attenuation in contrast-enhanced MDCT [HU systole = 92.4 vs HU diastole = 111.9, P < 0.001]. Systolic increase in attenuation due to myocardial contraction was far outweighed by the effects of intramyocardial contrast enhancement (correlation Pearson = 0.941). Dominant coronary perfusion did not lead to significant changes in contrast-enhanced myocardium (P = 0.299). Anatomical location and underlying cardiac phase represented significant factors in myocardial attenuation variation (P < 0.001). Conclusion: Myocardial attenuation is influenced by cardiac phase and myocardial location. These factors must be considered when analyzing myocardial perfusion with MDCT.

Background: Cardiovascular magnetic resonance imaging (CMR) has become an established noninvasive method for evaluating ventricular function utilizing three-dimensional volumetry. Postprocessing of volumetric measurements is still tedious and time consuming. Stroke volumes obtained by flow quantification across the aortic root or pulmonary trunk could be utilized to increase both speed of workflow and accuracy. Purpose: To assess accuracy of stroke volume quantification using MR volumetric imaging compared to flow quantification in patients with various cardiac diseases. Strategies for the augmentation of accuracy in clinical routine were deduced. Material and Methods: 78 patients with various cardiac diseases—excluding intra- or extracardiac shunts, regurgitant valvular defects, or heart rhythm disturbance—underwent cardiac function analysis with flow measurements across the aortic root and cine imaging of the left ventricle. Forty-six patients additionally underwent flow measurements in the pulmonary trunk and cine imaging of the right ventricle. Results: Left ventricular stroke volume (LVSV) and stroke volume of the aortic root (SVAo) correlated with r=0.97, and Bland-Altman analysis showed a mean difference of 0.11 ml and a standard error of estimation (SEE) of 4.31 ml. Ninety-two percent of the data were within the 95% limits of agreement. Right ventricular stroke volume (RVSV) and stroke volume of the pulmonary trunk (SVP) correlated with a factor of r=0.86, and mean difference in the Bland-Altman analysis was fixed at –2.62 ml (SEE 8.47 ml). For RVSV and SVP, we calculated r=0.82, and Bland-Altman analysis revealed a mean difference of 1.27 ml (SEE 9.89 ml). LVSV and RVSV correlated closely, with r=0.91 and a mean difference of 2.79 ml (SEE 7.17 ml). SVAo and SVP correlated with r=0.95 and a mean difference of 0.50 ml (SEE 5.56 ml). Conclusion: Flow quantification can be used as a guidance tool, providing accurate and reproducible stroke volumes of both ventricles. Combining both offers a highly accurate tool to gauge ventricular function in a routine clinical setting, increasing workflow speed.

The purpose of this study was to evaluate the accuracy of multi-detector row helical CT (MDCT) for detection and localization of acute upper and lower gastrointestinal (GI) hemorrhage or intraperitoneal bleeding. Thirty-six consecutive patients with clinical signs of acute bleeding underwent biphasic (16- or 40-channel) MDCT. MDCT findings were correlated with endoscopy, angiography or surgery. Among the 36 patients evaluated, 26 were examined for GI bleeding and 10 for intraperitoneal hemorrhage. Confirmed sites of GI bleeding were the stomach (n = 5), duodenum (n = 5), small bowel (n = 6), large bowel (n = 8) and rectum (n = 2). The correct site of bleeding was identifiable on MDCT in 24/26 patients with GI bleeding. In 20 of these 24 patients, active CM extravasation was apparent during the exam. Among the ten patients with intraperitoneal hemorrhage, MDCT correctly identified the bleeding source in nine patients. Our findings suggest that fast and accurate localization of acute gastrointestinal and intraperitoneal bleeding is achievable on MDCT.

Comparison of cleansing effects and colonic distension observed with two polyethyleneglycol-solution (PEG) containing bowel preparation techniques prior to CT-colonography (CTC).One hundred and three patients that received CTC in our institution were retrospectively evaluated. Fifty-one patients received preparation 1 (BP1; based on a GoLytely formulation+bisacodyl), 52 preparation 2 (BP2; based on a NuLytely formulation+bisacodyl). On multi-planar-reformatted (MPR) images, fluid residuals and colon distension were assessed in five colonic segments, from the ascending colon to the rectum.On average, significantly (p0.001) lower fluid residuals were assesses when using BP2 regardless of the patient position. In prone position, a significantly lower fluid level was observed in the sigmoid using bowel preparation 2. The average maximum diameter measured for the whole colon was 5.2+/-0.6 cm in prone position and 4.8+/-0.6 cm in the supine position in BP1 (p0.01). In BP2 the average maximum diameter measured for the whole colon was 5.3+/-0.6 cm in prone position and 4.7+/-0.5 cm in supine position, respectively (p0.001).Overall, lower fluid residuals were scored using BP2. In both preparation groups we achieved better colonic distension in prone position. We were not able to distend the sigmoid better when insufflating air during patient repositioning.

Objective: To assess ex vivo specimens of atherosclerotic coronary arteries by dual energy (DE) multidetector computed tomography (MDCT) imaging, and to correlate depicted vessel lumen morphology and detected tissue characteristics with histopathologic analysis. Methods: Coronary arteries were imaged on a 16-slice MDCT using a DE protocol consisting of a 90- and 140-kV scan. Coronary arteries were perfused with iodine- and gadolinium-based contrast agents. The DE K-edge subtractions were performed. Regions-of-interest were placed on histopathologically/radiographically-matched vascular lumen and wall, fibromuscular and calcified plaque, and fat tissues. Vascular/tissue contrast-to-noise ratios (CNR) were calculated, and their dependence on tissue type and contrast agent type was statistically evaluated. Results: Tissue CNR analysis confirmed that all tissue types were successfully distinguished. Vascular wall and fibromuscular plaque achieved a significant increase in CNR ratios when DE techniques were used compared with 140 kV protocols. Conclusions: Spectral DE MDCT imaging of ex vivo atherosclerotic coronary arteries allows successful tissue characterization and enhances depiction of coronary lumen.

Low motion phases for cardiac computed tomography reconstructions are currently detected manually in a user-dependent selection process which is often time consuming and suboptimal. The concept of motion maps was recently introduced to achieve automatic phase selection. This pilot study compared the accuracy of motion-map phase selection to that with manual iterative selection. The study included 20 patients, consisting of one group with low and one with high heart rate. The technique automatically derives a motion strength function between multiple low-resolution reconstructions through the cardiac cycle, with periods of lowest difference between neighboring phases indicating minimal cardiac motion. A high level of agreement was found for phase selection achieved with the motion map approach compared with the manual iterative selection process. The motion maps allowed automated quiescent phase detection of the cardiac cycle in 85% of cases, with best results at low heart rates and for the left coronary artery. They can also provide additional information such as the presence of breathing artifacts. Motion maps show promise as a rapid off-line tool to automatically detect quiescent cardiac phases in a variety of patients.

To evaluate aortic neck changes, specifically aortic neck dilatation, over a 10-year period in patients undergoing endovascular aneurysm repair (EVAR) for abdominal aortic aneurysm.All patients who underwent elective EVAR at our institution from 1998 through 2007 were analyzed retrospectively. Among these, 103 patients (96 men; mean age 71 years, range 35-84) who received the 3 most frequently implanted self-expanding stent-grafts (35 Talent, 39 Excluder, and 29 Zenith) and had a minimum 3-month imaging follow-up at our department were selected for this study. All diameters were measured perpendicular to the centerline of flow on computed tomography; baseline data were derived from the first postoperative scan. Stent-graft migration was measured from the lowermost renal artery to the first strut of the stent-graft. Based on intra- and interobserver error measurements, a minimum change of 2 mm defined aortic neck dilatation.During a mean follow-up of 39.4 months (range 3-108.8), infrarenal aortic neck dilatation (2 mm) was found in 10 patients (28.6%, 95% CI 14.6-46.3) in the Talent group, 4 (10.3%, 95% CI 2.9-24.2) in the Excluder group, and 9 (31.0%, 95% CI 15.3-50.8) in the Zenith group (p=0.299). In 7 (31%) of the 23 patients with neck dilatation, additional therapy was necessary. Suprarenal changes were found in 6 patients (17.1%, 95% CI 6.6-33.6), 8 patients (20.5%, 95% CI 9.3-36.5), and 5 patients (17.2%, 95% CI 5.8-35.8), respectively (p=0.218). Stent-graft migration2 mm was seen in 2 (1.9%) of the 103 patients in follow-up. The overall endoleak rates were 37.1% for Talent, 30.8% for Excluder, and 37.9% for Zenith (p0.05). Two patients were converted to open repair owing to Talent stent-graft migration and persistent type I leak (Zenith). The overall reintervention rate was 13.6% (13/103).Only a small number of the patients treated with self-expanding stent-grafts show notable infrarenal aortic neck dilatation, which does not appear to be related to the stent-graft model. Almost all cases of aortic neck dilatation exceeded the percentage of oversizing; less than a third of patients with aortic neck dilatation required therapy.

To evaluate the feasibility of MRI for static and dynamic assessment of the deployment of thoracic aortic stent grafts after emergency implantation in trauma patients. Twenty patients initially presenting with a rupture of the thoracic aorta were enrolled in this study. All patients underwent thoracic endovascular aortic repair (TEVAR). The deployment of the implanted stent graft was assessed by CTA and MRI, comprising the assessment of the aortic arch with and without contrast agent, and the assessment of the motion of the stent graft over the cardiac cycle. The stent graft geometry and motion over the cardiac cycle were assessable by MRI in all patients. Flow-mediated signal variations in areas of flow acceleration could be well visualised. No statistically significant differences in stent-graft diameters were observed between CT and MRI measurements. MRI appears to be a valuable tool for the assessment of thoracic stent grafts. It shows similar performance in the accurate assessment of stent-graft dimensions to the current gold standard CTA. Its capability of providing additional functional information and the lack of ionising radiation and nephrotoxic contrast agents may make MRI a valuable tool for monitoring patients after TEVAR.

As diffusion-weighted imaging is increasingly implemented into routine protocols of abdominal MRI, abnormal findings in expected and unexpected locations become more common. The aim of our retrospective study was to investigate the specificity of restricted diffusion in differentiation of benign from malignant abdominal disease.Two hundred thirty consecutively registered patients underwent abdominal MRI including diffusion-weighted imaging (single-shot spin-echo echo-planar sequence) with b values of 0, 150, 500, and 1,000 s/mm(2). Lesions were detected by two blinded readers using only the images with a b value of 1,000 s/mm(2), and representative apparent diffusion coefficients were measured. Lymph nodes were not documented.Fifty-two of the 230 patients had a total of 55 lesions with restricted diffusion (23.9%). The mean apparent diffusion coefficient was 809 mm(2)/s. Forty-three lesions (78.2%) were malignant. The 12 benign lesions were liver hemangioma, liver adenoma, autoimmune pancreatitis, pancreatic teratoma, two abscesses, three cases of inflammatory bowel wall thickening due to Crohn's disease, Bartholin cyst, hemorrhagic ovarian cyst, and renal Rosai-Dorfman disease.Restricted diffusion is generally considered to be associated with malignant tumors because of the high cellularity of these tumors. However, in interpretation of diffusion-weighted images, it should be kept in mind that a number of benign lesions, as many as 22% in our cohort, can exhibit restricted diffusion on images with high b values, thus mimicking malignant lesions.

The purpose of this study was to evaluate the potential clinical value of coronary plaque imaging with a new generation CT scanner and the interobserver variability of coronary plaque assessment with a new semiautomatic plaque analysis application. Thirty-five isolated plaques of the left anterior descending coronary artery from 35 patients were evaluated with a new semiautomatic plaque analysis application. All patients were scanned with a 256-slice MDCT scanner (Brilliance iCT, Philips Healthcare, Cleveland OH, USA). Two independent observers evaluated lesion volume, maximum plaque burden, lesion CT number mean and standard deviation, and relative lesion composition. We found 10 noncalcified, 16 mixed, and 9 calcified lesions in our study cohort. Relative interobserver bias and variability for lesion volume were -37%, -13%, -49%, -44% and 28%, 16%, 37%, and 90% for all, noncalcified, mixed, and calcified lesions, respectively. Absolute interobserver bias and variability for relative lesion composition were 1.2%, 0.5%, 1.5%, 1.3% and 3.3%, 4.5%, 7.0%, and 4.4% for all, noncalcified, mixed, and calcified lesions, respectively. While mixed and calcified lesions demonstrated a high degree of lesion volume interobserver variability, noncalcified lesions had a lower degree of lesion volume interobserver variability. In addition, relative noncalcified lesion composition had a very low interobserver variability. Therefore, there may a role for MDCT in serial noncalcified plaque assessment with semiautomatic analysis software.

To compare image quality and effective radiation dose for prospectively gated axial CT coronary angiography on 64- and 256-slice CT. The patient cohort consisted of 80 consecutive patients undergoing imaging with 256-slice CT and 80 patients with 64-slice CT. The two patient groups were demographically matched according to age, gender, body mass index and heart rate. For both groups, two observers independently assessed image quality for all coronary segments on a five-point ordinal scale. The two groups were compared with regard to image quality and effective radiation dose. Image quality scores less than 3 were considered non-diagnostic. Mean effective radiation dose did not significantly differ between the two groups (3.4 ± 0.8 mSv in both groups; p > 0.1). Purpose In this study, we investigated the influence of sublingual nitroglycerine (NTG) on the peripheral diameter, intraluminal contrast agent density, and image quality of coronary arteries during computed tomography coronary angiography (CTCA). Materials and methods Thirty patients with sublingual NTG application were matched to 30 patients without NTG. The diameters of the left anterior descending coronary artery (LAD), the left circumflex coronary artery and the right coronary artery were measured at 1-, 4-, and 8-cm length of each vessel as well as the intraluminal contrast agent density along the LAD. Vessel diameters and contrast attenuation at 4 and 8 cm were referenced against the values at 1 cm and processed as percentage reduction. Image quality of the posterior descending artery was assessed subjectively by 2 independent observers. Results The percentage of peripheral vessel diameter reduction and the peripheral attenuation of contrast agent density for all measured coronary arteries was significantly smaller in the group with NTG administration. The image quality of the posterior descending artery was significantly higher in the group with NTG. Conclusions Sublingual administration of NTG before CTCA results in improved diagnostic image quality because of a significant dilatation and improved intraluminal contrast agent density of the peripheral vessels.

OBJECTIVE The purpose of this study was to analyze the pulmonary vessel enhancement obtainable using high-density contrast material and saline flushing with automated bolus tracking for multidetector-row computed tomographic (MDCT) pulmonary angiography in a routine clinical setting. METHODS Attenuation values of pulmonary vessels were retrospectively measured in 51 CT scans performed on a 40-channel MDCT for unselected emergency patients with clinically suspected pulmonary embolism. RESULTS Mean vascular attenuation measured 326.7 +/- 104 Hounsfield units (HU) in the pulmonary trunk and 299.1 +/- 102 HU in the left distal lower lobe artery. Mean vascular attenuation values in the distal lower lobe artery were equal or higher than 150 HU for 94.1% (48/51) of patients. CONCLUSIONS In conclusion, the combination of high-density contrast media and saline flushing with automated bolus tracking on a fast CT scanner reliably results in a homogenously high attenuation of pulmonary vessels, allowing precise delineation of clots down to the subsegmental level in MDCT pulmonary angiography.

To assess image quality and radiation exposure with prospectively gated axial CT coronary angiography (PGA) compared to retrospectively gated helical techniques (RGH). Forty patients with suspected coronary artery disease (CAD) and a stable heart rate below 65 bpm underwent CT coronary angiography (CTCA) using a 64-channel CT system. The patient cohort consisted of 20 consecutive patients examined using a PGA technique and 20 patients examined using a standard RGH technique. Both groups were matched demographically according to age, gender, body mass index, and heart rate. For both groups, two independent observers assessed image quality for all coronary segments on an ordinal scale from 1 (nonassessable) to 5 (excellent quality). Image quality and radiation exposure were compared between patient groups. There were no significant differences in vessel-based image quality between the two groups (P > 0.05). Mean (± SD) effective radiation exposure in the PGA group was 3.7 ± 0.8 mSv compared to 18.9 ± 3.8 mSv in the RGH group without ECG-based tube current modulation (P < 0.001). Preliminary experience shows PGA technique to be a promising approach for CTCA resulting in a substantial reduction in radiation exposure with image quality comparable to that of standard RGH technique.

The aim of this study was to investigate whether a respiratory biofeedback system could increase navigator efficiency and maintain image quality compared to conventional respiratory-gated magnetic resonance coronary angiography (MRCA).Eighteen healthy volunteers underwent MRCA using three different respiratory-gating protocols. A conventional expiratory free-breathing (FB) sequence was compared to two approaches using navigator echo biofeedback (NEB), a midinspiratory approach (NEBin) and an expiratory approach (NEBex). Navigator data reflecting the position of the diaphragm relative to a 3-mm gating window were made available to the subject using a video projector in combination with a Plexiglas screen and mirror goggles. Image quality was graded by two radiologists in consensus using a visual score ranging from 1 (not visible) to 4 (excellent vessel depiction).The NEB approaches improved navigator efficiency (71.1% with NEBex and 68.0% with NEBin vs 42.2% with FB), thus reducing total imaging time. This difference was statistically significant (P(NEBin)=.007; P(NEBex)=.001). Image quality in the NEBex group was comparable to that in the FB group (median score, 2.44 vs 2.52), but it proved to be significantly lower (median score, 1.94 vs 2.52) for the right coronary artery and the left anterior descending coronary artery in the NEBin group.NEB maintains image quality and significantly increases navigator efficiency, thereby decreasing total imaging time by about 40% compared to a conventional FB acquisition strategy.

Purpose of this study was the evaluation of the thoracic aortic wall thickness as a potential identifier of patients at increased risk for future cardiac events. Thoracic aortic wall thickness was measured with MDCT in 160 patients. The CT-scans were implemented as non-invasive coronary angiography studies. Relationships between aortic wall thickness, sex, age, major risk factors and atherosclerotic plaque burden of the coronary arteries were explored. Higher values of maximum aortic wall thickness of the descending aorta (women P = 0.02, men P = 0.01) were found in patients with coronary atherosclerosis, compared to patients with same gender but excluded atherosclerosis. Aortic wall thickness of the mid-portion of the descending aorta of 3.0 mm is associated with coronary artery disease (CAD) with a specificity of 96.6% (sensitivity 27.5%) and a positive predictive value (PPV) of 93.3%. For patients with two or more major risk factors and a maximum wall thickness of equal or more than 2.6 mm we found a PPV of 100%. We conclude that measurements of maximum wall thickness of the descending aorta are a potential tool for detecting patients with coronary atherosclerosis. The potential effect of combining measurements of aortic wall thickness at routine chest CT studies with a possible cardiovascular screening is substantial and merits further study.

In a 42-year-old woman presenting with intermittent atypical angina, transthoracic echocardiography revealed a tumor of the left posterolateral ventricular wall (Movie I in the online-only Data Supplement and Figure 1). Computed tomography (CT) permits cross-sectional imaging of the heart. Temporal and spatial resolutions of the technique have been insufficient to cover the heart without motion artefacts until the recent advent of multidetector systems with more than 16 detector rows. The modality is now suited for noninvasive imaging of the coronary arteries, producing detailed morphologic images of the entire coronary tree with upto 0.4 mm of spatial resolution, within a single short breath-hold duration. CT imaging goes beyond the delineation of the coronary lumen as provided by selective invasive angiography; the plaque burden of the coronary artery wall can be visualized directly, utilizing soft-tissue contrast and a high sensitivity even for the small calcifications that are present in hard plaque formations. Therefore, CT combines elements of catheterization angiography for lumen imaging and of intravascular ultrasound imaging for coronary wall imaging. However current CT technology is not yet able to compete with the temporal or spatial resolution of catheterization angiography nor does it provide the detailed spatial or contrast resolution of intravascular ultrasound imaging. At present, its use is therefore restricted to complementing the invasive modalities in appropriate indications. Although CT entails significantly less risk than the invasive procedures, the risks of radiation dose exposure and contrast agent application are not negligible. In the foreseeable future, if the current rate of technological advancement continues, CT may replace the invasive modalities in routine care for diagnostic purposes.

The objective of our study was to validate the feasibility of the synergistic use of cardiac MR and CT data sets for ventricular motility analysis and to correlate measurement variability with underlying heart rate.Twenty patients underwent concurrent ECG-gated MDCT and MRI for evaluation of ventricular motility, expressed as ventricular wall thickening and motion. Initially, individual measurement repetition series were analyzed by determining intraobserver variability and detecting intraobserver bias related to heart rates. Subsequently, absolute measurement differences of CT or MR data were statistically evaluated. Finally, absolute measurement differences were correlated with underlying heart rates by curve estimation regression.Analysis of measurement reproducibility proved that data variability was dependent on only the anatomic localization of the analyzed ventricular segment, not on the imaging technique used or underlying heart rate, in normofrequent patients. Comparing MR and CT image data sets, no statistically significant differences were identified when ventricular motility was evaluated based on data sets of either imaging technique in normofrequent patients. Tachycardic frequencies, above 100 beats per minute, led to exponential error propagation due to insufficient temporal resolution of the current CT technology.This study proved that cardiac motility assessment based on ECG-gated CT and MR data sets resulted in comparable ventricular function results for normofrequent patients; however, the high spatial resolution of cardiac MDCT cannot compensate for the lack of temporal resolution in patients with tachycardia, thus emphasizing the necessity of reporting ventricular motility analysis results in combination with heart rate to allow consideration of this possible cause for measurement variation.

BACKGROUND AND PURPOSE: Compared with MR imaging at 1.5T T1-weighted spin-echo imaging at 3T shows up with reduced gray-to-white matter contrast. The purpose of the present study was to show the effects of alterations of different flip angles as an easily accessible parameter to increase gray-to-white matter contrast. METHODS: Spin-echo T1 sequences of 6 healthy volunteers were acquired in a 3T head scanner with 5 different flip angles. Observer-independent contrast-to-noise ratios for gray versus white matter from different flip angles, as well as subjective ratings of image quality from 2 blinded neuroradiologists, were compared statistically. RESULTS: Gray-to-white matter contrast increased significantly with decreasing flip angle. No artifacts were introduced by decreasing flip angles, and T1 contrast characteristics were robust and stable at lowered flip angles. Also, specific absorption ratios significantly decreased with decreasing flip angles. CONCLUSION: Using a flip angle of 50° significantly increases gray-to-white matter contrast in T1 spin-echo brain scans at 3T B0 field strength.

ContextMultislice computed tomography (MSCT) has recently evolved as a modality for noninvasive coronary imaging.ObjectiveTo assess the accuracy and robustness of MSCT vs the criterion standard of invasive coronary angiography for detection of obstructive coronary artery disease.Design, Setting, and PatientsProspective, single-center study conducted in a referral center setting in Germany and enrolling 103 consecutive patients (mean age, 61.5 [SD, 9.7] years) from November 2003–August 2004 who were undergoing both invasive coronary angiography and MSCT using a scanner with 16 detector rows.Main Outcome MeasuresBlinded results for both modalities compared using the patient as the primary unit of analysis, with supplementary segment- and vessel-based analyses.ResultsOne thousand three hundred eighty-four segments (≥1.5 mm diameter) were identified by invasive coronary angiography; nondiagnostic image quality of MSCT was identified for only 88 (6.4%) of these segments, mainly due to faster heart rates. Compared with invasive coronary angiography for detection of significant lesions (>50% stenosis), segment-based sensitivity, specificity, and positive and negative predictive values of MSCT were 95%, 98%, 87%, and 99%, respectively. Quantitative comparison of MSCT and invasive coronary angiography showed good correlation (r = 0.87, P50% left main artery stenosis and/or >70% stenosis in any other epicardial vessel). Threshold optimization allowed either detection of these patients with 100% sensitivity at a reasonable false-positive rate (specificity, 76.5%; MSCT stenosis, >66%) or optimization of both the sensitivity and specificity (>90%; MSCT stenosis, >76%).ConclusionsMultislice computed tomography provides high accuracy for noninvasive detection of suspected obstructive coronary artery disease. This promising technology has potential to complement diagnostic invasive coronary angiography in routine clinical care.

Purpose To compare a new 12-element body phased-array coil with a conventional four-element surface receiver coil array to provide increased signal-to-noise ratios (SNRs) for cardiac steady state free precession (SSFP) perfusion imaging. Materials and Methods Thirteen consecutive patients were included in the study. Patients were examined both with a four-element surface coil array and a 12-element body coil array. First-pass myocardial perfusion imaging using saturation recovery SSFP was acquired during antecubital injection of Gd-DTPA. Imaging parameters: TR 2.8 msec/TE 1.3 msec, flip angle 50°, bandwidth 960 Hz/pixel and half-Fourier acquisition. SNR was calculated using six regions of interest (ROI) for the myocardial perfusion scans. Calculations of corresponding ROIs using the two different coil setups were compared using analysis of variance (ANOVA). Semiquantitative perfusion parameters were calculated for both groups. Results The mean SNR in myocardial perfusion imaging increased by 21% using the 12-element coil setup (P < 0.001) when compared to the four-element coil. ROI comparisons revealed an increased signal inhomogeneity with the 12-element coil when compared to four-element coil experiments. Absolute normal range values of semiquantitative perfusion parameters were consistently higher using the 12-element coil setup (P < 0.001). Conclusion The 12-element coil array provides higher SNR, but these improvements come with trade-offs in image homogeneity. Increased SNR translates into higher semiquantitative perfusion values and offers the potential for improved detection of perfusion defects. J. Magn. Reson. To evaluate the effect of heart rate on the quality of coronary angiograms obtained with 16-detector row computed tomography (CT) by using temporally enhanced three-dimensional (3D) approaches.The local ethics committee approved the study, and informed consent was obtained from all patients. Fifty patients underwent coronary CT angiography (heart rate range, 45-103 beats per minute). Raw data from helical CT and electrocardiography (ECG) were saved in a combined data set. Retrospectively ECG-gated images were reconstructed at preselected phases (50% and 80%) of the cardiac cycle. A 3D voxel-based approach with cardiac phase weighting was used for reconstruction. Testing for correlation between heart rate, cardiac phase reconstruction window, and image quality was performed with Kruskal-Wallis analysis. Image quality (freedom from cardiac motion-related artifacts) was referenced against findings at conventional angiography in a secondary evaluation step. Regression analysis was performed to calculate heart rate thresholds for future beta-blocker application.A significant negative correlation was observed between heart rate and image quality (r = 0.80, P.001). Motion artifact-free images were available for 44 (88%) patients and were achieved consistently at a heart rate of 80 or fewer beats per minute (n = 39). Best image quality was achieved at 75 or fewer beats per minute. Segmental analysis revealed that 97% of arterial segments (diameteror = 1.5 mm according to conventional angiography) were assessable at 80 or fewer beats per minute. Premature ventricular contractions and rate-contained arrhythmia did not impede diagnostic assessment of the coronary arteries in 10 (83%) of the 12 patients affected.Motion-free coronary angiograms can be obtained consistently with 16-detector row CT scanners and adaptive multicyclic reconstruction algorithms in patients with heart rates of less than 80 beats per minute.

The purpose of the present study was to evaluate the role of 16 multislice computed tomography (MSCT) to identify the origin of anomalous coronary arteries and to confirm their anatomic course in relation to the great vessels. Accuracy of coronary artery disease (CAD) detection was a secondary aim and was tested with conventional angiograms (CA) serving as standard of reference. Two hundred and forty-two consecutive patients referred for noninvasive coronary CT imaging were reviewed for the study. Sixteen patients (6.6%) with anomalous coronary arteries were detected and included as the study group. MSCT and CA images were analyzed in a blinded fashion for accuracy of anomalous artery origin and path detection. Results were compared in a secondary consensus evaluation. Accuracy ratios to detect CAD with MSCT in all vessels were calculated. Coronary anomalies for all 16 patients were correctly displayed on MSCT. CA alone achieved correct identification of the abnormality in only 53% (P=0.016). Sensitivity and specificity of MSCT to detect significantly stenosed vessels was 90 and 92%. 16-MSCT is accurate to delineate abnormally branching coronary arteries and allows sufficiently accurate detection of obstructive coronary artery disease in distal branches. It should therefore be considered as a prime non-invasive imaging tool for suspected coronary anomalies.

A 48-year-old man with chest films suggesting pericardial calcifications was referred for cardiac MRI to assess diastolic function. The left ventricle (ejection fraction of 55%, end-diastolic volume of 127 mL, and stroke volume of 71 mL [MRI data, Siemens Symphony Maestro]) did not show any wall motion abnormalities. The diastolic filling pattern assessed with transmitral flow did not show signs of restriction. Pericardial calcifications were not apparent at the routine True-FISP cine sequence (Figure 1a). ContextMultidetector computed tomography (MDCT) has been proposed as a noninvasive method to evaluate coronary anatomy.ObjectiveTo determine the diagnostic accuracy of 16-row MDCT for the detection of obstructive coronary disease based exclusively on quantitative analysis and performed in a multicenter study.Design, Setting, and PatientsEleven participating sites prospectively enrolled 238 patients who were clinically referred for nonemergency coronary angiography from June 2004 through March 2005. Following a low-dose MDCT scan to evaluate coronary artery calcium, 187 patients with an Agatston score of less than 600 underwent contrast-enhanced MDCT. Conventional angiography was performed 1 to 14 days after MDCT. Conventional angiographic and MDCT studies were analyzed by independent core laboratories.Main Outcome MeasuresSegment-based and patient-based sensitivities and specificities for the detection of luminal stenosis of more than 50% (of luminal diameter) and more than 70% (of luminal diameter) based on quantitative coronary angiography.ResultsOf 1629 nonstented segments larger than 2 mm in diameter, there were 89 (5.5%) in 59 (32%) of 187 patients with stenosis of more than 50% by conventional angiography. Of the 1629 segments, 71% were evaluable on MDCT. After censoring all nonevaluable segments as positive, the sensitivity for detecting more than 50% luminal stenoses was 89%; specificity, 65%; positive predictive value, 13%; and negative predictive value, 99%. In a patient-based analysis, the sensitivity for detecting patients with at least 1 positive segment was 98%; specificity, 54%; positive predictive value, 50%; and negative predictive value, 99%. After censoring all nonevaluable segments as positive, the sensitivity for detecting more than 70% luminal stenoses was 94%; specificity, 67%; positive predictive value, 6%; and negative predictive value, 99%. In a patient-based analysis, the sensitivity for detecting patients with at least 1 positive segment was 94%; specificity, 51%; positive predictive value, 28%; and negative predictive value, 98%.ConclusionsThe results of this study indicate that MDCT coronary angiography performed with 16-row scanners is limited by a high number of nonevaluable cases and a high false-positive rate. Thus, its routine implementation in clinical practice is not justified. Nevertheless, given its high sensitivity and negative predictive value, 16-row MDCT may be useful in excluding coronary disease in selected patients in whom a false-positive or inconclusive stress test result is suspected.

The role of 16-slice spiral CT was evaluated in the diagnosis of coronary stenosis, with selective X-ray coronary angiography (SCA) serving as the reference standard. Sixty-five patients who were suspected of having coronary heart disease, without percutaneous transluminal coronary angioplasty or coronary bypass-grafting, were investigated using 16-slice CT. Eight patients with pre-scan heart rate of more than 80 beats/min were given beta-blockers. After the retrospectively ECG-gated axial imaging reconstruction, volume redering (VR), multi-planar reconstruction (MPR), curved MPR and maximum intensity projection (MIP) were used to reconstruct. Every segment of coronary artery with a diameteror = 1.5 mm was assessed, and the presence on CT with a stenosis exceeding 50% diameter reduction was compared with that on SCA. The reasons which lead to some segments unevaluable were analysed. Compared with SCA, 93% coronary segments and 94% main branches were evaluable. Residual cardiac motion artifacts, severe calcification and poor opacification made 58%, 28% and 14% of the remaining 60 segments unevaluable respectively. Without routine administration of beta-blockers, good coronary imaging quality can be acquired using 16-slice spiral CT. It is a reliable noninvasive method for detection of obstructive coronary artery disease.
