Detection and attribution (D and A) simulations were important components of CMIP5 and underpinned the climate change detection and attribution assessments of the Fifth Assessment Report of the Intergovernmental Panel on Climate Change. The primary goals of the Detection and Attribution Model Intercomparison Project (DAMIP) are to facilitate improved estimation of the contributions of anthropogenic and natural forcing changes to observed global warming as well as to observed global and regional changes in other climate variables; to contribute to the estimation of how historical emissions have altered and are altering contemporary climate risk; and to facilitate improved observationally constrained projections of future climate change. D and A studies typically require unforced control simulations and historical simulations including all major anthropogenic and natural forcings. Such simulations will be carried out as part of the DECK and the CMIP6 historical simulation. In addition D and A studies require simulations covering the historical period driven by individual forcings or subsets of forcings only: such simulations are proposed here. Key novel features of the experimental design presented here include firstly new historical simulations with aerosols-only, stratospheric-ozone-only, CO2-only, solar-only, and volcanic-only forcing, facilitating an improved estimation of the climate response to individual forcing, secondly future single forcing experiments, allowing observationally constrained projections of future climate change, and thirdly an experimental design which allows models with and without coupled atmospheric chemistry to be compared on an equal footing.

The lack of long and reliable time series of solar spectral irradiance (SSI) measurements makes an accurate quantification of solar contributions to recent climate change difficult. Whereas earlier SSI observations and models provided a qualitatively consistent picture of the SSI variability, recent measurements by the SORCE satellite suggest a significantly stronger variability in the ultraviolet (UV) spectral range and changes in the visible and near-infrared (NIR) bands in anti-phase with the solar cycle. A number of recent chemistry-climate model (CCM) simulations have shown that this might have significant implications on the Earth's atmosphere. Motivated by these results, we summarize here our current knowledge of SSI variability and its impact on Earth's climate. We present a detailed overview of existing SSI measurements and provide thorough comparison of models available to date. SSI changes influence the Earth's atmosphere, both directly, through changes in shortwave (SW) heating and therefore, temperature and ozone distributions in the stratosphere, and indirectly, through dynamical feedbacks. We investigate these direct and indirect effects using several state-of-the art CCM simulations forced with measured and modeled SSI changes. A unique asset of this study is the use of a common comprehensive approach for an issue that is usually addressed separately by different communities. Omissis. Current knowledge indicates that a descent of solar activity into an extended minimum state is a possible scenario. With aid of experiments from a state-of-the-art Earth system model, we investigate the impact of a future solar minimum on Northern Hemisphere climate change projections. This scenario is constructed from recent 11 year solar-cycle minima of the solar spectral irradiance, and is therefore more conservative than the 'grand' minima employed in some previous modeling studies. Despite the small reduction in total solar irradiance (0.36 W m^-2), relatively large responses emerge in the winter Northern Hemisphere, with a reduction in regional-scale projected warming by up to 40%. To identify the origin of the enhanced regional signals, we assess the role of the different mechanisms by performing additional experiments forced only by irradiance changes at different wavelengths of the solar spectrum. We find that a reduction in visible irradiance drives changes in the stationary wave pattern of the North Pacific and sea-ice cover. A decrease in UV irradiance leads to smaller surface signals, although its regional effects are not negligible. These results point to a distinct but additive role of UV and visible irradiance in the Earth's climate, and stress the need to account for solar forcing as a source of uncertainty in regional scale projections., Unión Europea. The 11 year solar-cycle component of climate variability is assessed in historical simulations of models taken from the Coupled Model Intercomparison Project, phase 5 (CMIP-5). Multiple linear regression is applied to estimate the zonal temperature, wind and annular mode responses to a typical solar cycle, with a focus on both the stratosphere and the stratospheric influence on the surface over the period approximately 1850-2005. The analysis is performed on all CMIP-5 models but focuses on the 13 CMIP-5 models that resolve the stratosphere (high-top models) and compares the simulated solar cycle signature with reanalysis data. The 11 year solar cycle component of climate variability is found to be weaker in terms of magnitude and latitudinal gradient around the stratopause in the models than in the reanalysis. The peak in temperature in the lower equatorial stratosphere (approximately 70 hPa) reported in some studies is found in the models to depend on the length of the analysis period, with the last 30 years yielding the strongest response. A modification of the Polar Jet Oscillation (PJO) in response to the 11 year solar cycle is not robust across all models, but is more apparent in models with high spectral resolution in the short-wave region. The PJO evolution is slower in these models, leading to a stronger response during February, whereas observations indicate it to be weaker. In early winter, the magnitude of the modeled response is more consistent with observations when only data from 1979-2005 are considered. The observed North Pacific high-pressure surface response during the solar maximum is only simulated in some models, for which there are no distinguishing model characteristics. The lagged North Atlantic surface response is reproduced in both high- and low-top models, but is more prevalent in the former. In both cases, the magnitude of the response is generally lower than in observations.

- Abstract
BACKGROUND & AIMS: Little is known about the effects of endoscopic balloon dilation (EBD) for strictures of the upper gastrointestinal (UGI) tract in patients with Crohn's disease (CD). We performed a pooled analysis of the efficacy and safety of EBD for UGI CD-associated strictures. METHODS: We searched Embase, Medline, and the Cochrane library, as well as bibliographies of relevant articles, for cohort studies of adults with CD and strictures of the stomach or duodenum (up to the ligament of Treitz) who underwent EBD through December 2016. We obtained data from 7 international referral centers on 94 patients who underwent 141 EBDs. We performed a patient-level meta-analysis of data from published and unpublished cohort studies to determine mechanical and clinical success. We performed a time-to-event analysis to assess symptom recurrence and need for redilation or surgery. The patients analyzed had strictures of the duodenum (n = 107), stomach (n = 30), or spanning both (n = 4). RESULTS: The rate of technical success for EBD was 100%, with 87% short-term clinical efficacy; major complications arose from 2.9% of all procedures. During a median follow-up period of 23.1 months, 70.5% of patients had a recurrence of symptoms, 59.6% required redilation, and 30.8% required surgical intervention. Patients whose disease was located in the small bowel had a higher risk for symptom recurrence (hazard ratio [HR], 2.1; P = .003). Asian race (HR, 2.8; P < .001) and location of disease in the small bowel (HR, 1.9; P = .004) increased the need for redilation. Prestenotic dilation was a risk factor for needing surgery earlier (HR, 1.9; P = .001). CONCLUSIONS: In a meta-analysis, we found EBD for CD-associated strictures of the UGI to be an effective alternative to surgery, with a high rate of short-term technical and clinical success, moderate long-term efficacy, and an acceptable rate of complications.

The surface response to the 11 year solar cycle is assessed in ensemble simulations of the twentieth century climate performed in the framework of the fifth phase of the Coupled Model Inter Comparison Project (CMIP5). A lead/lag multiple linear regression analysis identifies a multi model mean (MMM) global mean surface warming of about 0.07 K lagging the solar cycle by 1 2 years on average. The anomalous warming penetrates to approximately the first 80 100 m depth in the ocean. Solar signals in the troposphere show a similar time lag of 1 2 years and the strongest MMM warming is simulated in the Tropics above 300 hPa. At the surface the MMM response in a subset of models that show statistically significant global mean warming (CMIP5 SIG95) is characterized by an anomalous warming in the west equatorial Pacific Ocean and the Arctic at 1 2 years after solar maximum. The Arctic warming is twice as strong as the global mean response and appears in the winter months only. The surface warming in the equatorial Pacific Ocean is related to dynamical/thermodynamical processes. Different increase rates of global mean precipitation and atmospheric water vapour in response to a warmer surface lead to a weaker Walker circulation and anomalous westerly winds over the equatorial Pacific in the years following the solar maximum. Owing to atmosphere ocean coupling the anomalous westerly winds cool the subsurface and warm the surface in the western equatorial Pacific by approximate to 0.14 K. The CMIP5 SIG95 MMM surface warming in the equatorial Pacific and Arctic is weak but qualitatively similar compared with solar signals in the HadCRUT4 dataset.

- Abstract
The impact of changes in incoming solar irradiance on stratospheric ozone abundances should be included in climate model simulations to fully capture the atmospheric response to solar variability. This study presents the first systematic comparison of the solar-ozone response (SOR) during the 11 year solar cycle amongst different chemistry-climate models (CCMs) and ozone 5 databases specified in climate models that do not include chemistry. We analyse the SOR in eight CCMs from the WCRP/SPARC Chemistry-Climate Model Initiative (CCMI-1) and compare these with three ozone databases: the Bodeker Scientific database, the SPARC/AC&C database for CMIP5, and the SPARC/CCMI database for CMIP6. The results reveal substantial differences in the representation of the SOR between the CMIP5 and CMIP6 ozone databases. The peak amplitude of the 10 SOR in the upper stratosphere (1-5 hPa) decreases from 5% to 2% between the CMIP5 and CMIP6 databases. This difference is because the CMIP5 database was constructed from a regression model fit to satellite observations, whereas the CMIP6 database is constructed from CCM simulations, 1 Atmos. Chem. CC-BY 3.0 License. which use a spectral solar irradiance (SSI) dataset with relatively weak UV forcing. The SOR in the CMIP6 ozone database is therefore implicitly more similar to the SOR in the CCMI-1 models than 15 to the CMIP5 ozone database, which shows a greater resemblance in amplitude and structure to the SOR in the Bodeker database. The latitudinal structure of the annual mean SOR in the CMIP6 ozone database and CCMI-1 models is considerably smoother than in the CMIP5 database, which shows strong gradients in the SOR across the midlatitudes owing to the paucity of observations at high latitudes. The SORs in the CMIP6 ozone database and in the CCMI-1 models show a strong seasonal 20 dependence, including large meridional gradients at mid to high latitudes during winter; such seasonal variations in the SOR are not included in the CMIP5 ozone database. Sensitivity experiments with a global atmospheric model without chemistry (ECHAM6.3) are performed to assess the impact of changes in the representation of the SOR and SSI forcing between CMIP5 and CMIP6. The experiments show that the smaller amplitude of the SOR in the CMIP6 ozone database compared to 25 CMIP5 causes a decrease in the modelled tropical stratospheric temperature response over the solar cycle of up to 0.6 K, or around 50% of the total amplitude. The changes in the SOR explain most of the difference in the amplitude of the tropical stratospheric temperature response in the case with combined changes in SOR and SSI between CMIP5 and CMIP6. The results emphasise the importance of adequately representing the SOR in climate models to capture the impact of solar variability 30 on the atmosphere. Since a number of limitations in the representation of the SOR in the CMIP5 ozone database have been identified, CMIP6 models without chemistry are encouraged to use the CMIP6 ozone database to capture the climate impacts of solar variability.

This paper describes the recommended solar forcing dataset for CMIP6 and highlights changes with respect to CMIP5. The solar forcing is provided for radiative properties, namely total solar irradiance (TSI), solar spectral irradiance (SSI), and the F10.7 index as well as particle forcing, including geomagnetic indices Ap and Kp, and ionization rates to account for effects of solar protons, electrons, and galactic cosmic rays. This is the first time that a recommendation for solar-driven particle forcing has been provided for a CMIP exercise. The solar forcing datasets are provided at daily and monthly resolution separately for the CMIP6 preindustrial control, historical (1850–2014), and future (2015–2300) simulations. For the preindustrial control simulation, both constant and time-varying solar forcing components are provided, with the latter including variability on 11-year and shorter timescales but no long-term changes. For the future, we provide a realistic scenario of what solar behavior could be, as well as an additional extreme Maunder-minimum-like sensitivity scenario. This paper describes the forcing datasets and also provides detailed recommendations as to their implementation in current climate models.For the historical simulations, the TSI and SSI time series are defined as the average of two solar irradiance models that are adapted to CMIP6 needs: an empirical one (NRLTSI2–NRLSSI2) and a semi-empirical one (SATIRE). A new and lower TSI value is recommended: the contemporary solar-cycle average is now 1361.0 W m−2. The slight negative trend in TSI over the three most recent solar cycles in the CMIP6 dataset leads to only a small global radiative forcing of −0.04 W m−2. In the 200–400 nm wavelength range, which is important for ozone photochemistry, the CMIP6 solar forcing dataset shows a larger solar-cycle variability contribution to TSI than in CMIP5 (50 % compared to 35 %).We compare the climatic effects of the CMIP6 solar forcing dataset to its CMIP5 predecessor by using time-slice experiments of two chemistry–climate models and a reference radiative transfer model. The differences in the long-term mean SSI in the CMIP6 dataset, compared to CMIP5, impact on climatological stratospheric conditions (lower shortwave heating rates of −0.35 K day−1 at the stratopause), cooler stratospheric temperatures (−1.5 K in the upper stratosphere), lower ozone abundances in the lower stratosphere (−3 %), and higher ozone abundances (+1.5 % in the upper stratosphere and lower mesosphere). Between the maximum and minimum phases of the 11-year solar cycle, there is an increase in shortwave heating rates (+0.2 K day−1 at the stratopause), temperatures ( ∼ 1 K at the stratopause), and ozone (+2.5 % in the upper stratosphere) in the tropical upper stratosphere using the CMIP6 forcing dataset. This solar-cycle response is slightly larger, but not statistically significantly different from that for the CMIP5 forcing dataset.CMIP6 models with a well-resolved shortwave radiation scheme are encouraged to prescribe SSI changes and include solar-induced stratospheric ozone variations, in order to better represent solar climate variability compared to models that only prescribe TSI and/or exclude the solar-ozone response. We show that monthly-mean solar-induced ozone variations are implicitly included in the SPARC/CCMI CMIP6 Ozone Database for historical simulations, which is derived from transient chemistry–climate model simulations and has been developed for climate models that do not calculate ozone interactively. CMIP6 models without chemistry that perform a preindustrial control simulation with time-varying solar forcing will need to use a modified version of the SPARC/CCMI Ozone Database that includes solar variability. CMIP6 models with interactive chemistry are also encouraged to use the particle forcing datasets, which will allow the potential long-term effects of particles to be addressed for the first time. The consideration of particle forcing has been shown to significantly improve the representation of reactive nitrogen and ozone variability in the polar middle atmosphere, eventually resulting in further improvements in the representation of solar climate variability in global models.

Objective Ovarian cancer comprises several histological groups with widely differing levels of survival. We aimed to explore international variation in survival for each group to help interpret international differences in survival from all ovarian cancers combined. We also examined differences in stage-specific survival. Methods The CONCORD programme is the largest population-based study of global trends in cancer survival, including data from 60 countries for 695,932 women (aged 15–99years) diagnosed with ovarian cancer during 1995–2009. We defined six histological groups: type I epithelial, type II epithelial, germ cell, sex cord-stromal, other specific non-epithelial and non-specific morphology, and estimated age-standardised 5-year net survival for each country by histological group. We also analysed data from 67 cancer registries for 233,659 women diagnosed from 2001 to 2009, for whom information on stage at diagnosis was available. We estimated age-standardised 5-year net survival by stage at diagnosis (localised or advanced). Results Survival from type I epithelial ovarian tumours for women diagnosed during 2005–09 ranged from 40 to 70%. Survival from type II epithelial tumours was much lower (20–45%). Survival from germ cell tumours was higher than that of type II epithelial tumours, but also varied widely between countries. Survival for sex-cord stromal tumours was higher than for the five other groups. Survival from localised tumours was much higher than for advanced disease (80% vs. 30%). Conclusions There is wide variation in survival between histological groups, and stage at diagnosis remains an important factor in ovarian cancer survival. International comparisons of ovarian cancer survival should incorporate histology.

- Abstract
This paper describes the recommended solar forcing dataset for CMIP6 and highlights changes with respect to CMIP5. The solar forcing is provided for radiative properties, namely total solar irradiance (TSI), solar spectral irradiance (SSI), and the F10.7 index as well as particle forcing, including geomagnetic indices Ap and Kp, and ionization rates to account for effects of solar protons, electrons, and galactic cosmic rays. This is the first time that a recommendation for solar-driven particle forcing has been provided for a CMIP exercise. The solar forcing datasets are provided at daily and monthly resolution separately for the CMIP6 preindustrial control, historical (1850-2014), and future (2015-2300) simulations. For the preindustrial control simulation, both constant and time-varying solar forcing components are provided, with the latter including variability on 11-year and shorter timescales but no long-term changes. For the future, we provide a realistic scenario of what solar behavior could be, as well as an additional extreme Maunder-minimum-like sensitivity scenario. This paper describes the forcing datasets and also provides detailed recommendations as to their implementation in current climate models. For the historical simulations, the TSI and SSI time series are defined as the average of two solar irradiance models that are adapted to CMIP6 needs: an empirical one (NRLTSI2-NRLSSI2) and a semi-empirical one (SATIRE). A new and lower TSI value is recommended: the contemporary solar-cycle average is now 1361.0 W m-2. The slight negative trend in TSI over the three most recent solar cycles in the CMIP6 dataset leads to only a small global radiative forcing of -0.04 W m-2. In the 200-400 nm wavelength range, which is important for ozone photochemistry, the CMIP6 solar forcing dataset shows a larger solar-cycle variability contribution to TSI than in CMIP5 (50 % compared to 35 %).
