Abstract

The precipitation of struvite, a magnesium ammonium phosphate hexahydrate (MgNH4PO4 · 6H2O) mineral, from wastewater is a promising method for recovering phosphorous. While this process is commonly used in engineered environments, our understanding of the underlying mechanisms responsible for the formation of struvite crystals remains limited. Specifically, indirect evidence suggests the involvement of an amorphous precursor and the occurrence of multi-step processes in struvite formation, which would indicate non-classical paths of nucleation and crystallization. In this study, we use synchrotron-based in situ x-ray scattering complemented by cryogenic transmission electron microscopy to obtain new insights from the earliest stages of struvite formation. The holistic scattering data captured the structure of an entire assembly in a time-resolved manner. The structural features comprise the aqueous medium, the growing struvite crystals, and any potential heterogeneities or complex entities. By analysing the scattering data, we found that the onset of crystallization causes a perturbation in the structure of the surrounding aqueous medium. This perturbation is characterized by the occurrence and evolution of Ornstein-Zernike fluctuations on a scale of about 1 nm, suggesting a non-classical nature of the system. We interpret this phenomenon as a liquid-liquid phase separation, which gives rise to the formation of the amorphous precursor phase preceding actual crystal growth of struvite. Abstract

Cancer-associated venous thromboembolism (VTE) is common in women with cancer. Many clinical practice guidelines provide guidance for prevention and treatment; however, there are no specific recommendations for women. This is unfortunate because the proportion of women with breast- and gynecological cancers is high among patients with cancer-associated VTE. Thromboembolism often heralds cancer progression and poor prognosis and should—besides adequate anticoagulant management—also prompt reassessment and, if necessary, changes in cancer treatment. Recently, the new class of direct-acting oral anticoagulants (DOACs) has started to replace low-molecular-weight heparin as standard thromboprophylaxis and therapy in cancer patients. They are very effective, but they also carry a relevant risk of bleeding. Therefore, despite their ease of use, not every tumor patient qualifies for a DOAC, and this is especially true for gynecological tumor patients. Each prescription must be weighed individually. This review addresses specific aspects of VTE prophylaxis and management in women with cancer. Every physician who treats breast and gynecological cancers should be familiar with prophylaxis, diagnosis, and therapy of cancer-associated VTE. At the same time, patients should be informed by their physician what symptoms to look for and whom to contact if these symptoms occur, even outside of office hours and on weekends.

Abstract

Advanced chronic liver disease is accompanied with relevant changes in the corpuscular and plasmatic coagulation system. Due to thrombocytopenia that is regularly observed in these patients, platelet transfusions are often performed prior invasive procedures to prevent possible bleeding complications. However, platelet transfusions are associated with clinically significant adverse events and economically relevant health care costs. Thus, avoiding unnecessary platelet transfusions remains pivotal in daily clinical practice. The first step is to carefully check if increasing platelet counts prior to a planned invasive procedure is really necessary. Nowadays, two well-tolerated thrombopoetin-receptor agonists (TPO-RAs), Avatrombopaq and Lusutrombopaq, to treat thrombocytopenia preemptively before an invasive procedure in patients with liver cirrhosis are available. Abstract

Vernetzt, autonom, smart, elektrisch. Diese Schlagworte sind omnipräsent in der Diskussion um Mobilität auf politischer, wissenschaftlicher und gesellschaftlicher Ebene. Tatsächlich scheinen enorme technologische Fortschritte in der nächsten Zeit möglich und Visionen von lebenswerten Städten mit niederschwelligem Zugang zu einer nachhaltigen, umfeldverträglichen Mobilität erscheinen zum Greifen nahe. In der Forschung und Entwicklung werden von der Fahrzeug- und Dienstleistungsindustrie überwiegend Fragestellungen diskutiert, die sich entweder auf die technische Weiterentwicklung der Fahrzeuge und der Straßeninfrastruktur oder auf die Entwicklung neuer Geschäftsmodelle im Zusammenhang mit dem autonomen Fahren beziehen. Beide Aktivitäten zielen auf zusätzliche Mobilitätsoptionen für zusätzliche Personengruppen ab. An diesem Punkt setzt das Forschungsprojekt „Autonomes Fahren – Risiken und Chancen für die Städte“ (AutoRICH) an: AutoRICH nimmt Risiken in den Blick, die v.a. damit einhergehen, dass durch autonomes Fahren voraussichtlich Mehrverkehr generiert wird, der sowohl die Kommunen als auch die Straßenbaulastträger vor enorme Herausforderungen stellt. Diese werden den allseits diskutierten Chancen kritisch gegenübergestellt. Mit AutoRICH wird also die bislang stark anbieterseitige Perspektive erweitert: Mittels eines akteurszentrierten und interdisziplinären Ansatzes, der soziologische sowie verkehrs- und stadtplanerische Perspektiven aufeinander bezieht, wird zum einen danach gefragt, welche technischen, ökonomischen und sozialen Chancen und Risiken aus der Perspektive der Kommune mit dem autonomen Fahren (AF) einhergehen. Zum anderen wird berücksichtigt, welche Bedeutung dem autonomen Fahren aus Sicht der Endverbraucher:innen zukommt. Vor dem Hintergrund eines solchen integrierten Ansatzes werden Rahmenbedingungen bzw. Steuerungsmöglichkeiten definiert, mittels derer das autonome Fahren den erhofften Beitrag zu einer nachhaltigen und stadtverträglichen Mobilität leisten kann bzw. negative Risiken für Kommune und Endverbraucher:innen abgewendet werden können.

Fast alle zeigen einen belebten, urbanen und stark begrünten öffentlichen Raum mit hoher Aufenthaltsqualität. Radfahrer, Fußgänger und autonome Fahrzeuge bewegen sich in harmonischem Miteinander bewegen. Auch der Parkdruck ist verschwunden, abgesehen von vereinzelt wartenden autonomen Mini-Shuttles. Aber ist eine solche Entwicklung wirklich zu erwarten? Vor diesem Hintergrund entstand das Projekt AutoRICH: Es beleuchtet die Chancen und Risiken des autonomen Fahrens für die Städte und ihre Bewohner:innen und untersucht, welche Weichen heute schon gestellt werden müssten, um eine Entwicklung anzustoßen, damit das das autonome Fahren die oben beschriebenen Visionen wirklich eintreten können.

Abstract

Outcome of patients with primary refractory acute myeloid leukemia remains unsatisfactory. We conducted a prospective phase II clinical trial with gemtuzumab ozogamicin (3 mg/m2 intravenously on day 1), all-trans retinoic acid (45 mg/m2 orally on days 4–6 and 15 mg/m2 orally on days 7–28), high-dose cytarabine (3 g/m2/12 h intravenously on days 1–3) and mitoxantrone (12 mg/m2 intravenously on days 2–3) in 93 patients aged 18–60 years refractory to one cycle of induction therapy. Primary end point of the study was response to therapy; secondary end points included evaluation of toxicities, in particular, rate of sinusoidal obstruction syndrome after allogeneic hematopoietic cell transplantation. Complete remission or complete remission with incomplete blood count recovery was achieved in 47 (51%) and partial remission in 10 (11%) patients resulting in an overall response rate of 61.5%; 33 (35.5%) patients had refractory disease and 3 patients (3%) died. Allogeneic hematopoietic cell transplantation was performed in 71 (76%) patients; 6 of the 71 (8.5%) patients developed moderate or severe sinusoidal obstruction syndrome after transplantation. Four-year overall survival rate was 32% (95% confidence interval 24%-43%). Patients responding to salvage therapy and undergoing allogeneic hematopoietic cell transplantation (n=51) had a 4-year survival rate of 49% (95% confidence intervaI 37%-64%). Patients with fms-like tyrosine kinase internal tandem duplication positive acute myeloid leukemia had a poor outcome despite transplantation. In conclusion, the described regimen is an effective and tolerable salvage therapy for patients who are primary refractory to one cycle of conventional intensive induction therapy.
