Summary Background With the experience-based hypothesis of low donor site morbidity (DSM) for free flaps from the distal femur, this cohort study aimed to evaluate the DSM according to objective and reproducible criteria. Methods One hundred and fifty-six patients who had a flap harvest from either the medial or lateral femoral condyle region between 2005 and 2017 were included. A retrospective chart review was performed for all patients. In total, 97 patients were available for a follow-up examination. Outcomes were assessed according to objective (Knee Society Score; Larson Knee Score; OAK Score; 0–100 points), patient-reported (IKDC Score; KOOS Score; 0–100 points), and radiologic criteria (Kellgren and Lawrence Score; MRI Osteoarthritis Knee Score). Results The median follow-up time was 1,529 days (range: 248–4,810). The mean Knee Society Score (94.8 ± 10.1), Larson Knee Score (94.5 ± 10.1), and OAK Score (95.5 ± 6.6) showed nearly unimpaired knee function. The overall patient-reported DSM was low (IKDC Score: 86.7 ± 17.4; KOOS Score: 89.3 ± 17.1). Osteochondral (OC) flaps had a significantly higher DSM, regardless of the donor site. Bone flaps did not show any relevant radiologic morbidity in the Kellgren and Lawrence Score. Besides the procedure-associated cartilage lesions at the OC donor sites, MRI Osteoarthritis Knee Score did not show any significant presence of further knee pathologies in the bilateral MRI Scans. The obvious cartilage lesions did not have a relevant impact on the knee function of most patients. Conclusion The DSM for bone and soft-tissue flaps from the femoral condyle region is negligible. OC flaps are associated with a significantly higher DSM, although a clinically relevant impact on knee function was not evident in the majority of patients.

We report a case of a highly premature born infant in the 24th week of gestation that suffered from an extravasation injury. A central venous catheter caused a large full-thickness defect at the chest. Coverage was necessary and provided by using a split skin graft; however, at this age dermal maturation is not completed and the skin is much thinner and less functional in terms of mechanical strength and epidermal layering. When harvesting skin, this may result in secondary defects. The authors describe the successful and novel use of a dermal substitute with a superficial silicon layer (Integra® Bilayer) to bridge the time until neonatal skin development allows safe skin harvesting. A positive side effect is that the use of this dermal substitute also improved the scar quality in the premature infant.

Zusammenfassung Einleitung Hidradenitis suppurativa (HS) ist eine chronische, wiederkehrende, entzündliche Erkrankung der Haut, welche Bereiche des Körpers befällt, die reich an apokrinen Schweißdrüsen sind. Ausgedehnte Erkrankungen bedürfen oft einer chirurgischen Therapie mit antibiotischer Begleitung. Verwendete Substanzen sind etwa Clindamycin, auch in Kombination mit Rifampicin, oder Tetracycline. Für die perioperative Anwendung gibt es derzeit kaum genaue Empfehlungen. Patienten/Material und Methoden Eine anonyme Fragebogenuntersuchung wurde durchgeführt, um den Status quo der perioperativen antibiotischen Therapie zu erheben. Befragt wurden Ärztinnen und Ärzte an Abteilungen für Plastische Chirurgie, Allgemeinchirurgie und Dermatologie im deutschsprachigen Raum. Der Fragebogen wurde mit Limesurvey© erstellt und unter Vermittlung der Bundesministerien für Gesundheit an die infrage kommenden Abteilungen versandt. Die Antworten wurden mittels IBM® SPSS® Version 23 ausgewertet. Resultate Mit der antibiotischen Therapie wurde am öftesten präoperativ begonnen. Keimabstriche hatten die bedeutendste Rolle in der Auswahl der Präparate. Diese wurden ungeachtet des Verabreichungszeitpunktes großteils an die mikrobiologischen Ergebnisse angepasst. Clindamycin wurde sowohl topisch als auch systemisch am häufigsten eingesetzt. Die Kombination mit Rifampicin war die gängigste. Adalimumab war der am häufigsten verabreichte Antikörper gegen Tumornekrosefaktor-Alpha (TNF-α). In beinahe der Hälfte der Fälle wurden sie zusammen mit Antibiotika verabreicht, wobei deren Verabreichungsdauer oft deutlich länger war. Die Erkrankungsausprägung war der entscheidendste Faktor für die Notwendigkeit operativer Versorgungen. Ein großflächiges Debridement mit offener Wundbehandlung war in beinahe allen Regionen die operative Behandlungsmethode der Wahl. Schlussfolgerung Diese Studie hat gezeigt, dass große Unterschiede in der Behandlung zwischen den einzelnen Fachdisziplinen und Ländern bestehen. Sowohl die Schwere der Erkrankung, als auch die Erfahrungen und Vorlieben der Behandler selbst haben Einfluss auf die Therapie. Abhängig von der therapieführenden Fachdisziplin werden gewisse Behandlungen seltener oder gar nicht angeboten. Überarbeitete und aktuellere Leitlinien mit Integration aller behandelnden Disziplinen wären zur Vereinheitlichung der Therapie wünschenswert.

Zusammenfassung Einleitung Neben den klassischen Therapieverfahren konnte sich CCH (Collagenase Clostridium Histolyticum; Xiapex®) zur Behandlung der Dupuytrenschen Kontraktur als einfache und zuverlässige Therapiealternative etablieren. Ziel dieser Arbeit war die Analyse unserer Ergebnisse sowie die Diskussion der von uns durchgeführten Modifikationen in der Anwendung. Patienten und Methoden In die Studie eingeschlossen wurden 312 Finger, die zwischen 2011 und 2018 mittels CCH behandelt wurden (256 Patienten; 225 Männer; 31 Frauen). Bei 157 Fingern (50,3 %) lag eine isolierte Gelenkskontraktur vor, wohingegen bei 155 Fingern (49,7 %) multiple Gelenke betroffen waren. Das Durchschnittsalter zum Zeitpunkt der Intervention lag bei 65,8 Jahren. Der durchschnittliche Nachuntersuchungszeitraum betrug 41,2 Wochen. Im Rahmen einer retrospektiven Datenanalyse wurden die von uns erzielten Ergebnisse ausgewertet und der Einfluss der von uns durchgeführten technischen Modifikationen auf den Therapieerfolg diskutiert. Ergebnisse Der durchschnittliche, präinterventionelle Kontrakturgrad lag bei 63,3 Grad. Die durchschnittliche Kontrakturreduktion zum Ausgangsbefund betrug unmittelbar postinterventionell 94 % sowie 81 % zum Zeitpunkt der letzten Nachuntersuchung. Hauteinrisse traten in 22,1 % auf. In 99,4 % zeigte sich ein unauffälliger Behandlungsverlauf (minor Komplikationsrate: 0,6 %). Die Rezidivrate lag bei 14,4 %. Die Verlängerung des Zeitintervalls zur Aufdehnung (24 vs. 48 Stunden) zeigte keinen wesentlichen Einfluss auf die unmittelbare Erfolgsrate sowie die Rate an Hautläsionen. Schlussfolgerung Bei richtiger Indikationsstellung sowie routinierter Anwendung ist die Erfolgsrate von CCH mit jener der gängigen Therapiealternativen vergleichbar.

Hidradenitis suppurativa (HS) is a chronic, recurrent, inflammatory skin disease, which affects areas of the body that bear apocrine glands. In extensive cases, a combination of surgical and antibiotic treatment is often needed. Studies are available on the effects of clindamycin, also in combination with rifampicin, or tetracyclines. However, there are few precise recommendations for the perioperative application of antibiotic agents.An anonymous survey was conducted to determine the current status quo of perioperative antibiotic treatment. Physicians and surgeons were surveyed who worked at a department for plastic surgery, general surgery or dermatology in the German-speaking area. The questionnaire itself was created with Limesurvey© and was sent out to qualified departments, with the help from federal ministries of health. The responses were statistically analysed with IBMIn most cases, the antibiotic treatment started before surgical intervention. Bacterial swabs were the most important factor in the selection of antibiotic agents. Most of theparticipants adjusted the treatment to the microbiological results, regardless of the chosen moment of application. Clindamycin was the most often used topical and systemic agent. The combination with rifampicin was most common. Adalimumab was the most commonly used antibody against tumor necrosis factor-alpha (TNF-α). In nearly half of the cases, antibodies were combined with antibiotics, and then the duration of treatment was usually longer. The extent of the disease was the most important factor for the necessity of surgical interventions. The wide excision of the whole affected area with subsequent healing by secondary intention was the surgical method of choice in all but one area.This study has shown that there are significant differences in the treatment between disciplines and countries. On the one hand, the disease severity, on the other hand, individual experience and preferences affect the treatment. Depending on the discipline that determined the therapy, some treatments are provided more rarely or not at all. Summary Background The medial femoral condyle (MFC) flap is based on the descending genicular artery (DGA), which is a vessel with different variations in its course and branching patterns. Many studies have dealt with the vascular anatomy of the MFC. However, the results of the investigations differ markedly. Methods The authors performed a systematic literature search in MEDLINE for articles published until May 2017 on the vascular anatomy of the DGA. After the screening, 23 relevant studies with a similar topic were included into this comprehensive analysis. Results The systematic review examined the lengths and diameters of the individual arteries with regard to the vascularized bone flap of the MFC. The DGA is present in 94% of cases with an average length of 1.8 cm. In 63% of the investigated cases, the DGA divides into three terminal branches. The articular branch has an average length of 7.7 cm, the saphenous branch has a length of 10.7 cm, and the muscular branch has a length of 3.2 cm. Conclusion To ensure a secured survival of this free flap, a detailed understanding of the convoluted vascular anatomy above the MFC is necessary. We recommend the Dubois classification for a systematic classification of the anatomical patterns of the DGA.We present a summary of all anatomical studies dealing with the vascular supply to the MFC and the DGA to date.

CCH (Collagenase Clostridium Histolyticum; XiapexThe study included 312 fingers treated with CCH between 2011 and 2018 (256 patients; 225 men and 31 women). There were 157 fingers (50.3 %) with an isolated joint contracture, while multiple joints were affected in 155 fingers (49.7 %). The average age at surgery was 65.8 years. The follow-up period averaged 41.2 weeks. A retrospective data analysis evaluated our results and the effect of our technical modifications on treatment success.The average presurgical degree of contracture was 63.3 degrees. The average contracture reduction compared to the initial value was 94 % immediately after surgery and 81 % at the last follow-up. There were skin cracks in 22.1 %. The postsurgical course was uneventful in 99.4 %, with minor complications in 0.6 %. Einleitung: Die chirurgische Behandlung von Patienten mit Dupuytren-Kontraktur stellt angesichts der hohen Pravalenz der Erkrankung einen wesentlichen Kostenfaktor im Gesundheitssystem dar. Ziel dieser Studie war es erstmalig fur den deutschsprachigen Raum, eine vergleichende Kosteneffektivitatsanalyse der partiellen Fasziektomie und der Behandlung mit Collagenase Clostridium histolyticum (CCH) vorzunehmen. Methode: In eine retrospektive, monozentrischen Studie wurden im Zeitraum 2012–2014 insgesamt 40 Patienten mit bislang unbehandelter Dupuytren-Kontraktur eines Fingers eingeschlossen, 20 Patienten wurden ambulant einmalig mit CCH behandelt (Gruppe 1), bei 20 Patienten wurde die partielle Fasziektomie unter stationaren Bedingungen durchgefuhrt (Gruppe 2). Verglichen wurden die direkten prainterventionellen, die Behandlungs- und die postinterventionellen/postoperativen Kosten. Ergebnisse: Die unmittelbaren postinterventionellen/postoperativen Behandlungsergebnisse waren vergleichbar: In Gruppe 1 (CCH) ergab sich eine durchschnittliche Kontrakturreduktion um 96,4%, in Gruppe 2 (partielle Fasziektomie) von 97,7%. Komplikationen waren in Gruppe 1 seltener als in Gruppe 2. Die durchschnittlichen Gesamtbehandlungskosten der Gruppe 1 betrugen 1 458,60 € pro Patient, in der Gruppe 2 durchschnittlich 5 315,20 € pro Patient. Schlussfolgerung: Die Kosteneffektivitat der Behandlung mit CCH ist groser als jene bei partieller Fasziektomie. Ursachen sind der im Vergleich deutlich hohere personelle, zeitliche und materielle Aufwand der Operation bei vergleichbarer Effektivitat und hoherer Komplikationsrate nach Fasziektomie. Trotz der eingeschrankten Vergleichbarkeit decken sich die von uns erhobenen Daten mit den vorliegenden Erkenntnissen anderer internationaler Studien.

Introduction The efficiency of collagenase of Clostridium histolyticum (CCH; Xiapex) in the treatment of Dupuytren's contracture has been proved in phase III studies. This retrospective study aims to evaluate our clinical results after the use of CCH. Patients and methods The study included 40 Dupuytren's contractures in 37 patients. There were 32 male and 5 female patients; their average age was 66 years. The most affected finger was the ring finger (55%; 22/40), followed by the little finger (30%; 12/40) and the middle finger (15; 6/40). 14 fingers (35%) presented isolated contractures of the metacarpophalangeal joint whereas an isolated contracture of the proximal interphalangeal joint was evident in 8 (20%) fingers. 18 (45%) fingers presented combined MCP and PIP flexion contractures. None of the patients underwent any treatment prior to this study. A retrospective chart review was performed of all patients. Follow-up examinations were performed seven days, fourteen days, three months, six months and one year after the intervention. The follow-up examination included goniometry of each affected finger to assess the range of motion (ROM) before and after cord breaking. Further patient-reported outcome was accessed concerning postinterventional complaints, impairment of sensibility and satisfaction with the treatment. Results The range of motion improved in all fingers. Full extension of the affected finger without any contracture could be observed in 93% of the MCP contractures, 38% of the PIP contractures and in 28% of the combined MCP and PIP contractures. Incomplete cord breaking could be observed in 9 (22.5%) fingers. In 8 fingers (20%) skin tears occurred after joint manipulation but healed up without any further surgical intervention. The recurrence rate at the latest follow-up was 2.5% (1/40). Patient satisfaction was high and none of the patients reported any complaints at the latest follow-up. Conclusion The best results could be achieved in patients with isolated contractures of the MCP joint. Regarding the good functional results, the low complication rates and the high patient satisfaction, CCH represents a simple and effective treatment for Dupuytren's contracture in selected cases.

Summary Introduction Secondary reconstructions of isolated and combined tendon defects are still a challenge for plastic surgeons. Due to its reliable anatomy, reconstructive potential and low donor-site morbidity, the medial femoral condyle is an ideal area for harvesting isolated and combined tendon flaps. This study evaluates our preliminary results with the vascularized adductor magnus tendon flap. Patient and methods The study included six patients who received a vascularized tendon flap (upper extremity: three patients; lower extremity: three patients) from 2011 to 2015. For three patients, the adductor magnus tendon was used as a single flap; for the other three patients, the tendon was included in a composite flap. A retrospective chart review provided the patients' demographic data, surgical details and the post-operative course. The further objective and patient-reported outcome was evaluated with a long-term follow-up. Results All of the free vascularized flaps healed without complications and with good vascularization upon duplex ultrasonography. One patient did, however, require revision surgery in the late post-operative course. At the end point, all patients showed good functional results without any donor-site morbidity. Conclusion For carefully selected isolated and combined tendon defects on the upper and lower extremities, the vascularized adductor magnus tendon flap provides a reliable and versatile method for microsurgical reconstruction.

This study evaluated the clinical results, and especially the donor site morbidity of the posterior interosseous artery flap. A retrospective study included 40 patients with defects covered with posterior interosseous flaps. Twenty-one patients were available for a follow-up examination to assess donor site morbidity by evaluating the dimensions and quality of the donor site scar and the forearm contour as well as complaints and subjective satisfaction with the aesthetic result. The flaps and related donor sites healed uneventfully in 29 cases (72.5%); healing was delayed in 11 cases (27.5%), with total flap loss in two cases. Further surgery was required in six cases. The quality of the donor site scar rated with the Vancouver Scar Scale averaged 2.4 points. Eleven patients (55%) reported impaired sensibility around the donor site and four patients (20%) had physical complaints. Subjective and objective donor site evaluation revealed significantly lower donor site morbidity for directly closed as opposed to skin grafted donor sites, although subjectively, there was a high level of satisfaction in both groups. Our data indicated that the posterior interosseous flap is a valuable option for the management of soft-tissue defects on the dorsum of the hand, due to its anatomical reliability and soft and pliable tissue, its low donor site morbidity and high patient acceptance.
