To assess the analgesia and side effects of 10 mg oxycodone as compared to 30 mg of codeine phosphate associated with 500 mg of paracetamol after bilateral lower third molar extraction.This is a prospective, randomized, double-blind study applied to a sample of 16 patients. They were evaluated for seven days postoperatively, and the mean score of the visual analogue scale (VAS) of pain between test and control medications was assessed by the Wilcoxon distribution. The side effects of these medications were assessed by the Q Cochran test. AThe mean score of the VAS of pain was higher in the oxycodone side, where few patients reported the use of rescue analgesic. There was no report of rescue medication in codeine phosphate associated with paracetamol side. The most common side effects reported in both groups, predominantly in patients using the oxycodone, were drowsiness, dizziness, and headache.The use of codeine phosphate associated with paracetamol after the extraction of impacted mandibular third molars is a better choice than oxycodone for controlling postoperative pain.RBR-8ntwmyq 07/07/2021.

Introdução: Os fios de sutura são importantes substratos de adesão microbiana em cirurgias bucais. Objetivo: Comparar a adesão bacteriana em suturas de seda e de nylon quando impregnadas ou não por uma pomada antisséptica à base de iodofórmio e calêndula. Métodos: Foi realizado um ensaio clínico randomizado, duplo-cego, prospectivo, boca dividida. Após exodontias em uma das hemiarcadas, foi realizada uma sutura simples com o fio de nylon (grupo A) e outra sutura com o fio de seda (grupo B), ambos impregnados com a pomada antisséptica. No mesmo procedimento cirúrgico, foram realizadas as exodontias na hemiarcada contralateral e posterior sutura simples com o fio de nylon (grupo C) e outra com o fio de seda (grupo D) sem a pomada antisséptica. O teste t de Student comparou as médias das unidades formadoras de colônia (UFCs), enquanto o teste de Friedman analisou a distribuição da adesão bacteriana em cada grupo. Resultados: A média de adesão bacteriana foi maior na sutura de seda com a pomada antisséptica (M=5,7288log/UFC/mL) e menor na sutura de nylon sem a pomada (M=4,0587log/UFC/mL). Conclusão: Apesar da composição antisséptica da pomada, nota-se que sua impregnação, tanto ao fio de seda quanto ao fio de nylon, não reduziu a adesão bacteriana nas suturas.

Introdução: os fabricantes de brocas cirúrgicas não informam a durabilidade dos materiais após usos sucessivos, dificultando a escolha do instrumental mais eficaz pelo cirurgião-dentista. Objetivo: comparar a precisão e a perda de corte de duas brocas de carbeto de tungstênio em odontoseções de terceiros molares inferiores. Métodos: a odontosecção foi realizada em vinte pares de dentes homólogos, sendo feita com a broca Zecrya em um dente, e com a broca Talon 12 Surg no dente contralateral. A precisão e a perda de corte foram analisadas, respectivamente, por meio da espessura da odontosecção e dos desgastes na ponta ativa. To assess the analgesia and side effects of codeine phosphate associated with paracetamol (test medication) as compared to paracetamol (control medication) after the extraction of impacted mandibular third molars. Forty-seven patients removed the right and left impacted mandibular third molars. After one surgery, patients took the test medication and after the other surgery, they took the control medication. Patients with exacerbated pain were prescribed to use the rescue medication instead of the medication initially administered and were included in the rescue group. They were evaluated for 7 days postoperatively, and the mean score of the visual analogue scale (VAS) of pain between test and control medications was assessed by the Poisson distribution. INTRODUÇÃO: A impacção do incisivo central superior é uma condição desafiadora para os cirurgiões-dentistas e necessita, geralmente, de uma abordagem multidisciplinar. A não irrupção deste dente pode ocorrer devido à falta de espaço na arcada dentária, às alterações de erupções dos dentes decíduos e/ou permanentes, à presença de dentes supranumerários ou lesões patológicas adjacentes, e ao denso revestimento ósseo ou excesso de tecido mole local. O planejamento adequado e tratamento precoce podem minimizar os desconfortos estéticos e funcionais do paciente, evitando interferências negativas em seu desenvolvimento biopsicossocial. OBJETIVO: Apresentar um caso clínico de tracionamento de incisivo central superior impactado com apoio em barra transpalatina modificada em paciente infantil. RELATO DE CASO: Paciente do gênero masculino, 13 anos de idade, em desconforto estético com o sorriso devido à retenção prolongada do incisivo decíduo. Nos exames de imagem, notou-se a impacção do incisivo permanente, além de uma imagem sugestiva de odontoma composto e agenesia dos incisivos laterais permanentes. No primeiro momento, foi realizado a exodontia do incisivo e canino decíduo e exérese do odontoma composto, aguardando-se a erupção espontânea do incisivo permanente. Diante da irrupção incompleta do incisivo permanente no período de 6 meses, optou-se pelo seu tracionamento por meio da colagem de um botão ortodôntico e utilização de amarrilhos apoiados em uma barra transpalatina modificada. RESULTADO: Paciente encontra-se com o incisivo permamente já erupcionado, aguardando correção ortodôntica para melhor alinhamento e nivelamento dentário. CONCLUSÃO: A barra transpalatina modificada é um importante aparato ortodôntico para o apoio do amarrilho durante o tracionamento do incisivo impactado em pacientes com impossibilidade do uso do aparelho ortodôntico fixo.

Purpose The purpose of this study was to analyze the reliability of 7 panoramic radiographic signs for predicting proximity of the root apices of mandibular third molars to the mandibular canal using cone-beam computed tomography and to correlate these findings with the Pell and Gregory and the Winter classification systems. Materials and Methods An observational, cross-sectional, descriptive study was conducted on 74 patients with bilateral impacted mandibular third molars. Four panoramic radiographic signs were observed in the tooth root (darkening, deflection, and narrowing of the root apices, and bifid apices), and another 3 in the mandibular canal (diversion, narrowing, and interruption of the mandibular canal). Cone-beam computed tomography images were analyzed to identify disruption and diversion of the mandibular canal and root deflection. Paracoccidioidomycosis is a systemic fungal infection that involves lungs and other organs. The objective is to report 2 cases at different stages of the disease and to emphasize the importance of the correct diagnosis. Middle-aged male patients sought assistance because they presented with pain in the palate. The first one also presented with weight loss, odynophagia, cough, cervical lymphadenomegaly, and reported pulmonary fibrosis. On intra-oral examination, both patients presented with a reddish lesion on hard palate with granulomatous surface and muriform aspect, and the first one also presented with lesions on the alveolar ridge and vestibule. Incisional biopsy was performed in both cases, and a chest radiograph was requested. In the first case lung involvement was observed. Histopathology showed intense granulomatous inflammatory infiltrate and numerous yeasts with positive staining for PAS and Grocott, confirming the diagnostic hypothesis of paracoccidioidomycosis. Itraconazole therapy was started, and total remission of the lesions was observed.

O processo alveolar é uma estrutura dente dependente que sofre alterações dimensionais após a exodontia. Defeitos ósseos resultantes prejudicam a colocação de implantes e o sucesso em longo prazo. Diversas técnicas cirúrgicas e biomateriais tem sido apresentados como opções terapêuticas para preservação e recuperação dos rebordos edêntulos. Assim, o objetivo desta revisão narrativa é evidenciar o estado atual dos biomateriais disponíveis bem como as possíveis perspectivas futuras. A utilização de biomateriais para cirurgias de reconstrução e manutenção de rebordo alveolar com sucesso é evidente. A impressão em 3D de estruturas biocompatíveis, fatores de crescimento, as DDMs, parafusos e membranas reabsorvíveis podem ser as perspectivas futuras.

Introduction: Disc displacement without reduction with limited opening is an intracapsular biomechanical disorder involving the condyle-disc complex. With the mouth closed, the disc is in an anterior position in relation to the condylar head and does not reduce with mouth opening. This disorder is associated with persistent limited mandibular opening.Case report:The patient presented severe limitation to fully open the mouth, interfering in her ability to eat. Clinical examination also revealed maximum assisted jaw opening (passive stretch) with less than 40 mm of maximum interincisal opening. Magnetic resonance imaging was the method of choice to identify the temporomandibular disorders.Conclusion: By means of reporting this rare case of anterior disc displacement without reduction with limited opening, after traumatic extraction of a mandibular third molar, in which manual reduction of temporomandibular joint articular disc was performed, it was possible to prove that this technique is effective in the prompt restoration of mandibular movements. Introdução: o deslocamento do disco articular sem redução com abertura limitada é uma desordem intracapsular que envolve o complexo côndilo-disco. Na posição de boca fechada, o disco articular se encontra numa posição anterior em relação à cabeça da mandíbula e não sofre redução com a abertura de boca. Essa desordem está associada à abertura mandibular limitada e persistente.Caso clínico:o paciente relatava travamento da mandíbula que não permitia uma abertura completa da boca, interferindo, assim, na capacidade de se alimentar. Também era possível observar-se uma abertura assistida (alongamento passivo) com uma distância vertical menor que 40 mm entre os incisivos. A ressonância magnética foi o método de escolha para o diagnóstico das desordens temporomandibulares.Conclusão:por meio da descrição de um caso raro de deslocamento anterior do disco articular sem redução e com abertura limitada, após exodontia traumática do terceiro molar inferior, em que foi realizada a redução manual do disco articular da articulação temporomandibular, provou-se ser essa uma técnica eficaz no rápido restabelecimento dos movimentos mandibulares.

Dog bites represent lesions commonly found in Hospital Emergency Clinic. This type of lesion may cause severe harm to patients, but it rarely affects the underlying bone structure causes facial fracture. This study aims to illustrate a rare clinical case in which a pediatric patient presented a comminuted fracture in the mandible which evolved into a unilateral avulsion of the mandibular condyle, body fractures as well as a mandibular ramus and hemiface that had been deformed, with multiple lacerations and loss of soft-tissue mass. Intermaxillary fixation was performed using the Ivy method, followed by internal rigid fixation using miniplates and screws in attempt to reconstruct the child's mandible. O objetivo do estudo foi verificar a prevalência de sobrepeso e obesidade em escolares e sua associação com variáveis ambientais. Foram estudados 480 alunos da rede pública, através de avaliação antropométrica, peso e altura (IMC), segundo sexo e idade e de acordo com os parâmetros da OMS e IOTF. Os fatores ambientais foram obtidos a partir de questionário aplicado junto aos escolares. O programa Epi Info, versão 3.3.2, foi utilizado para entrada e análise dos dados. A prevalência de obesidade varia consideravelmente conforme o critério usado. Segundo parâmetros da IOTF, 7,1% dos escolares foram considerados obesos, ao passo que, de acordo com critérios da OMS, a prevalência de obesidade foi igual a 18%. O fator de risco significante para a obesidade da criança e adolescente foi apenas a obesidade do pai (RP = 1,08; IC: 0,57-2,04). O perfil nutricional dos escolares serve como subsídio para o monitoramento de tendências de sobrepeso nesse grupo e para a elaboração de medidas preventivas.This study assessed the association between environmental factors and the prevalence of overweight and obesity in public schools. It was studied 480 students, by means of anthropometric measurements evaluation, weight and height BMI by sex and age and in accordance with the parameters of WHO and IOTF. Environmental information was obtained using a questionnaire applied to students. The Epi Info Program, version 3.3.2, was used to data collection and analysis. Prevalence rates of obesity varied according to the criteria it was used. According to the IOTF, prevalence of obesity was 7.1%, while, according parameters of the WHO, the prevalence was 18%. The significant risk factor for obesity was the father obesity (PR = 1.08; CI: 0.57-2.04). We concluded that the nutritional profile of the school serves as a subsidy for tracking trends of overweight in this group. This study could instruct the rationalization of the use of the time and public financial resources, proving the need to invest in the school feeding and in the health students.

Objectives: analyzed the influence of school snacks on the prevalence of dental caries and obesity. Methods: a total of 480 students attending public schools at the city of Juiz de Fora/MG were analyzed by oral clinical examination, anthropometric evaluation (BMI – body mass index), and application of a dietary questionnaire. The software EPI-INFO (version 3.3.2) was used for entry and quali-quantitative analysis of data. Results: the mean age of the study population was 10.3 years, ranging from 4.0 to 18.0 years, being 50.4% females. Nearly 16.6% of the population was classified as having obesity and overweight. The mean dmft was 1.33 and the mean DMFT was 3.18. There were no statistically significant differences (p = 0.92) in the mean DMFT according to the BMI and in the dmft according to the BMI, according to the parametric ANOVA test. Conclusions: the quality of school snacks did not influence the obesity and the prevalence of dental caries.

Avaliar a eficácia analgésica da associação de 30mg do fosfato de codeína com 500mg do paracetamol após exodontias de terceiros molares inferiores impactados. Foi realizado um estudo clínico bilateral com uma amostra de 47 pacientes. Em um dos lados, todos os pacientes receberam a dosagem de 30mg do fosfato de codeína em associação com 500mg do paracetamol após exodontia (grupo teste). Para a exodontia contralateral, foi disponibilizado outro frasco contendo cápsulas idênticas, porém com a dosagem de 500mg de paracetamol (grupo controle). 100% dos pacientes do grupo teste não necessitaram utilizar a medicação resgate e não consumiram doses adicionais da medicação após as cirurgias. No grupo controle, 44,7% dos participantes relataram o uso do medicamento resgate. O consumo total de comprimidos no grupo teste foi, em média, inferior quando comparados ao lado contralateral. 80,8% dos pacientes relataram maior conforto, quanto ao critério da dor, no lado em que foi utilizado a dosagem de 30mg de fosfato de codeína associado a 500mg de paracetamol. Os efeitos colaterais estiveram mais presentes no grupo teste, sendo mais comum o relato de sonolência (34%) e tontura (31,9%), não havendo relato de abandono desta medicação por nenhum dos pacientes. Concluímos que a dosagem de 30mg do fosfato de codeína associada a 500mg de paracetamol apresentou resultados favoráveis no controle da dor e uma baixa incidência de efeitos colaterais. To assess the analgesic efficacy of regular dosage of codeine phosphate 30mg association with paracetamol 500mg after extraction of impacted lower third molars. We performed a bilateral clinical study analyzing a sample of 47 patients. All patients received a 30mg codeine phosphate dosage in combination with paracetamol 500mg after extraction (test group). For the contralateral tooth extraction, we had another bottle available containing identical capsules, with a 500mg paracetamol dosage (control group). 100% of the test group patients did not need to use rescue medication and did not consume additional doses of medication after surgeries. In the control group, 44.7% reported the use of rescue medication. Total consumption of pills in the test group was on average lower than the contralateral side. 80.8% of patients reported greater comfort, as the criterion of pain in the side that was used 30mg codeine phosphate dosage associated with paracetamol 500mg. The adverse effects were more present in the test group, with sleepiness being more common (34%) and dizziness (31.9%), without any patient medication abandonment. We conclude that the 30mg codeine phosphate dosage associated with paracetamol 500mg showed favorable results in controlling pain associated with a low incidence of side effects.

Van der Woude syndrome is a rare disorder of craniofacial development, characterized by the triad (congenital lip fistula, cleft lip and/or cleft palate). Discomfort caused by spontaneous or induced drainage of saliva/mucus when pressure is applied or during a meal as well as poor aesthetics match is one of the main complaints of patients with congenital lip fistula. This paper aims to describe an alternative technique for excision of the congenital fistula of lower lip in patient with Van der Woude syndrome, using gutta percha points and methylene blue for better identify the the fullest extent of the fistula. This approach aims to facilitate a dissection of fistula tissues providing better preservation of the tissues around the fistula, faster healing and better aesthetics lip of the patient.

Vascular anomalies can be academically divided into two groups: vascular tumors and vascular malformations. Among vascular malformations, we can highlight the arteriovenous malformations, a subtype of anomaly characterized by presenting a high blood flow. The diagnosis is beyond doubt a great challenge for the multi professional team involved in treating these anomalies. Imaging tests such as the ultrasonography, magnetic resonance, CT scan with contrast and angiography are extremely important for the orientation about the characteristics and hemodynamic properties of the blood vessels, for delivering predictable results and for risk reducing of complications. In this paper, we describe a rare case of extensive arteriovenous malformation in the face of a pediatric patient, its clinical characteristics and imaging manifestations as well as the satisfactory clinical results obtained with a combination of sclerotherapy and surgical excision.
