Objective This study investigated the factors associated with satisfactory early postoperative wound conditions. Method A prospective study was conducted with patients (n=179) submitted to osteosynthesis in general, in a hospital orthopedics service. In the preoperative period, patients underwent laboratory exams and the surgical indications were based on the type of fracture and the patient’s clinical conditions. In the postoperative period, patients were evaluated based on the presence of complications and considering their surgical wounds. Chi-square, Fisher, Mann-Whitney, and Kruskal-Wallis tests were used in the analysis. To identify the factors associated with wound condition, univariate and multiple logistic regression analysis was used. Results In the univariate analysis, each transferring unit reduction increased the chance of satisfactory outcome by 1.1% (p=0.0306; OR= 0.989 (1.011); 95%CI= 0.978;0.999; 1.001;1.023). The presence of SAH increased 2.7 fold the chance of satisfactory outcome (p=0.0424; OR= 2,667; 95%CI= 1,034;6,877). Hip fracture increased 2.6 fold the chance of satisfactory outcome (p=0.0272; OR=2.593; IC95%=1.113; 6.039). And the absence of a compound fracture increased 5.5 fold the chance of satisfactory wound outcome (p=0.0004; OR=5,493; 95%CI=2,132;14,149). In the multiple analysis, patients with non compound fractures were 9.7 times more likely to experience a satisfactory outcome when compared to patients with compound fractures (p=0.0014; OR=9,687; 95%CI= 2,399; 39,125). Conclusion There was an inverse relationship between plasma proteins levels and satisfactory surgical wounds outcome. Only exposure remained associated with wound conditions. Level Of Evidence: II, Prospective Study. RESUMO Objetivo Este estudo investigou os fatores associados à condição satisfatória de ferida pós-operatória precoce. Método Foi desenvolvido um estudo prospectivo com pacientes (n=179) submetidos a osteossínteses em geral, em um serviço de ortopedia hospitalar. No período pré-operatório os pacientes foram submetidos a exames laboratoriais e as indicações cirúrgicas foram baseadas no tipo de fratura e condições clínicas. No período pós-operatório, os pacientes foram avaliados conforme a presença de complicações e feridas operatórias. Foram utilizados na análise os testes Qui-quadrado, Fisher, Mann-Whitney, Kruskal-Wallis. Para identificar os fatores associados à condição da ferida, foi utilizada a análise de regressão logística univariada e múltipla. Resultados Na análise univariada, cada unidade a menos de transferrina, aumentou a chance de condição satisfatória em 1.1% (p= 0.0306; OR= 0.989 (1.011); IC95%= 0.978;0.999; 1.001;1.023). A presença de HAS aumentou a chance de condição satisfatória em 2.7 vezes (p=0.0424; OR= 2.667; IC95%= 1.034;6.877). Fratura de quadril aumentou a chance de condição satisfatória em 2.6 vezes (p=0.0272; OR=2.593; IC95%=1.113;6.039). E não apresentar fratura exposta, aumentou a chance de condição satisfatória da ferida em 5.5 vezes (p=0.0004; OR=5.493; IC95%=2.132;14.149). Na análise múltipla, fraturas sem exposição, apresentaram chance 9.7 vezes maior de apresentar condição satisfatória do que insatisfatória; quando comparadas às que apresentaram exposição (p=0.0014; OR=9.687; IC95%= 2,399; 39.125). Conclusão Houve uma relação inversa entre proteínas plasmáticas e presença de feridas operatórias satisfatórias. Apenas a exposição se manteve associada à condição da ferida. Traffic-accidents are a public health problem with repercussions on population morbimortality. Objective: To analyze the impact of the pandemic on the profile of motorcycle accidents assisted at the Tertiary Hospital in 2020. Methods: Cross-sectional, descriptive retrospective study in 2017 and 2020 of 260 medical records of care for motorcycle accidents in the emergency room of the Tertiary Hospital. Statistical analysis of data and their correlations using the chi-square test (p < 0.05). Results: Of the 105 medical records in 2017, 83% are men, mean age 29.8 years, and death rate of 3.90%. Fractures in 98.10%, 64.10% exposed and predominantly the tibia (61.90%). Of the 155 medical records in 2020, 91.61% are men, mean age 31.21 years, and no deaths. Fractures in 94.84%, 37.42% exposed and predominantly the tibia (28.57%). Between 2017 and 2020, Infosiga-SP showed a relevant reduction (p < 0.001) of deaths in the hospital environment (52.46% to 31.91%). Conclusion: The incidence of motorcycle accidents increased, in-hospital deaths dropped, but the epidemiological profile of accidents at the Hospital remained unchanged. Level of Evidence III, Comparative Retrospective Study. RESUMO Acidentes motociclísticos configuram um problema de saúde pública com repercussões na morbimortalidade populacional. Objetivo: Analisar o impacto da pandemia por COVID-19 no perfil de acidentes motociclísticos atendidos em um hospital terciário em 2020. Métodos: Estudo transversal, descritivo e retrospectivo que analisou 260 prontuários de acidentados de moto atendidos na Urgência e Emergência do Hospital Universitário Terciário em 2017 e 2020. Realizou-se uma análise estatística dos dados e suas correlações pelo teste qui-quadrado (p < 0,05). Resultados: Dos 105 prontuários de 2017, 83% são de homens (p < 0,001), com média de 29,8 anos, e índice de óbito de 3,90%. Houve fraturas em 98,10% dos casos, sendo 58,10% expostas e predominantemente da tíbia (61,90%). Dos 155 prontuários de 2020, 91,61% são homens (p < 0,001), com média de 31,21 anos e sem casos de óbito. Houve fraturas em 94,84%, sendo 37,42% expostas e predominantemente da tíbia (28,57%). Entre 2017 e 2020, o Infosiga-SP mostrou redução significativa (p < 0,001) de mortes em ambiente hospitalar (de 52,46% para 31,91%). Conclusão: Houve aumento na incidência dos acidentes motociclísticos e queda nos óbitos intra-hospitalares, mas o perfil epidemiológico dos acidentados no hospital permaneceu inalterado. Orientador: Mauricio Etchebehere Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: Introdução: A lesão do bíceps braquial distal acomete, na maioria dos casos, o membro dominante de homens entre a quinta e sexta décadas de vida, durante movimento de contração muscular. Essa lesão provoca dor aguda importante que, se não tratada adequadamente, causa a longo prazo déficits funcionais de flexão e supinação. Objetivo: Avaliação funcional pós-operatória da reinserção do bíceps distal através de via única,anterior e transversa no antebraço com uso de duas âncoras. Métodos: Estudo clínico prospectivo com avaliação de quinze pacientes com ruptura do tendão distal do bíceps submetidos à reinserção através de via única, anterior e transversa no antebraço com uso de duas âncoras. Os pacientes foram submetidos a um protocolo de reabilitação e, ao término de seis meses, realizaram avaliação do arco de movimento do cotovelo operado e da intensidade de força durante a flexão e a supinação. Os dados foram coletados de maneira prospectiva e foram analisados pelo teste de Mann-Whitney e teste de modelos mistos para avaliar a força entre o lado operado e o não operado. Resultados: Oitenta por cento dos pacientes eram homens, 60% sofreram lesão do lado dominante, 46% eram trabalhadores braçais e 73% não praticavam regularmente atividades físicas. O uso de anabolizante foi relatado por dois pacientes. Após o tratamento 98% dos pacientes recuperaram força de supinação e 94% a força de flexão. De acordo com questionário DASH, 73% dos pacientes encontravam-se dentro do esperado para uma população normal. Não ocorreram casos de ossificação heterotópica. Conclusão: A via única, anterior e transversa para o reparo do tendão com o uso de duas âncoras apresentou boa recuperação funcional da força e arco de movimento do cotovelo Abstract: Background: Distal brachial biceps lesion affects, in most cases, the dominant limb of men between the fifth and sixth decades of life, during movement of muscle contraction. This injury causes significant acute pain, and, if not treated correctly, causes long-term functional deficits of flexion and supination. Purposes: Functional postoperative evaluation of the reinsertion in a distal biceps lesion through a single, anterior and transverse approach in the forearm using two anchors. Methods: The prospective clinical study evaluated a fifteen patients with ruptured distal biceps tendon submitted to reinsertion via single, anterior and transverse approach using two anchors. They were submitted to a rehabilitation protocol and within six months, evaluation of the operated elbow movement arch and strength intensity during flexion and supination. The Data were collected prospectively and were analyzed by the Mann-Whitney test and mixed model test to evaluate the force between the operated and non-operated side. Results: 80% of the patients were men, 60% were injured on the dominant side, 46% were manual workers and 73% were sedentary. Two patients reported anabolic regular use. After treatment, patients recovered supination strength by 98% and flexion in 94%. According to DASH questionnaire 73% of patients presented the expected score of a healthy population. No heterotopic ossification occurring. Objective To demonstrate that the intraosseous (IO) access is more effective compared with the intravenous (IV) access for prophylactic antibiotic administration in knee joint surgeries, using 36 pigs as live models. Materials and Methods Skin, subcutaneous tissue, cartilage, and bone samples were collected, analyzed and compared after the administration of IV or IO antibiotic in different groups. Results When comparing the IO and IV groups, the IO group showed a higher concentration of prophylactic antibiotic in the skin (p = 0.049), cartilage (p = 0.018), and bone (p = 0.002), in the analysis of the first 24 hours after 30 minutes of infusion. Conclusion Since complications regarding this practice are rare, the use of this pathway may be an alternative to reduce the risk of surgical site infection in orthopedic surgeries, leading to a decrease in morbidity and mortality and hospital expenses with readmission or prolonged hospitalization time. However, further research and further experimental studies in humans are required, as the effectiveness of the method in pigs has been proved. Resumo Objetivo Demonstrar, em 36 porcos usados como modelos vivos, que o acesso intraósseo (IO) é mais eficaz em comparação com o acesso endovenoso (EV) na administração de antibiótico profilático em cirurgias na articulação do joelho. Materiais e Métodos Foram coletadas, analisadas e comparadas amostras de pele, tecido subcutâneo, cartilagem e osso, após administração de antibiótico profilático EV e IO em grupos diferentes. Resultado A comparação entre os grupos IO e EV indicou que no grupo IO houve maior concentração de antibiótico profilático na pele (p = 0,049), na cartilagem (p = 0,018) e no osso (p = 0,002), na análise das primeiras 24 horas após 30 minutos de infusão. Conclusão Visto que as complicações dessa prática são raras, o uso dessa via pode ser uma opção para a diminuição do risco de infecção do sítio cirúrgico nas cirurgias ortopédicas, pois leva à diminuição da morbimortalidade e dos gastos hospitalares com reabordagens ou com tempo prolongado de internação. Contudo, são necessários mais pesquisas e novos estudos experimentais em seres humanos, dado que está comprovada a eficácia do método em porcos.
15. Cost and time of hospitalization for elderly people with bone fractures in a reference hospital.

Objective: To describe and analyze the aspects regarding the cost and length of stay for elderly patients with bone fractures in a tertiary reference hospital., Methods: A cross-sectional retrospective study using data obtained from medical records between January and December 2020. For statistical analysis, exploratory analyses, Shapiro-Wilk test, χ2 test, and Spearman correlation were used., Results: During the study period, 156 elderly patients (62.2% women) with bone fractures were treated. The main trauma mechanism was a fall from a standing height (76.9%). The most common type of fracture in this sample was a transtrochanteric fracture of the femur, accounting for 40.4% of cases. The mean length of stay was 5.25 days. The total cost varied between R$2,006.53 and R$106,912.74 (average of R$15,695.76) (updated values). The mean daily cost of hospitalization was R$4,478.64. A positive correlation was found between the length of stay and total cost. No significant difference in cost was observed between the two main types of treated fractures., Conclusion: Fractures in the elderly are frequent, resulting in significant costs. The longer the hospital stay for treatment, the higher the total cost. No correlation was found between total cost and number of comorbidities, number of medications used, and the comparison between the treatment of transtrochanteric and femoral neck fractures.
