19. Avoiding the Use of Outpatient Rehabilitation Services Under Long-Term Care Insurance During the COVID-19 Pandemic.

Abstract

Introduction: This study clarifies why patients avoided the use of outpatient rehabilitation services covered by long-term care insurance during the COVID-19 pandemic. Method: Self-administered online questionnaires targeted 474 outpatient rehabilitation facilities under long-term care insurance in Fukuoka prefecture, Japan. Results: Main avoidance reasons were users avoiding contact with other users, families avoiding contact with users and other users, and users avoiding contact with staff. Users' physical and cognitive functions, motivation for activities, and assistance levels subsequently deteriorated. Conclusion: Methods for providing rehabilitation are needed in future pandemics to ensure patients maintain physical and cognitive functions and live independently. [ABSTRACT FROM AUTHOR]

34. Relationship between Cancer and Intention to Leave Work among Older Workers: A Cross-Sectional Internet-Based Study.

Abstract

Background and Objectives: Limited research has focused on the relationship between cancer, job loss, and factors associated with job loss among older workers. Therefore, in this study, we aimed to examine the relationship between cancer and intention to leave and between physical-health-related issues, mental-health-related issues, and cancer-related symptoms and intention to leave among older workers with cancer. Materials and Methods: This cross-sectional internet-based study included 4498 workers aged 60–75 years. Intention to leave was assessed based on whether individuals considered quitting their current jobs in the near future. Results: A multivariate logistic regression analysis showed a significant association between cancer and intention to leave (adjusted odds ratio [aOR]: 1.42, 95% confidence interval [CI]: 1.01–2.00, p = 0.045). In addition, physical-health-related issues (aOR: 2.33, 95% CI: 1.10–4.92, p = 0.026) and mental-health-related issues (aOR: 4.44, 95% CI: 1.80–10.98, p = 0.001) were significantly associated with the intention to leave. Conclusions: Healthcare providers and employers must address the physical- and mental-health-related issues facing older workers with cancer to help them secure their employment. Abstract

Membrane-bound cGMP-dependent protein kinase (PKG) II is a key regulator of bone growth, renin secretion, and memory formation. Despite its crucial physiological roles, little is known about its cyclic nucleotide selectivity mechanism due to a lack of structural information. Here, we find that the C-terminal cyclic nucleotide binding (CNB-B) domain of PKG II binds cGMP with higher affinity and selectivity when compared with its N-terminal CNB (CNB-A) domain. To understand the structural basis of cGMP selectivity, we solved co-crystal structures of the CNB domains with cyclic nucleotides. Our structures combined with mutagenesis demonstrate that the guanine-specific contacts at Asp-412 and Arg-415 of the αC-helix of CNB-B are crucial for cGMP selectivity and activation of PKG II. Structural comparison with the cGMP selective CNB domains of human PKG I and Plasmodium falciparum PKG (PfPKG) shows different contacts with the guanine moiety, revealing a unique cGMP selectivity mechanism for PKG II.
