17. Effects of Aerobic and Resistance Exercises on Physical Symptoms in Cancer Patients: A Meta-analysis

Abstract

OBJECTIVE: This study aimed to conduct a meta-analysis to establish the effect of exercise interventions on physical symptoms, including fatigue, nausea/vomiting, pain, dyspnea, insomnia, loss of appetite, constipation, and diarrhea in cancer patients and survivors. METHODS: We searched articles published before April 2017 using the following databases: Cochrane Library, PubMed/MEDLINE, CINAHL, Scopus, PEDro, Health & Medical Collection, and Psychology Database. Randomized controlled trials (RCTs) of exercise intervention in cancer patients, which evaluated cancer-related physical symptoms using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-C30, were included. Symptom scale data were extracted for meta-analysis. Subgroup analyses were performed for exercise types (aerobic, resistance, and mixed exercise programs). RESULTS: Of the 659 articles, 10 RCTs were included in the meta-analysis, of which the mean PEDro score was 5.43 (SD = 1.28). Fatigue, pain, dyspnea, and insomnia were significantly lower in the intervention group than in the control group at postintervention in cancer patients. However, exercise intervention did not promote or suppress nausea/vomiting, loss of appetite, constipation, and diarrhea in cancer patients. The effect of exercise type on each symptom was not different. CONCLUSION: Exercise intervention was confirmed to improve fatigue, pain, and insomnia and might have reduced dyspnea in cancer patients. However, the benefits of exercise on nausea/vomiting, loss of appetite, constipation, and diarrhea were not shown in any exercise type. 22. Frequency and Analysis of Factors Closely Associated with the Development of Depressive Symptoms in Patients with Scleroderma

Abstract

OBJECTIVE: To examine the frequency of depressive symptoms and also to identify factors closely associated with their development in patients with scleroderma (systemic sclerosis, SSc). METHODS: We evaluated 50 patients with SSc for factors associated with depressive symptoms using the following established scales: the Beck Depression Inventory (BDI); the Rheumatology Attitude Index for measuring helplessness; the Sense of Coherence (SOC) scale (a measure of an individual's resilience in the face of stress and capacity to cope with it); the modified Health Assessment Questionnaire for physical disability, working, and social function; support domains of Arthritis Impact Measurement Scales version 2; and a visual analog pain scale. In addition, disease severity of SSc, including skin thickness and internal organ involvement, was also examined in each patient. Multiple regression analysis was used to determine which factors correlated with depressive symptoms. RESULTS: Depressive symptoms ranging from mild to severe state were seen in 46% of the patients. Total BDI scores were significantly correlated with low working ability, low social activity, low SOC, pain, and helplessness, and not associated with disease severity variables including skin score and internal organ involvement. Multiple regression analysis showed that a high level of helplessness and a low level of SOC might be closely associated with depressive symptoms in SSc. CONCLUSION: Our results indicate that depressive symptoms are frequent in SSc patients. Medical staffs should pay attention to the possible risk factors for depressive symptoms, such as patient's helplessness and SOC.

23. Association of health-related quality of life with self-management and satisfaction of relationship with healthcare professionals in female patients with rheumatoid arthritis.

Abstract

Objectives: We explored associations between health-related quality of life (QOL) with self-management and satisfaction with relationships with medical professionals among female rheumatoid arthritis (RA) patients., Methods: Female RA outpatients completed anonymized self-reported questionnaires. Their confidence in self-managing different aspects of RA and satisfaction with relationships with medical professionals were assessed using a visual analog scale. Multiple regression analysis was performed to identify factors correlated with health-related QOL., Results: Valid responses were received from 145 subjects. Mean PCS and MCS scores were 43.0 and 50.4, respectively, suggesting that female RA patients experience reduced QOL despite low disease activity, without perceiving difficulties in their daily lives. PCS scores correlated negatively with the modified Health Assessment Questionnaire (mHAQ) scores, and MCS scores correlated positively with stress self-management and patient-provider satisfaction, but negatively with mHAQ scores. PCS and MCS scores were not significantly influenced by demographic or clinical characteristics including age, disease duration, Steinbrocker stage (or class), or biologic use., Conclusion: To improve health-related QOL in these patients, we must establish good patient-provider relationships and personalize strategies based on physical and mental conditions, enabling normal daily living.
