A 36-year-old man who presented multiple times with fever, headache, alteration of mental status, and focal neurological deficits. MRI revealed extensive white matter lesions that were partially reversed between episodes. Workup revealed persistently low complement factor C3, low factor B, and absent alternative complement pathway activity. Biopsy revealed neutrophilic vasculitis. Genetic testing revealed a homozygous mutation in complement factor I (CFI), that was felt to be pathogenic. CFI regulates complement mediated inflammation, and deficiency in this factor leads to unchecked alternative pathway activity and decrease in C3 and factor B through consumption. The patient has remained stable since starting IL-1β inhibition. Complement factor I is a rare disorder that should be considered in patients with atypical relapsing neurological disease associated with neutrophilic pleocytosis.

'The Index of Digital Humanities Conferences' is a database of conference metadata relevant to the digital humanities community dating back to the 1960s. It includes details about hundreds of conferences, and for a subset of them, details about work presented at those events. Weingart, Eichmann-Kalwara, Lincoln, and research assistants collected information by hand or with machine assistance from paper programs, listserv announcements, published proceedings, PDF programs, and conference websites. Data are in CSV tables with a data dictionary describing how they can be linked together, for use by digital humanists and bibliometricians to study the history of the digital humanities community.

COVID-19 has become a pandemic since its emergence in Wuhan, China. The disease process was initially defined by presence of respiratory symptoms; however, it is now well studied and shown in evidence that this is a multisystem process. Involvement of gastrointestinal (GI) system has been identified, and GI symptoms can be the only presenting symptoms in some patients. Hence, it is important to identify and understand the GI symptoms associated with COVID-19 for appropriate care of patient. We conducted a systematic review and metaanalysis to identify the GI symptoms of COVID-19 and identify association of diarrhea with severity of COVID-19. We performed extensive search of Medline and Embase from December 2019 to May 2020 to identify articles reporting GI symptoms in COVID-19 patients. The primary outcome was prevalence of GI symptoms in COVID-19 patients, and secondary outcome was the association of diarrhea with disease severity. A total of 38 studies with 8407 patients were included. Of the total patients, 15.47% patients had at least one GI symptom. The pooled prevalence of nausea/vomiting was 7.53% and diarrhea was 11.52%. On metaanalysis, patients with diarrhea as one of the presenting symptoms were more likely to have severe disease (OR 1.63, 95% CI: 1.11–3.38, p = 0.01). Our systematic review and metaanalysis demonstrated that GI symptoms are common in COVID-19. Presence of diarrhea as a presenting symptom is associated with increased disease severity and likely worse prognosis. Early recognition of patients is needed for prompt management of this at-risk population. According to most modern scholarship, it was during his time in Italy that Honthorst earned the nickname “Gherardo delle Notti,” or “Gerrit of the nights,” because of his talent for rendering dramatic nighttime images… The attentive reader will notice, however, that none of these texts credit the nickname to a contemporary source. When did Honthorst actually earn this name? An investigation of archival sources suggests that “Gherardo delle Notti” may not be nearly as contemporary as has been assumed.

Controlled oxidation reactions of hydrocarbons are practiced today in fixed-bed, fluid bed or transport bed reactors with oxygen or air as the oxidant. In fixed-bed reactors, due to the exothermicity of the oxidation reactions, heat removal and temperature control are critical in achieving safe and optimum reaction conditions that maximize conversion and selectivity. Industrial reactors often control hot spot formation by using an appropriate diluent (ballast) gas. Steam, nitrogen, methane and carbon dioxide are the most commonly used ballast gases. The scope of this paper is to elucidate the benefits of ballast gas use as a tool to optimize heat transfer and reduce the hot spot effect in controlled oxidations with fixed-bed reactors. Ballast gas does not participate in the reaction, but it is used to control heat removal and flammability. A homogeneous one-dimensional reactor model was used to study and compare two oxidation processes: (1) ethylene oxidation to ethylene oxide and (2) o-xylene oxidation to phthalic anhydride. For both processes hot spot temperature and conversion decreased as methane replaced nitrogen in the process ballast gas. Selectivity increased in the ethylene oxide case but decreased in the phthalic anhydride case, indicating that ballast gas effect on selectivity depends on the reaction mechanism. Thus, there is an optimum ballast gas composition that will optimize hot spot temperature and reactor yield.
