Simple Summary: Heartworm disease is a severe and neglected cardiovascular condition in domestic ferrets. Isolated cases of natural infection have been previously reported. However, knowledge of the disease is still considered poor. An unsterilized 2-year-old domestic male ferret was diagnosed using a heartworm antigen test. The animal was asymptomatic and had a good physical appearance. A surgical transvenous extraction was performed, and two worms (one female and one male) were removed. The patient evolved favorably, and repeated diagnostic tests 35 days after surgery showed improvement in the parameters previously evaluated. This clinical case was the first in which it was possible to extract all the parasites from a ferret by endovascular therapy and improves the knowledge of the management of heartworm disease in ferrets. Heartworm disease caused by Dirofilaria immitis is a serious and underdiagnosed cardiovascular condition in domestic ferrets. Hemodynamic changes caused by parasitization in ferrets cause a potentially fatal syndrome, but its clinical findings and treatment have not yet been standardized. The objective of this study was to describe the clinical case of a successful surgical extraction in a ferret infected by D. immitis. The patient was a 2-year-old, 1.5 kg asymptomatic male domestic ferret. The infection was diagnosed using a commercial test for the detection of D. immitis antigens. Subsequently, their clinical status was evaluated using serological and imaging diagnostic tests, and it was finally decided to perform surgical extraction of the adult worms. The ferret was anesthetized and placed in the left lateral decubitus position to perform a venotomy in the right jugular vein. Endoscopic extraction basket-shaped devices were used in the right atrial cavity under fluoroscopic guidance following the Seldinger endovascular surgery technique. With careful handling, two adult parasites were manually removed. A transthoracic echocardiogram performed after the procedure confirmed the absence of heartworms. The ferret recovered without complications and was discharged within 24 h. In the clinical review, 30 days after surgery, no notable alterations or symptoms were observed. Simple Summary: Pulmonary hypertension (PH) is a common and severe complication in dogs infected with Dirofilaria immitis, often persisting even after treatment. This study aimed to evaluate the progression of PH in dogs by assessing changes in radiographic parameters and the Right Pulmonary Artery Distensibility (RPAD) Index before and after treatment. Parameters were measured on the day of diagnosis (Day 0), at discharge (Day 90), and six months post-discharge (Day 270). The results indicated that in non-hypertensive dogs, the RPAD Index significantly improved following treatment. In contrast, hypertensive dogs exhibited a persistently low RPAD Index throughout the study, indicating ongoing PH. Additionally, hypertensive dogs showed consistently elevated VHS, CrPA/R4, and CdPA/R9 ratios compared to non-hypertensive dogs. These findings underscore the persistence of PH despite treatment, suggesting that regular radiographic monitoring of VHS, CrPA/R4, and CdPA/R9 ratios is crucial for assessing and managing long-term outcomes in dogs with heartworm disease. Data from 5841 dogs, 1203 cats, and 1604 humans were analyzed, considering geographical and climatic factors. Results revealed a decline in Dirofilaria immitis prevalence over the years: from 67.02% in dogs (1994) to 15.81% (2020), and from 33.03% (2010) to 17.20% (2020) in cats. Human incidence dropped from 18.66% (2008) to 8.27% (2020). Prevalence was highest in temperate cold zone (TC) and temperate mild zone (TM) climates. Simple Summary: The development of new echocardiographic techniques, such as two-dimensional speckle tracking echocardiography and assessment of longitudinal myocardial deformation, may be new alternative tools to analyse the presence and severity of pulmonary hypertension in heartworm disease. A total of 93 dogs were used, of which 71% were diagnosed with heartworm infection and 41% were found to have PH. The measurements evaluated were obtained using Right Ventricular Automated Function Imaging (RV AFI®) software. The results showed significant differences between animals with and without pulmonary hypertension. Cut-off values with high sensitivity and specificity were also obtained for the detection of pulmonary hypertension in the animals analysed. Echocardiographic measurements for functional assessment of the right ventricle using myocardial longitudinal strain have demonstrated their usefulness in heartworm-infected dogs. Out of the 93 dogs tested, 71% were diagnosed with heartworm infection. PH was identified in 41% of the infected dogs following the American College of Veterinary Internal Medicine (ACVIM) guidelines, based on the peak tricuspid regurgitation velocity to calculate the tricuspid regurgitation pressure gradient (TRPG), while other routine measurements were used, including the right pulmonary artery distensibility index (RPADi). The 2D-STE mode measurements were determined using Right Ventricular Automated Function Imaging (RV AFI®) software. The statistical analysis showed significant differences in the studied parameters among dogs with and without PH. Additionally, sensitivity (sen) and specificity (sp) cut-off values were obtained (GS ≥ −21.25%, sen 96%, sp 86.4%; FWS ≥ −21.95%, sen 92.56%, sp 95.5%; TMAD ≤ 0.85 cm, sen 70.4%, sp 83.3%). These results demonstrated that GS, FWS, and TMAD could be used as supplementary and alternative variables to conventional echocardiographic measurements when detecting PH in dogs with heartworm disease.
