- Abstract
En este artículo se presenta la evaluación realizada en el Museo Jurásico de Asturias (MUJA) en el periodo 2005/2007 y una segunda desarrollada en 2014. La primera se enfocaba hacia la mejora de la gestión de atención al público, que se había percibido como desorganizada en los primeros meses de la puesta en marcha del museo. La segunda investigación se corresponde con una síntesis de la evaluación realizada en el marco de un proyecto de I+D+i con el que se evalúan programas educativos en quince museos de España bajo el paradigma de la investigación cualitativa. La primera evaluación puede entenderse como un diagnóstico inicial mientras que el trabajo de 2014 trata de interpretar mejoras para la propuesta didáctica, incorporando nuevos planteamientos evaluativos enfocados a las visitas escolares.

Loss of mains detection strategies based on voltage and frequency displacements, passive or active, have been widely applied and conventionally thought for current controlled voltage source inverters. Its effectiveness is questionable when those islanding detection methods are applied on AC voltage droop based inverters. This paper exposes the ineffectiveness of this kind of detection algorithms when are used on AC droop-based inverters type. A power control error based solution, combined with the monitoring of the point of common coupling is proposed as a solution to extend the detection capability. But also, making evident the islanding detection inclusively under the zero power flow concept. A set of simulations supports the analysis and explores the validation scope.

Introducción: la medida de la calidad de vida relacionada con la salud (CVRS) tiene una reconocida importancia en la evaluación, el manejo y el seguimiento de la enfermedad inflamatoria intestinal. El instrumento de medida más utilizado es la versión de 32 ítems del Inflammatory Bowel Disease Questionnaire (IBDQ-32), que no está adaptado al español. Objetivo: traducir la versión del IBDQ-32 al español y determinar su validez, fiabilidad y sensibilidad tanto en la colitis ulcerosa como en la enfermedad de Crohn. Método: estudio prospectivo en dos fases: traducción y posterior validación del IBDQ-32 al español. La traducción se ha basado en la versión al español validada del IBDQ-36, y los ítems del IBDQ-32 no incluidos en el IBDQ-36 se tradujeron "de novo". Una vez terminada la traducción del IBDQ32, se aplicó un cuestionario específico de comprensión. Para determinar las propiedades psicométricas del IBDQ-32, un grupo de pacientes completó el IBDQ-36 ya validado al español y el IBDQ-32. Resultados: se han incluido 84 pacientes (53 con enfermedad de Crohn y 31 con colitis ulcerosa). La mediana de la puntuación global de ambos cuestionarios en los 84 pacientes no fue diferente (6,1 vs. 6,2, p = ns) y su correlación de Spearman fue muy significativa (r = 0,97, p < 0,001). La capacidad de discriminar entre pacientes con brote y remisión también fue equivalente en ambos cuestionarios (6,4 vs. 6,4 con r = 0,96 en remisión y 3,8 vs. 3,9 con r = 0,95 en actividad, p < 0,001 remisión vs actividad). Estos resultados se reprodujeron en el análisis para EC y CU independientemente (6,4 vs. 6,4 y 6,7 vs. 6,6 respectivamente, p = ns). Conclusión: la versión en castellano de 32 ítems del IBDQ es válida y discriminativa para ser aplicada en pacientes con enfermedad de Crohn o colitis ulcerosa. Introduction: the measurement of health-related quality of life (HRQoL) has an established relevance in the assessment, management, and follow-up of inflammatory bowel disease. The most commonly used measuring instrument is the 32-item version of Inflammatory Bowel Disease Questionnaire (IBDQ-32), which has never been adapted to Spanish. Objective: to translate IBDQ-32 into Spanish, and to establish its validity, reliability, and sensitivity both in ulcerative colitis and Crohn's disease. Method: a prospective study in two phases -translation into Spanish and subsequent validation of IBDQ-32. Translation was based on the validated Spanish version of IBDQ-36, and IBDQ-32 items not included in IBDQ-36 were translated from scratch. Once the IBDQ32 translation was completed a comprehension-specific questionnaire was administered. To establish IBDQ-32 psychometric properties a group of patients completed both the validated Spanish version of IBDQ-36 and IBDQ-32. Results: eighty-four patients (53 with Crohn's disease and 31 with ulcerative colitis) were included. Median overall scores in both questionnaires for all 84 patients did not differ (6.1 vs. 6.2, p = ns), and Spearman's correlation was highly significant (r = 0.97, p < 0.001). The ability to discriminate between patients in flare-up or remission was also equivalent for both questionnaires (6.4 vs. 6.4 with r = 0.96 in remission and 3.8 vs. 3.9 with r = 0.95 active, p < 0.001 for remission vs. activity). These results were obtained in both the analyses for CD and UC independently (6.4 vs. 6.4 and 6.7 vs. 6.6, respectively; p = ns). Conclusion: the Spanish version of IBDQ-32 is valid and discriminating for patients with Crohn's disease or ulcerative colitis.

Introducción: se ha sugerido una potencial asociación entre la enfermedad celíaca y la enfermedad inflamatoria intestinal, que puede justificar que ambas enfermedades puedan presentarse en un mismo enfermo o en sus familiares de primer orden con mayor frecuencia de lo esperado. Objetivo: determinar la prevalencia de la enfermedad de Crohn y la colitis ulcerosa en los enfermos celíacos y en sus familiares. Método: estudio epidemiológico prospectivo transversal en un grupo de pacientes celíacos, sus familiares de primer grado y un grupo control de características epidemiológicas similares, constituido por familiares de pacientes que acuden al Servicio de Urgencias por un problema agudo. Para detectar la existencia de colitis ulcerosa y enfermedad de Crohn en los celíacos y sus familiares, se realizó una entrevista semiestructurada. Resultados: se han incluido 86 celíacos y 432 familiares, que se han comparado con 809 controles (129 pacientes con una enfermedad aguda y 680 familiares de primer grado suyos). Se han detectado 3 casos de enfermedad de Crohn en el grupo de los enfermos celíacos y 4 casos de enfermedad de Crohn en sus familiares. Sólo se ha detectado 1 caso de enfermedad de Crohn en el grupo control (p < 0,01). No se ha identificado ningún caso de colitis ulcerosa en ninguno de los tres grupos de estudio. Conclusión: los pacientes con enfermedad celíaca y sus familiares tienen mayor predisposición a presentar una enfermedad de Crohn, que la población control. Introduction: a potential association between celic disease and inflammatory bowel disease hs been suggested, which may explain the fact that both disorders occasionally present in one patient or in his/her first-degree relatives more frequently than expected. Objective: to establish the prevalence of Crohn's disease and ulcerative colitis in celiac patients and their relatives. Method: a cross-sectional, prospective epidemiological study in a group of celiac patients, their first-degree relatives, and a control group with similar epidemiological characteristics including the relatives of patients presenting at the ER for acute conditions. A semistructured interview was used to identify the presence of Crohn's disease and ulcerative colitis in celiac patients and their relatives. Results: in all, 86 celiac patients and 432 relatives were included, who were compared to 809 control subjects (129 patients with acute conditions and 680 first-degree relatives). Three cases of Crohn's disease were identified among celiac patients, and 4 cases among their relatives. Only 1 case of Crohn's disease was detected in the control group (p < 0.01). No cases of ulcerative colitis were detected in any of the study groups. Conclusion: patients with celiac disease and their relatives have a greater predisposition to Crohn's disease versus the control population.

Algunas de las cuestiones que se plantean son cómo evolucionan las Interlenguas (IL), por qué etapas pasan los aprendices en su desarrollo hacia la lengua objeto (LO) y qué mecanismos activan los aprendices en cada una de las etapas del desarrollo; se intenta comprobar también si se observa algún cambio significativo en algún punto del proceso. Con el fin de adoptar una óptica razonada desde la que describir la evolución de las IL en lo que se refiere al objeto de estudio, en la primera parte de este estudio se razona el establecimiento de una caracterización unitaria del pretérito imperfecto que pueda dar cuenta de su interpretación en la máxima cantidad de contextos en los que puede aparecer. Para ello se revisa la propiedad o impropiedad de los valores temporales y aspectuales habitualemente atribuidos al imperfecto, sobre todo por oposición al indefinido, y se ordena su presencia en la estructura de dichas interpretaciones. El estudio de las sucesivas IL se ha realizado fundamentalmente a través del sistemático contraste de las producciones no nativas con las de un grupo de control que, habiéndose sometido a las mismas pruebas que los aprendices, se considera el grupo de control. Este trabajo persigue un fin práctico y otro teórico. En relación con el práctico, la descripción de cómo evolucionan las IL puede ser una aydua para la didáctica, en la medida que pone de manifiesto las áreas de mayor dificultad y permite medir la influencia del orden de presentación y explicación en la clase del funcionamiento de esta parte de la lengua. We also try to check if there is any significant change in any point/step of the process. According to this objectives, first of all we provide a single characterization, the main trait of the Spqanish Imperfect which would explain for the maximum amount of contexts in which it may appear. Therefore, in the first three chapters we study the notions of time and aspect (so little limited in descriptive grammars) so that it would be possible to revise the property or improperty of those values (specially those time and aspect values usually confered to the Spanish Inperfect as opposed to the Spanish Preterite), and also to put an order to their presence in the structure of the mentioned interpretation. In this way, we reason out a vision from which is possible to describe the development of the non native output. This study of the consecutive IL was bassically carried out through the systematic contrast of the non-native output with those productions from a group of native speakers, who had been tested in the same way as the leaners, and has considered as a checking group.

- Abstract
Chronic pancreatitis tends to develop a number of complications that may constitute the form of presentation of the disease. Some societies have issued guidelines for diagnosis and treatment of chronic pancreatitis complications, but the level of evidence for any topic is usually low and recommendations tend to be weak. We aimed to provide defined position statements for the clinician based on updated review of published literature and on multidisciplinary expert agreement. The goal was to propose defined terminology and rational diagnostic/therapeutic circuits based on current knowledge. To this end 14 sections related to complications and special forms of chronic pancreatitis (early chronic, groove and autoimmune pancreatitis) were reviewed by 21 specialists from 6 different fields to generate 32 statements. Featured statements assert common bile duct stenosis does not require invasive treatment (endoscopic or surgical) unless cholestasis, cholangitis, lithiasis or other symptoms develop. Pancreatic duct strictures and calculi should be approached (after ruling out malignancy) if causing pain, pancreatitis, pseudocysts or other complications. Treatment of symptomatic pseudocysts must be individualized, considering associated main duct stenosis, vascular and pericystic complications. Higher risk conditions for pancreatic cancer are advance age, smoking, genetic background, recent diagnosis of chronic pancreatitis or diabetes, and appearance of new symptoms. Groove pancreatitis can initially be treated with conservative measures. Both prednisolone or rituximab can induce remission and maintenance of autoimmune pancreatitis. Internal fistula, vascular complications, bacterial overgrowth, osteoporosis and renal lithiasis require specific therapeutic approaches.

- Abstract
Background: Periarterial divestment is a surgical technique to approach borderline resectable (BR) or locally advanced (LA) pancreatic ductal adenocarcinoma (PDAC) with arterial involvement. There are no reports in the literature regarding the role of endoscopic ultrasound and elastography (EUS-EG) in exploring the integrity of Inoue's level III and its correlation with the periarterial divestment technique feasibility. Our research is aimed at exploring the role of EUS-EG in this scenario., Methods: We describe our approach to Inoue's level II by EUS-EG in patients with BR and LA pancreatic cancer patients after neoadjuvant chemotherapy., Results: Between June 2019 and December 2020, four patients out of 25 were eligible to perform a preoperative EUS-EG. In all cases, Inoue's level III integrity was corroborated by EUS-EG and confirmed posteriorly in the surgical scenario where a periarterial divestment technique was feasible. Vein resections were necessary in all cases, with no need for arterial resection. An R0 (> 1 mm) margin was achieved in all patients, and the histopathological assessment showed the presence of neurovascular tissue at the peripheral arterial margin., Conclusion: Preoperatively, EUS-EG is a novel approach to explore the integrity of Inoue's level III and could be helpful to preclude a periarterial divestment technique in borderline resectable or locally advanced pancreatic adenocarcinoma with arterial involvement., (© 2023. - Abstract
Chronic pancreatitis is associated with impaired quality of life, high incidence of comorbidities, serious complications and mortality. Healthcare costs are exorbitant. Some medical societies have developed guidelines for treatment based on scientific evidence, but the gathered level of evidence for any individual topic is usually low and, therefore, recommendations tend to be vague or weak. In the present position papers on chronic pancreatitis from the Societat Catalana de Digestologia and the Societat Catalana de Pàncrees we aimed at providing defined position statements for the clinician based on updated review of published literature and on multidisciplinary expert agreement. The final goal is to propose the use of common terminology and rational diagnostic/therapeutic circuits based on current knowledge. To this end 51 sections related to chronic pancreatitis were reviewed by 21 specialists from 6 different fields to generate 88 statements altogether. Statements were designed to harmonize concepts or delineate recommendations. Part 2 of these paper series discuss topics on treatment and follow-up. The therapeutic approach should include assessment of etiological factors, clinical manifestations and complications. The complexity of these patients advocates for detailed evaluation in multidisciplinary committees where conservative, endoscopic, interventional radiology or surgical options are weighed. Specialized multidisciplinary units of Pancreatology should be constituted. Indications for surgery are refractory pain, local complications, and suspicion of malignancy. Enzyme replacement therapy is indicated if evidence of exocrine insufficiency or after pancreatic surgery. Response should be evaluated by nutritional parameters and assessment of symptoms. - Abstract
Chronic pancreatitis is a chronic fibroinflammatory disease of the pancreas with prevalence around 50 cases per 100,000 inhabitants. It appears to originate from diverse and yet mixed etiological factors. It shows highly variable presenting features, complication types and disease progression rates. Treatment options are as wide as the multiple personalized scenarios the disease might exhibit at a given time point. Some medical societies have developed guidelines for diagnosis and treatment based on scientific evidence. Although these efforts are to be acknowledged, the gathered level of evidence for any topic is usually low and, therefore, recommendations tend to be vague or weak. In the present series of position papers on chronic pancreatitis from the Societat Catalana de Digestologia and the Societat Catalana de Pàncrees we aimed at providing defined position statements for the clinician based on updated review of published literature and on interdisciplinary expert agreement. The final goal is to propose the use of common terminology and rational diagnostic/therapeutic circuits based on current knowledge. To this end 51 sections related to chronic pancreatitis were reviewed by 21 specialists from 6 different fields to generate 88 statements altogether. Statements were designed to harmonize concepts or delineate recommendations. Part 1 of this paper series discusses topics on aetiology and diagnosis of chronic pancreatitis. Main clinical features are abdominal pain, exocrine and endocrine insufficiency and symptoms derived from complications. Some patients remain symptom-free. - Abstract
Anisakiosis is a fish-borne disease with gastrointestinal and/or allergic symptoms caused by the consumption of raw or undercooked fish parasitized with nematode larvae of the genus Anisakis . In Europe, Anisakis pegreffii has been detected as the causative agent, although the sibling species Anisakis simplex sensu stricto (s.s.) is also known to cause the disease in other parts of the world, and discrepancies exist regarding their respective pathogenic potential. In Spain a high number of cases has been recorded, with marinated anchovies being the main source of infection, although no specific diagnosis has been documented in humans. In this study, we analyzed three cases of anisakiosis in patients from Barcelona (Spain) who had consumed undercooked hake. All patients described epigastric pain and several larval nematodes were removed endoscopically from their stomachs. Larvae were morphologically characterized as third-stage larvae of Anisakis simplex sensu lato (s.l.) and molecularly identified as A. simplex (s.s.) by means of PCR RFLP of the ITS region of the rDNA and sequencing of the elongation factor1 alpha1 (EF1 α-1) nDNA gen. This study represents the first specific identification of Anisakis larvae in clinical cases of anisakiosis reported in Spain. Specific molecular diagnosis is of crucial importance for assessing the health risk of Anisakis sibling species. Hake consumption stands out as a risk factor for anisakiosis, since this fish species can be highly parasitized.

- Abstract
Background: Investigations indicate that natural orifice translumenal endoscopic surgery (NOTES) procedures induce a less pronounced postoperative inflammatory response than open or laparoscopic surgery, inflicting less trauma. In NOTES procedures, no skin incision is performed. We compare the inflammatory response added by the type of incision by measuring C-reactive protein (CRP) and tumors necrosis factor-alfa (TNF-α)., Methods: Twenty-seven pigs were randomized to open surgical, laparoscopic, or transgastric NOTES abdominal access. After completion of the accesses, no surgery was performed. All accesses were left open for 40 minutes followed by closure, animals were survived for 7 days. Blood samples were drawn at the start of the accesses, at 20 and 40 minutes during the procedure, and at postoperative day (POD) 1, 3, and 7. Analyses of CRP and TNF-α were performed., Results: CRP increased in all animals until POD1. This increase was greater in the open group (P = .006). No significant differences in CRP-levels were found at POD 1, 3, or 7. TNF-α showed a peak during the procedure, at 20 and 40 minutes, with normalization at POD1 for 1/3 of the open and laparoscopic animals, but not for the NOTES animals. Due to variations within the groups, no statistical difference was shown between them. At postmortem, 1/3 of the pigs in the laparoscopic and open groups had wound infections, while no NOTES animals showed infections., Conclusions: This study provides no statistically significant differences in inflammatory response after the different abdominal accesses. However, the lack of a TNF-α-peak in the NOTES group might indicate a less pronounced response, supporting the initial theories.

Background: Crohn's disease (CD) and ulcerative colitis (UC) are chronic immunoinflammatory diseases that place a considerable burden on patients, their families, and society. Quality of care plays an important role for patients. A questionnaire to measure quality of care through the eyes of patients with inflammatory bowel disease (QUOTE-IBD) has been designed and validated specifically for the English language and culture. The objective was to translate the QUOTE-IBD into Spanish and to determine its validity in patients with IBD., Methods: This is a prospective study in 2 phases: first, translation and validation of the Spanish QUOTE-IBD. Translation was based on the validated QUOTE-IBD. Second, once the complete translation was finished, comprehension of the items was assessed with a specific questionnaire in a reduced number of patients. Criterion validity was assessed with the Pearson's correlation coefficient between scores of the QUOTE-IBD and visual analog scales (VAS). In order to analyze the reproducibility of the Spanish QUOTE-IBD, the questionnaire was completed by stable patients twice, with a span of time of at least 4 weeks., Results: A total of 103 patients (CD: 61, UC: 42) were included in the study. Pearson's correlation coefficient between total care Spanish QUOTE-IBD and VAS of health care items was 0.34 (P < 0.001). Correlations among all 6 care dimensions score of Spanish QUOTE-IBD and VAS were statistically significant (P < 0.01). Results of first and second administration of total care and dimensional care scores of Spanish QUOTE-IBD in 46 stable patients were not different., Conclusions: The Spanish QUOTE-IBD has proved to be a valid instrument to measure the quality of health care for patients with CD and UC.

Introduction: The measurement of health-related quality of life (HRQoL) has an established relevance in the assessment, management, and follow-up of inflammatory bowel disease. The most commonly used measuring instrument is the 32-item version of Inflammatory Bowel Disease Questionnaire (IBDQ-32), which has never been adapted to Spanish., Objective: To translate IBDQ-32 into Spanish, and to establish its validity, reliability, and sensitivity both in ulcerative colitis and Crohn s disease., Method: A prospective study in two phases -translation into Spanish and subsequent validation of IBDQ-32. Translation was based on the validated Spanish version of IBDQ-36, and IBDQ-32 items not included in IBDQ-36 were translated from scratch. Once the IBDQ32 translation was completed a comprehension-specific questionnaire was administered. To establish IBDQ-32 psychometric properties a group of patients completed both the validated Spanish version of IBDQ-36 and IBDQ-32., Results: Eighty-four patients (53 with Crohn s disease and 31 with ulcerative colitis) were included. Median overall scores in both questionnaires for all 84 patients did not differ (6.1 vs. 6.2, p = ns), and Spearman s correlation was highly significant (r = 0.97, p < 0.001). The ability to discriminate between patients in flare-up or remission was also equivalent for both questionnaires (6.4 vs. 6.4 with r = 0.96 in remission and 3.8 vs. 3.9 with r = 0.95 active, p < 0.001 for remission vs. activity). These results were obtained in both the analyses for CD and UC independently (6.4 vs. 6.4 and 6.7 vs. 6.6, respectively; p = ns)., Conclusion: The Spanish version of IBDQ-32 is valid and discriminating for patients with Crohn s disease or ulcerative colitis.

Introduction: A potential association between celic disease and inflammatory bowel disease hs been suggested, which may explain the fact that both disorders occasionally present in one patient or in his/her first-degree relatives more frequently than expected., Objective: To establish the prevalence of Crohn s disease and ulcerative colitis in celiac patients and their relatives., Method: A cross-sectional, prospective epidemiological study in a group of celiac patients, their first-degree relatives, and a control group with similar epidemiological characteristics including the relatives of patients presenting at the ER for acute conditions. A semistructured interview was used to identify the presence of Crohn s disease and ulcerative colitis in celiac patients and their relatives., Results: In all, 86 celiac patients and 432 relatives were included, who were compared to 809 control subjects (129 patients with acute conditions and 680 first-degree relatives). Three cases of Crohn s disease were identified among celiac patients, and 4 cases among their relatives. Only 1 case of Crohn s disease was detected in the control group (p < 0.01). No cases of ulcerative colitis were detected in any of the study groups., Conclusion: Patients with celiac disease and their relatives have a greater predisposition to Crohn s disease versus the control population.
