Abstract

Objective: Management of follow-up protocols after endovascular aortic repair (EVAR) varies significantly between centers and is not standardized according to sac regression. By designing an international expert-based Delphi consensus, the study aimed to create recommendations on follow-up after EVAR according to sac evolution. Methods: Eight facilitators created appropriate statements regarding the study topic that were voted, using a 4-point Likert scale, by a selected panel of international experts using a three-round modified Delphi consensus process. Based on the experts' responses, only those statements reaching a grade A (full agreement ≥75%) or B (overall agreement ≥80% and full disagreement <5%) were included in the final document. Results: One-hundred and seventy-four participants were included in the final analysis, and each voted the initial 29 statements related to the definition of sac regression (Q1-Q9), EVAR follow-up (Q10-Q14), and the assessment and role of sac regression during follow-up (Q15-Q29). At the end of the process, 2 statements (6.9%) were rejected, 9 statements (31%) received a grade B consensus strength, and 18 (62.1%) reached a grade A consensus strength. Of 27 final statements, 15 (55.6%) were classified as grade I, whereas 12 (44.4%) were classified as grade II. Experts agreed that sac regression should be considered an important indicator of EVAR success and always be assessed during follow-up after EVAR. Conclusions: Based on the elevated strength and high consistency of this international expert-based Delphi consensus, most of the statements might guide the current clinical management of follow-up after EVAR according to the sac regression. Future studies are needed to clarify debated issues.

Abstract

Purpose Radiation safety performance is often evaluated using dose parameters measured by personal dosimeters and/or the C-arm, which provide limited information about teams' actual radiation safety behaviors. This study aimed to develop a rating scale to evaluate team radiation safety behaviors more accurately and investigate its reliability. Materials and Methods A modified Delphi consensus was organized involving European vascular surgeons (VS), interventional radiologists, and interventional cardiologists. Initial items and anchors were drafted a priori and rated using five-point Likert scales. Participants could suggest additional items or adjustments. Consensus was defined as >= 80% agreement (rating >= 4) with Cronbach's alpha >= .80. Two VS with expertise in radiation safety evaluated 15 video-recorded endovascular repairs of infrarenal aortic aneurysms (EVAR) to assess usability, inter and intrarater reliability. Results Thirty-one of 46 invited specialists completed three rating rounds to generate the final rating scale. Five items underwent major adjustments. In the final round, consensus was achieved for all items (alpha = .804; agreement > 87%): 'Pre-procedural planning', 'Preparation in angiosuite/operating room', 'Shielding equipment', 'Personal protective equipment', 'Position of operator/team', 'Radiation usage awareness', 'C-arm handling', 'Adjusting image quality', 'Additional dose reducing functions', 'Communication/leadership', and 'Overall radiation performance and ALARA principle'. All EVARs were rated, yielding excellent Cronbach's alpha (.877) with acceptable interrater and excellent intrarater reliability (ICC = .782; ICC = .963, respectively). Conclusion A reliable framework was developed to assess radiation safety behaviors in endovascular practice and provide teams with formative feedback. The final scale is provided in this publication.

32. Physical characteristics of soybean cultivated under the conditions of integrated agrosystems

Abstract

Integrated crop-livestock-forest (ICLF) systems involve intercropping of various crops to achieve beneficial and synergistic outcomes, enhancing both economic viability and environmental sustainability. Considering the complexity of integrated agrosystems and the economic importance of soybean production in Brazil, we aimed to investigate the effects of two ICLF systems on the physical characteristics and quality of soybeans produced. The treatments comprised plots (two ha) with either single-row (ICLFS) or triple-row (ICLFT), tree configurations intercropped with soybean (maize and forage grass), and control plots (one ha), whose crops were cultivated under full sunlight (CFS). Soybeans were harvested from plants located at 3, 6, 10 and 15 m from tree bands in the north and south faces of the ICLF systems and at random positions in the CFS plots. The moisture content, electrical conductivity of the exudate solution, hue angle and chroma index of grains harvested from ICLFS and ICLFT were similar to those of CFS-grown soybeans. However, the mass of 1000 grains and the bulk density values of ICLF-grown soybeans were significantly higher (P ≤ 0.05) than those of grains harvested from CFS plots. We conclude that the quality of soybean seeds was not negatively affected by the conditions prevailing in the ICLF systems. Moreover, it appears that the forest component contributed positively to the ecosystem by providing a favorable microclimate for the development of soybean grains.

Abstract

Since the rst endovascular repair of an infrarenal abdominal aortic aneurysm (AAA) published by Parodi et al.,1 stent gras have undergone continuous improvement. Devices have evolved to highly complex, customdesigned devices incorporating branches and fenestrations. ese developments provide an endovascular solution for complex aortic aneurysms (CAAs) incorporating the visceral branches, iliac arteries and supra-aortic trunks (SAT), expanding the indications of EVAR.

PURPOSE: Morphologic abnormalities of the hippocampal formations (MAHF) are more frequently observed in magnetic resonance imaging (MRI). We wished to specify the types of disorders associated with these malformations based on a retrospective case series by studying the language of the children presenting these abnormalities. PATIENTS AND METHODS: From the data of all the MRIs taken in the neuroradiology ward of our center over 16 months in patients under 18 years of age, we retrospectively selected the children with an MAHF, isolated or associated with other malformations. The MAHFs were defined and described according to criteria of shape or orientation defects of the hippocampal formations. We studied the files of the patients with isolated MAHF again. Those whose clinical presentation was compatible with language assessment were tested in a prospective approach. RESULTS: Out of 2208 MRIs from 1 January 2007 to 30 April 2008, 96 (4.3%) showed an MAHF, including 61 (64%) boys and 35 (36%) girls, aged from 2 months to 17 years. Eighty-two (85%) had associated abnormalities, mainly including cerebral atrophy, corpus callosum agenesis or defect, and abnormal ventricular frontal horns. Fourteen (15%) had an isolated MAHF: 2 on the left hemisphere, 2 on the right hemisphere, and 10 on both. Of these 14, 9 were compatible with language assessment. From the test results, we divided these children into 2 groups, depending on the type and severity of the impairment. Four had very serious language disorders as part of mental retardation or autistic disorders; 4 others had language disorders predominantly in expression and phonology, with weak to pathological visual memory. This study showed no potential relation between the lateralization of MAHF and language disorders, nor between the existence of epilepsy and the severity of the language disorders. Of these 14 children, 9 had behavior and autism spectrum disorders and 7 were epileptic. CONCLUSION: Even though language disorders are often part of a larger deficiency presentation, the results we obtained suggest that isolated MAHFs are not only causes of amnestic disorders, but they could also directly underlie language disorders, particularly in expression.

Abstract

Type A aortic dissection remains the last challenge of endovascular repair due to the complex anatomy of the ascending aorta and its proximity to the heart and the supra-aortic trunks that makes precise deployment of endografts an extremely difficult procedure. Currently, there are no endovascular devices specifically developed for this aortic region. Thus, open repair is the gold standard approach, but high-risk patients are not eligible for cardiac surgery because of the significant morbidity and mortality associated with these procedures. Therefore, development of a less invasive treatment is an issue that must be addressed. To our best knowledge, there are only a few cases of type A aortic dissections reported in the literature which have been successfully treated with a totally endovascular approach. Two recent CT-based practicability-studies encourage this new approach, describing it as a suitable option for 31-39% of patients. Although the available literature is scant and the experience limited, endovascular repair appears as a treatment option for type A aortic dissections, but further evaluation is mandatory.

40. Effect of thinning eucalyptus trees on soybean productivity in integrated crop-livestock-forestry systems

Abstract

ABSTRACT: The aim of the study was to evaluate the effects of integrated crop-livestock-forestry (ICLF) systems and the thinning of eucalyptus trees on the agronomical performance of soybean. Treatments consisted of cultivation under: crops under full sunlight (CFS) conditions; ICLF with triple-row tree configuration (ICLFT) in which trees were submitted to selective thinning in the fifth year after planting through removal of 50% of trees while maintaining triple-row bands; and ICLF with single-row tree band configuration (ICLFS) in which the lateral rows of the triple-row tree bands were subjected to systematic thinning at the fourth year after planting. The physiological and agronomical variables of the soybean crop were evaluated at the R5 and R8 reproductive stages during the eighth harvesting season (2018/2019). Soybean sampling was carried out at five random positions in the CFS and in four transects at distances of 3, 6, 10 and 15 m from the tree bands in both the north and south faces of the ICLF systems. There were no differences between soybean grown under the ICLF and CFS systems with respect to specific leaf area, plant density, lodging index and mass of a thousand grains. However, the ICLF increased dry leaf mass and leaf area index and reduced soybean plant heights. Soybean productivity was reduced by 26% in ICLF-T and 14% in ICLF-S, that is, a 12% reduction in the productivity loss with systematic thinning. It is concluded that ICLF reduces soybean productivity in the effective grain production area of the system, regardless of the degree of thinning, although systematic thinning by removing the lateral tree lines to conversion of triple-rows into single-rows minimizes the loss of soybean grain yield.

Abstract

The aim of this study was to compare the results of complex aneurysm (hostile neck anatomies) repair in high-risk patients with two minimally invasive techniques, fenestrated endografting (f-EVAR where EVAR stands for endovascular aneurysm repair) and minilaparotomy.All high-risk patients (N.=50, group 1) with hostile neck abdominal aortic aneurysms (AAAs) operated in the vascular surgery department of the "Policlinico Universitario G. Martino" of Messina (Italy) during a 5-year period (January 2006-December 2010) were cross-matched with 50 similar patients (group 2) treated in the Vascular Surgery Department of the "Hopital Cardiologique" University of Lille (France) with similar anatomies, comorbidities and risk factors. The patients in group 1 underwent open minilaparotomy surgery, and the patients in group 2 were treated with f-EVAR. The aim of our study was to compare perioperative complications, survival and reintervention rates.Perioperative cardiac complications occurred in 5 patients (10%) in group 1, and 1 patient (2%) in group 2 (P0.092). Renal impairment not requiring permanent hemodialysis was significantly higher in group 1 (14% vs. 2% P0.027), as well as respiratory complications (32% vs. 2% P0.0001). Five patients (10%) in group 1 underwent reintervention vs. 4 patients in group 2 (P0.7268). There was no statistically significant difference for survival rates at 30 days (92% in group 1 and 96% in group 2; P=0.399); at six months (90% vs. 96%; P=0.239); at one year (90% vs. 96%; P=0.239); and at two years (84% vs. 94%; P=0.110). However, we observed statistically significant differences in survival rates at three years (74% vs. 94%; P0.006); at four years (70% vs. 86%; P0.005); and at five years (65% vs. 68%; P0.003).Our results showed that both techniques are effective in the treatment of AAA with hostile neck in high-risk patients. Although operative mortality rate was not statistically different, f-EVAR showed better results in terms of early complications and late survival.

Abstract

International audience; A design study of propeller hydrokinetic turbines is explored in the present paper, where the optimized blade geometry is determined by the classical Glauert theory applicable to the design of axial flow turbines (hydrokinetic and wind turbines). The aim of the present study is to evaluate the optimized geometry for propeller hydrokinetic turbines, observing the effect of the number of blades in the runner design. The performance of runners with different number of blades is evaluated in a specific low-rotational-speed operating conditions, using blade element momentum theory (BEMT) simulations, confirmed by measurements in wind tunnel experiments for small-scale turbine models. The optimum design values of the power coefficient, in the operating tip speed ratio, for two-, three- and four-blade runners are pointed out, defining the best configuration for a propeller 10 kW hydrokinetic machine.
