2. Qualitative and quantitative reservoir characterization using seismic inversion based on particle swarm optimization and genetic algorithm: a comparative case study.

- Abstract
Accurate reservoir characterization is necessary to effectively monitor, manage, and increase production. A seismic inversion methodology using a genetic algorithm (GA) and particle swarm optimization (PSO) technique is proposed in this study to characterize the reservoir both qualitatively and quantitatively. It is usually difficult and expensive to map deeper reservoirs in exploratory operations when using conventional approaches for reservoir characterization hence inversion based on advanced technique (GA and PSO) is proposed in this study. The main goal is to use GA and PSO to significantly lower the fitness (error) function between real seismic data and modeled synthetic data, which will allow us to estimate subsurface properties and accurately characterize the reservoir. Both techniques estimate subsurface properties in a comparable manner. Consequently, a qualitative and quantitative comparison is conducted between these two algorithms. Using two synthetic data and one real data from the Blackfoot field in Canada, the study examined subsurface acoustic impedance and porosity in the inter-well zone. Porosity and acoustic impedance are layer features, but seismic data is an interface property, hence these characteristics provide more useful and applicable reservoir information. The inverted results aid in the understanding of seismic data by providing incredibly high-resolution images of the subsurface. Both the GA and the PSO algorithms deliver outstanding results for both simulated and real data. The inverted section accurately delineated a high porosity zone ( > 20 % ) that supported the high seismic amplitude anomaly by having a low acoustic impedance (6000-8500 m/s ∗ g/cc). This unusual zone is categorized as a reservoir (sand channel) and is located in the 1040-1065 ms time range. In this inversion process, after 400 iterations, the fitness error falls from 1 to 0.88 using GA optimization, compared to 1 to 0.25 using PSO. The convergence time for GA is 670,680 s, but the convergence time for PSO optimization is 356,400 s, showing that the former requires 88 % more time than the latter., (© 2024. The Author(s).)

Due to the excessive growth of PM 2.5 in aerosol, the cases of lung cancer are increasing rapidly and are most severe among other types as the highest mortality rate. In most of the cases, lung cancer is detected with least symptoms at its later stage. Hence, clinical records may play a vital role to diagnose this disease at the correct stage for suitable medication to cure it. To detect lung cancer an accurate prediction method is needed which is significantly reliable. In the digital clinical record era with advancement in computing algorithms including machine learning techniques opens an opportunity to ease the process. Various machine learning algorithms may be applied over realistic clinical data but the predictive power is yet to be comprehended for accurate results. This paper envisages to compare twelve potential machine learning algorithms over clinical data with eleven symptoms of lung cancer along with two major habits of patients to predict a positive case accurately. The result has been found based on classification and heat map correlation. K-Nearest Neighbor Model and Bernoulli Naive Bayes Model are found most significant methods for early lung cancer prediction., (© 2024. The Author(s).)

4. Diagnostic dilemma for human immunodeficiency virus in a fatal case of acute myeloid leukemia.

- Abstract
National Human Immunodeficiency Virus (HIV) testing programs utilize antibody-based tests for confirming HIV diagnosis which has a diagnostic window period of 23-90 days. In Fiebig acute HIV Stage I-II, an individual has antibody-negative but RNA-positive test results. Here, we present a case of a 54-year-old complete remission acute myeloid leukemia patient, who was recently reported HIV negative by antibody-based tests used in National HIV testing programs. However, when his sample was further analyzed by more sophisticated HIV tests, there was the presence of early anti-HIV-1 gp160 antibodies in western blot and HIV-1 RNA in nucleic acid testing. Within 8 days of his HIV-negative result, his clinical condition deteriorated. Later, the patient expired despite the best of clinical efforts at the apex tertiary care center of India. The technical difficulty in confirming HIV diagnosis by antibody-based tests used in National HIV testing programs and thereby noninitiation of antiretrovirals in a case where cell-mediated immunity is already compromised by non-HIV reason could have serious consequences. 5. Covering assisted intuitionistic fuzzy bi-selection technique for data reduction and its applications.

- Abstract
The dimension and size of data is growing rapidly with the extensive applications of computer science and lab based engineering in daily life. Due to availability of vagueness, later uncertainty, redundancy, irrelevancy, and noise, which imposes concerns in building effective learning models. Fuzzy rough set and its extensions have been applied to deal with these issues by various data reduction approaches. However, construction of a model that can cope with all these issues simultaneously is always a challenging task. None of the studies till date has addressed all these issues simultaneously. This paper investigates a method based on the notions of intuitionistic fuzzy (IF) and rough sets to avoid these obstacles simultaneously by putting forward an interesting data reduction technique. To accomplish this task, firstly, a novel IF similarity relation is addressed. Secondly, we establish an IF rough set model on the basis of this similarity relation. Thirdly, an IF granular structure is presented by using the established similarity relation and the lower approximation. Next, the mathematical theorems are used to validate the proposed notions. Then, the importance-degree of the IF granules is employed for redundant size elimination. Further, significance-degree-preserved dimensionality reduction is discussed. Hence, simultaneous instance and feature selection for large volume of high-dimensional datasets can be performed to eliminate redundancy and irrelevancy in both dimension and size, where vagueness and later uncertainty are handled with rough and IF sets respectively, whilst noise is tackled with IF granular structure. Thereafter, a comprehensive experiment is carried out over the benchmark datasets to demonstrate the effectiveness of simultaneous feature and data point selection methods. 6. Evaluation of SARS-CoV-2 Specific IgG Antibodies in Individuals Vaccinated with Covaxin (BBV152): An Independent Assessment.

Post vaccination with Covaxin (BBV152), Serum from healthcare professionals of Microbiology Department at apex tertiary referral hospital of India were tested for SARS-CoV-2 specific IgG antibodies. 70% individuals at 14-30 days, 63.1% individuals at 30-60 days but only 36.8% individuals at 60-90 days after second dose of vaccine had detectable SARS-CoV-2 spike protein specific IgG antibodies. However, 80% individuals at 14-30 days, 89.4% individuals at 30-60 days while 94.7% individuals at 60-90 days after second dose of vaccine had detectable SARS-CoV-2 whole cell antigen specific IgG antibodies. Males were lacking SARS-CoV-2 spike protein specific IgG antibodies in higher proportion than females and had lower index wherever detected. Age and co-morbidities were non-significant factors for post vaccination IgG response but not in breakthrough infection. 8.3% individuals developed mild COVID-19 symptoms post 14-90 days of second dose and none had severe COVID-19. SARS-CoV-2 spike protein specific IgG antibodies induced by Covaxin are drastically reduced in 60-90 days among fully vaccinated individuals which could be a potential risk for breakthrough SARS-CoV-2 infection, if not severe COVID-19. - Abstract
Objectives: Evaluation of immunological factors responsible for discordant virological responses postcombination antiretroviral therapy (cART) in human immunodeficiency virus (HIV)-positive children aged <5 years., Materials and Methods: Immunological profiling of enrolled 30 HIV-positive children was done at enrollment, 6 and 12 months. Flow cytometric analysis was performed for enumeration of counts and percentage of CD4
+ , CD8+ , and CD19+ cells; expression of CD19, CD86, PD-1, CD3, CD8 and CD28 on lymphocytes was evaluated using whole blood staining technique with monoclonal antibodies. HIV-1 viral load was quantified using a real-time polymerase chain reaction. Serum levels of immunoglobulin G (IgG), immunoglobulin A (IgA), immunoglobulin (IgM), and interleukin (IL)-7 were quantitated using quantitative enzyme-linked immunosorbent assay kits. The HIV-infected children were categorized into virological responders (VRs; HIV-1 plasma viral load <47 copies/mL) and virological nonresponders (VNRs; HIV-1 plasma viral load >1000 copies/mL) following 1-year cART., Results: The frequency of CD28+ CTLs cells was higher (P < 0.0001), and the frequency of CD28-CTLs cells was lower (P < 0.0001) in VRs than VNRs. CD28+ and CD28-CTLs cells correlated with HIV-1 plasma viremia (r = -0.4695, P = 0.01; r = 0.40, P = 0.03, respectively). VRs had higher CD19 percentage (P = 0.04) and count (P = 0.01) than VNRs. CD19+ B cells in the VRs had lower expression of CD86 (P = 0.03) and PD-1 (P = 0.002) than VNRs. VR had lower levels of serum IgG (P = 0.03), IgM (P = 0.04), and IL-7 (P = 0.01) than VNRs., Conclusions: High baseline B-cell counts, lower serum IgG, IgM, IL-7 levels, lower activation and exhaustion of B cells, and higher frequency of CD28+ CTLs are associated with positive virological response, whereas elevated CD28-CTLs are associated with the poor virological outcomes in HIV-infected children., Competing Interests: None- Published

- Abstract
Timely HIV testing and diagnosis are necessary to prevent the development of AIDS and interrupt its transmission in society. We collected the data on HIV testing and diagnosis in 2020 and compared it with preceding years to examine how COVID-19 pandemic-related restrictions impacted HIV services. The number of people who underwent HIV testing at the apex tertiary referral hospital of India in 2020 reduced by 57% compared to 2019 or the average/year during 2019-2016. Hence, the diagnosis of new HIV infections decreased by 52% compared to 2019 and 54% compared to the average/year during 2019-2016. Provider-initiated testing and diagnosis were more affected than client-initiated. There was a non-significant change in the rate of HIV detection among tested individuals. The male testing saw a more notable drop than female testing. HIV testing between ≥50 years and ≤14 years was more affected than other age groups. The transmission via regular partner/spouse increased, whereas it decreased via heterosexual commercial sex workers.

- Abstract
Reinfection with SARS-CoV-2 is not frequent yet the incidence rate of it is increasing globally owing to the slow emergence of drift variants that pose a perpetual threat to vaccination strategies and have a greater propensity for disease reoccurrence. Long-term protection against SARS-CoV-2 reinfection relies on the induction of the innate as well as the adaptive immune response endowed with immune memory. However, a multitude of factors including the selection pressure, the waning immunity against SARS-CoV-2 over the first year after infection possibly favors evolution of more infectious immune escape variants, amplifying the risk of reinfection. Additionally, the correlates of immune protection, the novel SARS-CoV-2 variants of concern (VOC), the durability of the adaptive and mucosal immunity remain major challenges for the development of therapeutic and prophylactic interventions. Interestingly, a recent body of evidence indicated that the gastrointestinal (GI) tract is another important target organ for SARS-CoV-2 besides the respiratory system, potentially increasing the likelihood of reinfection by impacting the microbiome and the immune response via the gut-lung axis. In this review, we summarized the latest development in SARS-CoV-2 reinfection, and explored the untapped potential of trained immunity. We also highlighted the immune memory kinetics of the humoral and cell-mediated immune response, genetic drift of the emerging viral variants, and discussed the current challenges in vaccine development. Understanding the dynamics and the quality of immune response by unlocking the power of the innate, humoral and cell-mediated immunity during SARS-CoV-2 reinfection would open newer avenues for drug discovery and vaccine designs.

11. The Evolution of Regulatory Elements in the Emerging Promoter-Variant Strains of HIV-1 Subtype C.

- Abstract
In a multicentric, observational, investigator-blinded, and longitudinal clinical study of 764 ART-naïve subjects, we identified nine different promoter variant strains of HIV-1 subtype C (HIV-1C) emerging in the Indian population, with some of these variants being reported for the first time. Unlike several previous studies, our work here focuses on the evolving viral regulatory elements, not the coding sequences. The emerging viral strains contain additional copies of the existing transcription factor binding sites (TFBS), including TCF-1α/LEF-1, RBEIII, AP-1, and NF-κB, created by sequence duplication. The additional TFBS are genetically diverse and may blur the distinction between the modulatory region of the promoter and the viral enhancer. In a follow-up analysis, we found trends, but no significant associations between any specific variant promoter and prognostic markers, probably because the emerging viral strains might not have established mono infections yet. Illumina sequencing of four clinical samples containing a coinfection indicated the domination of one strain over the other and establishing a stable ratio with the second strain at the follow-up time points. Since a single promoter regulates viral gene expression and constitutes the master regulatory circuit with Tat, the acquisition of additional and variant copies of the TFBS may significantly impact viral latency and latent reservoir characteristics. - Abstract
CD38 on CD8 + T cells is considered a reliable marker of HIV disease progression. Withania somnifer, a traditional ayurvedic medicine, has Th1 immunomodulatory properties. PBMCs from 38 HIV patients were exposed to Withania somnifer root extract at standardized concentration. An overall decline in the percentage of CD38 expressing CD8 + T lymphocytes was observed, though the statistical significance was varied with different categories of HIV patients. Background: A seafarer is a person who navigates waterborne vessels or assists as a crewmember in their operation and maintenance in all tough weather, but little research has been done to identify conditions that may lead to assess seafarer general health as well as oral health., Aim: To assess oral diseases including dental caries and periodontal conditions among seafarer's population arrived in Mundra Port, Kutch, Gujarat, India., Materials and Methods: A descriptive cross-sectional survey was conducted to assess oral health condition of seafarer community of Mundra Taluka of Kutch District, Gujarat, India, from July 2014 to September 2014., Results: Total of 385 subjects participated in the survey. Adverse habits show the overall 72.3% prevalence among the study population. Occurrence rate of caries, periodontal disease and prosthetic status were 88%, 75.1% and 6.5%, respectively. The best predictors for Decayed Missing Filled Teeth (DMFT), Community Periodontal Index (CPI) and prosthetic status were oral hygiene practices, adverse habit and educational status., Conclusions: Findings of the present study suggest that oral health condition of seafarer community was relatively poor, with high caries prevalence and poor periodontal health. This epidemiological survey has provided baseline information to underpin the implementation of oral health programmes.

17. Experimental studies of active and passive flow control techniques applied in a twin air-intake.

- Abstract
The flow control in twin air-intakes is necessary to improve the performance characteristics, since the flow traveling through curved and diffused paths becomes complex, especially after merging. The paper presents a comparison between two well-known techniques of flow control: active and passive. It presents an effective design of a vortex generator jet (VGJ) and a vane-type passive vortex generator (VG) and uses them in twin air-intake duct in different combinations to establish their effectiveness in improving the performance characteristics. The VGJ is designed to insert flow from side wall at pitch angle of 90 degrees and 45 degrees. Corotating (parallel) and counterrotating (V-shape) are the configuration of vane type VG. It is observed that VGJ has the potential to change the flow pattern drastically as compared to vane-type VG. While the VGJ is directed perpendicular to the side walls of the air-intake at a pitch angle of 90 degree, static pressure recovery is increased by 7.8% and total pressure loss is reduced by 40.7%, which is the best among all other cases tested for VGJ. For bigger-sized VG attached to the side walls of the air-intake, static pressure recovery is increased by 5.3%, but total pressure loss is reduced by only 4.5% as compared to all other cases of VG.

19. "Shankha prakshalana" (gastrointestinal lavage) in health and disease.

- Abstract
Shankha Prakshalana is an easy and effective technique for cleansing the gastrointestinal canal, thereby removing all the autointoxicants present in the alimentary canal. Moreover, it is a very economical procedure which can be performed by the patients at his own residence. The role of Shankha Prakshalana from our own experience is very effective in overcoming the irritable bowel syndrome. Shankha Prakshalana may also be given to the patients of anxiety neurosis with satisfactory results. Its role in the treatment of renal disorder is encouraging in early cases only. However, in the second and third degree chronic renal failure cases its role is yet to be established and needs further investigation. Similarly, in the case of thyrotoxicosis no conclusive evidence could be observed due to very limited number of cases.
