Hidradenitis suppurativa (HS) is a chronic inflammatory disease that affects the axilla, inframammary folds, buttocks, inner thighs, and anogenital regions. Patients with moderate to severe HS often seek care in the emergency department and may require hospitalization; however, the lack of clear admission criteria and management guidelines presents significant challenges. To address these issues, we conducted a clinical review aimed at hospitalists and consulting dermatologists considering hospital admission for patients experiencing severe HS flares. Admission offers a unique opportunity to optimize care through specialized consultations, pain management, rescue therapy, and surgical planning. Furthermore, multimodal treatments, particularly biologics, are needed to achieve clinical remission in severe disease. Streamlining care during hospitalization for the early initiation of these therapies can significantly improve flare management and overall clinical outcomes for HS patients. Pressure injuries are a significant comorbidity and lead to increased overall healthcare costs. Several European and global studies have assessed the burden of pressure injuries; however, no comprehensive analysis has been completed in the United States. In this study, we investigated the trends in the burden of pressure injuries among hospitalised adults in the United States from 2009 to 2019, stratified by sociodemographic subgroups. The length of admission, total cost of hospitalisation, and sociodemographic data was extracted from the National Inpatient Sample provided by the Healthcare Cost and Utilisation Project, Agency for Healthcare Research and Quality. Overall, the annual prevalence of pressure injuries and annual mean hospitalisation cost increased ($69,499.29 to $102,939.14), while annual mean length of stay decreased (11.14–9.90 days). Among all races, minority groups had higher average cost and length of hospitalisation. Our findings suggest that while the length of hospitalisation is decreasing, hospital costs and prevalence are rising. In addition, differing trends among racial groups exist with decreasing prevalence in White patients. Further studies and targeted interventions are needed to address these differences, as well as discrepancies in racial groups. [ABSTRACT FROM AUTHOR]

Autoimmune connective tissue disorders, including systemic lupus erythematosus, systemic sclerosis (SSc) and dermatomyositis (DM), often manifest with debilitating cutaneous lesions and can result in systemic organ damage that may be life‐threatening. Despite recent therapeutic advancements, many patients still experience low rates of sustained remission and significant treatment toxicity. While genetic predisposition plays a role in these connective tissue disorders, the relatively low concordance rates among monozygotic twins (ranging from approximately 4% for SSc to about 11%–50% for SLE) have prompted increased scrutiny of the epigenetic factors contributing to these diseases. In this review, we explore some seminal studies and key findings to provide a comprehensive understanding of how dysregulated epigenetic mechanisms can contribute to the development of SLE, SSc and DM. This article discusses the potential use of psychiatric medications for the treatment of atopic dermatitis (AD), a chronic inflammatory skin disease. AD is associated with psychological distress and mental health comorbidities, which can worsen the quality of life for patients. The article presents a subanalysis of a systematic review that examined the effectiveness of tricyclic antidepressants, selective serotonin reuptake inhibitors (SSRIs), and mirtazapine in reducing chronic itch and improving AD skin lesions. The study found that SSRIs, such as paroxetine and fluvoxamine, showed promise in reducing itch in AD patients, while escitalopram and bupropion-SR also demonstrated positive effects on skin lesions. Objective: This systematic study examines the quality of hair loss information disseminated on social media platforms, as well as its potential effects on users' decision-making, emotional states, and societal attitudes toward hair loss., Methods: A thorough search of MEDLINE and PubMed databases was conducted until May 13th, 2023 to identify research into the role of social media in disseminating hair loss knowledge., Results: The systematic review encompasses 11 studies investigating platforms such as Facebook, Twitter, Instagram, TikTok, and YouTube. We discovered significant variations in the quality of hair loss-related information on social media. The DISCERN scores ranged from 1.47 to 2.66, while the percentage of videos/posts created by dermatologists ranged from 0 to 18.9%. While certain reliable content from medical professionals was present, there was an overabundance of personal anecdotes, unproven remedies, and recommendations based on personal experiences. The potential impacts of such information are wide-ranging, potentially influencing user decision-making and societal perceptions. Misinformation and subpar content could also heighten distress and anxiety among individuals seeking hair loss solutions., Conclusion: This review highlights the need for enhanced content quality on social media platforms. Diabetic foot ulcers (DFUs) are a prevalent complication of diabetes mellitus (DM) and lead to significant morbidity and mortality. Patients with DM have a lifetime risk of DFUs as high as 34%. The pathogenesis of DFUs is multifactorial, and the most common underlying causes are poor glycemic control, peripheral neuropathy, peripheral vascular disease, foot deformity, and poor foot care. Diabetic lower-extremity complications are also a significant burden in terms of healthcare costs. In the United States alone, the direct cost of diabetic foot care has been estimated to be $8,659 per patient, with total annual medical costs for managing diabetic foot disease ranging from $9 to $13 billion. Given the risk of amputation and poor wound healing, the fast, accurate diagnosis and treatment of DFUs are critical. Measures to prevent DFUs include glycemic control and annual foot inspections. For patients with DFUs, off-loading and local wound care are critical for wound healing. Debridement is the standard of care for DFU wounds, and several techniques exist. In this review, we discuss the current practices of diabetic wound care, different methods of debridement and their practical use in DFUs, and novel debridement approaches with the potential for improving wound-healing outcomes.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disorder that is characterized by painful nodules, abscesses and tunnels in skin folds. Current management of HS often requires a multidisciplinary approach, including medical, procedural, surgical, and psychosocial intervention. Here we review surgical techniques for the treatment of HS. While many surgical options are available for patients with HS, it is imperative that surgical planning focuses on medical optimization, patient risk factors, disease severity, and patient preferences to achieve the best outcomes.
