Whereas posthemorrhagic hydrocephalus is well known after preterm birth, its association with syringomyelia has been reported only once. Here, we describe two additional patients showing this rare constellation. The children had been born after 27 and 28 completed weeks of gestation, respectively. Both had developed neonatal cerebral hemorrhages. At the age of 4 years, patient 1 presented with progressive ataxia leading to the diagnosis of internal hydrocephalus and extensive syringomyelia. In patient 2, progressive ventriculomegaly and syringomyelia were diagnosed at the age of 4 weeks. In both children, ventriculoperitoneal shunting resulted in clinical improvement, decrease of the ventricular size, and regression of the syringomyelia. After surgery, patient 1 developed a subdural hematoma and patient 2 parenchymatous cerebral hemorrhages. The combination of syringomyelia and posthemorrhagic hydrocephalus may be more frequent than commonly assumed. Therefore, neurological deterioration may mandate spinal MRI in addition to cerebral MRI. Syringomyelia seems to result from impaired ventricular cerebrospinal fluid drainage as it regresses after ventriculoperitoneal shunting. Bleeding may complicate the postsurgical relaxation of distended brain parenchyma.

Object Previous reports suggest that the use of adjustable differential-pressure valves may improve shunt performance. The absence of a flow- or siphon-controlled mechanism, however, is a concern. The goal of this prospective study was to assess the efficacy of valve adjustments in preventing slitlike ventricles in children < 6 months old after the first shunt insertion. Methods A total of 15 infants < 6 months of age who were undergoing initial shunt placement were included. Imaging was performed preoperatively, at 14 days postoperatively, and every 4 weeks thereafter. Ventricle size was assessed using ultrasonography and MR imaging or cerebral CT scanning at 1 and 2 years postoperatively. Clinical follow-up duration was 24 months for all patients. Valve settings were changed by 50 mm H2O if ventricle size decreased by 30% compared to preoperative size. Results The valve pressure setting was increased to 200 mm H2O in 11 children within the follow-up time, whereas ventricle size decreased from 0.6 ± 0.08 to 0.39 ± 0.09 (frontal/occipital horn ratio, mean ± standard deviation). There was neither clinical nor radiological evidence of underdrainage. Conclusions The adjustable differential-pressure valve used in this study was not effective in preventing slitlike ventricles in the majority of patients. Despite the small number of patients, this study provides a rationale for examining whether new shunt designs (gravitational shunt valves) are superior to conventional shunt systems in managing challenging hydrocephalus problems.

The main problem faced by the increasing numbers of patients presenting for spinal surgery are receiving concurrent medication with low-dose aspirin, leading to dysfunctional circulating platelets. The contribution of low-dose aspirin to increased peri-operative risk of bleeding and blood loss is a contentious issue with conflicting published results from different surgical groups. Data from neurosurgical spine patients is sparse, but aspirin has been identified as an important risk factor in the development of post-operative hematoma following intracranial surgery. We surveyed the opinions and working practices of the neurosurgical facilities performing spinal operations in Germany regarding patients who present for elective spinal surgery. Identical questionnaires were sent to 210 neurosurgical facilities and proffered five main questions: (1) the adherence of any policy of stopping aspirin pre-operatively, (2) the personal risk assessment for patients with spinal surgery under low-dose aspirin medication, (3) the preferred method of treatment for excessive bleeding in this context, (4) personal knowledge of hemorrhagic complications in this group of patients, and (5) the characteristics of the neurosurgical units concerned. There were 145 (69.1%) responses of which 142 (67.6%) were valid. Of the respondents, 114 (80.3%) had a (written) departmental policy for the discontinuation of pre-operative aspirin treatment, 28 (19.7%) were unaware of such a policy. The mean time suggested for discontinuation of aspirin pre-operatively was 6.9 days (range: 0-21 days), with seven respondents who perform the operations despite the ongoing aspirin medication. Ninety-four respondents (66.2%) considered that patients taking low-dose aspirin were at increased risk for excessive peri-operative hemorrhage or were indetermined (8.6%), and 73 (51.4%) reported having personal experience of such problems. Ninety-two respondents (65.5%) would use special medical therapy, preferably Desmopressin alone or in combination with other blood products or prohemostatic agents (46.1%), if hemorrhagic complications developed intra- or post-operatively. The average number of spinal operations per year in each service was 607.9 (range: 40-1,500). Despite the existence of distinct departmental policies concerning the discontinuation of low-dose aspirin pre-operatively in the majority of neurosurgical facilities performing spinal operations, there is a wide range of the moment of this interruption with an average of 7 days. Two-thirds of the respondents felt that aspirin was a risk factor for hemorrhagic complications associated with spinal procedures, and more than half of the interviewees reported having personal experience of such problems. Finally, various medicamentous methods of counteracting aspirin-induced platelet dysfunction and excessive bleeding in this context are elicited, discussed and evaluated.

Objective: Shunting of the lateral ventricle and the posterior fossa cyst is the advocated surgical therapy for children with Dandy-Walker malformation (DWM) and associated aqueductal obstruction. The high rate of complications of combined shunting stimulated the authors to search for an alternative surgical solution. Clinical Presentation/Intervention: After transtentorial endoscopic ventriculocystostomy, a cystoventricular catheter, connected to a peritoneal shunt, was placed in a neonate with DWM and associated aqueductal obstruction. Immediately prior to ventriculocystostomy, the presence of a blocked third ventricular outflow was reconfirmed by contrast medium injection. Neuronavigation was required to define the surgical path from the lateral ventricle through the tentorium and the overlying small rim of brain parenchyma into the posterior fossa cyst. The postoperative clinical course was uneventful with radiologically proven reduction of the size of the ventricular system and the cyst. Conclusion: Cystoventriculoperitoneal shunt placement after transtentorial endoscopic ventriculostomy is a surgical alternative in very young children with DWM and associated aqueductal obstruction.

More sensitive imaging techniques, such as MRI, have led to an increase in the number of reported cases of spinal cord cavernous haemangioma (SCCH). Complete surgical resection has been performed with good outcomes. However, operative findings do not always confirm preoperative MRI as to the size and site (superficial or deep) of the lesion. We evaluated whether MRI can be used to predict whether or not SCCH reach the surface of the spinal cord, since this has an impact on surgical strategy. We reviewed the preoperative MRI, case-notes and video recordings of 12 patients who underwent surgery, at which five superficial and seven deep-seated lesions were identified. T1-weighted images correctly indicated the site of the lesion in ten, T2-weighted images in only eight. One deep lesion was thought to be superficial on both T1- and T2-weighted images. Intravenous contrast medium was not helpful in diagnosis or localisation. In no case was a surgically proven superficial lesion interpreted as deep in the spinal cord.

Objectives: To investigate if intraoperative focused high frequency repetitive transcranial magnetic stimulation (rTMS) can localise the primary motor cortex without exposure of the cortical surface. Methods: A high frequency train (357 Hz) of four suprathreshold magnetic stimuli was delivered transcranially to the region of the rolandic area during brain tumour operations in 12 patients. To induce a focal magnetoelectric field, the flat figure of eight coil (outer diameter of each loop 7 cm) was used. Motor evoked potentials (MEP) were recorded in eight muscles of the upper and lower contralateral extremities. The first stimulation site was 2.5 cm behind the bregma, the second site 2 cm, and the third site 4 cm dorsal to the first stimulation site. If no MEP were obtainable, stimulation was repeated in anteroposterior direction at more laterally located sites. Using neuronavigation, each positive stimulation site was correlated with the underlying cortical anatomy. Results: Stimulation was performed at a total of 42 sites (in two patients, maximum stimulation at the three initial sites failed to evoke a motor response). In four patients, MEP were obtained only from one stimulation site. This site exactly overlayed the primary motor cortex. In eight patients, MEP could be elicited from more than one stimulation site. In seven of the eight patients, the site from which MEP with peak amplitudes were elicited, corresponded to the primary motor cortex. In total, the primary motor cortex was correctly identified on the basis of electrophysiological findings in 11 of 12 patients (92 %). In two patients, only the more lateral stimulation sites permitted MEP recording. Conclusion: Intraoperative focused rTMS is highly sensitive for localisation of the primary motor cortex. Focused rTMS as a localising instrument alleviates the need of motor cortex exposure and, thereby, can contribute to minimise the surgical approach to brain tumours in the rolandic area.

Background Bacterial orbital cellulitis is a relatively uncommon infective process, which can threaten the function of orbital structures. Apart from antibiotic therapy, sinus surgery with or without abscess drainage via an orbito-otorhinolaryngological approach might be necessary. Case description We present three cases of severe orbital cellulitis, leading to increasing loss of vision, proptosis, afferent pupillary disturbances and restriction of extra-ocular movements, despite antibiotic therapy. After extended pterional orbital decompression and reducing the orbital pressure by removal of the lateral and superolateral orbital walls, all patients showed distinct improvement of initial symptoms, without any complications related to the operation. Interpretation Extended pterional orbital decompression represents an effective treatment alternative and supplement in cases of a severe, threat to ocular function due to orbital cellulitis, where acute reduction of pressure on orbital, neural and vascular structures is intended.

Induction of heme oxygenase-1 (HO-1) in the spinal cord was studied in adult wildtype and transgenic mice overexpressing the antioxidant copper, zinc superoxide dismutase (CuZn SOD) 24 h after intrathecal infusion of heterologous lysed blood. Double immunolabeling techniques were used to determine the extent to which HO-1 was induced in astrocytes and microglia/macrophages. HO-1 was induced in both astrocytes and microglia/macrophages in the dorsal horns near the site of infusion of lysed blood in all mice. However, the number of HO-1 labeled cells was significantly less in the transgenic as compared to the wildtype animals. Together, these findings suggest that lysed blood preferentially induces HO-1 in glia and macrophages through the generation of oxidative stress.

In the present investigation, regional ATP, glucose, and lactate contents were examined in the cortical and subcortical structures after cold lesion in rats. Bioluminescence imaging of ATP, glucose, and lactate was performed in serial tissue sections at 4 h (n = 4), 12 h (n = 4) and 24 h (n = 4) after cold injury or sham surgery. Bioluminescence images were analyzed by computer-assisted densitometry, at the lesion site, in cortical areas, in the hippocampus, and in the thalamus. ATP and glucose content were significantly decreased at the lesion site as well as on the contralateral side after 4, 12, and 24 h postinjury Lactate content increased significantly in the hippocampal area on the ipsilateral side at 12 h. Cortical lactate was bilaterally unchanged. The cold lesion injury led to a characteristic ischemic profile in the hippocampus signaled by low ATP and glucose content paralleled by high lactate levels. The otherwise global depletion of glucose and ATP suggests that other factors besides cerebral blood flow may contribute to the impairment of energy metabolism.

Traumatic spinal cord injury results in the disruption of neural and vascular structures (primary injury) and is characterized by an evolution of secondary pathogenic events that collectively define the extent of functional recovery. This article reviews the vascular responses to spinal cord injury, focusing on both early and delayed events, including intraparenchymal hemorrhage, inflammation, disruption of the blood-spinal cord barrier, and angiogenesis. These vascular-related events not only influence the evolution of secondary tissue damage but also define an environment that fosters neural plasticity in the chronically injured spinal cord.

A case of lateral sinus pericranii associated with ipsilateral venous anomaly is reported. No treatment was instituted in accordance with the parents' wishes. The role of MRI and cerebral angiography in the diagnostic workup is discussed.

Symptomatic overdrainage in children with shunt-treated hydrocephalus represents one of the more difficult shunt-related diseases and may require repeated surgery. Gravity-assisted valve design has become a standard device to avoid overdrainage in many European pediatric hydrocephalus centers. However, the use of a gravitational valve for relieving symptoms associated with overdrainage has not yet been addressed. The goal of this study was to evaluate the effectiveness of a gravitational valve in the treatment of symptomatic overdrainage in children with shunts.Seventeen children with an adjustable shunt system and symptomatic overdrainage were treated by insertion of a gravitational valve. Clinical and radiological outcome were monitored for a minimum of 12 months after surgery.Implantation of a gravitational valve resulted in either resolution (n = 12) or improvement (n = 5) of the symptoms. In 1 patient, symptoms remained almost unchanged and the gravitational valve had to be upgraded, resulting in resolution of the symptoms. During follow-up, the pressure setting of the adjustable differential pressure valve had to be changed in 7 patients.The gravitational valve was effective in improving symptomatic overdrainage in the majority of patients in the present study. Because the ideal pressure setting for a given patient is hard to determine a priori, adjustable valve systems appear to be beneficial.

In adults, atlas burst fractures (Jefferson fractures) are not uncommon. In the pediatric population, they are extremely rare, and only few cases have been reported so far. They all showed a highly consistent clinical presentation with rare neurological deficits as well as recovery with full function without any surgical intervention. We describe a case of a Jefferson fracture in a 7.5-year-old-boy after falling onto the top of his head, presenting solely with slight neck pain. The initial radiological characteristics are presented on magnetic resonance imaging (MRI) and computer tomography (CT); the bony bridging and fusion of the fracture sites are demonstrated on CT during the course of the healing. The patient was placed in a rigid cervical collar, and his physical examination results remained normal according to age. Complete fusion of both fracture sites was seen on the third follow-up 6.5 months after presentation. In this rare case of a Jefferson fracture in a child monitored by radiological imaging techniques, important aspects of conservatively treated atlas burst fractures are illustrated. It underlines their benign character and favorable outcome with full functional recovery without any surgical intervention.

✓ Among spinal cord vascular malformations, dural arteriovenous fistulas (DAVFs) must be distinguished from intradural malformations. The concurrence of both is extremely rare. The authors report the case of a 35-year-old man who suffered from progressive myelopathy and who harbored both a DAVF and an intradural perimedullary fistula. During surgery, both fistulas were identified, confirmed, and subsequently obliterated. The fistulas were located at two levels directly adjacent to each other. Although the incidence of concurrent spinal DAVFs is presumed to be approximately 2%, the combination of a dural and an intradural fistula is exceedingly rare; only two other cases have been reported in the literature. One can speculate whether the alteration in venous drainage caused by the (presumably congenital) perimedullary fistula could possibly promote the production of a second dural fistula due to elevated pressure with concomitant venous stagnation and subsequent thrombosis. The authors conclude that despite the rarity of dual pathological entities, the clinician should be aware of the possibility of the concurrence of more than one spinal fistula in the same patient.

The primary objective of neurophysiologic monitoring during surgery is to avoid permanent neurological injury resulting from surgical manipulation. To prevent motor deficits, either somatosensory (SSEP) or transcranial motor evoked potentials (MEP) are applied. This prospective study was conducted to evaluate if the combined use of SSEP and MEP might be beneficial. Combined SSEP/MEP monitoring was attempted in 100 consecutive procedures, including intracranial and spinal operations. Repetitive transcranial electric motor cortex stimulation was used to elicit MEP from muscles of the upper and lower limb. Stimulation of the tibial and median nerves was performed to record SSEP. Critical SSEP/MEP changes were defined as decreases in amplitude of more than 50% or increases in latency of more than 10% of baseline values. The operation was paused or the surgical strategy was modified in every case of SSEP/MEP changes. Combined SSEP/MEP monitoring was possible in 69 out of 100 operations. In 49 of the 69 operations (71%), SSEP/ MEP were stable, and the patients remained neurologically intact. Critical SSEP/ MEP changes were seen in six operations. Critical MEP changes with stable SSEP occurred in 12 operations. Overall, critical MEP changes were recorded in 18 operations (26%). In 12 of the 18 operations, MEP recovered to some extent after modification of the surgical strategy, and the patients either showed no (n = 10) or only a transient motor deficit (n = 2). In the remaining six operations, MEP did not recover and the patients either had a transient (n = 3) or a permanent (n = 3) motor deficit. Critical SSEP changes with stable MEP were observed in two operations; both patients did not show a new motor deficit. Our data again confirm that MEP monitoring is superior to SSEP monitoring in detecting impending impairment of the functional integrity of cerebral and spinal cord motor pathways during surgery. Detection of MEP changes and adjustment of the surgical strategy might allow to prevent irreversible pyramidal tract damage. Stable SSEP/MEP recordings reassure the surgeon that motor function is still intact and surgery can be continued safely. The combined SSEP/ MEP monitoring becomes advantageous, if one modality is not recordable.

Objective To determine the normal values (latency, amplitude) of motor evoked potentials (MEP) of the abductor pollicis brevis (APB) and tibialis anterior (TA) muscle after high-frequency repetitive transcranial magnetic stimulation of the motor cortex (rTCMS), and to evaluate stimulation-dependent MEP modulations. Patients and methods 29 healthy volunteers underwent rTCMS with 2 and 4 stimuli. The interstimulus interval (ISI) was 2, 3, and 4 ms respectively, which corresponded to frequencies between 250 and 500 stimuli/s. The evoked potentials of the relaxed and voluntarily contracted APB and TA were registered. Results Depending on the frequency and number of stimuli, the mean corticomuscular latency to the relaxed APB varied between 22.2 and 22.9 ms, and to the relaxed TA between 30.4 and 32.0 ms. The intra- and interindividual variability of the amplitudes was substantial. Voluntary contraction of the target muscle always led to a decrease in latency and increase in amplitude (p Conclusion The high variability of the amplitudes does not allow the computation of meaningful normal values. The latencies after rTCMS are close to those of normal data after single TCMS, which indicates that in awake humans identical cortical and spinal structures are similarly activated. The discrete variations of latency and amplitude after changing the frequency and stimulus number suggest that inhibitory and excitatory mechanisms on the cortical and/or spinal level modulate the muscle response.

Spinal cord blood flow and the induction of heme oxygenase-1 (HO-1), an indicator of oxidative stress, were studied in the spinal cords of adult wild-type and transgenic mice overexpressing the antioxidant copper, zinc superoxide dismutase (CuZn SOD) after intrathecal administration of the potent vasoactive peptide endothelin-1 (ET-1). Gelfoam, saturated with ET-1 (40, 80, or 400 micromol/L), was positioned in the intrathecal space at the midthoracic level in anesthetized animals. Blood flow was continuously monitored by laser Doppler for 10 min after the intrathecal application of ET-1. There was a significant reduction in spinal cord blood flow to approximately 40% of control values by 10 min after the intrathecal application of the peptide in both wild-type and transgenic mice. Moreover, SB209670, a nonselective endothelin receptor antagonist, blocked this reduction in flow. Each animal was euthanized 24 h after the intrathecal administration of ET-1, and the spinal cord was prepared for quantitative immunocytochemistry. HO-1 was primarily induced in astrocytes near the dorsal surface of the spinal cord in wild-type mice. This induction was attenuated in both wild-type, treated with SB209670, and untreated transgenic mice. Together, these findings suggest that ET-1 mediates oxidative stress in the spinal cord through the modulation of spinal cord blood flow.

Background Isoflurane and propofol reduce the recordability of compound muscle action potentials (CMAP) following single transcranial magnetic stimulation of the motor cortex (sTCMS). Repetition of the magnetic stimulus (repetitive transcranial magnetic stimulation, rTCMS) might allow the inhibition caused by anaesthesia with isoflurane or propofol to be overcome. Methods We applied rTCMS (four stimuli; inter-stimulus intervals of 3, 4, 5 ms (333, 250, 200 Hz), output 2.5 Tesla) in 27 patients and recorded CMAP from the hypothenar and anterior tibial muscle. Anaesthesia was maintained with fentanyl 0.5–1 μg kg−1 h−1 and either isoflurane 1.2% (10 patients) or propofol 5 mg kg−1 h−1 with nitrous oxide 60% in oxygen (17 patients). Results No CMAP were detected during isoflurane anaesthesia. During propofol anaesthesia 333 Hz, four-pulse magnetic stimulation evoked CMAP in the hypothenar muscle in 75%, and in the anterior tibial muscle in 65% of the patients. Less response was obtained with 250 and 200 Hz stimulation. Conclusions In most patients, rTCMS can overcome suppression of CMAP during propofol/nitrous oxide anaesthesia, but not during isoflurane anaesthesia. A train of four magnetic stimuli at a frequency of 333 Hz is most effective in evoking potentials from the upper and lower limb muscles. The authors conclude that rTCMS can be used for evaluation of the descending motor pathways during anaesthesia.

OBJECTIVES: Space-occupying cystic lesions may develop in a variable time after resection of particular intracranial tumors, representing a small amount of complications of these procedures. We present our experience with the development and operative treatment of such postoperative cystic lesions in order to identify possible risk factors and to optimize the treatment. METHODS: The records and neuroradiological findings of patients, operated on either gliomas and meningeomas or craniopharyngeomas, who developed symptomatic cystic lesions in the former tumor resection area during the last ten years, were analyzed. RESULTS: 31 patients (2.5%) out of a total of 1240 corresponding tumor operations were identified. The mean age among the 20 female and 11 male patients was 47 years (12-74 years). In 17 patients (55%) the cystic lesion occured within 6 months after tumor resection (mean 5.6 weeks) and in 14 patients (45%) later than 6 months postoperatively (mean 3.6 years). 22 patients (71%) had malignant tumors and 16 patients (52%) had previous radiation therapy. 14 patients (45%) had more than one tumor resection at the same location and one patient had a postoperative meningitis as predisposing factor for the cyst-formation. All patients profited of the various definitive treatment modalities: repetitive percutaneous puncture/external drainage (5 patients), craniotomy for cyst-resection/-fenestration without (5 patients) and with Rickham-catheter implantation (10 patients), endoscopic cyst-fenestration with Rickham-catheter implantation (3 patients) and implantation of cysto-atrial or cysto-peritoneal shunts (8 patients). CONCLUSIONS: Symptomatic cystic lesions developing after brain tumor resection may occur as early - (5.6 weeks) or as late - (3.6 years) complications and though predisposing factors, like malignant primary tumor, preceding radiation therapy and multiple tumor resections can be identified, the reason for their occurrence remains unclear. A variety of effective therapy options is applicable but should consider the patients condition and prognosis.

Objects. Cerebrospinal fluid (CSF) leakages through the operation wound following shunt placement are commonly treated by head elevation when the shunt is patent and signs of infection are absent. Methods and results. In two cases in which the standard therapy of head elevation failed, the leakage was successfully managed by temporary lowering of the opening pressure of the implanted programmable valve. Case 1 was that of a preterm baby with repaired meningomyelocele and shunted hydrocephalus. A shunt infection made shunt removal necessary. Five days after insertion of a new ventriculoperitoneal shunt, transcutaneous CSF flow occurred. The CSF leak was cured by a temporary reduction of the opening pressure from 70 to 30 mmH20. Case 2 was in a 9-month-old child with shunted posthaemorrhagic hydrocephalus. Shunt infection was treated by shunt removal, placement of external ventricular drainage, and antibiotics. CSF leakage through the operation wound developed 24 h after placement of the new ventriculoperitoneal shunt. The CSF leakage was successfully treated by temporarily lowering the opening pressure of the valve from 100 to 50 mmH20. Conclusion. The authors assume that head elevation alone did not generate a sufficient difference between the ventricular and the abdominal pressure for the selected opening pressure of the valve to be exceeded, with the result that CSF flowed around the ventricular catheter and through the operation wound. It is believed that reduction of the opening pressure of the valve led to CSF flow through the shunt and made siphoning possible, resulting in cessation of the transcutaneous CSF flow. The authors propose the prophylactic implantation of a programmable valve in children at high risk of postoperative transcutaneous CSF leakage.

Oxidative stress contributes to secondary injury after spinal cord trauma. Among the consequences of oxidative stress is the induction of heme oxygenase-1 (HO-1), an inducible isozyme that metabolizes heme to iron, biliverdin, and carbon monoxide. Here we examine the induction of HO-1 in the hemisected spinal cord, a model that results in reproducible degeneration in the ipsilateral white matter. HO-1 was induced in microglia and macrophages from 24 h to at least 42 days after injury. Within the first week after injury, HO-1 was induced in both the gray and the white matter. Thereafter, HO-1 expression was limited to degenerating fiber tracts. HSP70, a heat shock protein induced mainly by the presence of denatured proteins, was consistently colocalized with HO-1 in the microglia and macrophages. This study to demonstrates long-term induction of HO-1 and HSP70 in microglia and macrophages after traumatic injury and an association between induction of HO-1 and Wallerian degeneration. White matter degeneration is characterized by phagocytosis of cellular debris and remodeling of surviving tissue. This results in the metabolism, synthesis, and turnover of heme and heme proteins. Thus, sustained induction of HO-1 and HSP70 in microglia and macrophages suggests that tissue degeneration is an ongoing process, lasting 6 weeks and perhaps even longer.

Vascular events after spinal cord injury: contribution to secondary pathogenesis [Physical Therapy 2000;80(7):673-87]
