Abstract

Background: The association between volume, complications and pathological outcomes is still under debate regarding colorectal cancer surgery. The aim of the study was to assess the association between centre volume and severe complications, mortality, less-than-radical oncologic surgery, and indications for neoadjuvant therapy. Methods: Retrospective analysis of 16,883 colorectal cancer cases from 80 centres (2018-2021). Outcomes: 30-day mortality; Clavien-Dindo grade >2 complications; removal of ≥ 12 lymph nodes; non-radical resection; neoadjuvant therapy. Quartiles of hospital volumes were classified as LOW, MEDIUM, HIGH, and VERY HIGH. Independent predictors, both overall and for rectal cancer, were evaluated using logistic regression including age, gender, AJCC stage and cancer site. Results: LOW-volume centres reported a higher rate of severe postoperative complications (OR 1.50, 95% c.i. 1.15-1.096, P = 0.003). The rate of ≥ 12 lymph nodes removed in LOW-volume (OR 0.68, 95% c.i. 0.56-0.85, P < 0.001) and MEDIUM-volume (OR 0.72, 95% c.i. 0.62-0.83, P < 0.001) centres was lower than in VERY HIGH-volume centres. Of the 4676 rectal cancer patients, the rate of ≥ 12 lymph nodes removed was lower in LOW-volume than in VERY HIGH-volume centres (OR 0.57, 95% c.i. 0.41-0.80, P = 0.001). A lower rate of neoadjuvant chemoradiation was associated with HIGH (OR 0.66, 95% c.i. 0.56-0.77, P < 0.001), MEDIUM (OR 0.75, 95% c.i. 0.60-0.92, P = 0.006), and LOW (OR 0.70, 95% c.i. 0.52-0.94, P = 0.019) volume centres (vs. VERY HIGH). Conclusion: Colorectal cancer surgery in low-volume centres is at higher risk of suboptimal management, poor postoperative outcomes, and less-than-adequate oncologic resections. Centralisation of rectal cancer cases should be taken into consideration to optimise the outcomes.

Abstract

Selective chemical preconditioning of geometrically patterned substrates and parts is necessary to enable capillary part-to-substrate self-assembly. On the other hand, long preparation procedures may preclude the throughput enhancement potentially brought by the technique. In this paper, we investigate the fast chemical preconditioning of patterned substrates by the chemisorption of thiolates from the liquid phase. We found that even under conservative conditions, deposition times as short as 15 s (i.e., about two orders of magnitude shorter than previously reported in specific literature) are sufficient for the intended purpose, which is changing the wetting character of metallic binding sites enough to allow their selective dip-coating with hydrophobic fluid lenses. Moreover, perfect fluid coating conformality is achieved when such short deposition times are combined with the use of recessed binding sites. Our findings further confirm organic species to be remarkable tools for the chemical preconditioning of patterned surfaces as they may help actualize the full benefits of capillary self-assembly for electronic packaging. Abstract

This paper describes the microfabrication and electrical characterization of aluminum-coated superconducting through-silicon vias (TSVs) with sharp superconducting transition above 1 K. The sharp superconducting transition was achieved by means of fully conformal and void-free DC-sputtering of the TSVs with Al, and is here demonstrated in up to 500μ m-deep vias. Full conformality of Al sputtering was made possible by shaping the vias with a tailored hourglass profile, which allowed a metallic layer as thick as 430 nm to be deposited in the center of the vias. Single-via electric resistance as low as 160 mΩ at room temperature and superconductivity at 1.27 K were measured by a three-dimensional (3D) cross-bridge Kelvin resistor structure. This work establishes a CMOS-compatible fabrication process suitable for arrays of superconducting TSVs and 3D integration of superconducting silicon-based devices. Abstract

We present a wafer-scale fabricated, PDMS-based platform for culturing miniaturized engineered heart tissues (EHTs) which allows highly accurate measurements of the contractile properties of these tissues. The design of the platform is an anisometrically downscaled version of the Heart-Dyno system, consisting of two elastic micropillars inside an elliptic microwell with volume ranging from 3 down to 1μL which supports EHT formation. Size downscaling facilitates fabrication of the platform and makes it compatible with accurate and highly reproducible batch wafer-scale processing; furthermore, downscaling reduces the cost of cell cultures and increases assay throughput. After fabrication, the devices were characterized by nanoindentation to assess the mechanical properties of the pillars and transferred to 96-well plates for cell seeding. Regardless the size of the platform, cell seeding resulted in successful formation of EHTs and all tissues were functionally active (i.e. showed cyclic contractions). The precise characterization of the stiffness of the micropillars enabled accurate measurements of the contractile forces exerted by the cardiac tissues through optical tracking of micropillar displacement. Abstract

We describe a microfabrication process that, thanks to a specifically tailored sidewall profile, enables for the first-time wafer-scale arrays of high-aspect ratio through-silicon vias (TSVs) coated with DC-sputtered Aluminum, achieving at once superconducting and CMOS-compatible 3D interconnects. Void-free conformal coating of up to 500μm-deep and 50μm-wide vias with a mere 2μm-thick layer of Al, a widely available metal in for IC manufacturing, was demonstrated. Single-via electric resistance as low as 468 mΩ at room temperature and superconductivity at 1.25 K were measured by a cross-bridge Kelvin resistor structure. Abstract

This paper presents the fabrication and electrical characterization of superconducting high-aspect ratio through-silicon vias DC-sputtered with aluminum. Fully conformal and void-free coating of 300 μm-deep and 50 μmwide vias with Al, a CMOS-compatible and widely available superconductor, was made possible by tailoring a funneled sidewall profile for the axisymmetric vias. Single-via electric resistance as low as 80.44 mΩ at room temperature and superconductivity below 1.28 K were measured by a crossbridge Kelvin resistor structure. Abstract

In this work, we present the fabrication technology of a monolithically integrated photonic platform combining key components for optical coherence tomography (OCT) imaging, thereby including a photonic interferometer, a collimating lens, and a 45◦ reflecting mirror that directs the light from the interferometer to the collimator. The proposed integration process simplifies the fabrication of an interferometric system and inherently overcomes the complexity of costly alignment procedures while complying with the necessarily stringent optical constraints. Fabricated waveguide characterization shows total optical losses as low as 3 dB, and less than 1 dB of additional loss due to the Si 45◦ mirror facet. The alignment standard deviation of all components is within 15 nm. The integrated lens profile achieves a divergence angle smaller than 0.7◦, which is close to that of a collimator. Abstract

We present an extremely compact field effect transistor (FET)-based electrochemical sensor for in situ real-time and label-free measurement of ion concentrations in the cell culture area of organs-on-chip (OoCs) devices. This sensor replaces the functionality of an external reference electrode, crucial in standard electrochemical sensing, by controlling the FET threshold voltage via a capacitive control gate. The silicon- and polymer-based charge sensor can be integrated in OoC platforms by means of a wafer-scale and CMOS-compatible microfabrication process. This fabrication approach inherently allows a superior level of accuracy, repeatability and scalability compared to common OoC manufacturing methods. The sensor combines in a single device the complementary benefits of silicon-based electronics and of flexible polymer membranes with integrated microelectrodes – congenial substrates to sustain dynamic stimuli and mimic physiological tissue microenvironments. Abstract

Organ-on-chip (OoC) devices are in rising demand for high-throughput and low-cost development and toxico-logical screening of chemicals and pharmaceuticals, as they accurately mimic in vitro physiological conditions as in the human body. In particular, OoCs are urgently needed for screening cardiovascular drug toxicity. Physiological relevance of cardiovascular cell cultures requires moving substrates. To date cell culture substrates have been commonly actuated by pneumatic systems, which are bulky, expensive and non-user-friendly, and may thus limit the adoption of OoCs by researchers and industry. In this paper we propose the first actuating and sensing smart material-based OoC device and demonstrate its functionality by culturing human vascular smooth muscle cells (vSMC). Our device utilizes a single ionic polymer metal composite (IPMC)-based transducer to provide both actuation and sensing. The soft IPMC substrate allows to intermittently apply cyclic loading to tissues and to sense their spontaneous contractions. We integrated the transducer within a compact, easy-to-operate, economically affordable and scalable OoC prototype, which achieves an actuation range of 0.2 mm and 0.72 V/mm sensing resolution. The 0.1 % strain induced by actuation on cells accurately corresponds to in vitro strains for vSMCs. We successfully grew vSMCs on the IPMC substrate, and actuated them for 150 min at 1 Hz. Fluorescent staining showed no evidence of adverse effects. Abstract

Self-assembly is a ubiquitous process that can generate complex and functional structures via local interactions among a large set of simpler components. The ability to program the self-assembly pathway of component sets elucidates fundamental physics and enables alternative competitive fabrication technologies. Reprogrammability offers further opportunities for tuning structural and material properties but requires reversible selection from multistable self-assembling patterns, which remains a challenge. Here, we show statistical reprogramming of two-dimensional (2D), noncompact self-assembled structures by the dynamic confinement of orbitally shaken and magnetically repulsive millimeter-scale particles. Under a constant shaking regime, we control the rate of radius change of an assembly arena via moving hard boundaries and select among a finite set of self-assembled patterns repeatably and reversibly. By temporarily trapping particles in topologically identified stable states, we also demonstrate 2D reprogrammable stiffness and three-dimensional (3D) magnetic clutching of the self-assembled structures. Our reprogrammable system has prospective implications for the design of granular materials in a multitude of physical scales where out-of-equilibrium self-assembly can be realized with different numbers or types of particles. Abstract

The extent to which one can use a thermodynamic description of turbulent flow as a source of stochastic kinetic energy for 3D self‐assembly of magnetically interacting macroscopic particles is investigated. It is confirmed that the speed of the objects in the flow field generated in this system obeys the Maxwell–Boltzmann distribution, and their random walk can be defined by a diffusion coefficient following from the Einstein relation. However, it is discovered that the analogy with Brownian dynamics breaks down when considering the directional components of the velocity. For the vectorial components, neither the equipartition theorem nor the Einstein relation is obeyed. Moreover, the kinetic energy estimated from the random walk of individual objects is one order of magnitude higher than the value estimated from Boltzmann statistics on the interaction between two spheres with embedded magnets. Abstract

A novel, simple, low-cost method for the void-free filling of high aspect ratio (HAR) through-silicon-vias (TSVs) is presented. For the first-time pure indium, a type-I superconductor metal, is used to fill HAR vias, 300 to 500 μm in depth and 50 to 100 μm in diameter. The low electrical resistivity achieved without sintering, its reproducibility and straightforward processing steps, and the short time required to fill large arrays of vias at wafer scale - all make this method one of the simplest and quickest options for filling HAR TSVs for MEMS 3D integration. Abstract

We present a pick-and-place approach driven by capillarity for highly precise and cost-effective assembly of mesoscopic components onto structured substrates. Based on competing liquid bridges, the technology seamlessly combines programmable capillary grasping, handling, and passive releasing with capillary self-alignment of components onto prepatterned assembly sites. The performance of the capillary gripper is illustrated by comparing the measured lifting capillary forces with those predicted by a hydrostatic model of the liquid meniscus. Two component release strategies, based on either axial or shear capillary forces, are discussed and experimentally validated. The release-and-assembly process developed for a continuously moving assembly substrate provides a roll-to-roll-compatible technology for high-resolution and high-throughput component assembly.

36. Modeling capillary forces for large displacements

Abstract

Originally applied to the accurate, passive positioning of submillimetric devices, recent works proved capillary self-alignment as effective also for larger components and relatively large initial offsets. In this paper, we describe an analytic quasi-static model of 1D capillary restoring forces that generalizes existing geometrical models and extends the validity to large displacements from equilibrium. The piece-wise nature of the model accounts for contact line unpinning singularities ensuing from large perturbations of the liquid meniscus and dewetting of the bounding surfaces. The superior accuracy of the generalized model across the extended displacement range, and particularly beyond the elastic regime as compared to purely elastic models, is supported by finite element simulations and recent experimental evidence. Limits of the model are discussed in relation to the aspect ratio of the meniscus, contact angle hysteresis, tilting and self-alignment dynamics.

This paper introduces a new integration technology for cost-effective high-precision mechanical and electrical integration of mesoscopic functional foil components onto foil substrates. The foil-to-foil assembly process is based on topological surface structuring via laser patterning that enables accurate capillarity-driven self-alignment of foil dies. The concurrent establishment of high-yield electrical interconnections is obtained through conductive adhesives. The foil surface energy controls the acceptance window of initial offsets for optimal self-alignment performance. The proposed topological patterning and system design enable alignment accuracies for centimeter-sized foil dies as high as 15 µm, barely influenced by the evaporation of the assembly liquid and curing of the conductive paste. Full foil-to-foil system integration is demonstrated through the electrically functional assembly of an array of Au-sputtered capacitive humidity sensors onto a patterned base foil circuitry. Abstract

International audience; Self-assembly and self-alignment driven by capillary meniscus forces are presently at the core of many important technological applications, including solder bonding in flip-chip assembly and fluidic self-assembly for microelectronic packaging. Lateral capillary meniscus forces were object of substantial theoretical and numerical modeling in recent years. Anyway, these studies were unsatisfactorily supported by direct experimental investigations. In this paper we present a comprehensive study of lateral capillary forces of cylindrical menisci, where experimental, analytical and numerical analyses of the same physical system are compared. We describe the conceptually simple experimental apparatus we designed to investigate lateral forces arising from small perturbations of cilindrical liquid menisci. The apparatus allowed controlling all physical and geometrical parameters relevant to the experiments. We then reproduce our experimental data with a novel analytical model of lateral fluid meniscus forces and with finite element simulations. The remarkable agreement between our experiments and models, while confirming earlier reports, provides a solid foundation for all applications of lateral capillary forces. Moreover, our experimental apparatus may be used as testbed for further experimental investigations of confined fluid menisci.

Abstract

We present an experimental study of the complete in-plane dynamics of capillary self-alignment. The two translational (shift) and single rotational (twist) in-plane modes of square millimetric transparent dies bridged to shape-matching receptor sites through a liquid meniscus were selectively excited by preset initial offsets. The entire self-alignment dynamics was simultaneously monitored over the three in-plane degrees of freedom by high-speed optical tracking of the alignment trajectories. The dynamics of the twist mode is shown to qualitatively follow the sequence of dynamic regimes also observed for the shift modes, consisting of initial transient wetting, acceleration toward, and underdamped harmonic oscillations around the equilibrium position. Systematic analysis of alignment trajectories for individually as well as simultaneously excited modes shows that, in the absence of twist offset, the dynamics of the degenerate shift modes are mutually independent. In the presence of twist offset, the three modes conversely evidence coupled dynamics, which is attributed to a synchronization mechanism affected by the wetting of the bounding surfaces. The experimental results, justified by energetic, wetting, and dynamic arguments, provide substantial benchmarks for understanding the full dynamics of the process.

Abstract

We report experimental evidence for three sequential, distinct dynamic regimes in the capillary self-alignment of centimeter-sized foil dies released at large uniaxial offsets from equilibrium. We show that the initial transient wetting regime, along with inertia and wetting properties of the dies, significantly affect the alignment dynamics including the subsequent constant acceleration and damped oscillatory regimes. An analytical force model is proposed that accounts for die wetting and matches quasi-static numerical simulations. Discrepancies with experimental data point to the need for a comprehensive dynamical model to capture the full system dynamics. (C) 2013 American Institute of Physics.
