Background Acute post-cataract endophthalmitis (APE) is a rare complication potentially causing irreversible visual loss. A 10-year study of APE was conducted to determine its incidence, microbiological spectra and antibiotic resistance profile of APE-related pathogens at a major tertiary referral center in Brazil. Methods APE cases reported between January 2010 and December 2019 were included. Phacoemulsification and extracapsular cataract techniques were eligible; combined procedures, traumatic and congenital cataract were excluded. Vitreous samples were cultured and antimicrobial resistance was compared for the periods of 2010–2014 and 2015–2019. The results were analyzed with Fisher’s exact test. Results Our sample consisted of 40,491 cataract surgeries and 51 (0.126%) APE cases. Culture was positive in 35 cases (71.4%), of which 31 (88.6%) Gram-positive, 3 (8.6%) Gram-negative, and 1 (2.9%) fungal. The most frequently isolated organism was Staphylococcus epidermidis (n = 17/35, 48.6%), followed by Staphylococcus aureus (n = 4/35, 11.4%). From 2010–2014 to 2015–2019, antimicrobial resistance increased against moxifloxacin (11.1–54.5%, p = 0.07), ciprofloxacin (54.5–72.7%, p = 0.659) and oxacillin (66.7–93.3%, p = 0.13). Conclusions The observed incidence and microbial spectra were compatible with previous studies. A trend towards growing moxifloxacin and ciprofloxacin resistance was observed. Surveillance remains crucial to prevent treatment failure from antimicrobial resistance.

To study and compare visual acuity, foveal thickness, outer limiting layer, ellipsoid zone, and recurrence rate in patients undergoing removal of the epiretinal membrane with and without the removal of the internal limiting membrane (ILM).Sixty-three patients who had the epiretinal membrane removed by a single surgeon were randomly assigned into 2 groups: Group 1 without additional removal of the ILM and Group 2 with removal of the ILM. Patients were followed up and evaluated at the first month, third month, and sixth month, postoperatively.Patients from both groups had a gradual improvement in their vision over time. There was no significant difference in the improvement in visual acuity between the two groups. About tomographic assessment of alterations, no significant differences were found between the groups; however, Group 1 had a higher relapse rate (17%) compared with Group 2 (3.6%) (P = 0.09).Epiretinal membrane removal with and without ILM peeling shows similar functional and anatomical improvements, but the group in which the ILM was not removed seemed to have a higher recurrence rate.

Pegaptanib sodium is a selective RNA aptamer that inhibits the 165 isoform of vascular endothelial growth factor (VEGF), a key regulator of pathological vascular growth and permeability and a contributor to vision loss in diseases such as age-related macular degeneration (AMD), diabetic retinopathy and retinal vein occlusion. The development of pegaptanib represented an important achievement in that it was both the first approved anti-VEGF agent for the treatment of ocular disease and the first clinically validated aptamer therapeutic. The safety and efficacy of pegaptanib for the treatment of neovascular AMD were established in the VEGF Inhibition Study in Ocular Neovascularization (VISION) trials. Investigations into the use of pegaptanib have now been expanded to include diabetic macular edema, macular edema secondary to central retinal vein occlusion and other ocular vascular diseases. Pegaptanib is also being used investigationally in combination with other agents.

Pegaptanib sodium (Macugen) is a selective RNA aptamer that inhibits vascular endothelial growth factor (VEGF) 165 , the VEGF isoform primarily responsible for pathologic ocular neovascularization and vascular permeability, while sparing the physiological isoform VEGF 121 . After more than 10 years in development and preclinical study, pegaptanib was shown in clinical trials to be effective in treating choroidal neovascularization associated with age-related macular degeneration. Its excellent ocular and systemic safety profile has also been confirmed in patients receiving up to three years of therapy. Early, well-controlled studies further suggest that pegaptanib may provide therapeutic benefit for patients with diabetic macular edema, proliferative diabetic retinopathy and retinal vein occlusion. Notably, pegaptanib was the first available aptamer approved for therapeutic use in humans and the first VEGF inhibitor available for the treatment of ocular vascular diseases.

Objective To study effects of intravitreal pegaptanib (Macugen) on retinal neovascularization. Design Retrospective analysis of a randomized clinical trial. PARTICIPANTS, INTERVENTION, AND MAIN OUTCOME MEASURES: Individuals with retinal neovascularization identified from a multicenter, randomized, controlled trial evaluating pegaptanib for treatment of diabetic macular edema, with a best-corrected visual acuity letter score between 68 and 25 (approximate Snellen equivalent between 20/50 and 20/320) and receiving a sham injection or intravitreal pegaptanib (0.3 mg, 1 mg, 3 mg) administered at study entry, week 6, and week 12, with additional injections and/or focal photocoagulation as needed during the ensuing 18 weeks, up to a maximum of 6 pegaptanib/sham therapies, were evaluated. Scatter panretinal photocoagulation before study enrollment was permitted, but not within 6 months of randomization and study entry. Changes in retinal neovascularization were assessed on fundus photographs and fluorescein angiograms graded at a reading center in a masked fashion. Results Of 172 participants, 19 had retinal neovascularization in the study eye at baseline. Excluding 1 who had scatter photocoagulation 13 days before randomization and 2 with no follow-up photographs, 1 of the remaining 16 subjects had panretinal photocoagulation during study follow-up. Of these 16 subjects, 8 of 13 (62%) in a pegaptanib treatment group (including the one receiving panretinal photocoagulation), 0 of 3 in the sham group, and 0 of 4 fellow (nonstudy) eyes showed either regression of neovascularization on fundus photographs or regression or absence of fluorescein leakage from neovascularization (or both) at 36 weeks. In 3 of 8 with regression, neovascularization progressed at week 52 after cessation of pegaptanib at week 30. Conclusions Most subjects with retinal neovascularization at baseline assigned to pegaptanib showed regression of neovascularization by week 36. These findings suggest a direct effect of pegaptanib upon retinal neovascularization in patients with diabetes mellitus.

Background: Variability is introduced in three-dimensional (3-D) ultrasound assessments of intraocular tumours when multiple observers examine the same tumour or when multiple scans and measurements of the same tumour are carried out. We evaluated the magnitude and sources of variation associated with 3-D ultrasound measurements of choroidal tumour volume. Methods: Three examiners performed 3-D ultrasonography of five choroidal tumours. Each examiner performed three scans of each tumour and then measured each scan three times to determine the volume. We determined the variation attributable to differences between examiners, scans and measurements by evaluating the contribution of each to the overall variation in tumour volume. A similar process was performed for tumour height for comparison purposes. Results: The reliability of tumour volume measurements was found to be very high. The amount of overall variation, as defined by the coefficient of variation, was similar for tumour volume and tumour height and ranged from 4% to 17%. A total of 97% of the variation in tumour volume measurements and 84% of the variation in tumour height measurements was due to differences between tumours. Differences between examiners accounted for the most of the remaining variation (2% for volume and 7% for height). Interpretation: Three-dimensional ultrasonography of intraocular tumours was highly reliable. Differences between examiners were the main source of variation.

PURPOSE : To report the foveal anatomy in patients with perifoveal posterior vitreous detachments (PPVD) using optical coherence tomography. DESIGN : Retrospective observational case series. METHODS : A retrospective study of fellow eyes of patients of patients with macular holes and those with early macular hole states were examined with optical coherence tomography, biomicroscopy, and contact b-scan ultrasonography. Twenty-three eyes of 23 patients were discovered to have PPVD, which was defined as persistent attachment of the vitreous to the central macula with a limited detachment of the posterior vitreous in the perifoveal region. RESULTS : The mean diameter of persistent vitreous attachment in eyes with no foveal deformation was 1829 μm, flattening of the foveal depression 840 μm, and with foveal cavitation 281 μm ( P P = .001, Kruskal-Wallis test, all pairwise comparisons showing significant difference using the Mann-Whitney test). CONCLUSIONS : This study found that the diameter of the vitreous attachment in eyes with PPVD correlated with induced changes in foveal anatomy. The diameters of vitreous attachment were consistent with known regions of robust attachment of the vitreous determined histologically. Although the actual force loading on the central macula cannot be determined in patients with perifoveal posterior vitreous detachments, the stress, which is force / unit area may well increase with smaller areas of attachment leading to mechanical failure of the macula.

Purpose: To evaluate and describe the precautions involved in the technique of intravitreal injection of antiangiogenic drugs adopted by the ophthalmologists who are members of the Brazilian Society of Retina and Vitreous (SBRV). Methods: A questionnaire containing 22 questions related to precautions taken before, during, and after intravitreal injection was sent electronically to 920 members of SBRV between November 15, 2013 and April 31, 2014. Results: 352 responses (38%) were obtained. There was a predominance of men (76%) from the southwest region of Brazil (51%). The professional experience varied between 6 and 15 years after medical specialization (50%). Most professionals (76%) performed an average of 1 to 10 intravitreal injections a week, and 88% of the procedures were performed in the operating room using povidone iodine (99%), sterile gloves, and blepharostat (94%). For inducing topical anesthesia, usage of anesthetic eye drops was the most used technique (65%). Ranibizumab (Lucentis®) was the most common drug (55%), and age-related macular degeneration (AMD) was the most treated disease (57%). Regarding the complications treated, 6% of the ophthalmologists had treated at least one case of retinal detachment, 20% had treated cases of endophthalmitis, 9% had treated cases of vitreous hemorrhage, and 12% had encountered cases of crystalline lens touch. Conclusion: Intravitreal injection is a procedure routinely performed by retina specialists and has a low incidence of complications. Performing the procedure in the operating room using an aseptic technique was preferred by most of the respondents. Ranibizumab was the most used drug, and AMD was the most treated disease. Objetivo: Avaliar e descrever os cuidados envolvidos durante o procedimento de injeção intravítrea de drogas antiangiogênicas realizado pelos oftalmologistas membros da Sociedade Brasileira de Retina e Vítreo (SBRV). Métodos: Foi enviado um questionário aos 920 membros da SBRV, por meio de correio eletrônico, entre o período de 15/11/2013 a 31/04/2014, contendo 22 questões, relacionado aos cuidados pré, intra e pós-operatório da injeção intravítrea. Resultados: Foram obtidas 352 respostas (38% dos sócios). Houve um predomínio do sexo masculino (76%), procedentes da região Sudeste (51%). O tempo de experiência profissional se concentrou entre 6 a 15 anos após o término da especialização (50%). A maioria dos participantes tem média semanal de 1 a 10 (76%), sendo 88% das vezes realizado dentro do centro cirúrgico, utilizando iodopovidona (99%), luvas e blefarostato estéreis (94%). A anestesia tópica com colírio anestésico foi a técnica mais utilizada (65%). Entre os participantes, ranibizumabe (Lucentis®) é a droga mais utilizada (55%) e a degeneração macular relacionada a idade (DMRI) é a doença mais tratada (57%). Das complicações citadas pelos oftalmologistas, 6% já vivenciaram pelo menos um caso de descolamento de retina, 20% endoftalmite, 9% hemorragia vítrea e 12% toque cristaliniano. Conclusão: A injeção intravítrea é um procedimento realizado rotineiramente por retinólogos, com baixo índice de complicações. A realização do procedimento no centro cirúrgico com técnica asséptica é preferida pelos pesquisados. A droga mais utilizada foi o ranibizumabe e a doença mais tratada foi a DMRI.

Objective To differentiate polypoidal choroidal vasculopathy (PCV) from central serous chorioretinopathy (CSC). Design A retrospective, observational case series. Participants Thirteen patients originally diagnosed with CSC proved to have PCV after more extensive evaluation and follow-up. Methods A clinical and angiographic review of patients with manifestations of CSC, including macular detachment. Main outcome measures Demographic data, funduscopic examination, and fluorescein and indocyanine green (ICG) angiographic findings. Results Thirteen patients initially suspected of having CSC were ultimately diagnosed as having PCV. These eyes had exudative macular detachments secondary to a small caliber, polypoidal choroidal vascular abnormality or so-called polypoidal choroidal neovascularization. The clinical manifestations in the fundus varied. They included multiple, variably sized serous pigment epithelial detachments, neurosensory retinal detachment, lipid deposition, patchy atrophy of the pigment epithelium and indistinct staining from decompensation of the posterior blood-retinal barrier on fluorescein angiography. In reality, the suspected PEDs proved to be polypoidal lesions of PCV when imaged with ICG angiography. Conclusions The clinical diagnosis of CSC or PCV generally poses little challenge to the experienced retinal specialist. However, in CSC with persistent and/or recurrent exudation, a myriad of retinal pigment epithelial changes may evolve that make it difficult to differentiate these two entities. In such patients, ICG angiography is useful in differentiating CSC from PCV. An accurate clinical diagnosis is important since each of these entities, CSC and PCV, may differ in terms of their risk factors, natural course, and visual prognosis.

Objective To assess the rate of pegaptanib-associated sustained intraocular pressure (IOP) elevation. Methods A posthoc analysis was conducted on all IOP measurements, except the immediate 30-min postinjection, from all subjects randomised to pegaptanib 0.3 mg or sham injections continuously in the first 2 years of the Vascular endothelial growth factor Inhibition Study in Ocular Neovascularisation (V.I.S.I.O.N.) study. Measurements were taken with Goldmann applanation tonometer or Tonopen, except at baseline and in cases of an IOP reading >30 mm Hg when a Goldmann applanation tonometer was mandatory. Results Of 221 subjects, IOP measurements ≥22 mm Hg were seen in 28/114 and 23/107 subjects of the pegaptanib and sham subgroups, respectively (p=0.6338) and measurements ≥24 mm Hg were observed in eight and eight subjects in the pegaptanib and sham groups, respectively. More than two measurements ≥22 mm Hg occurred in six and 10 subjects (p=0.3025), and more than two measurements ≥24 mm Hg were observed in one and four subjects in the pegaptanib and sham groups, respectively. One patient with sustained IOP elevation in the pegaptanib study group, and four in the sham group, had IOP lowering medication added during the course of the study. No subject required glaucoma surgery. Conclusions In V.I.S.I.O.N., after 2 years, there was no evidence of sustained IOP elevation associated with pegaptanib 0.3 mg use. Aims: To evaluate the safety of up to 3 years of pegaptanib sodium therapy in the treatment of neovascular age-related macular degeneration (NV-AMD). Methods: Two concurrent, prospective, multicentre, double-masked studies randomised subjects with all angiographic lesion compositions of NV-AMD to receive intravitreous pegaptanib sodium (0.3, 1 and 3 mg) or sham injections every 6 weeks for 54 weeks. Those initially assigned to pegaptanib were rerandomised to continue or discontinue therapy for 48 more weeks; sham-treated subjects continued sham, discontinued or received pegaptanib. At 102 weeks, subjects receiving pegaptanib 0.3 mg or 1 mg in years 1 or 2 continued; those receiving pegaptanib 3 mg or who did not receive treatment in years 1 and 2 were rerandomised to 0.3 mg or 1 mg for year 3. Results: As in years 1 and 2, pegaptanib was well tolerated in year 3. Adverse events were mainly ocular in nature, mild, transient and injection-related. Serious adverse events were rare. No evidence of systemic safety signals attributed to vascular endothelial growth factor inhibition arose in year 3. There were no findings in relation to vital signs or electrocardiogram results suggesting a relationship to pegaptanib treatment. Conclusion: The 3-year safety profile of pegaptanib sodium was favourable in patients with NV-AMD.

Objective To evaluate the safety and efficacy of pegaptanib sodium injection (pegaptanib) in the treatment of diabetic macular edema (DME). Design Randomized, double-masked, multicenter, dose-ranging, controlled trial. Participants Individuals with a best-corrected visual acuity (VA) between 20/50 and 20/320 in the study eye and DME involving the center of the macula for whom the investigator judged photocoagulation could be safely withheld for 16 weeks. Intervention Intravitreous pegaptanib (0.3 mg, 1 mg, 3 mg) or sham injections at study entry, week 6, and week 12 with additional injections and/or focal photocoagulation as needed for another 18 weeks. Final assessments were conducted at week 36. Main outcome measures Best-corrected VA, central retinal thickness at the center point of the central subfield as assessed by optical coherence tomography measurement, and additional therapy with photocoagulation between weeks 12 and 36. Results One hundred seventy-two patients appeared balanced for baseline demographic and ocular characteristics. Median VA was better at week 36 with 0.3 mg (20/50), as compared with sham (20/63) (P = 0.04). A larger proportion of those receiving 0.3 mg gained VAs of > or =10 letters (approximately 2 lines) (34% vs. 10%, P = 0.003) and > or =15 letters (18% vs. 7%, P = 0.12). Mean central retinal thickness decreased by 68 microm with 0.3 mg, versus an increase of 4 microm with sham (P = 0.02). Larger proportions of those receiving 0.3 mg had an absolute decrease of both > or =100 microm (42% vs. 16%, P = 0.02) and > or =75 microm (49% vs. 19%, P = 0.008). Photocoagulation was deemed necessary in fewer subjects in each pegaptanib arm (0.3 mg vs. sham, 25% vs. 48%; P = 0.04). All pegaptanib doses were well tolerated. Endophthalmitis occurred in 1 of 652 injections (0.15%/injection; i.e., 1/130 [0.8%] pegaptanib subjects) and was not associated with severe visual loss. Conclusions In this phase II trial, subjects assigned to pegaptanib had better VA outcomes, were more likely to show reduction in central retinal thickness, and were deemed less likely to need additional therapy with photocoagulation at follow-up.

RESUMO A hipotonia ocular secundária à ciclodiálise provoca alterações funcionais e anatômicas que podem comprometer definitivamente a acuidade visual. Quatro casos consecutivos, secundários a trauma e com hipotonia, são descritos. Três casos foram tratados com fotocoagulação (laser de argônio) da esclera, corpo ciliar e raiz da íris. Os parâmetros de laser utilizados foram mira de 100 μm, tempo de exposição de 0,2s e intensidade variando entre 1 e 1,2 W, em até 2 sessões. Um dos casos foi submetido a ciclopexia direta, com fixação do corpo-ciliar a esclera. Todos os casos evoluíram com fechamento da ciclodiálise e pressão intra-ocular entre 13 e 18 mmHg. Os autores concluem que a hipotonia ocular secundária à ciclodiálise pode ser revertida satisfatoriamente com o uso de laser ou, em casos refratários, através de ciclopexia direta, associada ou não ao laser.

Objective To describe peripapillary detachment in pathologic myopia (PDPM), a newly recognized fundus lesion. Design Retrospective medical record review. Methods We evaluated a series of myopic eyes that had a yellow-orange elevation of the retina and retinal pigment epithelium at the inferior border of the myopic conus. Results Twenty eyes of 15 patients were identified during a 17-year period to have characteristic findings of PDPM. The mean age of the patients was 58 years. They were followed up for an average of 6 years. The mean spherical equivalent correction was −11.00 diopters (D) (range, −6.00 to−16.00 D). The mean axial length was 27.4 mm (range, 25.3-28.9 mm). In each case, ophthalmic coherence tomographic examination showed a localized detachment of the retinal pigment epithelium and retina corresponding to the PDPM lesion. During the follow-up period, the lesion remained stable in all cases except for 1. No apparent negative effect on visual function was noted. Conclusions Peripapillary detachment in pathologic myopia is an asymptomatic, yellow-orange peripapillary detachment of the retinal pigment epithelium and retina in pathologic myopia. Recognition of this lesion is important to distinguish it from other fundus pathologic conditions, such as tumors or choroidal neovascularization, which require further investigation and treatment.

Editor,—American tegumentary leishmaniasis is an endemic disease occurring in Latin America, especially in Brazil.1 The agent is the protozoan Leishmania , transmitted by sand flies. Leishmania v braziliensis causes the mucocutaneous form of the disease, in which systemic dissemination to mucous membranes follows the primary ulcerative skin lesion. We report a patient with nasal and conjunctival mucous leishmaniasis who developed an epidermoid carcinoma in the orbit. ### CASE REPORT A 58 year old man presented with a painless conjunctival mass of 2 months' duration. He reported nasal and mouth wounds and nasal flattening for 3 years and had a depressed scar in his right leg caused by an ulcer that developed 6 years before. RESUMO Existem poucas descricoes de traumas oculares resultantes de explosao de bombas caseiras. Este artefato e geralmente utilizado para a construcao de pocos que funcionarao como fossas assepticas. O presente trabalho descreve dois casos de pacientes com este tipo de acidente e discute sua conduta e prevencao. O primeiro paciente apresentou evisceracao traumatica do olho direito, multiplas perfuracoes corneanas e esclerais no olho esquerdo, ferimentos corto-contusos suturados em ambas as palpebras inferiores e um na mao direita, alem de perfuracao timpanica bilateral. O outro paciente apresentou corpos estranhos de cornea e intra-oculares, opacidade de cristalino, massas na câmara anterior e sinequias anteriores em ambos os olhos, alem de amputacao de membro inferior esquerdo.

Objetivos: Determinar o efeito da panfotocoagulação retiniana (PFC) nos parâmetros topográficos do disco óptico e na camada de fibras nervosas da retina (CFNR) peripapilar em pacientes com retinopatia diabética proliferativa (RDP). Métodos: Este é um estudo observacional prospectivo, de centro único. Trinta e oito olhos de 26 pacientes diabéticos foram submetidos a PFC para retinopatia diabética proliferativa. As estereofotografias (EFs) e os parâmetros do disco óptico foram avaliados usando o retinógrafo Visucam da Zeiss e a Tomografia de varredura a laser (TVL), respectivamente. A espessura da CFNR peripapilar foi medida por tomografia de coerência óptica (OCT) e polarimetria de varredura a laser (PVL). Todas as medições foram realizadas no início e 12 meses após a conclusão da PFC. Resultados: Trinta e oito olhos de 26 pacientes (15 mulheres) com média de idade de 53,7 anos (faixa etária de 26 a 74 anos) foram recrutados. Nenhuma diferença significativa foi encontrada entre a média horizontal e vertical para relação escavação/disco óptico (E/D) determinadas pelas EFs antes e após o tratamento com PFC (P = 0,461 e 0,839, respectivamente). Os valores globais dos parâmetros do disco óptico analisados com a TVL não mostraram nenhuma mudança significativa entre o baseline e 12 meses após o tratamento (área do disco, área da escavação, área da rima, volume da escavação, volume da rima, relação área da escavação/área do disco, relação linear do tamanho da escavação/disco, profundidade média da escavação, profundidade máxima da escavação, medida do formato da escavação, variação na altura do contorno, espessura média da CFNR e área transversal da CFNR). O OCT e o PVL não encontraram diferença significativa entre a CFNR peripapilar antes e após a PFC (P = 0,114 e P = 0,813, respectivamente). Conclusões: Nossos resultados sugerem que a PFC nos padrões realizados no presente estudo não causa significantes alterações morfológicas no disco óptico em pacientes diabéticos com RDP após um ano de acompanhamento Purpose: To determine the effect of panretinal photocoagulation (PRP) on optic disc topographic parameters and peripapillary retinal nerve fiber layer (RNFL) in nonglaucomatous patients with proliferative diabetic retinopathy (PDR). Methods: This is a prospective, single center, observational study. Thirty-eight eyes of 26 diabetic patients underwent PRP for proliferative diabetic retinopathy. Stereoscopic disc photographs and optic nerve head (ONH) parameters were evaluated using the Zeiss fundus camera and the confocal scanning laser ophthalmoscopy (CSLO), respectively. The peripapillary RNFL thickness was measured by optical coherence tomography (OCT) and scanning laser polarimetry (SLP). All measurements were performed at baseline and 12 months after completion of PRP. Results: Thirty-eight eyes of 26 patients (15 female) with a mean age of 53.7 years (range 26 to 74 years) were recruited. No significant difference was found between mean horizontal and vertical cup to disc (C/D) ratio determined by stereoscopic disc photographs before and after PRP treatment (P=0.461 and 0.839, respectively). The global values of ONH parameters analyzed with the CSLO showed no significant change from baseline to 12 months (disc area, cup area, rim area, cup volume, rim volume, C/D area ratio, linear C/D ratio, mean cup depth, maximum cup depth, cup shape measure, height variation contour, mean retinal nerve fiber layer thickness and cross-sectional area). The OCT and SLP did not find significant difference between peripapillary RNFL before and after PRP (P=0.114 and P=0.813, respectively). Conclusion: Our results suggest that PRP as performed in this study does not cause significant morphological optic disk changes in diabetic PDR patients after one year of follow-up

Objetivo: Estudar e comparar a acuidade visual, metamorfopsia, espessura foveal, camada limitante externa e zona elipsoide por meio da Tomografia de Coerência Óptica (OCT), e a taxa de recorrência dos pacientes operados de remoção de membrana epirretiniana, com e sem a retirada da membrana limitante interna. Métodos: Pacientes com MER operados por um único cirurgião e randomizados, aleatoriamente, em dois grupos. Todos os pacientes tiveram a retirada da membrana epirretiniana: 35 pacientes do Grupo 01, sem a adicional retirada de membrana limitante interna, e 28 pacientes do Grupo 02, com a retirada dessa membrana. Os pacientes foram seguidos e avaliados no primeiro, terceiro e sexto mês. Resultados: Setenta pacientes operados no total, sendo sete excluídos por perda de seguimento. Os pacientes de ambos os grupos evoluíram com melhora gradativa da visão ao longo do tempo. No Grupo 01, a média de acuidade visual inicial foi 0,60 logMAR (±0,3 desvio padrão - DP), no primeiro mês foi 0,49 logMAR (±0,26 DP), no terceiro mês, 0,39 logMAR (±0,30 DP) e no sexto mês, 0,27 logMAR (±0,25 DP). No Grupo 02, a média de acuidade visual inicial foi 0,63 logMAR (±0,25 DP), no primeiro mês foi 0,44 logMAR (±0,26 DP), no terceiro mês, 0,41 logMAR (±0,35 DP), e no sexto mês, 0,43 logMAR (0,44 DP). Não houve diferença estatisticamente significante quanto à melhora da acuidade visual entre os dois grupos. Não foi encontrada diferença estatisticamente significante em relação às alterações na OCT entre os grupos. O Grupo 02 apresentou maior taxa de recidiva (17%) quando comparada com o Grupo 01 (3,6%), embora esta diferença não foi estatisticamente significante (p=0,09). Conclusão: Ambos os grupos apresentaram melhora funcional e anatômica semelhantes, mas o grupo no qual não se removeu a membrana limitante interna mostrou uma possível tendência à maior taxa de recidiva Objective: To study and compare the visual acuity, metamorphosis, foveal thickness, the outer limiting layer and the ellipsoid zone, studied by Optic Coherence Tomography (OCT), and the recurrence rate of patients undergoing removal of epiretinal membrane with and without the removal of the internal limiting membrane. Methods: Seventy patients undergoing removal of epiretinal membrane, by a single surgeon and randomly assigned into two groups. Group 1, without additional removal of internal limiting membrane, 35 patients, and group 2 with the removal of the internal limiting membrane, 28 patients. Both groups were followed and evaluated in the first, third and sixth month. Results: Patients of both groups developed with a gradual improvement of vision over time. In group 1, the mean initial visual acuity was 0.60 logMAR (±0.3 standard deviation - SD), the average visual acuity of the first month was 0.49 logMAR (±0.26 SD), in the third month 0.39 logMAR (±0.30 DP), and in the sixth month logMAR 0.27 (±0.25 SD). In group 2, the mean initial visual acuity was 0.63 logMAR (±0.25 SD), average visual acuity of the first month was 0.44 (±0.26 DP) logMAR, in the third month 0.41 logMAR (±0.35 DP), and in the sixth month 0.43 logMAR (±0.44 DP). There was no statistical difference in improvement in visual acuity between the two groups, there was no statistically significant differences related to tomographic alterations between the groups, but the group 2 showed a higher relapse rate (17%) compared to the group 1 (3.6%). Although the difference is not statistically significant (p=0.09). Conclusion: Both groups showed similar functional and anatomical improvement, but the group which the internal limiting membrane was not removed showed a higher recurrence rate

Objetivo: Comparar o efeito da terapia da fotocoagulação panretiniana (PFC) associada à injeção intravítrea de Ranibizumabe (RBZ) versus terapia isolada com PFC em pacientes com retinopatia diabética proliferativa (RDP) precoce, virgens de tratamento, com ou sem edema macular diabético (DME) durante 6 meses de acompanhamento. Projeto: Estudo prospectivo intervencionista, randomizado e controlado. Métodos: Sessenta olhos de 30 pacientes com RDP bilateral precoce foram randomizados para o grupo de estudo (GE) que foram tratados com PFC associado a duas injeções de RBZ intravítreo (0.5mg/0.05ml) ou para o grupo controle (GC) tratados apenas com PFC. Mudanças na acuidade visual (AV) corrigida, na sensibilidade ao contraste (SC) e na espessura foveal (EF) foram comparados no início, e nos 1, 3 e 6 meses após o tratamento. Resultados: No GE, a diferença na média da AV do baseline para o mês 6 teve um aumento significativo de + 3,4 letras (p = 0,006) e uma diminuição significativa na EF de - 47.6um (p < 0,001). No GC, a diferença na média da AV teve uma diminuição de - 3,4 letras (p = 0,04) e uma mudança na EF de -3.8 um (p = 0,96). Com relação ao teste de SC dentre os 28 olhos do GE, houve uma melhora no mês 6 em relação ao baseline nos ciclos: 1,5 (p < 0.001) e 3,0 ciclo (p=0.023). Dentre os 30 olhos do GC, não houve uma diferença estatística nos momentos estudados. Conclusão: A injeção intravítrea de RBZ associado com PFC pode ser um tratamento eficaz em olhos de pacientes com RDP precoce e EMD Purpose: To compare the efficacy of therapy with panretinal photocoagulation (PRP) and intravitreal ranibizumab (RBZ) injection versus PRP alone in patients with treatment-naive bilateral non-high risk proliferative diabetic retinopathy (PDR) with and without diabetic macular edema (DME) with a 6-month follow-up. Design: Prospective, interventional, randomized controlled trial. Methods: Sixty eyes of 30 patients with bilateral non-high risk PDR were randomized either to the study group (SG) receiving PRP plus two intravitreal ranibizumab injections (0.5mg/0.05ml), the first one week before and the second four weeks after the PRP or to the control group (CG) receiving PRP alone. Mean change in best-corrected visual acuity (BCVA), contrast sensitivity (CS) and central macular thickness (CMT) were compared at baseline and 1, 3 and 6 months after treatment. Results: Changes from baseline to 6 months showed in the SG an increased in the BCVA by + 3.4 letters (p= 0.006) with a decrease in CMT by - 47.6um (p < 0.001). In the CG, a decrease by - 3.4 letters (p = 0.04) and an decrease by -3.8um (p= 0.96). Regarding the CS in the SG, there was an improvement compared to baseline for the sixth month in the 1.5 (p < 0.001) and 3.0 cycles (p = 0.023). The CG did not show significant results from baseline to month 6. Conclusion: Intravitreal RBZ associated with PRP can be an effective treatment in eyes with non-high risk PDR and DME

OBJETIVO: Descrever prospectivamente o curso da doença de Vogt-Koyanagi-Harada (DVKH) com integração de parâmetros de atividades clínicos, estruturais e funcionais. MÉTODOS: Foram incluídos pacientes com diagnóstico da DVKH na fase aguda (parte I) e não aguda (tempo de doença maior que 12 meses; parte II). Os pacientes na fase aguda receberam tratamento inicial padronizado com pulsoterapia de metilprednisolona seguido de corticoterapia oral em doses lentamente regressivas, pelo período de 15 meses. As avaliações consistiram em exame clínico, retinografia, angiografias com fluoresceína (AGF) e indocianina verde (AIV) e tomografia de coerência óptica (TCO). Foram realizadas nos seguintes momentos: parte I, no diagnóstico e meses 1, 2, 4, 6, 9 e 12; parte II, na inclusão e a cada três meses. Eletrorretinograma campo total (ERGct) e eletrorretinograma multifocal (ERGmf) foram realizados na parte I, no 1.o mês e a cada seis meses e, na parte II, na inclusão e com 12 meses. A leitura dos exames, na parte I, foi efetuada por duas leitoras, não mascaradas; na parte II, foi realizada por três leitores mascarados e treinados, sendo considerada a leitura concordante entre, pelo menos, dois examinadores. As angiografias e TCO foram realizadas no aparelho Spectralis® (HRA+OCT, Heidelberg Engineering). Tratamento adicional com corticoterapia em doses imunossupressoras ou intensificação da imunossupressão sistêmica foi indicado nos casos com recidivas clínicas, na presença de sinais de atividade à AGF ou duas pioras consecutivas >= 30% no ERGct. Os sinais de atividade detectados na AGF, AIV e TCO foram denominados sinais subclínicos. RESULTADOS: Na parte I, foram incluídos nove pacientes (7F/2M) com idade mediana de 33 anos e intervalo mediano entre início dos sintomas e tratamento de 13 dias. Na apresentação inicial, sinais clínicos característicos da doença (coroidite difusa com hiperemia do disco óptico, descolamento seroso de retina e uveíte anterior acompanhados de sinais extraoculares) melhoraram dentro dos primeiros 30 dias em todos os casos. Os principais sinais subclínicos variaram no tempo de melhora ou desaparecimento: espessura de coroide (EC) subfoveal diminuiu para o valor mediano de 347u m aos 30 dias; dark dots diminuíram ao longo do seguimento, porém ainda estavam presentes aos 12 meses. Piora da inflamação foi observada em 17 de 18 olhos no tempo mediano de sete meses quando a redução do corticoide oral atingiu a dose média de 0,3mg/kg/d. Os sinais subclínicos mais frequentemente observados foram dark dots, fuzzy vessels e aumento da EC. Em 10 destes 17 olhos a piora foi acompanhada de queda da função pelo ERG. Três padrões de evolução puderam ser caracterizados: sem recidivas clínicas ou subclínicas (padrão A, 1 olho), com recidivas subclínicas somente (padrão B, 11 olhos) e com recidivas clínicas (padrão C, 6 olhos). Identificou-se que a EC aos 30 dias após início do tratamento >= 506u m teve sensibilidade e especificidade > 80% na detecção dos casos com recidivas clínicas (padrão C). A função pelo ERGct e ERGmf permaneceu alterada em relação ao grupo controle com 24 meses, apesar da melhora progressiva observada desde o início do tratamento. Na análise longitudinal dos pacientes, a função entre 12 e 24 meses permaneceu estável no grupo de doentes que recebeu tratamento adicional (8 olhos), enquanto no grupo que não o recebeu (4 olhos) houve deterioração da função ( < 0,001). Na análise dos grupos segundo padrão de recidiva, observou-se que os olhos com padrão B sem tratamento adicional tinham piora funcional maior em relação àqueles com padrão C ou B tratados (p < 0,001). Na parte II, foram incluídos 20 pacientes (17F/3M), com idade mediana ao diagnóstico de 31 anos, intervalo mediano entre início de sintomas e tratamento de 19 dias e tempo mediano de doença à inclusão de 55 meses. Na avaliação da concordância interobservador na leitura dos sinais subclínicos, EC teve concordância substancial (kappa=0,8), enquanto sinais angiográficos tiveram concordância sutil (kappa < 0,2). O curso da doença em 85% dos pacientes foi com recidiva clínica (padrão C, 11 casos) ou recidiva subclínica (padrão B, 6 casos). Nas 11 avaliações com detecção de células na câmara anterior (CA), sinais subclínicos de inflamação de segmento posterior foram concomitantemente observados em 64% dos olhos. Esta mesma concomitância de sinais subclínicos de inflamação de segmento posterior na presença de células na CA foi observada na parte I do estudo. Nos pacientes com padrão B, a variação da EC foi o principal sinal subclínico observado. A função pelo ERG foi realizada sequencialmente em 13 casos. Olhos com padrão C (7 pacientes), com grande comprometimento funcional desde a inclusão, evoluíram com piora mais acentuada do que aqueles com padrão B (5 pacientes). Ao se individualizar os olhos com padrão B, observou-se que esse diferencial (padrão B melhor que C) devia-se ao grupo padrão B com tratamento (p < 0,001). CONCLUSÕES: Neste estudo prospectivo de pacientes com DVKH em seguimento mínimo de 12 meses, desde as fases aguda e não aguda, três padrões de evolução foram observados, sendo que 94% (parte I) e 85% (parte II) dos pacientes apresentaram recidiva/piora clínica (padrão C) e/ou subclínica (padrão B). Na parte I do estudo, a piora da inflamação foi detectada aos sete meses de evolução durante dose regressiva do corticoide equivalente a 0,3mg/kg/d, apesar do tratamento inicial com corticoides em altas doses lentamente regressivo. A EC aferida 30 dias após o início do tratamento acima de 506 ?m mostrou-se um fator com sensibilidade e especificidade acima de 80% na identificação dos casos que evoluíram com recidivas clínicas. Dentre os sinais para detecção de inflamação subclínica, as alterações na EC são confiáveis, enquanto que sinais angiográficos devem ser interpretados com cautela. Exames sequenciais tornam a leitura mais confiável. A presença de células na CA comportou-se como a \"ponta do iceberg\" de uma inflamação mais difusa. O estudo eletrorretinográfico demonstrou resultado subnormal mesmo após 24 meses de seguimento na parte I; tratamento adicional pode evitar piora funcional nos pacientes com sinais subclínicos de inflamação. A pior função da retina em pacientes com inflamação clínica (padrão C) da parte II e dos pacientes com inflamação subclínica (padrão B) de ambas as partes do estudo sugerem que o tratamento ideal das recidivas inflamatórias ainda deve ser alvo de futuros estudos OBJECTIVES: To describe the course of Vogt-Koyanagi-Harada disease (VKHD) prospectively, integrating clinical, structural and functional parameters. METHODS: Patients with VKHD in the acute (part I) and non-acute (more than 12 months from diagnosis) phases (part II) were included. Patients in the acute phase received a standard treatment with methylprednisolone pulsetherapy followed by high-dose oral corticosteroids with slow tapering during 15 months. Evaluations included clinical exams, fluorescein (FA) and indocyanine green (ICGA) angiographies and optical coherence tomography (OCT). In part I, they were performed at inclusion, then after 1,2,4,6,9,and 12 months; in part II, they were performed at inclusion then every 3 months for up to 12 months. Functional evaluation using electroretinography (ERG) was performed at inclusion and every 6 months in part I and at inclusion and at 12 months in part II. Two non-blinded readers analyzed the imaging exams in part I. In part II, three trained and blinded-readers performed the imaging exams analysis. For study`s purpose, at least two concordant readings were considered. Imaging exams utilized the Spectralis® (HRA+OCT, Heidelberg engineering). Inflammatory signs detected on FA, ICGA and OCT were denominated as subclinical signs. Additional treatment with high doses of corticosteroids or more intensive systemic immunosuppression was indicated in cases with clinical signs of inflammation, with subclinical signs on FA or with two consecutive worsening > 30% on ERG. RESULTS: Nine patients (7F/2M) were included in part I; median age was 33 years old and median time elapsed from onset of symptoms to treatment was 13 days. At disease presentation, classic signs (choroiditis, anterior uveitis, serous retinal detachment, optic disc hyperemia and extraocular manifestations) were observed; they improved in 30 days after treatment. Subclinical signs improved in variable periods of time: subfoveal choroidal thickness (CT) decreased to a median value of 347 ?m, 30 days after the beginning of treatment, dark dots diminished during the follow-up but they were still observed at 12 months. Relapse (worsening of inflammation) was noticed in 17 of 18 eyes at a median follow up time of seven months, when tapering schedule corticosteroid dosage reached the mean dose of 0.3mg/kg/d of prednisone. Dark dots, fuzzy vessels and choroid thickening were the most frequent subclinical signs. Relapses in 10 of 17 eyes were concomitant with worsening on ERG. Three patterns of evolution could be delineated: no signs of inflammation (pattern A, 1 eye), only subclinical signs of inflammation (pattern B, 11 eyes) and clinical signs of inflammation (pattern C, 6 eyes). CT>=506 ?m 30 days after the beginning of treatment was more than 80% sensitive and specific to detect more severe cases (pattern C). ERG parameters at 24 months were subnormal as compared to the control group, despite improvement during follow-up. Further long-term results after 24 month demonstrated stabilization of ERG parameters in patients that had received additional treatment, whereas there was worsening in those patients who had not received additional treatment (p= 506 ?m had a greater than 80% sensitivity and specificity to detect cases with pattern C evolution. Considering subclinical signs, CT increase reliably detected recurrence, whereas angiographic signs required cautious interpretation. Sequential analysis was more conclusive than an isolated exam. Anterior chamber cells seemed to be the \"tip of the iceberg\" of a more diffuse inflammation. ERG analysis was subnormal even after 24 months of follow up since disease onset; additional treatment could prevent functional worsening in patients with subclinical signs of inflammation. Worse retinal function in patients with clinical recurrences (pattern C) in part II and subclinical recurrences (pattern B) in parts I and II suggest that ideal treatment of recurrences should be further pursued

INTRODUÇÃO: O presente estudo avaliou o tratamento com injeções intravítreas de Bevacizumabe (IVB) associadas à panfotocoagulação (PFC) da retina na retinopatia diabética proliferativa (RDP) de alto risco com ou sem edema macular (EM). MÉTODOS: Ensaio clínico randomizado, prospectivo, aberto e mascarado composto por pacientes com Diabetes melitos (DM) tipo 2. A acuidade visual (AV) foi medida com a tabela Early Treatment Diabetic Retinopathy Study e a sensibilidade ao contraste (SC) pela da tabela Vistech Consultants Incorporation 6500. Os pacientes foram submetidos a exame de angiofluoresceinografia para observação de neovascularização retiniana e isquemia macular e à tomografia de coerência óptica (OCT), para se obter a espessura foveal (EF) e o volume macular (VM). Após os exames, um dos olhos do mesmo paciente foi randomizado para realizar somente PFC, grupo controle (GC), e o outro para PFC associado a injeções IVB, grupo de estudo (GE). A hemorragia vítrea (HV) e a presença de complicações também foram avaliadas. RESULTADOS: Dos 42 pacientes incluídos, 35 completaram o estudo. A média de idade foi de 56±8 anos, com predominância do gênero masculino 21 (60%). Vinte e seis (74%) pacientes eram portadores de Hipertensão Arterial Sistêmica com média de duração de 9±10 anos. A média de duração do DM foi de 18±9 anos sendo 23 (66%) usuários de insulina e 21 (68,5%), fácicos. A AV e a SC não demonstraram diferença entre os grupos no total da amostra. O GE demonstrou melhora em comparação ao GC na EF no 1º mês, e no VM nos 1° e 3º meses de seguimento. Quanto aos 12 pacientes com EM bilateral somente a EF demonstrou redução no GE no 1º mês de seguimento. Ao se avaliar os grupos separadamente, o GC apresentou agravamento da AV e SC durante todo seguimento. Houve também aumento da EF nos 1º e 6º meses e VM nos 1º , 3º e 6º meses de seguimento. O GE demonstrou estabilização da AV, SC, EF e VM. Correlacionado às funções visuais, AV com a SC, toda vez que houve piora da AV esta foi acompanhada pelo agravamento da SC em todos os momentos no GC e GE. Quando correlacionadas as AV e SC com as EF e VM, toda vez que a espessura macular aumentava, havia piora da função visual. Dos sete pacientes excluídos do estudo por apresentarem HV, cinco integravam o GC e dois o GE. Não houve aparecimento de catarata, endoftalmite e/ou aumento significativo da pressão ocular. CONCLUSÃO: Na RDP de alto risco, o uso adjuvante de injeções intravítreas de Bevacizumabe associadas à panfotocoagulação da retina pode estabilizar a AV, SC, EF e VM, diminuir a incidência de HV e reduzir a da espessura macular. Em relação à correlação entre as variáveis, quando houve piora da AV, esta foi acompanhada da piora da SC e o aumento da EF e VM causaram piora da AV e SC INTRODUCTION: This study evaluated the treatment with intravitreal injections of Bevacizumab (IVB) associated with panretinal photocoagulation (PRP) in high-risk proliferative diabetic retinopathy (PDR) with or without diabetic macular edema (DME). METHODS: Prospective, open and masked, randomized clinical trial, composed of patients with type 2 Diabetes Mellitus (DM). The visual acuity (VA) was measured with the Early Treatment Diabetic Retinopathy Study charts and the contrast sensitivity (CS) through the chart of Vistech Consultants Incorporation 6500. Patients were submitted to a fluorescein angiography examination to observe retinal neovascularization and macular ischemia and to an optical coherence tomography (OCT) to obtain the foveal thickness (FT) and macular volume (MV). After the tests, one of the eyes from the same patient was randomized to realize only the PRP, the control group (CG), and the other for PRP associated to IVB injections, the study group (SG). Vitreous hemorrhage (VH) and presence of complications were also evaluated. RESULTS: Thirty-five of the forty-two patients included, completed the study. The mean age was 56±8 years, with a predominance of 21 (60%) males. Twenty-six (74%) patients had systemic hypertension with a mean duration of 9±10 years. The mean duration of DM was 18±9 years, of which 23 (66%) were insulin users and 21 (68.5%) were phakic. The VA and CS showed no difference between groups in the total sample. The SG showed improvement compared to the CG in FT for the 1st month, and in MV for the 1st and 3rd month of follow-up. As for the 12 patients with bilateral ME, only the FT showed a reduction in the SG for the 1st month of follow-up. When evaluating the groups separately, the CG showed worsening of VA and CS at all times. There was also an increase of FT for the 1st and 6th months and of MV for the 1st, 3rd and 6th month follow-up. The SG showed stabilization of VA, CS, FT and MV. When correlated to visual functions, VA and CS, a worsening of the VA was accompanied every time by a worsening of the CS in both the CG and SG. When VA and CS are correlated to FT and MV, there was worsening of visual function whenever macular thickness increased. Of the seven patients excluded from the study by presenting VH, 5 belonged to the CG and the 2 to the SG. There was no incidence of cataracts, endophthalmitis and/or significant increase in intraocular pressure. CONCLUSION: In high-risk PDR, intraocular injections of Bevacizumab as an adjuvant treatment to PRP, can stabilize VA, CS, FT and MV, reduce of the incidence of VH and decrease the macular thickness. Regarding the correlation between variables, when there was a worsening of VA, this was accompanied by a worsening of the CS, and an increase in FT and MV caused the worsening of the VA and CS
